Abstract:Anterior crown fractures are a common form of traumatic dental injuries that mainly affect the maxillary incisors, especially in children and adolescents. Complicated crown fractures although less common, pose a challenge to the clinician. The main objective while treating such cases is successful pain management and immediate restoration of function, esthetics and phonetics. Since the development of the adhesive dentistry, the patient's own fragment can be used to restore the fractured tooth. Reattachment of fractured tooth fragments offers a viable restorative alternative. The procedure provides good and long-lasting esthetics, because the original morphology, color, and surface texture are preserved. This article reports management of complicated crown fracture in a young adult and a middle-aged adult that were successfully treated by reattachment technique. The teeth were endodontically treated followed by esthetic reattachment of the fractured fragment using the glass fiber post. To improve the adhesion between fractured and remaining fragment, circumferential beveling and internal groove placement were done.

Abstract:Dental trauma may have a severe impact on the social and psychological wellbeing of a patient. Most cases of dental injuries involve anterior teeth, especially the maxillary upper incisors. Crown fractures, with or without pulp exposure, are the most common trauma in permanent dentition. There are many methods of management, in which the initial state of the pulp, the time since the injury, and the presence of an accompanying injury play a key role. This case report aimed at showing a possible conservative treatment after complicated tooth fracture that consisted of partial pulpotomy followed by adhesive reattachment of the tooth fragment using a technique based on heated resin composite. Such a specific procedure represents a conservative approach to traumatic coronal lesions, providing a suitable opportunity to maintain the tooth vitality, aesthetics, and function. Indeed, reattachment of tooth fragment using a composite/adhesive is a simple technique to achieve excellent results in terms of aesthetic and function.

Abstract:Crown-root fracture is one of the most challenging fracture types in the dental traumatology literature. Traumatized anterior teeth require quick functional and esthetic repair. In the case of a complex crown fracture of the maxillary left central incisor, requiring endodontic treatment, a fiber-reinforced post was used to create a central support stump to restore the dental morphology. This report describes the clinical procedures involved in the treatment. After two years of follow-up, the clinical and radiographic findings demonstrated that the adopted clinical protocol was successful and yielded healthy periodontal tissues with no signs of periradicular pathology.

Abstract:In case of traumatic injuries to the anterior teeth, fragment reattachment is considered a minimally invasive technique. Re-attaching the same tooth fragment, possesses the advantage, of biomimicry by complete restoration of form, function, texture, lustre, and translucency. Due to advancements in adhesive dentistry, simple re-attachment to complex fragment rehabilitation have become predictable and reliable. Various techniques have been proposed for fragment reattachment based on the extent of injury, type of injury, condition of the fragment, and time- lapse. This case report demonstrates the endo-aesthetic rehabilitation of a complicated crown root fracture using a fiber post on lateral incisor with fracture line on the cervical 1/3of the labial aspect, running obliquely and apically towards the palatal surface after 1 year follow-up.

Abstract:Dental trauma requires immediate treatment. However, some conventional treatments (such as an implant and/or partial fixed denture) will not always be immediately possible, due to the patient's age (mainly in pediatric cases) or lack of immediate financial resources. In such situations, the technique of adhesive reattachment plays an important role as a treatment option in the interim. This case report describes the use of adhesive fragment reattachment in the rehabilitation of a maxillary central incisor with a complicated crown-root fracture. A 1-year follow-up affirmed that the adhesive reattachment efficiently resolved the crown-root fracture, allowing for the retention of the tooth structure. This more conservative multidisciplinary approach restored function, esthetics, and self-esteem to the patient, while preserving all bone and gingival architecture for future implant treatment.
Using an existing crown to repair a damaged cast post and core restoration

Abstract:A fractured coronal tooth structure beneath an intact crown is a common clinical occurrence. If the underlying root is healthy, the tooth is restored with a post and core followed by refabrication of the crown. This paper describes a technique of using the existing intact crown for the above-mentioned situation. A 34-year-old female was referred with a fractured right canine with an intact crown. A post was found fractured in the canal which was subsequently retrieved. A new fiber post was cemented in the post space followed by adaptation of 50 μm polytetrafluoroethylene (teflon) tape on the tissue surface of the crown. Dual-cured core build-up composite resin was injected into the crown and adapted to the fractured tooth. On curing and removal of the crown, a new composite resin core was found bonded to the tooth structure. The Teflon tape was removed from the crown, and the crown cemented to the core using glass ionomer cement. This technique of building up the core of the tooth using teflon tape adapted to the tissue surface of the crown was found to be successful even after 1 year of follow-up.

Abstract:Coronal fractures of the anterior teeth are common sequelae of dental trauma. In case of complex fractures, where the fractured segment is available and there is close approximation of the segment to the remaining tooth, root canal treatment followed by reattachment of the fractured segment with fiber post reinforcement is a feasible option. The procedure is simple and economic and needs less chair-side time as compared to many conventional methods. Abstract:Background: The maxillary permanent central incisors are the anterior teeth most at risk of experiencing tooth fracture injuries due to trauma. Dental trauma is caused by impact resulting from sufficient mechanical energy to involve the exposed pulp. Appropriate therapy for technical ability to restore teeth due to trauma with exposed pulp with root canal treatment accompanied by fiber post restoration and jacket crown. Case report: a 22 year old male patient diagnosed with pulp necrosis accompanied by asymptomatic apical periodontitis and root canal treatment was performed on the upper right central incisor. Objective examination results showed negative vitality and positive percussion. The results of the radiological examination showed a radiolucent area in the apical area and a widened periodontal membrane in the apical 1/3 of the tooth. The tooth undergoes root canal treatment by isolating the working area, then preparing the access cavity. Negotiate the root canal using #10 K-files and measure the working length with an apex locator. The root canal was irrigated using 2.5 % NaOCl followed by 17% EDTA. and 2% chlorhexidine. Preparation is carried out using hand-held instruments. Filling the root canal with endomethasone sealer using the lateral condensation obturation technique and followed by making a fiber post restoration and Porcelain Fused Metal crown at the next visit. Conclusion: Root canal treatment in the case of the upper right central incisor with a fused metal porcelain jacket crown restoration and fiber post which was carried out in this case showed clinical and radiological success

Abstract:Two fractured maxillary central incisors were restored via the crown fragment reattachment technique with adhesive systems and composite resin material. A long-term successful outcome was achieved after 4 years of follow-up. The patient presented with fractured maxillary central incisors, and a comprehensive intraoral and radiographic examination revealed that both teeth had extended crown fractures with no pulpal exposure (Class II, Ellis and Davey). The tooth fragments were thoroughly cleaned and inspected before being repositioned. Both fragments were in excellent condition with almost no pieces missing. Therefore, the proposed treatment was fragment reattachment. After verifying the repositioning and adaptation of the fractured fragments, a flowable composite was used to perform the reattachment procedure under rubber dam isolation. With modern dental adhesives, the reattachment of tooth fragments in good condition is considered a more conservative option for restoring tooth morphology, esthetics, and function. Although one tooth showed signs of irreversible pulpitis and required root canal treatment 2 weeks after the restoration, the clinical and radiographic evaluations at the 4-year follow-up visits confirmed the success of the restorative treatment. The restorations showed good functional and esthetic outcomes after 4 years of follow-up.

Dentists are confronted with managing dental trauma and restoring fractured teeth on a regular basis. Hence the techniques that speed and simplify the treatment, restore esthetics and improve long term success rate are considered of potential value. The reattachment of dental fragments, if possible, is a conservative and effective technique, which is utilized in this case. This case presentation concerns an 18-year-old male patient with a root-crown fracture of the left maxillary central incisor, which involves the periodontal biologic space. The treatment was the reattachment of the dental fragment with a composite resin and adhesive, in association with periodontal surgery. The reattachment technique described in this case report is simple, but it provides long-lasting esthetics and improved function via a very conservative approach.

Abstract:OBJECTIVES: The aim of this in vitro study was to evaluate the static load-bearing capacity and the failure mode of endodontically treated maxillary incisors restored with complete crowns made of experimental composite resin (FC) with short fiber fillers, with and without root canal posts. Further aim was to evaluate the effect of fiber-reinforced composite resin (FRC) on the failure mode of the restoration.MATERIAL AND METHODS: The experimental composite resin (FC) was prepared by mixing 22.5 wt.% of short E-glass fibers (3mm in length) and 22.5 wt.% of semi-interpenetrating polymer network (IPN) resin with 55 wt.% of silane treated silica fillers. The clinical crowns of 30 human extracted maxillary incisors were sectioned at the cemento-enamel junction. Five groups of direct complete crowns were fabricated (n=6); Group A: made from particulate filler composite resin (PFC) (Grandio Caps, VOCO, control), Group B: PFC with fiber post (everStick, StickTeck), Group C: made from PFC with everStick fiber post and FRC-substructure, Group D: made from FC, Group E: made from FC with FRC-substructure. The root canals were prepared and posts were cemented with resin cement (ParaCem Universal). All restored teeth were stored in water at room temperature for 24h before they were statically loaded with speed of 1.0 mm/min until fracture. Data were analyzed using ANOVA (p=0.05). Failure modes were visually examined.RESULTS: ANOVA revealed that restorations made from experimental fiber composite resin had higher load-bearing capacity (349N) (p<0.05) than the control restorations (173N). No significant difference was found in load-bearing capacity between restorations reinforced with FRC-substructure and those without (p>0.05).CONCLUSIONS: Restorations made from short glass fiber containing composite resin with IPN-polymer matrix showed better load-bearing capacity than those made with either plain PFC or PFC reinforced with fiber post.

Abstract:Anterior teeth with subgingival fractures require a multidisciplinary management approach with regards to biological, functional and aesthetic factors. This case report emphasises the use of a minimally invasive technique combined with a sequence of therapies to treat a complicated crown-root fracture and reviews the critical factors to ensure predictable outcomes. Endodontic treatment was undertaken due to exposure of the pulp in a complicated crown-root fracture of the maxillary right central incisor. Extrusion of the fractured fragment was performed to expose the fracture margin under the alveolar bone. Modified crown-lengthening surgery was used to reconstruct the biologic width. The traumatised tooth was restored with a glass fibre post and resin core, which fit the biological requirements. Finally, an all-ceramic crown fulfilled biomimetic aesthetics. This multidisciplinary approach achieved a good long-term prognosis with regards to periodontal and periapical healing.

Abstract:Aim Several treatment modalities have been used to restore crown fractures in case of uncomplicated crown fractures. Although many treatment modalities have been reported, there is no consensus on which one has the best results in terms of success and durability. This systematic review of treatment modalities in uncomplicated crown fractures aims to provide the most common and successful treatment modality for each patient's unique needs, balancing conservation, aesthetics, and long‐term durability. Materials and Methods Three electronic databases (PubMed, Scopus, and Google Scholar) were searched for articles regarding treatment modalities in uncomplicated crown fractures in anterior maxillary teeth. Results Following the application of the inclusion and exclusion criteria, 10 case reports, four case series, and two retrospective studies were selected for the review. These studies reported on fragment reattachment, direct composite resin, and veneers for the management of uncomplicated crown fractures. Fragment reattachment was the most preferred treatment modality, with milk, water, or sterile saline used as storage mediums. Direct composite restoration was preferred for missing or bad‐shaped fragments or longevity. Veneers were given for aesthetic consideration and fracture resistance when the crown structure was majorly affected by the fracture. Fragment reattachment was favored in 63% of the articles included in the review followed by direct composite restoration in 26% of the articles. Venners were used in about 11% of the articles. Composites depicted a higher survival rate and a better outcome than fragment reattachment. Follow‐up periods ranged from 1 week to 4 years, and all articles successfully re‐established function and aesthetics, except for one tooth that required root canal treatment. Conclusion Fragment reattachment, direct composite restoration, and veneers are successful treatment modalities in uncomplicated crown fractures of anterior teeth. The choice of treatment depends on various factors like availability of fragments, age, and financial condition of the patient. Trial Registration PROSPERO Registration ID: CRD42020153091

Abstract:Fractured anterior teeth from accidental falls and sports injuries are very common amongst teenagers and young adults. Management of such critical emergencies is time sensitive for preserving the health of traumatized teeth. Timely management also helps in reassuring the patient and instilling confidence in regards to ideal esthetic outcome and patient satisfaction. However, there is no standard treatment that can be applied to all clinical case scenarios and the treatment has to be innovative and adaptive to the specific case. This case report, specifically aims to highlight a clinical case that was managed by re-attaching the fractured segment on a permanent central incisor. A 24-year-old young man, visiting the town for a wedding, walked in with a piece of broken front tooth in his hand. After a clinical and radiographic exam, though presented with advanced treatment options of a crown or a veneer for long term success, patient insisted on keeping the broken piece and "gluing" it back. Would a direct composite restoration be successful as an interim procedure until the patient returns to his primary dentist? And even if it does, would the patient be happy disposing off his broken piece of tooth? This presents a paradox between clinician’s judgement for best outcome and patient satisfaction. The success of direct composite restorations is never disappointing in a skilled clinician's hands. Let's delve into some literature review, step by step management and conclusion of this clinical situation with a WIN-WIN outcome.

Abstract:One of the common forms of dental injury is anterior crown fractures, which mainly affects the teenagers and young adults. Fractures of the coronal portion of the permanent incisors characterize 18–22% of total traumatic injuries to dental hard tissues; of which 96% of them comprise the maxillary incisors. Uncomplicated fracture of crown is one of the most common types of dental traumatic injury. Dental trauma has an emotional impact on the patient’s overall health and can serious harm to the dentition. The treatment as well as prognosis of the fracture of coronal portion is a major challenge for a dentist because it has to accomplish various parameters like need to obtain an aesthetical result that approaches itself to its natural form and measurement, opaqueness and translucency of the original tooth structure in obtaining a effective restoration. It is suggested that reattachment of the fractured fragment is the best procedure for restoring an uncomplicated fracture of a crown, if a fragment is present. Reattachment of the fractured fragment offers major advantages over the conventional composite restoration. In reattachment, enhanced aesthetics is obtained because enamel’s true shape, colour, intensity, and surface texture are preserved. It re-establishes the major function and produces the favourable emotive and social response from the side of the patient. The reattached fragments are susceptible to further fracture when the restored teeth undergo further trauma. The resistance of the fractured teeth that has been reattached is the subject of the majority of concerns. Preconditioning the fractured fragments with remineralizing agents may aid in hydration. Thus, study will be conducted to evaluate the resistance of the fracture of a tooth that is reattached and pre-treated with remineralizing agents such as sodium fluoride and casein phosphopeptide-amorphous calcium phosphate and further reattached using one of the self-adhesive bioactive composite.

Abstract:Dental anterior fractures are common injuries, especially in those who practice extreme sports. This report describes a 25-year-old Bolivian patient who attended our private dental clinic in La Paz, Bolivia after experiencing an accident during downhill mountain biking. An intraoral examination revealed a fracture line on the buccal side in the middle third of the coronal portion of the right central maxillary incisor which extended towards the proximal and lingual sides. Multidisciplinary treatment, including crown lengthening, osteotomy, root canal treatment, fiberglass post insertion, and reattachment of the fracture segment was performed. A follow-up examination at 10 months later revealed that the tooth was completely reestablished both functionally and esthetically and that there was no periapical pathosis or discomfort. This outcome suggests that if a patient seeks a dental consultation soon after a complex crown-root fracture has occurred, and if the broken tooth segment is available, then reattachment offers an economical and simple treatment option which will allow immediate restoration of functionality and esthetic standards. Continued follow-up should form part of such a treatment plan to allow long-term pulp vitality and periodontal health status to be monitored.
