Abstract:BACKGROUND: In Latin America, community-acquired pneumonia remains a major cause of morbidity and mortality among children. Few studies have examined the etiology of pneumonia in Ecuador.METHODS: This observational study was part of a randomized, double blind, placebo-controlled clinical trial conducted among children aged 2-59 months with severe pneumonia in Quito, Ecuador. Nasopharyngeal and blood samples were tested for bacterial and viral etiology by polymerase chain reaction. Risk factors for specific respiratory pathogens were also evaluated.RESULTS: Among 406 children tested, 159 (39.2%) had respiratory syncytial virus (RSV), 71 (17.5%) had human metapneumovirus (hMPV), and 62 (15.3%) had adenovirus. Streptococcus pneumoniae was identified in 37 (9.2%) samples and Mycoplasma pneumoniae in three (0.74%) samples. The yearly circulation pattern of RSV (P = 0.0003) overlapped with S. pneumoniae, (P = 0.03) with most cases occurring in the rainy season. In multivariable analysis, risk factors for RSV included younger age (adjusted odds ratio [aOR] = 1.9, P = 0.01) and being underweight (aOR = 1.8, P = 0.04). Maternal education (aOR = 0.82, P = 0.003), pulse oximetry (aOR = 0.93, P = 0.005), and rales (aOR = 0.25, P = 0.007) were associated with influenza A. Younger age (aOR = 3.5, P = 0.007) and elevated baseline respiratory rate were associated with HPIV-3 infection (aOR = 0.94, P = 0.03).CONCLUSION: These results indicate the importance of RSV and influenza, and potentially modifiable risk factors including undernutrition and future use of a RSV vaccine, when an effective vaccine becomes available.TRIAL REGISTRATION: ClinicalTrials.gov NCT 00513929.

Influenza and Other Respiratory Viruses

Abstract:Background Pediatric pneumonia remains a significant health challenge, while the viral risk factors for adverse outcomes in pediatric pneumonia are not yet fully clear. Methods A matched case-control study of pediatric patients with pneumonia was carried out in Beijing, China, between 2007 and 2015. The study enrolled 334 intensive care unit patients who developed life-threatening diseases and 522 controls matched to the sex, age, ethnicity, admission dates, and residing district of the cases suffered from pneumonia. Nasopharyngeal aspirates were taken from all participants and tested by PCR for 18 common respiratory viruses. Results At least, one virus was detected in 257 (77%) of the cases and 409 (78%) of the controls. We observed no difference in the prevalence of 17 respiratory viruses between cases and controls but found a higher frequency of influenza A virus (IFV-A) in the cases than in the controls (7% vs 4%, P = .036). After adjusting for comorbid conditions and a history of reactive airway diseases, IFV-A was associated with an increase in life-threatening pneumonia (adjusted odds ratio = 2.55, 95% CI = 1.24-5.24). Young age and congenital heart disease (aOR = 10.16-10.27, P < .001) were also independent risk factors. Conclusions The prevention of IFV infection is critical in decreasing the risk of life-threatening pneumonia in children.
Etiology and the challenge of diagnostic testing of community-acquired pneumonia in children and adolescents

Abstract:Abstract Background Pneumonia is the leading cause of mortality in pediatric population. The etiology of pneumonia in this population is variable and changes according to age and disease severity and where the study is conducted. Our aim was to determine the etiology of community-acquired pneumonia (CAP) in children aged 1 month to 17 years admitted to 13 Colombian hospitals. Methods Prospective cohort study. Hospitalized children with radiologically confirmed CAP and ≤ 15 days of symptoms were included and followed together with a control group. Induced sputum (IS) was submitted for stains and cultures for pyogenic bacteria and Mycobacterium tuberculosis , and multiplex PCR (mPCR) for bacteria and viruses; urinary antigens for pneumococcus and Legionella pneumophila ; nasopharyngeal swabs for viruses, and paired serology for atypical bacteria and viruses. Additional cultures were taken at the discretion of primary care pediatricians. Results Among 525 children with CAP, 71.6% had non-severe pneumonia; 24.8% severe and 3.6% very severe pneumonia, and no fatal cases. At least one microorganism was identified in 84% of children and 61% were of mixed etiology; 72% had at least one respiratory virus, 28% pyogenic bacteria and 21% atypical bacteria. Respiratory syncytial virus, Parainfluenza, Rhinovirus, Influenza, Mycoplasma pneumoniae , Adenovirus and Streptococcus pneumoniae were the most common etiologies of CAP. Respiratory syncytial virus was more frequent in children under 2 years and in severe pneumonia. Tuberculosis was diagnosed in 2.3% of children. IS was the most useful specimen to identify the etiology (33.6%), and blood cultures were positive in 3.6%. The concordance between all available diagnostic tests was low. A high percentage of healthy children were colonized by S. pneumoniae and Haemophilus influenzae , or were infected by Parainfluenza, Rhinovirus, Influenza and Adenovirus. Conclusions Respiratory viruses are the most frequent etiology of CAP in children and adolescents, in particular in those under 5 years. This study shows the challenges in making an etiologic diagnosis of CAP in pediatric population because of the poor concordance between tests and the high percentage of multiple microorganisms in healthy children. IS is useful for CAP diagnosis in pediatric population.

Abstract:BACKGROUND: In 1994, Uruguay included Haemophilus influenzae b (Hib) conjugated vaccine in a 3 + 1 schedule. In March 2008, 7-valent pneumococcal conjugate vaccines (PCV7) was included in a 2 +1 schedule. In 2010, 13-valent PCV replaced PCV7. Catch-up immunization was offered. The aim of this study was to describe the etiology of community-acquired pneumonia (CAP) in children 0-14 years of age hospitalized at the Hospital Pediatrico-Centro Hospitalario Pereira Rossell between 2003 and 2012.METHODS: Annual hospitalization rates (per 10,000 discharges) for CAP and bacterial-confirmed CAP in children 0-14 years of age was described prior PCV7 vaccination (2003-2007), during the year of implementation of PCV7 (2008) and after the introduction of PCV7 (2009-2012). Data regarding age, strains isolated from pleural fluid and/or blood, vaccination status, pneumococcal and H. influenzae serotypes were obtained from Hospital Pediatrico-Centro Hospitalario Pereira Rossell databases and vaccination records.RESULTS: Hospitalization rates for CAP and pneumococcal CAP between prevaccine years and the last year after introduction of vaccination with PCV (2012) significantly decreased by 78.1% and 92.4%, respectively. Significant reduction for 13-valent PCV vaccine serotypes and significant increase for nonvaccine serotypes was observed. A decrease in Staphylococcus aureus pneumonia was observed. Hospitalization rates for H. influenzae CAP remain stable before and after pneumococcal vaccination.CONCLUSIONS: Three years after PCV7/13 introduction into the routine vaccination schedule, there was a rapid and significant reduction in rates of CAP and P-CAP. An increase of etiology of CAP by other agents was not observed.

Abstract:Community-acquired pneumonia remains a leading cause of hospitalizations among children worldwide. The diagnosis is based on the history, the physical examination results in children with fever plus respiratory signs and symptoms, and chest radiography. The microbiological etiology is confirmed by viral testing and hemocultures. The most likely etiology depends on the age of the child. The features of childhood pneumonia vary between countries and territories. The purpose of this study was to describe the epidemiological characteristics and current microbial ecology of community-acquired pneumonia in children in French Guiana. We performed a retrospective, descriptive, and monocentric study between January 1, 2015, and December 31, 2017, in the pediatric ward of the Cayenne Hospital in French Guiana. The studied population was aged from 0 to 15 years and 3 months and hospitalized for acute community-acquired pneumonia. A total of 415 patients (mean age 3.62 years) were included. A pathogen was identifiable in 22.4% of cases, including bacteria in 61.3%, viruses in 43%, and coinfections in 14%. The main pathogens were respiratory syncytial virus (31.2%), Streptococcus pneumoniae (20.4%), Haemophilus influenzae (11.8%), and Mycoplasma pneumoniae (10.8%). The burden of hospitalization for children with community-acquired pneumonia was highest among less than 2 years, in whom respiratory viruses were the most commonly detected causes of pneumonia. The share of vaccine-preventable diseases (S. pneumoniae, H. influenzae, and influenza) remains high. With the vaccination requirement imposed since 1 January 2018 against pneumococcus, Haemophilus influenzae, and whooping cough and the possibility of practicing multiplex PCR in our hospital, it will be interesting to study the impact of this law enforcement on new child generations and compare these new data to our study.
Blood culture and respiratory syncytial virus identification in acute lower respiratory tract infection

Abstract:Even though the incidence of pneumonia in developed and developing countries is similar, the mortality is five times higher in developing countries. This study aimed to determine the prevalence of bacteremia in children with acute lower respiratory tract infection (LRTI) and relative contribution of respiratory syncytial virus (RSV). One hundred and one children under five years of age who attended a primary care level clinic with diagnosis of acute LRTI, were enrolled. Diagnosis and management of pneumonia were done according to the WHO guidelines. Two blood cultures were drawn at the time of admission. A nasopharyngeal sample was taken for detection of RSV by indirect immunofluorescence. Blood cultures were positive for pathogenic bacteria (Streptococcus pneumoniae, Haemophilus influenzae and Staphylococcus aureus) in three patients. The detection for RSV was positive in 24 patients (23.7%). The clinical and radiographic presentations were not significantly different between patients with and without RSV (p > 0.05). RSV is a common cause of LRTI in children younger than five years old. Blood cultures are not commonly positive in outpatients with acute LRTI. The practice of obtaining blood cultures in primary and secondary care clinics is not useful to guide the treatment of patients with community-acquired pneumonia.
Pneumonia in childhood bacterial meningitis–Experience from three continents

Abstract:Introduction Although concomitant pneumonia is sometimes diagnosed in childhood bacterial meningitis, its role in the illness course and prognosis is not known. We examined these associations using prospectively collected data from Finland, Latin America and Angola. Methods This was a secondary descriptive analysis of prospectively collected data (clinical and laboratory findings at admission, during hospitalisation and outcome) from five clinical bacterial meningitis trials. We included children aged 2 months to 15 years from sites with confirmed bacterial meningitis and potential concomitant pneumonia (diagnosed clinically with or without a chest radiograph). Results Pneumonia was not observed in the 341 children included in Finland. Pneumonia was observed in 8% (51/606) of children in Latin America and in 46% (377/819) in Angola (p 3 days and non‐meningococcal meningitis. Concomitant pneumonia increased the severity of the disease and disabling sequelae. Conclusion Bacterial meningitis with pneumonia is a major, previously undescribed entity of severe bacterial meningitis, especially in Angola.

Abstract:A new time-averaged frame subtraction technique is introduced for vibration analysis by digital speckle shearing interferometry. The technique permits the enhancement of fringes by subtracting two Bessel fringe patterns at different forcing levels. Compared with the phase-shift method, this method is more efficient and easier to implement for qualitative vibration measurement, providing a means for fast inspection of plate vibration behavior. It is also capable of tracing contours of zero strain and locating antinodes on vibrating plates.
Mixed infections in children with Mycoplasma pneumonial pneumonia

Abstract:Objective To investigate the incidence and clinical features of mixed infections in children with Mycoplasma pneumoniae (MP) pneumonia.Method A total of 201 cases diagnosed as MP pneumonia were investigated for mixed infections by sputum bacterial culture,respiratory virus antigen detection and serum Chlamydia pneumoniae antibody test.For those with the indications for bronchoscopy,we also did bronchoalveolar lavage and lavage bacterial culture. Result A high incidence ( 103/201,51.2％ ) of mixed infections in children with MP pneumonia was revealed. The most frequent co-infected pathogen was Chlamydia pneumoniae (52,25.9％),followed by viruses ( 29,14.4％ ),and bacteria( 22,10.9％ ). Among viruses,respiratory syncytial virus was the most common (17,8.5％),followed by adenovirus( 6,3.0％ ),parainfluenza virus type Ⅲ (4,2.0％ ) and influenza virus type B ( 2,1.0％ ).Sputum bacterial culture was positive in 14/201 (7.0％) cases,Streptococcus pneumonia being most common(6,3.0％ ).BALF culture yielded positive results in 11.6％ (8/69),Streptococcus pneumonia was also common(5,7.3％ ).Among 29 cases with MP and virus coinfection,26 were younger than 3 years (89.7％ ),while for MP and Chlamydia pneumoniae coinfection,most of them were older than 3 years (40/52,76.9％ ). Compared with non-mixed infections,those with mixed infections had longer fever duration (24.5％ and 40.8％ longer than 10 d),more frequently developed pleural effusion ( 11.2％,23.3％ ) and large area of shadow in chest imaging (35.7％,51.5％ ).White blood cell[ ( 14.28 ±4.99) × 109/L],Creactive protein (CRP) [69(32.5-99.5) mg/L] and neutrophil ratio in BALF[0.86(0.63-0.91 ) ] were much higher in children with mixed bacterial infections than that in non-mixed infections [ (9.06 ± 3.47) ×109/L,3 (0-31.0) mg/L,0.44 (0.03-0.88 ) ].But no significant difference was found in peripheral blood neutrophil proportion between mixed bacterial infections (0.38 ± 0.25 ) and non-mixed infections(0.51 ±0.19).Conclusion More than half of cases with MP pneumonia had mixed infections,most commonly caused by Chlamydia pneumonia followed by viruses.The incidence of mixed infections with bacteria was low.Mixed infections with virus were more common in young children,while mixed infection with Chlamydia pneumoniae was more common in older ones.Bacterial infections should be paid more attention,especially those caused by Streptococcus pneumoniae,for those with high peripheral white blood cell counts,high CRP levels and high proportion of neutrophils in BALF.
Etiology of pneumonia, sepsis and meningitis in infants younger than three months of age in Ethiopia

Abstract:Methods: Within a multicenter study coordinated by WHO, an investigation of the etiologic agents of pneumonia, sepsis and meningitis was performed among infants younger than 3 months of age seen at the Ethio-Swedish Children's Hospital in Addis Ababa for a period of 2 years. Of the 816 infants enrolled 405 had clinical indications for investigation. Results: There were a total of 41 isolates from blood cultures from 40 infants. The study showed that the traditionally known acute respiratory infection pathogen Streptococcus pneumoniae was most common in this extended neonatal age group, found in 10 of 41 blood isolates. Streptococcus pyogenes was a common pathogen in this setting (9 of 41 blood isolates), whereas Salmonella group B was found in 5 of 41 isolates. Streptococcus agalactiae, which is a common pathogen in developed countries, was absent. A study of the susceptibility pattern of these organisms suggests that a combination of ampicillin with an aminoglycoside is adequate for initial treatment of these serious bacterial infections, but the combination is not optimal for the treatment of Salmonella infections. Among 202 infants on whom immunofluorescent antibody studies for viruses were performed based on nasopharyngeal aspirates, respiratory syncytial virus was found in 57 (28%) infants, and Chlamydia trachomatis was isolated in 32 (15.8%) of 203 infants.
Aetiological agents of pneumonia among HIV and non-HIV infected children in Ghana: A case-control study

Abstract:Pneumonia is the leading cause of death in children, however, the microbial aetiology of pneumonia is not well elucidated in low- and middle-income countries. Our study was aimed at determining the microbial aetiologies of childhood pneumonia and associated risk factors in HIV and non-HIV infected children. We conducted a case-control study that enrolled children with pneumonia as cases and non-pneumonia as controls from July 2017 to May 2020. Induced sputum and blood samples were investigated for microbial organisms using standard microbiological techniques. DNA/RNA was extracted from sputum samples and tested for viral and bacterial agents. Four hundred and four (404) subjects consisting of 231 (57.2%) cases and 173 (42.8%) controls were enrolled. We identified a significant (p = 0.011) proportion of viruses in cases (125; 54.1%, 95%CI: 47.4–60.7) than controls (71; 33.6%, 95%CI: 33.6–48.8) and these were mostly contributed to by Respiratory Syncytial Virus. Staphylococcus aureus (16; 4.0%), Klebsiella spp. (15, 3.7%) and Streptococcus pneumoniae (8, 2.0%) were the main bacterial agents identified in sputum or induced sputum samples. HIV infected children with viral-bacterial co-detection were found to have very severe pneumonia compared to those with only viral or bacterial infection. Indoor cooking (OR = 2.36; 95%CI:1.41–3.96) was found to be associated with pneumonia risk in patients. This study demonstrates the importance of various microbial pathogens, particularly RSV, in contributing to pneumonia in HIV and non-HIV paediatric populations. There is a need to accelerate clinical trials of RSV vaccines in African populations to support improvement of patient care.

Abstract:OBJECTIVE: To determine the prevalence of viral and atypical bacteria Mycoplasma pneumoniae infection in children experiencing asthma exacerbation and compare positive and negative subjects with regard to exacerbation severity, need for hospitalization, and treatment.METHODS: One hundred sixty-nine asthmatic children aged 2-15 years old who were admitted to emergency rooms in Bogota, Colombia for acute asthma exacerbation were interviewed. Nasopharyngeal aspirates were taken for DNA and RNA extraction. M. pneumoniae and virus were detected by PCR using specific primers.RESULTS: The prevalence of M. pneumoniae and viral infection in the study population was 12.4% and 83.7%, respectively. All subjects positive for M. pneumoniae were also positive for viral infection. Rhinovirus was the most frequently detected viral agent. No significant differences in severity of asthma exacerbations or in need for hospitalization between the virus or M. pneumoniae positive and negative groups were observed. A significantly lower percentage of M. pneumoniae positive subjects had used inhaled steroids over the six months prior to asthma exacerbation compared to M. pneumoniae negative subjects (38.1% vs. 68.2%), suggesting that inhaled corticosteroids may have a protective effect against M. pneumoniae infections.CONCLUSIONS: The M. pneumoniae and virus prevalence found in this study were similar to those described in the literature. The 100% co-infection rate observed suggests that viral infection can predispose patients to M. pneumoniae infection, and that this interaction may trigger asthmatic exacerbation. Further studies should be done to confirm the protective effect of inhaled corticosteroids on M. pneumoniae infection in patients with asthma exacerbations.

Abstract:BACKGROUND Respiratory syncytial virus is the leading cause of acute lower respiratory infection in children. We aimed to describe the clinical-epidemiologic pattern and risk factors for mortality associated with RSV infection. METHODS This is a prospective, cross-sectional study of acute lower respiratory infection in children admitted to the Children's Hospital during 2000 to 2017. Viral diagnosis was made by fluorescent antibody techniques or real-time-polymerase chain reaction. We compared clinical-epidemiologic characteristics of RSV infection in nonfatal versus fatal cases. Multiple logistic regression was used to identify independent predictors of mortality. RESULTS Of 15,451 patients with acute lower respiratory infection, 13,033 were tested for respiratory viruses and 5831 (45%) were positive: RSV 81.3% (4738), influenza 7.6% (440), parainfluenza 6.9% (402) and adenovirus 4.3% (251). RSV had a seasonal epidemic pattern coinciding with months of lowest average temperature. RSV cases show a case fatality rate of 1.7% (82/4687). Fatal cases had a higher proportion of prematurity (P < 0.01), perinatal respiratory history (P < 0.01), malnourishment (P < 0.01), congenital heart disease (P < 0.01), chronic neurologic disease (P < 0.01) and pneumonia at clinical presentation (P = 0.014). No significant difference between genders was observed. Most deaths occurred among children who had complications: respiratory distress (80.5%), nosocomial infections (45.7%), sepsis (31.7%) and atelectasis (13.4%). Independent predictors of RSV mortality were moderate-to-severe malnourishment, odds ratio (OR): 3.69 [95% confidence interval (CI): 1.98-6.87; P < 0.0001]; chronic neurologic disease, OR: 4.14 (95% CI: 2.12-8.08; P < 0.0001); congenital heart disease, OR: 4.18 (95% CI: 2.39-7.32; P< 0.0001); and the age less than 6 months, OR: 1.99 (95% CI: 1.24-3.18; P = 0.004). CONCLUSIONS RSV showed an epidemic pattern affecting mostly young children. Malnourishment, chronic neurologic disease, congenital heart disease and the age less than 6 months were the independent risk factors for RSV mortality.

Etiology and Clinical Epidemiology of Children with Acute Pneumonia in Shanghai Area

Abstract:Objective: To investigate the etiology and the characteristics of clinical epidemiology of acute pneumonia in children. Methods; Sputum specimens were collected from 724 children with acute pneumonia by nasotracheal aspiration and detection of pathogens by monoclonal antibody bridge linked enzyme assay, sputum culture and polymerase chain reaction (PCR) technique. Results; Microbial etiology were confirmed in 441 (60. 9% ) of 724 patients, among them viral infections were 160 cases (36. 3%), bacterial infections 125 cases (28. 3%), Mycoplasma pneumoniae 81 cases (18. 4%) and Chlamydia pneumoniae 25 cases (5. 7%). Pathogens detected in 724 cases were respiratory syncycial virus 129 (17.8%), adenovirus 19 cases (2.6%), parainfluen-za virus type 1, 3 13 cases (1.8%) and type2 2 cases (0.3%), Klebsiella pneumoniae 24 cases (3. 3%) , Streptococcus pneumoni-ae 23 cases (3.2%), Moraxella catarrhalis 23 cases (3.2%), Haemophilia influenzae 22 cases (3.0%), Staphylococcus aureus 19 cases (2. 6%), Haemophilus parahaemolyticus 18 cases (2. 5%), coagulase-negative Staphlococci 8 cases (1. 1%) and Es-cherichia coli 4 cases (0.6%), Mycoplasma pneumoniae 103 cases (14.2%) and Chlamydia pneumonias 21 cases (3.7%) respectively. Conclusions: Viral infection is the most common cause of acute pneumonia in children, followed by bacteria, Mycoplasma pneumoniae and Chlamydia pneumoniae. Respiratory syncycial virus is the most common pathogen, and the incidence of adenovirus pneumonia is decreasing. There was no evidence of outbreak of influenze and parainfluenzae virus infection. Haemophilus influenza and Streptococcus pneumoniae are the prominent pathogens of bacterial pneumonia, Klebsiella pneumoniae and Moraxella catarrhalis infection are increasing. Viral and Mycoplasma pneumoniae infection are more common in winter and spring while bacterial pneumonia are common in the early summer, however all can be seen in the whole year round. . The average age of patients with Mycoplasma pneumoniae infection become younger and Chlamydia pneumoniae infection is more common in infants.

Abstract:OBJECTIVE:To understand the etiology of pneumonia in hospitalized patients less than 3 years of age.METHODS:A total of 316 children with pneumonia admitted to the Children's Hospital of Suzhou University in Jiangsu Province from March, 2006 to January, 2007 were enrolled in this study. Sputum samples were obtained by deep nasotracheal aspiration technique for bacterial and viral cultures.RESULTS:Of the 316 samples, specific microbial etiology was obtained in 192 cases (60.8%). Bacterial infection was found in 162 cases (51.3 %), viral infection in 19 cases (6.3%)and compound infection with virus and bacteria in 11 cases (3.5 %). Haemophilus influenzae was the most common agent (46 cases; 14.6%) in bacterial infection, followed by Streptococcus pneumoniae (32 cases; 10.1%). Respiratory syncycial virus (RSV) was the most common agent (12 cases; 4.0%) in viral infection, followed by adenovirus (11 cases; 3.6%).CONCLUSIONS:Bacterial infection was a leading cause of pneumonia in children less than 3 years of age in Suzhou area. Haemophilus influenzae was the most common agent, followed by Streptococcus pneumoniae.

Abstract:Background. In Ukraine over the past 20 years, the incidence of pneumonia in children has increased. The aim of the study is to assess the contribution of respiratory viruses and bacteria to pneumonia in children in Ukraine. Materials and methods. The etiology of community-acquired pneumonia was studied in 175 children at Medical Center “Eurolab” and Clinical Hospital 2 (Kyiv, Ukraine) over 2018–2022. Viral and bacterial pathogens were detected using polymerase chain reaction, chromatographic immunoassay. Results. The prevalence of viral pneumonia among total cases of pneumonia was 78.9 % in Kyiv during 2018–2022. Thirty-seven (21.1 %) cases of CAP were classified as bacterial. The most common were respiratory syncytial virus (18.1 %), metapneumovirus (13 %), adenovirus (10.1 %), parainfluenza virus (10.1 %). The other causative viral agents were influenza virus type A (7.2 %), rhinovirus (5 %), bocavirus (2.9 %), SARS-CoV-2 (1.4 %) and measles virus (1.4 %). Etiological spectrum is similar to the results of studies conducted in other European countries. Conclusions. Respiratory viruses, particularly respiratory syncytial virus and metapneumovirus, are the major contributors to community-acquired pneumonia in children in Kyiv.
Bacterial and viral etiology of serious infections in very young Filipino infants

Abstract:Objective: Pneumonia, meningitis and other serious infections are leading causes of death in developing countries. As part of a multicenter study we aimed to determine the etiology of pneumonia, meningitis and other serious infections in a cohort of Filipino infants ages 90 days or younger. Method: During a 2-year period, 2053 infants age 90 days or younger presenting to 1 of 3 Manila community hospitals were screened; 873 had signs or symptoms suggestive of an infectious illness, and 608 were judged to have clinical features suggestive of severe infection and had laboratory workup including blood for culture and white blood cell count, nasopharyngeal aspirate for virology, cerebrospinal fluid culture when indicated and chest radiograph. Chest radiographs were read independently by 3 radiologists without knowledge of clinical findings. Results: Of the 873 enrolled infants, 81 died (91%). After exclusion of presumed contaminants, positive bacterial culture from blood and/or cerebrospinal fluid was obtained in 35 infants (5.8%; 95% confidence interval 4%, 8%), 9 of whom died. The organisms responsible for meningitis were Acinetobacter spp. (4), Streptococcus pneumoniae (2), Escherichia coli (2), Enterobacter spp. (1), Pseudomonas aeruginosa (1), Haemophilus influenzae (1) and Staphylococcus aureus (1); those responsible for the other clinical diagnoses were Salmonella spp. (6), Enterobacter spp. (3), Streptococcus pyogenes (3), other Gram-negative organisms (8), S. pneumoniae (1) and Staphylococcus aureus (2). In 685 infants examined for viral causes of their illness, 223 viruses were isolated from 219 infants (32%; 95% confidence interval 28%, 36%). Enteroviruses were the most common potential pathogens identified (22% of infants studied), followed by respiratory syncytial virus (17%), rhinovirus (10%) and adenovirus (4%). Concomitant virus identification occurred in 10 of those with positive bacterial culture (29%; 95% confidence interval, 15%, 46%), with enterovirus being found in 7 of these cases. Conclusion: Many young Filipino infants with life-threatening illness were evaluated in this study. Thirty-five had infections attributable to bacteria, with Salmonella spp. being the most common, followed by Gram-negative organisms. Pneumococcus was an unusual cause.

Abstract:v = pkDkJ (1) kL? + kD in which kD and kL are the controlling rate parameters at high and low light, respectively, ? is a concentration factor and I is the average light intensity in the reaction mixture given by ? = I?(l e~<-n)/^ (2) In the latter expression, ?c is the attenuation factor for the light field along the axis of light propogation and a is the depth of the cell. I0 is the intensity incident on the front face of the cell. It should be noted that ?c is a characteristic of a given suspension of chloroplast fragments and measures the decay in the field intensity due to absorption only since, to a first approximation in a cell of large cross section and small a, scattering will not attenuate the intensity. It also has been shown that the "microscopic,, rate law applying in the region of a few hundred chlorophyll molecules is given by p'kDkLI v = ?r\ (3) kLI + kD in which I is the constant local intensity. It is the purpose of this paper to explore the implications of the microscopic rate law using deductions based on the form of equation 3 and recent observations on the fluorescence of chloroplast fragments by Mayne et al (2, 3). In this way we shall show that a very simple picture of the reactants and reactions suffices to explain the rate law and the fluorescence observations, thus leading to a particularly useful basis for the interpretation of the rate parameters and the changes these parameters experience when experimental variables are altered. The Model: Mayne and co-workers (3) have demonstrated that the fluorescence-yield of chloroplast fragments is simply related to the steady state Hillreaction velocity through the expression bV [Q] = a ?(4) Vmax in which a and b are constants characteristic of a fragment preparation and Q is the natural quencher of chlorophyll fluorescence. As an aid to the present discussion, consider a modern picture of the photosynthetic apparatus in which the photophosphorylation processes of Arnon and co-workers (4) are included but an added oxidant replaces the natural oxidant.

Abstract:Abstract Background An improved understanding of childhood pneumonia etiology is required to inform prevention and treatment strategies. Lung aspiration is the gold standard specimen for pneumonia diagnostics. We report findings from analyses of lung and pleural aspirates collected in the Pneumonia Etiology Research for Child Health (PERCH) study. Methods The PERCH study enrolled children aged 1–59 months hospitalized with World Health Organization–defined severe or very severe pneumonia in 7 countries in Africa and Asia. Percutaneous transthoracic lung aspiration (LA) and pleural fluid (PF) aspiration was performed on a sample of pneumonia cases with radiological consolidation and/or PF in 4 countries. Venous blood and nasopharyngeal/oropharyngeal swabs were collected from all cases. Multiplex quantitative polymerase chain reaction (PCR) and routine microbiologic culture were applied to clinical specimens. Results Of 44 LAs performed within 3 days of admission on 622 eligible cases, 13 (30%) had a pathogen identified by either culture (5/44) or by PCR (11/29). A pathogen was identified in 12/14 (86%) PF specimens tested by either culture (9/14) or PCR (9/11). Bacterial pathogens were identified more frequently than viruses. All but 1 of the cases with a virus identified were coinfected with bacterial pathogens. Streptococcus pneumoniae (9/44 [20%]) and Staphylococcus aureus (7/14 [50%]) were the predominant pathogens identified in LA and PF, respectively. Conclusions Bacterial pathogens predominated in this selected subgroup of PERCH participants drawn from those with radiological consolidation or PF, with S. pneumoniae and S. aureus the leading pathogens identified.

Abstract:To investigate potential respiratory pathogens in children with community-acquired pneumonia (CAP) and risk factors for severe disease. This prospective study was conducted among 467 children at the Thai Binh Paediatric Hospital, Vietnam between 1 July 2020 and 30 June 2021. Clinical data and laboratory results were collected. Twenty-four respiratory microorganisms were tested from nasopharyngeal swabs using real-time PCR. Logistical regression was used to estimate a factor's adjusted odd ratios of the severity of disease. Mean age of patients = 15.4 ± 13.3 months, 63.0% were male. Over 97% of patients had a positive PCR result. 87% of patients were positive for multiple (up to eight) microorganisms. Rhinovirus (46%), respiratory syncytial virus (RSV) (24%), enterovirus (17%), and parainfluenza viruses-3 (13%) were the most frequent viruses. H. influenzae (61%), S. pneumoniae (45%) and M. catarrhalis (30%) were the most common bacteria. 128 (27%) cases were classified as severe pneumonia. Presence of smokers at home (aOR 2.11, 95% CI 1.27–3.52, P value = 0.004), CRP level ≥ 50 mg/dL (aOR 6.11, 95% CI 3.86–9.68, P value < 0.0001), RSV (aOR 1.78, 95% CI 1.07–2.96, P value = 0.03) and H. influenzae (aOR 1.66, 95% CI 1.03–2.67, P value = 0.04) PCR detection associated with a higher risk of severe pneumonia; ,. Causative agents of pneumonia in children are complex. Children positive with RSV and H. influenzae need to be closely monitored to prevent severe pneumonia.
