Systemic amyloidosis and non-hematologic malignancy in a large autopsy series

Abstract:A series of 24,388 consecutive autopsies yielded 148 cases of amyloidosis, for which the associated diseases were tabulated. 13 solid primary malignancies were found in eleven patients, including one mucoepidermoid cancer of the parotid and one thymoma; two out of eleven had evidence of pyelonephritis. Five patients suffered from past or present renal adenocarcinoma, the most common carcinomatous cause of amyloidosis. Systemic amyloid deposits thus occurred in 2.1% of autopsied patients with renal carcinoma, but showed no obvious correlation with tumor stage or histologic type. Details are presented of an unusual case of hypernephroma producing a stable bronchial metastasis cuffed by nodular amyloid and dissociated by a lymphoid infiltrate containing plasma cells. The occurrence of systemic amyloidosis in non-hematologic malignancy thus appears to be a rare event, which, especially in the case of renal cancer, is assumed to be due to amyloid-fibril protein AA.
Primary Carcinoma Ex Pleomorphic Adenoma Of The Bronchus: Report Of Two Cases And Review Of The Literature

Abstract:Primary carcinoma ex pleomorphic adenoma in the tracheobronchial system is an exceedingly rare neoplasm arising from minor salivary glands. Two cases of this unusual entity were presented. All the lesions were associated with bronchial system, while neither of the patients had a history of a head and neck salivary gland neoplasm. Both cases were followed up for more than 3.5 years. Thus, further data concerning biological behavior and prognosis were made availably. In addition, we provided a review of all cases published to date and summarized the data presently known, for the first time, with the emphasis on the computed tomography (CT) findings. The diagnosis of carcinoma ex pleomorphic adenoma should be considered in elderly patients with slowly progressive symptoms of dyspnea and cough. The continued clinical evaluation and long-term follow-up seems to be of utmost importance for better diagnosis and treatment of primary carcinoma ex pleomorphic adenoma of the bronchus.

Abstract:An 86-year-old man was admitted with cough and weight loss. He reported episodes of mild hemoptysis and denied fever or other symptoms. He was a smoker (40 pack-years) with a previous history of multiple myeloma. Physical examination demonstrated wheezing. Laboratory test results were unremarkable. Chest CT revealed tracheal and bronchial wall thickening, and lytic lesions on the ribs and vertebral bodies with partial collapse (Figure 1). The patient was referred for fiberoptic bronchoscopy with BAL. Bronchoscopy showed tracheal and bronchial wall thickening, with swelling and hypertrophy of a brittle, and easily bleeding mucosa, as well as submucosal plaques. BAL fluid was negative for neoplastic cells, bacteria, and fungi. Biopsy of the tracheal walls revealed a stroma occupied by an amorphous eosinophilic material that was positive on Congo red staining and exhibited apple-green birefringence under polarized light, consistent with amyloidosis. Multiple myeloma and amyloidosis are characterized by abnormal accumulation and deposition of monoclonal plasma cells and extracellular protein fibrils. [Pulmonary nodular amyloidosis mimicking multiple pulmonary metastases of carcinoma of the corpus uteri]

Abstract:Introduction: Pulmonary nodular amyloidosis (PNA) is a phenomenon that is rarely diagnosed anywhere in the world. Methods: We report a case of a 63-year-old woman who smoked in whom a chest X-ray examination 5 years after diagnosis and radical treatment of a highly differentiated carcinoma of the corpus uteri showed multiple lung metastasis. To elucidate these findings by bronchoscopy and thoracoscopy we took a specimen from the right pleura and from one of the suspicious nodules, which were up to 3 cm in diameter; we also obtained some of the bronchial secretion. Results: The nodules were histopathologically diagnosed as PNA. Conclusion: Even if it is rare, PNA also belongs in the differential diagnosis of metastatic cancers. For us evidence of Pseudomonas fluorescens in the sputum is a reason for discussing a chronic, clinical unobtrusive local inflammation with hyperactivity of the B-cells as the hypothetical etiology of the amyloidomas.
Case report: Pulmonary amyloidosis in two patients with chronic obstructive lung disease (COPD)

Abstract:We present two cases of patients with chronic obstructive lung disease (COPD) who developed different forms of pulmonary amyloidosis. In both cases malignancy was considered as primary diagnosis. Transthoracic biopsy confirmed pulmonary amyloidosis. In the first case the patient presented with progressive dyspnoea over a two years period. Initial assessment was consistent with a diagnosis of COPD but progressive changes in symptoms and lung functions and subsequently CT Thorax revealed possible airway obstruction. Bronchoscopy confirmed an obstructive lesion initially considered to be malignant but was found to be due to tracheobronchial amyloid (TBA). Our second case presented with symptoms and signs consistent with COPD. Follow up chest X-rays revealed a pulmonary nodule which on CT examination was considered to be malignant. Transthoracic biopsy confirmed pulmonary amyloidosis. Although it is a rare condition amyloid disease should to consider as the part of the differential diagnosis in COPD patients who present with signs and symptoms consistent with pulmonary malignancy.

Abstract:Objective: To define the prognosis for and radiographic presentation of patients with pulmonary amyloidosis. Design: Retrospective review of the Mayo Clinic experience with biopsy-proven pulmonary amyloidosis from 1980 to 1993. Setting: Tertiary care center. Patients: Patients with pulmonary biopsy specimens showing amyloid deposition. Measurements: Medical records were reviewed, and pertinent information was recorded, including demographic data, type of pulmonary biopsy, results of biopsies of nonpulmonary sites and of immunoelectrophoresis, and other clinical, radiographic, and laboratory information necessary for distinguishing localized pulmonary amyloidosis, primary systemic amyloidosis, secondary amyloidosis, and familial amyloidosis. Results: 35 of 55 patients with pulmonary amyloidosis had primary systemic amyloidosis that presented radiographically as an interstitial or reticulonodular pattern with or without pleural effusion. The median survival after diagnosis was 16 months. Nodular pulmonary "amyloidomas" (nodular amyloid lesions) were not associated with systemic disease and were associated with a benign prognosis. Three of 4 patients with localized tracheobronchial amyloidosis required Nd:YAG (neodymium:yttrium-aluminum-garnet) laser therapy for obstructive symptoms. "Senile" amyloid deposition was an incidental finding in some patients at autopsy. Conclusions: Localized amyloidomas are characterized by a benign course and are not associated with systemic amyloidosis. Despite its localized nature, tracheobronchial amyloid deposition may be asymptomatic or may result in significant morbidity due to obstructive phenomena. Pulmonary amyloidosis associated with primary systemic amyloidosis generally presents as a diffuse interstitial pattern with or without pleural effusion. Complete survival data indicate that long-term outcome is poor after diagnosis. We describe the largest series of patients diagnosed by bronchoscopic lung biopsy. Despite reports to the contrary, we have found bronchoscopic lung biopsy to be a safe and effective diagnostic technique.

Abstract:Primary localized amyloidosis of the airways is an uncommon disorder characterized by amyloid deposits in the airway mucosa. In contrast to systemic amyloidosis other organs are not involved. Among the entities of airway amyloidosis, tracheobronchial amyloidosis is comparatively the most common subtype in the lower respiratory tract and laryngeal amyloidosis in the upper respiratory tract. The pathophysiology of localized airway amyloidosis is poorly understood. The clinical presentation is variable and often non-specific. No general consensus exists with regard to optimal treatment resulting in a variety of modalities used in clinical practice to manage this disorder. We report the case of a 50 year old woman with multifocal localized amyloidosis of the tracheobronchial tree and the upper airways. Tracheobronchial amyloidosis was treated with endoscopic debulking and external beam radiation, sinunasal amyloid deposits were surgically excised and are currently under surveillance. The importance of this extremely rare case lies in the multifocal presentation of an uncommon disorder requiring a multidisciplinary approach to offer optimal treatment including external beam radiation.

Abstract:Background: Amyloidosis is an uncommon condition, which results from accumulation of misfolded extracellular insoluble protein in tissues and organs of the body, causing its damage and dysfunction. Histologically, after staining with Congo red, the amyloid deposits show an apple-green birefringence under polarized light microscope. Amyloidosis can affect all organ systems and is classified into hereditary or acquired, localized or systemic. Respiratory involvement occurs in 50% of the patients with amyloidosis and it may take tracheobronchial, nodular parenchymal, diffuse alveolar septal and lymphatic forms. Methods: We report four cases of pulmonary amyloidosis. A female patient with localized form of tracheobronchial and nodular parenchymal pulmonary amyloidosis, which was initially misdiagnosed as sarcoidosis. A male patient who was referred to our department for further evaluation of multiple tumors in lungs accompanied by mediastinal lymphadenopathy, liver and peritoneal tumors. A male patient with suspect of lung malignancy. A male patient with diagnosed idiopathic pulmonary fibrosis and the possibility of malignancy. Results: All the diagnoses were established by demonstration of amyloid protein in tissue specimens obtained in transbronchial or open lung biopsies. Conclusions: Due to its nonspecific clinical and radiological findings, amyloidosis can often mimic other diseases and should be considered as one of the differential diagnoses. In order to confirm the diagnosis, proving the presence of amyloid deposition with positive Congo red staining in respiratory specimen is mandatory.
A case of sarcoidosis associated with lung cancer

Abstract:A 65-year-old woman presented at the First Affiliated Hospital of Zhejiang University with chest pain and cough. She had been diagnosed with sarcoidosis 4 years ago. Two years after the initial diagnosis and treatment, she presented with multiple facial subcutaneous nodules, which were pathologically diagnosed as sarcoidosis after biopsy at another hospital. The aforementioned symptom recurred on September 22, 2009, and computed tomography of the chest performed at the time revealed lymphadenectasis at both the hila of the lungs and at the mediastinum, in addition to the appearance of nodular lesions at the apex of upper lobe of the left lung, which was indicative of lung cancer. Radical resection of the cancer was performed on October 26, 2009. The results of postoperative pathological examinations showed evidence of posterior segment adenocarcinoma at the apex of the upper lobe of the left lung as well as granulomatous inflammation (on inspection of the lymph nodes), thereby confirming the diagnosis of sarcoidosis. The patient underwent postoperative chemotherapy and has since been maintained in a stable condition.
Tracheobronchial amyloidosis in a patient with Sjogren syndrome

Abstract:Pulmonary amyloidosis in the setting of Sjogren syndrome is rare. It most commonly presents in form of multinodular disease with or without cysts formation. Amyloid plaques may also deposit in the airway submucosa, causing airway narrowing; the condition referred as "tracheobronchial amyloidosis" (TBA). Patients with this condition most commonly present with postobstructive pneumonia, wheezing, and occasionally hemoptysis. Endoscopic therapies, using flexible forceps with or without laser therapy is crucial to alleviate the obstruction and control the bleeding. Other therapeutic modalities such as external beam radiation has been shown to be promising; however, further data are still needed. To our knowledge, our patient is the first reported case of TBA in a setting of Sjogren syndrome.
Airway Amyloidosis: A Retrospective Analysis of 43 Patients

Abstract:Background: Amyloidosis can involve any compartment in the thorax. We aimed to explore the clinical and radiologic presentation, treatment, and clinical course of airway amyloidosis. Methods: A computer-assisted search was performed to identify patients who had biopsy-proven airway amyloidosis and were evaluated at Mayo Clinic in Rochester, MN, from January 1, 1997 through December 31, 2019. Demographic, clinical, and radiologic features along with clinical outcomes were analyzed. Results: We identified 43 patients who had airway amyloidosis. Median age was 60 years (range: 33 to 91 y), and 58% were female. Shortness of breath (63% of patients) and cough (44%) were the most common presenting symptoms. Most patients (82%) had localized amyloidosis with light chain being the most common amyloid type; 63% had tracheobronchial amyloidosis, and 23% had tracheal and upper airway involvement. On computed tomography of the chest, the most common findings were airway wall thickening with nodularity (60% of patients), airway calcification (53%), and airway occlusion without collapse (47%). On bronchoscopy (33 patients), the extent of amyloid involvement was most commonly submucosal (n=15) or nodular (n=8). External beam radiotherapy was the most common treatment modality. Among the 30 patients who had follow-up at our institution, the prognosis appeared to depend on the extent of the disease and whether patients had localized or systemic amyloidosis. Conclusion: Computed tomography of the chest, bronchoscopy, and biopsy are needed to establish the diagnosis of airway amyloidosis, and systemic amyloidosis should be ruled out. Treatment of amyloidosis requires a multidisciplinary approach.

A Rare Lung Malignancy in a Case of Systemic Sclerosis

Abstract:Systemic sclerosis (SSc) is one of the chronic autoimmune diseases characterized by the infiltration of excess collagen in various organs, especially the skin. It is found to be associated with a higher prevalence of internal malignancies, particularly lung carcinoma. Herein, we report a case of adenosquamous carcinoma confining within the lung in a patient who had long-standing SSc. She was a 55-year-old female patient presenting with progressive dry cough and breathlessness for six months. She had been a known case of diffuse cutaneous SSc for over a decade, based on 2013 American College of Rheumatology (ACR) criteria. The diagnosis is made based on her findings of bilateral thickening of the fingers on both hands, extending up to the metacarpophalangeal joints. Furthermore, she had telangiectasia at the upper chest wall and neck, multiple pitting scars at the toes, Raynaud's esophageal dilatation, and interstitial lung disease (ILD). She had been treated on Mycophenolate Mofetil 500 mg twice daily and low-dose prednisolone 5 mg once daily for 10 years. The patient's high-resolution computed tomography (HRCT) of the chest revealed a subpleural nodule in the posterior basal segment of the left lower lobe with areas of reticular opacities and interlobular septal thickening on bilateral lung fields six months earlier. The current computed tomography of the lung revealed a new 2.6 x 2.5 cm ill-defined lesion with irregular margins at the left lower lobe. A CT-guided biopsy was done for the lesion, which revealed adenosquamous carcinoma. Immunohistochemistry was consistent with a diagnosis of primary pulmonary adenosquamous carcinoma. The patient did not accept any further investigations and/or treatment. Herein, we present a rare lung malignancy, adenosquamous carcinoma of the lung with an underlying long-term diffuse cutaneous SSc in a nonsmoking female, which highlights the importance of lung cancer screening in individuals with SSc complicated with ILD and supports the fact that there is an increased prevalence of lung cancer among SSc-ILD patients than that of the regular population.

Abstract:Amyloidosis of the respiratory tract was diagnosed in 2 patients. The patients were men, 62 and 55 years ofage. The first patient presented with dyspnoea and diffuse parenchymal lung abnormalities. There was a rapidly progressive obstructive lung function disorder and a severe diffusion impairment. The second patient had haemoptysis due to tracheobronchial amyloidosis. Amyloidosis of the respiratory system is rarely diagnosed. Nearly all cases ofclinically relevant respiratory amyloidosis are due to light chain amyloid (AL amyloidosis). The described diffuse lung parenchymal abnormalities are a manifestation of systemic AL amyloidosis. On the other hand, tracheobronchial amyloidosis is a disorder which usually remains localised in the airways. Systemic AL amyloidosis may be treated with chemotherapy or stem cell transplantation. It is unknown whether this treatment leads to a decrease of pulmonary function abnormalities. Tracheobronchial amyloidosis can be treated by endobronchial therapy. Often this treatment is required repeatedly. The first patient died 2 months after diagnosis due to pneumonia. The second patient was treated with endobronchial argon plasma coagulation and diathermy and has been symptom-free for 3 years since.
Primary Laryngotracheal Amyloidosis With Bilateral Vocal Cord Involvement and Associated Bronchiectasis

Abstract:Localized tracheobronchial amyloidosis is a rare disease that results from submucosal deposition of insoluble amyloid proteins in the large airways. Amyloidosis affecting the larynx and subglottic space typically results in unilateral, nodular vocal cord infiltration. It rarely can present with bilateral vocal cord involvement and can progress to lifethreatening respiratory failure due to upper airway obstruction. In these patients, typical treatment modalities such as CO2 laser ablation are often ineffectual. Bronchiectasis is a predisposing risk factor associated with the secondary (AA) form of disease. We present a patient with the primary (AL) form of localized laryngotracheal amyloidosis with pre-existing bronchiectasis, and also review the existing literature on this disease.
Primary amyloid tumors of the lungs--six cases

Abstract:Over a period of nine years, the authors followed up six cases of primary amyloid tumors of the lungs in patients at the mean age of 58.5 years. All the patients were suspected of bronchial carcinoma and they were subjected to surgical treatment. The duration of follow-up varied, but their postoperative status was satisfactory. Immunohistochemical reactions showed deposits of AL amyloid in five cases; in one of these patients, pulmonary amyloid tumors were related to marginal zone lymphoma of the lung. In one case, the accumulated amyloid was transthyretin.

Abstract:Background Pulmonary is an uncommon site of extramedullary involvement in multiple myeloma (MM). Diffuse parenchymal amyloidosis as pulmonary manifestation of MM is even rarer. We report a rare case of diffuse parenchymal pulmonary amyloidosis associated with MM diagnosed by video-assisted thoracoscopic lung biopsy (VATLB). Case presentation A 58-year-old woman complained of cough and shortness of breath. HRCT disclosed diffuse ground-glass opacifications with interlobular septal thickening in bilateral lungs. A lung-biopsy sample obtained by VATLB revealed Congo Red-positive amorphous eosinophilic deposits in the alveolar septa. Surgical biopsy of abdominal wall skin and subcutaneous fat was also performed, which showed the apple-green birefringence with polarized light on Congo red stain was demonstrated in dermis. The serum immunoelectrophoresis showed monoclonal lambda light chains. A bone marrow biopsy specimen comprised 11.5% plasma cells. She was therefore diagnosed with diffuse parenchymal pulmonary amyloidosis accompanied by MM. The patient was referred to the hematology department for further chemotherapy. Conclusions It is important to recognize diffuse parenchymal pulmonary amyloidosis to avoid misdiagnosis.
Bronchiectatic Actinomycosis with Osseous Metaplasia Masquerading as Lung Cancer

Abstract:Bronchial involvement in pulmonary actinomycosis is rare and has been reported in the literature rarely. However, these reports describe endobronchial actinomycosis secondary to foreign body aspiration (for example, a fish bone). Our case did not have any history or clinical evidence suggesting foreign body aspiration, which makes it even more rare. A 55-year-old woman presented with complaints of on and off haemoptysis and cough for three weeks. In view of the haemoptysis and consolidation seen on imaging, a bronchoalveolar lavage was done and sent for cytological assessment. Few atypical cells with nuclear hyperchromasia and prominent nucleoli were noted. In view of the persistent haemoptysis, worsening symptoms, and non-resolution of the consolidation despite antibiotics, and the finding of atypical cells, segmental resection was done. A final diagnosis of bronchiectatic actinomycosis with osseous metaplasia was given. The patient was started on prolonged antibiotics with good response and recovery. Other risk factors associated with pulmonary actinomycosis include alcoholism, diabetes, haematological diseases, human immunodeficiency viral infection, use of immunosuppressants, and rarely chronic lung diseases, such as bronchiectasis. Our case had this rare association of bronchiectasis with bronchial actinomycosis. Bronchiectatic actinomycosis is a rare infection and it can mimic several lung disorders like unresolving pneumonia, pulmonary tuberculosis, foreign body, and even lung tumours. The pathologists and clinicians should be aware of this entity and thus help in the early diagnosis and better management of patients with this disease.

Abstract:Tracheobronchial amyloidosis is a manifestation of amyloidosis of the respiratory tract characterized by focal or diffuse deposition of amyloid in the submucosa of the trachea and proximal bronchi. Tracheobronchial amyloidosis is not associated with systemic amyloidosis or pulmonary parenchymal involvement. It affects predominantly men aged over fifty. Depending on the part of the tracheobronchial tree that is affected, stenosis of the airways causes a variety of unspecific symptoms. Diagnosis is reached by means of typical presentation in CT scan followed by bronchoscopy and histopathological confirmation. Tracheobronchial amyloidosis should be borne in mind in the differential diagnosis of patients with chronic cough and/or dyspnea or recurrent respiratory infections.
