Abstract:The main objective of this research is to study the relationships of offense-supportive cognitions and sexual fantasies with sex crime. The research involved 48 men: 26 convicted of sexual offenses against minors and 22 convicted of sexual offenses against adults from different prisons in the Community of Madrid, Spain. We used the RAPE Scale and the Sex With Children Scale to evaluate offense-supportive cognitions and an ad hoc adaptation of the Multidimensional Developmental, Sex and Aggression Inventory to evaluate sexual fantasies with minors and sadomasochistic fantasies. The results show that both groups present similar offense-supportive cognitions, while each group had significantly more sexual fantasies related to their specific crime. Participants who had sexual fantasies about minors presented significantly more offense-supportive cognitions justifying child sexual abuse than those who did not present these fantasies, while participants with sadomasochistic fantasies did not present more offense-supportive cognitions about rape. After collecting this information, we ran four mediation models to assess potential relationships between fantasies, offense-supportive cognitions, and specific sexual crime. The mediation models showed that both sexual fantasies with minors and sadomasochistic fantasies had direct relationships with sex crimes. Upon further confirmation with studies with larger sample sizes, our findings support the importance of dealing with sexual fantasies in treatment of people convicted of sexual offenses and imply a need for differentiated treatment, since the content of sexual fantasies was different in each group.
Animal models of chronic obstructive pulmonary disease exacerbations.

Abstract:Modeling acute exacerbations of chronic obstructive pulmonary disease (AECOPD) in animals has proven challenging due to the clinical and pathological complexity of the underlying disease. This has hindered the progress in understanding the cellular and molecular mechanisms that lie beneath AECOPD. In this chapter, we will address modeling possibilities of AECOPD that may be drawn from the current knowledge of factors that cause exacerbations. Importantly, since it is widely accepted that the most common causes of AECOPD are viral and bacterial infections, animal models of AECOPD should incorporate both the causative agents of exacerbation: viruses and bacteria. However, other factors that are also believed to determine both progression of COPD, as well as the frequency and severity of AECOPD, such as proteolytic enzymes, cigarette smoke or other noxious stimuli must also be considered. Such animal models will provide mechanistic insight into the etiology of AECOPD and will prove invaluable in furthering our understanding of key events in disease pathogenesis.
A Qualitative Analysis of Sexual Fantasies Among Childhood Sexual Abuse Survivors and Non-Victims

Abstract:Research suggests childhood sexual abuse (CSA) has an impact on survivor's sexual fantasies; however, empirical research is inconclusive. As sexual fantasies play a crucial role in sexual well-being, more exploration of sexual fantasies in CSA survivors and non-victims is needed. An online survey-based content analysis was conducted on fantasies reported by CSA survivors ( n = 48) and non-victims ( n = 44). Results unveiled nine fantasy categories reported by both groups. An additional category, versatility , was reported solely by survivors. These findings highlight the diverse fantasy content experienced by survivors, prompting a reconsideration of theories suggesting a linear relationship between CSA and the development of sadomasochistic or violent fantasies in adulthood.

[Nontuberculous mycobacterial lymphadenitis with an emerging pathogen and a novel management approach].

Abstract:Nontuberculous mycobacterial infections in immunocompetent children usually presents as chronic lymphadenitis involving the neck and face. Mycobacterium avium complex is the most common pathogen, although recent series found Mycobacterium haemophilum, to be a major cause of chronic lymphadenitis in otherwise healthy children. The authors describe a 9-month-old baby who presented with a 4-month history of preauricular lymphadenitis. Mycobacterial culture yielded Mycobacterium haemophilum. A no-intervention approach was chosen. At the 6-month follow-up, the swelling had ameliorated and the skin showed a small scar with near-normal overlying skin color. A literature review of the clinical manifestations and diagnosis of Mycobacterium haemophilum lymphadenitis and of the different management options for nontuberculous mycobacterial lymphadenitis in otherwise healthy children is presented.
Links between Aggressive Sexual Fantasies and Sexual Coercion: A Replication and Extension of a Multifactorial Model

Abstract:Current research indicates that aggressive sexual fantasies (ASF) are related to sexual aggression, above and beyond other risk factors for this behavior. There have, however, rarely been explicitly considered in multifactor models aiming to explain sexual aggression. One exception is the multifactorial Revised Confluence Model of Sexual Aggression that was replicated in two samples of male individuals who were convicted of sexual offenses and a small sample of men from the general population and evidenced a high relevance of ASF, respectively. There were, however, no further attempts to replicate the model in larger samples from the general population. We, therefore, used a subsample from the Finnish Genetics of Sexuality and Aggression project including 3269 men (age: M = 26.17 years, SD = 4.76) to do so. Cross-sectional latent structural equation models corroborated previous research and the assumption that ASF are a central component in multifactor models that aim to explain sexual aggression: ASF and antisocial behavior/aggression were equally important associates of sexual coercion when also considering adverse childhood experiences, hypersexuality, and callous-unemotional traits. Additionally, ASF mediated the links between hypersexuality, callous-unemotional traits, as well as childhood sexual abuse and sexual coercion. These links held stable when entering further risk factors, that is, distorted perceptions, rape-supportive attitudes, and violent pornography consumption into the model. Contrasting assumptions, alcohol consumption and antisocial behavior/aggression did not interact. These results illustrate the potential importance of ASF for sexual aggression. They indicate that ASF require consideration by research on sexual aggression as well as in the treatment and risk assessment of sexual perpetrators.
Do internet-based sexual offenders reduce to normal, inadequate and deviant groups?

Abstract:How heterogeneous men who download paedophilic content from the Internet are is unknown. Cluster analysis of 422 Internet-based sexual offenders examined whether they fell into clear groups according to their scores on a standard psychometric screening battery. Three clusters were identified; the apparently normal (near or within the normal range on all measures), the inadequate, and the deviant. Though the inadequate cluster had clear socio-affective deficits, they were not high in pro-offending measures. The deviant group were characterised by very poor victim empathy. Test differences between clusters were statistically significant. These findings were supported by a factor analysis which revealed two underlying test dimensions, emotional inadequacy and deviancy, upon which the inadequate and deviant were respectively raised. Emotional inadequacy correlated more with impulsivity than did deviancy. These results were not attributable to social desirability. Our clusters are equivalent to contact sexual offender groupings, and support general interventions with such persons.

Abstract:A group of sex offenders (clinical group: n = 19) was compared to a nonclinical sample matched by age, years of education, and gender (control group A: n = 19) to verify a higher incidence of insecure attachment models among sex offenders. In addition, we tested whether sex offenders were characterized by specific childhood experiences, compared to control adults (control group B: n = 19) with the same secure/insecure attachment classification. Results showed significant differences between offenders and control adults on both the AAI continuous score and the distribution of the two-way attachment classifications. Furthermore, sex offenders reported more intense experiences of rejection by the father figure and abuse in the family context during early childhood compared to not offenders subjects with the same attachment classification.

Abstract:Sex offenders and non-sexual violent offenders display distinct cognitive distortions that shape their offenses. Some evidence suggests that early maladaptive schemas which reflect individuals' fundamental cognitions and beliefs, contribute to these cognitive distortions. However, these have not been systematically appraised. We conducted a systematic review to synthesize the evidence regarding the schema profiles of sex offenders and non-sexual violent offenders, and to explore differences in schemas between these offender types. Eight studies met the inclusion criteria. Within the sex offender category, rapists and child sex offenders endorsed similar schemas. For non-sexual violent offenders, schemas related to self-control issues and self-entitlement were most prominent. Sex offenders demonstrated more schemas that endorsed negative perceptions of themselves and others than violent offenders. Findings also suggest that atypical sexual interests and mental disorders may be important. This evidence highlights the need for a more fine-tuned understanding of the cognitive profiles of sexual and non-sexual violent offenders and for developing schema-based offender management and rehabilitation services.

[Phenomenology and history of the origin of exhibitionism]

Abstract:Exhibitionism should not be misunderstood as a throw back into phylogenetically highly primitive "sexual advertising". The issue is also missed if one shares the widespread belief that at least some of the cases could be explained by an abnormally increased sexual drive. This sexual deviation mostly appears for the first time at the turn of the second to the third decade of life. The relatively close association with age is not conspicuous when we take a look at first police reports or at the sum total of culpable criminal offences. Only about one-tenth of the cases concern late delinquency beyond 35 or 40 years of age. More than 75% of the adult delinquents had married. Marriage practically never implied a turning-point within the sexual maldevelopment, perhaps also largely due to the specific choice of the spouse. It is more likely that the original conflicts are even aggravated as a result of the confrontation with the marriage partner. There is a sharp contrast between the general tendency on the part of the exhibitionists to follow a set behavioural pattern which is stereotyped and rather mechanical, on the one hand, and the wide variety of "victims" selected by many of these delinquents on the other. There is also a noticeable affinity to theft and misappropriation of property--a criminologically interesting aspect. Among the factors contributing to a lack of self-concept and deficient feeling of male identity, the following have been determined to act in combination with other negative influences: The delinquents grow up in a large family comprising many brothers and sisters, a highly significant factor being a predominance of boys among the siblings; often, there is no positive male reference person as would be represented by the father; or the patient is himself an illegitimate offspring. There may often be a physical malformation or handicap acting as a kind of social stigma. This can result in amplifying the subjective feeling of rejection or non-acceptance. Conflicts of the type of a dysmorphophobia were in far more than one-third of the examined 40 patients contributory to the prerequisites of sexual anomaly. Lack of confidence in one's own capacities is one of the main factors in preventing the delinquents from being professionally as successful as their fathers--which would offer a chance of raising their self-esteem. Self-limitation because of strong aggressive inhibition seems to be the main reason for the albeit slight deterioration in status-determined behaviour and hence social status.(ABSTRACT TRUNCATED AT 400 WORDS)
Significance, Nature, and Direction of the Association Between Child Sexual Abuse and Conduct Disorder

Abstract:To elucidate the significance, nature, and direction of the potential relationship between child sexual abuse and conduct disorder, all the pertinent studies were reviewed. Ten databases were searched. Blind assessments of study eligibility and quality were performed by two independent researchers. Thirty-six studies including 185,358 participants and meeting minimum quality criteria that were enough to ensure objectivity and to not invalidate results were analyzed. Across the majority of studies, conduct disorder was significantly and directly related to child sexual abuse, especially repeated sexual molestation and abuse involving penetration, even after controlling for various sociodemographic, family, and clinical variables. The association between child sexual abuse and conduct disorder was not confounded by other risk factors, such as gender, socioeconomic status, school achievement, substance problems, physical abuse, parental antisocial behavior or substance problems, parent–child relationships, and family disruption, conflict, or violence. Evidence for a significant interactive effect between child sexual abuse and monoamine oxidase A gene on conduct disorder was scant. Early sexual abuse might predispose to the subsequent onset of conduct disorder which, in turn, may lead to further sexual victimization through association with sexually abusive peers or involvement in dangerous situations or sexual survival strategies.
Childhood Attachments, Sexual Abuse, and Their Relationship to Adult Coping in Child Molesters

Abstract:The present study examined the parent-child attachments, typical coping styles, and childhood sexual abuse among 30 child molesters, 24 nonsexual offenders, and 29 nonoffenders. The results indicated that all subjects reported greater security in their attachments to their mothers than to their fathers and the insecure patterns of childhood attachments were related to ineffective adult coping. The only difference observed in characteristic coping showed that child molesters were more likely to engage in emotion focused strategies. Child molesters reported having experienced high levels of childhood sexual abuse and these experiences appear to have been more distressing to them than to other subjects. The results are discussed in terms of their implications for theory and treatment.
Fantasy Sexual Material Use by People with Attractions to Children

Abstract:Purpose of review: With the Internet allowing consumers easy access to fantasy and fictional sexual materials (FSM), it is becoming increasingly important to understand the context of their use among specific populations. Of particular, social, clinical, and legal interest is FSM use by people who are attracted to children and whether this may have a risk-enhancing or protective impact on their likelihood of committing a contact or non-contact sexual offence. Recent findings: There is a lack of data currently available in relation to the use of FSM by those with sexual attractions to children. Evidence from allied areas appears to show no meaningful associations between FSM use and sexual aggression. We propose a novel research program and some initial research questions that provide a theoretical framework for more evidence-based inquiry on FSM use by people who experience attractions to children.
Discriminating Coercive from Sadomasochistic Sexuality

Abstract:Sadomasochistic (SM) sexual interest is reported by a wide range of individuals. Within the sex offender literature, the presence of SM interest is often viewed as a herald of criminal sexual behavior; however, research indicates that SM interests are not predictive of coercive sexual behavior. In the current study, we measured a range of sexual fantasies and behaviors, and then applied cluster analyses to determine (1) if individuals endorsed elevated SM interests also endorsed coercive fantasies and, (2) to explore cluster membership correlates using measures of sensation seeking, externalizing and antisocial behaviors, attitudes toward rape, and empathic capacity. A total of 550 participants were recruited through a variety of on-line discussion areas. A four cluster solution best fit the data with distinct clusters observed for those endorsing SM behaviors and those reporting coercive sexual behavior. Additional analyses revealed greater endorsement of victim blaming attitudes and lower empathic concern among members of the coercive sexual behavior cluster. Elevated sensation seeking and externalizing behaviors were reported by members of clusters marked by SM and coercive sexual behavior. Results provide further support for the differentiation between SM and coercive sexual behaviors as representative of individuals with divergent attitudes, traits, and behaviors.
Childhood Attachment and Adult Attachment in Incarcerated Adult Male Sex Offenders

Abstract:Forty-eight incarcerated sex offenders were compared with 16 property offenders and 16 nonoffenders on self-report measures of childhood maternal and paternal attachment and adult attachment. The combined sex-offender groups reported significantly less secure maternal, paternal, and adult attachment than did the nonoffenders and significantly less secure maternal attachment than did the property offenders. Intrafamilial child molesters were found to have had particularly problematic relationships with their mothers, reporting a combination of anxious and avoidant qualities in their maternal attachment experiences. By contrast, stranger rapists were found to have had particularly problematic relationships with their fathers and were significantly more likely to have regarded their fathers as having been characteristically unsympathetic, uncaring, abusive, and violent toward them. These results suggest that insecure childhood attachments may be related to offending behavior generally and that certain combinations of childhood attachment experiences may relate more specifically to different kinds of sexual offending.

Abstract:Predictability of dangerousness in association with mental disorders remains elusive, outside of a few relatively well-established risk factors for the prognostication of violence, such as male sex, the presence of a psychotic disorder, and comorbid substance abuse. In clinical practice, inquiry into the presence of aggressive or violent ideation, in the form of ideas of homicide or suicide, is part of a standard mental status examination. Nonetheless, fantasy life, when it concerns harm toward others, may not be as reliable an indicator of imminent danger as it may be in the case of self-harm. Five cases of young Italian men with Asperger syndrome and recurrent and extremely violent femicide fantasies are presented. While there is no direct correlation between autism spectrum conditions and violence, as other humans, persons with an autistic condition are capable of committing crimes, including homicide. All five had in common a number of characteristics and behaviors felt to be pathoplastic: All had been bullied, all had been romantically rejected, all were long-standing First Person Shooter (FPS) game players, and all were avid violent pornography consumers. The potential for an actual neurocognitive impact of violent video games, well documented in the literature, and its combination with personal life history and chronic habituation following long-standing violent pornography use is discussed in the context of social and emotional vulnerabilities. While aggressive fantasies cannot and should not be underestimated, in countries where duty to protect legislation does not exist, a clinical approach is imperative, as, incidentally, should be anywhere.

Paraphilic Interests Versus Behaviors: Factors that Distinguish Individuals Who Act on Paraphilic Interests From Individuals Who Refrain

Abstract:Little is known about distinct factors linked with acting on paraphilic interests or refraining from engaging in paraphilic behaviors. Participants from Canada and the United States (N = 744), aged 19-42 years (M = 29.2; SD = 3.18), were recruited through Amazon's Mechanical Turk. Participants completed questionnaires about their paraphilic interests and behaviors, as well as potential key factors linked to behavioral engagement (i.e., perceptions of consent, sexual excitation/inhibition, impulsivity, moral disengagement, empathy). Results indicated that higher moral disengagement and impulsivity, lower sexual control (i.e., high sexual excitation, low sexual inhibition), and maladaptive understandings of consent were best able to differentiate individuals who reported highly stigmatized (e.g., hebephilia, pedophilia, coprophilia) or Bondage and Dicipline, Dominance and Submission, Sadism and Masochism(BDSM)/Fetish paraphilic interests and engagement in the paraphilic behaviours associated with these interests relative to individuals who did not report such paraphilic interests or behaviors. Moreover, higher moral disengagement, impulsivity, and maladaptive perceptions of consent were best able to differentiate non-consensual paraphilic interests and behaviours (e.g., voyeurism, exhibitionism) compared to individuals who did not report these paraphilic interests or behaviours. These results provide future directions for the exploration of mechanisms that may contribute to engagement in paraphilic behaviors and may be targets for intervention aimed at preventing engagement in potentially harmful paraphilias.

Abstract:A novel orientation-neutral Hypersexuality and Risky Sexual Behaviors Scale provided evidence consistent with the idea that both sexual addiction and a low interest in sex as an adult have their origins during childhood and adolescence. Adult interest in sex and the likelihood of engaging in risky sexual behaviors tended to be increased if participant's first experiences with masturbation and partner sex had occurred early in life. Conversely, adult interest in sex tended to be lowest when neither masturbation nor sex with a partner had occurred prior to 18 years of age. Both findings were consistent with critical period learning.

Abstract:People with sexual attractions to children are often subject to heavy stigmatization, and several studies have started to look at how this affects their mental health. This is likely due to a conflation with offending and sexual risk within society, which translates into academic work on the topic. In this context, little is known about how the experiences of non-offending individuals with attractions to children inform their treatment needs in non-forensic contexts. We conducted anonymous semi-structured interviews with large sample of 31 non-offending adult men who are attracted to children, recruited through online forums. The results focus on two superordinate themes central to interpersonal and intrapersonal experiences, and the effects of these on both everyday functioning ("Living with a sexual interest in children"), and perceived treatment needs ("Establishing treatment targets"). These findings hold significance for the effective design and delivery of both preventative and healthcare-related support services, particularly in relation to early intervention and assisting this population in living full lives with their sexual attractions.
Impact of esophageal screening in patients with head and neck cancer.

Abstract:We have reviewed the records of 1,982 patients who were treated for head and neck cancer or esophageal cancer from 1962 to 1986. Forty-one patients had primary cancer at both sites. Twenty-one cases of these multiple primaries occurred synchronously and twenty were metachronous. The overall incidence of esophageal cancer in our head- and neck-cancer patients was 2.5 per cent and the incidence of head and neck cancer in our esophageal cancer patients was 7.1 per cent. From 1980 to 1986, 574 cases with a diagnosis of head and neck or esophageal cancer were routinely screened for other aerodigestive malignancies at the time of initial diagnosis. From this group, only six patients had simultaneous lesions of the head and neck and esophagus with only one asymptomatic esophageal carcinoma. Median survival of all 41 multiple primary patients after diagnosis of esophageal cancer was 5.3 months. Two- and three-year survivals were 6.7 per cent and 0 per cent, respectively. There was no significant survival difference for lesions diagnosed simultaneously, synchronously, and metachronously before 1980 or after 1980. All patients died with uncontrolled esophageal cancer except for one patient who died of head- and neck-cancer recurrence. Our experience indicates that active screening of head- and neck-cancer patients for simultaneous esophageal cancer has a low yield and there appears to be no survival advantage for these patients compared with those with subsequently diagnosed esophageal tumors.
