Abstract:The global mental health crisis is looming with a rapid increase in mental disorders, limited resources, and the social stigma of seeking treatment. As the field of artificial intelligence (AI) has witnessed significant advancements in recent years, large language models (LLMs) capable of understanding and generating human-like text may be used in supporting or providing psychological counseling. However, the application of LLMs in the mental health domain raises concerns regarding the accuracy, effectiveness, and reliability of the information provided. This paper investigates the major challenges associated with the development of LLMs for psychological counseling, including model hallucination, interpretability, bias, privacy, and clinical effectiveness. We explore potential solutions to these challenges that are practical and applicable to the current paradigm of AI. From our experience in developing and deploying LLMs for mental health, AI holds a great promise for improving mental health care, if we can carefully navigate and overcome pitfalls of LLMs.

However, applying LLMs to the mental health field has also raised a series of questions regarding the accuracy, effectiveness, and reliability of the information provided. The paper mainly explores five major challenges related to the development of LLMs for psychological counseling services: 1. **Model hallucination**: It refers to the situation where a language model generates content that seems coherent but lacks factual basis or accurate representation of the source input. This limits the application of LLMs in the medical field, especially in providing accurate mental health knowledge and advice. 2. **Model interpretability**: That is, the way the model processes and generates responses is difficult to understand, which affects transparency and accountability. In mental health counseling, this lack of interpretability may lead to unexpected biases and potential harm. 3. **Privacy and regulatory issues**: Especially when using patients' electronic health records (EHR), how to ensure data security and personal privacy is an important issue. 4. **Clinical methods and effectiveness**: Although LLMs can provide emotional support, they lack true emotional understanding and the ability to provide customized guidance, which may affect their effectiveness as psychological counseling services. 5. **Bias and data**: The knowledge and style of LLMs largely depend on the training data set. If the training data lacks cultural and socioeconomic diversity, it may limit the utility of LLMs in mental health counseling. The paper also proposes potential solutions to these challenges, including improvements in data and training strategies, the establishment of protection measures, transparent and responsible development practices, human - machine collaborative deployment, and methods to reduce bias. Through these measures, the aim is to improve the applicability and safety of LLMs in the mental health counseling field.
