Abstract:Glypican-3 (GPC3) is a promising new marker for hepatocellular carcinoma, but the reported values for serum GPC3 differ markedly between currently available kits. Here we isolated Affimer non-antibody binding proteins against GPC3 by phage display and developed a new sandwich chemiluminescence immunoassay (CLIA) combining an Affimer with a monoclonal antibody (Affimer-MAb CLIA). The proposed CLIA assay demonstrated a wide linear range 0.03–600 ng/mL) with a good linear correlation coefficient (0.9999), a high detection limitation (0.03 ng/mL) and specificity (0–0.002%) for detection of GPC3. The accuracy, hook effect and stability were demonstrated to be satisfactory. The mean level of GPC3 in serum was higher (>8.5 fold, P < 0.001) in hepatocellular carcinoma patients compared to healthy and other liver disease individuals. A poor correlation (correlation coefficients ranged from −0.286 to 0.478) was observed through pairwise comparison within different kits. However, only this newly developed CLIA test showed high specificity and correlated with the “gold standard” GPC3-immunohistochemistry. This study indicates that Affimer-MAb CLIA can be used to generate a sensitive immunodiagnostic kit, which offers the potential for a highly specific clinically-relevant detection system.
Quantitative, single-step dual measurement of hemoglobin A1c and total hemoglobin in human whole blood using a gold sandwich immunochromatographic assay for personalized medicine

Abstract:We describe a gold nanoparticle-based sandwich immunoassay for the dual detection and measurement of hemoglobin A1c (HbA1c) and total hemoglobin in the whole blood (without pretreatment) in a single step for personalized medicine. The optimized antibody-functionalized gold nanoparticles immunoreact simultaneously with HbA1c and total hemoglobin to form a sandwich at distinctive test lines to transduce visible signals. The applicability of this method as a personal management tool was demonstrated by establishing a calibration curve to relate % HbA1c, a useful value for type 2 diabetes management, to the signal ratio of captured HbA1c to all other forms of hemoglobin. The platform showed excellent selectivity (100%) toward HbA1c at distinctive test lines when challenged with HbA0, glycated HbA0 and HbA2. The reproducibility of the measurement was good (6.02%) owing to the dual measurement of HbA1c and total hemoglobin. A blood sample stability test revealed that the quantitative measurement of % HbA1c was consistent and no false-positive results were detected. Also, this method distinguished the blood sample with elevated HbF from the normal samples and the variants. The findings of this study highlight the potential of a lateral flow immunosensor as a simple, inexpensive, consistent, and convenient strategy for the dual measurement of HbA1c and total Hb to provide useful % HbA1c values for better on-site diabetes care.

Abstract:The concentration ratio of glycosylated hemoglobin A1c (HbA1c) to total hemoglobin (Hb) has long been used to accurately determine stagewise diabetes because this parameter represents a reliable and accurate biomarker of mean 90-day blood glucose values. In this paper, we report a time-resolved chemiluminescence assay that can detect both Hb and HbA1c. For the determination of Hb, the interaction of heme in Hb with H2O2 in NaOH solution was performed to generate a chemiluminescence peak. HbA1c was detected using a sandwich immunoassay based on an acridine ester-labeling method using the same Hb chemiluminescence trigger system. The results showed that the repeatability %CV of the proposed method for multiple detections of HbA1c and Hb ranged from 1.22 to 2.21%, with a median value of 1.73%, while the within-site reproducibility %CV ranged from 2.13 to 3.27%, with a median value of 2.81%. Compared with the conventional HPLC method (BIO-RAD D10 system), the correlation coefficient was 0.9959. In conclusion, a time-resolved multiple chemiluminescence immunoassay biosensor for HbA1c/Hb detection was established, and the method has excellent reproducibility and accuracy, thus demonstrating great potential for clinical application.

Abstract:Glycated hemoglobin (HbA1c) levels are an important index for the diagnosis and long-term control of diabetes. This study is the first to use a direct and label-free photoelectric biosensor to determine HbA1c using bacteriorhodopsin-embedded purple membranes (PM) as a transducer. A biotinylated PM (b-PM) coated electrode that is layered with protein A-oriented antibodies against hemoglobin (Hb) readily captures non-glycated Hb (HbA0) and generates less photocurrent. The spectra of bacteriorhodopsin and Hb overlap so the photocurrent is reduced because of the partial absorption of the incident light by the captured Hb molecules. Two HbA0 and HbA1c aptasensors that are prepared by conjugating specific aptamers on b-PM coated electrodes single-step detect HbA0 and HbA1c in 15 min, without cross reactivity, with detection limits of ≤0.1 μg/mL and a dynamic range of 0.1–100 μg/mL. Both aptasensors exhibit high selectivity and long-term stability. For the clinical samples, HbA0 concentrations and HbA1c levels that are measured with aptasensors correlate well with total Hb concentrations and the HbA1c levels that are determined using standard methods (correlation gradient = 0.915 ± 0.004 and 0.981 ± 0.001, respectively). The use of these aptasensors for diabetes care is demonstrated.

A sensitive and quantitative immunochromatographic assay for HBsAg based on novel red silica nanoparticles

Abstract:An immunochromatographic assay (ICA) based on red silica nanoparticles (red-SiNPs) was developed for quantitative detection of hepatitis B surface antigen (HBsAg) in serum. Red SiNPs have unique optical properties and remarkable stability compared to red latex microspheres. Red-SiNPs functionalized with anti-HBsAg mAb1 were employed as detection immuno-probes. The sandwich-type immunoreaction structures were formed on ICA strips, and the accumulation of red SiNPs-mAb1 on the test line produced characteristic red color bands, enabling visual detection of HBsAg without instrumentation. Quantitative detection was performed by researching the relationship between the HBsAg concentration and the corresponding peak area of the gray value of the test line with a camera and image analysis software Image J. Under optimal conditions, the visual limit of detection (vLOD) of the strip for the qualitative detection of HBsAg is 1 ng mL(-1), which is lower than that of the routinely used red latex microsphere-based ICA. In the quantitative detection, the peak area of the gray value of the test line and the HBsAg concentration show a good linearity when the HBsAg concentration is in the range of 5-500 ng mL(-1). The calibration equation was y = 370.6118 + 9.6673x (R2 = 0.9962) and the limit of detection (LOD) of the method was 0.97 ng mL(-1) (S/N = 3). Our proposed method provides promising and versatile opportunities for the rapid and quantitative screening of other proteins at relatively sensitive levels by combining a colored label with ICA.

Abstract:As a good biomarker to reflect the average level of blood glucose, glycated hemoglobin (HbA1c) is mainly used for long-term glycemic monitoring and risk assessment of complications in diabetic patients. Previous analysis methods for HbA1c usually require complex pretreatment processes and large-scale biochemical analyzers, which makes it difficult to realize the point-of-care testing (POCT) of HbA1c. In this work, we have proposed a three-electrode dry chemistry-based electrochemiluminescence (ECL) biosensor and its self-contained automatic ECL analyzer. In this enzymatic biosensor, fructosyl amino-caid oxidase (FAOD) reacts with the hydrolysis product of HbA1c, and the produced hydrogen peroxide further reacts with luminol under the appropriate driving voltage, generating photons to realize the quantitative detection of HbA1c. Under optimized conditions, the biosensors have a good linear response to different concentrations of fructosyl valine (FV) ranging from 0.05 to 2 mM, with a limit of detection of 2 μM. The within-batch variation is less than 15%, and the biosensors still have 78% of the initial response after the accelerated aging test of 36 h at 37 °C. Furthermore, the recoveries for different concentrations of samples in whole blood were within 92.3–99.7%. These results illustrate that the proposed method has the potential for use in POCT of HbA1c.

Abstract:Assessment tests for glycemic control of patients with diabetes mellitus commonly use blood glucose and glycated hemoglobin A1C (HbA 1C ) levels for estimating acute and long-term glycemia, respectively. Diabetes diet, as well as treatment, are adapted accordingly and also the risk of diabetic complications can be estimated. Glycated human serum albumin (GHSA) has been proposed as alternative to HbA1C since it possesses a four times shorter lifespan and its blood concentration is not affected by hemic diseases. Here, an electrochemical aptasensor is proposed for the simultaneous detection of human serum albumin (HSA) and GHSA utilizing flexible multielectrode arrays. The polymer chip carries two individual sets of gold electrodes, which are separately modified by the corresponding aptamers. The dual-target aptasensors possessed detection limits as low as 13 nM and 25 nM for HSA and GHSA, respectively, together with detection ranges lasting from 40 nM to 10 μM. Moreover, our aptasensor quantified selectively its target in diluted full-blood samples. This first PoC, low-cost plastic aptasensor facilitates the simultaneous and quantitative determination of HSA and GHSA ratios by analyzing the current signals from several redundant electrodes in the same sample, which permits a reliable and affordable estimation of long-term glycemia at point-of-care or home.

Abstract:Glycated hemoglobin is an important indicator for diagnosing diabetes and monitoring blood sugar control levels. This study establishes an analytical method for detecting glycated hemoglobin based on the immune enhancement technology of physically adsorbed nanospheres. The optimal reaction conditions were screened out: nanosphere particle size of 125 nm, nanosphere concentration of 2%, antibody concentration of 80 mg/L, buffer system of HEPES with a pH value of 7.5, Tween-20 concentration of 0.12%, sodium chloride concentration of 40 g/L, and glycerol concentration of 7%. Compared with high-performance liquid chromatography and immunochromatography, the evaluation indicators are as follows: analytical sensitivity of 0.08%, intra-batch CVs of 1.19%, 0.85% and 0.66% for detection concentrations of 3.18%, 6.36% and 11.13%, respectively, inter-batch CVs of 1.99%, 1.15% and 0.84%, respectively, a linear range of2.0%-16%, r: 0.9994, and a relative deviation within ±3.5% for external quality assessment. The established method for measuring glycated hemoglobin using nanosphere immune enhancement technology has high consistency with high-performance liquid chromatography in terms of precision and linear range, and is superior to immunochromatography. This method is suitable for fully automatic biochemical analyzers, with advantages such as high automation, fast detection speed, and low cost, which can meet the requirements of clinical testing.

Abstract:In this research, we developed a rapid and easy-to-operate point-of-care testing (POCT) strip based on fluorescent affinity immunochromatography to quantitatively determine HbA1c concentrations in whole blood. This assay, based on a sandwich method performed on test strips, effectively utilized the principle of an affinity chromatography column, which was commonly used in the detection of HbA1c, and the technology of traditional fluorescence immunochromatographic test strips (FICTS) were combined. In our test strips, the test line of traditional FICTS was transformed into the region of affinity chromatography, while improving the linearity and reducing the interference of the precursor of HbA1c and hemoglobin variants. The test strips could quantitatively detect HbA1c over a wide range (3–13.8%) with excellent linearity (R2 > 0.99), and the assay accuracy was demonstrated by comparing with high-performance liquid chromatography (HPLC) (R2 > 0.95). The simple, rapid, effective and quantitative strips will provide a novel method for the detection of HbA1c in clinical diagnosis.

Abstract:Background The determination of glycosylated hemoglobin (HbA 1c ) is crucial for diabetes diagnosis and can provide more substantial results than the simple measurement of glycemia. While there is a lack of simple methods for the determination of HbA 1c using a point-of-care test (POCT) compared to glycemia measurement. In particular, high-performance liquid chromatography (HPLC) is considered the current gold standard for determining HbA 1c levels. However, commercial HPLC systems usually have some sort of disadvantages such as bulky size, high-cost and need for qualified operators. Therefore, there is an urgent demand to develop a portable, and fast HbA1c detection system consuming fewer reagents. Results We present a novel microchip that integrates a micromixer , passive injector, packed column and detection cell. The integrated microchip, in which all the microstructures were formed in the CNC machining center through micro-milling, is small in size (30 mm × 70 mm × 10 mm), and can withstand 1600 psi of liquid pressure. The integrated design is beneficial to reduce the band broadening caused by dead volume. Based on the microchip, a microchip liquid chromatography (LC) system was built and applied to the analysis of HbA 1c . The separation conditions of HbA 1c in blood calibrator samples were optimized using the microchip LC system. Samples containing four levels of HbA 1c were completely separated within 2 min in optimal gradient conditions, with an inaccuracy (<3.2 %), a coefficient of variation (c.v. < 2.1 %) and a correlation coefficient (R 2 = 0.993), indicating excellent separation efficiency and reproducibility. Significance The POCT of HbA 1c is critical for diabetes diagnosis. The microchip chromatography system was developed for HbA 1c determination, which contains an integrated microchip and works under a gradient elution . It surpasses existing chip technology in terms of separation performance and detection speed, providing a competitive advantage for POCT of HbA 1c . It is considered one important step for realizing efficient portable systems for timely and accurate diabetes diagnosis.

Abstract:Hemoglobin A1c (HbA1c) has been proven to be a key biomarker for diabetes screening, and glutathiolation of HbA1c (viz., GSS-HbA1c) has been identified. However, the impact of GSS-HbA1c on the measurement of HbA1c for diabetes screening has not been quantitatively assessed yet. To address the issue, the micropreparative capillary isoelectric focusing (cIEF) developed in our previous work was used for the high resolution separation and purification of hemoglobin (Hb) species. The main fractions of HbA0, HbA3 and HbA1c extracted from the developed cIEF were identified by validated Mono S method. The proposed GSS-HbA1c fractions in the cIEF were pooled and identified by electrospray ionization mass spectrometry (ESI-MS). The HbA1c enzyme-linked immunosorbent assay (ELISA) kit was employed for further quantitative analysis of GSS-HbA1c. A total of 34 blood samples with HbA1c levels from 4.2% to 13.4% were assessed via the above comprehensive strategy of IEF-HPLC-MS-ELISA. It was demonstrated that the HbA1c levels detected by cation exchange LC were considerably influenced by the glutathiolation of Hb and the range of detected GSS-HbA1c values was between 0.23% and 0.74%. The results and developed cIEF methods have considerable significances for investigation of diabetes and clinical diagnosis.

Abstract:Over 500 million people worldwide are affected by diabetes mellitus , a chronic disease that leads to high blood glucose levels and causes severe side effects. The predominant biological marker for diagnosis of diabetes is glycated haemoglobin (GHb). In human blood the predominant reducing sugar, glucose, irreversibly conjugates onto accessible amine groups within Hb. Most methods for diagnosis and monitoring of diabetes selectively detect N-terminal glycation at Val-1 on the β-globin chain, but not glycation at other sites. Detection of other glycated epitopes of GHb has the potential to provide new information on the extent, duration and timing of elevated glucose, facilitating personalised diagnosis and intelligent diabetic control. In this work, a new anti-GHb Fab antibody (Fab-1) specific for haemoglobin A1c (HbA1c) with nanomolar affinity was discovered via epitope-directed immunisation and phage display. A single chain variable fragment (scFv) antibody derived from Fab-1 retained affinity and specificity for HbA1c, and affinity was enhanced tenfold upon addition of an enhanced green fluorescent protein tag. Both the scFv and Fab-1 recognised an epitope within HbA1c that was distinct from β-Val-1, and our data suggest that this epitope may include glycation at Lys-66 in the β-globin chain. To our knowledge, this is the first report of an scFv/Fab anti-glycated epitope antibody that recognises a non-A1c epitope in GHb, and confirms that fructosamine attached to different, discrete glycation sites within the same protein can be resolved from one another by immunoassay.

Abstract:A novel microfluidic immunoassay system for specific detection of hemoglobin A1c (HbA1c) was developed based on a three-component shell/shell/core structured magnetic nanocomposite Auichitosan/Fe3O4, which was synthesized with easy handling feature of Fe3O4 by magnet, high affinity for gold nanoparticles of chitosan and good immobilization ability for anti-human hemoglobin-A1c antibody (HbA1c mAb) of assembled colloidal gold nanoparticles. The resulting HbA1c mAb/Au/chitosan/Fe3O4 magnetic nanoparticles were then introduced into microfluidic devices coupled with a gold nanoband microelectrode as electrochemical detector. After that, three-step rapid immuno-reactions were carried out in the sequence of HbA1c, anti-human hemoglobin antibodies (Hb mAb) and the secondary alkaline phosphatase (AP)-conjugated antibody within 20 min. The current response of 1-naphtol obtained from the reaction between the secondary AP-conjugated antibody and 1-naphthyl phosphate (1-NP) increased proportionally to the HbA1c concentration. Under optimized electrophoresis and detection conditions, HbA1c responded linearly in the concentration of 0.05-1.5 mu g mL(-1), with the detection limit of 0.025 mu g mL(-1). This system was successfully employed for detection of HbA1c in blood with good accuracy and renewable ability. The proposed method proved its potential use in clinical immunoassay of HbA1c. Detecting glycated hemoglobin in human blood samples using a transistor-based nanoelectronic aptasensor

Abstract:Glycated hemoglobin A1c (HbA1c) is a measure of long-term glycemic control in chronic diabetes-related complications. In this study, the glycated N-terminus of the hemoglobin β-chain, i.e., the glycated peptide (GP) of Fru-Val-His-Leu-Thr-Pro-Glu-COOH, was used as a template to isolate a novel aptamer (denoted by AptGP) via the systematic evolution of ligands by the exponential enrichment (SELEX) technique. Molecular docking/simulation studies revealed that both H-bonding and nonbonded interactions dominate the GP-AptGP association. While the fructose group of GP binds to the hairpin region of AptGP, the peptide chain aligns with the groove surface of AptGP. This AptGP was then modified on a silicon nanowire field-effect transistor (AptGP/SiNW-FET) to recognize HbA1c in physiological solutions. To mitigate the Debye-Hückel screening effect, a porous and HbA1c-permeable polyethylene glycol (PEG) polymer was comodified on the AptGP/SiNW-FET surface (termed PEG:AptGP/SiNW-FET) to enable the detection of HbA1c in peripheral human blood. The binding free energy (−12.30 ± 0.05 kcal/mol) of the GP-AptGP complex determined experimentally by PEG:AptGP/SiNW-FET matches excellently with theoretical calculations. This reusable PEG:AptGP/SiNW-FET aptasensor detects HbA1c levels in human blood as accurately as conventional capillary electrophoresis but has the advantages of being fast and cost-effective and thus is amenable to point-of-care diagnostics.

Abstract:The glycated hemoglobin (HbA(1c)) level, which is defined as the ratio of HbA(1c) to total hemoglobin (tHb, including glycated and unglycated hemoglobin), is considered one of the preferred indicators for diabetes monitoring. Abstract:Quantifying glycated albumin (GA) levels in the blood is crucial for diagnosing diabetes because they strongly correlate with blood glucose concentration. In this study, a biotic/abiotic sandwich assay was developed for the facile, rapid, and susceptible detection of human serum albumin (HSA) and GA. The proposed sandwich detection system was assembled using a combination of two synthetic polymer receptors and natural antibodies. Molecularly imprinted polymer nanogels (MIP-NGs) for HSA (HSA-MIP-NGs) were used to mimic capture antibodies, whereas antibodies for HSA or GA were used as primary antibodies and fluorescent signaling MIP-NGs for the Fc domain of IgG (F-Fc-MIP-NGs) were used as a secondary antibody mimic to indicate the binding events. The HSA/anti-HSA/F-Fc-MIP-NGs complex, formed by incubating HSA and anti-HSA antibodies with F-Fc-MIP-NGs, was captured by HSA-MIP-NGs immobilized on the chips for fluorescence measurements. The analysis time was less than 30 min, and the limit of detection was 15 pM. After changing the anti-HSA to anti-GA (monoclonal antibody), the fluorescence response toward GA exceeded that of HSA, indicating successful GA detection using the proposed sandwich detection system. Therefore, the proposed system could change the detection property by changing a primary antibody, indicating that this system can be applied to various target proteins and, especially, be a powerful approach for facile and rapid analysis methods for proteins with structural similarity.

Abstract:Glycosylated hemoglobin A1c (HbA1c) is a useful marker for the diagnosis of diabetes mellitus. Commercial column separation methods for HbA1c measurement were lacking throughput and sometimes interfered with hemoglobin variants. In this work, we developed a high-throughput and specific method for HbA1c by quantitative measurement of N-terminal peptides (NT method). Two thousand specimens could be measured in 8 h. The high-throughput was achieved by using a fast analysis of matrix-assisted laser desorption ionization time of flight mass spectrometry (MALDI-TOF MS) and an efficient proteolysis accelerated by laser irradiation. An intensity ratio of glycosylated to non-glycosylated hemoglobin N-terminal peptides was used to calculate the HbA1c level in blood. Interference from Hb variants of N-terminal peptides could be excluded by a highly accurate mass selection. The coefficient of variation (CV) of intra-assay precision was 9.8 and 9.9 %, respectively. The CVs of inter-assay precision over 20 days were 9.1 and 8.4 %, respectively. Measurement results were well correlated with the commercially available column method ( r = 0.995). The NT method is promising for large-scale screening for diabetes mellitus among people.

Abstract:Antibody glycosylation plays a crucial role in the humoral immune response by regulating effector functions and influencing the binding affinity to immune cell receptors. Previous studies have focused mainly on the immunoglobulin G (IgG) isotype owing to the analytical challenges associated with other isotypes. Thus, the development of a sensitive and accurate analytical platform is necessary to characterize antibody glycosylation across multiple isotypes. In this study, we have developed an analytical workflow using antibody-light-chain affinity beads to purify IgG, IgA, and IgM from 16 μL of human plasma. Dual enzymes, trypsin and Glu-C, were used during on-bead digestion to obtain enzymatic glycopeptides and protein-specific surrogate peptides. Ultra-high-performance liquid chromatography coupled with triple quadrupole mass spectrometry was used in order to determine the sensitivity and specificity. Our platform targets 95 glycopeptides across the IgG, IgA, and IgM isotypes, as well as eight surrogate peptides representing total IgG, four IgG classes, two IgA classes, and IgM. Four stable isotope-labeled internal standards were added after antibody purification to calibrate the preparation and instrumental bias during analysis. Calibration curves constructed using serially diluted plasma samples showed good curve fitting (R2 > 0.959). The intrabatch and interbatch precision for all the targets had relative standard deviation of less than 29.6%. This method was applied to 19 human plasma samples, and the glycosylation percentages were calculated, which were comparable to those reported in the literature. The developed method is sensitive and accurate for Ig glycosylation profiling. It can be used in clinical investigations, particularly for detailed humoral immune profiling.
