Abstract:Innovative technology-based solutions in mental healthcare promise significant improvements in care quality and clinical outcomes. However, their successful implementation is profoundly influenced by the levels of trust patients hold toward their treatment providers, organizations, and the technology itself. This paper delves into the complexities of building and assessing patient trust within the intensive mental health care context, focusing on inpatient settings. We explore the multifaceted nature of trust, including interpersonal, institutional, and technological trust. We highlight the crucial role of therapeutic trust, which comprises both interpersonal trust between patients and providers, and institutional trust in treatment organizations. The manuscript identifies potential key barriers to trust, from sociocultural background to a patient's psychopathology. Furthermore, it examines the concept of technological trust, emphasizing the influence of digital literacy, socio-economic status, and user experience on patients' acceptance of digital health innovations. By emphasizing the importance of assessing and addressing the state of trust among patients, the overarching goal is to leverage digital innovations to enhance mental healthcare outcomes within intensive mental health settings.
Trust in technology-mediated collaborative health encounters: constructing trust in passive user interactions with technologies

Abstract:The present study investigated factors that explain patient trust in health technology and the relationship between patient trust in technology and trust in their care provider. Sociotechnical systems theory states that changes in one part of the system are likely related to other parts of the system. Therefore, attitudes about technologies, like trust, are likely related to other aspects of the system. Contributing to appropriate trust at the technological, interpersonal, and system levels can potentially lead to positive health outcomes. The study described in this manuscript used data collected from 101 patients with a Trust in Medical Technology instrument. The instrument measured patients' trust in (1) their providers, (2) the technology, and (3) how their providers used the technology. Measure 3 was positively associated with measures 1 and 2, while measures 1 and 2 were not positively or negatively associated with one another. These results may indicate that patient assessments of the trustworthiness of care providers and technologies are based on their observations of how providers use technologies. Practitioner summary: Though patients are not active users of technologies in health care, the results of this study show that their perceptions of how providers use technology are related to their trust in both technology and the care provider. Study findings have implications for how trust is conceptualised and measured in interpersonal relationships and in technologies.
Holding AI to Account: Challenges for the Delivery of Trustworthy AI in Healthcare

Abstract:The need for AI systems to provide explanations for their behaviour is now widely recognised as key to their adoption. In this paper, we examine the problem of trustworthy AI and explore what delivering this means in practice, with a focus on healthcare applications. Work in this area typically treats trustworthy AI as a problem of Human-Computer Interaction involving the individual user and an AI system. However, we argue here that this overlooks the important part played by organisational accountability in how people reason about and trust AI in socio-technical settings. To illustrate the importance of organisational accountability, we present findings from ethnographic studies of breast cancer screening and cancer treatment planning in multidisciplinary team meetings to show how participants made themselves accountable both to each other and to the organisations of which they are members. We use these findings to enrich existing understandings of the requirements for trustworthy AI and to outline some candidate solutions to the problems of making AI accountable both to individual users and organisationally. We conclude by outlining the implications of this for future work on the development of trustworthy AI, including ways in which our proposed solutions may be re-used in different application settings.

What Is Trust? Ethics and Risk Governance in Precision Medicine and Predictive Analytics

Abstract:Trust is a ubiquitous term used in emerging technology (e.g., Big Data, precision medicine), innovation policy, and governance literatures in particular. But what exactly is trust? Even though trust is considered a critical requirement for the successful deployment of precision medicine initiatives, nonetheless, there is a need for further conceptualization with regard to what qualifies as trust, and what factors might establish and sustain trust in precision medicine, predictive analytics, and large-scale biology. These new fields of 21st century medicine and health often deal with the "futures" and hence, trust gains a temporal and ever-present quality for both the present and the futures anticipated by new technologies and predictive analytics. We address these conceptual gaps that have important practical implications in the way we govern risk and unknowns associated with emerging technologies in biology, medicine, and health broadly. We provide an in-depth conceptual analysis and an operative definition of trust dynamics in precision medicine. In addition, we identify three main types of "trust facilitators": (1) technical, (2) ethical, and (3) institutional. This three-dimensional framework on trust is necessary to building and maintaining trust in 21st century knowledge-based innovations that governments and publics invest for progressive societal change, development, and sustainable prosperity. Importantly, we analyze, identify, and deliberate on the dimensions of precision medicine and large-scale biology that have carved out trust as a pertinent tool to its success. Moving forward, we propose a "points to consider" on how best to enhance trust in precision medicine and predictive analytics.
Hope and trust in diagnostic imaging contexts - Constituting technology and liminal patients

Abstract:Introduction: This study investigates patients' experience of selected imaging examinations and how human experiences are transformed through technological mediation. Method: A qualitative study of patients' experiences during selected imaging examinations, centering on human-technology relations, was conducted. Data was gathered in two Danish hospitals, through semi-structured interviews and participant observations. The study included fifteen diagnostic imaging examination cases, distributed over three Computed Tomography (CT)-guided lung biopsy interventions, seven conventional CT examinations, and five Positron Emission Tomography CT examinations. The participating patients were undergoing investigation for cancer within Fast Track Cancer Referral Programs (FTCRP). The study has a philosophical approach to practice grounded in Postphenomenology and draws on anthropological studies of healthcare practice as its theoretical and analytical framework. Results: Diagnostic imaging technologies were found to have existential implications creating hope and trust in patients. Patients demonstrated technological readiness, in their willingness to comply with examination criteria and the professional's instructions. The patient's primary concern was achieving a good examination result, for which they were prepared to push themselves beyond their usual limits. Participating in diagnostic imaging examinations may be viewed as a life crisis ritual, wherein patients are constituted as liminal beings, existing between healthy and sick. Conclusions: The existential implications of an imaging examination were clear in terms of hope and trust. Hope is related to life and death in two temporal perspectives: A future hope of surviving cancer and a present hope of being cancer-free. Hope was linked to avoiding despair and rested on trusting oneself to technology, thus, forming a circle of interrelated concepts. Implications for practice: Professionals need to recognize the patients' liminal passings yet, unnoticed transformative and existential implications, to perform better patient-centered care.

Abstract:"Trust" can describe many different positive features of our social relationships with others. In this exploratory paper, we reflect on some of the ways in which people orient themselves toward others in the context of a psychological crisis, a time when trust may be threatened or eroded. We draw upon qualitative data extracts from two previously reported studies, in order to illustrate and develop some observations about the dynamics of relational trust during such periods of acute distress. We show how these dynamics arise out of particular contexts and have particular consequences for psychological health. We discuss how, in a relationship mediated by trust, it can sometimes be an act of care – toward self, or toward others – to filter and limit the extent of what one entrusts to another. We suggest that further consideration by philosophers can be very helpful to the mental health field here, if we are to understand the negotiation of interpersonal trust in the context of mental health crises. We note that it would be extremely helpful to understand more about how to create the kinds of environments which afford trust.

Transparency in HRI: Trust and Decision Making in the Face of Robot Errors

Abstract:Robots are rapidly gaining acceptance in recent times, where the general public, industry and researchers are starting to understand the utility of robots, for example for delivery to homes or in hospitals. However, it is key to understand how to instil the appropriate amount of trust in the user. One aspect of a trustworthy system is its ability to explain actions and be transparent, especially in the face of potentially serious errors. Here, we study the various aspects of transparency of interaction and its effect in a scenario where a robot is performing triage when a suspected Covid-19 patient arrives at a hospital. Our findings consolidate prior work showing a main effect of robot errors on trust, but also showing that this is dependent on the level of transparency. Furthermore, our findings indicate that high interaction transparency leads to participants making better informed decisions on their health based on their interaction. Such findings on transparency could inform interaction design and thus lead to greater adoption of robots in key areas, such as health and well-being.

Abstract:Abstract Artificial intelligence (AI) could revolutionise health care, potentially improving clinician decision making and patient safety, and reducing the impact of workforce shortages. However, policymakers and regulators have concerns over whether AI and clinical decision support systems (CDSSs) are trusted by stakeholders, and indeed whether they are worthy of trust. Yet, what is meant by trust and trustworthiness is often implicit, and it may not be clear who or what is being trusted. We address these lacunae, focusing largely on the perspective(s) of clinicians on trust and trustworthiness in AI and CDSSs. Empirical studies suggest that clinicians’ concerns about their use include the accuracy of advice given and potential legal liability if harm to a patient occurs. Onora O’Neill’s conceptualisation of trust and trustworthiness provides the framework for our analysis, generating a productive understanding of clinicians’ reported trust issues. Through unpacking these concepts, we gain greater clarity over the meaning ascribed to them by stakeholders; delimit the extent to which stakeholders are talking at cross purposes; and promote the continued utility of trust and trustworthiness as useful concepts in current debates around the use of AI and CDSSs.

Abstract:Abstract Background In the UK, the solidaristic character of the NHS makes it one of the most trusted public institutions. In recent years, the introduction of data-driven technologies in healthcare has opened up the space for collaborations with private digital companies seeking access to patient data. However, these collaborations appear to challenge the public’s trust in the. Main text In this paper we explore how the opening of the healthcare sector to private digital companies challenges the existing social contract and the NHS’s solidaristic character, and impacts on public trust. We start by critically discussing different examples of partnerships between the NHS and private companies that collect and use data. We then analyse the relationship between trust and solidarity, and investigate how this relationship changes in the context of digital companies entering the healthcare system. Finally, we show ways for the NHS to maintain public trust by putting in place a solidarity grounded partnership model with companies seeking to access patient data. Such a model would need to serve collective interests through, for example, securing preferential access to goods and services, providing health benefits, and monitoring data access. Conclusion A solidarity grounded partnership model will help establish a social contract or licence that responds to the public’s expectations and to principles of a solidaristic healthcare system.

Trust and acceptability of data-driven clinical recommendations in everyday practice: A scoping review

Abstract:Background: Increasing attention is being given to the analysis of large health datasets to derive new clinical decision support systems (CDSS). However, few data-driven CDSS are being adopted into clinical practice. Trust in these tools is believed to be fundamental for acceptance and uptake but to date little attention has been given to defining or evaluating trust in clinical settings. Objectives: A scoping review was conducted to explore how and where acceptability and trustworthiness of data-driven CDSS have been assessed from the health professional's perspective. Methods: Medline, Embase, PsycInfo, Web of Science, Scopus, ACM Digital, IEEE Xplore and Google Scholar were searched in March 2022 using terms expanded from: "data-driven" AND "clinical decision support" AND "acceptability". Included studies focused on healthcare practitioner-facing data-driven CDSS, relating directly to clinical care. They included trust or a proxy as an outcome, or in the discussion. The preferred reporting items for systematic reviews and meta-analyses extension for scoping reviews (PRISMA-ScR) is followed in the reporting of this review. Results: 3291 papers were screened, with 85 primary research studies eligible for inclusion. Studies covered a diverse range of clinical specialisms and intended contexts, but hypothetical systems (24) outnumbered those in clinical use (18). Twenty-five studies measured trust, via a wide variety of quantitative, qualitative and mixed methods. A further 24 discussed themes of trust without it being explicitly evaluated, and from these, themes of transparency, explainability, and supporting evidence were identified as factors influencing healthcare practitioner trust in data-driven CDSS. Conclusion: There is a growing body of research on data-driven CDSS, but few studies have explored stakeholder perceptions in depth, with limited focused research on trustworthiness. Further research on healthcare practitioner acceptance, including requirements for transparency and explainability, should inform clinical implementation.
Trust as a dyadic mechanism of action: a call to explore patient-provider relationships in the twenty-first century

Abstract:Background: There is general agreement that trust between patients and providers influences patient knowledge, behaviors, and adherence to provider-recommendations--with subsequent impacts on patient health-related outcomes and provider practices. There is less academic agreement on the processes by which trust is formulated and changed over time and how trust with ongoing healthcare providers can influence health-related outcomes over time.Methods: This opinion draws on social constructionism and symbolic interactionism to posit the possibility that trust can emanate through the communication process, during which a patient and provider transmit and attend to words, images, and paralanguage to convey their states of being and to induce responses, usually acknowledgement, suasion, or physical behaviors, from one another.Results: Theoretical bases for this construct are provided as are qualitative, quantitative, and mixed measurement approaches for multiple healthcare settings.Conclusions: A mechanistic approach to understand how trust is established through patient-provider communication and how trust informs patient health-related outcomes can contribute over time to improve communication in healthcare encounters.

Blockchain in Healthcare Today

Abstract:A substantial administrative burden is placed on healthcare professionals as they manage and progress through their careers. Identity verification, pre-employment screening and appraisals: the bureaucracy associated with each of these processes takes precious time out of a healthcare professional's day. Time that could have been spent focused on patient care. In the midst of the COVID-19 crisis, it is more important than ever to optimize these professionals' time. This paper presents the synthesis of a design workshop held at the Royal College of Physicians of Edinburgh (RCPE) and subsequent interviews with healthcare professionals. The main research question posed is whether these processes can be re-imagined using digital technologies, specifically Self-Sovereign Identity? A key contribution in the paper is the development of a set of user-led requirements and design principles for identity systems used within healthcare. These are then contrasted with the design principles found in the literature. The results of this study confirm the need and potential of professionalising identity and credential management throughout a healthcare professional's career.

Abstract:The role of Artificial Intelligence (AI) in clinical decision-making raises issues of trust. One issue concerns the conditions of trusting the AI which tends to be based on validation. However, little attention has been given to how validation is formed, how comparisons come to be accepted, and how AI algorithms are trusted in decision-making. Drawing on interviews with collaborative researchers developing three AI technologies for the early diagnosis of pulmonary hypertension (PH), we show how validation of the AI is jointly produced so that trust in the algorithm is built up through the negotiation of criteria and terms of comparison during interactions. These processes build up interpretability and interrogation, and co-constitute trust in the technology. As they do so, it becomes difficult to sustain a strict distinction between artificial and human/social intelligence.

Fifty Years of Trust Research in Health Care: A Synthetic Review

Abstract:Policy Points First, policymakers can create conditions that will facilitate public trust in health care organizations by making creating and enforcing health policies that make exploitative behavior costly. Second, policymakers can bolster the trustworthiness of health care markets and organizations by using their regulatory authority to address and mitigate harm from conflicts-of-interest and regulatory capture. Third, policymakers and government agencies can further safeguard the public's trust by being transparent and effective about their role in the provision of health services to the public. Context: Trust plays a critical role in facilitating health care delivery and calls for rebuilding trust in health care are increasingly commonplace. This article serves as a primer on the trust literature for health policymakers, organizational leaders, clinicians, and researchers based on the long history of engagement with the topic among health policy and services researchers. Methods: We conducted a synthetic review of the health services and health policy literatures on trust since 1970. We organize our findings by trustor-trustee dyads, highlighting areas of convergence, tensions and contradictions, and methodological considerations. We close by commenting on the challenges facing the study of trust in health care, the potential value in borrowing from other disciplines, and imperatives for the future. Findings: We identified 725 articles for review. Most focused on patients' trust in clinicians (n = 499), but others explored clinicians' trust in patients (n = 11), clinicians' trust in clinicians (n = 69), and clinician/patient trust in organizations (n = 19) and systems (n = 127). Across these five subliteratures, there was lack of consensus about definitions, dimensions, and key attributes of trust. Researchers leaned heavily on cross-sectional survey designs, with limited methodological attention to the relational or contextual realities of trust. Trust has most commonly been treated as an independent variable related to attitudinal and behavioral outcomes. We suggest two challenges have limited progress for the field: (1) conceptual murkiness in terms and theories, and (2) limited observability of the phenomena. Insights from philosophy, sociology, economics, and psychology offer insights for how to advance both the theoretical and empirical study of health-related trust. Conclusion: Conceptual clarity and methodological creativity are critical to advancing health-related trust research. Although rigorous research in this area is challenging, the essential role of trust in population health necessitates continued grappling with the topic.
Trust in Data Science: Collaboration, Translation, and Accountability in Corporate Data Science Projects

Abstract:The trustworthiness of data science systems in applied and real-world settings emerges from the resolution of specific tensions through situated, pragmatic, and ongoing forms of work. Drawing on research in CSCW, critical data studies, and history and sociology of science, and six months of immersive ethnographic fieldwork with a corporate data science team, we describe four common tensions in applied data science work: (un)equivocal numbers, (counter)intuitive knowledge, (in)credible data, and (in)scrutable models. We show how organizational actors establish and re-negotiate trust under messy and uncertain analytic conditions through practices of skepticism, assessment, and credibility. Highlighting the collaborative and heterogeneous nature of real-world data science, we show how the management of trust in applied corporate data science settings depends not only on pre-processing and quantification, but also on negotiation and translation. We conclude by discussing the implications of our findings for data science research and practice, both within and beyond CSCW.

Abstract:It is widely acknowledged that trust plays an important role for the acceptability of data sharing practices in research and healthcare, and for the adoption of new health technologies such as AI. Yet there is reported distrust in this domain. Although in the UK, the NHS is one of the most trusted public institutions, public trust does not appear to accompany its data sharing practices for research and innovation, specifically with the private sector, that have been introduced in recent years. In this paper, we examine the question of, what is it about sharing NHS data for research and innovation with for-profit companies that challenges public trust? To address this question, we draw from political theory to provide an account of public trust that helps better understand the relationship between the public and the NHS within a democratic context, as well as, the kind of obligations and expectations that govern this relationship. Then we examine whether the way in which the NHS is managing patient data and its collaboration with the private sector fit under this trust-based relationship. We argue that the datafication of healthcare and the broader 'health and wealth' agenda adopted by consecutive UK governments represent a major shift in the institutional character of the NHS, which brings into question the meaning of public good the NHS is expected to provide, challenging public trust. We conclude by suggesting that to address the problem of public trust, a theoretical and empirical examination of the benefits but also the costs associated with this shift needs to take place, as well as an open conversation at public level to determine what values should be promoted by a public institution like the NHS.

Abstract:In this commentary, I argue that societies are facing major crises in trust that extends well beyond health systems, outlining actions that can enhance trust in public institutions and benefit health systems. There are also areas where strengthening health systems can serve to build broader trust and social cohesion, such as by providing social protection and health services that are responsive to people's needs. Understanding the dimensions of "trustworthiness" for different actors in a health system also provide insights on how to build, restore, and maintain trust. Whereas research evidence claims a foundational role for trustworthy intervention among health professions, other factors may be more influential for others. These include the credibility of the source, participation in the intervention with observably fair distribution of the benefits, the ethical behavior of key actors, reliability in service delivery and its results, transparent and consistent communications, and addressing breaches in trust.

A Vision to Enhance Trust Requirements for Peer Support Systems by Revisiting Trust Theories

Abstract:This vision paper focuses on the mental health crisis impacting healthcare workers (HCWs), which exacerbated by the COVID-19 pandemic, leads to increased stress and psychological issues like burnout. Peer Support Programs (PSP) are a recognized intervention for mitigating these issues. These programs are increasingly being delivered virtually through Peer Support Systems (PSS) for increased convenience and accessibility. However, HCWs perception of these systems results in fear of information sharing, perceived lack of safety, and low participation rate, which challenges these systems ability to achieve their goals. In line with the rich body of research on the requirements and properties of trustworthy systems, we posit that increasing HCWs trust in PSS could address these challenges. However, extant research focuses on objectively defined trustworthiness rather than perceptual trust because trustworthy requirements are viewed as more controllable and easier to operationalize. This study proposes a novel approach to elicit perceptual trust requirements by proposing a trust framework anchored in recognized trust theories from different disciplines that unpacks trust into its recognized types and their antecedents. This approach allows the identification of trust requirements beyond those already proposed for trustworthy systems, providing a strong foundation for improving the effectiveness of PSS for HCWs. Perceptions of the authority of health information. Case study: digital interactive television and the Internet

Abstract:As part of a Department of Health funded project nearly 2000 people were surveyed as to their use of two digital health information services, one on the Web and the other on digital interactive television (DiTV). The website was of a commercial company-Surgerydoor-and the DiTV service NHS based. This paper concentrates on the issue of trust in digital health information. Two of the main findings were that advertising was found to have an effect on trust, though the quality and type of advertising will impact in different ways on trustworthiness. DiTV subscribers who had either used the Living Health channel which carried NHS branded health information or had heard of the service, were more likely to say that the NHS was a symbol of trust for them compared with DiTV subscribers who had not used the service.
