Abstract:Objective Mediastinitis is a raue but clinically relevant to postorperative morbidity and mortality after cardiac sur-gery.From Janusry l995 to October 2006,mediastinitis developed in 65(1.03%) of 6311 consecutive cardiac surgery procedures via median sternal incision.We reviewed the variation of clinical characteristics and treatment in these patients.Methods The records of 65 cases of postoperative mediastinitis were reviewed .The mean age was (58.7 4±12.6) years (rabge3 months to 73 years),Four-ty-six were male and 19 femele.Samples of sternal drainage and arterial drainage and arterial blood were sent for Gram stain and culture to establish a diag-nosis and to tailor antimicrobial therapy.All patients received broed spectrum antibiotics while awaiting results of stains and cultures. Antibiotics were selected bassd on the culture and sensitivity data and were administered for 2 to 6 weeks.Provide immediate treatment for mediastinitis following cardiac surgery depends on the duration of the infection and the condition of the mediastinum.Only local drainage and irrigation with antiseptic solution were given in 6 because of the mild infection.Other 59 patients underwent extensive sternal and soft tissue debridement and closed mediastianl irrigation with antiseptic solution and drainage throught tubes.Five of them were given plastics procedures using rectus abdominus flaps of pectoralis major muscle flaps.Irrigation-drainage tubes were placed pos-terior to the sternum in all patients.The duration of irringation-drainage depended on the amount of debris drained and the clinical progress of the the pstient,usually 1 to 2 weeks,rarely exceeded 2 weeks,Results The mean interval from operation to onset of meddias-tinitis was (7.4±4.3) days (range,3 to 22 days),and 63 cases presented within 2 weeks following surgery.Most patients had puru-lent discharge from the chest wound,fever,and leukocytosis.Bacteria were isolated from the stemal drainage in 83.10% of the cases. Bacteria were cultured from the arterial blood in 25.9% of the cases(14/54).There were 12 hospital (18.5%) for uncon-trolled infectio,multi-organ failure or bleeding.No specific bacterial etiology was associated with increased mortality nor was pres-ence of bacteriemia.Three surgical re-interventions were required for sternal infection recurred and there was no death during the fol-low-up of 1-12 years.Conclusion Mediastinitis associated with cardiac surgery via sternal incision is a life-threatening condition with a high mortality rate.EarLier recognition and prompt surgical intervention are important.Immediately drainage,debridement and mediastinal irringation could improve the outcome of this dreadly complication.Muscle flap techniques should be considered when direct closure was not possible or failed.
