Locoregional Recurrence Analysis in Young Women (≤35 Years) Receiving Mastectomy with Operable Breast Cancer

Abstract:Young women with breast cancer are at higher risk of locoregional recurrence in comparison with elder women. The study aimed to determine the factors related to locoregional recurrence and the impact of post-mastectomy radiotherapy in women ≤35 years old. Between February 1989 and December 2004, 459 patients ≤35 years old with operable breast cancer who treated mastectomy were retrospectively analyzed. The type of surgery included: radical mastectomy (47.4%), extended radical mastectomy (11.5%) and modified radical mastectomy (MRM; 39.6%). The percentage for T1, T2, and T3 tumor was 22.0%, 52.3%, and 10.0%, respectively. Lymph node without metastasis accounted for 25.7% of all and N1, N2, and N3 lymph node accounted for 25.5%, 20.7%, and 14.6%. Four hundred twenty-three patients received chemotherapy and 160 patients received endocrine therapy. PMRT was administered in 220 patients (47.9%). Radiation field included supraclavicular lymph node and internal mammary nodes only in 187 and 170 patients while chest wall was treated in 25 patients. Median age of this cohort was 32 years old and median follow-up was 50.1 months. The median disease-free interval to first isolated locoregional recurrence was 17.8 ± 2.7 months. The 5-year and 10-year isolated locoregional control rate (ILRCR) was 74.9% and 70.0%, respectively. The most common site of recurrence was CW (106/131) followed by SN (18/131). The 5-year CW control rate decreased with more advanced T and N Stage (p<0.05). Compared to the patients with no ILR, patients with ILR had a shorter 5-year metastasis-free survival (MFS) (66.6% vs. 53.8%; p = 0.003). In univariate analysis, surgical modality, tumor size, lymph node status, chemotherapy and ILR were significant predictors for overall survival (OS). In multivariate analysis, only higher T-Stage (p = 0.041; HR = 1.69; 95% CI = 1.02 – 2.16) was the independent predictor for isolated chest-wall recurrence (ICWR) while higher N-Stage (p = 0.005; HR = 1.58; 95% CI = 1.15 – 2.78) was the additional predictor for ILR besides higher T-Stage (p = 0.047; HR = 1.58; 95% CI = 1.02 – 2.42). Higher T-Stage (p = 0.014; HR = 1.52; 95% CI = 1.09 – 2.12), higher N-Stage (p < 0.001; HR = 2.25; 95% CI = 1.67 – 3.03), the use of endocrine therapy (p = 0.014; HR = 0.59; 95% CI = 0.39 – 0.90) and ICWR (p = 0.006; HR = 1.87; 95% CI = 1.20 – 2.91) are independent predictors for MFS. As to OS, all factors above were independent prognostic factors for OS except for the endocrine therapy administration (p > 0.05). Young women with operable breast cancer receiving mastectomy have a high risk of ILR and shorter disease-free interval. It is critical to re-evaluate the impact of PMRT in young patients. Chest-wall remains the region with higher risk of recurrence and should not be neglected as PMRT target.
Analysis In Early Stage Triple-Negative Breast Cancer Treated With Mastectomy Without Adjuvant Radiotherapy: Patterns Of Failure And Prognostic Factors

Abstract:BACKGROUND The objective of this study was to evaluate and identify patterns of failure and prognostic factors for locoregional recurrence (LRR) that could justify postmastectomy radiotherapy after modified radical mastectomy in patients with early stage triple-negative breast cancer. METHODS Between January 2000 and July 2007, the authors retrospectively analyzed 390 patients who had triple-negative breast cancer with T1/T2 tumors and from zero to 3 positive lymph nodes (pathologic T1-T2N0-N1) who underwent modified radical mastectomy without postmastectomy radiotherapy at the author's institution. The 5-year cumulative incidence for events was calculated using Kaplan-Meier analysis, and subgroups were compared using the log-rank test. Multivariate analysis was performed using a Cox proportional hazards model. RESULTS Overall, 86.4% of patients received chemotherapy. At a median follow-up of 60.5 months, the 5-year cumulative rates of local recurrence, regional recurrence, LRR, and distant metastasis were 5.4%, 4.7%, 8%, and 13.4%, respectively. On multivariate analysis, age <50 years, the presence of lymphovascular invasion, grade 3 tumor, and 3 involved lymph nodes were associated significantly with an increased risk of LRR. The 5-year LRR rate for patients who had 0 or 1 risk factor, 2 risk factors, and 3 or 4 risk factors was 4.2%, 25.2%, and 81% (P<.0001), respectively. The presence of lymphovascular invasion and having 3 involved lymph nodes were statistically significant predictors of regional recurrence, and the patients who had regional recurrence had a significantly greater risk of distant metastases compared with patients who had local recurrence (59.1% vs 20.9%; P < .0001). CONCLUSIONS Several risk factors were identified in this study that correlated independently with a greater incidence of LRR in patients who had early stage triple-negative breast cancer. The current results indicated that postmastectomy radiotherapy should be considered for those patients who have 2 or more of these factors. Cancer 2013;119:2366-2374. Abstract:Purpose Postmastectomy radiation therapy (PMRT) in T1–T2 tumors with 1–3 positive axillary lymph nodes (ALNs) is controversial. This study was to identify prognostic factors of locoregional control (LRC) following mastectomy with or without PMRT for patients with T1-2N1 breast cancer and to discuss the selection of patients who might omit PMRT. Materials and methods Between January 2006 and December 2012, the data of 1474 postmastectomy patients staged pT1-2N1 were analyzed. PMRT was applied in 663 patients. LRC and disease-free survival (DFS) were calculated using the Kaplan–Meier method. Cox regression model was applied in the univariate and multivariate analyses to recognize the recurrence risk factors. Results With the median follow-up duration of 93 months (range, 5–168 months), 78 patients (5.3%) failed to secure LRC and 220 patients (14.9%) experienced any recurrence. The 7.7-year LRC and DFS was 94.9% and 85.4% respectively in the entire cohort. PMRT significantly improved 7.7-year LRC from 93.4% to 96.6% ( p = 0.005), but not the DFS ( p = 0.335). Multivariate analysis revealed that PMRT was an independent prognostic factor of LRC ( p < 0.001), meanwhile, age ≤ 40 years ( p = 0.012), histological grade 3 ( p = 0.004), 2–3 positive nodes ( p < 0.001) and tumor size of 3–5 cm ( p = 0.045) were significantly associated with decreased LRC. The 7.7-year LRC for patients with 0, 1, and 2–4 risk factors was 97.7% / 98.9% ( p = 0.233), 95.3% / 98.0% ( p = 0.092), and 80.3% / 94.8% ( p < 0.001) in the non-PMRT and PMRT group, respectively. Conclusions In patients with T1-2N1 breast cancer, clinical-pathological factors including young age, histological grade 3, 2–3 positive nodes, and tumor size of 3–5 cm were identified to be predictors of a poorer LRC following mastectomy. Patients with 0–1 risk factor might consider the omission of PMRT.
Risk Stratification Model for Prediction of Locoregional Recurrence in Patients with Pathologic T1-2N0 Breast Cancer after Mastectomy

Abstract:To identify the risk factors of locoregional recurrence (LR) in patients with pathologic T1-2N0 breast cancer in a real-world and distinguish individuals warrant postmastectomy radiotherapy (PMRT) Patients who had pathologic T1-2N0 breast cancer from 1999 to 2014 were retrospectively reviewed. Locoregional recurrence was defined as reappearance in the ipsilateral chest wall, axilla, infra/supraclavicular fossa, or internal mammary chain. The association of locoregional recurrence with pretreatment factors was evaluated using multi-variable proportional hazards regression. A total of 4841 patients were eligible patients in this study, and all underwent mastectomy plus axillary node dissection without PMRT. 186 LR developed within a median follow-up period for survivors of 56.4 months (range, 1–222 months). Multivariate analysis revealed age < 50yeras old (p<0.001, hazard ratio [HR] =1.795), T2 stage (p=0.008, HR=1.510), negative expressions of estrogen receptor (ER) and progesterone receptor (PR) (p<0.001, HR=1.921), and tumor location in inner quadrant (p<0.001, HR=1.960) were independent predictor of LR. There were 768 patients (15.9%) with no risk factors, 3328 patients (68.7%) with 1-2 risk factors, and 745 patients (15.4%) with 3-4 risk factors. Patients with 0, 1-2, and 3-4 risk factors had 5- and 10-year LR failure of 2.3% and 5.6%, 3.6% and 5.9%, 7.1% and 8.2%, respectively (p < 0.001). 5-year metastasis-free survival and overall survival (OS) were significantly lower for patients with 3-4 factors (89.2% and 93.9%) compared with patients with 1-2 (92.7% and 96.4%) or 0 (97.4% and 99.1%) risk factors. Risk model based on age, T stage, ER/PR and tumor location can stratify patients with pT1-2 N0 breast cancer into subgroups with different LR and outcomes. Patients with 3-4 risk factors may benefit from PMRT.

This study was to identify prognostic clinical-pathological factors of locoregional control (LRC) following mastectomy and axillary dissection with or without adjuvant RT for patients with T1-2N1 breast cancer, and to discuss the selection of patients who might not benefit from PMRT. Between January 2006 and December 2010, the data of 839 post-mastectomy patients staged pT1-2N1M0 were analyzed. Radiotherapy was applied in 483 patients. LRC was assessed using the Kaplan-Meier method and log-rank test. At a median follow-up of 87 months (range, 1-167months), 47 (5.6%) patients developed locoregional recurrence (LRR). The 7-year LRC was 94.2% and 94.3% in the non-RT group and RT group respectively (P > .05). Multivariate analysis revealed that RT was an independent prognostic factor of LRC (HR, 0.46; 95% CI, 0.24-0.89; P = .02), meanwhile, age < 40 years, histological grade 3 tumor, and 3 involved lymph nodes were associated significantly with decreased LRC. The 7-year LRC for patients with 0, 1, and 2 or more risk factors was 97.3% / 95.7% (P = .97), 92.7% / 96.9% (P = .04), and 72.9% / 87.2% (P = .18) in the non-RT and RT group respectively. In patients with T1-2N1 breast cancer, clinical-pathological factors such as adjuvant RT, old age, grade 1-2, and 1-2 involved nodes were identified to be prognostic of decreased risk of LRR following mastectomy, and might be helpful in selecting candidates for whom adjuvant RT could be omitted.

Abstract:Purpose Young women with breast cancer have a higher risk of local-regional recurrence (LRR) than older women. Chemotherapy was given to 423 patients, and 160 patients received endocrine therapy. PMRT was administered in 220 patients (47.9%). Statistical analysis was carried out with SPSS, version 16.0 (SPSS Inc, Chicago, IL). Results The median age of this cohort was 32 years, and median follow-up was 50.1 months. The median disease-free interval before the first isolated local-regional recurrence (ILRR) was 18.8 ± 2.7 months. The 5-year and 10-year isolated local-regional control rates were 74.9% and 70.0%, respectively. Compared to the patients without ILRR, patients with ILRR had a shorter 5-year metastasis-free survival (MFS; 66.6% vs. 53.8%, P = 0.003). Double-blind comparison of ketanserin with atenolol: antihypertensive activity and effect on platelet function.

Abstract:In a randomized double-blind study with parallel design involving 20 hypertensive patients, the antihypertensive activity and effect on platelet function of ketanserin have been compared with those of atenolol. Patients in both treatment groups were matched for age, body weight, duration of hypertension and blood pressure values on placebo. After 12 weeks of oral treatment, both ketanserin and atenolol significantly reduced blood pressure from 183/113 and 187/111 mmHg, at the end of the placebo run-in period, to 159/91 and 169/99 mmHg, respectively (P less than 0.05). No significant differences were noted between the hypotensive effect of both drugs. In contrast to ketanserin, atenolol significantly reduced the heart rate after 12 weeks of therapy (P = 0.025). Ketanserin significantly inhibited serotonin-induced platelet aggregation and serotonin uptake by thrombocytes, whereas atenolol failed to alter these variables. The results indicate that the antihypertensive activity of the serotonin antagonist ketanserin is comparable with that of the beta-receptor blocker atenolol.
Risk Factors of Locoregional Relapse in Locally Advanced Breast Cancer Treated with Neoadjuvant Chemotherapy Following Mastectomy and Radiotherapy

Abstract:We seek to investigate the prognostic factors that could possibly increase the locoregional recurrence of breast cancer patients who do not achieve pathological complete response after neoadjuvant chemotherapy, and to build a prognostic nomogram to predict patients' outcome. The retrospective analysis included 510 patients who had received neoadjuvant chemotherapy followed by surgery and radiotherapy. 62 locoregional events occurred after a median 61 months of follow-up. The five-year cumulative incidence of local recurrence and regional recurrence were 8.63% and 4.31%, respectively. Multivariate analysis revealed that positivity for ≥ 4 lymph nodes and Ki-67 index ≥ 14% were independent factors. According to our prognostic model, the 5-year locoregional free survival rates in the low, intermediate, and high-risk groups were 95.5%, 89.1%, and 67.1%, respectively (p < 0.001). Annual recurrence curves indicated that the relapse peak after mastectomy emerged in the first 1 year. Positivity for ≥ 4 lymph nodes and Ki-67 index ≥ 14% were independent factors for locoregional recurrence. This prognostic model has considerable clinical value in predicting locoregional recurrence, which could help clinicians to design appropriate locoregional treatment specifically and to perform surveillance individually.
Establishment of a risk scoring system for predicting locoregional recurrence in T1 to T2 node-negative breast cancer patients treated with mastectomy: Implications for postoperative radiotherapy.

Abstract:To establish a risk scoring system for predicting locoregional recurrence (LRR) and explore the potential value of radiotherapy in T1 to T2 node-negative breast cancer patients treated with mastectomy. From January 2001 to February 2008, a total of 353 node-negative T1 to T2 breast cancer cases treated with mastectomy without adjuvant radiotherapy were retrospectively analyzed. Preliminary screening of the prognostic factors was accomplished by Kaplan-Meier univariate analysis, and survival curves between different groups were compared by log-rank test. Risk factors were determined using Cox proportional hazards model. A categorical risk scoring system was generated according to the Cox model, weighing the relative importance of each risk variable. Median follow-up was 115.7 months (range, 1.2-238.4 months). The overall 5-year locoregional recurrence-free survival (LRFS) was 89.8% (95% confidence interval [CI]=86.7%-92.9%). Chest wall (53.8%) was found to be the most common site of LRR, followed by supraclavicular nodes (48.7%). Age 40 years, primary tumor size 4.5cm and number of nodes resected 10 were found to be independent factors for poor prognosis of LRR. Two risk stratifications based on the scoring system were subsequently obtained. The 5-year LRFS was 91.6% (95% CI=88.5%-94.7%) with low risk (score <2) and 75.7% (95% CI=61.8%-89.6%) with high risk (score 2), respectively ((2)=7.544, P=.006). In addition, significant differences in overall survival (P=.045) and disease-free survival (P=.019) were presented between them. Patients with T1-2N0M0 breast cancer achieved favorable prognosis in general. Those with risk factors, including age 40 years, primary tumor size 4.5cm and number of nodes resected 10, were at higher risk of LRR. The established scoring system could help to distinguish the subgroups that might potentially benefit from postoperative radiotherapy.
Benefit of post-mastectomy radiotherapy of the supra-/infraclavicular lymphatic drainage area in breast cancer patients

Abstract:Background: This study investigated the survival benefit of radiotherapy (RT) of the supra- and infraclavicular lymphatic drainage area in Chinese women with T1-2N1M0 breast cancer receiving mastectomy. Methods: A total of 593 cases were retrospectively reviewed from 1998 to 2007. The relationship between supra- or infraclavicular fossa relapse (SCFR) and post-operative RT at the supra-/infraclavicular lymphatic drainage area was evaluated. Results: The majority of patients (532/593; 89. 8%) received no RT while 61 patients received RT. The median follow-up was 85 months. Among patients without RT, 54 (10. 2%) developed recurrence in the chest wall or ipsilateral SCFR. However, none of the 61 patients who underwent RT demonstrated SCFR. One patient who received RT (1. 6%) experienced recurrence in the chest wall. Univariate analysis revealed that age and molecular subtype (both P < 0. 05) were two prognostic factors related to supraclavicular and infraclavicular fossa relapse-free survival (SFRFS). Multivariate analysis revealed that only Her-2 positive status (P = 0. 011) was an independent predictor of SFRFS. RT had no influence on distant metastasis (P = 0. 328) or overall survival (P = 0. 541). SCFR significantly affected probability of distant metastasis (P < 0. 001) and overall survival (P < 0. 001). Conclusion: Although RT was not significantly associated with SFRFS, postoperative RT was significantly associated with a lower locoregional (i. e. , supraclavicular/infraclavicular and chest wall) recurrence rate. SCFR significantly influenced distant metastasis-free survival, which significantly influenced the overall survival of T1- 2N1M0 breast cancer patients after mastectomy. Thus, prophylactic RT is recommended in T1-2N1M0 breast cancer patients, especially those who have Her-2 positive lesions.
Radiotherapy for and Prognosis of Breast Cancer Patients with Local-Regional Recurrence after Mastectomy

Abstract:BACKGROUND AND OBJECTIVE:Controversies remain regarding the therapeutic principle for and prognosis of breast cancer patients with isolated local-regional recurrence after mastectomy. This study was to evaluate the role of radiotherapy in treating these patients and to investigate the prognosis.METHODS:Clinical data of 255 breast cancer patients with chest-wall and/or regional lymph node recurrence as first failure after mastectomy from 1990 to 2005 were analyzed. All patients received radiotherapy for recurrence.RESULTS:The median follow-up time was 45 months (9 months-15.5 years). The median disease-free interval (DFI) was 22 months (2-260 months); it was 37 months in patients with positive hormonal receptor and 17 months in those with unknown or negative receptor. The 2-, 5-, and 8-year overall survival (OS) rates were 86.4%, 56.5%, and 35.0%, respectively. The median survival time was 79 months. The 2-, 5-and, 8-year local control rates were 6.1%, 36.3%, and 27.6%, respectively. Univariate prognostic analysis showed that DFI, site and number of recurrence, receptor status, short-term therapeutic response, initial T status and axillary involvement significantly affected the survival (all P<0.05); multivariate analysis showed that DFI, receptor status, site and number of recurrence were independent prognostic factors. Prognostic index was established to classify the patients. The 2-, 5-and 8-year OS rates were 100%, 91.6%, and 56.4% in the favorable prognosis group, 88.1%, 59.1%, and 36.8% in the medium prognosis group, 68.0%, 8.5%, and 0 in the poor prognosis group (P<0.001).CONCLUSION:Radiotherapy is effective for breast cancer patients with isolated local-regional recurrence after mastectomy. Prognostic index could be applied to predict the prognosis.

Abstract:Abstract Background Postmastectomy radiotherapy (PMRT) improves survival by eliminating potential occult lesions in the chest wall and lymphatic drainage area. Meta-analysis has shown that PMRT reduces mortality and local recurrence of patients with node positive breast cancer, but there is no specific data about the effectiveness of PMRT in a subgroup of patients with a high number of positive axillary lymph nodes (PALN). The aim of the study was to analyse the impact of the number of PALN on local and distant metastasis occurrence, overall survival (OS) and distant metastases free survival (DMFS) in patients treated with PMRT. Patients and methods We reviewed medical records of 129 consecutive breast cancer patients with PALN, treated at Institute of Oncology Ljubljana with PMRT between January 2003 and December 2004. We grouped patients according to the number of PALN as follows: Group 1 (less than 15 PALN) and Group 2 with more than 15 PALN. All patients received adjuvant systemic therapy according to the clinical guidelines. We analysed number of locoregional (LR) recurrences, distant metastasis, overall survival (OS), progression free survival (PFS) and DMFS. Results After the median follow-up time of 11.5 years, the Kaplan-Meier survival analysis of PALN showed significantly shorter OS (p = 0.006), shorter PFS (p = 0.002) and shorter DMFS (p 15 PALN. Only one LR was found in the group of patients with more than 15 PALN. In multivariate analysis more than 15 PALN and treatment with anthracycline chemotherapy statistically significantly influenced OS and DMFS. For PFS presence of more than 15 PALN were the only independent factor of shorter survival. Conclusions Patients with more than 15 PALN have shorter DMFS, PFS and OS as compared to patients with less than 15 PALN, though they receive the same LR treatment. More studies with higher number of patients included are needed to further evaluate our findings.

Abstract:PurposeTo evaluate the role of postmastectomy radiation therapy (PMRT) in patients with node-negative breast cancer of 5cm or larger tumors undergoing mastectomyMaterials and MethodsMedical records of 274 patients from 18 institutions treated with mastectomy between January 2000 and December 2016 were retrospectively reviewed. Among these, 202 patients underwent PMRT, while 72 did not. Two hundred and forty-one patients (88.0%) received systemic chemotherapy, and 172 (62.8%) received hormonal therapy. Patients receiving PMRT were younger, more likely to have progesterone receptor-positive tumors, and received adjuvant chemotherapy more frequently compared with those without PMRT (p <0.001, 0.018, and <0.001, respectively). Other characteristics were not significantly different between the two groups.ResultsWith a median follow-up of 95 months (range, 1-249), there were 9 locoregional recurrences, and 20 distant metastases. The 8-year locoregional recurrence-free survival rates were 98.0% with PMRT and 91.3% without PMRT (p=0.133), and the 8-year disease-free survival (DFS) rates were 91.8% with PMRT and 73.9% without PMRT (p=0.008). On multivariate analysis incorporating age, histologic grade, lymphovascular invasion, hormonal therapy, chemotherapy, and PMRT, the absence of lymphovascular invasion and the receipt of PMRT were associated with improved DFS (p=0.025 and 0.009, respectively).ConclusionLocoregional recurrence rate was very low in node-negative breast cancer of 5cm or larger tumors treated with mastectomy regardless of the receipt of PMRT. However, PMRT was significantly associated with improved DFS. Further investigation is needed to confirm these findings.

Abstract:Purpose. Several studies have demonstrated poor locoregional control in patients with triple-negative breast cancer (TNBC), compared with other molecular subtypes of breast cancer. We sought to evaluate whether or not postmastectomy radiotherapy (PMRT) improves locoregional recurrence-free survival (LRFS) and disease-free survival (DFS) outcomes in TNBC patients.Methods and Materials. Between January 2000 and July 2007, 553 TNBC patients treated with modified radical mastectomy from a single institution were analyzed retrospectively. Patients were categorized into three groups: low risk (stage T1-T2N0), intermediate risk (stage T1-T2N1), and high risk (stage T3-T4 and/or N2-N3). Cox proportional hazards models were used to evaluate the association between PMRT and LRFS and DFS times after adjusting for other clinicopathologic covariates.Results. With a median follow-up of 65 months (range, 1-140 months), 51 patients (9.2%) developed locoregional recurrence and 135 patients (24.4%) experienced disease recurrence. On multivariate analysis, PMRT was associated with significantly longer LRFS and DFS times in the entire cohort. In the intermediate-risk group, PMRT was associated with a longer DFS time but not with the LRFS interval. In the high-risk group, PMRT was associated with significantly longer LRFS and DFS times.Conclusion. PMRT is associated with longer LRFS and DFS times in high-risk TNBC patients and a longer DFS time in intermediate-risk TNBC patients. Prospective randomized studies are needed to investigate the best locoregional treatment approaches for patients with this molecular subtype of breast cancer. The Oncologist 2013;18:141-147
Outcomes in Patients with pT3N0M0 Breast Cancer with and without Postmastectomy Radiotherapy

Abstract:Purpose: The role of adjuvant postmastectomy radiotherapy (PMRT) remains controversial for patients with pT3N0M0 breast cancer, especially when patients are treated with the updated adjuvant chemotherapy. Our study aimed to compare locoregional recurrence-free survival (LRFS), disease-free survival (DFS), and breast cancer-specific survival (BCSS) in pT3N0M0 patients with and without postmastectomy radiotherapy. Patients and methods: Between October 2000 and 8 September 2016, the database of the Breast Cancer Center of Shanghai yielded 114 patients with node-negative non-metastatic breast cancer larger than 5 cm. Univariate and multivariate analyses were performed to assess the risk factors for survivals. Differences between the two groups were compared using the Log rank test. Results: Fifty-nine (51.8%) of the patients received adjuvant PMRT. The median follow-up was 62.3 months. Five-year LRFS was 100% in the PMRT group vs 98.1% in the non-PMRT group (P=0.17); 5-year DFS was 97.1% for the entire cohort, 98.0% for the PMRT group vs 96.2% for the non-PMRT group (P=0.18). Univariate analysis identified that family history of malignant tumors, lymphovascular invasion (LVI), or triple-negative breast cancer (TNBC) molecular subtype were associated with higher locoregional recurrence (LRR) (P<0.05). No PMRT was the only risk factor independently associated with poorer DFS (P=0.048) on multivariate analysis. No difference in BCSS was observed between the two groups. Conclusion: The present study demonstrated a low LRR rate and good survival for node-negative breast cancer >5 cm. Patients with family history of malignant tumors, TNBC subtype, LVI positivity, or grade 3 disease are at high risk for LRR and might benefit from PMRT.

Abstract:Purpose: To evaluate the effects of postmastectomy radiotherapy (PMRT) in breast cancer patients with pathologically negative lymph nodes (ypN0) after neoadjuvant chemotherapy (NAC).Patients and methods: This multicenter retrospective study assessed the data of 766 breast cancer patients with ypN0 and established a recurrent risk score model based on a disease-free survival (DFS) rate–based multivariate Cox model. Moreover, the locoregional control (LRC), disease-free survival (DFS), and overall survival (OS) rates in PMRT and non-PMRT patients were calculated using the Kaplan–Meier method.Results: All patients received a median of four NAC cycles, followed by mastectomy and axillary lymph node dissection; moreover, 353 (46.1%) patients underwent PMRT. Over a median follow-up duration of 74 months, the 5-year LRC, DFS, and OS rates for all patients were 96.5%, 89.1%, and 95.3%, respectively. The differences in the LRC, DFS, and OS rates between PMRT and non-PMRT patients were nonsignificant in the univariate and multivariate analyses. By using our recurrent risk score model based on five factors (i.e., age, clinical N stage, NAC cycle number, lymphovascular invasion, and pathological tumor stage after NAC), we stratified the patients into low-, intermediate- and high-risk groups; their 5-year rates of LRC (98.6%, 95.5%, and 89.8%, respectively), DFS (94.4%, 87.4%, and 71.5%, respectively), and OS (97.6%, 93.2%, and 90.0%, respectively) were significantly different (all P < 0.001). PMRT did not improve survival outcomes in any of the subgroupsConclusion: Our recurrence risk score model effectively distinguished ypN0 patients with different recurrent risk stratifications. However, no subgroup was identified to benefit from PMRT.

Abstract:Introduction Post-mastectomy radiotherapy is commonly recommended for T3N0M0 breast cancer, particularly in the presence of adverse prognostic factors. However, for T3N0M0 ipsilateral recurrences following breast-conserving surgery and adjuvant radiotherapy, the situation is distinct. Recurrence alone signifies a negative prognostic factor. Moreover, tumor relapses within previously irradiated areas exhibit enhanced radioresistance, and reirradiation of the chest wall carries an escalated risk of radiation-induced toxicity. This study aimed to assess the impact of post-mastectomy reirradiation (PM-reRT) on patient outcomes in cases of ipsilateral T3N0M0 breast tumor recurrence, using data from the SEER database. Materials and methods We identified all patients who underwent treatment for primary non-metastatic breast cancer with breast-conserving surgery followed by adjuvant radiotherapy in the SEER database; among them, those who later experienced a localized T3N0M0 breast tumor recurrence and underwent total mastectomy were included. The study's goal was to compare overall survival (OS) and cancer-specific survival (CSS) between patients who underwent only mastectomy versus those who had mastectomy followed by adjuvant PM-reRT for their ipsilateral T3N0M0 breast tumor relapse. Results From 2000 to 2020, the SEER database recorded 44 patients with an ipsilateral T3N0M0 breast tumor recurrence after initial conservative treatment, managed with total mastectomy. No statistically significant differences in OS or CSS were observed between patients undergoing mastectomy (MT) alone versus those receiving MT combined with PM-reRT ( p = 0.68 and p = 0.86, respectively). Five-year OS rates for the MT and MT + PM-reRT cohorts were 49.5% [95% CI: 29.9–81.8] and 41.7% [10.0–100.0], respectively, while 5‐year CSS rates were 51.6% [12.0–99.5] and 58.3% [15.2–100.0], respectively. Conclusion For patients undergoing total mastectomy after an ipsilateral T3N0M0 breast tumor recurrence, subsequent to initial breast cancer treatment involving breast-conserving surgery and adjuvant radiotherapy, chest wall reirradiation does not enhance survival outcomes. As such, it should not be routinely performed.

Abstract:Background: Extracapsular tumor spread (ECS) has been identified as a possible risk factor for breast cancer recurrence, but controversy exists regarding its role in decision making for regional radiotherapy. This study evaluates ECS as a predictor of local, axillary, and supraclavicular recurrence. Patients and methods: International Breast Cancer Study Group Trial VI accrued 1475 eligible pre- and perimenopausal women with node-positive breast cancer who were randomly assigned to receive three to nine courses of classical combination chemotherapy with cyclophosphamide, methotrexate, and fluorouracil. ECS status was determined retrospectively in 933 patients based on review of pathology reports. Cumulative incidence and hazard ratios (HRs) were estimated using methods for competing risks analysis. Adjustment factors included treatment group and baseline patient and tumor characteristics. The median follow-up was 14 years. Results: In univariable analysis, ECS was significantly associated with supraclavicular recurrence (HR = 1.96; 95% confidence interval 1.23-3.13; P = 0.005). HRs for local and axillary recurrence were 1.38 (P = 0.06) and 1.81 (P = 0.11), respectively. Following adjustment for number of lymph node metastases and other baseline prognostic factors, ECS was not significantly associated with any of the three recurrence types studied. Conclusions: Our results indicate that the decision for additional regional radiotherapy should not be based solely on the presence of ECS.

Abstract:Objective To analyze the outcome and prognostic factors in breast cancer at high-risk of recurrence and evaluate the role of radiotherapy. Methods 381 breast cancer patients treated with mastec-tomy and axillary dissection were retrospectively analyzed. The including criterias were pathologic diagnosis of invasive breast cancer, T_3-T_4 and/or four or more positive axillary nodes. The survival rates was calculat-ed by Kaplan-Meier method, and compared by Logrank test. Cox regression model was used to select poten-tial prognostic variables. Results The median follow up was 48 months. The 5-year overall survival (OS) and locoregional recurrence-free survival (LRFS) rates were 76.8% and 89.7%, respectively. Radiothera-py significantly improved the OS (80.9% vs. 62.3%, χ~2=15.47, P=0.001) and LRFS (93.4% vs. 77.1% χ~2=19.95,P=0.000). The use of ipsilateral chest wall and supraclavicular nodal radiation was associated with increased 5-year chest wall recurrence free survival (96.8% : 86.2%, χ~2= 12.66, P=0.001) and 5-year supraclavicular node recurrence free survival (97.7% : 90.7 %, χ~2= 9.98, P=0.002).However, axillary irradiation had no impact on 5-year axillary recurrence free survival (98.4% : 96.1% ,χ~2=0.74, P=0.389). In multivariate analysis, absence of radiotherapy (χ~2=14.42, P=0.000), 10 or more positive axillary nodes (χ~2=21.60, P=0.000), and T_4 stage (χ~2=10.79, P=0.001) were inde-pendent unfavorable prognostic factors for overall survival. Conclusions Radiotherapy improves the overall survival of breast cancer patients with T_3, T_4 and/or four or more positive axillary nodes. The chest wall and supraclavicular nodal radiation should be given to this group of patients.

Abstract:Among breast cancer patients treated with neoadjuvant chemotherapy (NAC) and mastectomy, locoregional recurrence (LRR) rates are unclear in women with ER+ tumors treated with adjuvant endocrine therapy without postmastectomy radiation (PMRT). To determine if PMRT is needed in these patients, we compared LRR rates of patients with ER+ tumors (treated with adjuvant endocrine therapy) with women who have non-ER+ tumors. 85 consecutive breast cancer patients (87 breast tumors) treated with NAC and mastectomy without PMRT were reviewed. Patients were divided by residual nodal disease (ypN) status (ypN+ versus ypN0) and then stratified by receptor subtype. Among ypN+ patients ( n = 35 ), five-year LRR risk in patients with ER+, Her2+, and triple negative tumors was 5%, 33%, and 37%, respectively ( p = 0.02 ). Among ypN+/ER+ patients, lymphovascular invasion and grade three disease increased the five-year LRR risk to 13% and 11%, respectively. Among ypN0 patients ( n = 52 ), five-year LRR risk in patients with ER+, Her2+, and triple negative tumors was 7%, 22%, and 6%, respectively ( p = 0.71 ). In women with ER+ tumors and residual nodal disease, endocrine therapy may be sufficient adjuvant treatment, except in patients with lymphovascular invasion or grade three tumors where PMRT may still be indicated.
