Abstract:Purpose of review: This review explores the pharmacokinetics, benefits, and risks of proton pump inhibitors (PPIs) in cirrhotic patients, focusing on the appropriateness of their use and potential adverse effects. Recent findings: Recent studies highlight significant pharmacokinetic alterations in PPIs among cirrhotic patients, with marked increases in lansoprazole and pantoprazole exposure and relatively stable levels of esomeprazole. While effective for managing acid-related disorders and post-band ulcer rebleeding, evidence supporting PPI use for portal hypertension-related bleeding is lacking. Emerging research suggests potential adverse effects such as hepatic decompensation, spontaneous bacterial peritonitis, hepatic encephalopathy, and increased mortality, possibly linked to dysbiosis and bacterial translocation. PPI use in cirrhotic patients alters pharmacokinetics significantly, with esomeprazole potentially safer in advanced cirrhosis. The review advises caution in routine PPI use beyond acid-related conditions due to limited evidence and substantial risks. It underscores the need for careful risk-benefit assessments and exploration of alternative therapies. Future research should aim to identify safer management strategies for portal hypertension complications and to develop evidence-based guidelines for PPI use in patients with cirrhosis.
Proton Pump Inhibitor Therapy Does Not Affect Prognosis of Cirrhosis Patients with Acute Decompensation and Acute-on-Chronic Liver Failure: A Single-Center Prospective Study.

Abstract:Background: The aim of this study was to investigate the impact of proton pump inhibitor (PPI) therapy on complications and prognosis in cirrhosis patients with and without acute-on-chronic liver failure (ACLF).Materials and Methods: Cirrhosis patients with acute decompensation (AD) (n = 489) admitted in our center were enrolled in this prospective observational cohort study. According to treatment received, patients were identified as users or nonusers of PPI. Clinical and laboratory data, complications during hospitalization, and overall survival were recorded in all the patients.Results: Of the 489 patients, 299 (61.1%) patients received PPI therapy. The logistic regression analysis showed that age, albumin, history of previous hepatic encephalopathy (HE), and the chronic liver failure-sequential organ failure assessment (CLIF-SOFA) score were independent risk factors for HE in patients with decompensated cirrhosis [odds ratio (OR) = 1.07, 95% CI: 1.03-1.12, p = 0.001; OR = 1.13, 95% CI: 1.04-1.24, p = 0.006; OR = 242.52, 95% CI: 40.17-1464.11, p < 0.001; and OR = 2.89, 95% CI: 2.11-3.96, p < 0.001, respectively]. Previous severe liver injury and previous bacterial infections were independent risk factors for spontaneous bacterial peritonitis (SBP) in patients with decompensated cirrhosis (OR = 3.43, 95% CI: 1.16-10.17, p = 0.026 and OR = 6.47, 95% CI: 2.29-18.29, p < 0.001, respectively). The multivariate Cox proportional hazards regression model showed that the type and dose of the PPI used were not related to 28-day and 90-day mortality in cirrhosis patients with AD or ACLF.Conclusion: PPI use does not appear to increase mortality or the risk of HE and SBP in the hospitalized cirrhosis patients with and without ACLF.
The Use of Proton Pump Inhibitors in Patients with Liver Cirrhosis: Real Life Experience

Abstract:(1) Background: Proton pump inhibitors (PPIs) are commonly prescribed for gastric disorders. In patients with liver cirrhosis, PPI use is associated with an increased risk of spontaneous bacterial peritonitis and increased mortality rates; therefore, they should be used with caution. This study aims to evaluate the appropriateness of PPI prescriptions in hospitalized cirrhotic patients against current clinical guidelines to identify patterns of misuse and guide better prescribing practices. (2) Methods: A retrospective study was conducted on liver cirrhosis inpatients in an internal medicine department from January 2022 to May 2023. The primary measure was the proportion of PPI prescriptions aligned with clinical guidelines. Medical files were entirely reviewed by researchers to assess the appropriateness of PPI prescriptions using the current guidelines. Outcomes included the identification of common reasons for PPI prescription and the rate of inappropriate PPI use among the study population. (3) Results: The study included 189 cirrhotic patients, with PPIs prescribed to 95 (50.2%) patients during hospitalization and 75 (39.7%) patients at discharge. Among those, 47.4% of the inpatients and 34.7% at discharge had no valid indication for PPI administration. The most common reason for PPI prescription during hospital stays was gastritis, followed by antiplatelet use in high-risk patients, ulcers, and upper gastrointestinal bleeding. The most common inappropriate indication was portal hypertensive gastropathy (PHG), followed by treatment with corticosteroids and anticoagulants alone. We did not find an association between PPI administration during hospital stays and infections. Only in 4% of cases patients should have received PPIs and did not. (4) Conclusions: There is a concerning overprescription of PPIs in cirrhotic patients, often deviating from established guidelines. It subjects patients to unnecessary risks. There is an urgent need for increased awareness and adherence to clinical guidelines regarding PPI prescriptions in cirrhotic patients.
Proton pump inhibitor use and risk of hepatic encephalopathy: A multicentre study

Abstract:Background & aims: Data on the association between proton pump inhibitor (PPI) use and hepatic encephalopathy (HE) are conflicting, and data from multicentre studies are scarce. The aim of this study was to dissect the potential association between PPI use and minimal (MHE) and overt HE (OHE). Methods: Data from patients with cirrhosis recruited at seven centres across Europe and the US were analysed. MHE was defined by the psychometric hepatic encephalopathy score (PHES). PPI use was recorded on the day of testing with PHES. Patients were followed for OHE development and death/liver transplantation. Results: A total of 1,160 patients with a median MELD of 11 were included (Child-Pugh stages: A 49%/B 39%/C 11%). PPI use was noted in 58% of patients. Median follow-up time was 18.1 months, during which 230 (20%) developed an OHE episode, and 224 (19%) reached the composite endpoint of death/liver transplantation. In multivariable analyses, PPI use was neither associated with the presence of MHE at baseline nor OHE development during follow-up. These findings were consistent in subgroup analyses of patients with Child-Pugh A or B cirrhosis and after excluding patients with a history of OHE. PPI use was also not associated with a higher risk of OHE, neither in patients with an indication for treatment nor in patients without an indication. Conclusions: PPI use is not associated with a higher risk of HE in patients with cirrhosis. Based on these findings, at present, a prescription should not be prohibited in case of a generally accepted indication. Impact and implications: Data on the association between proton pump inhibitor (PPI) use and hepatic encephalopathy (HE) are conflicting. In this study, PPI use was not associated with a higher risk of minimal HE at baseline or overt HE during follow-up in patients with cirrhosis. Based on these findings, prescription of a PPI for a generally accepted indication should not be prohibited in patients with cirrhosis.

Abstract:The use of high‐dose PPI was independently associated with increased risks of mortality and cirrhotic complications in patients with advanced liver cirrhosis who experienced hepatic encephalopathy. Abstract:Proton pump inhibitors (PPIs) are very effective drugs in treatment and prevention of acid-related disorders. They are widely used in gastroesophageal reflux disease, gastroduodenal ulcers, Zollinger-Ellison syndrome or prevention and treatment of NSAIDs induced gastric lesions. They are also used in many other indications, although not always is their use appropriate. PPIs are generally well tolerated and overall benefits outweigh potential harm in most of the patients. However, their long-term use is not without a risk of adverse effects and drug-drug interactions and contributes to medical expenses and polypharmacy. As PPIs belong to most prescribed drugs, we should carefully consider the length of therapy and appropriateness of their use and regularly reevaluate the need for chronic therapy.

Abstract:Proton pump inhibitors (PPIs) are among the most commonly prescribed medications, usually with a short-term indication and a recommended duration of less than 8 weeks ( 1–3 ). A major short-term indication is stress ulcer prophylaxis (SUP) ( 4 ), especially in ICUs, where certain critically ill patients are at increased risk of gastrointestinal bleeding ( 5–7 ). Although there is controversy about which patients in the ICU benefit from SUP, the latter is currently initiated in up to 90% of all critically ill patients ( 8–12 ). However, regardless of the debate on which ICU patients should receive PPI therapy, its timely cessation once the indication has expired is of utmost importance since PPI therapies not only result in an economic burden but may also evoke serious side effects ( 3 , 13 , 14 ). Greater risk of clostridium difficile infection ( 15–18 ), gastritis ( 17 ), pneumonia ( 15 ), cardiovascular events ( 19 , 20 ), chronic renal failure ( 21 , 22 ), greater frequency of various neoplasms ( 23–27 ), possibly malabsorption disorders evoking vitamin B12 deficiency ( 28 , 29 ), hypocalcemia ( 30 ), and hypomagnesemia ( 30 ) are specific predescribed side effects of a long-term PPI therapy. Even an increased risk of mortality has been observed in retrospective studies ( 31–34 ).

Proton pump inhibitors use and risk of developing spontaneous bacterial peritonitis in cirrhotic patients: A systematic review and meta-analysis

Abstract:Background: Spontaneous bacterial peritonitis (SBP) is one of the most common infectious diseases in patients with cirrhosis and is associated with serious prognosis. A prevailing dogma posits that SBP is exacerbated by the frequent use of proton pump inhibitors (PPIs). Aims: To re-assess the association between PPIs use and SBP incidence with larger and better-quality data. Method: The studies were identified by searching Proquest, Medline, and Embase for English language articles published between January 2008 and March 2020 using the following keywords alone or in combination: anti-ulcer agent, antacid, proton pump inhibitor, proton pumps, PPI, omeprazole, rabeprazole, lansoprazole, pantoprazole, esomeprazole, peritonitis, spontaneous bacterial peritonitis, SBP, ascites, cirrhosis, ascitic and cirrhotic. Three authors critically reviewed all of the studies retrieved and selected those judged to be the most relevant. Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement was followed. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated. Sub-group analyses were done to decrease the heterogeneity. Results: A total of twenty-three studies: seven case-control, and sixteen cohorts, involving 10,386 patients were analyzed. The overall results showed a statistically significant association between SBP and PPIs use (pooled odds ratio (OR): 1.80, 95% CI of 1.41 to 2.31). Substantial heterogeneity was observed. On subgroup analysis involving cohort studies, the association was weaker (OR: 1.55 with 95% CI of 1.16 to 2.06 p < 0.00001) but still statistically significant and with high heterogeneity (Chi2p = 57.68; I2 = 74%). For case-control studies, the OR was 2.62 with a 95% CI of 1.94 to 3.54. The funnel plot was asymmetric and Egger's test confirmed asymmetry suggesting publication bias (intercept = - 0.05, SE = 0.27, P = 0.850 two-tailed). Conclusion: This meta-analysis sheds light on the conflicting results raised by previous studies regarding the association of SBP with PPIs use. Our meta-analysis showed that there is a weak association, although statistically significant, between SBP and PPIs use. However, the magnitude of the possible association diminished when analysis focused on higher quality data that were more robust. Thus, this updated meta-analysis suggests judicious use of PPIs among cirrhotic patients with ascites.
Long-Term Intake of Proton-Pump Inhibitors Could Be Associated with an Increased Incidence of Liver Cancer in Women

Abstract:Background: Proton pump inhibitors (PPIs) are among the most commonly prescribed drugs in gastroenterology. Although PPIs are mostly well tolerated, long-term PPI intake has been linked with diabetes mellitus, osteoporosis and infectious disease. In the present study, we evaluated a potential association between PPI intake and a subsequent diagnosis of liver cancer in a large real-world cohort of outpatients in Germany. Methods: A total of 1766 patients with liver cancer, as well as 8830 propensity-score-matched controls, were identified from the Disease Analyzer database (IQVIA). The outcome of the study was the association between PPI use and a subsequent diagnosis of liver cancer, which was evaluated using multivariable logistic regression analyses. Results: Overall, 42.9% of the liver cancer patients and 39.0% of the controls received at least one PPI prescription before the index date. PPI prescriptions at any time before the index date were associated with an increased risk of subsequent liver cancer (OR: 1.18; 95% CI: 1.06–1.31). The positive association was observed in all age groups, as well as in women and men, but only in women (OR: 1.30; 95% 1.09–1.55) did it reach the predefined level of significance (p < 0.01). When considering the duration of PPI therapy, only PPI therapy for at least two years was significantly associated with an increased risk of liver cancer (OR: 1.28; 95% 1.09–1.50). In an analysis stratified by age and sex, this association was strongest in the age group < 60 years (OR: 1.99; 95% 1.21–3.26). Conclusions: Our data suggest that long-term PPI intake in women as well as in patients < 60 years might be associated with an increased risk of liver cancer. These findings support current efforts to reduce the inappropriate use of PPIs in routine clinical practice and to link PPI prescribing to a clear medical indication.

Abstract:Background: Spontaneous bacterial peritonitis (SBP) is one of the most serious complications of cirrhotic patients. A few studies have shown that use of proton pump inhibitors (PPI) may increase the risk of occurrence of SBP in cirrhotic patients. However, controversial results have been obtained in the last decades. Therefore, the purpose of our analysis was to investigate the relationship between the risk of development of Spontaneous bacterial peritonitis (SBP) and the administration of proton pump inhibitors (PPI) in cirrhotic patients. Methods: Observational studies, about the relationship between PPI therapy and SBP in cirrhotic patients, were searched in PubMed, Medline, Embase, Web of Knowledge and other databases, and retrieval time ended in Jun 2015. Data from the identified studies were evaluated by odds ratios (ORs) and 95% confidence interval (CI) using traditional meta-analytic techniques. Results: Eleven studies (n = 4567 patients) met the inclusion criteria. The results showed that the risk of development of SBP significantly increased (OR = 2.04, 95% CI 1.56-2.69, P < 0.00001) in cirrhotic patients with PPI. The fixed effect model was used because of low heterogeneity, P < 0.1 and I-2 = 44%. Subgroup analysis was performed in different study regions. The risk of developing SBP in cirrhotic patients with PPI treatment was significantly increased (OR = 2.22, 95% CI 1.72-2.87, P < 0.0001; OR = 1.70, 95% CI 0.70-4.13, P = 0.24; OR = 1.99, 95% CI 1.19-3.32, P = 0.009) in Asian, European, and American patients. Moderate heterogeneity existed in Euro-American patients. Conclusion: There was a significant correlation between the development of SBP and the use of PPI in cirrhotic patients. Therefore, clinicians should be aware of the indications of PPI, and use PPI cautiously in these patients. Further research need to clarify the correlation between the type, dose and course of treatment of PPI and the development of SBP in cirrhotic patients.
A pragmatic assessment of proton pump inhibitors vs. histamine type 2 receptor antagonists on clinically important gastrointestinal bleeding and mortality when used for stress ulcer prophylaxis in the ICU

Abstract:ObjectiveApproximately 1-5% of critically ill patients experience clinically important gastrointestinal bleeding (CIGB). This study assessed the effectiveness and safety of proton-pump inhibitors (PPIs) compared to histamine type 2 receptor antagonists (H2RAs) for prevention of CIGB in mechanically ventilated patients. DesignRetrospective, single-center, pharmacoepidemiolgic study. SettingSix intensive care units (ICUs). PatientsCritically ill adults admitted between 9/1/14 and 9/1/19 who received PPIs or H2RAs within 24 hours of intubation and for ≥ 48 hours. Measurements and Main ResultsPrimary outcomes were CIGB occurring 48 hours after ICU admission and hospital mortality. Secondary outcomes were pneumonia, Clostridioides difficile infection (CDI), acute kidney injury, myocardial infarction / ischemia, thrombocytopenia, and delirium. Outcomes were defined using International Classification of Diseases, Ninth Revision (ICD-9) and Tenth Revision (ICD-10)-codes with manual cross reference for a hemoglobin drop, transfusion, or hemodynamic compromise to further define CIGB. Of 3,873 patients, 2,061 (53.2%%) received PPIs. CIGB was rare but higher in the PPI group (0.34% vs. 0%, RR=1, 95% CI, 1-1; p=0.013); however substantial group differences existed possibly predisposing the PPI group to CIGB. Hospital mortality was higher in the PPI group (42.1% vs. 29.1%, RR=1.23, 95% CI, 1.17-1.29; p<0.0001). PPIs remained an independent risk factor for mortality after multivariate adjustment (RR=1.61, 95% CI, 1.39-1.88; p<0.0001). Rates of secondary outcomes were similar between groups except thrombocytopenia (4.3% vs. 2.2%, RR=1.02, 95% CI, 1.01-1.03; p=0.0003) and delirium (83.7% vs. 78.1%, RR=1.34, 95% CI, 1.18-1.53; p<0.0001) that were higher in the PPI group. ConclusionPPIs were associated with CIGB; however, the overall rate of CIGB was low. Compared to H2RAs, PPIs were associated with hospital mortality. Further identification of appropriate selection criteria for ulcer prophylaxis and comparisons of pharmacologic prevention strategies are warranted.

Abstract:Background & aims: Sustained virological response (SVR) by direct-acting antivirals (DAAs) may reverse the hypercoagulable state of HCV cirrhosis and the portal vein thrombosis (PVT) risk. We evaluated the incidence and predictive factors of de novo, non-tumoral PVT in patients with cirrhosis after HCV eradication. Methods: Patients with HCV-related cirrhosis, consecutively enrolled in the multi-center ongoing PITER cohort, who achieved the SVR using DAAs, were prospectively evaluated. Kaplan-Meier and competing risk regression analyses were performed. Results: During a median time of 38.3 months (IQR: 25.1-48.7 months) after the end of treatment (EOT), among 1609 SVR patients, 32 (2.0%) developed de novo PVT. A platelet count ≤120,000/μL, albumin levels ≤3.5 mg/dL, bilirubin >1.1 mg/dL, a previous liver decompensation, ALBI, Baveno, FIB-4, and RESIST scores were significantly different (p < 0.001), among patients who developed PVT versus those who did not. Considering death and liver transplantation as competing risk events, esophageal varices (subHR: 10.40; CI 95% 4.33-24.99) and pre-treatment ALBI grade ≥2 (subHR: 4.32; CI 95% 1.36-13.74) were independent predictors of PVT. After HCV eradication, a significant variation in PLT count, albumin, and bilirubin (p < 0.001) versus pre-treatment values was observed in patients who did not develop PVT, whereas no significant differences were observed in those who developed PVT (p > 0.05). After the EOT, esophageal varices and ALBI grade ≥2, remained associated with de novo PVT (subHR: 9.32; CI 95% 3.16-27.53 and subHR: 5.50; CI 95% 1.67-18.13, respectively). Conclusions: In patients with HCV-related cirrhosis, a more advanced liver disease and significant portal hypertension are independently associated with the de novo PVT risk after SVR.

Pleiotropic effect of the proton pump inhibitor esomeprazole leading to suppression of lung inflammation and fibrosis

Abstract:Background: The beneficial outcome associated with the use of proton pump inhibitors (PPIs) in idiopathic pulmonary fibrosis (IPF) has been reported in retrospective studies. To date, no prospective study has been conducted to confirm these outcomes. In addition, the potential mechanism by which PPIs improve measures of lung function and/or transplant-free survival in IPF has not been elucidated. Methods: Here, we used biochemical, cell biological and preclinical studies to evaluate regulation of markers associated with inflammation and fibrosis. In our in vitro studies, we exposed primary lung fibroblasts, epithelial and endothelial cells to ionizing radiation or bleomycin; stimuli typically used to induce inflammation and fibrosis. In addition, we cultured lung fibroblasts from IPF patients and studied the effect of esomeprazole on collagen release. Our preclinical study tested efficacy of esomeprazole in a rat model of bleomycin-induced lung injury. Furthermore, we performed retrospective analysis of interstitial lung disease (ILD) databases to examine the effect of PPIs on transplant-free survival. Results: The cell culture studies revealed that esomeprazole controls inflammation by suppressing the expression of pro-inflammatory molecules including vascular cell adhesion molecule-1, inducible nitric oxide synthase, tumor necrosis factor-alpha (TNF-α) and interleukins (IL-1β and IL-6). The antioxidant effect is associated with strong induction of the stress-inducible cytoprotective protein heme oxygenase-1 (HO1) and the antifibrotic effect is associated with potent inhibition of fibroblast proliferation as well as downregulation of profibrotic proteins including receptors for transforming growth factor β (TGFβ), fibronectin and matrix metalloproteinases (MMPs). Furthermore, esomeprazole showed robust effect in mitigating the inflammatory and fibrotic responses in a murine model of acute lung injury. Finally, retrospective analysis of two ILD databases was performed to assess the effect of PPIs on transplant-free survival in IPF patients. Intriguingly, this data demonstrated that IPF patients on PPIs had prolonged survival over controls (median survival of 3.4 vs 2 years). Conclusions: Overall, these data indicate the possibility that PPIs may have protective function in IPF by directly modulating the disease process and suggest that they may have other clinical utility in the treatment of extra-intestinal diseases characterized by inflammatory and/or fibrotic phases.
Proton Pump Inhibition for Secondary Hemochromatosis in Hereditary Anemia, a Phase III Placebo Controlled Randomized Cross-over Trial in Progress

Abstract:Background: Iron overload is an emerging and underestimated problem in management of patients with hereditary anemia characterized by chronic hemolysis or ineffective erythropoiesis in the absence of regular red cell transfusions. Liver iron overload was present in 65% of the patients with hereditary hemolytic anemia who were never transfused. (Van Straaten et al., Br J Haematol. 2018) Iron-loading results from inadequately high intestinal iron absorption in response to low hepcidin and ultimately leads to iron overload in liver and other organs. Iron chelation treatment is recommended, however drug-related toxicity is considerable, and treatment with iron chelators is expensive. Alternative treatment options for anemic patients not tolerating iron chelating agents are not available. For adequate iron absorption, a low luminal pH is required to solubilize dietary iron salts, and to promote the reduction of dietary Fe(III) to Fe(II) by ferriredutases as only Fe(II) can be absorbed. Proton pump inhibitors (PPI) block gastric acid secretion and thereby impede iron absorption. No studies are available that quantify effects of PPIs on dietary iron absorption in human. However, PPIs have shown to minimize phlebotomy requirements in patients with hereditary hemochromatosis. (Vanclooster et al., Gastroenterology. 2017) Here we report the design of the currently ongoing PPI SHINE AGAIN study that evaluates the efficacy and safety of PPI treatment in patients with non-transfusion dependent hereditary anemias. Methods: The PPI SHINE AGAIN is a phase 3, multicentre, randomized, placebo-controlled, cross-over clinical trial (Netherlands Trial Register, identifier [NL6659]). Transfusion independent adults with a form of hemolytic or dyserythropoietic anemia and mild to moderate iron overload are randomized in a 1:1 ratio to start with either esomeprazole 40 mg (administered orally BID) or placebo. Mild to moderate iron overload is defined as baseline liver iron content (LIC) of 3-15 mg Fe/g dry weight as measured by T2* MRI (MRQuantif Software Université de Rennes, https://imagemed.univ-rennes1.fr/en/mrquantif/quantif.php) without or on stable iron chelation therapy with no expected dose adjustments over the next 2 years. Additional criteria included baseline Hb ≤ 11.3 g/dL, expected to receive less than 4 red cell transfusions in the following 12 months, and no phlebotomies. The trial consists of two treatment periods of 12 months each (Figure), directly starting after baseline MRI. The primary study end-point is effectiveness of PPI treatment defined as difference in delta LIC after one-year treatment with esomeprazole compared to one-year treatment with placebo. Key secondary endpoints include tolerability, quality of life (assessed by EQ5D-5L questionnaire) and cost-effectiveness (assessed by iMTA Productivity Cost Questionnaire and iMTA Medical Consumption Questionnaire). Inclusion in the PPI SHINE AGAIN was completed in April 2019. Thirty patients are enrolled (non-transfusion dependent β-thalassemia n=10; pyruvate kinase deficiency n=8; congenital dyserythropoietic anemia n=3; sideroblastic anemia n=3; SCD n=2; HbH disease n=2; hereditary elliptocytosis n=1; G6PD deficiency n=1) with a median age of 44 years (IQR 28, 54). Results are expected Q3 2020. The trial is funded by ZonMW, The Netherlands Organization for Health and Research Development, and by the Innovatiefonds Zorgverzekeraars. Abstract:The introduction of proton pump inhibitors (PPIs) has been a cornerstone in the treatment of acid-related disorders, such as gastroesophageal reflux and peptic ulcer, and their use has increased rapidly during the last decades. Being highly lipophilic drugs, they may potentially affect several pathophysiological pathways involved in cardiovascular and kidney morbidity, immune response and infections, absorption of selected nutrients, bone metabolism and cognitive function. Clinical epidemiology evidence coming from well-designed analyses of observational data consistently reported that long-term use of PPIs may increase the risk of cardiovascular events among patients treated with thienopyridines, tubular-interstitial nephritis and chronic kidney disease, hypomagnesemia, and fractures. Conversely, currently available evidence about the impact of PPIs on cardiovascular risk among patients not treated with thienopyridines, infections, nutritional disorders, cognitive impairment and dementia is limited by confounding. Given that randomized controlled trials investigating these issues are unlikely to be realized, the application of modern pharmacoepidemiology principles is expected to mitigate limitations of observational studies while addressing these relevant knowledge gaps. Meanwhile, physicians should be aware of potential issues related to long-term use of PPIs and weigh benefits of PPI therapy for appropriate indications along with the likelihood of the potential risks. A deprescription trial should be considered for all PPI users who do not have definite indications for long-term therapy.

Abstract:Purpose of review: This review summarizes the recent literature on the potential side-effects of proton pump inhibitors (PPIs) and known interactions with the metabolism/absorption of other drugs. Recent findings: Data confirm that PPIs are a very well tolerated drug class. Their high safety, efficacy and wide distribution lead to overuse, inappropriate dosage or excessive duration of treatment. Despite the absorption of micronutrients or other plausible effects on the development of bacterial infections linked to PPI-induced hypochlorhydria, it is difficult to demonstrate an association between PPI and specific symptoms. A possible negative effect of PPIs on bone integrity appears weak, but hypomagnesemia is likely a PPI drug class effect. A higher risk of Clostridium difficile infection and other infectious diseases such as small intestinal bacterial overgrowth and spontaneous bacterial peritonitis remain controversial in PPI users. However, the careful use of PPIs in cirrhotic or otherwise fragile patients is mandatory. Short-term or long-term PPI use may trigger microscopic colitis, and the management of this condition may include PPI withdrawal. The effect of PPIs on stimulating exocrine or endocrine gastric cell proliferation is poorly understood. A diagnostic delay or masking of diseases such as gastrinoma is difficult to evaluate. Summary: Short-term standard dose PPI treatment is low risk. Long-term PPI use may complicate health conditions by various mechanisms linked to PPIs and/or to hypochlorhydria.
