Abstract:OBJECTIVE To assess the prevalence of unmet health care demands among rheumatoid arthritis (RA) patients, and to determine if these unmet demands indicate underuse. METHODS A total of 679 patients with RA participated in a questionnaire survey and clinical examination. Unmet health care demands and health care use were assessed for orthopedic care, allied health care, home care, and psychosocial care. Indications for underuse were determined by comparing health outcomes of patients with unmet health care demands and of health care users. RESULTS Of the 679 patients, 28.7% had an unmet demand for 1 of the 4 services: 13.4% for allied health care, 9.7% for orthopedic care, 9.4% for home care, and 6.2% for psychosocial care. Underuse of allied health care, home care and psychosocial care was observed. CONCLUSION Unmet demands for health care are frequent among RA patients. Most unmet demands indicate underuse. Health care professionals should therefore be more responsive to the demands of patients.
Changes of calcium channel characteristics and its relationship with pancreatic islet β cell parasecretion in rat born small for gestational age

Abstract:Method The animal model of small for gestation in rat was made under matemal calorie restriction by 50% from d1 of gestation until term, while the control groups named AGA rats, were obtained from normal pregnant rats. Both of SGA and AGA newborn rats were fed normally till 4 weeks and 8 weeks. Single islet b cell isolation and pancreatic tissue-slice technique were used to obtain b cells in neonatal rats and adult rats, respectively. Using whole cell patch clamp to record the characteristics of b cell voltage dependent calcium channel (such as current, reversal potential, I-V curve, activation curve, inactivation curve) in both SGA and AGA groups. Using capacitance measurement induced by a sequence of sine wave stimulus to reflect the insu lin secretion. In aw ord, study the tendency of calcium channel kinetics and secretion property by timing. Results The membrane capacitance of SGA was smaller than AGA rat after excluding the influence of whole cell membrane capacitance deviation both in neonatal rats, 4 weeks and 8 weeks (P 0.05). The same result was obtained in activation curve and inactivation curve of calcium channel, whose half activated voltage/ half inactivated voltage V1/2 and slope factor k between SGA and AGA both showed no significant difference (P>0.05) ; 2) adult rats: there was no statistical difference found in all average calcium current density (Id), including the total calcium, L-type and T-type. No significant difference was observed in the peak current density of I-V curve in SGA and AGA (P>0.05). However, the reversal potential in SGA group was much lower than control (P<0.05). The two group had the same shape of activation curve and inactivation curve of calcium channel, and 50% activated voltage between SGA and AGA had no significant difference, but the calcium channel of SGA was easily to be closed, the inactivated voltage in SGA group was much higher than control (P<0.01). Conclusion The effect of intrauterine malnourishment on pancreatic isletbcell last in postnatal life and the destroy of bcell tends to aggravate with time. Low calcium current density on b cell membrane devotes to the decrease of b cell secretion in SGA neonatal rat. However, the current of calcium channel returns to normal level in childhood. Abstract:OBJECTIVE: It has been reported that there are differences between in vitro and in vivo matured oocytes for their survival rates following vitrification The purpose of this study is to determine the competency of cryodamage resistance of in vitro matured oocytes in the presence of cysteamine during in vitro maturation (IVM).DESIGN: Animal-model study.MATERIALS AND METHODS: Female mice (CD-1) were stimulated with 10IU PMSG. Oocytes were collected after 48h and the collected oocytes were divided into following groups: (1) Cumulus-oocyte-complex (COC); (2) denuded groups. Both COC and denuded oocytes were cultured in IVM medium containing 75mIU/mL FSH and LH, 10% FBS for 16 h in the presence or absence of cysteamine (200μM). Following culture, the maturated oocytes were either vitrified or inseminated immediately. Vitrification and thawing procedures were performed with McGill Cryoleaf as reported previously. Vitrified oocytes were thawed and inseminated on the other day. Fertilization rate were recorded 24 h after insemination, and early embryonic development were assessed. Statistical differences were considered significant at p<0.05.RESULTS: The presence of cysteamine during IVM significantly increased maturation rate in COC group but had no significant difference in denuded group. The presence of cyseamine during IVM does not improve the survival rate of in vitro matured oocytes with or without cumulus cells during IVM. However, the presence of cumulus cells during IVM had better fertilization rate (p<0.05) compared to the denuded oocytes after vitrification-thawing. The presence of cysteamine during IVM significantly reduced the fragmentation rate in cumulus-intact group compared to cumulus-free group following vitrification-thawing.CONCLUSIONS: The presence of cysteamine during IVM has beneficial effect on both COC and denuded oocytes in terms of their competency of subsequent fertilization and early embryonic development following vitrification. Aging Alters Histone H3 Lysine 4 Methylation in Mouse Germinal Vesicle Stage Oocytes

Abstract:Decreasing oocyte competence with maternal aging is a major factor in mammalian infertility. One of the factors contributing to this infertility is changes to chromatin modifications, such as histone acetylation in old MII stage oocytes. Recent studies indicate that changes in histone acetylation at MII arise at the germinal vesicle (GV) stage. We hypothesised that histone methylation could also change in old GV oocytes. To test this hypothesis, we examined mono-, di- and trimethylation of histone H3 lysine 4 (H3K4 me1, me2 and me3, respectively) in young and older oocytes from 6-8- and 42-44-week-old mice, respectively. We found that H3K4 me2 and me3 decreased in older compared with young GV oocytes (100% vs. 81% and 100% vs. 87%, respectively; P<0.05). H3K4 me2 later increased in older MII oocytes (21% vs. 56%; P<0.05). We also examined the expression of genes encoding the H3K4 demethylases lysine (K)-specific demethylase 1A (Kdm1a) and retinol binding protein 2 (Rbp2). Expression of Kdm1a increased at both the mRNA and protein levels in older GV oocytes, but decreased in older MII oocytes (P<0.05), and was negatively correlated with H3K4 me2 levels. Conversely, expression of Rbp2 mRNA and protein decreased in older GV oocytes (P<0.05), and this was not correlated with H3K4 me3 levels. Finally, we showed that inhibition of Kdm1a of older oocytes at the GV stage restored levels of H3K4 me2 at the MII stage to those seen in 'young' oocytes (41% vs. 38%; P>0.05). These results suggest that changes in expression of H3K4 me2 and Kdm1a in older GV oocytes may represent a molecular mechanism underlying human infertility caused by aging.

Abstract:Inhibiting oocyte spontaneous activation (SA) is essential for successful rat cloning by nuclear transfer (NT). This study tested the hypothesis that activities of the Na(+)/Ca(2+) exchanger (NCX) would decrease with oocyte aging and that SA of rat oocytes could be inhibited if the intraoocyte Ca(2+) rises were prevented by activating the NCX through increasing Na(+) concentrations in the culture medium. Elevating Na(+) levels in culture medium by supplementing NaCl inhibited SA of rat oocytes, while maintaining a constant level of maturation-promoting factor and mitogen-activated protein kinase activities. Experiments using the NCX inhibitor bepridil, the Na(+)/K(+)-ATPase inhibitor ouabain, and an assay for intraoocyte Ca(2+) concentrations showed that extracellular Na(+) inhibited rat oocyte SA by enhancing NCX activity and preventing intracellular Ca(2+) rises. Immunohistochemical quantification indicated that the density of NCX1 decreased significantly in aged oocytes that were prone to SA compared with that in freshly ovulated oocytes whose SA rates were low during in vitro culture. Cumulus cell NT showed that sham enucleation caused marked SA in freshly ovulated rat oocytes and that Na(+) supplementation prevented the manipulation-induced SA and improved the in vitro and in vivo development of rat somatic cell NT embryos. Taken together, the results have confirmed our hypothesis that the NCX is active in rat oocytes and its activity decreases with oocyte aging and that activating the NCX by increasing extracellular Na(+) inhibits SA of rat oocytes and improves the development of rat somatic cell NT embryos. These data are also important for understanding the mechanisms of oocyte aging.
The Attenuating Effect of the Intraovarian Bone Morphogenetic Protein 4 on Age-Related Endoplasmic Reticulum Stress in Chicken Follicular Cells.

Abstract:In the poultry, only less than 5% primordial follicles in the ovary can develop into the prehierarchical follicles (PHFs) leading to progressive development, ovulation, and egg formation. This low rate of recruitment indicates a huge potential for improvement of the laying performance. A great reduction in egg production is caused by aging with extensive follicular atresia. In this study, age-related changes in the laying performance and ovarian status were compared between the peak-lay (D280) and aged (D580) chickens. Subsequently, a cross coculture of PHFs and granulosa cells (GCs) from D280 or D580 hens was adopted to reveal the mechanism of declined follicle development. Results showed that persistent endoplasmic reticulum (ER) stress in GCs of the aged hens was accompanied with intensified apoptosis. Bone morphogenetic protein 4 (BMP4) secreted by GCs of PHFs in D280 hens was capable of relieving ER stress and improving follicular dominance for selection in D580 hens. During this action, BMP4 reduced free calreticulin (CALR, an ER marker) content and attenuated cell apoptosis in PHFs of D580 hens via the PERK-CHOP-BCL2/caspase3 or CALR-Ca2+-BCL2-caspasel2 pathway. Furthermore, BMP4 prevented follicular atresia by promoting production of steroid hormones to improve survival of GCs in PHFs from the aged hens. In conclusion, intensified ER stress and apoptosis occurred in GCs of PHFs in aged chickens, while BMP4 secreted by GCs was capable of improving follicular viability by alleviating ER stress to promote follicular development.

Abstract:[Ca 2+ ] i is essential for mammalian oocyte maturation and early embryonic development, as those processes are Ca 2+ dependent. In the present study, we investigated the effect of [Ca 2+ ] i on in vitro maturation and reprogramming of oocytes in a lower calcium model of oocyte at metaphase II (MII) stage, which was established by adding cell-permeant Ca 2+ chelator BAPTA-AM to the maturation medium. Results showed that the extrusion of the first polar body (PB1) was delayed, and oocyte cytoplasmic maturation, including mitochondrial and endoplasmic reticulum distribution, was impaired in lower calcium model. The low-calcium-model oocytes presented a poor developmental phenotype of somatic cell nuclear transfer (SCNT) embryos at the beginning of activation of zygotic genome. At the same time, oxidative stress and apoptosis were observed in the low-calcium-model oocytes; subsequently, an RNA-seq analysis of the lower-calcium-model oocytes screened 24 genes responsible for the poor oocyte reprogramming, and six genes (ID1, SOX2, DPPA3, ASF1A, MSL3, and KDM6B) were identified by quantitative PCR. Analyzing the expression of these genes is helpful to elucidate the mechanisms of [Ca 2+ ] i regulating oocyte reprogramming. The most significant difference gene in this enriched item was ID1. Our results showed that the low calcium might give rise to oxidative stress and apoptosis, resulting in impaired maturation of bovine oocytes and possibly affecting subsequent reprogramming ability through the reduction of ID1.

Abstract:The presence of aromatic compounds in water is an important topic in environmental sciences. Silver-polydimethylsiloxane nanocomposites (Ag-PDMS) have recently been demonstrated as promising substrates for the detection of model environmental pollutants via surface-enhanced Raman spectroscopy (SERS). This work discusses how different variables such as pH and matrix composition can affect the sorption and SERS activity of these chemicals. The results show that the conjugate base of weak acids can interact more efficiently with the substrate, leading to an increased signal at higher pH, while amino-aromatic compounds interact more efficiently at a lower pH. The sorption of these chemicals is an essential step in the process and has been attributed to the absorption of the analyte into the PDMS followed by its adsorption to the metallic surface. In addition, the presence of moderate concentrations (1 × 10−4 M) of a supporting electrolyte such as nitrate or fluoride can improve the sorption of 4-hydroxybenzoic acid to the Ag-PDMS nanoparticles. Other ions such as phosphate and chloride cause rapid oxidation of the substrates even at concentrations as low as 1 × 10−5 M. The effect of these variables in the analysis of real samples is presented. The potential use of liquid chromatography for isolating the model pollutants from detrimental matrix components in natural waters is also shown.

Abstract:With increasing periods of time following ovulation, the metaphase II (MII)-stage oocyte experiences overproduction of reactive oxygen species and elevated levels of lipid peroxidation that are implicitly linked with functional deficiencies acquired during postovulatory oocyte aging. We have demonstrated that the electrophilic aldehydes 4-hydroxynonenal (4HNE), malondialdehyde, and acrolein are by-products of nonenzymatic lipid peroxidation in the murine MII-stage oocyte, adducting to multiple proteins within the cell. The covalent modification of oocyte proteins by these aldehydes increased with extended periods of time postovulation; the mitochondrial protein succinate dehydrogenase (SDHA) was identified as a primary target for 4HNE adduction. Time- and dose-dependent studies revealed that exposure to elevated levels of electrophilic aldehydes causes mitochondrial reactive oxygen species production, lipid peroxidation, loss of mitochondrial membrane potential, and eventual apoptosis within the MII oocyte, presumably as a consequence of electron transport chain collapse following SDHA adduction. Additionally, we have determined that short-term exposure to low doses of 4HNE dramatically impairs the oocyte's ability to participate in fertilization and support embryonic development; however, this loss of functionality can be prevented by supplementation with the antioxidant penicillamine. In conclusion, this study has revealed that the accumulation of electrophilic aldehydes is linked to postovulatory oocyte aging, causing reduced fertility, oxidative stress, and apoptosis of this highly specialized cell. These data highlight the importance of timely fertilization of the mammalian oocyte postovulation and emphasize the potential advantages associated with antioxidant supplementation of oocyte culture medium in circumstances where reinsemination of oocytes may be desirable (i.e., rescue intracytoplasmic sperm injection), or where in vitro fertilization may be delayed.

Urinary excretion of iodide and fluoride from supplemented food grade salt.

Abstract:Iodide and fluoride supplemented food grade salt (NaCl) is a common source of these two micronutrients. In a pilot study, we investigated whether increased intake of NaCl supplemented with iodide (I-) and fluoride (F-) results in their higher bioavailability. Twelve healthy adult human volunteers ingested increasing quantities (1, 3, 6 and 9 g) of NaCl with usual diet over 8 days. Sodium (Na+), I- and F- were measured in 24 hour urine specimen. During the 4 day basal period when no additional NaCl was ingested, ingestion of NaCl calculated from urinary Na+ concentration and diuresis was 8.25 +/- 0.67 g/24 h. During the same period 0.11 +/- 0.01 and 0.61 +/- 0.04 mg of I- and F- respectively were excreted in the urine per 24 h. Increased ingestion of supplemented NaCl resulted in higher urinary excretion of sodium while urinary creatinine remained stable. 92% of I- and 40% of F- contained in the additional amount of NaCl ingested were excreted in the urine. These results indicate that with increased ingestion of supplemented (I- and F-) NaCl, almost the totality of I- is excreted in the urine while fluoride is either incompletely absorbed or retained by the body to a higher extent. I- and F- supplemented NaCl is, therefore, an effective vehicle to provide these micronutrients when ingested with diet.
Intracellular pH of rat eggs is not affected by fertilization and the resulting calcium oscillations

Abstract:Penetration of the oocyte by a spermatozoon is the first in the series of events resulting in the transition of the egg from a quiescent to a proliferative state. A critical regulatory role for intracellular calcium ([Ca2+]i) ion activity has been demonstrated in all species studied so far. On the other hand, it has been demonstrated that the intracellular pH (pHi) changes, but only in a small number of species. This change also has been proposed as one of the most important events in egg activation. The present study was undertaken to monitor pHi in rat eggs during fertilization, using the membrane-permeable indicator BCECF-AM and fluorescence ratio imaging. Furthermore, we proposed to evaluate the relationship between pHi and [Ca2+]i changes during egg activation. We found that the ovulated rat egg has a cytoplasmic pH significantly different from that of the follicular oocyte. Insemination with capacitated sperm resulted in a microscopically visible sperm attachment, yet no change in pHi was observed. Eggs double-loaded with fura-2-AM and BCECF-AM before insemination were used to measure [Ca2+]i and pHi simultaneously. Eggs with a normal pattern of [Ca2+]i transients (i.e., fertilized eggs) did not show any change in pHi at least for 30 min following sperm binding. Data for eggs fertilized in vivo were recorded at later times after sperm binding; these served to exclude the possibility of a transient change that occurs between sperm-egg interaction and the pronuclear stage. We conclude that the pHi of rat eggs does not change during fertilization and therefore that fertilization-induced [Ca2+]i changes do not affect pHi in these eggs.

Abstract:Calcium-induced calcium release (CICR) may function widely in calcium-mediated cell signaling, but has been most thoroughly characterized in muscle cells. In a homogenate of sea urchin eggs, which display transients in the intracellular free calcium concentration ([Ca 2+ ] i ) during fertilization and anaphase, addition of Ca 2+ triggered CICR. Ca 2+ release was also induced by the CICR modulators ryanodine and caffeine. Responses to both Ca 2+ and CICR modulators (but not Ca 2+ release mediated by inositol 1,4,5-trisphosphate) were inhibited by procaine and ruthenium red, inhibitors of CICR. Intact eggs also displayed transients of [Ca 2+ ] i when microinjected with ryanodine. Cyclic ADP-ribose, a metabolite with potent Ca 2+ -releasing properties, appears to act by way of the CICR mechanism and may thus be an endogenous modulator of CICR. A CICR mechanism is present in these nonmuscle cells as is assumed in various models of intracellular Ca 2+ wave propagation.

Abstract:Human ribosomal protein S16 is homologous to prokaryotic S9, which contacts the 16S rRNA region formed by helices H29, H30, H38-H41, and H43 by X-ray data. A study was made of the interaction of recombinant human S16 with an RNA transcript corresponding to human 18S rRNA region 1203-1236/1521- 1698 (helices H28-30 and H41-43), homologous to the 16S rRNA region harboring part of the S9-binding site. S16 specifically bound to the transcript with an apparent dissociation constant of (1.3 ± 0.1) · 10 -8 M at 20°C. Enzymatic and chemical footprinting was used to identify the nucleotides whose accessibility for RNases and modifying chemical agents changes upon S16 binding. In the presence of S16, a substantially higher accessi- bility was observed for C1544 (internal loop of hairpin H41), C1618-U1622, C1629-A1634 (hairpin H42), C1521-C1523, U1530, C1532 (helix H30), C1645, C1646, and G1648 (base of hairpin H43) and an apprecia- bly lower accessibility was observed for C1670-A1675 (hairpin H43). Involvement of ER-calreticulin-Ca2+ signaling in the regulation of porcine oocyte meiotic maturation and maternal gene expression

Abstract:Calcium is one of the most ubiquitous signaling molecules, and controls a wide variety of cellular processes. It is mainly stored in the endoplasmic reticulum (ER), bound to lumenal proteins. Calreticulin is the major Ca(2+)-binding chaperone in oocytes, and is integral to numerous cellular functions. To better understand the role of the ER- calreticulin-Ca(2+) pathway in oocyte maturation and early embryogenesis, we characterized the porcine calreticulin gene and investigated its expression profile during oocyte maturation and early embryonic development. Calreticulin was widely expressed in pig tissues and its transcripts were downregulated during maturation, especially at 44 hr, and were undetectable at the blastocyst stage. We also investigated the effect of increased cytosolic Ca(2+) induced by the Ca(2+)-ATPase inhibitor, cyclopiazonic acid (CPA), on pig oocyte maturation and maternal gene expression. CPA at 10 microM did not inhibit germinal vesicle breakdown, but did result in the arrest of 38.6% oocytes at or before the MI stage. In addition, expression of the maternal genes C-mos, BMP15, GDF9, and Cyclin B1 was significantly increased in CPA-treated MII oocytes compared with control groups. These data were supported by the results of poly(A)-test PCR, which revealed that the cyclin B1 short isoform (CB-S), GDF9, and C-mos underwent more intensive polyadenylation modification in CPA-treated oocytes than control oocytes, suggesting that polyadenylation may influence Ca(2+)-modulated changes in gene expression. Furthermore, CPA treatment decreased the percentage of four-cell parthenotes that developed into blastocysts, suggesting the need for functional SR/ER Ca(2+)-ATPase pumps or Ca(2+) signals during early embryo development after zygotic genome activation. Together, these data indicate that ER-calreticulin-associated Ca(2+) homeostasis plays a role in oocyte and embryo development, and that alterations in maternal gene expression may contribute to the underlying molecular mechanism, at least partially, via polyadenylation.
On the Function and Evolution of Electric Organs in Fish

Abstract:1. The electric discharges of Gymnarchus niloticus and of representative species of seven genera of the Mormyridae have been examined in their natural habitat in Africa and in the laboratory. 2. Comparable investigations of the South American Gymnotidae have shown the existence of two discharge types in both these unrelated fish families. 3. The first type of electric discharge consists of very regular sequences of continuously emitted, monophasic pulses, varying from species to species in frequency, and within narrower limits from individual to individual. 4. Fish emitting this first type of pulses include Gymnarchus , Hypopomus and Eigenmannia . The frequency range for these fish lies between 60 and 400 discharges/sec. 5. The frequency does not alter with the state of excitation of the fish. The duration of individual pulses is relatively long, i.e. 2-10 msec. 6. The second type of discharge is less regular in frequency, the pulse duration much shorter and the pulse shape more complex. The individual discharge from the whole electric organ lasts about 0.2 msec, in Petrocephalus . 7. This type of discharge is found in all the examined species of the Mormyridae and in such forms as Gymnotus carapo and Staetogenes elegans . 8. The basic discharge rate of a resting mormyrid is somewhat variable and not strictly rhythmical. It usually lies between 1 and 6 pulses/sec. 9. Stimuli which excite the mormyrids cause an increase in the discharge frequency. The recorded maximum is about 130 pulses/sec. 10. Suitable stimuli can inhibit the discharges of the Mormyridae for prolonged periods. 11. In Gymnotus carapo and Staetogenes elegans the basic discharge rate is higher and of regular rhythmicity. Depending on temperature the frequencies lie between 30 and 87 pulses/sec. When these fish are excited the frequencies are increased up to 200 pulses/sec, for a short time. 12. The shape of the electric field, which is set up with each pulse around the fish, has been examined. 13. A theory has been proposed which suggests that these fish, by means of their electric pulses, can locate objects if their electrical conductivity differs from that of water. 14. These fish have shown themselves extremely sensitive to influences affecting the electric field. This has been studied by applying artificial electric stimuli, by studying the effects of conductors and non-conductors introduced into the field, and the reactions towards magnetic fields and electrostatic charges. 15. Conditioned reflex experiments with Gymnarchus niloticus and Gymnotus carapo have shown that these fish can detect the presence of a stationary magnet, and that they can discriminate between conductors and non-conductors. 16. The prey of these fish does not appear to be affected by the discharges. Inter alia , the electric pulses have a social significance. 17. This locating mechanism may be considered as an adaptation to life in turbid water. 18. Gymnotidae and Mormyridae (taken to include Gymnarchus ) show striking features of convergent evolution. 19. Unusual locomotory adaptations such as swimming by means of the dorsal fin ( Gymnarchus ), the anal fin (Gymnotidae) and ‘Gemminger9s bones’ (Mormyridae) may be considered as a means which tends to make the axis of symmetry of the fish and of its electric field coincide during active movements. 20. A new theory for the evolution of electric organs has been suggested. A major prerequisite appears to be a receptor sensitive to electrical stimulation. 21. It is suggested that special sensory and nervous differentiations of the lateralis system (‘mormyromasts’, valvulae cerebelli) are concerned with the perception and integration of electric stimuli. 22. Muscular action potentials have been recorded in the water at some distance from non-electric fish. 23. The easiest explanation for the evolution of strong electric organs would appear to start from such muscular action potentials, and proceed via weak electric organs used for orientation, to the powerful offensive and defensive electric organs.
Postovulatory aging causes the deterioration of porcine oocytes via induction of oxidative stress

Abstract:Negative effects of postovulatory aging on fertilization ability and subsequent embryo development have been reported in rodents; however, the molecular and cellular changes during this process have not been fully defined. Here, we used porcine oocytes, a model that is physiologically and developmentally similar to humans, to explore the molecular mechanisms that underlie how postovulatory aging affects oocyte quality and fertilization capacity. We found that postovulatory aging caused the morphologic change of porcine oocytes by exhibiting the incompact expansion of cumulus cells and an increased occurrence of fragmentation. Aging also impaired oocyte quality by disrupting organelle structures, including the spindle assembly, actin polymerization, and mitochondrial integrity. Moreover, postovulatory aging led to the abnormal distribution of cortical granules and ovastacin, which, in turn, resulted in defective sperm binding and consequently compromised fertilization potential. Of note, we observed that postovulatory aging induced oxidative stress with a high level of reactive oxygen species and apoptotic rate in oocytes, thereby resulting in the deterioration of critical factors in the maintenance of oocyte quality and fertilization capacity. Abstract:Atrial fibrillation (AF) is a common clinical problem that is associated with an impaired quality of life, thromboembolism, heart failure and death. Medical treatment of AF remains suboptimal and is associated with potentially serious side effects. The disappointing outcomes with medical therapy have spurred the age of catheter ablation of AF. In the last 10 years, catheter ablation of AF has evolved dramatically and has been shown to be superior to medical therapy in multiple studies. As a result, catheter ablation may be offered to the symptomatic patient as first-line therapy in lieu of antiarrhythmic medications, which have limited efficacy and are associated with significant toxicity.

Abstract:A strong correlation exists between increasing paternal age and a decline in reproductive function. Testis aging is associated with testicular atrophy, increased DNA damage, and de novo mutations. It is unclear whether these problems arise from the spermatogonial stem cells (SSCs), a buildup of anomalies as older germ cells progress through spermatogenesis, or both. We hypothesize that with the continual divisions of SSCs that maintain the germ cell population, an alteration of these cells occurs over time. To test this, we utilized young (4-mo-old) and aged (18- and 21-mo-old) transgenic rats that express GFP in germ cells only. We first examined the number and activity of SSCs from the different age groups by transplantation. Aged rats had numerically fewer SSCs than young rats (<50%; not significant) despite the lack of testicular atrophy, and 21-mo-old rats show a significant reduction in colony length, suggesting that the quality of SSCs also deteriorates. To evaluate any molecular changes occurring in the early cells of spermatogenesis with age, we isolated an SSC-enriched population of CD9-positive (CD9(+)) cells using fluorescence-activated cell sorting (confirmed by transplantation studies) and extracted RNA for microarray analysis. In the aged CD9(+) cells, 60 transcripts were upregulated and more than 500 downregulated compared to the young cells. An altered expression was found for transcripts involved in mitosis and in DNA damage response. These results suggest molecular alterations in the SSC-enriched population of aged CD9(+) cells, implying that reproductive aging originates in the undifferentiated cells of spermatogenesis.
