Abstract:Noncovalent complexes of a tumorigenic benzo[a]pyrene diol epoxide with the guanine-cytosine (GC) and adenine-thymine (AT) base pairs have been examined computationally. (+)-BaP DE-2 forms covalent adducts with DNA via nucleophilic attack on the (+)-BaP DE-2 epoxide. Computational results predict five thermodynamically accessible complexes of AT with (+)-BaP DE-2 that are compatible with intact DNA. Among these, two are expected to lead to adenine adducts. In the lowest energy AT...(+)-BaP DE-2 complex, which has a gas-phase interaction energy of -20.9 kcal mol(-1), the exocyclic NH(2) of adenine is positioned for backside epoxide attack and formation of a trans adduct. The most energetically favorable complex leading to formation of a cis ring-opened adduct lies only 0.6 kcal mol(-1) higher in energy. For GC...(+)-BaP DE-2, there are only two thermodynamically accessible complexes. The higher-lying complex, bound in the gas phase by 24.4 kcal mol(-1) relative to separated GC and (+)-BaP DE-2, would lead to a trans ring-opened N(2)-guanine adduct. In the global minimum energy GC...(+)-BaP DE-2 complex, bound by 27.3 kcal mol(-1), the exocyclic NH(2) group of cytosine is positioned for cis epoxide addition. However, adducts of (+)-BaP DE-2 with cytosine are rarely observed experimentally. The paucity of cytosine adducts, despite the predicted thermodynamic stability of this GC...(+)-BaP DE-2 complex, is attributed to the electrostatic destabilization of the benzylic cation intermediate thought to precede cis addition.
Wearability and preference of mouthguard during sport in patients undergoing orthodontic treatment with fixed appliances: a randomized clinical trial

Abstract:Summary Background Orthodontic patients wearing fixed appliances are susceptible to traumatic dental injuries during a wide range of sporting activities. This randomized clinical trial investigated wearability and preference of mouthguards during sporting activities in patients undergoing orthodontic treatment with fixed appliances. Methods A prospective three-arm crossover randomized clinical trial conducted in the UK. Thirty patients in active orthodontic treatment with fixed appliances undertaking at least 120 minutes of contact sport per 6–8-week observation period were randomly assigned to one of six mouthguard allocation sequences consisting of three mouthguard types: (MG1) custom-made laboratory constructed, (MG2) mouth-formed OPRO® Gold Braces, and (MG3) pre-fabricated Shock Doctor® Single Brace. Patients completed a nine-outcome 100-mm visual analogue scale (VAS) questionnaire relating to mouthguard wearability during sport. Once feedback was completed, subjects were allocated the next mouthguard in the sequence. At study-end, subjects were asked to identify their preferred mouthguard. Results Twenty-four patients (median age = 13; inter-quartile range 12–14.5 years) completed n = 72 follow-up questionnaires with most playing rugby union or field hockey. Considering VAS score as a continuous variable, for comfort, stability, hardness, ability to breathe, ability to not cause nausea, and inclination to chew, MG2 performed better than MG3. For categorization of VAS score into low (less than 80 mm) or high (at least 80 mm) wearability, for comfort, stability, ability to not cause nausea, and inclination to chew, MG1 and MG2 also rated superior to MG3. Patients preferred MG1 overall. Conclusions This randomized clinical trial found that during contact sport patients in fixed appliances reported superior wearability for custom-made and mouth-formed mouthguards in comparison to pre-fabricated. Abstract:In this paper, the dynamic analysis of mental sweating for sound stimulus of a few tens of eccrine sweat glands is performed by the time-sequential piled-up en-face optical coherence tomography (OCT) images with the frame spacing of 3.3 sec. In the experiment, the amount of excess sweat can be evaluated simultaneously for a few tens of sweat glands by piling up of all the en-face OCT images. Strong non-uniformity is observed in mental sweating where the amount of sweat in response to sound stimulus is different for each sweat gland. Furthermore, the amount of sweat is significantly increased in proportion to the strength of the stimulus.

Abstract:The positive effects of wearing a mouthguard have been indicated in various epidemiological surveys and experiments, and their usage appears to be increasing in many sports. However, many preventable sports-related dental injuries still occur even with the use of a conventional mouthguard. We have developed a mouthguard (the Hard & Space mouthguard) with sufficient injury prevention ability (more than 95% shock absorption ability against impact with a steel ball carrying 15.2 kg m(2) S(-2) potential energy) and ease of clinical application. This mouthguard consists of an outer and an inner EVA layer and a middle layer of acrylic resin (hard insert), with a space to prevent contact between the inner surface of the mouthguard and the buccal surfaces of the maxillary front teeth or teeth already weakened through prior damage or treatment. The purpose of this article is to describe the method by which the Hard & Space mouthguard may easily be fabricated. We believe that this new type of mouthguard has the potential to reduce sports-related dental injuries.

Abstract:For athletes, the use of a mouthguard is fundamental. This device allows for the absorption and dissipation of energy from the impact. These rehabilitated patients will be subjected to oral-facial injuries and trauma again once they return to contact sports. Evaluate the mechanical effect of an impact over a maxilla region where implant-abutment-crown are present, and this mechanical behavior will be analyzed through the evaluation of maximum stress distributions. A three-dimensional geometry of an implant system (implant-abutment-crown and the bone) was obtained from computed tomography and transformed into separate meshes. A mouthguard was created to cover the surface, and a steel ball with a velocity of 1 m/s was used as the impact object. The results were divided into the individual analysis of the stress generated on each variable studied. The mouthguard was capable of reducing the stress on the implant by 71.81%, stress on the abutment by 73.20%, on both pieces by 74.30%, as well as reducing the impact on the prosthetic crown by 98.32%, thus preventing its fracture. A mouthguard is capable of reducing stress on the implant-abutment-crown complex and bone cortex.

Abstract:Mouthguards (MG) are protective devices that can reduce the risks of facial trauma. However, many athletes do not use them. Additionally, MG wear with coincidental parafunctional activity has not been considered. The aim of this study was to evaluate the stress distribution as a consequence of a direct impact comparing a conventional MG with a novel hybrid appliance (HMG). Using computer-aided design (CAD) software, a human skull was modeled with the teeth inserted into their respective alveolus. The models were divided according to the MG type (conventional or hybrid). The geometries were exported to the computer-aided engineering (CAE) software and the materials were considered isotropic. Fixation was defined at the base of the maxilla. The load was applied using a hockey puck. The total deformation (mm) and the von Mises stress (MPa) results were obtained for the MGs (conventional and hybrid), upper teeth, lower teeth, and maxillary bone. Despite the presence of an MG, it is still possible to observe generated stress in all structures. However, the hybrid design was more efficient than the conventional design in reducing the displacement during the impact and consequently the stress on the upper teeth, lower teeth, and maxillary bone. Higher stress magnitude was more concentrated at the inner portion of the hybrid design than the conventional device. The HMG appliance decreased the stress concentration in the teeth and in the bone, limiting the areas susceptible to injuries to the regions directly impacted by the hockey puck. Although the novel HMG may mitigate injury, some stress will still result, and any possible injury should be evaluated by a dental professional.

Abstract:A unidirectionally twinned, {ital c} oriented YBa{sub 2}Cu{sub 3}O{sub 7{minus}{delta}} film was prepared on a (001) NdGaO{sub 3} substrate. In the normal state between 100 and 300K, the unidirectional twin plane lattice induces a strong anisotropy of {rho}{sub {ital ab}}, leading to a temperature independent ratio of 6 between the resisitivities across and along the twin boundaries. At 77K, the self-field critical current parallel to twin planes is 1.2{times}10{sup 6} A/cm{sup 2}, a value which is 25 times higher than along the perpendicular direction. This shows that, at this temperature, twin boundaries control the critical current values in YBCO films by either channeling or pinning effects. {copyright} {ital 1996 The American Physical Society.}

The effect of dental restoration geometry and material properties on biomechanical behaviour of a treated molar tooth: A 3D finite element analysis

Abstract:Objectives: To test the hypothesis that restoration of class II mesio-occlusal-distal (MOD) cavities can be strengthened through judicious choice of restoration geometry and material properties. Methods: An intact extracted human maxillary molar tooth was digitized, segmented, reconstructed, and four 3D restored tooth models were developed with four different restoration geometries: one straight, one single-curved, and two double-curved. Stress analysis was conducted for representative loading using finite element analysis, and maximum principal stresses were determined at the dentine-enamel and restoration-enamel junctions. A range of restorative material elastic moduli (5-80 GPa) and Poisson's ratios (0.25-0.35) were studied. Vertical loads of 400 N were applied on occlusal points, while the roots of the molar teeth, below the crevices, were supported in all directions. All the materials were modelled as homogeneous, isotropic, and elastic. Results: The maximum principal stresses at the restoration-enamel junctions were strongly dependent on the MOD restoration geometries. Peak stresses occurred along the palatal surface of the restoration rather than the opposite buccal surface. Double-curved restorations showed the lowest peak stress at restoration-enamel junctions. Choice of the mechanical properties of restorative material in the range of 5-35 GPa further reduced stress concentrations on the enamel. Significance: Class II MOD restorations may be stronger if designed with double-curved marginal geometries that can reduce stress concentrations. Designs with convex and concave geometries were particularly effective because they reduced stress concentrations dramatically. Results suggest that relatively minor changes to the geometry of a restoration can have a substantial effect on stress at the restoration-enamel junction and motivate future experimental analysis.

Abstract:Custom sports mouthguards are used in various sports to protect teeth, temporomandibular joints, and soft tissues from impact forces. The present article demonstrates a digital workflow to fabricate a 3-dimensionally (3D) printed individualized sports mouthguard. An optical jaw tracking system is used to record a repeatable reference position, and mandibular excursive movements to achieve a completely balanced occlusion. The technique simplifies the fabrication of a custom-fit mouthguard over the conventional approach by providing increased thickness accuracy, control of design, and integration of jaw motion.

Abstract:Background: Orofacial injuries are common occurrences during contact sports activities. However, there is an absence of data regarding the performance of hybrid occlusal splint mouthguards (HMG), especially during compressive loading. This study amid to evaluate the biomechanical effects of wearing a conventional custom mouthguard (MG) or the HMG on the teeth, bone, and the device itself. Methods: To evaluate the total deformation and stress concentration, a skull model was selected and duplicated to receive two different designs of mouthguard device: one model received a MG with 4-mm thickness and the other received a novel HMG with the same thickness. Both models were subdivided into finite elements. The frictionless contacts were used, and a nonlinear analysis was performed simulating the compressive loading in occlusion. Results: The results were presented in von-Mises stress maps (MPa) and total deformation (mm). A higher stress concentration in teeth was observed for the model with the conventional MG, while the HMG design displayed a promising mechanical response with lower stress magnitude. The HMG design displayed a higher magnitude of stress on its occlusal portion (7.05 MPa) than the MG design (6.19 MPa). Conclusion: The hybrid mouthguard (HMG) reduced (1) jaw displacement during chewing and (2) the generated stresses in maxillary and mandibular teeth.

Abstract:Aim: To evaluate the stress distribution of calculated standardized force on maxillary and mandibular jaw bones with proclined maxillary anterior teeth with and without the use of a mouthguard. Materials and methods: It's a finite element analysis (FEA) study. Maxillary and mandibular jaw models with and without mouthguard (4 mm) were created using a cone-beam computed tomography (CBCT) scan of a 13-year-old male patient with proclined maxillary anterior teeth. Calculated standardized forces were applied on the maxillary central incisors in the horizontal direction of the created model. The von Mises stresses (MPa) were analyzed on the maxillary and mandibular jaw. Results: Higher von Mises stresses were analyzed in the maxillary and mandibular jaw model without a mouthguard compared to the model with a mouthguard. Forces were more evenly distributed in the maxillary and mandibular jaw models with a mouthguard. Conclusion: A mouthguard can be used to reduce and evenly distribute the stress on the maxillary and mandibular jaw with proclined maxillary anterior teeth when exposed to real-life situation forces. Clinical significance: The force of injury is influenced by the size of the object or the velocity at which the athlete hits the object/individual. There is a need to evaluate the force at which the individual athlete hits the object/surface based on average height, weight, and velocity/performance at that particular age. As the age of the athlete's advances, there is a change in the height, weight as well as performance. This factor will influence the impact and severity of the trauma caused during sports activities. Abstract:Objective To survey the shear resistance of the bonding interface between the mouthguard and the liner.To investigate the feasibility of relining the custom-made mouthguards for children who had transitional dentition. Methods Custom-made mouthguards were made for 22 children.A longitudinal clinical evaluation were carried through.Two unfitted mouthguards were relined.The shear resistance of the bonding interface between the mouthguard and the liner was measured. Results Seventeen of 22 users((77.3%)) had previous history of dental trauma.Model preparation with wax were carried out for 8 children whose permanent teeth eruption were uncompleted.Two unfitted mouthguards were relined a half-year later.Almost all mouthguard showed perfect retention whatever in stable dentition and in transitional dentition.In the usage of mouthguard 4 children encountered at least once knock in the maxillofacial area,but no injury of tooth and soft tissue occurred.The difference of bonding interface's shear resistance between the control group and three experiment groups which steeped in(37℃) water for 10 days,20 days,and 30 days separately was no significant.Relining had no influence on mouthguard's durability. Conclusion The method of relining is administer to prolong the service life of mouthguard in transitional dentition.Custom-made mouthguard's popularizing in children will benefit from it.

Abstract:Background/Aim Mouthguards are crucial for protecting athletes against orofacial injuries, yet concerns persist regarding their potential impact on oral functions. This study aimed to investigate the effects of sports mouthguards on oral functions and speech over time. Material and Methods Thirty national rugby players received custom‐fitted mouthguards. Questionnaire responses and speech recordings were collected before mouthguard use and at various intervals after using mouthguards: immediately, 1 week, 2 month, and 6 months. Spectrographic analysis was performed to measure voice onset time (VOT) for /p, b, t, d/ phonemes. Questionnaire responses were assessed with Friedman's test, while VOT changes were examined using one‐way repeated measure analysis of variance. Abstract:Ethylene vinyl acetate mouthguards are the most often used custom protective intraoral appliances by combat sports practitioners. However, due to the difficulties in maintaining the hygiene of such mouthguards and thinning during fabrication, resulting in difficulty in predicting final dimensions, they may not be the optimal solution. The aim of this research was to evaluate an innovative method of mouthguard formation using intraoral modeling of the mouthguard pattern, hybrid acrylic material, and the addition of ZnO nanoparticles. Seventeen mouthguards patterns and 34 custom mouthguards were evaluated: 17 hybrid and 17 nanohybrid. A total of 1122 measurements were performed: each mouthguard and pattern was measured at 22 points. Statistical analyses were performed with the use of IBM® SPSS® Statistics 27.0.0 software (IBM, Armonk, NY USA). The mean thickness of the patterns and mouthguards at all labial areas of central incisors were between 4.65 and 4.80 mm. The thickness at the buccal surface of the first molar was between 3.71 and 4 mm, and at the occlusal surface between 3.40 and 3.56 mm in the cusp area. All measurements of hybrid and nanohybrid mouthguards were strongly and highly correlated with the measurements of the mouthguard patterns. Hybrid and nanohybrid mouthguards are an advantageous alternative to thermoformed custom appliances.

Chairside Digital Design and Manufacturing Method for Children's Band and Loop Space Maintainers.

Abstract:OBJECTIVES:This study proposes a chairside digital design and manufacturing method for band and loop space maintainers and preliminarily validates its clinical feasibility.METHODS:Clinical cases of 10 children requiring space maintenance caused by premature loss of primary teeth were collected. Intraoral scan data of the affected children were also collected to establish digital models of the missing teeth. Using a pediatric band and loop space maintainer design software developed by our research team, a rapid personalized design of band and loop structures was achieved, and a digital model of an integrated band and loop space maintainer was ultimately generated. A chairside space maintainer was manufactured through metal computer numerical control machining for the experimental group, whereas metal 3D printing in the dental laboratory was used for the control group. A model fitting assessment was conducted for the space maintainers of both groups, and senior pediatric dental experts were invited to evaluate the clinical feasibility of the space maintainers with regard to fit and stability using the visual analogue scale scoring system. Statistical analysis was also performed.RESULTS:The time spent in designing and manufacturing the 10 space maintainers of the experimental group was all less than 1 h. Statistical analysis of expert ratings showed that the experimental group outperformed the control group with regard to fit and stability. Both types of space maintainers met clinical requirements.CONCLUSIONS:The chairside digital design and manufacturing method for pediatric band and loop space maintainers proposed in this study can achieve same-day fitting of space maintainers at the first appointment, demonstrating good clinical feasibility and significant potential for clinical application.

Abstract:BACKGROUND/AIM: It is difficult to characterize the impact behavior of mouthguards on the basis of their components. Impact behavior tests should be performed on mouthguard formed to simulate their intra-oral performance. The aim of this study was to compare the impact behavior of six models of mouthguards using a standardized experimental protocol.MATERIAL AND METHODS: Four commercially available mouth-formed mouthguards (SDI™, Gel Nano™, Opro Shield Gold™ and Kipsta R300™), one mouth-formed mouthguard prototype and one custom-made mouthguard were tested. The procedures recommended by the manufacturers (injecting procedure for custom-made mouthguard and "boil-and-bite" procedures for mouth-formed mouthguards) were used to adapt five samples per model on steel jaws. Impact performances were assessed according to labial aspect thickness and maximum contact load (FMax) during impact using a drop tower.RESULTS: SDI™ and Opro Shield Gold™ had the thinnest labial aspect thickness (P<.01), followed by the Gel Nano™ and the Kipsta R300™ (P<.01) with a thickness of about 3 mm. The prototype and custom-made mouthguard were thicker (almost 4 mm). The custom-made mouthguard, the Kipsta R300™ and the prototype had the best impact performances, but the labial aspect thickness of the Kipsta R300™ was significantly lower than that of the custom-made mouthguard and the prototype. Analysis of force curves and position of the mouthguard on the impacted zone showed that the Kipsta R300™ was less well adapted.CONCLUSION: Thickness and impact performance are not sufficient criteria to characterize performance of mouthguards.

Motivators and Barriers to Mouthguard Compliance by Adult Gaelic Football Athletes

Abstract:Introduction Dental injuries contribute to 57% of reported maxillofacial injuries in Gaelic Football, with associated psychological and economic impacts on the affected athletes. Mouthguards have been developed in efforts to mitigate the incidence and severity of dental injuries, and use is mandatory in Gaelic Football. Dental claims have reduced by over 50% since mouthguards became mandatory, but costs of dental injuries are still prevalent. The aim of this study was to determine the mouthguard compliance rates in adult Gaelic Football players, as well as the motivations and barriers to compliance. Methods This cross-sectional study utilised an online survey to determine the self-reported compliance of adult Gaelic Footballers with mouthguard use, the perceived peer compliance, and the motivations and barriers to compliance. Chi-square tests were used to examine differences between sex and between elite and sub-elite players. Results A total of 545 Gaelic Footballers completed the survey. During training, 22% of players reported to always wear a mouthguard, with 48% never wearing it, and 30% occasionally wearing it. For games, 48% of players reported to wear a mouthguard, with 11% never wearing it and 41% occasionally wearing it. Motivating factors included teeth protection, gum protection and the rules of the game. The main barriers were discomfort, difficulty breathing and difficulty speaking. Females had significantly poorer compliance in training, but had significantly better compliance in games when compared to males. Conclusion Mouthguard compliance is relatively poor amongst adult Gaelic Football players. Although compliance improves for games compared to training, there are still 1 in 2 players not wearing a mouthguard for games. Discomfort and challenges with breathing and speaking suggest that players may benefit from having a custom-fit mouthguard. Coaches, refereeing officials and governing bodies should strive to implement the rules more often and improve education around the benefits of mouthguard use within the sport.
