Abstract:Purpose: Recent advancements in infrared sensing technology have made it possible to visualize tear film dynamics in real time, enabling evaluation of tear film quality during blinking. A retrospective clinical evaluation was conducted to explore this by grading videos of the tear film and comparing grading data with dry eye diagnostic results using the OCULUS keratograph (K5M). Methods: Videos were used to grade patients' tear film perturbations as compared with healthy control subjects. The grading was then correlated with the ocular surface disease index (OSDI) scores, tear film breakup time (TFBUT), tear meniscus height (TMH), corneal staining, redness, and meibography data. Results: Infrared imaging of the ocular surface revealed instantaneous and recurring dynamic characteristics of the tear film, allowing for the differentiation between normal and abnormal tear films. Abnormal features included a complete absence of a spreading tear film, hindered spreading of the tear film after blinking, areas of tear film instability, or a combination of the latter 2. Some of these features show a resemblance to the tear film appearance after fluorescein staining. The grading of these features correlated with TFBUT and, to a lesser extent, with TMH but did not show significant correlation with any other diagnostic data from the K5M. Furthermore, the speed of tear film spreading after blinking showed a positive correlation with TMH. Conclusions: Direct visualization of the tear film across the entire palpebral aperture using infrared sensing offers a noninvasive, reproducible, and rapid method for assessing the health and quality of the tear film.

Abstract:Purpose: To visualize the precorneal tear film with ultrahigh-resolution spectral domain optical coherence tomography, enabling quantification of tear film thickness in healthy subjects. Methods: A custom-built spectral domain optical coherence tomography system comprising a broadband titanium:sapphire laser operating at 800 nm and a high-speed charge coupled device (CCD) camera with a read-out rate of 47 kHz was used for measurement of precorneal tear film thickness. The system provides a theoretical axial resolution of 1.2 μm in tissue. The signal-to-noise ratio close to the zero delay was measured with 94 dB. A total of 26 healthy subjects were included in this study. Measurement was started immediately after blinking and averaged over a period of 1 second. In a subset of eight healthy subjects, the reproducibility of the approach was studied by measuring the tear film thickness every 10 minutes over 1 hour. Results: The average central tear film thickness of the measured population was 4.79 ± 0.88 μm. Reproducibility was very high, with an intraclass correlation coefficient of 0.97. A breakup of the tear film was observed in one subject after 14 seconds. Conclusions: Our data indicate that the human precorneal tear film can be measured with excellent reproducibility using ultrahigh-resolution optical coherence tomography. This technique may be a valuable tool in the management of dry eye syndrome.

Abstract:Dry Eye Disease (DED) is one of the most common ocular diseases: over five percent of US adults suffer from DED. Tear film instability is a known factor for DED, and is thought to be regulated in large part by the thin lipid layer that covers and stabilizes the tear film. In order to aid eye related disease diagnosis, this work proposes a novel paradigm in using computer vision techniques to numerically analyze the tear film lipid layer (TFLL) spread. Eleven videos of the tear film lipid layer spread are collected with a micro-interferometer and a subset are annotated. A tracking algorithm relying on various pillar computer vision techniques is developed. Our method can be found at https://easytear-dev.github.io/.

Abstract:Purpose: Tear evaporation should increase fluorescein concentration, causing fluorescence dimming from self-quenching for high but not low fluorescein concentration. This prediction was tested and compared to the predicted effect of "tangential flow" that fluorescence dimming should be similar for high and low concentrations. Methods: A custom optical system was used for video recording of tear film fluorescence in 30 subjects. The subjects were asked to blink at the start of the recording and try to keep their eyes open for the rest of the 60-second recording. An initial recording was made after instillation of 1 μL 0.1% fluorescein followed by further recordings at 5-minute intervals using 0.5% and 5% fluorescein. Results: Decay of fluorescence was considerably greater for the high (5%) concentration condition than for the low (0.1%) concentration. This is shown by "ratio images" (ratio of the intensity of a fluorescence image at a later time divided by that of an earlier image), fluorescence decay curves, fluorescence decay rates, and histograms of estimated tear thickness decrease. For example, for the high concentration condition, decay rates were higher than for the low concentration for all 30 subjects (P < 0.0001, binomial test). Additionally, breakup time was significantly reduced for the high compared to the low concentration condition. Conclusions: The greater fluorescence decay and more rapid breakup for the high concentration condition are the results expected if thinning and breakup are mainly due to evaporation, hence causing self-quenching. Fluorescence decay rate for the low concentration condition was not significantly greater than zero.

On the Effect of Liquid Crystal Orientation in the Lipid Layer on Tear Film Thinning and Breakup

Abstract:The human tear film (TF) is thin multilayer fluid film that is critical for clear vision and ocular surface health. Its dynamics are strongly affected by a floating lipid layer and, in health, that layer slows evaporation and helps create a more uniform tear film over the ocular surface. The tear film lipid layer (LL) may have liquid crystalline characteristics and plays important roles in the health of the tear film. Previous models have treated the lipid layer as a Newtonian fluid in extensional flow. We extend previous models to include extensional flow of a thin nematic liquid crystal atop a Newtonian aqueous layer with insoluble surfactant between them. We derive the resulting system of nonlinear partial differential equations for thickness of the LL and aqueous layers, surfactant transport and velocity in the LL. Evaporation is taken into account, and is affected by the LL thickness, internal arrangement of its rod-like molecules, and external conditions. Despite the complexity, this system still represents a significant reduction of the full system. We solve the system numerically via collocation with finite difference discretization in space together with implicit time stepping. We analyze solutions for different internal LL structures and show significant effect of the orientation. Orienting the molecules close to the normal direction to the TF surface results in slower evaporation, and other orientations have an effect on flow, showing that this type of model has promise for predicting TF dynamics.

Abstract:The preocular tear film has recently been described as a bilaminar structure,including lipid layer and aqueous/mucin gel layer.The lipid layer has two phases,a relatively thick outer layer,containing nonpolar lipids such as wax esters,sterol esters,hydrocarbons, and triglycerides;and a thin polar inner layer,predominantly consisting of phospholipids.It s confirmed that there are differences in meibomian fatty acid composition between healthy and evaporative dry eye patients.Thus,therapies targeted at replenishing or stabilizing the lipid layer are key points to the treatment of evaporative dry eye,either as monotherapy or in conjunction with therapies designed to enhance aqueous production.

Abstract:Purpose To explore the complex interface between the tear film, a unique mucosal fluid which is fundamental to ocular homeostasis and optimal vision, and an in-situ contact lens. This study exploits the use of a unique tear envelope (TE) extraction technique, which harvests the material-influenced layer of tear film that is in intimate contact with the lens during wear, to specifically investigate the influence of contact lens wear on tear film protein dynamics. Methods TEs were collected from freshly removed worn lens using a novel microcentrifuge 'piggyback' technique. Two distinct ex vivo studies were performed to investigate the key influencing factors involved. Non lens-wearing tear samples were also collected from all wearers. A compositional protein profile for each TE and tear film (TF) sample was obtained using an Agilent 2100 Bioanalyzer lab-on-a-chip microfluidic assay which detected proteins in a 14–230 kDa range. Results The data demonstrated that the TE protein compositional profile was quite distinct from either that of tear components deposited on the lens or those held in the tear menisci. For example, for one of the participant subgroups the tear protein average values in tears (n = 39) were determined at 35.2 ± 2.5 % lysozyme, 17.2 ± 0.6 % lipocalin, 7.3 ± 1.6 % IgA, 20.3 ± 1.3 % lactoferrin and 0.4 ± 0.4 % albumin as a function of total protein detected. In contrast, the average TE values were measured at 49.2 ± 3.7 %, 21.3 ± 3.9 %, 7.8 ± 1.6 % and 10.2 ± 1.7 % and 1.3 ± 2.8 % respectively with omafilcon A wear. In addition, 63 % of all TE samples (n = 180) (wearing lotrafilcon B and omafilcon A lenses) were albumin positive compared with only 19 % of all pre-lens insertion tear film samples (n = 237). Conclusions The TE approach not only allows material differentiation, but it can determine changes in the ocular host response that may otherwise be missed by sole non lens-wearing tear film sample analysis.

Abstract:Purpose: The purpose of this study was to examine Tear Film Imager (TFI, AdOM, Israel) generated parameters across controls and dry eye (DE) subgroups and examine the changes in TFI parameters with contact lens (CL) placement. Methods: The retrospective study (n = 48) was conducted at the Miami Veterans Hospital. Symptoms were assessed through validated questionnaires and signs of tear function by tear break-up time and Schirmer scores. Participants were grouped as 1) healthy, 2) evaporative, 3) aqueous deficient, and 4) mixed DE based on tear function. Seventeen individuals had a baseline scan and a repeat scan following CL placement. Descriptives were compared across groups and over time. Results: The median age was 27 years, 74% self-identified as White, 45% as male, and 51% as Hispanic. Subjects in the aqueous deficiency category had lower muco-aqueous layer thickness (MALT) (2672 vs. 3084 nm) but higher lipid layer thickness (47.5 vs. 38.3 nm), lipid break-up time (4.4 vs. 2 seconds), and interblink interval (13.9 vs. 5.4 seconds) compared with the evaporative group. Subjects in the evaporative group had the highest MALT values (3084 vs. 2988, 2672, 3053 nm) compared with healthy, aqueous-deficient, and mixed groups. Symptoms were not significantly correlated with TFI parameters. CL placement significantly decreased MALT values (2869 → 2175 nm, P = 0.001). Conclusions: The TFI provides unique information regarding the dynamic function of the tear film not captured by clinical examination. TFI generated metrics demonstrate a thinner aqueous layer in individuals with aqueous deficiency but highlight a thicker aqueous layer in those with evaporative DE.

Abstract:Purpose: To evaluate the efficacy of a recently proposed method for characterizing tear film dynamics using non-invasive high speed videokeratoscopy in assessing the loss of homeostasis of tear film. Methods: Thirty subjects, from a retrospective study, of which 11 were classified as dry eye and 19 as normal, were included. High speed videokeratoscopy measurements were performed using E300 videokeratoscope (Medmont Pty., Ltd, Melbourne, Australia). Raw data was analyzed using a recently proposed method to estimate the dynamics of the tear film, based on a fractal dimension approach. This method provides three time-varying indicators related to the regularity of the reflected rings: Tear Film Surface Quality (TFSQ) indicator, Breaks Feature Indicator (BFI) and Distortions Feature Indicator (DFI). From each indicator five parameters were extracted and analyzed, including non-invasive break up time, mean value of the indicator in the stability phase, mean value of the indicator in the whole inter blink interval, mean value of the indicator in the levelling phase and the general trend of the time series. Receiver Operating Characteristic were used to determine the sensitivity and specificity of each parameter in dry eye detection. Results: The best discrimination performance between dry eye and normal subjects was achieved with the BFI non-invasive break up time parameter, with an area under the curve of 0.85. For a cut off value of 10 s the sensitivity was 100% and the specificity 84%. Conclusions: The analyzed method improves the assessment of tear film homeostasis in comparison to previous high speed videokeratoscopy methods showing higher potential in assisting dry eye diagnosis.
Computer-synthesis of an interference color chart of human tear lipid layer, by a colorimetric approach

Abstract:Purpose: To synthesize an interference color chart for the specific tear lipid layer interference camera, DR-1, for the conversion of the tear film lipid layer thickness into color graphic information--that is, for the quantification of the tear interference image--by a colorimetric approach. Methods: Because the color of the tear lipid layer interference image is visualized by a white light source interference phenomena, to produce gamma-corrected red, green, and blue (RGB) values of a specific interference color at a certain tear lipid film thickness, XYZ tristimulus values of the Commission Internationale de l'Eclairage (CIE) were obtained. XYZ tristimulus values were calculated from the light source spectrum of the DR-1 camera, the color-matching function of CIE, and the reflectance of the tear interference image in wavelengths ranging from 380 to 780 nm. These calculated interference colors were synthesized ranging from 0 nm to 1000 nm of lipid film thickness to produce a color chart. The applicability of the new color chart in the analysis of the lipid layer thickness was tested on a healthy control subject with normal tear function and a patient with dry eye who had aqueous tear deficiency and meibomian gland obstruction. Results: The specific tear interference color chart for the DR-1 camera was obtained with RGB and XYZ tristimulus values. The interference chart ranged from 0 to approximately the 5th interference order. The interference colors from clinical DR-1 images could be converted to lipid thickness data by using the color chart system. Conclusions: A new tear interference color chart was developed in this study, which may be of benefit in converting tear interference color information to data describing the thickness of the tear film lipid layer.

Abstract:Meibomian Glands (MG) are sebaceous glands responsible for the production of meibum, the main component of the Tear Film Lipid Layer (TFLL). The TFLL facilitates the spread of the tear film over the ocular surface, provides stability and reduces tear evaporation. Alterations in meibum composition lead to different ocular alterations like Meibomian Gland Dysfunction (MGD) and subsequent Evaporative Dry Eye (EDE). The aim of the present study was to investigate the composition and abundance of meibum lipids and their relationship with eyelid margin abnormalities, lipid layer patterns and MG status. The study utilizes a lipidomic approach to identify and quantify lipids in meibum samples using an Elute UHPLC system. This system considered all four dimensions (mass/charge, retention time, ion mobility and intensity) to provide the accurate identification of lipid species. Samples were categorized as healthy or low/no signs of alteration (group 1) or severe signs of alteration or EDE/MGD (group 2). The current investigation found differences in Variable Importance in Projection lipid abundance between both groups for the MGD signs studied. Changes in meibum composition occur and are related to higher scores in eyelid margin hyperaemia, eyelid margin irregularity, MG orifice plugging, MG loss and lipid layer pattern.

Model for Ocular Tear Film Function

Abstract:Blepharitis patients have a number of disturbances in their tear film associated with meibomian gland dysfunction that affect evaporation and tear osmolarity. We tested a series of 156 consecutive patients, with a presumed diagnosis of blepharitis, dry eye, or allergic disease, and a series of 72 normals. We compared their tear film characteristics using tear osmolarity, tear volume, tear production (fluorophotometric and Schirmer test), tear turnover (decay constant), tear evaporation, and meibomian gland function evaluated by gland drop-out, expressed lipid viscosity, and volume. Of the 156 patients tested, we found 37 had dry eye, 10 had only allergic disease, 73 had meibomian gland dysfunction and dry eye, and 36 had only meibomian gland dysfunction. We created a model of the relative influence some of these factors had on each other using their correlation coefficients. The highest correlations for osmolarity were Schirmer test (-0.44), lipid volume low (-0.44), lipid viscosity high (0.39), gland drop-out (0.39), and tear evaporation (0.36). With regression analysis we accounted for 47% of the total variation in osmolarity, but only 17% of the variation in tear evaporation. We also present our classification system for blepharitis and dry eye patients based on our measurable physiologic parameters.

Modeling and Artificial Intelligence in Ophthalmology

Abstract:Purpose: Several elements are developed to quantitatively determine the contribution of different physical and chemical effects to tear breakup (TBU) in subjects with no self-reported history of dry eye (DED) or other ocular surface disease. Fluorescence (FL) imaging is employed to visualize the tear film (TF) and to determine TF thinning and potential TBU. Methods: An automated system using a convolutional neural network that was trained and tested on more than 50,000 images from FL imaging experiments was deployed. The trained system could identify multiple TBU instances in each trial. Once identified, extracted FL intensity data was fit by mathematical models that included tangential flow along the eye, evaporation, osmosis, and FL intensity of emission from the TF. The mathematical models consisted of systems of ordinary differential equations for the aqueous layer thickness, osmolarity, and the FL concentration; they are a local approximation to TF thinning and/or TBU dynamics. FL intensity was computed using the resulting thickness and FL concentration. Optimizing the fit of the models to the FL intensity data determined the mechanism(s) driving each instance of TBU and produced an estimate of the osmolarity within TBU. Results: Initial estimates for FL concentration and initial TF thickness agree well with prior results. Fits were produced for N = 467 instances of potential TBU from 15 non-DED subjects. The results showed a distribution of causes of TBU in these healthy subjects, as reflected by estimated flow and evaporation rates, which appear to agree well with previously published data. Final osmolarity depended strongly on the TBU mechanism, generally increasing with evaporation rate but complicated by the dependence on flow. Conclusion: The method has the potential to classify TBU instances based on the mechanism and dynamics, and to estimate the final osmolarity at the TBU locus. The results suggest that it might be possible to classify individual subjects and provide a baseline for comparison and potential classification of DED subjects.

Abstract:The tear film is a nourishing, lubricating and protecting layer that bathes the ocular surface. It is continuously replenished through cycles of production and elimination via evaporation, absorption and drainage. These processes are often referred to as tear film dynamics. Osmolality is an objective clinical measurement that provides insight into the balance of these complex tear film dynamics. Balanced tear production and elimination is vital for tear film integrity, stability and normal osmolality. Imbalances cause alterations of the tear film structure and composition, ultimately leading to tear film instability and measurable tear film hyperosmolality. Elevated tear film osmolality is considered a core mechanism in dry eye, forming the basis of dry eye symptoms and leading to ocular surface damage. Despite its immense potential in the diagnosis of dry eye, tear film osmolality is not commonly assessed. This review will focus on the current knowledge of tear film dynamics and tear film osmolality.

Abstract:A healthy tear film is very important for many major functions of the ocular surface. Dry eye disease is a significant clinical problem that needs to be solved but the poor correlation between clinical signs and reported symptoms makes it difficult for the clinician to apply a scientific basis to his clinical management. The problem is compounded by the difficulties of evaluating the tear film due to its transparency, small volume and complex composition. Practical insight into tear film composition would be very useful to the clinician for patient diagnosis and treatment but detailed analysis is restricted to expensive, laboratory-based systems. There is a pressing need for a simple test. The tear ferning test is a laboratory test but it has the potential to be applied in the clinic setting to investigate the tear film in a simple way. Drying a small sample of tear fluid onto a clean, glass microscope slide produces a characteristic crystallisation pattern, described as a 'tear fern'. This test is currently not widely used because of some limitations that need to be overcome but several studies have demonstrated its potential. Such limitations need to be resolved so that tear ferning could be used in the clinic setting to assess the tear film.

Abstract:This study explores the impact of electrostatic interactions and hydrogen bonding on tear film stability, a crucial factor for ocular surface health. While mucosal and meibomian layers have been extensively studied, the role of electrolytes in the aqueous phase remains unclear. Dry eye syndrome, characterized by insufficient tear quantity or quality, is associated with hyperosmolality, making electrolyte composition an important factor that might impact tear stability. Using a model buffer solution on a silica glass dome, we simulated physiologically relevant tear film conditions. Sodium chloride alone induced premature dewetting through salt crystal nucleation. In contrast, trace amounts of solutes with hydroxyl groups (sodium phosphate dibasic, potassium phosphate monobasic, and glucose) exhibited intriguing phenomena: quasi-stable films, solutal Marangoni-driven fluid influx increasing film thickness, and viscous fingering due to Saffman-Taylor instability. These observations are rationalized by the association of salt solutions with increased surface tension and the propensity of hydroxyl-group-containing solutes to engage in significant hydrogen bonding, altering local viscosity. This creates a viscosity contrast between the bulk buffer solution and the film region. Moreover, these solutes shield the glass dome, counteracting sodium chloride crystallization. These insights not only advance our understanding of tear film mechanics but also pave the way for predictive diagnostics in dry eye syndrome, offering a robust platform for personalized medical interventions based on individual tear film composition.
Lipidomics Analysis of the Tears in the Patients Receiving LASIK, FS-LASIK, or SBK Surgery

Abstract:Purpose: Tear film lipid layer (TFLL) plays a vital role in maintaining the tear film stability and, thus, the lipid composition of the tears could greatly affect the physiological function and biophysical integrity of the tear film. The objective of this study is to assess the tear lipid composition of the patients receiving laser-assisted in situ keratomileusis (LASIK), femtosecond LASIK (FS-LASIK), or sub-Bowman's keratomileusis (SBK) surgery preoperatively and postoperatively. Methods: Tear samples were collected from the left eye of the patient who receiving LASIK ( n = 10), FS-LASIK ( n = 10), or SBK ( n = 10) surgery in week 0, week 1, week 4, and week 52. A rapid direct injection shotgun lipidomics workflow, MS/MS ALL (<2 min/sample), was applied to examine the tear lipidome. Results: In week 52, the SBK group demonstrated a similar lipidome profile compared to week 0, while the FS-LASIK and LASIK groups shifted away from week 0. Two lipids, ganglioside (GD3) 27:4 and triacylglycerol (TAG) 59:3, were found to be associated with the lipidome changes preoperatively and postoperatively. No statistical significance was found in the overall lipid classes from the FS-LASIK group. The LASIK group showed significant alteration in the phospholipid and sphingolipid over time, while the SBK group demonstrated a significant difference in the (O-acyl)-ω-hydroxy fatty acid (OAHFA) and phospholipid. Conclusion: LASIK showed the greatest impact on the tear lipidome changes over time, while SBK demonstrated minimal impact among the three types of refractive surgeries after 1 year.
