Abstract:Objective:To evaluate the influence of atorvastatin on ion pump activity of smooth muscle cells and intima proliferation in rats after arterial intimal injury. Method:Twenty-seven rats were divided randomly into sham group (n=7), control group (n=10) and atorvastatin group (n=10). Animal models of carotid arterial intimal injury were established in the latter two groups, and those in atorvastatin group were administrated Atorvastatin 30 mg·kg -1·d -1 from the third before the operation to the 28th day after the operation. On the 28th day, all of the rats were killed for the measurement of Na+-K+-ATPase and Ca 2+-Mg 2+-ATPase activities of smooth muscle cells from carotid artery. Intimal hyperplasia was observed with HE staining, and its maximum thickness were detected by a computer image analysis system. Result:Activities of Na+-K+-ATP ase and Ca 2+-Mg 2+-ATPase were significantly higher in sham and atorvastatin groups than those in control group, P<0.01. The maximum thickness of intimal hyperplasia in control group was markedly higher than that in atorvastatin group,P<0.01. Conclusion:Atorvastatin might inhibit the process of vascular remodeling to prevent vascular restenosis by increasing ion pump activity of vascular smooth muscle cells.

Abstract:The onset of impotence was related to penile trauma in ten of 90 patients who underwent angiography. Angiographic results facilitated diagnosis in all patients. Trauma was secondary to a variety of injuries including fractures, soft-tissue perineal injuries without fractures, and repeated perineal pounding during long, jarring drives. The final diagnosis was arteriogenic impotence (n = 2), arteriovenogenic impotence (n = 2), venogenic impotence (n = 3), venoneurogenic impotence (n = 1), neurogenic impotence (n = 1), and psychogenic impotence (n = 1). Proper angiographic evaluation required refined angiographic techniques such as selective magnification pharmacoarteriography, pharmacocavernosography, and pharmacocavernosometry. A variety of therapeutic options were either recommended or applied, depending on the angiographic findings. These included venous ligation, arterial bypass, injections of papaverine hydrochloride, and insertion of penile prostheses.

Abstract:Poor graft function (PGF) is a serious complication after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Murine studies suggest that endothelial progenitor cells (EPCs) are preferential supporting cells for hematopoietic stem cells in the bone marrow (BM) microenvironment. Our previous work found that a reduced number of BM EPCs was an independent risk factor for the occurrence of PGF after allo-HSCT. However, little is known about the functional role of BM EPCs and how to improve impaired BM EPCs in PGF. In the current study, we evaluated the function of BM EPCs in subjects with PGF postallotransplant. Moreover, we investigated whether atorvastatin could enhance the number and function of BM EPCs derived from subjects with PGF in vitro. Dysfunctional BM EPCs, which were characterized by impaired proliferation, migration, angiogenesis, and higher levels of reactive oxygen species and apoptosis, were revealed in subjects with PGF. Activation of p38 and its downstream transcription factor cyclic adenosine monophosphate-responsive element-binding protein were detected in BM EPCs from subjects with PGF. Furthermore, the number and function of BM EPCs derived from subjects with PGF were enhanced by atorvastatin treatment in vitro through downregulation of the p38 MAPK pathway. In summary, dysfunctional BM EPCs were observed in subjects with PGF. Atorvastatin treatment in vitro quantitatively and functionally improved BM EPCs derived from subjects with PGF through downregulation of the p38 MAPK pathway. These data indicate that atorvastatin represents a promising therapeutic approach for repairing impaired BM EPCs in subjects with PGF postallotransplant.
Atorvastatin Decreased Low Density Lipoprotein Level and Improved Coronary Artery Lesion in Patients after Percutaneous Coronary Intervention

Abstract:Objective To evaluate antihyperlipidemia effect of atorvastatin treatment on coronary artery disease (CAD) prognostic index in patients underwent percutaneous coronary intervention (PCI) after treatment. Methods CAD patients diagnosed CAD by CAG and underwent PCI between 2005-01 - 2008-01 were enrolled. Low density lipoprotein (LDL) , creatine kinase (CK) , alanine aminotransferase (ALT) level were determined before and after treatment. The coronary artery pathological changes were estimated with CAD prognostic index. Patients received atorvastatin (20 mg/d) for average (15. 1±3. 1) months. Based on decreases in LDL levels, patients were divided into decreases >30% or <30% of baseline levels, Concomitantly, the CAG indexes were estimated in relative to the level of LDL-C. Results According to " China Adult Dyslipidemia Prevention Guide", 74. 4% of the patients achieved the ideal LDL level. Compared with the non-progress CAG index group, a smaller decreased of LDL[(29. 6±24. 5) % vs (9. 1±37. 0) % , P<0. 01] were shown in CAG index progressed patients. However, all these patients had an apparent increases in ALT [(28. 8±15. 1) vs (25. 2±13. 8) U/L, P<0. 05] and CK level [(99. 0±58. 2) vs (83. 9±45. 3) U/L, P<0. 01] after atorvastatin treatment. Patients with LDL decrease <30% are more likely to have increased CAD prognostic indexes(43. 1% vs 64. 9%, P< 0. 05). Conclusion The ideal LDL-C level could be achieved by atorvastatin (20 mg/d) in 74. 4% post-PCI patients. The magnitude of decreases in LDL-C was related to the amelioration of coronary artery disease.

Abstract:Statins, the 3-hydroxy-3-methylglutaryl-cooenzyme A (HMG-CoA) reductase inhibitors, have been shown to inhibit the migration of macrophages and smooth muscle cell proliferation leading to an antiproliferative effect. Although this beneficial effect of statins has been suggested to be independent of lipid lowering properties, the possible mechanisms responsible for this action is largely unknown. Gap junctions, which serve as channels for direct intercellular exchange of ions, secondary messengers, and small signaling molecules, play an important role in tissue homeostasis and regulation of growth, differentiation, and development. This study was designed to test the hypothesis that expression of the component proteins of gap junctions, connexins 40 and 43 (Cx40 and Cx43), is upregulated in arteries subjected to balloon injury and that this upregulation can be suppressed by statin therapy. Male New Zealand white rabbits were subjected to injury in which an angioplasty catheter was introduced into the right iliac artery from the femoral artery under fluoroscopic guidance. Five groups of rabbits (n = 6 to 7) were treated for 2 weeks with one of the following: balloon injury (BL); BL+lovastatin (BL+L, 10 mg/kg/day); BL+fluvastatin (BL+F, 10 mg/kg/day); sham operation (Sham); and control (Con). Immunohistochemistry studies showed that Cx40 and Cx43 were expressed in normal smooth muscle cells (SMC) throughout the media. Reverse transcription-polymerase chain reaction and Western blot analysis showed that Cx40 and Cx43 mRNA and protein expression was elevated after injury (P < .001 for both proteins and both assays), and these elevations were suppressed by lovastatin and fluvastatin to a similar degree (P < .05 for both drugs and both assays). Immunostaining of Cx40 and Cx43 was consistently enhanced in the neointimal area after injury and lovastatin and fluvastatin reduced staining of these proteins in the lessened neointimal layer. Transmission electron microscopy revealed that there were abundant gap junctions between neointimal SMC as well as fewer and smaller gap junctions after statin treatment. Therefore, balloon injury causes upregulation of Cx40 and Cx43 in neointimal SMC. Lovastatin and fluvastatin suppress upregulated Cx40 and Cx43 expression and reduce neointimal proliferation, suggesting that Cx40 and Cx43 may play a role in statin-induced antiproliferative effect.
Statins Contribute to Enhancement of the Number and the Function of Endothelial Progenitor Cells from Peripheral Blood

Abstract:The aim of the present study was to investigate whether fluvastatin augments the number of endothelial progenitor cells (EPCs), and promotes EPCs proliferation, migration and adhesion. Total mononuclear cells (MNCs) were isolated from peripheral blood by Ficoll density gradient centrifugation. The cells were then plated on fibronectin-coated culture dishes. After being cultured for 7 d, the attached cells were stimulated with fluvastatin (final concentrations: 0.01, 0.1, 1, 10 micromol/L), simvastatin (1 micromol/L) or a vehicle for the respective time points (6, 12, 24 and 48 h). EPCs were characterized as adherent cells double positive for DiLDL-uptake and lectin binding by direct fluorescent staining under a laser scanning confocal microscope. EPCs were further documented by demonstrating the expression of KDR, VEGFR-2 and AC133 with flow cytometry. EPCs proliferation, migration and in vitro vasculogenesis activity were assayed by MTT assay, modified Boyden chamber assay and in vitro vasculogenesis kit, respectively. EPCs adhesion assay was performed by replating it on fibronectin-coated dishes, and the adherent cells were then counted. In addition, we also studied EPCs culture assay of peripheral blood from fluvastatin-treated animals in vivo. Incubation of isolated human MNCs with fluvastatin dose- and time-dependently increased the number of EPCs, while reached the maximum 24 h after the administration at 1 micromol/L, (2.5-fold increase, P<0.05). Moreover, treatment of rats with fluvastatins elevated the number of EPCs (3-fold increase, P<0.05), thus extending the in vitro data. In addition, fluvastatin also promoted EPC proliferation, migration, adhesion and in vitro vasculogenesis in a concentration-dependent manner. The effects of fluvastatin on EPCs were compared with those of simvastatin at the same concentration (1 micromol/L), with a result of no statistical difference. The results of the present study define a novel mechanism of the action of statins: the augmentation of EPCs with enhanced functional activity.
The Effect and Underlying Mechanism of Statins in the Prevention of Post-Contrast Acute Kidney Injury: A Multicenter Retrospective Observational Study

Abstract:Abstract Background: Post-Contrast acute kidney injury (PC-AKI) is a severe complication of coronary angiography (CAG) and percutaneous coronary intervention (PCI). At present, the mechanism and effect of statins on PC-AKI and its mechanism are still unclear. Methods: It was a multicenter retrospective obeservational study. We reviewed 4386 patients who underwent CAG or PCI, with serum creatinine recorded pre- or post-procedure within 72hrs after PCI, from December, 2006 to December, 2019 admitted to Sir Run Run Shaw Hospital and its medical consortium hospitals. Multivariate logical regression was used to explore whether preoperative use of statins was a protective factor for PC-AKI, and then path analysis model was used to explore the specific mechanism of statins. Results: In total, 4386 patients were enrolled in the study, the mean age was 67 yrs old, 17.9% with PC-AKI, 83.3% with pre-operative statin therapy. The incidence of PC-AKI was significantly lower in statin therapy group than non-statins therapy group. Multivariate regression indicated that pre-operative statin therapy was significantly negatively associated with percentage of elevated creatinine (β: -0.118, p<0.001) and PC-AKI (OR: 0.575, p<0.001). In pre-operative statin therapy group, no statistically significant deference was detected between atorvastatin and rosuvastatin group (OR: 1.052, p=0.558). Pathway model analysis indicated the direct protective effectiveness of pre-operative statin therapy on PC-AKI (p<0.001), but neither with its lipid-lowering effect (p=0.277) nor anti-inflammatory effect (p=0.596). Furthermore, It was found that “LDL-C→CRP” mediated the relationship between pre-operative statin therapy and PC-AKI (p=0.007); however, only explained less than 1% of the pre-operative statin therapy’s protective effects on PC-AKI. Conclusion: Pre-operative statin therapy is an independent protective factor of PC-AKI, which is not affected by the type of statins and not achieved by lipid-lowering effect or anti-inflammatory effect. [Effects of Atorvastatin on Endothelium Protection in Spontaneously Hypertensive Rats].

Abstract:OBJECTIVE:To investigate the effects of two different dosage of atorvastatin on endothelium protection in spontaneously hypertensive rats (SHR).METHODS:SHRs (n=18) were randomly divided into three groups (n=6): SHR control group, 50 mg atorvastatin group and 10 mg group. Six male Wistar-Kyoto rats were selected as normal control group (WKY group). All animals were given vehicle once a day by gavage for 10 weeks. Systolic blood pressure (SBP) was measured before and after treatment with atorvastatin every 2 weeks. Plasma NO and vWF were measured by nitrate reductase method and double antibodies ELISA, respectively. Plasma lipids were also measured.RESULTS:SBP in all SHR groups was much higher than that in WKY group before experiment (P<0.01). Compared with SHR control group, SBP significantly decreased in 50 mg atorvastatin group at 6, 8 and 10 weeks and in 10 mg atorvastatin group merely at 10 weeks (P<0.05 or P<0.01). The plasma levels of NO in SHR control group was significantly lower than those in WKY group (P<0.01). After 10 weeks, plasma NO levels in 50 mg and 10 mg atorvastatin groups were markedly higher than those in SHR control group (P<0.01 or P<0.05). The plasma levels of vWF in SHR control group was significantly higher than those in WKY group (P<0.01). After 10 weeks, plasma vWF levels in 50 mg and 10 mg atorvastatin groups were markedly lower than those in SHR control group (P<0.01). Plasma lipids in 50 mg and 10 mg atorvastatin groups were lower than those in SHR control group (P<0.05 or P<0.01).CONCLUSION:Atorvastatin can decrease blood pressure and significantly improve endothelial function in SHR by increasing plasma NO level and decreasing plasma vWF level.
Efficacy of High-dose Statins Pretreatment on PCI-related Inflammation and Myocardial Injury

Abstract:Objective To explore the effect of atorvastat in pretreatment on percutaneous coronary intervention ( PCI )-related inflammation and myocardial injury. Methods Eighty six patients with coronary heart diseases undergoing PCI were randomized into A group (n=30), B group (n=26) and C group (n=30). They were given 20, 80 and 0 mg atorvastatin tablet for 2 days before PCI, respectively. A and C group patients took 20 mg atorvastatin every night after PCI, well the B group took 40 mg per night. The neutrocyte count, the levels of high sensitivity C reactive protein (hs-CRP), enzyme, troponin T (cTnT), liver function, kidney function, blood lipids and electrocardiogram were determined before and after PCI. The incidence of perioperative myocardial infarction (MI) after successful PCI was also recorded. Results The changes of hs- CRP level, neutrocyte count were significant after PCI (P0.05). The degree of △hs-CRP, creatine kinase MB (CK-MB), cTnT elevation in B group was significantly less than that of A and C group (P0.05). Incidence of perioperative MI was lowest in the B group (P0.05). The highest compliance rate of LDL-C postoperative also in the B group (P0.05). The changes of liver and kidney function, creatine kinase were not significant before and after PCI in 3 groups (P0.05). Conculsion Pretreatment with atorvastatin 80 mg for patients under going PCI for 48 hours can inhibit PCI-related inflammation and have a protective effect against myocardial injury. Safety did not differ between high-dose and low-dose atorvastatin.
Effects of Atorvastatin Loading Prior to Primary Percutaneous Coronary Intervention on Endothelial Function and Inflammatory Factors in Patients with ST-segment Elevation Myocardial Infarction

Abstract:Previous studies have demonstrated the beneficial effect of statin loading prior to elective and early percutaneous coronary intervention (PCI), in which the ‘pleiotropic effects’ of statins may contribute to these clinical benefits. The aim of the present study was to examine the potential effects of atorvastatin loading prior to primary PCI on coronary endothelial function and inflammatory factors in patients with acute ST-segment elevation myocardial infarction (STEMI). A total of 60 patients with STEMI were randomized into three groups: Loading dose (80 mg atorvastatin prior to PCI; n=20), regular dose (20 mg atorvastatin prior to PCI; n=20) and control (without atorvastatin prior to PCI; n=20). The plasma samples were collected prior to, and immediately, 6 and 24 h after PCI in all the patients. The plasma concentrations of endothelial nitric oxide synthase (eNOS), nitric oxide (NO), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and intercellular adhesion molecule-1 (ICAM-1) were examined using ELISA. The plasma eNOS levels immediately and 24 h after PCI were significantly higher in the regular dose group compared with the other groups. However, there were no significant differences in the plasma eNOS concentration prior to and 6 h after PCI, or in the plasma NO concentration at any of the time-points among the three groups. The plasma IL-6 levels prior to PCI were significantly lower in the loading dose group compared with the other groups; however, there were no significant differences in the plasma concentration of IL-6 following PCI or in the concentrations of TNF-α and ICAM-1 at any of the time-points among the three groups. In conclusion, atorvastatin loading in patients with STEMI undergoing primary PCI may not have protective effects on endothelial function and the inflammatory reaction.
The Predicted Coiled-coil Domain of Myosin 10 Forms a Novel Elongated Domain That Lengthens the Head*

Abstract:Myosin 10 contains a region of predicted coiled coil 120 residues long. However, the highly charged nature and pattern of charges in the proximal 36 residues appear incompatible with coiled-coil formation. Circular dichroism, NMR, and analytical ultracentrifugation show that a synthesized peptide containing this region forms a stable single α-helix (SAH) domain in solution and does not dimerize to form a coiled coil even at millimolar concentrations. Additionally, electron microscopy of a recombinant myosin 10 containing the motor, the three calmodulin binding domains, and the full-length predicted coiled coil showed that it was mostly monomeric at physiological protein concentration. In dimers the molecules were joined only at their extreme distal ends, and no coiled-coil tail was visible. Furthermore, the neck lengths of both monomers and dimers were much longer than expected from the number of calmodulin binding domains. In contrast, micrographs of myosin 5 heavy meromyosin obtained under the same conditions clearly showed a coiled-coil tail, and the necks were the predicted length. Thus the predicted coiled coil of myosin 10 forms a novel elongated structure in which the proximal region is a SAH domain and the distal region is a SAH domain (or has an unknown extended structure) that dimerizes only at its end. Sequence comparisons show that similar structures may exist in the predicted coiled-coil domains of myosins 6 and 7a and MyoM and could function to increase the size of the working stroke.

Abstract:Although several studies have suggested that intensive statin pretreatment could reduce the incidence of procedure-related myocardial infarction in western population, the data on the effect in Asian patients have been still limited. The aim of the study was to investigate the efficacy and safety of intensive atorvastatin load in Chinese patients undergoing elective PCI. A total of 1202 patients with stable angina or non-ST-segment elevation acute coronary syndrome (NSTE-ACS) scheduled to undergo PCI received either intensive statin treatment (80 mg atorvastatin daily x 2 days before PCI and 40 mg daily x 30 days after PCI) or usual care. The primary endpoint was incidence of major adverse cardiac events (cardiac death, myocardial infarction, or unexpected target vessel revascularization) within 30 days after PCI. Safety endpoints include the incidence of contrast induced nephropathy (CIN), ALT/AST >3 upper limit of normal (ULN), CK >5 ULN. The incidence of 30-day MACE did not significantly differ between the intensive group and control group (19.4 vs 18.3%, P = 0.63). Multivariate analysis revealed age (OR = 1.024, 95% CI 1.003-1.045, P = 0.023) and total stent length as an independent predictor of 30-day MACE (OR = 1.012, 95% CI 1.007-1.018, P < 0.0001). The incidence of CIN was comparable between intensive group and control group (4.09 vs 4.39%, P = 0.795). No significant differences were observed in other safety profile at all follow-ups between treatment groups. The ISCAP trial demonstrated that serial intensive atorvastatin therapy did not improve the clinical outcome with similar safety profile comparing with usual care among Chinese patients undergoing elective PCI.

Abstract:ObjectivesWe investigated the effects of short-term use of atorvastatin on CD34+/VEGF-R2+/CD133+/CD45- endothelial progenitor cell (EPC) count after on-pump coronary artery bypass surgery (CABG).MethodsBetween Feb-2010 and May-2010, we randomly assigned, in a placebo-controlled, double-blind study, 60 consecutive patients who underwent isolated, first-time CABG to receive either 14-day atorvastatin (40 mg/day) or placebo preoperatively. Urgent CABG and recent myocardial infarction were excluded. EPCs were quantified (cells/μl) by flow cytometric phenotyping obtained from venous blood samples collected preoperatively (T1), 6-hours (T2), and on the 5th day postoperatively (T3). Levels of markers of inflammation and serum cardiac troponin I were also measured preoperatively and daily until day-5 after surgery.ResultsThere were no differences in baseline risk factors including cholesterol profiles, and EuroSCORES between the groups. The composite primary end-point, favored statin group with higher amount of circulating, early EPC count (cells/μl) at all time points compared with placebo (T1, 2.30 ± 0.02 versus 1.58 ± 0.03, p < 0.001; T2, 5.00 ± 0.06 versus 2.19 ± 0.06, p < 0.001; T3, 3.03 ± 0.08 versus 1.78 ± 0.02, p < 0.001). Postoperative hsCRP rise were inversely correlated with EPC count, and were significantly lower in the statin group (T1, 0.8 ± 0.1 versus 2.2 ± 1.5, p < 0.001; T2, 72.9 ± 3.2 versus 96.0 ± 3.6, p < 0.001; T3, 4.3 ± 1.2 versus 11.4 ± 4.1, p < 0.001). Furthermore, the incidence of postoperative atrial fibrillation was significantly lower in the statin group compared to placebo (3.3% versus 23%, p = 0.02).ConclusionsShort-term atorvastatin use increases circulating early EPCs both pre- and post-operatively and is associated with better preservation of sinus rhythm and reduced hsCRP levels. Abstract:Objectives The Novel approaches for preventing or limiting events (NAPLIES) and The Atorvastatin for Reduction of Myocardial Damage during Angioplasty (ARMYDA) studies demonstrated a beneficial effect of statin loading in preventing major adverse cardiac events (MACE) after elective percutaneous coronary intervention (PCI) for stable angina, unstable angina, non-ST-segment-elevation myocardial infarction (NSTEMI). The so called ‘pleiotropic effects’ of statins include modulation of endothelial function, inhibition of inflammation, and attenuation of thrombosis, all of which could provide clinical benefits in the setting of elective PCI by reducing postprocedural incidence of myocardial and MACE. So far, the efficacy of atorvastatin loading in patients with acute ST-segment-elevation myocardial infarction (STEMI) undergoing primary PCI has not been confirmed. Also, whether the ‘pleiotropic effects’ of statins could explain the possible mechanism(s) needs to be discussed. This study sought to explore potential protective effects of statin loading before primary PCI on coronary endothelial function, inflammation, and MACE. Methods A total 60 patients with STEMI were randomised into loading dose group (80 mg atorvastatin before PCI, n=20), regular dose group (20 mg atorvastatin before PCI, n=20), and control group (without atorvastatin before PCI, n=20). All patients received primary PCI and routine treatment. The plasma samples were collected before, immediately after, 6 h after and 24 h after PCI in all the patients. Plasma concentrations of endothelial nitric oxide synthase (eNOS), Nitric Oxide (NO), interleukin-6 (IL-6), tumour necrosis factor (TNF-а), intercellular adhesion molecule-1 (ICAM-1) were tested by ELISA. The results of coronarography, electrocardiogram, myocardial enzyme, high-sensitivity C-reactive protein (hs-CRP), amino terminal-pro brain natriuretic peptide (NT-proBNP), echocardiography, MACE, and the safety of statin loading were also collected. Results Plasma eNOS immediately and 24 h after PCI were higher in the regular dose group (p<0.05). Plasma eNOS before and 24 h after PCI, along with plasma NO at any time point did not show significant differences among the 3 groups. Plasma IL-6 before PCI were lower in the loading dose group (90.773±7.646 pg/ml vs 95.592±4.269 pg/ml vs 94.324±3.692 pg/ml, p=0.023). Plasma IL-6 after PCI, plasma TNF-a and ICAM-1 at any time point did not show significant differences among the 3 groups. MACE occurred in 2 (10.0%) patients in the loading dose group, 2 (10.0%) patients in the regular dose group, 3 (15.0%) patients in the control group, respectively (p=0.855). Conclusions Atorvastatin loading in patients with STEMI undergoing primary PCI may not have protective effects on coronary endothelial function, inflammation, and MACE.
Atorvastatin Regulates Pericardial Patch Healing Via the Microrna140-Adam10-ephrinb2 Pathway

Abstract:Background: Pericardial patches are frequently used in vascular surgery to close arteriotomies. The early healing of these patches is mediated by attraction of CD34 and ephrinB2-positive cells. Atorvastatin is a commonly used statin drug that promotes healing of cardiovascular injury. We hypothesized that atorvastatin attracts ephrinB2-positive cells by regulating the microRNA140-ADAM10-ephrinB2 pathway during patch healing in the arterial environment. Methods: Pericardial patches were used to close an infra-renal aortic arteriotomy in Wistar rats (male, 200-400 g). Atorvastatin was given to rats at a daily dose of 0 mg, 2.5 mg, 5 mg or 10 mg. Patches were harvested at 1 or 4 weeks and analyzed by histology, immunohistochemistry, immunofluorescence, western blot and qPCR. Result: Animals treated with atorvastatin showed a higher number of infiltrating cells and a thicker patch neointima than the control animals. Furthermore, ADAM10 protein expression decreased (P<0.01) and ephrinB2 expression increased (P<0.01) in time- and atorvastatin dose-dependent manner. Similarly, ADAM10 mRNA expression decreased (P<0.01), while the expression of ephrinB2 mRNA and miR-140 mRNA expression increased (P<0.01; P<0.01) in a time- and dose-dependent manner. Conclusion: Atorvastatin regulates neointimal growth after pericardial patch angioplasty; atorvastatin is associated with infiltration of ephrinB2-positive cells, diminished ADAM10 expression, and increased ephrinB2 and miR-140 expression. These results suggest new mechanisms for regulating neointimal formation after vascular procedures. Clinical relevance: This study may help physicians to know more healing mechanism after pericardial patch angioplasty. Further, it may reveal some mechanism that how atorvastatin play roles in endothelium repair of the cardiovascular system.
Atorvastatin Calcium Inhibits PDGF-ββ-Induced Proliferation and Migration of VSMCs Through the G0/G1 Cell Cycle Arrest and Suppression of Activated PDGFRβ-PI3K-Akt Signaling Cascade

Abstract:Background/aims: Abnormal proliferation of vascular smooth muscle cells (VSMCs) is a hallmark of vascular lesions, such as atherosclerosis and restenosis. PDGF-ββ, an isoform of PDGF (platelet-derived growth factor), has been demonstrated to induce proliferation and migration of VSMCs. Atorvastatin calcium, a selective inhibitor of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase, has favorable protective effects on VSMCs. This study examined the effects of atorvastatin calcium on the proliferation and migration of PDGF-ββ-treated VSMCs, as well as its underlying mechanisms. Methods: MTT assays, Edu imaging, cell cycle analysis, wound healing assays, transwell migration assays, and western blot analysis were performed. Results: Atorvastatin calcium significantly inhibited cell proliferation, DNA synthesis and cell migration of PDGF-ββ-treated VSMCs. We demonstrated that atorvastatin calcium induced cell cycle arrest in the G0/G1 phase in response to PDGF-ββ stimulation and decreased the expression of G0/G1-specific regulatory proteins, including proliferating cell nuclear antigen (PCNA), CDK2, cyclin D1, cyclin E and CDK4 in PDGF-ββ-treated VSMCs. Moreover, pretreatment with atorvastatin calcium inhibited the PDGF-ββ-treated phosphorylation of PDGFRβ and Akt, whereas atorvastatin calcium did not affect the phosphorylation of PLC-γ1 or (ERK) 1/2. Conclusion: Our data suggested that atorvastatin calcium inhibited abnormal proliferation and migration of VSMCs through G0/G1 cell cycle arrest and suppression of the PDGFRβ-Akt signaling cascade.

Abstract:Stem cell transplantation represents a unique therapeutic tool in tissue engineering and regenerative medicine. However, it was shown that the post-injection survival of stem cells is poor, warranting a more comprehensive understanding of activated regenerative pathways. Numerous studies indicate that statins improve the therapeutic efficacy of stem cells in regenerative medicine. In the present study, we investigated the effect of the most widely prescribed statin, atorvastatin, on the characteristics and properties of bone-marrow-derived mesenchymal stem cells (BM-MSCs) cultured in vitro. We found that atorvastatin did not decrease the viability of BM-MSCs, nor did it change the expression of MSC cell surface markers. Atorvastatin upregulated the mRNA expression levels of VEGF-A and HGF, whereas the mRNA expression level of IGF-1 was decreased. In addition, the PI3K/AKT signaling pathway was modulated by atorvastatin as indicated by the high mRNA expression levels of PI3K and AKT. Moreover, our data revealed the upregulation of mTOR mRNA levels; however, no change was observed in the BAX and BCL-2 transcripts. We propose that atorvastatin benefits BM-MSC treatment due to its ability to upregulate angiogenesis-related genes expression and transcripts of the PI3K/AKT/mTOR pathway.

Abstract:Proliferative vitreoretinopathy (PVR) develops after an unsuccessful or complicated recovery from rhegmatogenous retinal detachment (RRD) surgery. Intraocular scar formation with the contribution of epithelial-mesenchymal transition (EMT) in RPE cells is prominent in the pathology of PVR. In the present study, the EMT process was experimentally induced in human retinal pigment epithelium (RPE; ARPE-19) cells, and the effect of atorvastatin on the process was studied. The mRNA and protein levels of mesenchymal markers actin alpha 2 (ACTA2) / alpha-smooth muscle actin (α-SMA) and fibronectin (FN), and epithelial markers occludin (OCLN) and zonula occludens-1 (ZO-1) were measured using quantitative real-time PCR (qRT-PCR) and western blot methods, respectively. In addition, α-SMA and FN were visualized using immunofluorescence staining. Cells were photographed under a phase contrast light microscope. Changes in the functionality of cells following the EMT process were studied using the IncuCyte scratch wound cell migration assay and the collagen cell invasion assay with confocal microscopy. The induction of EMT in ARPE-19 cells increased the expression of mesenchymal markers ACTA2/α-SMA and fibronectin and reduced the expression of epithelial marker OCLN both at mRNA and protein levels. The mRNA levels of ZO-1 were lower after EMT, as well. Increased levels of α-SMA and FN were confirmed by immunofluorescence staining. Atorvastatin further increased the mRNA levels of mesenchymal markers ACTA2 and FN as well as the protein levels of α-SMA and reduced the mRNA levels of epithelial markers OCLN and ZO-1 under the EMT process. EMT promoted wound closure and cell invasion into the 3D collagen matrix when compared to untreated control cells. These data present cellular changes upon the induction of the EMT process in ARPE-19 cells and the propensity of atorvastatin to complement the effect. More studies are needed to confirm the exact influence of the EMT process and atorvastatin treatment on the PVR development after RRD surgery.

The Effects of Atorvastatin on Interventional Therapy in Patients with Acute Myocardial Infarction

Abstract:BACKGROUND: The aim of this study was to analyze the changes of interleukin-6 (IL-6), C-reactive protein (CRP), blood lipids and myocardial indexes after treatment of patients with acute myocardial infarction (AMI) with intensive atorvastatin and interventional therapy, and its clinical significance. METHODS: A total of 78 patients diagnosed with AMI in our hospital from March 2016 to February 2017 were selected and divided into treatment group (N.=39) and control group (N.=39). Patients in treatment group were treated with intensive atorvastatin based on conventional therapy before and after percutaneous coronary intervention (PCI), while those in control group were treated with conventional therapy before and after PCI. The levels of serum IL-6, CRP, blood lipids (total cholesterol [TC], triglyceride [TG], high-density lipoprotein cholesterol [HDL-C] and low-density lipoprotein cholesterol [LDL-C]) and myocardial enzyme indexes (troponin T [TnT] and creatine kinase-MB [CK-MB]) at different time points were detected. The correlations among serum CRP, TC, and TnT in treatment group before treatment were detected using the linear regression analysis, and changes in serum inflammatory factors, blood lipids and myocardial enzyme indexes in treatment group before and after treatment were analyzed. RESULTS: There were no statistically significant differences in demographics and clinical data between treatment group and controls (P>0.05). There were no significant differences, either, in levels of serum IL-6 and CRP before treatment between treatment group and control group, but they were decreased after treatment, and the curative effect in treatment group was significantly superior to that in control group. The differences were statistically significant (P<0.05). There were no significant differences in serum TC, TG, HDL-C and LDL-C levels before treatment between the two study groups. TC, TG and LDL-C were significantly decreased and HDL-C was significantly increased after treatment compared to before treatment. TnT and CK-MB were significantly increased at 24 hours after treatment (P<0.05). At 1 week, TnT was still higher, whereas CK-MB has returned to normal; at 2 weeks, they were both decreased. The curative effect in the treatment group was significantly superior to that in control group. CRP (r=0.793, P<0.001) and TC (r=0.668, P<0.001) were found to be positively correlated with TnT. The levels of serum inflammatory factors and blood lipids in the treatment group before treatment and at 24 hours, 1 week, and 2 weeks showed a decreasing trend, and TnT was increased at first, and then decreased. CONCLUSIONS: The application of intensive atorvastatin for AMI patients, especially before PCI, has high safety, which can effectively reduce levels of serum inflammatory factors and blood lipids, protect myocardial cells after PCI and avoid injury.

Abstract:Abstract Introduction Percutaneous coronary intervention (PCI) is the gold standard for the management of acute coronary syndrome, and has been benefited from advances in pharmacotherapy and device innovation. Nevertheless, in-stent restenosis and stent thrombosis remain serious complications following PCI procedure with stent implantation. Proprotein convertase subtilisin kexin type 9 (PCSK9) is an enzyme known for its role in plasma cholesterol homeostasis. Efficiently reducing plasma cholesterol, PCSK9 inhibition recently became a pivotal treatment strategy for hypercholesterolemia. However, the effect of PCSK9 inhibition on stent implantation outcomes remains uninvestigated. Methods Carotid artery bare-metal stent (BMS) implantation was performed on C57Bl/6 mice, which then a parallel assessment of various inflammatory mediators was performed. To inhibit PCSK9, alirocumab was administered weekly to antiplatelet-treated mice started before stent implantation. Six weeks after the stent implantation procedure, animals were harvested and histomorphometric analysis was performed on stented arteries. In vitro cell migration and cellular senescence were also assessed on alirocumab-treated primary human aortic endothelial (EC) and vascular smooth muscle cells (VSMC). Results Compared to sham intervention, stent implantation was associated with increased expression of several inflammatory mediators, including PCSK9. Interestingly, the increase in PCSK9 level was confirmed in the stented vascular tissue, but not in plasma (Fig 1A). Following alirocumab treatment, PCSK9 inhibition results in an increased intimal hyperplasia in the stented vascular segment of alirocumab-treated animals compared to controls (Fig 1B). In vitro, alirocumab promoted migration and inhibited the onset of senescence in primary human VSMC, while blunted the migration and increased the senescence of EC. Conclusion The results of this study demonstrated that antibody-based PCSK9 inhibition worsen bare-metal stent implantation outcome in mice. This observed effect is mediated, in part, by the differential effect on VSMC and EC senescence, thus promoting in-stent intimal hyperplasia while blunting vascular healing. The herein-reported data warrant additional investigations concerning the use of PCSK9 inhibitors in patients undergoing PCI with stent implantation.
