Diagnostic Value of Different MRI Features in Subaxial Cervical Ligament Injury

Abstract:Objective To evaluate the reliability of different MRI features in diagnosis of subaxial cervical ligment injury. Methods MRI was performed in 132 patients with subaxial cervical spinal injury before operation. MRI features of discontinuity on sagittal T1-weighted image(T1D),longitudinal high signal intensity on sagittal T2-weighted image(T2L),transversal high or intermediate signal intensity on sagittal T2-weighted image(T2T)was used to assess the status of anterior longitudinal ligament(ALL) and posterior longitudinal ligament(PLL) by two observers. The imaging results were compared with operational findings; the inter-observer consistency was determined by the kappa value. The sensitivity, specificity, accuracy, positive and negative predictive value of different MRI features in detection of ALL and PLL injury were calculated using operative findings as the gold standard. Results Inter-observer consistency of T1D for the assessment of ALL and PLL injury was not satisfactory (Kappa value=0.112, 0.285), and the sensitivity, specificity, accuracy, positive and negative predictive value of T1D was low. For T2L, the inter-observer consistency was fair(Kappa value=0.667, 0.537), and there was a high specificity (78.8～85.9%, 90.3～97.2%), however, the sensitivity was only 39.4～50.0% and 23.8～49.2%. By contrast, the inter-observer consistency of T2T for the assessment of ALL and PLL injury was excellent (Kappa value= 0.852, 0.799), and there was high sensitivity, specificity, accuracy, positive and negative predictive value. Conclusion The T1D and T2L of MRI cannot be used as reliable indicators of ligament injury in subaxial cervical spine. However, the T2T of MRI is recommended for detection of ALL and PLL injury in subaxial cervical spine and would be helpful in assessing the stability of spine.
Multivariate Analysis for Prognostic Factors on Non-Operative Treatment of Cervical Spinal Cord Injury Without Fracture or Dislocation

Abstract:OBJECTIVE:To analyze the factors in the non-operative treatment of cervical spinal cord injury without fracture or dislocation.METHODS:The clinical data of 122 patients with cervical spinal cord injury without fracture or dislocation from January 2009 to December 2012 treated by non-operative treatment were retrospectively reviewed. There were 84 males and 38 females, aged from 18 to 83 years with an average of (52.37 +/- 13.27) years. The clinical features, such as age, gender, cause of injury, time from injury to treatment, ASIA grade of spine cord injury, MRI type of spine cord injury, range of spine cord injury, effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, segment of intervertebral disc herniation, intervertebral disc ligament complex injury, treatment of high-dose methyiprednisolone, were observed. Single factor and multiple factor Logistic regression analysis were used in the clinical data in order to analyze the influencing factors of above items to prognosis.RESULTS:For univariate analysis, the factors such as MRI type of spine cord injury, extent of spine cord injury, effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, segment of intervertebral disc herniation, ASIA grade of spine cord injury, associated with prognosis (P < 0.05). Multiple linear regression analysis showed that the main prognostic factors including MRI type of spine cord injury, range of spine cord injury, effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, ASIA grade of spine cord injury according to its effective intension (P < 0.05).CONCLUSION:The main prognostic factors on non-operative treatment of cervical spinal cord injury without fracture or dislocation were MRI type and extent of spine cord injury, meanwhile, correlate with effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, ASIA grade of spine cord injury. It is deliberative to choose no-operative treatment, for it only refers to the patients with the mild localized edema type or no signal change of spinal cord in MRII. Operative treatment shoud be recommended for other patients.
Radiological and Clinical Significance of Cervical Dynamic Magnetic Resonance Imaging for Cervical Spondylotic Myelopathy

Abstract:Objective: The study compared the morphometric changes of the cervical spinal cord using dynamic magnetic resonance imaging (MRI) in patients with cervical spondylotic myelopathy (CSM) and assessed the correlation with kinematic changes, cord cross-sectional area (CSA), and high signal intensity (SI) on T2-weighted imaging (T2WI).Methods: Patients with CSM were evaluated through dynamic MRI for sagittal and axial CSA changes of the cervical cord, cerebrospinal fluid (CSF) reserve ratio, degree of cord impingement, cord compression rate, range of motion (ROM), and severity of SI on T2WI. The degree of cord impingement was evaluated using the Muhle grading system. Clinical outcomes were assessed using Japanese Orthopaedic Association scoring and Nurick grade.Results: The study included 191 patients (113 males) with a mean age of 55.34 ± 12.09 years. The lowest sagittal CSF reserve ratio and cord occupation rate were observed during extension. Cord impingement and SI change were more prevalent in extension-positioned MRI. There was no difference between ROM on dynamic radiographs and dynamic MRI. Preoperative cervical ROM was greater in patients with intensely high SI change.Conclusion: Dynamic MRI is useful for evaluating neck movement. Patients with high SI had greater ROM before surgery but worse outcomes after. Neck extension exacerbated cervical stenosis and cord compression compared to flexion, and cervical spinal motion contributed to the severity of CSM. Cervical spinal motion should be carefully evaluated, particularly in hyperextension, to prevent worsening of CSM.

Abstract:STUDY DESIGN: A retrospective study evaluating magnetic resonance imaging, computed tomographic myelography, and clinical parameters in patients with cervical spondylotic myelopathy.OBJECTIVES: To investigate whether magnetic resonance imaging can predict the surgical outcome in patients with cervical spondylotic myelopathy.SUMMARY OF BACKGROUND DATA: No previous studies have established whether areas of high signal intensity in T2-weighted magnetic resonance images can be a predictor of surgical outcomes.METHODS: Fifty patients with cervical spondylotic myelopathy were examined by magnetic resonance imaging and computed tomographic myelography before surgery and by delayed computed tomographic myelography after surgery. The correlation between the recovery rate and the clinical and imaging parameters was analyzed.RESULTS: The best prognostic factor was the transverse area of the spinal cord at maximum compression (correlation coefficient, R = 0.58). The presence of high signal intensity areas on T2-weighted magnetic resonance images correlated poorly with the recovery rate (R = -0.29). However, patients with multisegmental areas of high signal intensity on T2-weighted magnetic resonance images tended to have poor surgical results associated with muscle atrophy in the upper extremities. Postoperative delayed computed tomographic myelography showed that multisegmental areas of high signal intensity on T2-weighted magnetic resonance images probably represent cavitation in the central spinal cord.CONCLUSIONS: Patients with multisegmental areas of high signal intensity on T2-weighted magnetic resonance images tended to have poorer surgical results. However, the transverse area of the spinal cord at the level of maximum compression was a better prognostic indicator.

Abstract:Background The phrase "compressive myelopathy" refers to compression of the spinal cord, either internally or externally. This compression might arise from various sources such as a herniated disc, post-traumatic compression, and epidural abscess as well as epidural or intradural neoplasms. Magnetic resonance imaging (MRI) plays a crucial role in differentiating between compressive and non-compressive myelopathy. After eliminating compressive lesions, attention is directed toward intrinsic cord-related causes of acute myelopathy including vascular, infectious, and inflammatory pathologies. Aims The study aimed to assess different etiologies of compressive myelopathy, analyze the MRI features of spinal cord compressive lesions, classify the lesions depending on site, and correlate MRI findings with intraoperative findings and histopathology in operated cases. Material & methods A total of 50 patients, who exhibited clinical symptoms indicative of compressive myelopathy sent to the Radiology department, Rangaraya Medical College (RMC), Kakinada for MRI spine were included in the study. It's an observational cross-sectional study. Statistical Package for Social Sciences (SPSS) version 22.0 (IBM Corp., Armonk, USA) was used for statistical calculations. Result Among the 50 cases of compressive myelopathy, the etiologies are distributed as follows: trauma (22 cases), infection (12 cases), primary neoplasm (eight cases), and secondary neoplasm (eight cases); extradural compressive lesions (84%) and Intradural-extramedullary lesions (16%). Conclusion Utilizing MRI successfully assessed the spinal cord integrity and characterized spinal tumors. Consequently, the study concludes that MRI is a highly definitive, sensitive, and accurate tool for evaluating compressive myelopathy.

Abstract:Study Design: We performed a prospective observational study of 52 patients who were clinically suspected of cervical spondylotic myelopathy (CSM), based on the modified Japanese Orthopaedic Association (mJOA) score, and were referred for magnetic resonance imaging (MRI) of the cervical spine.Purpose: To evaluate the quantitative parameters of the diffusion tensor imaging (DTI) matrix (fractional anisotropy [FA] and apparent diffusion coefficient [ADC] values) and determine the subsequent correlation with the clinical assessment of disease severity in CSM.Overview of Literature: Conventional MRI is the modality of choice for the identification of cervical spondylotic changes and is known to have a low sensitivity for myelopathy changes. DTI is sensitive to disease processes that alter the water movement in the cervical spinal cord at a microscopic level beyond the conventional MRI.Methods: DTI images were processed to produce FA and ADC values of the acquired axial slices with the regions of interest placed within the stenotic and non-stenotic segments. The final quantitative radiological derivations were matched with the clinical scoring system.Results: Total 52 people (24 men and 28 women), mean age 53.16 years with different symptoms of myelopathy, graded as mild (n=11), moderate (n=25), and severe (n=16) as per the mJOA scoring system, underwent MRI of the cervical spine with DTI. In the most stenotic segments, the mean FA value was significantly lower (0.5009±0.087 vs. 0.655.7±0.104, p<0.001), and the mean ADC value was significantly higher (1.196.5±0.311 vs. 0.9370±0.284, p<0.001) than that in the non-stenotic segments. The overall sensitivity in identifying DTI metrics abnormalities was more with FA (87.5%) and ADC (75.0%) than with T2 weighted images (25%).Conclusions: In addition to the routine MRI sequences, DTI metrics (FA value better than ADC) can detect myelopathy even in patients with a mild grade mJOA score before irreversible changes become apparent on routine T2 weighted imaging and thus enhance the clinical success of decompression surgery.
Magnetization Transfer Ratio and Morphometrics of the Spinal Cord Associates with Surgical Recovery in Patients with Degenerative Cervical Myelopathy

Abstract:Objectives: We assessed the prognostic value of the preoperative magnetization transfer ratio (MTR) and morphometrics of the spinal cord in patients with degenerative cervical myelopathy (DCM) in a longitudinal cohort study. Methods: Thirteen subjects with DCM underwent 3T magnetization transfer imaging. The MTR was calculated for the spinal cord regions and specific white matter tracts. Morphometric measures were extracted. Clinical (modified Japanese Orthopaedics Association [mJOA] and Nurick scale scores) and health-related quality of life scores were assessed before and after cervical decompression surgery. The association between the magnetic resonance imaging (MRI) metrics and postoperative recovery was assessed (Spearman's correlation). Receiver operating characteristics were used to assess the accuracy of MRI metrics in identifying ≥50% recovery in function. Results: Preoperative anterior cord MTRs were associated with recovery in mJOA scores (ρ = 0.608; P = 0.036; area under the curve [AUC], 0.66). Preoperative lateral cord MTR correlated with the neck disability index (ρ = 0.699; P = 0.011) and pain interference scale (ρ = 0.732; P = 0.007). Preoperative rubrospinal tract MTR was associated with mJOA score recovery (ρ = 0.573; P = 0.041; AUC, 0.86). Preoperative corticospinal tract and reticulospinal MTRs were related to recovery in pain interference scores (ρ = 0.591; P = 0.033; and ρ = 0.583; P = 0.035, respectively). Eccentricity of the cord was associated with Nurick scores (ρ = 0.606; P = 0.028) and mJOA scores (ρ = 0.651; P = 0.025; AUC, 0.92). Conclusions: Preoperative MTR and eccentricity measurements of the spinal cord have prognostic value in assessing the response to surgery and recovery in patients with DCM. Advanced MRI and atlas-based postprocessing techniques can inform interventions and advance the healthcare received by patients with DCM.

Abstract:Objective To investigate the clinical significance and effect estimate of intramedullary high intensity lesion on T2 weighted MRI in the cervical spondylotic myelopathy. Methods This study is comprised of 53 patients of cervical spondylotic myelopathy,with an average course of 25.7 months(range,2 months to 11 years).18 patients were combined with OPLL.According to the Japanese Orthopedic Association(JOA) score,the pre-and postoperative neurological statuses and MRI images were compared to observe the relationship between the differences of intramedullary high signal on T2 weighted MRI and the clinical prognosis. Results Average follow-up was 6 months(range,3 to 12 months).Patients with long courses of disease and multisegmental intramedullary high signal on T2 weighted MRI usually had serious conditions and bad prognosis(P0.05);the cases with short courses and single-segmental high signal received better recovery. Conclusions Cervical intramedullary high intensity lesion on T2-weighted MR images is significant for the estimate of clinical prognosis in cervical spondylotic myelopathy.

Abstract:This meta-analysis was designed to elucidate whether preoperative signal intensity changes could predict the surgical outcomes of patients with cervical spondylosis myelopathy on the basis of T1-weighted and T2-weighted magnetic resonance imaging images. We searched the Medline database and the Cochrane Central Register of Controlled Trials for this purpose and 10 studies meeting our inclusion criteria were identified. In total, 650 cervical spondylosis myelopathy patients with (+) or without (-) intramedullary signal changes on their T2-weighted images were examined. Weighted mean differences and 95% confidence intervals were used to summarize the data. Patients with focal and faint border changes in the intramedullary signal on T2 magnetic resonance imaging had similar Japanese Orthopaedic Association recovery ratios as those with no signal changes on the magnetic resonance imaging images of the spinal cord did. The surgical outcomes were poorer in the patients with both T2 intramedullary signal changes, especially when the signal changes were multisegmental and had a well-defined border and T1 intramedullary signal changes compared with those without intramedullary signal changes. Preoperative magnetic resonance imaging including T1 and T2 imaging can thus be used to predict postoperative recovery in cervical spondylosis myelopathy patients.

The Prediction of Neurological Prognosis for Cervical Spondylotic Myelopathy Using Diffusion Tensor Imaging

Abstract:Objective: Although cervical spondylotic myelopathy (CSM) can be easily diagnosed using magnetic resonance imaging (MRI), prediction of surgical effect using preoperative radiological examinations remains difficult. In previous studies, it was reported that diffusion tensor imaging (DTI) may be used for the prediction of surgical effect; however, these studies did not consider the influences of spinal cord compression even though the values of DTI indexes can be distorted by compressive lesions in patients with CSM. Therefore, it is uncertain whether preoperative DTI indexes can actually predict the surgical effect. The aim of this study was to investigate DTI metrics that are hardly affected by spinal cord compression and can accurately predict neurological status after decompressive surgery.Methods: Twenty-one patients with CSM who underwent surgery and 10 healthy volunteers were enrolled in this study. The subjects underwent cervical MRI, and values of DTI indexes including axial diffusivity (AD), radial diffusivity (RD), apparent diffusion coefficient (ADC), and fractional anisotropy (FA) were recorded at each intervertebral level. Further, the Japanese Orthopaedic Association (JOA) score of each patient with CSM was recorded before and after surgery for neurological status evaluation. Preoperative and postoperative values of DTI indexes were compared, and correlations between preoperative DTI parameters and postoperative neurological recovery were assessed.Results: After surgery, the lesion-adjacent (LA) ratios of RD and ADC increased (p = 0.04 and p = 0.062, respectively), while the LA ratio of FA decreased (p = 0.075). In contrast, the LA ratio of AD hardly changed. A negative correlation was observed between preoperative LA ratio of AD and JOA recovery rate 6 months after surgery (r = -0.379, p = 0.091). Based on preoperative LA ratio of AD, the patients were divided into a low AD group and a high AD group, and JOA recovery rate 6 months after surgery was found to be higher in the low AD group than in the high AD group (p = 0.024).Conclusion: In patients with CSM, preoperative LA ratio of AD is seldom affected by spinal cord compression, and it negatively correlates with JOA recovery rate 6 months after surgery.

Histogram Analysis of Lipid-Core Plaques in Coronary Computed Tomographic Angiography: Ex Vivo Validation Against Histology

Abstract:PurposeIn coronary computed tomographic angiography (CTA), low attenuation of coronary atherosclerotic plaque is associated with lipid-rich plaques. However, an overlap in Hounsfield units (HU) between fibrous and lipid-rich plaque as well as an influence of luminal enhancement on plaque attenuation was observed and may limit accurate detection of lipid-rich plaques by CTA. We sought to determine whether the quantitative histogram analysis improves accuracy of the detection of lipid-core plaque (LCP) in ex vivo hearts by validation against histological analysis. Materials and MethodsHuman donor hearts were imaged with a 64-slice computed tomographic scanner using a standard coronary CTA protocol, optical coherence tomography (OCT), a histological analysis. Lipid-core plaque was defined in the histological analysis as any fibroatheroma with a lipid/necrotic core diameter of greater than 200 &mgr;m and a circumference greater than 60 degrees as well as a cap thickness of less than 450 &mgr;m. In OCT, lipid-rich plaque was determined as a signal-poor region with diffuse borders in 2 quadrants or more. In CTA, the boundaries of the noncalcified plaque were manually traced. The absolute and relative areas of low attenuation plaque based on pixels with less than 30, less than 60, and less than 90 HU were calculated using quantitative histogram analysis. ResultsFrom 5 hearts, a total of 446 cross sections were coregistered between CTA and the histological analysis. Overall, 55 LCPs (12%) were identified by the histological analysis. In CTA, the absolute and relative areas of low attenuation plaque less than 30, less than 60, and less than 90 HU were 0.14 (0.31) mm2 (4.22% [9.02%]), 0.69 (0.95) mm2 (18.28% [21.22%]), and 1.35 (1.54) mm2 (35.65% [32.07%]), respectively. The low attenuation plaque area correlated significantly with histological lipid content (lipid/necrotic core size [in square millimeter] and a portion of lipid/necrotic core on the entire plaque) at all thresholds but was the strongest at less than 60 HU (r = 0.53 and r = 0.48 for the absolute and relative areas, respectively). Using a threshold of 1.0 mm2 or greater, the absolute plaque area of less than 60 HU in CTA yielded 69% sensitivity and 80% specificity to detect LCP, whereas sensitivity and specificity were 73% and 71% for using 25.0% or higher relative area less than 60 HU. The discriminatory ability of CTA for LCP was similar between the absolute and relative areas (the area under the curve, 0.744 versus 0.722; P = 0.37). Notably, the association of the low attenuation plaque area in CTA with LCP was not altered by the luminal enhancement for the relative (P = 0.48) but for the absolute measurement (P = 0.03). Similar results were achieved when validated against lipid-rich plaque by OCT in a subset of 285 cross sections. ConclusionsIn ex vivo conditions, the relative area of coronary atherosclerotic plaque less than 60 HU in CTA as derived from quantitative histogram analysis has good accuracy to detect LCP as compared with a histological analysis independent of differences in luminal contrast enhancement.
Relationship Between Signal Changes on T2-Weighted Magnetic Resonance Images and Cervical Dynamics in Cervical Spondylotic Myelopathy

Abstract:STUDY DESIGN:Technical note. OBJECTIVE:To determine the correlation between the high-intensity lesions observed on T2-weighted magnetic resonance images (T2W MRI) and the cervical dynamic characteristics of patients with cervical spondylotic myelopathy (CSM). SUMMARY OF BACKGROUND DATA:Intramedullary high signal intensity is frequently observed on T2W MRI of CSM patients and represents pathologic changes in the spinal cord. However, few studies have attempted to identify the effects on cervical dynamics associated with such changes in MRI signals. METHODS:This study included 71 CSM patients who were admitted to our hospital between May, 2009 and May, 2012 (44 men, 27 women; average age, 52.5±11.7 y). They were divided into 3 groups depending on T2W MRI data: group 1, no hyperintensity; group 2, slight hyperintensity; and group 3, bright hyperintensity. The Cobb angle on cervical flexion-extension radiographs was measured as a parameter of cervical spine dynamics. RESULTS:Total hyperflexion, hyperextension curvature, range of movement (ROM), and segmental hyperflexion curvature did not differ among the groups (P>0.05). Segmental hyperextension curvature and ROM were greater in groups 2 and 3 than in group 1 (P<0.05) but did not differ significantly between groups 2 and 3 (P>0.05). CONCLUSIONS:Increased segmental hyperextension curvature (≥10 degrees) and ROM are risk factors for high-intensity lesions on T2W MRI in CSM patients.
Usefulness of Conventional Magnetic Resonance Imaging, Diffusion Tensor Imaging and Neurite Orientation Dispersion and Density Imaging in Evaluating Postoperative Function in Patients with Cervical Spondylotic Myelopathy.

Abstract:Objective: The objective of this study was to evaluate the usefulness of T2 high signal intensity (T2-HSI) and decreased anteroposterior diameter (APD), diffusion tensor imaging (DTI) and neurite orientation dispersion and density imaging (NODDI) in evaluating postoperative cervical cord function. Methods: The study included 57 postoperative cervical spondylotic myelopathy patients. Clinical evaluation and functional recovery assessments were performed using the modified Japanese Orthopaedic Association (mJOA) score and recovery rate. The presence of T2-HSI and decreased APD was recorded for exploring the relevance. Spearman correlation was applied to investigate the relationships between DTI and NODDI metrics and mJOA score. Multiple comparisons of T2 signal intensity, APD and diffusion metrics were evaluated by using multiple linear regression. Results: Only the recovery rate was significantly different between T2-HSI and non-T2-HSI (nT2-HSI) patients (chi(2)= 4.466, p = 0.045). Significant differences were not observed between cervical cords with and without decreased APD. Diffusion metrics, including fractional anisotropy (p = 0.0005), mean diffusivity (p = 0.0008), radial diffusivity (p = 0.0003) and intracellular volume fraction (p = 0.001), were significantly correlated with mJOA score. The ability of T2 signal intensity (p = 0.421) and APD (p = 0.420) to evaluate the postoperative function was inferior to that of fractional anisotropy (p = 0.002), mean diffusivity (p = 0.001), radial diffusivity (p = 0.001) and intracellular volume fraction (p = 0.004). Conclusion: Conventional magnetic resonance imaging signs could be considered as a reference to make an approximate assessment, whereas DTI and NODDI could be better quantitative tools for evaluating the postoperative function and may help in interpreting residual symptoms. The translational potential of this article: DTI and NODDI could provide reliable postoperative evaluation and analysis for cervical spondylotic myelopathy patients. (C) 2018 The Authors. Correlation between postoperative MRI signal changes and operative outcomes in cervical spondylotic myelopathy

Abstract:Objective To preliminarily discuss the relationship between pre- and postoperative changes of high signal on MR T2W images and operative outcome of cervical spondylotic myelopathy. Methods The participants in this study were 26 patients (including 15 males, 11 females, with the mean age of 49.8 years) who underwent decompressive surgery for cervical spondylotic myelopathy in our hospital. MRI was performed in all patients. Spinal cord signal intensity changes were evaluated by comparing the pre- and postoperative grading scores on sagittal T2WI, according to Yukawa method. The transverse sectional area at the site of maximal compression was also measured in axial position. The Japan Orthopaedic Association (JOA) scoring system was used for evaluation of pre- and postoperative neurological status. The relationship between pre- and postoperative changes of high signal on MR T2WI and operative outcome of cervical spondylotic myelopathy were analyzed. Results According to the postoperative changes of the spinal cord signal intensity, patients were divided into three groups, signal intensity decreasing group (SID) for 18 patients, signal intensity unchanging (SIU) group for 7 patients, signal intensity increasing group (SII) for 1 patient. There was no significant difference in the preoperative signal intensity between SID and SIU groups. No significant relationship was observed between preoperative signal intensity with postoperative JOA score and function improvement in SID and SIU groups. Significant differences were observed in SID vs SIU groups at age[(51.94±11.04) vs (61.12±10.14), P=0.048], the preoperative transverse size[(60.90±14.77) vs (42.05±18.05), P=0.010], postoperative JOA [(14.44±1.82) vs (11.00±3.89), P=0.042], and recovery rate [(64%±23%) vs (38%±30%), P=0.027]. The SII group was excluded from the control study for the lack of number. Conclusion The changes of intramudullary high signal intensity on T2WI are the good predictor of postoperative outcome.
Could MR Diffusion Tensor Imaging Help to Solve Clinical-Radiological Mismatch in Cervical Spondylotic Myelopathy?

Abstract:Abstract Background: Cervical spondylotic myelopathy (CSM) is a major cause of motor dysfunction. The radiological diagnosis of CSM has to be made as early as possible, since surgical interference in earlier stages during the disease course was reported to be more successful and associated with favorable post-operative clinical outcome when compared with later stages. Diffusion tensor imaging (DTI) is a promising technique that could help to detect early structural cord changes in patients presenting with cervical compressive myelopathy. Aim of Study: To investigate the utility of MR diffusion tensor imaging using its quantitative fractional anisotropy (FA) and apparent diffusion coefficient (ADC) parameters as an early detector biomarker of microstructural cord affection in cervical spondylotic myelopathy (CSM) patients with no detectable intramedullary hyperintensity in the conventional T2 WIs. Patients and Methods: This study is a prospective study. 60 patients presented with neurological symptoms and signs of cervical compressive myelopathy with cervical spondylotic changes and normal cord signal on conventional T2 WIs were included in this study. The clinical severity of compressive myelopathy was assessed based on the mJOA score. MR diffusion tensor imaging was done for all patients. FA and ADC values of the cervical cord at level of most severe disc lesions and at non-stenotic level (C2-3, considered as internal reference) were measured and compared. The DTI parameters at level of most severe disc lesions were also correlated with disc severity score and mJOA score. Results: At the level of most severe disc lesions; we found statistically significant reduction of FA values (mean 0.57 ±0.10 vs. 0.67±0.06 with p-value <0.001) and increase of ADC values (mean 1.02±0.34 vs. 0.88±0.31 with p-value 0.015) compared to non-stenotic reference level. FA & ADC parameters had high sensitivity for detection of early myelop-athy in CSM patients with normal cord signal in conventional T2WIs (sensitivity 86.66% and 80% respectively). FA and ADC values show significant correlation with both disc severity (p-value <0.001 & 0.008) and clinical function mJOA score (p-value 0.015 & 0.031 respectively).Conclusion: MR diffusion tensor imaging using its quan-titative FA and ADC parameters is more sensitivity than conventional T2 WIs in detection of early microstructural cord affection in CSM patients and could help as an early detector biomarker of cervical myelopathy before the appear-ance of any detectable signal abnormalities on T2 WIs. Our study enhanced the utility of DTI in cases with CSM to solve the clinical/radiological mismatch; enabling the decision of appropriate timing of cervical decompression surgery before the chronic irreversible myelopathy changes become settled.

Abstract:Background Cervical spondylotic myelopathy (CSM) is a common cause of disability in elderly patients. Previous studies have shown that spinal cord cell apoptosis due to spinal cord compression plays an important role in the pathology of myelopathy. Although changes in magnetic resonance imaging (MRI) T2 signal intensity ratio (SIR) are considered to be an indicator of CSM, little information is published supporting the correlation between changes in MRI signal and pathological changes. This study aims to testify the correlation between MRI T2 SIR changes and cell apoptosis using a CSM animal model. Methods Forty-eight rabbits were randomly assigned to four groups: one control group and three experimental chronic compression groups, with each group containing 12 animals. Chronic compression of the cervical spinal cord was implemented in the experimental groups by implanting a screw in the C3 vertebra. The control group underwent sham surgery. Experimental groups were observed for 3, 6, or 9 months after surgery. MRI T2-weighted SIR Tarlov motor scores and cortical somatosensory-evoked potentials (CSEPs) were periodically monitored. At each time point, rabbits from one group were sacrificed to determine the level of apoptosis by histology (n=6) and Western blotting (n=6). Results Tarlov motor scores in the compression groups were lower at all time points than the control group scores, with the lowest score at 9 months (P <0.001). Electrophysiological testing showed a significantly prolonged latency in CSEP in the compression groups compared with the control group. All rabbits in the compression groups showed higher MRI T2 SIR in the injury epicenter compared with controls, and higher SIR was also found at 9 months compared with 3 or 6 months. Histological analysis showed significant apoptosis in the spinal cord tissue in the compression groups, but not in the control group. There were significant differences in apoptosis degree over time (P <0.001), with the 9-month group displaying the most severe spinal cord apoptosis. Spearnnan's rank correlation test showed that there was close relation between MRI SIR and degree of caspase-3 expression in Western blotting (t=0.824. P<0.001). Conclusions Clear apoptosis of spinal cord tissue was observed during chronic focal spinal compression. Changes in MRI T2 SIR may be related to the severity of the apoptosis in cervical spinal cord.
The Clinical Efficacy of Magnetic Resonance Imaging in Neuroimaging

Abstract:PURPOSE: To assess the clinical efficacy of magnetic resonance imaging (MRI) for neuroimaging and to provide guidelines for clinical practice.STUDY SELECTION: After a comprehensive literature search, studies of the diagnostic accuracy of MRI alone or compared with other tests and of any impact on therapeutic choices or patient outcomes were reviewed by independent readers, followed by discussion to reach consensus conclusions.DATA EXTRACTION: Of 3125 citations retrieved, 156 studies with original data could be rated according to methodologic criteria for study design. One article contributed grade A quality information about diagnostic accuracy, 28 were graded B or C, and 113 were graded D. One randomized trial and 2 comparison studies contributed grade B or C information about the impact on therapeutic choices. Only 2 studies surveyed health status before and after magnetic resonance scanning.RESULTS: For most abnormalities, the sensitivity of MRI is equal to or better than competing technologies. Magnetic resonance imaging shows greater contrast and detail than computed tomography (CT) but also shows more clinically silent abnormalities or incidental findings. A few studies found a modest impact on therapeutic choices but no impact on quality of life or disability. Costs for MRI are high. Computed tomography is sufficient for initial diagnosis of most mass lesions or intracranial hemorrhages requiring immediate intervention. Magnetic resonance imaging is more accurate in the temporal lobes, posterior fossa, brainstem, and spinal cord. For lumbar radiculopathy, MRI and plain spinal CT are as accurate as post-myelographic CT and are less invasive. The role of magnetic resonance angiography for carotid artery stenosis is being studied.CONCLUSIONS: Although suggestions for appropriate use of MRI in clinical practice can be made, the supporting evidence in published studies is weak. Firm guidelines for appropriate use of MRI should be based on further clinical research using more rigorous methods.

Abstract:Background: Complex degenerative cervical spondylotic myelopathy (DCM) is characterized by a variety of complex imaging features. The surgical method for DCM remains controversial. This study aimed to examine the correlation between the imaging characteristics of DCM with varying degrees of complexity and the surgical approach and clinical outcome. Methods: A retrospective cohort study involving retrospective data collection was performed. A total of 139 patients with DCM who underwent surgery between January 2015 and January 2018 in the Orthopedics Department of Shanxi Bethune Hospital were divided into 3 groups according to the complexity of imaging features: 18 patients in the mild group, 66 patients in the moderate group, and 55 patients in the severe group. The Visual Analog Scale (VAS) and Japanese Orthopaedic Association (JOA) scores were used to compare the effects of neck pain and neural function prior to surgery according to the rate of improvement as of the last follow-up. Routine X-ray films were obtained at the follow-up of 3-6 months. The necessity of computed tomography (CT) and magnetic resonance imaging (MRI) examinations was determined based on clinical findings and X-ray images. Analysis of variance (ANOVA) was used to compare groups, the least significant difference (LSD) test was used for multiple comparisons, and the Chi-square test was used to compare classification indicators (imaging manifestations, gender), with P<0.05 being statistically significant. Binary logistic regression analysis was performed to determine the primary influencing factors of the JOA recovery rate. Results: In all three groups, JOA and VAS scores at the final follow-up were significantly higher than those before surgery (P<0.001). There were significant differences in the preoperative VAS and JOA scores between any two groups, as well as in the VAS and JOA scores and improvement rates at the last follow-up between the mild group and the moderate group and between the mild group and the severe group (P<0.001). Age, preoperative JOA scores, MRI intramedullary hyperintensity signal, and the degree of spinal cord compression were primarily related to the nervous system recovery rate (P<0.001). Conclusions: Age, MRI intramedullary hyperintensity signal, degree of spinal cord compression, and other variables were associated with the improvement of neural function in patients with DCM. Therefore, in addition to the JOA improvement rate or VAS score, additional factors, such as the patient's condition, the improvement in quality of life, and the patient's financial capacity, should be considered in evaluating the improvement of postoperative neck pain and neural function.

Abstract:Background Diffusion magnetic resonsance imaging (dMRI) can potentially predict the postoperative outcome of cervical spondylotic myelopathy (CSM). Purpose To explore preoperative dMRI parameters to predict the postoperative outcome of CSM through multifactor correlation analysis. Study Type Prospective. Population Post‐surgery CSM patients; 102 total, 73 male (52.42 ± 10.60 years old) and 29 female (52.0 ± 11.45 years old). Field Strength/Sequence 3.0 T/Turbo spin echo T1/T2‐weighted, T2*‐weighted multiecho gradient echo and dMRI. Assessment Spinal cord function was evaluated using modified Japanese Orthopedic Association (mJOA) scoring at different time points: preoperative and 3, 6, and 12 months postoperative. Single‐factor correlation and t test analyses were conducted based on fractional anisotropy (FA), mean diffusivity, intracellular volume fraction, isotropic volume fraction, orientation division index, increased signal intensity, compression ratio, age, sex, symptom duration and operation method, and multicollinearity was calculated. The linear quantile mixed model (LQMM) and the linear mixed‐effects regression model (LMER) were used for multifactor correlation analysis using the combinations of the above variables. Statistical Tests Distance correlation, Pearson's correlation, multiscale graph correlation and t tests were used for the single‐factor correlation analyses.
