Abstract:It has been proved that continued training recovery for the patients who suffer limb movement disorder helps them in therapy protocols. However, the specialty and numbers of therapist often limit the rehabilitation to these patients. Thus, employing robotic equipment to provide the treatment capability is a way out of this dilemma. Considering the compliance and continuous deformation capacity, a robotic exoskeleton actuated by Pneumatic Arti?cial Muscles (PAMs) is designed as the rehabilitation equipment for elbow joint. Moreover, to precisely control the PAM soft actuator of the proposed equipment, a bio-inspired control approach is explored. Finally, several simulations have been conducted to verify the effectiveness of the proposed system.

Abstract:People with movement disorders are plagued with debilitating conditions, which significantly degrade their quality of life. Traditional rehabilitation typically involves intensive interaction between patients and therapists. While effective, traditional rehabilitation cannot keep abreast of the increasing patient population primarily attributed to a higher surviving rate after diseases and/or injuries. Furthermore, patients living in the rural areas have fairly limited access to rehabilitation services. In the past two decades, tremendous efforts have been put into developing rehabilitation and assistive robots to facilitate the rehabilitation training while relieving the physical involvement of therapists and/or lowering the related cost. Most notably, the rehabilitation and assistive robots have been significantly advanced with developments in actuators, sensors, microprocessors, and mobile software platforms. However, unlike traditional robotics, the intimate interaction between robot and human in rehabilitation robots indicates that the success is also closely related to a thorough understanding of the human neuromuscular aspects and human-machine interaction. This special issue primarily aims to gather the latest achievements in rehabilitation robots, exoskeletons, and prostheses including the following topics: (a) development of rehabilitation robots, exoskeleton, and upper/lower limb prostheses driven by bionics; (b) functional evaluation of rehabilitation robots, exoskeleton, and upper/lower limb prostheses with an emphasis on human movement biomechanics; (c) musculoskeletal modeling and simulation of human movements while wearing exoskeleton or prostheses; (d) noninvasive human-machine interface based on electromyography and/or electroencephalogram; (e) sensors for monitoring kinematics/kinetics, as well as biological signals in real time; (f) innovative actuators and control algorithms applied to rehabilitation robots, exoskeletons, and prostheses. In this special issue, collective studies address the aforementioned key elements via both technical and biomechanical approaches. A reconfigurable robotic hand exoskeleton was proposed to meet the fast growing need in hand rehabilitation. A novel control algorithm integrating sliding model control with cerebellar model articulation controller neural network was implemented in lower limb exoskeleton to enhance the coordination between patient and exoskeleton. An upper limb exoskeleton was enhanced with integrated optical cameras to offer more accurate estimation of joint posture than traditional motion capture system. A hybrid upper limb rehabilitation system consisting of a shoulder-elbow-forearm exoskeleton and a robotic manipulator was validated and tested in the clinic. The characteristics of muscle-tendon stimulation such as perception threshold and vibration frequency significantly influenced the muscle forces as well as the reaction time. Patellar retention was found to be superior to patellar replacement in knee arthroplasty via a comprehensive computer simulation. These collective studies, as part of the latest representative work, offered some new insights into the development and implementation of rehabilitation and assistive robots. Abstract:One typical application of a robotic exoskeleton is to automate rehabilitation, where the robot is worn by a stroke patient and provides assistance to help perform repetitive motions and regain motor functions. The deployment of exoskeletons can alleviate the shortage of experienced therapists and would also play a vital role in countries with aging populations. However, the intelligence level of existing exoskeletons is relatively low, wherein a robot cannot adapt to either the online change of a subject’s motion (e.g., the gait pattern) or the variation of his/her body parameters (i.e., a new subject who is going to wear the robot), potentially resulting in conflict between human and robot and possibly even leading to physical damage. As such, the application of a robotic exoskeleton in clinical studies is limited. This article introduces a new bilateral exoskeletal assistive robot for rehabilitation (BEAR-H) in which the main novelty is the integration of multiple intelligent features, such as gait recognition and synchronization, cloud-computing diagnosis, and individualized gait generation. Such an integration helps the robot to better understand the patient’s condition and hence provide effective assistance, by the end achieving smart rehabilitation. BEAR-H is a successfully commercialized product, and its performance has been validated in actual clinical studies with 30 patients, producing experimental results from different aspects that are analyzed and presented.

Abstract:The incidence of stroke in the middle-aged and elderly population is high, and it will lead to different degrees of hemiplegia, which will make the patient’s exercise capacity decline and affect their lif e. The lower limbs are the central support part of the human body, and stroke survivors need rehabilitation training to restore their athletic ability. However, traditional rehabilitation training limits the rehabilitation effect of patients due to some shortcomings such as one to one treatment, long rehabilitation cycles, and so on. As an auxiliary means of innovation, the application of rehabilitation robot -assisted training has broad prospects. In this paper, we propose a dual-arm lower limb rehabilitation robot (DLLRR) that can simulate the skill of the therapist to improve the rehabilitation effect of the patient. Firstly, the mechanism of the robot is designed based on the patient’s needs. Then, the kinematics model of the DLLRR is built, and the workspace is calculated in MATLAB. Furthermore, the hardware control system of the DLLRR is constructed, and the cooperative control strategy of the robot arms is introduced. The speed response experiments verify the superior dynamic performance of robotic joints, and the position control experiment and walking experiment show that the DLLRR has good tracking performance and can help the subject to carry out rehabilitation training. These results provide evidence for the feasibility of the designed robot.

Abstract:In view of the urgent need for intelligent rehabilitation equipment for some disabled people, an intelligent, upper limb rehabilitation training robot is designed by applying the theories of artificial intelligence, information, control, human-machine engineering, and more. A new robot structure is proposed that combines the use of a flexible rope with an exoskeleton. By introducing environmentally intelligent ergonomics, combined with virtual reality, multi-channel information fusion interaction technology and big-data analysis, a collaborative, efficient, and intelligent remote rehabilitation system based on a human’s natural response and other related big-data information is constructed. For the multi-degree of the freedom robot system, optimal adaptive robust control design is introduced based on Udwdia-Kalaba theory and fuzzy set theory. The new equipment will help doctors and medical institutions to optimize both rehabilitation programs and their management, so that patients are more comfortable, safer, and more active in their rehabilitation training in order to obtain better rehabilitation results.

Abstract:Numerous robots have been widely used to deliver rehabilitative training for hemiplegic patients to improve their functional ability. Because of the complexity and diversity of upper limb motion, customization of training patterns is one key factor during upper limb rehabilitation training. Most of the current rehabilitation robots cannot intelligently provide adaptive training parameters, and they have not been widely used in clinical rehabilitation. This article proposes a new end-effector upper limb rehabilitation robot, which is a two-link robotic arm with two active degrees of freedom. This work investigated the kinematics and dynamics of the robot system, the control system, and the realization of different rehabilitation therapies. We also explored the influence of constraint in rehabilitation therapies on interaction force and muscle activation. The deviation of the trajectory of the end effector and the required trajectory was less than 1 mm during the tasks, which demonstrated the movement accuracy of the robot. Besides, results also demonstrated the constraint exerted by the robot provided benefits for hemiplegic patients by changing muscle activation in the way similar to the movement pattern of the healthy subjects, which indicated that the robot can improve the patient's functional ability by training the normal movement pattern.

Abstract:Rehabilitation training for patients with motor disabilities usually requires specialized devices in rehabilitation centers. Home-based multi-purpose training would significantly increase treatment accessibility and reduce medical costs. While it is unlikely to equip a set of rehabilitation robots at home, we investigate the feasibility to use the general-purpose collaborative robot for rehabilitation therapies. In this work, we developed a new system for multi-purpose upper-limb rehabilitation training using a generic robot arm with human motor feedback and preference. We integrated surface electromyography, force/torque sensors, RGB-D cameras, and robot controllers with the Robot Operating System to enable sensing, communication, and control of the system. Imitation learning methods were adopted to imitate expert-provided training trajectories which could adapt to subject capabilities to facilitate in-home training. Our rehabilitation system is able to perform gross motor function and fine motor skill training with a gripper-based end-effector. We simulated system control in Gazebo and training effects (muscle activation level) in OpenSim and evaluated its real performance with human subjects. For all the subjects enrolled, our system achieved better training outcomes compared to specialist-assisted rehabilitation under the same conditions. Our work demonstrates the potential of utilizing collaborative robots for in-home motor rehabilitation training.

Abstract:As a promising alternative to hospital-based manual therapy, robot-assisted tele-rehabilitation therapy has shown significant benefits in reducing the therapist's workload and accelerating the patient's recovery process. However, existing telerobotic systems for rehabilitation face barriers to implementing appropriate therapy treatment due to the lack of effective therapist-patient interactive capabilities. In this paper, we develop a home-based tele-rehabilitation system that implements two alternative training methods, including a haptic-enabled guided training that allows the therapist to adjust the intensity of therapeutic movements provided by the rehabilitation device and a surface electromyography (sEMG)-based supervised training that explores remote assessment of the patient's kinesthetic awareness. Preliminary experiments were conducted to demonstrate the feasibility of the proposed alternative training methods and evaluate the functionality of the developed tele-rehabilitation system. Results showed that the proposed tele-rehabilitation system enabled therapist-in-the-loop to dynamically adjust the rehabilitation intensity and provided more interactivity in therapist-patient remote interaction.

Abstract:Background: With the increasing number of hemiplegic patients in recent years, traditional rehabilitation methods cannot meet the current social needs. In order to help patients recover as soon as possible, the combination of the human intelligence and the mechanical power device of the manmachine interface of the exoskeletal rehabilitation robot is becoming more and more popular. Objective: To provide an overview of recent exoskeletal rehabilitation robot for upper limb and introduce their respective characteristics and development. Methods: This paper reviews various representative patents related to the exoskeletal rehabilitation robot for upper limb. The structural characteristics and applications of the typical exoskeletal rehabilitation robots for upper limb are introduced. Results: Different types of exoskeletal rehabilitation robots for upper limb are enumerated, and different characteristics of them are analyzed. The existing problems of exoskeletal rehabilitation robots for upper limb are summarized, and the current development including future of patents on exoskeletal rehabilitation robots for upper limb are discussed. Conclusion: The optimization of the structure and performance of the exoskeletal rehabilitation robots for upper limb greatly improves the efficiency of rehabilitation. For further development, more and more patents on exoskeletal rehabilitation robots for upper limb should be designed and invented. Abstract:Rehabilitation robotics aims to facilitate the rehabilitation procedure for patients and physical therapists. This field has a relatively long history dating back to the 1990s; however, their implementation and the standardisation of their application in the medical field does not follow the same pace, mainly due to their complexity of reproduction and the need for their approval by the authorities. This paper aims to describe architecture that can be applied to industrial robots and promote their application in healthcare ecosystems. The control of the robotic arm is performed using the software called SmartHealth, offering a 2 Degree of Autonomy (DOA). Data are gathered through electromyography (EMG) and force sensors at a frequency of 45 Hz. It also proves the capabilities of such small robots in performing such medical procedures. Four exercises focused on shoulder rehabilitation (passive, restricted active-assisted, free active-assisted and Activities of Daily Living (ADL)) were carried out and confirmed the viability of the proposed architecture and the potential of small robots (i.e., the UR3) in rehabilitation procedure accomplishment. This robot can perform the majority of the default exercises in addition to ADLs but, nevertheless, their limits were also uncovered, mainly due to their limited Range of Motion (ROM) and cost.
Design and remote collaboration control of asymmetric rigid-flexible hybrid-driven lower limb rehabilitation robot

Abstract:A double-end lower limb rehabilitation robot is introduced in the paper, which can realize the rehabilitation training of the abduction and abduction, internal rotation and external rotation of human lower limb. The kinematics and dynamics of the robot are analyzed. Based on the motion trajectory planning of the lower limb rehabilitation robot, the motion state and dynamic state of the cable are analyzed. In order to realize the good remote cooperative control between the rehabilitation physician terminal and the patient rehabilitation terminal, the bilateral PD control method and the patient terminal force feedback control method based on the absolute stability theory are analyzed by using the dual-port network model. The simulation results show that the performance of the patient terminal force feedback control method is excellent when the force on the moving platform and the external interference force are changed suddenly. The mapping model of double-ended mechanism is established in the SimMechanics. The simulation results show that the mapping model of double-ended mechanism has a good position tracking ability and their workspaces can meet the requirements of trajectory mapping. The remote collaborative rehabilitation training experiment was done on the established experimental platform. The experimental result shows that the experiment system has a good tracking performance under the guidance of the patient terminal force feedback control method. The feasibility and operability of the remote rehabilitation cooperation technology are verified.

Abstract:Rehabilitation robots are used to promote structural and functional recovery of the nervous system with repetitive, task-oriented training and have been gradually applied to clinical rehabilitation training. This paper proposes an upper limb exoskeleton rehabilitation robot system that could realize shoulder-elbow-wrist joint rehabilitation training. Firstly, a motion equivalent model was established based on the upper limb movement mechanism, the robot mechanism configuration was designed, and the optimization algorithm and spatial mechanism theory were used to optimize and analyze the structural parameters and human–machine compatibility of the robot, which will guide the design of the robot's model. Then, the robot kinematics were solved, and its maximum motion range, dexterity distribution, and daily motion trajectory were simulated. Finally, a system prototype was built to test the maximum range of robot-assisted human upper limb training by laser tracker, while the pressure of human–machine interaction during training was captured and analyzed by flexible sensors. The results show that the proposed rehabilitation robot could nearly completely cover the range of motion of upper limb joints and meet the needs of trajectory training, and the linear velocity dexterity and angular velocity dexterity in the motion space are maximum 0.55 and 0.89, and the human–machine interaction pressures during the training process are all less than 10 kPa. Besides, this paper also conducted a system evaluation based on the fuzzy comprehensive evaluation model, and the evaluation result was 0.39, with an excellent evaluation grade, it indirectly indicates that the robot's overall performance was good.

Patient-Specific Exercises with the Development of an End-Effector Type Upper Limb Rehabilitation Robot

Abstract:End-effector type upper limb rehabilitation robots (ULRRs) are connected to patients at one distal point, making them have simple structures and less complex control algorithms, and they can avoid abnormal motion and posture of the target anatomical joints and specific muscles. Given that the end-effector type ULRR focuses more on the rehabilitation of the combined motion of upper limb chain, assisting the patient to perform collaborative tasks, and its intervention has some advantages than the exoskeleton type ULRR, we developed a novel three-degree-of-freedom (DOF) end-effector type ULRR. The advantage of the mechanical design is that the designed end-effector type ULRR can achieve three DOFs by using a four-bar mechanism and a lifting mechanism; we also developed the patient-specific exercises including patient-passive exercise and patient-cooperative exercise, and the advantage of the developed patient-cooperative exercise is that we simplified the human-robot coupling system model into a single spring system instead of the mass-spring-damp system, which efficiently improved the response speed of the control system. In terms of the organization structure of the work, we introduced the end-effector type ULRR's mechanical design, control system, inverse solution of positions, patient-passive exercise based on the inverse solution of positions and the linear position interpolation of servo drives, and patient-cooperative exercise based on the spring model, in sequence. Experiments with three healthy subjects have been conducted, with results showing good trajectory tracking performance in patient-passive exercise and showing effective, flexible, and good real-time interactive performance in patient-cooperative exercise.

Abstract:Most lower limb rehabilitation robots are limited to specific training postures to adapt to stroke patients in multiple stages of recovery. In addition, there is a lack of attention to the switching functions of the training side, including left, right, and bilateral, which enables patients with hemiplegia to rehabilitate with a single device. This article presents an exoskeleton robot named the multistage hemiplegic lower-limb rehabilitation robot, which has been designed to do rehabilitation in multiple training postures and training sides. The mechanism consisting of the thigh, calf, and foot is introduced. Additionally, the design of the multi-mode limit of the hip, knee, and ankle joints supports delivering therapy in any posture and training sides to aid patients with hemiplegia in all stages of recovery. The gait trajectory is planned by extracting the gait motion trajectory model collected by the motion capture device. In addition, a control system for the training module based on adaptive iterative learning has been simulated, and its high-precision tracking performance has been verified. The gait trajectory experiment is carried out, and the results verify that the trajectory tracking performance of the robot has good performance.

Abstract:Cerebrovascular accidents, commonly known as strokes, represent a prevalent neurological event leading to significant upper limb disabilities, thereby profoundly affecting individuals' activities of daily living and diminishing their quality of life. Traditional rehabilitation methods for upper limb recovery post-stroke are often hindered by limitations, including therapist and patient fatigue, reliance on singular training methodologies, and lack of sustained motivation. Addressing these challenges, this study introduces an upper limb rehabilitation robot, which uses intelligent feedback motion control to improve therapeutic outcomes. The system is distinguished by its capability to adjust the direction and magnitude of force feedback dynamically, based on the detection of spastic movements during exercises, thereby offering a tailored therapeutic experience. This system is equipped with four distinct training modes, intelligent assessment of joint range of motion, and the ability to personalize training programs. Moreover, it provides an immersive interactive gaming experience coupled with comprehensive safety measures. This multifaceted approach not only elevates the engagement and interest of participants beyond traditional rehabilitation protocols but also demonstrates significant improvements in upper limb functionality and the activities of daily living among hemiplegic patients. The system exemplifies an advanced tool in upper limb rehabilitation, offering a synergistic blend of precision, personalization, and interactive engagement, thereby broadening the therapeutic options available to stroke survivors.
Design and Analysis of an Upper Limb Rehabilitation Robot Based on Multimodal Control

Abstract:To address the rehabilitation needs of upper limb hemiplegic patients in various stages of recovery, streamline the workload of rehabilitation professionals, and provide data visualization, our research team designed a six-degree-of-freedom upper limb exoskeleton rehabilitation robot inspired by the human upper limb’s structure. We also developed an eight-channel synchronized signal acquisition system for capturing surface electromyography (sEMG) signals and elbow joint angle data. Utilizing Solidworks, we modeled the robot with a focus on modularity, and conducted structural and kinematic analyses. To predict the elbow joint angles, we employed a back propagation neural network (BPNN). We introduced three training modes: a PID control, bilateral control, and active control, each tailored to different phases of the rehabilitation process. Our experimental results demonstrated a strong linear regression relationship between the predicted reference values and the actual elbow joint angles, with an R-squared value of 94.41% and an average error of four degrees. Furthermore, these results validated the increased stability of our model and addressed issues related to the size and single-mode limitations of upper limb rehabilitation robots. This work lays the theoretical foundation for future model enhancements and further research in the field of rehabilitation.

In this paper a novel robot neuro-rehabilitation for upper limb, with many merits, is developed. Mechanism and control of the robot are introduced in detail. By a clinical trial involving 30 stroke patients with three months at Rehabilitation Center of China, some results are obtained 1) robot-aided therapy does not have adverse effects, 2) patients can accept the procedure, 3) the therapy may reduce muscular tension, and 4) manipulation of the impaired upper limb may influence brain recovery. Some new ideas are also presented to make rehabilitation therapy more effectively. These ideas will be helpful to further quicken the development of the robot neuro-rehabilitation

Abstract:Introduction: For patients with functional motor disorders of the lower limbs due to brain damage or accidental injury, restoring the ability to stand and walk plays an important role in clinical rehabilitation. Lower limb exoskeleton robots generally require patients to convert themselves to a standing position for use, while being a wearable device with limited movement distance. Methods: This paper proposes a reconfigurable behavioral assistive robot that integrates the functions of an exoskeleton robot and an assistive standing wheelchair through a novel mechanism. The new mechanism is based on a four-bar linkage, and through simple and stable conformal transformations, the robot can switch between exoskeleton state, sit-to-stand support state, and wheelchair state. This enables the robot to achieve the functions of assisted walking, assisted standing up, supported standing and wheelchair mobility, respectively, thereby meeting the daily activity needs of sit-to-stand transitions and gait training. The configuration transformation module controls seamless switching between different configurations through an industrial computer. Experimental protocols have been developed for wearable testing of robotic prototypes not only for healthy subjects but also for simulated hemiplegic patients. Results: The experimental results indicate that the gait tracking effect during robot-assisted walking is satisfactory, and there are no sudden speed changes during the assisted standing up process, providing smooth support to the wearer. Meanwhile, the activation of the main force-generating muscles of the legs and the plantar pressure decreases significantly in healthy subjects and simulated hemiplegic patients wearing the robot for assisted walking and assisted standing-up compared to the situation when the robot is not worn. Discussion: These experimental findings demonstrate that the reconfigurable behavioral assistive robot prototype of this study is effective, reducing the muscular burden on the wearer during walking and standing up, and provide effective support for the subject's body. The experimental results objectively and comprehensively showcase the effectiveness and potential of the reconfigurable behavioral assistive robot in the realms of behavioral assistance and rehabilitation training.

Abstract:Upper limb rehabilitation robot can effectively help patients recover motor ability. Existing rehabilitation robots are usually driven by rigid motors and the mechanical structures cannot adapt to the different patients with the different physical parameters and different rehabilitation needs. This paper designs a reconfigurable upper limb rehabilitation exoskeleton for elbow and wrist joints driven by pneumatic muscle actuators (PMAs). The exoskeleton can assist patients to achieve elbow flexion/extension, wrist flexion/ extension and adduction/abduction by integrating soft elbow and wrist joint modules which can work separately or together. The wrist joint can realize two degrees of freedom (2-DoF) movement via adjustable modules. To conquer the dynamic model errors and load disturbances when reconstructing the modular joints, a non-singular fast terminal sliding mode control method based on nonlinear disturbance observer (NFTSMC-NDO) is proposed, and a position/force hierarchical control method is formed to ensure the controllability of the soft modular robot. Experimental results show that the proposed method can achieve high-precision motion control of soft modular joints and provide reconfigurable assistance for patients, improving the adaptability and compliance of rehabilitation training.

Abstract:Towards the rehabilitation and training problems of older persons and lower limb disabilities, the paper pro-poses a new robot with lower limb exoskeleton for rehabilitation and walking assistance. The components and mechanical design of the robot are introduced in detail. Through the function combination of lower limb exoskeleton and wheelchair, the robot can help the users to maintain the lower limb movement effectively and provide them with a convenient tools for movement. The pedal-actuated motion training strategy can ensure the safety of users. The master-slave operational mode is put forward with multimodal rehabilitation training process control. Correctness and feasibility of the rehabilitation system are validated by computer simulation and experiment.
