Abstract:Background: There are no available guidelines regarding preserving or ligating an arteriovenous fistula following successful kidney transplantation. Therefore, we performed a meta-analysis to formulate a clear opinion regarding the status of arteriovenous fistula in renal allograft recipients. Methods: We performed an electronic literature search of PubMed, EMBASE, and the Cochrane Library for eligible studies up to January 2019. We aimed to systematically evaluate cardiac systolic and diastolic function and allograft function after arteriovenous fistula closure. The primary outcome was the differences in left ventricular mass index, left ventricular end-diastolic diameter, ejection fraction, and serum creatinine levels between patients with occluded arteriovenous fistula versus patent arteriovenous fistula. Results: Renal transplant recipients with occluded arteriovenous fistula had lower left ventricular mass index values and left ventricular end-diastolic diameter compared with those with patent arteriovenous fistula. The pooled mean differences were 21.67 (95% confidence interval [CI] 19.94 to 23.39, P < 0.001) and 2.75 (95% CI 1.47 to 4.04, P < 0.001), respectively. Patients with occluded arteriovenous fistula also had lower serum creatinine levels versus those with patent arteriovenous fistula. The pooled mean difference was 0.10 (95% CI 0.04 to 0.17, P = 0.003). We saw no difference regarding ejection fraction between the groups. The pooled mean difference was 0.84 (95% CI -0.85 to 2.53, P = 0.33). Conclusions: Our meta-analysis demonstrated that arteriovenous fistula closure improves cardiac morphology and has better kidney graft function. Arteriovenous fistula closure may be considered in patients with a well-functioning allograft.

Abstract:Key Points: Surgical AVF ligation in KTRs is associated with a significant increase in diastolic BP while systolic BP remains stable.AVF closure in KTRs leads to an improvement of LV and LA morphology and a decrease in serum NT-proBNP levels.There is no significant effect of AVF ligation on kidney allograft function: The eGFR remains stable over time. Background: Kidney transplantation is considered as the best kidney replacement therapy, and arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis. The systematic ligation of a functioning AVF in stable kidney transplant recipients (KTRs) remains debatable. Methods: In this prospective study, we investigated the hemodynamic effect of the surgical closure of AVF in KTRs. Forty-three KTRs underwent an ambulatory BP monitoring before surgical closure of AVF (T0) and 12 months later (M12), as well as measurement of serum cardiac biomarkers (i.e., soluble suppression of tumorigenicity 2, N-terminal pro b-type natriuretic peptide [NT-proBNP], and galectin-3). Serum tests were also performed 6 months after AVF closure (M6). An echocardiographic examination was performed at each time point. All serum creatinine values were collected to compare the individual eGFR slopes before versus after AVF closure. The latest measure of the AVF flow before kidney transplantation was recorded. Results: Diastolic BP significantly rose from T0 to M12: +4.4±7.3 mm Hg (P = 0.0003) for 24h, +3.8±7.4 mm Hg (P = 0.0018) during the day, and +6.3±9.9 mm Hg (P = 0.0002) during the night, leading to an increased proportion of KTRs with European Society of Hypertension (ESH)-defined arterial hypertension after AVF ligation. No change was observed for systolic BP. NT-proBNP significantly dropped between T0 and M6 (345 [190; 553] to 230 [118; 458] pg/ml, P = 0.0001) and then remained stable from M6 to M12 while suppression of tumorigenicity 2 and galectin-3 levels did not change from T0 to M12. We observed a significant decrease in left ventricular (LV) end-diastolic volume, LV end-systolic volume, LV mass, interventricular septum diameter, left atrial volume, and tricuspid annular plane systolic excursion from T0 to M6 and then a stability from M6 to M12. LV ejection fraction and eGFR slope remained stable during the whole study. These observations remained unchanged after adjustment for AVF flow. Conclusion: The closure of a patent AVF in KTRs is associated with elevation of diastolic BP, drop in serum NT-proBNP levels, reduction of left ventricular and atrial dimensions, and stability of eGFR slope.
Management of Arteriovenous Fistula After Successful Kidney Transplantation in Long-Term Follow-Up

Abstract:Arteriovenous fistula (AVF) is the best method of vascular access for hemodialysis. This approach can lead to several complications, such as hyperkinetic heart failure due to a hyperfunctional AVF or dilatation of the feeding artery. These are late complications, especially in patients after a successful kidney transplantation. An observational study was performed focusing on patients more than 12 months after kidney transplantation. The AVF was evaluated by ultrasound and, if the outflow exceeded 1.5 L/min, an echocardiogram was performed. Surgical management was indicated if the cardiac index was higher than 3.9 L/min/m2 or upon finding a brachial artery aneurysm. A total of 208 post- kidney transplantation patients were examined over a 3-year period, of which 46 subjects (22.11%) had hyperfunctional AVF and 34 cases (16.34%) of feeding artery dilatation were determined. In total, 40 AVF flow reduction and 6 AVF ligation procedures were performed. The median AVF flow before and after the reduction was 2955 mL/min and 1060 mL/min, respectively. Primary patency after flow reduction was 88.3% at 12 months. Late AVF complications in patients following kidney transplantation are quite common. It is necessary to create a screening program to monitor AVFs in these patients.

Abstract:Purpose:In order to investigate the effects of repair of arteriovenous fistula on heart function soas to widen the range of the indication of allogenetic transplantation of kidney. Methods :Repair of arteriovenousfistula was performed in 8 patients who have received long term hemodialysis and then complicated with repeatedattack of heart failure 5 months before renal transplantation. Results:There are 7 patients had their cardiac cham-bers returned to normal, and 1 returned to normal upper limit. Renal transplantation was successfully performedin 6 patients, but death occurred in 1 case resulting from liver function failure. Superacute rejection and sponta-neous rupture of the kidney occurred in 1 patient. Conclusions:It is considered that the repair of arteriovenous fis-tula before renal transplantation is beneficial to improvement of heart function in these patients so as to widen therange of operative indications and create favourable condition for renal transplantation and decrease complica-tions. Color doppler sonography is of great value in monitoring pre-and postoperative heart function.

Abstract:Chronic renal failure in childhood is mostly caused by a congenital disorder or an acquired form of glomerulonephritis. We describe a case of a 13-year-old boy from Africa who presented with a cerebrovascular accident, malignant hypertension and renal insufficiency. Aetiological workup of his hypertension revealed underlying chronic renal failure due to histologically confirmed haemolytic uraemic syndrome. This case serves to remind clinicians of the serious complications of undiagnosed chronic renal failure in a child.

Abstract:The study includes the analysis of the results of using the different types of arteriovenous anastomoses in the formation of arteriovenous fistulas and their impact on fistula maturation in the early postoperative period. One of the main aspects of the work is to determine the optimal type of anastomosis to ensure the proper functioning of arteriovenous fistula that is an important element of vascular access in patients on renal replacement therapy. It is important to consider such factors as the diameter of formed arteriovenous fistula, the blood flow velocity through it, the incidence of complications and the duration of its functioning. The study also takes into account the impact of patients’ characteristics, such as age, condition of arteries and veins, as well as general health indicators on the results of surgery. Taking these factors into account allows to identify the groups of patients who can benefit most from a particular type of anastomosis. During the sample period, 384 patients with terminal stage of renal failure aged 20 to 84 years undergoing programmed hemodialysis, the native arteriovenous fistula was formed. It was found that 94.01 % of arteriovenous fistulas were successful in the early postoperative period. The average age of these patients was 44.2 years, while unsuccessful cases developed in people with an average age of 66.8 years. It was also found that within an hour after surgery for the formation of an arteriovenous fistula with the type end-to-side anastomosis, its diameter increases from 2.08±0.42 mm to 2.96±1.68 mm, and the blood flow velocity increases from 21.4±7.6 ml/min to 212.4±74.6 ml/min. The risk factor for effective fistula formation, in addition to age, is the diameter of vessels, anatomical features of the venous system, the presence of concomitant pathology (primarily diabetes mellitus). The obtained results can serve as a basis for the developing of recommendations for the optimal choice of anastomosis type for certain categories of patients who will undergo renal replacement therapy. The study is aimed at improving the results of surgical treatment and increasing the quality of life of patients with chronic renal failure.
Left ventricular structure and function in renal transplant patients after graft failure

Abstract:Objective To investigate the left ventricular structure and function in renal transplant patients after graft failure.Methods Thirty nine renal graft failure patients undergoing maintenance hemodialysis (graft failure group) and 46 nontransplant patients with end-stage renal disease undergoing hemodialysis (primary hemodialysis group) were enrolled in the study.The Doppler echocardiography and laboratory examinations were performed before primary dialysis.The laboratory parameters,left ventricular structure and function indexes were compared between the two groups.Results There were no significant differences in Hb,serum albumin,TC,TG,parathyroid hormone between the two groups (P ＞0.05).The serum creatinine,serum iron,total iron binding capacity and transferrin saturation in renal graft failure group were lower,and the C reactive protein was higher than those in primary hemodialysis group (P＜0.05).There were no significant differences between the two groups in left atrial diameter,left ventricular systolic and diastolic end diameter,left ventricular ejection fraction,and left ventricular short axis shortening rate (P ＞0.05).The ventricular septal end systolic thickness,ventricular septal end diastolic thickness,left ventricular posterior wall end systolic thickness,left ventricular posterior wall end diastolic thickness and left ventricular mass index in the renal graft failure group were higher than those in primary hemodialysis group (P＜0.05).The incidence of left ventricular hypertrophy was higher in the renal graft failure group (P＜0.05).There was no significant difference in the incidence of left ventricular systolic and diastolic dysfunction between the two groups(P ＞0.05).Conclusion The general state,cardiac structure and function in kidney-allograft-loss patients are poor,which may be associated with the use of immunosuppressive agents.
A Systematic Review Exploring the Impact of Arteriovenous Fistula Ligature on High-Output Heart Failure in Renal Transplant Recipients

Abstract:Background: Cardiovascular disease is the most common cause of death in renal transplant recipients (RTrs). High-output heart failure (HoHF) is a classic problem of RTrs with patent arteriovenous fistulae (AVF). Central to the entire discipline of transplant nephrology is the ligation of AVF in RTrs, with a patent AVF presenting with signs and symptoms of HoHF. AVF ligation has long been a topic of great interest in this population. To date, little attention has been paid to the effects of arteriovenous graft ligation on HoHF. This study systematically reviews the data for AVF ligation, aiming to provide its impact on HoHF in RTrs. Methods: The present study adopts the Preferred Reporting Items for Systematic Reviews and Meta-analysis 2020 guidelines. Published studies were identified using a search strategy in PubMed, Scopus, PubMed Central, Science Direct, and Medline. The primary inclusion criterion for this review was RTrs with a patent AVF who exhibited clinical or imaging findings of HoHF. Articles dating back to the last decade that involved the human species were included in our review, and the search was restricted to the English language. Studies involving both male and female genders and those describing the adult population (aged > 19 years) were also a part of our inclusion criteria. Results: After applying eligibility criteria, our electronic search yielded 1,461 articles. A total of 16 studies that involved 18,735 subjects were included in our review, which comprised 6 cohort studies, 4 case reports, 2 randomized control trials, 2 narrative reviews, 1 meta-analysis, and 1 case series. While the risk of bias of the narrative reviews was low, 1 of the randomized control trials had some overall concerns. The meta-analysis included in our review had moderate risk of bias, while 4 of the 6 cohort studies were of good quality. All of the case reports and series included in our review were of good quality. Of the 12 studies that reported genders, 10,949 were male and 6,416 were female. There was a notable reduction in left ventricular mass, left ventricular mass index, left ventricular end diastolic dimension, cardiac output, velocity index, and systemic vascular resistance index upon AVF ligation. Conclusions: A complete resolution of the clinical signs and symptoms of HoHF can be anticipated after AVF ligation in RTrs. Clinicians should always be on the lookout for signs and symptoms of cardiovascular decompensation in asymptomatic RTrs.

Abstract:There is currently a huge worldwide demand for donor kidneys for organ transplantation. Consequently, numerous marginal donor kidneys, such as kidneys with microthrombi, are used to save patients' lives. While some studies have shown an association between the presence of microthrombi in donor kidneys and an increased risk for delayed graft function (DGF) (McCall et al., 2003; Gao et al., 2019), other studies have demonstrated that microthrombi negatively impact the rate of DGF (Batra et al., 2016; Hansen et al., 2018), but not graft survival rate (McCall et al., 2003; Batra et al., 2016; Gao et al., 2019). In contrast, Hansen et al. (2018) concluded that fibrin thrombi were not only associated with reduced graft function six months post-transplantation but also with increased graft loss within the first year of transplantation. On the other hand, Batra et al. (2016) found no significant differences in the DGF rate or one-year graft function between recipients in diffuse and focal microthrombi groups. To date, however, the overall influence of donor kidney microthrombi and the degree of influence on prognosis remain controversial, necessitating further research.

Abstract:Background: Guidelines recommend placing native arteriovenous fistulas (AVFs) as far distally in the upper extremity as possible. If there are adequate veins and adequate arteries, a wrist fistula, which offers notably lower risks than grafts and catheters, would be the first choice for long-term hemodialysis. With increasing failure and difficulty to create wrist fistulas, we reviewed outcomes of the proximal radial AVF (PRAAVF) and demonstrate that it is an effective technique. Methods: A systemic literature research was conducted in PubMed and related bibliographies. The focus of data extraction was primary failure, primary patency rates, and secondary patency rates after 1 and 2 years. Estimates were pooled with the random effects model, and meta-regression and sensitivity analysis were performed to explore heterogeneity. Results: According to selection criteria formulated a priori, 10 articles (n [ 1310) were included and finally analyzed after screening 1687 articles. The pooled primary failure was 12.3% (95% confidence interval [ CI], 7.6%-17.0%; chi(2) = 70.8, I-2 = 87.3%), the primary patency, including primary failure, was 73.6% (95% CI, 52.4%-94.9%; chi(2) = 71.3, I-2 = 97.2%) at 1 year and 70.5% (95% CI, 50.6%-90.5%; chi(2) = 58.8, I-2 = 96.6%) at 2 years. Secondary patency was 80.0% (95% CI, 72.8%-87.2%; chi(2) = 24.42, I-2 = 75.4%) at 1 year and 73.7% (95% CI, 65.2%-82.2%; chi(2) = 28.51, I-2 = 79.0%) at 2 years. Individual variate meta-regression analysis found the definition of primary failure was a significant source of heterogeneity (P = .009). Steal syndromes developed in four of 832 (0.5%) of the PRAAVFs, and venous hypertension developed in four of 284 (1.4%). Conclusions: The PRAAVF presented low to moderate primary failure and high primary and secondary patency rates with acceptable complications. Consideration of the specific fistula is required when creating a vascular access, especially when a wrist fistula has failed or is predicted to be unsuccessful.
Tissue Ki67 proliferative index expression and pathological changes in hemodialysis arteriovenous fistulae: Preliminary single-center results

Abstract:Background: Arteriovenous fistula (AVF) for hemodialysis integrates outward remodeling with vessel wall thickening in response to drastic hemodynamic changes. Aim of this study is to determine the role of Ki67, a well-established proliferative marker, related to AVF, and its relationship with time-dependent histological morphologic changes. Materials and methods: All patients were enrolled in 1 year and stratified in two groups: (A) pre-dialysis patients submitted to first AVF and (B) patients submitted to revision of AVF. Morphological changes: neo-angiogenesis (NAG), myointimal thickening (MIT), inflammatory infiltrate (IT), and aneurysmatic fistula degeneration (AD). The time of AVF creation was recorded. A biopsy of native vein in Group A and of arterialized vein in Group B was submitted to histological and immunohistochemical (IHC) analysis. IHC for Ki67 was automatically performed in all specimens. Ki67 immunoreactivity was assessed as the mean number of positive cells on several high-power fields, counted in the hot spots. Results: A total of 138 patients were enrolled, 69 (50.0%) Group A and 69 (50.0%) Group B. No NAG or MIT were found in Group A. Seven (10.1%) Group A veins showed a mild MIT. Analyzing the Group B, a moderate-to-severe MIT was present in 35 (50.7%), IT in 19 (27.5%), NAG in 37 (53.6%); AD was present in 10 (14.5%). All AVF of Group B with the exception of one (1.4%) showed a positivity for Ki67, with a mean of 12.31 ± 13.79 positive cells/hot spot (range 0–65). Ki67-immunoreactive cells had a subendothelial localization in 23 (33.3%) cases, a myointimal localization in SMC in 35 (50.7%) cases. The number of positive cells was significantly correlated with subendothelial localization of Ki67 ( p = 0.001) and with NA ( p = 0.001). Conclusions: Native veins do not contain cycling cells. In contrast, vascular cell proliferation starts immediately after AVF creation and persists independently of the time the fistula is set up. The amount of proliferating cells is significantly associated with MIT and subendothelial localization of Ki67-immunoreactive cells, thus suggesting a role of Ki-67 index in predicting AVF failure.
The Role of Cardio-Renal Inflammation in Deciding the Fate of the Arteriovenous Fistula in Haemodialysis Therapy

Abstract:Vascular access is an indispensable component of haemodialysis therapy for end-stage kidney disease patients. The arteriovenous fistula (AVF) is most common, but importantly, two-year failure rates are greater than fifty percent. AVF failure can occur due to a lack of suitable vascular remodelling, and inappropriate inflammation preventing maturation, or alternatively neointimal hyperplasia and vascular stenosis preventing long-term use. A comprehensive mechanistic understanding of these processes is still lacking, but recent studies highlight an essential role for inflammation from uraemia and the AVF itself. Inflammation affects each cell in the cascade of AVF failure, the endothelium, the infiltrating immune cells, and the vascular smooth muscle cells. This review examines the role of inflammation in each cell step by step and the influence on AVF failure. Inflammation resulting in AVF failure occurs initially via changes in endothelial cell activation, permeability, and vasoprotective chemokine secretion. Resultingly, immune cells can extravasate into the subendothelial space to release inflammatory cytokines and cause other deleterious changes to the microenvironment. Finally, all these changes modify vascular smooth muscle cell function, resulting in excessive and unchecked hyperplasia and proliferation, eventually leading to stenosis and the failure of the AVF. Finally, the emerging therapeutic options based off these findings are discussed, including mesenchymal stem cells, small-molecule inhibitors, and far-infrared therapies. Recent years have clearly demonstrated a vital role for inflammation in deciding the fate of the AVF, and future works must be centred on this to develop therapies for a hitherto unacceptably underserved patient population.

Abstract:Abstract Background The hemodynamic effects of a functioning haemodialysis arteriovenous fistula (AVF) can cause or exacerbate heart failure (HF). We investigated whether the presence of an AVF at time of kidney transplant (KT) is associated with de novo HF. Methods This was an observational cohort study including adult patients who received a KT in the West of Scotland between 2010 and 2020. We evaluated the risk and associations of pretransplant factors with de novo HF, alone and as a composite cardiovascular (CV) outcome (including non-fatal myocardial infarction (MI), non-fatal stroke, de novo HF, and CV death). Multivariable proportional hazard regression and sensitivity analyses were used to identify independent correlates of the outcomes. Results Among 1 330 included patients, the incident rate of de novo HF after transplantation was 58 (50–67) per 1 000 person-years in AVF patients (n = 716) compared to 33 (27–41) per 1 000 person-years in non-AVF patients (n = 614). De novo HF was associated with the presence of an AVF (adjusted hazard ratio [aHR] 2.14; 1.40–3.26), duration of dialysis (aHR 1.03 per year increase; 1.01–1.04), age at transplant (aHR 1.03 per year increase; 1.02–1.05), female sex (aHR 1.93; 1.40–2.65), and pretransplant diabetes (aHR 2.43; 1.48–4.01). The presence of an AVF was also associated with the composite CV outcome (aHR 1.91; 1.31–2.78). Conclusions The presence of an AVF may be an under-recognised modifiable predictor of de novo HF posttransplantation.

Abstract:Objective To investigate the autogenous arteriovenous fistula in patients of hemodialysis (AVF) effect of diameter on blood pressure and heart function and correlation.Methods 60 patients received AVF production in hemodialysis patients as the research object,according to the postoperative color Doppler ultrasound measured diameter (AVFD),the patients were divided into A group of 20 cases (diameter of 2.1-3.0 mm);B group of 20 cases (diameter 3.1-4.0 mm);20 cases in group C (diameter of 4.1-5.0 mm).Surgical patients before and after heart color Doppler ultrasound,observed and strictly record blood pressure (BP),heart rate (HR),cardiac output (CO),cardiac index (CI),ejection fraction (EF),arteriovenous fistula blood flow (AVFB),analysis of 3 different effects of AVFD on the blood pressure and heart function in patients with hemodialysis and correlation.Results 3 groups of patients with AVF after BP,HR,CO,CI,EF values increased compared with the preoperative (P ＜ 0.05 or P ＜ 0.01).3 groups of patients with AVFB,AVFD and CO difference (ΔCO),CI difference (Δ CI) had a positive linear correlation (r =0.518,P ＜ 0.05).A,B,C and 3 in group AVFB,Δ CO,Δ CI as AVFD increases,and ask each significant differences (P ＜ 0.05).Conclusion The changes of AVF in hemodialysis patients fistula orifice diameter and blood pressure,cardiac function is closely related to the appropriate diameter of the fistula orifice for reducing the effect of AVF on blood pressure and cardiac function,to maintain long-term patency crucial AVF flow.
Association Between Attempted Arteriovenous Fistula Creation and Mortality in People Starting Hemodialysis via a Catheter: A Multicenter, Retrospective Cohort Study

Abstract:Background: In North America, most people start hemodialysis via a central venous catheter (“catheter”). These patients are counseled to undergo arteriovenous fistula (“fistula”) creation within weeks of starting hemodialysis because fistulas are associated with improved survival. Objectives: To determine whether attempting to create a fistula in patients who start hemodialysis via a catheter is associated with improved mortality. We also sought to determine whether differences in baseline patient characteristics, vascular procedures for access-related complications, or days in hospital may confound or mediate the relationship between attempted fistula creation and mortality. Design: Multicenter, retrospective cohort study. Setting: Six dialysis programs located in Ontario, Alberta, and Manitoba. Patients: Patients aged ≥18 years who initiated hemodialysis via a catheter between January 1, 2004, and May 31, 2012, who had not had a previous attempt at fistula creation. We excluded those who had a life expectancy less than 1 year, who transitioned to peritoneal dialysis within 6 months of starting dialysis, and people who started hemodialysis via a graft. Measurements: Attempted fistula creation, all-cause mortality, patient characteristics and comorbidities, vascular procedures for access-related complications, and days spent in hospital. Methods: We used survival methods, including marginal structural models, to account for immortal time bias and time-varying confounding. Results: In total, 1832 patients initiated hemodialysis via a catheter during the study period and met inclusion criteria. Of these patients, 565 (31%) underwent an attempt at fistula creation following hemodialysis start. As compared to those who did not receive a fistula attempt, these people were younger, had fewer comorbidities, and were more likely to have started dialysis as an outpatient and to have received pre-dialysis care. In a marginal structural model controlling for baseline characteristics and comorbidities, attempted fistula creation was associated with a significantly lower mortality (hazard ratio [HR] = 0.53; 95% confidence interval [CI] = 0.43-0.66). This effect did not appear to be confounded or mediated by differences in the number of days spent in hospital or vascular procedures for access-related complications. It also remained similar in analyses restricted to patients who survived at least 6 months (HR = 0.60; 95% CI = 0.47-0.77) and to patients who started hemodialysis as an outpatient (HR = 0.48; 95% CI = 0.33-0.68). Limitations: There is likely residual confounding and treatment selection bias. Conclusions: In this multicenter cohort study, attempting fistula creation in people who started hemodialysis via a catheter was associated with significantly reduced mortality. This reduction in mortality could not be explained by differences in patient characteristics or comorbidities, days spent in hospital, or vascular procedures for access-related complications. Residual confounding or selection bias may explain the observed benefits of fistulas for hemodialysis access. Trial Registration: Not applicable (cohort study).
Echocardiographic assessment of cardiac performance in patients with arteriovenous fistulas

Abstract:This noninvasive echocardiographic study of cardiac performance in 27 patients with chronic renal failure and vascular access documented the magnitude of forward cardiac output to be 65 per cent of the observed index. Hemodialysis resulted in acute hemodynamic changes, including an average fall in the cardiac index of 57 per cent, presumably because of a 40 per cent decrease in left ventricular filling volume. In patients in whom the vascular access had functioned longest, a time related cardiac decompensation is suggested by a cardiac index of significantly less than normal, p, 0.0013, with occlusion. Ventricular function was suboptimal three months post-transplant, as reflected by a continued low cardiac index and depressed ejection fraction, although the general reduction in cardiac index and diastolic volume on postoperative day 7 was of the magnitude expected from arteriovenous access occlusion.
Effects of accessory vein on the early fistula failure of forearm arteriovenous fistulas

Abstract:Objective To investigate the effect of accessory vein on the early failure of forearm arteriovenous fistula.Methods A total of 38 uremia patients who underwent arteriovenous fistula surgery for the first time were randomly divided into experimental group (n=19) and control group (n=19).During the surgery,the accessory vein was blocked by thread ligation in the experimental group,and had no treatment in the control group.Both groups received standard end-to-side arteriovenous anastomosis in forearm.After operation for 8 and 20 weeks,the effects of the two surgical methods on blood flow and early fistula failure were compared.Results After operation for 8 weeks,blood flow rate was higher in the experimental group than in the control group (t=3.402,P=0.002);early fistula failure ratio was lower in the experimental group than in the control group (x2=4.502,P=0.034).After operation for 20 weeks,blood flow rate was still higher in the experimental group than in the control group (t=3.102,P=0.004),and early fistula failure ratio was lower in the experimental group than in the control group (x2=5.246,P=0.022).Conclusion The presence of the accessory vein can decrease the blood flow in arteriovenous fistula and increase the ratio of early fistula failure.The treatment of the accessory vein is important for the forearm arteriovenous fistula.
Effect of Anticoagulant and Antiplatelet Drugs on Outcome of Arterio-Venous Fistula Created for Hemodialysis Access

Abstract:Background: Arteriovenous fistula (AVF) creation is the primary mode of achieving vascular access for hemodialysis in chronic renal failure (CRF). Because of high complication rates like thrombosis, maintenance of the fistula is a major challenge. Although antiplatelet and anticoagulant drugs are emerging rapidly for improving the outcome of AVF but fear of bleeding, hematoma, subsequent compression of AV fistula and blockade restrict their use in many dialysis centers. Objective: To see the effect of anticoagulant & antiplatelet drugs in improving primary patency rate and reducing early thrombus formation of AVF. Methods: This clinical trial was conducted in the Department of Urology Bangabandhu Sheikh Mujib Medical University, Dhaka from November, 2018 to October, 2019. Ninety six ESRD patients who underwent AV fistula with immediate postoperative had thrill and murmur were included as study population as per selection criteria. Informed written consent was taken from all the patients. Patients were divided into two groups using computer generated random number (group I and group II). In group I, patients anticoagulation and antiplatelet drugs were used and in group II, patients anticoagulation and antiplatelet drugs were not used after creation of AVF. Patients were followed up on 3rd POD, at 2 weeks and at 4 weeks after the operation and re-evaluated with clinical examinations and investigations. The outcome were evaluated and compared between two groups. Results: In this study maximum patients were more than 40 years old in both groups. Mean age was 47.89 ± 12.54 years in group I and 46.29 ± 14.78 in group II. Males were predominant in both groups. Male to female ratio was 3.36:1 and 5.8:1 in group I and group II respectively in this study. In this study 35.4% in Group I and 45.8% in Group II patients had hypertension. 47.9% in Group I and 60.4% in Group II patients had DM. Thrombosis was significantly lower in study group (group I) than control group (group II). Primary patency was significantly higher at 4th week after treatment in group I than group II. Bleeding was observed in one (2.1%) patients at 1rd POD in group I who came with emergency & managed conservatively. Haematoma was observed in 1 (2.1%) patients at 3rd POD in group I. Conclusion: Use of antiplatelet and anticoagulant agents prevented the loss of AV patency. But it still needs to be determined whether the potential benefits of anticoagulant & antiplatelet therapy outweights the risk of adverse events. Bangladesh J. Urol. 2021; 24(1): 90-98

Predictors associated with early and late restenosis of arteriovenous fistulas and grafts after percutaneous transluminal angiography

Abstract:BACKGROUND: The objective of this study was to assess postintervention patency and analyze the predictive factors associated with early and late restenosis after intervention in hemodialysis arteriovenous fistulas (AVF) and arteriovenous grafts (AVG).METHODS: This study retrospectively analyzed 284 hemodialysis patients who underwent percutaneous transluminal angioplasty (PTA) due to AVF and AVG stenosis. A total of 350 PTA procedures were performed. Clinical, anatomical, biochemical, and technical variables were analyzed. Using univariate and multivariate analyses, we assessed the postintervention patency of PTA by follow-up, and identified the predictive factors taking into account competing risks.RESULTS: Postintervention patency rates at 3, 6, 12, and 24 months were 86.5%, 66.4%, 42.6%, and 29.8%, respectively, with a median patency duration of 11±0.71 months. Kaplan-Meier analysis showed that the patency rate of the AVF group (n=271) was dramatically higher than the AVG group (n=79) at 3, 6, and 12 months after PTA, respectively (88.9% vs. 78.5%, 69.0% vs. 57.4%, 48.8% vs. 20.0%, P<0.01). Cox survival analysis revealed that the factors associated with postintervention patency of AVF included age of fistulas, serum albumin (ALB) levels, location of stenoses, lesion length longer than 2 cm, multiple stenoses, and maximal pressure of dilatation lower than 16 atm. In addition, factors related to postintervention patency of AVG included the presence of diabetes and hypertension, and serum ALB.CONCLUSIONS: This study demonstrated that the risk factors associated with postintervention patency of AVF included age of fistulas, lower levels of serum ALB, location of stenoses, lesion length longer than 2 cm, multiple stenoses, and maximal pressure of dilatation lower than 16 atm. In addition, risk factors related to postintervention patency of AVG included the presence of diabetes and lower levels of serum ALB, while the presence of hypertension was found to be a protective factor for reducing patency loss of AVG. Among all these factors, serum ALB and multiple stenoses tended to predict early restenosis, while pressure of dilatation tended to predict late restenosis.

Abstract:This study aimed to investigate ultrasound features of arteriovenous fistula stenosis and their relationship with primary patency after percutaneous transluminal angioplasty (post-intervention primary patency) and compare this classification with that using lesion location. Hemodialysis patients who underwent ultrasound-guided percutaneous transluminal angioplasty for arteriovenous fistula stenosis from July 2020 to December 2021 were retrospectively evaluated. Lesions (excluding inflow arteries) were categorized into five groups based on ultrasound features, and the clinical characteristics and risk factors affecting the post-intervention primary patency of the arteriovenous fistula were analyzed. Among 185 patients, 100 (54.05%), 36 (19.46%), 22 (11.89%), 11 (5.95%), and 16 (8.65%) were classified into the intima-dominant, non-intima-dominant, valve obstruction, vascular calcification, and mixed groups, respectively. The dialysis duration and arteriovenous fistula use time were the highest in the vascular calcification group at 86 (interquartile range: 49–140) and 77 (interquartile range: 49–110) months, respectively. Diabetes mellitus was most common in the intima-dominant group (42.0%). In Kaplan–Meier and univariate Cox analysis, type III lesion location (stenosis in the venous confluence site) was associated with the lower post-intervention primary patency. In the multivariate Cox analysis, percutaneous transluminal angioplasty times (the number of times patients were treated with percutaneous transluminal angioplasty for arteriovenous fistula stenosis dysfunction), vascular calcification, calcification at the lesion site requiring percutaneous transluminal angioplasty, and serum parathyroid hormone levels were independent risk factors for post-intervention primary patency. Ultrasound features showed that calcification of the arteriovenous fistula was detrimental to the post-intervention primary patency of arteriovenous fistula.
