Abstract:Our aim in this study was to investigate the changes of serum high-sensitive C-reactive protein (hs-CRP) and uric acid (UA), and evaluate the synergistic effect of amlodipine and atorvastatin on blood pressure and left ventricular remodeling in hypertensive patients with primary hypercholesterolemia. One hundred and twenty-six hypertensive patients with hypercholesterolemia were randomized into amlodipine group (10 mg/day, group A, n = 65) and amlodipine (10 mg/day) plus atorvastatin group (20 mg/day, group B, n = 61), treated for 4 months continuously. Serum concentrations of total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, triglycerides, hs-CRP, and UA were determined, and blood pressure of both groups was examined before and after treatment. Left ventricular posterior wall thickness and interventricular spectum thickness were measured by echocardiography, and left ventricular mass index (LVMI) was calculated. After 4-months of treatment with atorvastatin, serum concentrations of total cholesterol, low-density lipoprotein cholesterol, triglycerides, hs-CRP, and UA were significantly decreased in group B ( P < 0.05, P < 0.01), while serum concentrations of high-density lipoprotein cholesterol was elevated ( P < 0.05). Meanwhile, systolic blood pressure and diastolic blood pressure were reduced in both groups ( P < 0.05), and blood pressure in group B was markedly lower than that in group A after treatment ( P < 0.05). Compared with that before treatment, LVMI in both groups decreased ( P < 0.05), to a significantly lower degree in group B than in group A ( P < 0.05). Atorvastatin can decrease serum concentrations of hs-CRP and UA. The amlodipine-atorvastatin combination markedly reduces blood pressure and reverses left ventricular hypertrophy more than amlodipine monotherapy. The positive effect suggests that in hypertensive and hypercholesterolemic patients, the combination of amlodipine and atorvastatin could be the treatment of choice.
Efficacy and Safety of the Compound of Candesartan Cilexetial and Hydrochlorothiazide in Patients with Essential Hypertension

Abstract:Objective To evaluate the efficacy and safety of combination of candesartan cilexetial and hydrochlorothiazide (16.0 mg/12.5 mg) in patients with mild and moderate hypertension.Methods A 8 weeks,multicenter, double-blind,randomized,parallel-group study was carried out.After 2 weeks wash-out phase,treatment started with once-daily 8 mg candesartan cilexetil in the 392 patients for 4 weeks(phase 1 study);if target sitting diastolic BP(SeDBP90 mm Hg) was not achieved,the dose were titrated to candesartan 16 mg alone or candesartan 16.0 mg/hydrochlorothiazide 12.5 mg pill once daily for 8 weeks(phase 2 study).Results The reduction of BP were 10.2±10.6/6.5±5.7 mm Hg after 4 weeks treatment of 8 mg candesartan cilexetial alone(n=353),only 54 (15.3%) patients achieved the target DBP.During the consequential 8 weeks treatment,the antihypertensive effect of combined treatment(n=134) was much greater than that by candesartan alone(n=142)(11.1±11.2/10.7±6.6 mm Hg vs 7.8±11.1/7.8±6.3 mm Hg,P0.01).The number of patients who achieved the target DBP were 73/142(51.4%) by monotherapy vs 107/134(79.9%) by combined treatment(P0.01).Patients were well tolerated in both groups with no difference in drug-related adverse events.Conclusion The combined candesartan cilexetiain and hydrochlorothiazide treatment is more effective and well tolerated in patients with mild and molderate hypertension than that by candesartan monotherapy.
Beneficial Effects of Candesartan, an Angiotensin II Type 1 Receptor Blocker, on Β-Cell Function and Morphology in Db/db Mice

Abstract:Several epidemiological studies suggested that treatment with angiotensin II type 1 receptor blocker (ARB) provided a risk reduction of developing type 2 diabetes. In this study, we investigated whether and how ARB treatment can improve abnormalities of pancreatic islets in diabetes state. We randomized db/db mice, a model of type 2 diabetes with obesity, at the age of 8 weeks to receive candesartan, an ARB, for 6 weeks. We also studied age-matched db/misty mice as control. Glucose tolerance test revealed that candesartan treatment improved glucose tolerance with the modest increase in serum insulin level in db/db mice. Concurrently, candesartan increased beta-cell mass, increased staining intensity of insulin, and decreased staining intensity of components of NAD(P)H oxidase, p22phox and gp91phox, and those of oxidative stress markers in beta-cells. These changes were accompanied by reduction of mitochondrial volume. Treatment with candesartan also reduced fibrosis in and around the islets and prevented the loss of endothelial cells in islets. Our results showed that candesartan partially prevented deterioration of glucose tolerance by providing protection against progressive beta-cell damage in diabetes.

Abstract:Abstract Background and Aims Oxidative stress and activation of the renin-angiotensin system (RAS) due to chronic hyperglycemia are the pathogenesis of diabetic nephropathy (DN). Until now, therapy for DN has not shown satisfactory results. Rosmarinic acid (RA) which has antioxidant, anti-inflammatory, anti-fibrosis effects, and ARB which can inhibit RAS activation may have a synergistic effect to prevent DN progression. This study aims to determine the effect of inhibiting mesangial expansion in diabetic rat models after being given a combination of RA and ARB (candesartan or losartan). Method True experimental laboratory trial with randomized post-test only control group method on Rattus norvegicus strain Wistar rats. Diabetic rats were prepared by giving a high-fat diet (60% fat calories) and a single injection of 40 mg/kg streptozotocin. Mice were divided into 4 groups, each consisting of 4 rats: negative control (healthy rats); positive control (placebo diabetic rats); diabetic rats that received RA and candesartan; diabetic rats that received RA and losartan. The rat glomerulus was evaluated to assess mesangial expansion using ImageJ 1.48 software after 8 weeks of treatment by a pathologist blindly. Data analysis with One-way ANOVA and Tukey's post hoc tests. Results The results of the percentage of the mesangial matrix in the positive control group (81.49% ± 1.45) were higher than the negative control group (58.82% ± 0.99). The results of the percentage of the mesangial matrix in the RA + candesartan treatment group (61.56% ± 0.35) and the RA + losartan treatment group (62.28% ± 0.77) showed lower results than the positive control group, with the One-way ANOVA test results showing significant differences (p: 0.000). Tukey's post hoc tests showed the positive control group that received a high-fat diet and induced diabetes with STZ significantly increased mesangial expansion compared to negative control (MD: [22.66]; 95% CI: [20.62], [24.72]; p: 0.000). The RA + candesartan treatment group significantly inhibited mesangial expansion compared to the positive control (MD: [-19.92]; 95% CI: [-21.97], [-17.87]; p: 0.000), as well as the RA + losartan treatment group (MD: [-19.21]; 95%CI: [-21.26], [-17.16]; p: 0.000). The RA + candesartan treatment group showed a better effect in inhibiting mesangial expansion compared to the RA + losartan treatment group, but not significantly (p: 0.735). Conclusion The combination of rosmarinic acid with candesartan or losartan has the effect of inhibiting mesangial expansion in diabetic rats.

Endothelin Receptor Antagonist Combined with a Calcium Channel Blocker Attenuates Renal Injury in Spontaneous Hypertensive Rats with Diabetes.

Abstract:Objective To investigate the effects of the mixed endothelin receptor antagonist, bosentan, combined with the long-acting calcium channel blocker, amlodipine, compared to the angiotensin-converting enzyme inhibitor, cilazapril, on the progressive renal injury in spontaneous hypertensive rats (SHR) with diabetes.Methods Diabetic hypertensive rats (SHR-DM) were induced by streptozotozin injected in male SHR (7-week-old), and divided into an untreated and three treated groups: (1) cilazapril treated group; (2) bosentan + amlodipine treated group; and (a) amlodipine treated group. Wistar Kyoto rats (WKY) and SHR rats served as normotensive and hypertensive control, respectively. The mean arterial blood pressure, renal function, endothelin and angiotensin R levels as well as the protein expression of renal extracellular matrix components and transforming growth factor (TGF)-beta1 were determined at the end of the 4th week.Results Mean arterial blood pressure significantly increased in SHR and SHR-DM rats compared to WKY rats. All the therapies reduced the blood pressure to normal levels. However, the enhanced urinary protein excretion, the decreased creatinine clearance as well as the increased plasma and intrarenal endothelin and angiotensin II levels were found in the untreated SHR-DM and prevented by treatment with bosentan + amlodipine and cilazapril. Similarly, these two kinds of therapies in SHR-DM abolished the overexpression of renal TGF-beta1 by Western blot analysis and reduced the accumulation of collagen type IV, laminin and fibronectin proteins by an immunochemical approach. Amlodipine monotherapy had no detectable effects on the above parameters.Conclusion Bosentan combined with amlodipine can offer similar renoprotective effects on that of cilazapril and may be a potent therapy to attenuate renal injury by reducing renal protein levels of TGF-beta1 in diabetes with a hypertensive state.

Abstract:Objective: Angiotensin II type 1 receptor blockers (ARB) are widely recognized to have a vasculoprotective effect. Accumulating data have revealed that calcium antagonists also retard atherosclerosis. We examined the possibility that combination therapy of ARB and calcium antagonists could more effectively prevent atherosclerosis than monotherapy. Methods and results: We observed a marked increase in the atherosclerotic area, associated with the exaggerated expression of nicotinamide adenine dinucleotide (phosphate), reduced form [NAD(P)H] oxidase subunits (p22 and p47) and superoxide anion production, in the aorta of apolipoprotein E-deficient mice maintained on a 1.25% high-cholesterol diet for 10 weeks. A calcium antagonist, azelnidipine, at a dose of 1 mg/kg a day or an ARB, olmesartan, at a dose of 3 mg/kg a day, significantly inhibited these parameters, with no change in systolic blood pressure and the blood cholesterol level. Moreover, the co-administration of lower doses of azelnidipine (0.1 mg/kg a day) and olmesartan (1 mg/kg a day) significantly inhibited the atherosclerotic area and oxidative stress, whereas azelnidipine or olmesartan alone at these doses did not affect these parameters. Furthermore, we observed similar inhibitory effects of azelnidipine or olmesartan on angiotensin II-induced NADPH oxidase activity and Akt activity in cultured vascular smooth muscle cells. Conclusion: These results suggest that the co-administration of calcium antagonists and ARB synergistically blunts oxidative stress at least partly through the inhibition of Akt activity and enhances the beneficial effects of these drugs on atherosclerosis compared with monotherapy.
Angiotensin II receptor blocker provides pancreatic β-cell protection independent of blood pressure lowering in diabetic db/db mice

Abstract:Aim: Several epidemiological studies have suggested that treatment with angiotensin II type 1 receptor blocker provided a risk reduction of developing type 2 diabetes. The aim of this study was to investigate whether and how chronic candesartan treatment can attenuate the deleterious influence of the hyperactive local intra-islet renin-angiotensin system in the diabetes state. Methods: Eight-week-old db/db mice were randomized to candesartan 1 mg/kg, candesartan 10 mg/kg, manidipine 10 mg/kg, or placebo via gavage for 6 weeks. Their age-matched nondiabetic littermates db/m mice were treated with placebo and acted as nondiabetic controls. After 6 weeks’ treatment, an intraperitoneal glucose tolerance test, immunohistochemical staining of oxidative stress markers, insulin, CD31, azan staining and an electron microscopy observation were performed. Results: Chronic candesartan treatment provided an improvement of glucose tolerance, and greatly rescued islet β-cell mass. Candesartan treatment also notably decreased staining intensity of oxidative stress markers, as well as attenuating intra-islet fibrosis and improving blood supply in the islet. In the electron microscopy observation, candesartan-treated animals exhibited improved granulation and less remarkable endoplasmic reticulum and Golgi bodies; furthermore, candesartan treatment greatly relieved the swelling of mitochondria to nearly normal. Both the benefits of reducing oxidative stress and ultrastructure protection were in a dose-dependent and blood pressure-independent manner. Conclusion: After diabetes was initiated, candesartan treatment could not reverse the state of diabetes, but it effectively improved glucose tolerance and protected β-cell function by attenuating oxidative stress, islet fibrosis, sparsity of blood supply and ultrastructure disruption in a dose-dependent and blood pressure-independent manner.
Effects of combination of irbesartan and perindopril on calcineurin expression and sarcoplasmic reticulum Ca 2+ -ATPase activity in rat cardiac pressure-overload hypertrophy

Abstract:Aim: To observe effects of angiotensin (Ang) II receptor antagonist (AT1) irbesartan and angiotensin-converting enzyme (ACE) inhibitor perindopril on rat myocardium calcineurin expression and sarcoplasmic reticulum Ca 2+ -ATPase activity in the model of pressure-overload cardiac hypertrophy. Methods: Forty male adult Sprague Dawley rats were divided into 5 groups. One group was treated by sham operation; four groups were myocardium hypertrophy cases caused by banding aortic above renal artery. Drugs were given one week after operation. Group 1: sham group, rats ( n =8) were gavaged with normal saline 2 ml/(kg·d) (ig); Group 2: control group, rats ( n =8) were treated with normal saline 2 ml/(kg·d) (ig); Group 3: rats ( n =8) were given perindopril 2 mg/(kg·d) (ig); Group 4: rats ( n =8) were treated with irbesartan 20 mg/(kg·d) (ig); Group 5: rats ( n =8) were given irbesartan 20 mg/(kg·d) plus perindopril 2 mg/(kg·d) (ig). Morphometric determination, calcineurin expression and sarcoplasmic reticulum Ca 2+ -ATPase activity were done at the end of 6 week of drug intervention. Expression of calcineurin in myocardium was detected by immunohistochemistry. Results: Left ventricular mass index (LVMI), transverse diameter of myocardial cell (TDM), calcineurin activity were remarkably decreased after drug intervention and this decrease was most remarkable in the combination drug therapy group. Sarcoplasmic reticulum Ca 2+ -ATPase activity was increased after drug intervention, especially in the combined drug therapy group. Calcineurin expression in myocardium was remarkably decreased after drug intervention. LVMI was positively correlated with TDM and calcineurin, negatively correlated with sarcoplasmic reticulum Ca 2+ -ATPase. Conclusion: These data suggest that irbesartan and perindopril inhibit cardiac hypertrophy through the increased activity of sarcoplasmic reticulum Ca 2+ -ATPase and decreased expression of calcineurin. Their combination had better effects on regressing of ventricular hypertrophy.
Candesartan-Induced Gene Expression in Five Organs of Stroke-Prone Spontaneously Hypertensive Rats

Abstract:To test the functional consequences of blocking the local renin-angiotensin system (RAS), we investigated the effects of an angiotensin II type 1 receptor blocker (ARB), candesartan, on the systemic gene expression profile of five important organs (brain, heart, kidney, liver and adipose tissues) in the stroke-prone spontaneously hypertensive rat (SHRSP), an established model of essential hypertension and cardiovascular disorders, and its normotensive control, the Wistar Kyoto (WKY) rat. Rats were treated with candesartan (5 mg/kg/d) for 4 weeks from 12 to 16 weeks of age. DNA microarray technology was used to identify changes in gene expression. Four weeks of treatment with candesartan significantly lowered systolic blood pressure in male rats of both the SHRSP and the WKY strains (p<0.0005). Candesartan differentially modulated the gene expression profile in an organ-specific manner in male SHRSP; of the five organs tested, gene expression was most prominently altered in the hearts of SHRSP. In contrast, candesartan treatment exerted minimal or no significant effects on the gene expression profile of the corresponding organs of male WKY rats. The inter-strain differences in gene expression changes induced by candesartan were considered to be associated with both blood pressure–dependent and independent mechanisms. These results help to delineate the mechanisms that underlie the organ or tissue protection conferred by ARB at the levels of cellular biology and genomics in the context of the local RAS. Further studies are warranted to investigate not only individual genes of interest but also genetic “networks” that involve differential organ- or tissue-specific gene expression induced by the blockade of RAS in essential hypertension.

O4‐01‐04: Beneficial Effects of Angiotensin II Receptor Blockers on Metabolic Disorder‐induced Cognitive Impairment

Abstract:Recent basic and clinical evidence indicate that renin-angiotensin system (RAS) is involved in metabolic syndrome and diabetes mellitus (DM) as well as hypertension. Metabolic syndrome and DM have been reported to be crucial risk factor for cognitive impairment including Alzheimer's disease. Increasing number of patients with metabolic syndrome and DM suggests that such cognitive decline will become a major worldwide clinical problem in the future. Here, we investigated the roles of angiotensin II and effect of angiotensin II receptor blockers (ARB) in cognitive function. Various mice models were employed. C57BL/6J mice with normal diet (ND), C57BL/6J mice with high-salt and high-cholesterol diet (HCD) as metabolic syndrome-like state, KK-Ay mice as type 2DM, and Tsukuba-Hypertensive-Mice (THM) as human angiotensin II overproducing model were employed in this study. These mice were subjected to passive avoidance task every week with or without ARB. 1) HCD mice showed a significant reduction of avoidance rate compared with ND mice. This impairment was prevented by treatment with non-hypotensive dose of ARB, olmesartan, partly due to a decrease in superoxide anion production. 2) Learning ability evaluated by an increase in avoidance rate was less in KK-Ay mice and significant cognitive impairment was observed after 14 weeks-old. Administration of non-hypotensive dose of candesartan from 8 weeks-old markedly improved learning ability and its administration from 14 weeks-old prevented further cognitive decline. Candesartan significantly decreased serum glucose and insulin levels and increase glucose-uptake in the brain. 3) THM exhibited a failure to increase the avoidance rate compared with control mice. Treatment with ARB, olmesartan, which decreases blood pressure to the same level as C57BL/6J, improved the avoidance rate. However, treatment with hydralazine for the THM did not show an improvement of avoidance rate although hydralazine decreased blood pressure to the same level of olmesartan treatment. Olmesartan attenuated superoxide anion production in the brain and improved the reduction of cerebral blood flow observed in THM. These results suggest that brain RAS plays an important role in cognitive function. Regulation of RAS using ARB is expected to prevent cognitive decline associated with metabolic syndrome, DM and hypertension.

Abstract:Background: We investigated the effects of candesartan (an angiotensin II antagonist) alone, enalapril alone, and their combination on exercise tolerance, ventricular function, quality of life (QOL), neurohormone levels, and tolerability in congestive heart failure (CHF). Methods and results: Seven hundred sixty-eight patients in New York Heart Association functional class (NYHA-FC) II to IV with ejection fraction (EF) <0.40 and a 6-minute walk distance (6MWD) <500 m received either candesartan (4, 8, or 16 mg), candesartan (4 or 8 mg) plus 20 mg of enalapril, or 20 mg of enalapril for 43 weeks. There were no differences among groups with regard to 6MWD, NYHA-FC, or QOL. EF increased (P=NS) more with candesartan-plus-enalapril therapy (0.025+/-0.004) than with candesartan alone (0.015+/-0.004) or enalapril alone(0.015+/-0.005). End-diastolic (EDV) and end-systolic (ESV) volumes increased less with combination therapy (EDV 8+/-4 mL; ESV 1+/-4 mL; P<0.01) than with candesartan alone (EDV 27+/-4 mL; ESV 18+/-3 mL) or enalapril alone (EDV 23+/-7 mL; ESV 14+/-6 mL). Blood pressure decreased with combination therapy (6+/-1/4+/-1 mm Hg) compared with candesartan or enalapril alone (P<0.05). Aldosterone decreased (P<0.05) with combination therapy (23.2+/-5.3 pg/mL) at 17 but not 43 weeks compared with candesartan (0.7+/-7.8 pg/mL) or enalapril (-0.8+/-11. 3 pg/mL). Brain natriuretic peptide decreased with combination therapy (5.8+/-2.7 pmol/L; P<0.01) compared with candesartan (4. 4+/-3.8 pmol/L) and enalapril alone (4.0+/-5.0 pmol/L). Conclusions: Candesartan alone was as effective, safe, and tolerable as enalapril. The combination of candesartan and enalapril was more beneficial for preventing left ventricular remodeling than either candesartan or enalapril alone.
Phase III randomized clinical trial of efficacy and safety of amlodipine and candesartan cilexetil combination for hypertension treatment

Abstract:Effective antihypertensive therapy is essential for achieving optimal blood pressure (BP) control and reducing cardiovascular events. This double-blind, multicenter, randomized trial aimed to compare the antihypertensive efficacy and safety of a combination of amlodipine (AML) and candesartan cilexetil (CC) versus AML monotherapy in patients with essential hypertension (HTN). After a 4-week run-in period with AML 5 mg, patients whose HTN remained uncontrolled (diastolic BP [DBP]) ≥ 90 mmHg and < 120 mmHg) were randomized to receive either AML + CC or AML alone for 8 weeks. Efficacy was assessed by measuring changes in DBP and systolic BP (SBP). The primary safety measure was the incidence of adverse events (AEs). A total of 174 participants were included in the efficacy analysis. After 8 weeks, DBP decreased by -9.92 ± 0.86 mmHg in the AML + CC arm and - 2.08 ± 0.86 mmHg in the AML arm ( p < 0.0001). SBP decreased by -14.27 ± 1.39 mmHg in the AML + CC arm versus - 2.77 ± 1.39 mmHg in the AML arm ( p < 0.0001). AEs occurred in 11.24% of the AML + CC group and 5.62% of the AML group ( p = 0.1773). AML + CC combination therapy demonstrated superior efficacy with good tolerance, making it a promising option for patients with inadequately controlled hypertension on amlodipine alone.

Abstract:This report analyzes age-specific glucose (PG) and immunoassayable insulin (IRI) responses during oral glucose tolerance testing (OGTT) and examines test results in children and adolescents with OGTT abnormality using the age-appropriate control data. Controls' (n = 93) and patients' (n = 63) results were compared on the basis of statural age (SA) at the time of testing. Control tests (n = 101) showed significant positive correlation of fasting and fourhour postingestion PG with SA (p <0.001), but mean area under the PG curves did not vary between the SA groups (I—18-69 months, 11—70-131 months, III—132+ months). The absence of differences at other sampling times permits uniform diagnostic criteria for this age group. IRI was positively correlated with SA at aU testing times, and mean levels differed significantly between each SA group at every sampling time; the mean areas under the IRI curve also differed significantly between SA groups as did the mean ratios of IRI area to PG area (group I—0.2639 ± 0.0175 S.E.M., groupII—0.3864 ± 0.0235, group III—0.6262 ± 0.0491). Patient tests (n = 110) were separated into normal (N), borderline (B), and chemical-diabetic (C) for each SA group. IRI means were above control data for each test type in each SA group at all sampling times; one fourth of these differences were significant. IRI responses also increased within each SA group from N to B to C tests. Mean IRI areas and IRI area to PG area mean ratios were higher than in controls, and this difference was greatest with the most abnormal (C) test type in each SA group. A subgroup of three patients who had low IRI responses from the outset and developed overt diabetes in one to three years was excluded from the analysis. In contrast to apparent relative insulin inefficiency with normal maturation and with chemical diabetes, they had exceptional responsiveness to their low IRI levels. Variable involvement of alpha as well as beta cells in the pathophysiology of diabetes is suggested as one explanation for these paradoxic observations. Changing receptor affinity might also be implicated.
Effect of combining valsartan and amlodipine in the treatment of type 2 diabetes mellitus complicated by hypertension

Abstract:Purpose: To investigate the efficacy of combining valsartan and amlodipine on patients who were co-morbid with type 2 diabetes mellitus (T2DM) and hypertension. Methods: The medical records of 110 patients treated in North China Electric Power University Hospital, Beijing, China from July 2018 to October 2022 were retrospectively analyzed. Control group (CG; n = 50) was administered oral amlodipine (5 mg daily for 6 months), while the study group (SG; n = 60) received one tablet of co-formulated valsartan (80 mg)) and amlodipine (5 mg) daily for 6 months. All the patients, in addition to the above drugs, were also placed on insulin and atorvastatin calcium. After a 6-month treatment period, the therapeutic effect and incidence of adverse effects were compared between CG and SG. Blood glucose-related indices (glycosylated hemoglobin (HbA1c), 2 h postprandial blood glucose (2 h PG), and fasting blood glucose (FBG)) and blood pressure indices, viz, (diastolic blood pressure (DBP), systolic blood pressure (SBP)) were compared. Results: Compared to the pre-treatment values, both groups showed significant decrease in 2 h PG, FBG, HbA1c, SBP and DBP after treatment (p < 0.05), but the 2 h PG, FBG, HbA1c, SBP and DBP in SG were significantly lower in comparison to CG (p < 0.05). The overall response rate (ORR) of CG was significantly lower in comparison to SG (p < 0.05). In SG, there was significant reduction in the incidence of adverse effects compared to CG (p < 0.05). Conclusion: The combination of valsartan and amlodipine is effective in treating T2DM co-morbid with hypertension, and satisfactorily controls blood glucose level and blood pressure with a relatively high level of safety. Further experiments would be required to ascertain the reliability of these findings.

[Effects of Angiotensin II Type 1 Receptor Blocker on Islet of Diabetic Db/db Mice].

Abstract:OBJECTIVE:Several epidemiological studies suggested that treatment with angiotensin II type 1 receptor blocker provided a risk reduction of developing type 2 diabetes. The aim of this study was to investigate whether and how chronic candesartan treatment can attenuate the deleterious influence of hyperactive local intra-islet renin-angiotensin system in diabetes state and relieve hyperglycemia using diabetic (db/db) mice.METHODS:8-week-old db/db mice were randomized to candesartan cilexetil 1 mg/kg, candesartan cilexetil 10 mg/kg, manidipine 10 mg/kg, or placebo via gavage for 6 weeks. Their age-matched nondiabetic littermates db/m mice were given with placebo and acted as non-diabetic controls. After 6 weeks of treatment, intraperitoneal glucose tolerance test, immunohistochemical staining of oxidative stress markers, insulin, and electron microscopy observation of the pancreatic islet cells were performed.RESULTS:Chronic candesartan treatment provided a slight improvement of glucose tolerance, but greatly rescued islet beta-cell mass. Candesartan treatment also notably decreased staining intensity of oxidative stress markers, as well as candesartan-treated animals exhibited improved granulation and less remarkable endoplasmic reticulum and Golgi bodies. Furthermore, candesartan treatment greatly relieved the swelling of mitochondria to nearly normal.CONCLUSIONS:After diabetes is initiated, candesartan treatment does not reverse the state of diabetes, but it slightly improves glucose tolerance, and protects beta-cell function by attenuating oxidative stress and ultrastructure disruption. These benefits are independent of blood pressure lowering. The characteristics of candesartan may make itself a novel therapeutic means for protecting from progressive beta-cell failure in diabetes.
AHU377+Valsartan (LCZ696) Modulates Renin-Angiotensin System (RAS) in the Cardiac of Female Spontaneously Hypertensive Rats Compared With Valsartan

Abstract:Background: Hypertension is a major cause of death and morbidity worldwide and is increasing in prevalence. The Renin-angiotensin system (RAS) is the most common mechanism involved in the pathophysiology of hypertension. Understanding the mechanism of the pathophysiologic processes will help direct potential therapeutic strategies to treat hypertension and improve cardiac function. Recently, a novel drug LCZ696 containing both an angiotensin receptor blocker valsartan and a neprilysin inhibitor (AHU377) has shown a promising effect on the treatment of hypertension. However, the effects of LCZ696 on the expression of main components of RAS, namely, angiotensin-converting enzyme (ACE), angiotensin-converting enzyme 2 (ACE2), angiotensin II type 1 receptor (AT1 R), angiotensin II type 2 receptor (AT2 R), and angiotensin (1-7) receptor/Mas receptor (MasR) remain unclear. The aim of the present study was to evaluate the effects of LCZ696 on the protective arms of RAS in the cardiac tissue when compared with valsartan under the equal inhibition of AT1 R. We hypothesized that the superior effects of LCZ696 may contribute to its greater effect on the RAS than valsartan. Materials and methods: Sixteen-week-old female spontaneously hypertensive rats (SHRs) were used in this study. Wistar-Kyoto (WKY) rats were used as controls. All rats were randomly divided into LCZ696 (n = 10), valsartan (n = 10), SHR (n = 10), and WKY (n = 10) groups under a 12-hour dark and 12-hour light cycle and provided with regular chow diet and water. The tail-cuff method was performed to measure blood pressure. Cardiac function was assessed by echocardiography. Results: The blood pressure value was lower in LCZ696 than valsartan in SHR after 12 weeks of treatment. Further, LCZ696 inhibits the ACE and AT1 R protein expression in the cardiac of SHR and significantly upregulate the protective axis of RAS components, including ACE2, MasR, and AT2 R. Left ventricular AT2 R messenger RNA (mRNA) expression was higher in the LCZ696+SHR group compared with valsartan. In addition, real-time polymerase chain reaction analysis revealed that LCZ696 enhanced the mRNA expression of antihypertensive components AT2 R, ACE2, and MasR and decreased the expression of AT1 R. However, only AT2 R and ACE2 mRNA expressions have a statistical difference between the LCZ696 and valsartan groups. No difference was observed in the mRNA expression of ACE and MasR. The stronger positive signal of transforming growth factor β in the left ventricle was inhibited in each administrated group compared with SHR groups. Conclusions: LCZ696 ameliorates the vasoconstrictor axis of the RAS AT1 R and stimulate the protective arm effectors, ACE2 and AT2 R, as well as reverses the compensatory upregulation of neuronal nitric oxide synthase and endothelial nitric oxide synthase in SHR. These findings suggest the mechanistic insight of the cardiac-protective and greater hypotensive effects of LCZ696.
Amlodipine Improves Endothelial Progenitor Cell Mobilization and Neo-vascularization in Experimental Diabetic Rats After Myocardial Infarction

Abstract:Objective: To observe the effect of amlodipine on bone marrow endothelial progenitor cell (EPC) mobilization, neo-vascularization and cardiac function in diabetic rats after myocardial infarction (MI) with the possible mechanisms. Methods: A total of 60 male SD rats were divided into 2 groups. Normal group, n=20. Diabetic group, n=40, the rats were fed with high fat diet (HFD) for 4 weeks and then received streptozotocin followed by left anterior descending coronary artery ligation to establish MI model, those rats were further divided into 2 sub-groups:Control group, the rats received sodium carboxymethylcellulose 1 ml/day with HFD and Treatment group, the rats received amlodipine 2 mg/kg/day with HFD, n=20 in each sub-group, all animals were treated for 4 weeks. The EPC level in peripheral blood CD45-/low+/CD133+/KDR+ at before and 1, 3, 5, 7, 14, 28 days after operation were examined by lfow cytometry, plasma vascular endothelial growth factor (VEGF) level was measured by ELISA, capillary density in MI area was determined by CD31 staining, EPC related protein expressions were detected by western blot analysis and the cardiac function was evaluated by echocardiography. Results: EPC in CD45-/low+/CD133+/KDR+in Treatment group at 7 days after operation was increased than Control group at 5 days after operation (112 ± 30/106) vs (55 ± 10/106), plasma VEGF in Treatment group was higher than Control group (5.63 ± 1.33) ng/L vs (3.68 ± 0.98) ng/L; Treatment group presented increased expressions of protein kinase B, endothelial nitric oxide synthase (eNOS) and matrix metallopeptidase-9, increased capillary density in MI area, higher LVEF and left ventricular fractional shorting, all P<0.05-0.01. Conclusion: Amlodipine improves EPC mobilization, neo-vascularization and cardiac function in diabetic-MI rats, it may be related to VEGF/eNOS cascade activation.

Abstract:Combined neprilysin (NEP) inhibition (sacubitril) and angiotensin type 1 receptor (AT1R) antagonism (valsartan) is used in the treatment of congestive heart failure and is gaining interest for other angiotensin II (AngII)-related cardiovascular diseases. In addition to heart failure, AngII promotes hypertension, atherosclerosis, and abdominal aortic aneurysms (AAAs). Similarly, NEP substrates or products have broad effects on the cardiovascular system. In this study, we examined NEP inhibition (with sacubitril) and AT1R antagonism (with valsartan) alone or in combination on AngII-induced hypertension, atherosclerosis, or AAAs in male low-density lipoprotein receptor–deficient mice. Preliminary studies assessed drug delivery via osmotic minipumps for simultaneous release of sacubitril and/or valsartan with AngII over 28 days. Mice were infused with AngII (1000 ng/kg per minute) in the absence (vehicle) or presence of sacubitril (1, 6, or 9 mg/kg per day), valsartan (0.3, 0.5, 1, 6, or 20 mg/kg per day), or the combination thereof (1 and 0.3, or 9 or 0.5 mg/kg per day of sacubitril and valsartan, respectively). Plasma AngII and renin concentrations increased 4-fold at higher valsartan doses, indicative of removal of AngII negative feedback on renin. Sacubitril doubled plasma AngII concentrations at lower doses (1 mg/kg per day). Valsartan dose-dependently decreased systolic blood pressure, aortic atherosclerosis, and AAAs of AngII-infused mice, whereas sacubitril had no effect on atherosclerosis or AAAs but reduced blood pressure of AngII-infused mice. Combination therapy with sacubitril and valsartan did not provide additive benefits. These results suggest limited effects of combination therapy with NEP inhibition and AT1R antagonism against AngII-induced hypertension, atherosclerosis, or AAAs.SIGNIFICANCE STATEMENTThe combination of valsartan (angiotensin type 1 receptor antagonist) and sacubitril (neprilysin inhibitor) did not provide benefit above valsartan alone on AngII-induced hypertension, atherosclerosis, or abdominal aortic aneurysms in low-density lipoprotein receptor–deficient male mice. These results do not support this drug combination in therapy of these AngII-induced cardiovascular diseases.

Effects of the CB1 Receptor Antagonists AM6545 and AM4113 on Insulin Resistance in a High-Fructose High-Salt Rat Model of Metabolic Syndrome

Abstract:Background and Objectives: Insulin resistance (IR) is a serious condition leading to development of diabetes and cardiovascular complications. Hyper-activation of cannabinoid receptors-1 (CB1) has been linked to the development of metabolic disorders such as IR. Therefore, the effect of blocking CB1 on the development of IR was investigated in the present study. Materials and Methods: A 12-week high-fructose/high-salt feeding model of metabolic syndrome was used to induce IR in male Wistar rats. For this purpose, two different CB1-antagonists were synthesized and administered to the rats during the final four weeks of the study, AM6545, the peripheral neutral antagonist and AM4113, the central neutral antagonist. Results: High-fructose/salt feeding for 12 weeks led to development of IR while both AM6545 and AM4113, administered in the last 4 weeks, significantly inhibited IR. This was correlated with increased animal body weight wherein both AM6545 and AM4113 decreased body weight in IR animals but with loss of IR/body weight correlation. While IR animals showed significant elevations in serum cholesterol and triglycerides with no direct correlation with IR, both AM6545 and AM4113 inhibited these elevations, with direct IR/cholesterol correlation in case of AM6545. IR animals had elevated serum uric acid, which was reduced by both AM6545 and AM4113. In addition, IR animals had decreased adiponectin levels and elevated liver TNFα content with strong IR/adiponectin and IR/TNFα correlations. AM6545 inhibited the decreased adiponectin and the increased TNFα levels and retained the strong IR/adiponectin correlation. However, AM4113 inhibited the decreased adiponectin and the increased TNFα levels, but with loss of IR/adiponectin and IR/TNFα correlations. Conclusions: Both CB1 neutral antagonists alleviated IR peripherally, and exerted similar effects on rats with metabolic syndrome. They also displayed anti-dyslipidemic, anti-hyperurecemic and anti-inflammatory effects. Overall, these results should assist in the development of CB1 neutral antagonists with improved safety profiles for managing metabolic disorders.
