Abstract:Background: Mammographic density is one of the strongest predictors of breast cancer risk. The mechanism by which breast density increases breast cancer risk is unclear although it has been hypothesized that breast density reflects cumulative exposure to estrogens. Methods: To evaluate this hypothesis, we conducted a cross-sectional study among 520 postmenopausal women in the Nurses' Health Study that examined the relation between circulating sex hormones and mammographic density. Women were postmenopausal and not taking exogenous hormones at the time of blood collection and mammogram. Percent breast density was measured from digitized mammograms using a computer-assisted method. Circulating estrone, estradiol, androstenedione, testosterone, DHEA, DHEA sulfate, sex hormone-binding globulin, progesterone, and prolactin were measured in plasma. Results: In contrast to the prior hypothesis, circulating estrogens were inversely related to percent mammographic density. The mean percent mammographic density was 25.6% among women in the lowest quartile of circulating estradiol compared with 14.4% among women in the highest quartile [Spearman correlation (r) = -0.22, P < 0.0001]. Circulating estrogens alone explained 1% to 5% of the variation of mammographic density. Body mass index was positively associated with circulating estradiol levels (r = 0.45, P < 0.0001) and inversely related to percent mammographic density (r = -0.51, P < 0.0001). After adjustment for body mass index, there was no association between estradiol and breast density (r = 0.01, P = 0.81). Likewise, there was no relation between the other sex hormones measured or prolactin and mammographic density after adjustment for body mass index. Conclusion: These findings indicate that in postmenopausal women, mammographic density is independent of circulating sex hormone levels.

Association between sex hormones, glucose homeostasis, adipokines, and inflammatory markers and mammographic density among postmenopausal women

Abstract:The biological mechanisms underlying the relationship between mammographic density and breast cancer risk are unknown. Our objective was to examine the association between mammographic density and circulating factors that are putative breast cancer intermediate endpoints. Biologic data from a year-long aerobic exercise intervention trial conducted in 302 postmenopausal women aged 50-74 years were analyzed. Sex hormones, markers of glucose homeostasis, inflammatory markers, and adipokines were assayed in fasting blood drawn at baseline and after 1 year. Area and volumetric measurements of mammographic dense fibroglandular and nondense fatty tissue were made. Multiple linear regression was used to examine the association between the circulating factors and mammographic measures and partial correlations were estimated. Mammographic nondense volume was positively correlated with concentrations of estradiol (r = 0.28), estrone (r = 0.13), insulin (r = 0.41), glucose (r = 0.15), leptin (r = 0.49), and C-reactive protein (r = 0.22), and negatively correlated with sex hormone binding globulin (r = -0.30) and adiponectin (r = -0.12) but correlations became null after adjustment for overall body adiposity as represented by body mass index and waist circumference. With adjustment for overall adiposity, mammographic dense volume, a measure that represents fibroglandular tissue, was negatively correlated with leptin (r = -0.19) and C-reactive protein (r = -0.19). As expected, circulating factors originating from or correlated with adipose tissue were also correlated with mammographic measures of breast adipose tissue, but not after adjustment for overall body adiposity. Interpreting correlations between adiposity-derived factors and mammographic measures whose validity may be affected by adiposity is problematic. To rectify this problem, future studies with very good measures of the volume of fibroglandular tissue in the breast will be necessary.

Abstract:Accurate estimates of the binary, ternary, and quaternary interactions in aqueous ionic mixtures with uncommon ions with hydrophobic character are presented. For this purpose, the values of the excess Gibbs free energy of mixing, Delta m G(E), obtained from our earlier isopiestic osmotic coefficients (Kumar, A. J. Phys. Chem. B2003, 107, 2808) for the mixtures of NaCl with four guanidinium (Gn+) salts-CH3COOGn, GnNO3, GnClO4, and Gn2SO4-are analyzed with the help of the method developed by Leifer and Wigent. The methodology of Leifer and Wigent is based on the equations of Scatchard-Rush-Johnson and Friedman's cluster integral expansion theory. The Scatchard-Rush-Johnson theory explicitly considers the quaternary and higher-order ionic interactions in the mixtures as compared to the specific ion interaction theory of Pitzer, which accounts for binary and ternary interactions only. The contributions due to binary, ternary, and quaternary interaction terms to total Delta m G(E) are estimated and discussed critically. Also, the interaction between the same two cations, for example, Gn+ - Gn+, is estimated and found significant, which otherwise cannot be obtained by the use of Pitzer's theory. The information obtained from the analysis of Delta(m)G(E) is also supported by the newly measured excess volumes of mixing, Delta m V(E), at 298.15 K. The individual contributions of the binary, ternary, and quaternary interaction terms to total Delta m V(E) are described. The binary, ternary, and quaternary interaction terms for both Delta m G(E) and Delta m V(E) are analyzed in terms of Friedman's cluster integral expansion theory.

Abstract:Background: We investigated the association between established risk factors for breast cancer and mammographic breast density in Korean women. Methods This large cross-sectional study included 8 460 928 women aged >40 years, who were screened for breast cancer between 2009 and 2018. Breast density was assessed using the Breast Imaging Reporting and Data System. This study used multiple logistic regression analyses of age, BMI, age at menarche, menopausal status, menopausal age, parity, breastfeeding status, oral contraceptive use, family history of breast cancer, physical activity, smoking, drinking and hormone replacement therapy use to investigate their associations with mammographic breast density. Analyses were performed using SAS software. Results Of 8 460 928 women, 4 139 869 (48.9%) had nondense breasts and 4 321 059 (51.1%) had dense breasts. Factors associated with dense breasts were: earlier age at menarche [<15 vs. ≥15; adjusted odds ratio (aOR), 1.18; 95% confidence interval (CI), 1.17–1.18], premenopausal status (aOR, 2.01; 95% CI, 2.00–2.02), later age at menopause (≥52 vs. <52; aOR, 1.23; 95% CI, 1.22–1.23), nulliparity (aOR, 1.64; 95% CI, 1.63–1.65), never breastfed (aOR, 1.23; 95% CI, 1.23–1.24) and use of hormone replacement therapy (aOR, 1.29; 95% CI, 1.28–1.29). Women with a higher BMI and the use of oral contraceptives were more likely to have nondense breasts. Conclusion Lower BMI, reproductive health and behavioral factors were associated with dense breasts in Korean women. Additional research should investigate the relationship between mammographic breast density, breast cancer risk factors and breast cancer risk.

Abstract:More than 30 years ago, John Wolfe was the first to observe and describe the association between breast density on mammography and increased breast cancer risk. Following this pioneer work, there is now compelling evidence that density in the highest quartile represents a 4-6 times higher risk of breast cancer. This magnitude of risk is only topped by age and BRCA1/2 mutation. The density-based risk is independent of age and other risk factors. Apart from epidemiologic risk factors, additional genetic factors seem to influence density. This could be the reason behind the well-known interaction between genes and environment. Reliable and reproducible breast density measurements are a prerequisite for the use of breast density to monitor primary prevention strategies and for the use of mammographic density to define women at higher breast cancer risk who would benefit from intensified early detection and surveillance protocols.

Abstract:Breast density, the amount of fibroglandular versus fatty tissue in the breast, is a strong breast cancer risk factor. Understanding genetic factors associated with breast density may help in clarifying mechanisms by which breast density increases cancer risk. To date, 50 genetic loci have been associated with breast density, however, these studies were performed among predominantly European ancestry populations. We utilized a cohort of women aged 40-85 years who underwent screening mammography and had genetic information available from the Penn Medicine BioBank to conduct a Genome-Wide Association Study (GWAS) of breast density among 1323 women of African ancestry. For each mammogram, the publicly available "LIBRA" software was used to quantify dense area and area percent density. We identified 34 significant loci associated with dense area and area percent density, with the strongest signals in GACAT3, CTNNA3, HSD17B6, UGDH, TAAR8, ARHGAP10, BOD1L2, and NR3C2. There was significant overlap between previously identified breast cancer SNPs and SNPs identified as associated with breast density. Our results highlight the importance of breast density GWAS among diverse populations, including African ancestry populations. They may provide novel insights into genetic factors associated with breast density and help in elucidating mechanisms by which density increases breast cancer risk.
Causal effects of breast cancer risk factors across hormone receptor breast cancer subtypes: A two-sample Mendelian randomization study

Abstract:Background: It is unclear if established breast cancer risk factors exert similar causal effects across hormone receptor breast cancer subtypes. We estimated and compared causal estimates of height, body mass index (BMI), type 2 diabetes, age at menarche, age at menopause, breast density, alcohol consumption, regular smoking, and physical activity across these subtypes. Methods: We used a two-sample Mendelian randomization approach and selected genetic instrumental variables from large-scale risk factor GWAS. Publicly available summary-level data for the following subtypes were included: luminal A-like; luminal B/HER2-negative-like; luminal B-like; HER2-enriched-like; triple negative. We employed multiple methods to evaluate the strength of causal evidence for each risk factor-subtype association. Results: Collectively, our analyses indicated that increased height and decreased BMI are probable causal risk factors for all five subtypes. For the other risk factors, the strength of evidence for causal effects differed across subtypes. Heterogeneity in the magnitude of causal effect estimates for age at menopause and breast density was explained by null findings for triple negative tumours. Regular smoking was the sole risk factor for which there was no evidence for a causal effect on any subtype. Conclusions: This study suggests that established breast cancer risk factors differ across hormone receptor subtypes.
[Osseointegration of dental implants after radiotherapy for oral cancer].

Abstract:The goal of rehabilitation following radical surgery and radiotherapy for oral cancer is the restoration of oral functions and aesthetics. Osseointegrated implants improve prosthesis stability. Previous radiotherapy was originally considered a contraindication for implant placement. The aim of this study was to evaluate the survival of dental implants following radiotherapy. Nine oral cancer patients who had undergone radical surgery and radiotherapy were selected. A total of 23 implants were inserted. Twenty-one implants (91.3%) have been functioning without discomfort or infection. This study shows that osseointegrated implants should be considered part of the treatment plan for the rehabilitation of oral cancer patients after radiotherapy.

Abstract:Solubilization properties of α‐chymotrypsin and alcohol dehydrogenase (LADH) in reverse micelles are reported for three different solubilization techniques. The solubilization properties for these two proteins depend on the method used for protein addition. The addition of a dry protein powder to a reverse‐micelle‐containing organic phase does not appreciably solubilize the protein until the diameter of the reverse micelle is similar to that of the protein. However, when an aqueous protein solution is injected an organic phase, protein solubilization is not strongly dependent on micelle size. For chymotrypsin, multiple protein occupancy occurs at large micelle size, with as many as 11 chymotrypsin molecules solubilized in one reverse micelle. The solubilization of chymotrypsin using a phase‐transter technique with a positively charged surfactant follows the expected traned based on protein‐surfactant electrostatic interactions. When a negatively charged sufactants is used for phase transfer, at low pH the solubilization data do not fit this electrostatic interaction mechanism. In this case, proteinsurfactant aggregation may be occurring at the aqueousorganic interface.
Lumbar Laminectomy for the Resection of Synovial Cysts and Coexisting Lumbar Spinal Stenosis or Degenerative Spondylolisthesis: An Outcome Study

Abstract:Study Design. Surgeon- and patient-based (SF-36) outcome measures were used to assess the results of decompressive laminectomies for the excision of synovial cysts with coexistent lumbar spinal stenosis (45 patients) or for synovial cysts with coexistent lumbar stenosis and degenerative spondylolisthesis (35 patients). Objectives. To evaluate the results following laminectomy and the excision of synovial cysts/stenosis with or without olisthy. Summary of Background Data. SF-36 outcome studies evaluating these surgical results deserve further investigation. Methods. Before surgery, patients with synovial cysts/ stenosis (45 patients) or cysts/stenosis/degenerative spondylolisthesis (35 patients), respectively, exhibited low back pain (40 and 33 patients), radiculopathy (43 and 33 patients), and neurogenic claudication (41 and 26 patients). Surgery required average 3.8 and 3.5 level laminectomies, respectively, for patients with cysts/stenosis and cysts/stenosis and olisthy. Outcomes were assessed 2 years after surgery. Results. Five of 45 patients undergoing laminectomy alone for cysts/stenosis developed postoperative olisthy. Of 35 patients with cysts/stenosis and preoperative Grade 1 degenerative spondylolisthesis, olisthy increased after surgery to Grade 2 in 11 patients. Good/excellent results (58% and 63%) and SF-36 improvement on the Physical Function Scale (+44 and +38 points) were, respectively, documented for these two groups. Conclusions. Using both surgeon and SF-36 outcome measures, 2 years following laminectomy for synovial cysts/lumbar stenosis with or without olisthy, patients exhibited a moderate degree of improvement. As synovial cysts reflect disruption of the facet joint and some degree of instability, primary fusion should be considered to improve operative results for patients in both categories.

Abstract:Background: Mammogram risk scores based on texture and density, determined by brightness thresholds, are associated with breast cancer risk differently and could reveal distinct information about breast cancer risk. This study aimed to investigate the causal relationships between these intercorrelated risk scores and the familial aggregation of a texture-based mammogram measure, shedding light on the relevance to breast cancer aetiology. Methods: We analysed data from the Australian Mammographic Density Twins and Sisters Study, including 3195 breast-cancer-free women from 1519 families. The dataset comprised 527 pairs of monozygotic (MZ) twins, 271 pairs of dizygotic (DZ) twins, and 1599 sisters. The average age at mammography for sister pairs ranged from 40 to 70 years. Mammogram risk scores based on texture (named Cirrus) and three density-defined areas (light, bright, and brightest) that were spatially independent were generated. The Inference about Causation from Examination of Familial Confounding (ICE FALCON) method was employed for causal inference. Familial aggregation and the variance of Cirrus were assessed as a function of age using a multivariate normal model for pedigree analysis. The classic twin model was employed but allowing for varying shared environmental effects among different sister pairs. The single-nucleotide polymorphism (SNP)-based heritability was estimated using genetic variants from both closely and distantly related individuals. Model fits were compared using likelihood ratio tests and the Akaike Information Criterion. Results: Causal analysis using ICE FALCON revealed significant positive causal effects, with Cirrus, light areas, and bright areas influencing the brightest areas (accounting for 34%, 55%, and 85% of the associations, respectively). Similarly, light areas and bright areas demonstrated causal effects on Cirrus (accounting for 37% and 28% of the associations, respectively). No age-related differences in familial correlations or Cirrus variance were found for women aged 40 to 70 years. Familial correlations were estimated at 0.51 (0.03) for MZ pairs and 0.16 (0.03) for combined DZ and non-twin sister pairs, with no significant difference between DZ and non-twin sister pairs (P=0.3). Additive genetic effects accounted for up to 32% (5%) of the variance, consistent with SNP-based heritability estimates of 36% (12%). MZ-specific environmental effects contributed to at least 20% (3%), while shared environmental effects for DZ and non-twin sibling pairs accounted for less than 40% of that for MZ pairs. Conclusion: In a mammogram, the lighter (less dense) areas causally influence the brightest (highly dense) areas, including through textural features. These causal relationships provide insights into the relative importance of different mammogram features in breast cancer aetiology. For example, the brightest areas are more aetiologically important for screen-detected breast cancer, while the light areas are more aetiologically important for interval breast cancer. Additionally, specific textural features capture aetiologically independent breast cancer risk information from dense areas. We also confirm the partial heritability of Cirrus, the texture-based mammogram measure, substantially influenced by environmental factors (familial and non-familial). As well as genetic factors, Cirrus could be determined by factors operating at times of life when MZ pairs share their environments to a greater extent than do DZ and non-twin sisters, starting in utero and prior to adulthood, and remaining constant thereafter. These insights from twin and family data enhance our understanding of mammogram risk scores and their implications for breast cancer risk assessment. Genetically Predicted Sex Hormone Levels and Health Outcomes: Phenome-Wide Mendelian Randomization Investigation.

Abstract:Background Sex hormone-binding globulin (SHBG), testosterone and oestradiol have been associated with many diseases in observational studies; however, the causality of associations remains unestablished. Methods A phenome-wide Mendelian randomization (MR) association study was performed to explore disease outcomes associated with genetically proxied circulating SHBG, testosterone and oestradiol levels by using updated genetic instruments in 339 197 unrelated White British individuals (54% female) in the UK Biobank. Two-sample MR analyses with data from large genetic studies were conducted to replicate identified associations in phenome-wide MR analyses. Multivariable MR analyses were performed to investigate mediation effects of hormone-related biomarkers in observed associations with diseases. Results Phenome-wide MR analyses examined associations of genetically predicted SHBG, testosterone and oestradiol levels with 1211 disease outcomes, and identified 28 and 13 distinct phenotypes associated with genetically predicted SHBG and testosterone, respectively; 22 out of 28 associations for SHBG and 10 out of 13 associations for testosterone were replicated in two-sample MR analyses. Higher genetically predicted SHBG levels were associated with a reduced risk of hypertension, type 2 diabetes, diabetic complications, coronary atherosclerotic outcomes, gout and benign and malignant neoplasm of uterus, but an increased risk of varicose veins and fracture (mainly in females). Higher genetically predicted testosterone levels were associated with a lower risk of type 2 diabetes, coronary atherosclerotic outcomes, gout and coeliac disease mainly in males, but an increased risk of cholelithiasis in females. Conclusions These findings suggest that sex hormones may causally affect risk of several health outcomes.

Thus, we conducted a comprehensive two-sample Mendelian randomization (MR) analysis to systematically investigate the causal associations of 109 traits with both CAD and cancer to identify their shared risk and protective factors. Methods: The genetic association datasets pertaining to exposure and outcomes were reviewed using the most recent and public genome-wide association studies (GWAS). Inverse variance weighting (IVW), weighted median (WM), and MR-Egger strategies were implemented for the MR analyses. The heterogeneity and pleiotropy were measured utilizing leave-one-out sensitivity testing, MR-PRESSO outlier detection, and Cochran's Q test. Results: The IVW analyses revealed that genetic-predicted mean sphered cell volume (MSCV) is a protective factor for CAD, and weight is a risk factor. MSCV and weight also show similar effects on cancer. Furthermore, our study also identified a set of risk and protective factors unique to CAD and cancer, such as telomere length. Conclusions: Our Mendelian randomization study sheds light on shared and unique risk and protective factors for CAD and cancer, offering valuable insights that could guide future research and the development of personalized strategies for preventing and treating these two significant health issues.

Abstract:Background: Higher mammographic density (MD), a radiological measure of the proportion of fibroglandular tissue in the breast, and lower terminal duct lobular unit (TDLU) involution, a histological measure of the amount of epithelial tissue in the breast, are independent breast cancer risk factors. Previous studies among predominantly white women have associated reduced TDLU involution with higher MD. Methods: In this cohort of 611 invasive breast cancer patients (ages 23-91 years [58.4% ≥ 50 years]) from China, where breast cancer incidence rates are lower and the prevalence of dense breasts is higher compared with Western countries, we examined the associations between TDLU involution assessed in tumor-adjacent normal breast tissue and quantitative MD assessed in the contralateral breast obtained from the VolparaDensity software. Associations were estimated using generalized linear models with MD measures as the outcome variables (log-transformed), TDLU measures as explanatory variables (categorized into quartiles or tertiles), and adjusted for age, body mass index, parity, age at menarche and breast cancer subtype. Results: We found that, among all women, percent dense volume (PDV) was positively associated with TDLU count (highest tertile vs. zero: Expbeta = 1.28, 95% confidence interval [CI] 1.08-1.51, ptrend = < .0001), TDLU span (highest vs. lowest tertile: Expbeta = 1.23, 95% CI 1.11-1.37, ptrend = < .0001) and acini count/TDLU (highest vs. lowest tertile: Expbeta = 1.22, 95% CI 1.09-1.37, ptrend = 0.0005), while non-dense volume (NDV) was inversely associated with these measures. Similar trend was observed for absolute dense volume (ADV) after the adjustment of total breast volume, although the associations for ADV were in general weaker than those for PDV. The MD-TDLU associations were generally more pronounced among breast cancer patients ≥ 50 years and those with luminal A tumors compared with patients < 50 years and with luminal B tumors. Conclusions: Our findings based on quantitative MD and TDLU involution measures among Chinese breast cancer patients are largely consistent with those reported in Western populations and may provide additional insights into the complexity of the relationship, which varies by age, and possibly breast cancer subtype.

Abstract:The purpose of the present study was to examine whether metabolic acidosis affects the expression of rat renal Na(+)/citrate cotransporter. Female Wistar rats were pair-fed with normal rat chow and drinking water (control) or water with 0.28 mol/L NH4Cl (metabolic acidosis). The mRNA of two renal Na(+)/citrate cotransporters, which were respectively expressed on apical and basolateral membrane, were measured by Northern blot with two probes, SDCT1 and SDCT2. Animals were sacrificed on day 1,3 and 7. On the 1st day, the blood plasma HCO(-)3 of acidosis group decreased significantly (P<0.01), but the mRNA abundance did not change. On the 3rd day in the acidosis group, the blood plasma HCO(-)3 increased slightly more than that on the 1st day, but was still significantly lower than that of the control group (P<0.01). Both the probes detected some increase in mRNA of brush border and basolateral Na(+)/citrate cotransporter. On the 7th day, the blood plasma HCO(-)3 of the acidosis group continuously increased and there was no significant difference between the two groups. The abundance of brush border and basolateral Na(+)/citrate cotransporter mRNA increased, but there was no difference between those of the 3rd day and the 7th day. It is concluded that metabolic acidosis can induce increase of Na(+)/citrate cotransporter mRNA, which may be responsible for hypocitraturia.
The association between mammographic density and breast cancer molecular subtypes: a systematic review and meta-analysis

Abstract:Aim: To conduct a systematic review and meta-analysis to evaluate the whether high mammographic density (MD) is differentially associated with all subtypes of breast cancer. Materials and methods: The PubMed, Cochrane Library, and Embase databases were searched systematically in October 2022 to include all studies that investigated the association between MD and breast cancer subtype. Aggregate data of 17,193 breast cancer cases from 23 studies were selected, including five cohort/case-control and 18 case-only studies. The relative risk (RR) of MD were combined using random/fixed effects models for case-control studies, and for case-only studies, relative risk ratios (RRRs) were a combination of luminal A, luminal B, and HER2-positive versus triple-negative tumours. Results: Women in the highest density category in case-control/cohort studies had a 2.24-fold (95% confidence interval [CI] 1.53, 3.28), 1.81-fold (95% CI 1.15, 2.85), 1.44-fold (95% CI 1.14, 1.81), and 1.59-fold (95% CI 0.89, 2.85) higher risk of triple-negative, HER-2 (human epidermal growth factor receptor 2) positive, luminal A, and luminal B breast cancer compared to women in the lowest density category. RRRs for breast tumours being luminal A, luminal B, and HER-2 positive versus triple-negative in case-only studies were 1.62 (95% CI 1.14, 2.31), 1.81 (95% CI 1.22, 2.71) and 2.58 (95% CI 1.63, 4.08), respectively, for BIRADS 4 versus BIRADS 1. Conclusion: The evidence indicates MD is a potent risk factor for the majority of breast cancer subtypes to different degrees. Increased MD is more strongly linked to HER-2-positive cancers compared to other breast cancer subtypes. The application of MD as a subtype-specific risk marker may facilitate the creation of personalised risk prediction models and screening procedures.
