Abstract:Background and Aims In recent decades, nighttime temperatures have increased faster than daytime temperatures. The increasing prevalence of nocturnal heat exposure may pose a significant risk to cardiovascular health. This study investigated the association between nighttime heat exposure and stroke risk in the region of Augsburg, Germany, and examined its temporal variations over 15 years. Methods Hourly meteorological parameters, including mean temperature, relative humidity, and barometric pressure, were acquired from a local meteorological station. A data set was obtained consisting of 11 037 clinical stroke cases diagnosed during warmer months (May to October) between the years 2006 and 2020. The average age of cases was 71.3 years. Among these cases, 642 were identified as haemorrhagic strokes, 7430 were classified as ischaemic strokes, and 2947 were transient ischaemic attacks. A time-stratified case-crossover analysis with a distributed lag non-linear model was used to estimate the stroke risk associated with extreme nighttime heat, as measured by the hot night excess (HNE) index after controlling for the potential confounding effects of daily maximum temperature and other climatic variables. Subgroup analyses by age group, sex, stroke subtype, and stroke severity were performed to identify variations in susceptibility to nighttime heat. Results Results suggested a significant increase in stroke risk on days with extreme nighttime heat (97.5% percentile of HNE) (odds ratio 1.07, 95% confidence interval 1.01–1.15) during the full study period. When comparing the results for 2013–20 with the results for 2006–12, there was a significant increase (P < .05) in HNE-related risk for all strokes and specifically for ischaemic strokes during the more recent period. Furthermore, older individuals, females, and patients with mild stroke symptoms exhibited a significantly increased vulnerability to nighttime heat. Conclusions This study found nocturnal heat exposure to be related to elevated stroke risk after controlling for maximum daytime temperature, with increasing susceptibility between 2006 and 2020. These results underscore the importance of considering nocturnal heat as a critical trigger of stroke events in a warming climate.

Abstract:Background: Acute ischemic stroke (AIS) is a major global public health issue. There is limited research on the relationship between ambient temperature and AIS hospital admissions, and the results are controversial. Our objective is to assess the short-term impact of ambient temperature on the risk of AIS hospital admissions in Yancheng, China. Methods: We collected data on daily AIS hospital admissions, meteorological factors, and air quality in Yancheng from 2014 to 2019. We used Poisson regression to fit generalized linear models and distributed lag non-linear models to explore the association between ambient temperature and AIS hospital admissions. The effects of these associations were evaluated by stratified analysis by sex and age. Results: From 2014 to 2019, we identified a total of 13,391 AIS hospital admissions. We observed that the influence of extreme cold and heat on admissions for AIS manifests immediately on the day of exposure and continues for a duration of 3-5 days. Compared to the optimal temperature (24.4 °C), the cumulative relative risk under extreme cold temperature (-1.3 °C) conditions with a lag of 0-5 days was 1.88 (95%CI: 1.28, 2.78), and under extreme heat temperature (30.5 °C) conditions with a lag of 0-5 days was 1.48 (95%CI: 1.26, 1.73). Conclusions: There is a non-linear association between ambient temperature and AIS hospital admission risk in Yancheng, China. Women and older patients are more vulnerable to non-optimal temperatures. Our findings may reveal the potential impact of climate change on the risk of AIS hospital admissions.
Spatially Explicit Mapping of Heat Health Risk Utilizing Environmental and Socioeconomic Data

Abstract:Extreme heat events, a leading cause of weather-related fatality worldwide, are expected to intensify, last longer, and occur more frequently in the near future. In heat health risk assessments, a spatiotemporal mismatch usually exists between hazard (heat stress) data and exposure (population distribution) data. Such mismatch is present because demographic data are generally updated every couple of years and unavailable at the subcensus unit level, which hinders the ability to diagnose human risks. In the present work, a human settlement index based on multisensor remote sensing data, including nighttime light, vegetation index, and digital elevation model data, was used for heat exposure assessment on a per-pixel basis. Moreover, the nighttime urban heat island effect was considered in heat hazard assessment. The heat-related health risk was spatially explicitly assessed and mapped at the 250 m × 250 m pixel level across Zhejiang Province in eastern China. The results showed that the accumulated heat risk estimates and the heat-related deaths were significantly correlated at the county level (Spearman's correlation coefficient = 0.76, P ≤ 0.01). Our analysis introduced a spatially specific methodology for the risk mapping of heat-related health outcomes, which is useful for decision support in preparation and mitigation of heat-related risk and potential adaptation.

Abstract:Recent decades of epidemiological and clinical research have suggested that heat exposure could be a potential risk factor for ischemic stroke. Despite climate factors having a minor impact on individuals compared with established risk factors such as smoking, their widespread and persistent effects significantly affect public health. The mechanisms by which heat exposure triggers ischemic stroke are currently unclear. However, several potential mechanisms, such as the impact of temperature variability on stroke risk factors, inflammation, oxidative stress, and coagulation system changes, have been proposed. This article details the potential mechanisms by which heat exposure may induce ischemic stroke, aiming to guide the prevention and treatment of high-risk groups in hot climates and support public health policy development.

Abstract:Stroke, a leading issue of public health has been linked to outdoor temperature. However, the association has been rarely examined by nation-scale study in China. Making use of 11,144 stroke (including ischemic and hemorrhagic stroke) or transient ischemic attack (TIA) patients from 339 communities and temperature estimates from an assimilation of in-situ observations, satellite measurements and simulates of Weather Research Forecast model, we conducted a case-crossover study during 2000-2016. Occurrence of stroke or TIA was associated to temperature anomaly by a lag-distributed nonlinear model. The variations of the association were further explored by strata of subpopulations and by geographic regions. Attributable fraction was finally derived to quantify risk burden of stroke. Either ischemic or hemorrhagic stroke was strongly associated to both cold or heat temperature at different lags, while TIA was weakly linked to cold weather at lag 0 day. During lag 0-2 day, the risk of stroke increased by 20% (95% confidence interval: 3%, 40%) for extreme cold or 19% (3%, 37%) for extreme heat. We also found that re-occurred stroke was more strongly associated to temperature than first stroke. In China, irregular variation of temperature might attributed 3.42% of stroke attacks. More stroke occurrences were attributed to heat (2.05%) than cold (1.38%). During 2000-2016, stroke risk was estimated to increase by 0.02% per year with the changed temperature, particularly the increment of heat events. Our findings enrich epidemiological evidences on the association between stroke and temperature, and support China’s policymakers to plan stroke prevention and climate change mitigation.

Abstract:BACKGROUND:Little evidence exists on the relationship between heat and subtypes of stroke mortality, especially in China. Moreover, few studies have reported the effect modification by individual characteristics on heat-related stroke mortality. In this study, we aimed to evaluate the effect of heat exposure on total, ischemic, and hemorrhagic stroke mortality and its individual modifiers in 12 cities in Jiangsu Province, China during 2009 to 2013.METHODS:We first used a distributed lag non-linear model with quasi-Poisson regression to examine the city-specific heat-related total, ischemic, and hemorrhagic stroke mortality risks at 99th percentile vs. 75th percentile of daily mean temperature in the whole year for each city, while adjusting for long-term trend, season, relative humidity, and day of the week. Then, we used a random-effects meta-analysis to pool the city-specific risk estimates. We also considered confounding by air pollution and effect modification by gender, age, education level, and death location.RESULTS:Overall, the heat-related mortality risk in 12 Jiangsu cities was 1.54 (95%CI: 1.44 to 1.65) for total stroke, 1.63 (95%CI: 1.48 to 1.80) for ischemic stroke, and 1.36 (95%CI: 1.26 to 1.48) for hemorrhagic stroke, respectively. Estimated total, ischemic, and hemorrhagic stroke mortality risks were higher for women versus men, older people versus younger people, those with low education levels versus high education levels, and deaths that occurred outside of hospital. Air pollutants did not significantly influence the heat-related stroke mortality risk.CONCLUSIONS:Heat exposure significantly increased both ischemic and hemorrhagic stroke mortality risks in Jiangsu Province, China. Females, the elderly, and those with low education levels are particularly vulnerable to this effect.

Abstract:Abstract There is no consensus in previous studies on the association between ischemic stroke (IS) and extreme ambient temperature. Few studies have explored the effects of extreme ambient temperature on different etiologies of IS. We collected 3079 IS cases from two stroke centers in Central China (Wuhan and Yichang cities) continuously from January 1, 2016 to December 31, 2017, meteorological data and pollutant data published by China Meteorological Data Network for the same period. According to the Trial of ORG 10172 in Acute Stroke Treatment (TOAST), IS cases were divided into large artery atherosclerosis (LAA), cardioembolism (CE), small artery occlusion (SAO), and stroke of other etiology (SOE). Distributed lag nonlinear models of time series analysis were used to explore the association between extreme ambient temperature and the onset of IS and its etiology subtypes after controlling for potential covariates. By comparing single-day relative risk (RR) with 95% confidence interval (CI), extreme ambient temperature did not increase the onset of total IS, SAO or SOE; extreme cold temperature increased the onset of LAA (RR =1.70,95%, CI =1.04–2.75, threshold = 3.5 °C, log = 2) and CE (RR =2.96,95%, CI = 1.05–8.39, threshold = 2.2 °C, log = 0; RR =2.92,95%, CI =1.11–7.71, threshold = 3.5 °C, log = 0). The overall cumulative association between ambient temperature and the onset of LAA and SOE was statistically significant. There was no association between extreme temperature and total IS. Extreme cold temperature is a risk factor for the onset of LAA and CE.

Abstract:International Journal of Stroke, Ahead of Print. Background:Stroke risks associated with rapid climate change remain controversial due to a paucity of evidence.Aims:To examine the risk of subarachnoid hemorrhage (SAH), intracranial hemorrhage (ICH), and ischemic stroke (IS) associated with meteorological parameters.Methods:In this time-stratified case-crossover study, adult patients hospitalized for their first stroke between 2011 and 2020 from the insurance claims data in Taiwan were identified. The hospitalization day was designated as the case period, and three or four control periods were matched by the same day of the week and month of each case period. Daily mean and 24-h variations in ambient temperature, relative humidity, air pressure, and apparent temperature were measured. Conditional logistic regression models were applied to assess the risk of stroke associated with exposure to weather variables, using the third quintile as a reference, controlling for air pollutant levels.Results:There were 7161 patients with SAH, 40,426 patients with ICH, and 107,550 patients with IS. There was an inverse linear relationship between mean daily temperature and apparent temperature with ICH. Elevated mean daily atmospheric pressure was associated with an increased risk of ICH. A greater decrease in apparent temperature over a 24-h period was associated with increased risk of ICH but decreased risk of IS (odds ratio (95% confidence interval) for the first vs. third quintile of changes in apparent temperature, 1.141 (1.053–1.237) and 0.946 (0.899–0.996), respectively).Conclusions:There were considerable differences in short-term associations between meteorological parameters and three main pathological types of strokes.Data access statement:The authors have no permission to share the data.

Abstract:Objective: To examine the association between short-term exposure to air pollution and ischemic stroke (IS) onset and the role of lifestyles on the association. Methods: We performed a time-stratified case-crossover study to estimate the association between short-term exposure to air pollution and IS onset in Yinzhou, China from July 1st, 2015 to May 31st, 2020. Conditional logistic regression models were used to assess the effect of air pollution on IS onset. Furthermore, we explored whether these effects were modified by lifestyle factors by using stratified analyses. Results: A total of 700 subjects were included in analyses. We found that positive associations between exposure to O-3 and IS onset with the ORIQRs of 1.23 (95% CI: 1.01-1.49) at lag04 and 1.28 (95% CI: 1.03-1.59) at lag06, respectively. Stronger associations were observed between NO2 at lag04 and lag06 and IS onset after adjusting for other air pollutants, and the corresponding ORIQRs were 2.08 (95% CI: 1.37-3.15) and 2.04 (95% CI: 1.27-3.27), respectively. However, no association was observed for exposure to particulate matters and IS onset. Stronger associations were observed for NO2 and risk of IS onset among abnormal weight subjects, alcohol drinkers, smokers, and subjects who exercised 1-4 times per week or barely exercised every week. We also observed stronger associations of SO2, CO, PM2.5 and PM10 with IS onset among subjects who exercised 1-4 times per week. Conclusions: In this case-crossover study, we found positive associations between short-term exposure to air pollution, especially for NO2, and risk of IS onset. Moreover, our study highlighted that lifestyle factors, such as alcohol consumption and physical activity, had significant impacts on the association of ambient air pollution with risk of IS onset.
Both Low And High Temperature May Increase The Risk Of Stroke Mortality

Abstract:Objective: To examine temperature in relation to stroke mortality in a multicity time series study in China.Methods: We obtained data on daily temperature and mortality from 8 large cities in China. We used quasi-Poisson generalized additive models and distributed lag nonlinear models to estimate the accumulative effects of temperature on stroke mortality across multiple days, adjusting for long-term and seasonal trends, day of the week, air pollution, and relative humidity. We applied the Bayesian hierarchical model to pool city-specific effect estimates.Results: Both cold and hot temperatures were associated with increased risk of stroke mortality. The potential effect of cold temperature might last more than 2 weeks. The pooled relative risks of extreme cold (first percentile of temperature) and cold (10th percentile of temperature) temperatures over lags 0-14 days were 1.39 (95% posterior intervals [PI] 1.18-1.64) and 1.11 (95% PI 1.06-1.17), compared with the 25th percentile of temperature. In contrast, the effect of hot temperature was more immediate. The relative risks of stroke mortality over lags 0-3 days were 1.06 (95% PI 1.02-1.10) for extreme hot temperature (99th percentile of temperature) and 1.14 (95% PI 1.05-1.24) for hot temperature (90th percentile of temperature), compared with the 75th percentile of temperature.Conclusions: This study showed that both cold and hot temperatures were associated with increased risk of stroke mortality in China. Our findings may have important implications for stroke prevention in China.
Spatially Explicit Assessment of Heat Health Risks Using Multi-source Data:A Case Study of the Yangtze River Delta Region, China

Abstract:The increase in the frequency and intensity of extreme heat events (EHEs), potentially associated with climate change in the near future, highlights the importance of heat health risks assessment, which is an important starting point for the reduction of heat-related mortality and sustainable development. However, there is a spatiotemporal mismatch between hazard data (at pixel-level) and exposure data (at census unit level) in heat health risks assessment. Based on multisensor remote sensing data and demographic and socioeconomic statistical data, we used a human settlement index to assess heat exposure. Heat health risks and its driving factors were spatially explicitly assessed and mapped at the 250 m × 250 m pixel-level across the Yangtze River Delta region (YRD). The visual inspection suggests that the high risks areas were mainly distributed in urbanized areas of YRD, including the downtown of Shanghai, Changzhou, Hangzhou, Ningbo, Wuxi, Jiaxing and Taizhou, which was mostly driven by the high human exposure and heat hazard index. In less-urbanized cities and the suburban and rural areas of mega-cities, the heat health risks come second. Even though the human exposure index was low in other less-developed areas, the heat health risks in those areas were high due to the high heat hazard and human vulnerability index. It's of great importance to identify the driving factors of high heat health risks to provide science-based support for adaptation strategies and emergency planning.

The Asia and Pacific region have been more vulnerable to climate change and extreme heat exposure than other regions of the world. Despite the growing concern for heat waves, few studies have investigated heat exposure and the risk of Stroke in the elderly considering effect of Asian meteorological factors. Purpose We aimed to examine the association between heat exposure and the risk of Stroke in the elderly and to evaluate possible effect-measure modifications by the rainy season, which is a characteristic climatic occurrence in East Asia. Methods Our study was designed as a time-stratified case-crossover. The study included 3367 residents in the city, Japan, aged 65 years or older who were dispatched to emergency hospitals between 2012 and 2019 for the onset of Stroke during and a few months after the rainy seasons (Picture 1). We examined the association between temperature and Stroke onset during and a few months after the rainy seasons with conditional logistic analysis adjusted for relative humidity, barometric pressure, and particulate matter less than 2.5 μm in diameter (pm 2.5). Furthermore, we examined analyses on effect-measure modification where is the exposure of a 1°C increase in temperature and potential effect modifier as stratified during and after the rainy seasons. Results Heat exposure during one month after the rainy season was associated with Stroke risk; the adjusted odds ratio (OR) for a 1°C increase in temperature was 1.34 (95% confidence interval [CI]: 1.29-1.39). In stratified type of the stroke, Hemorrhagic stroke (OR: 1.37, 95% CI: 1.24-1.51), Ischemic stroke (OR: 1.37, 95% CI: 1.24-1.53), and Transient ischemic attack (OR: 1.43, 95%CI: 1.23-1.68) were significantly associated with heat exposure during one month after the rainy season respectively (Picture 2). In the results of analyses on effect modification, one month after the rainy season group was highest than other groups when reference was during the rainy seasons (OR: 1.34, 95% CI: 1.3-1.39, P<0.001 [Measure of effect modification on additive scale]). Conclusions The present study suggests that heat exposure increases the risk of Stroke onset during the month after the end of the rainy season in the elderly. We propose that public health measures, such as maintenance designed high-insulated housing and improved air conditioning control, be implemented in the Asia and Pacific region to address heat exposure during one month after the rainy season, which can affect the incidence of stroke in the elderly.

Quantifying the contribution of temperature anomaly to stroke risk in China

Abstract:Abstract Epidemiological studies have quantitatively linked adverse health outcomes, including stroke, to ambient temperature. However, such analyses cannot be applied to estimate disease burden attributable to climate change, because they assume a theoretical minimum risk reference based on an optimal temperature, which is a post hoc estimator that cannot be generalized to the unstudied locations. In this study, we used temperature anomaly (TA), an alternative indicator of climate change, in epidemiological studies to address this issue. We associated geo-coded nationwide stroke data (n = 11 144) from 2000 to 2016 in China to TA with a lag-distributed nonlinear model. For 0–2 lagged days, the risk of stroke increased by 20% (95% confidence interval [CI]: 3%–40%) for irregular cold and 19% (95% CI: 3%–37%) for irregular heat. The exposure-response function was estimated as a U-shaped curve centered at a TA value of 0. Based on the function, an estimated 3.42% of stroke cases were attributable to TA. In addition, there were more strokes attributable to heat (2.05%) than cold (1.38%). TA-attributable strokes due to climate change increased by 0.019% (95% CI: 0.008%–0.031%) per year. Furthermore, the long-term trend was dominated by the increase in events involving heat. In contrast to cold-attributable stroke, which potentially decreased by 0.004% (95% CI: −0.005%–0.013%) per year, the heat-attributable burden increased by 0.023% (95% CI: 0.012%–0.034%) per year. We also found that ischemic stroke was associated with cold (odds ratio [OR] = 1.18, 95% CI: 1.00–1.38), while hemorrhagic stroke was associated with heat (OR = 1.66, 1.07–2.58). Our findings are of importance to public health for climate change mitigation and for predictions of future effects in climate change scenarios.

Spatial Analysis of the Effect of the 2010 Heat Wave on Stroke Mortality in Nanjing, China.

Abstract:To examine the spatial variation of stroke mortality risk during heat wave, we collected 418 stroke mortality cases with permanent addresses for a severe heat wave (July 28-August 15, 2010) and 624 cases for the reference period (July 29-August 16, 2009 and July 27-August 14, 2011) in Nanjing, China. Generalized additive models were used to explore the association between location and stroke mortality risk during the heat wave while controlling individual-level risk factors. Heat wave vulnerability was then applied to explain the possible spatial variations of heat-wave-related mortality risk. The overall risk ratio (95% confidence intervals) of stroke mortality due to the heat wave in Nanjing was 1.34 (1.21 to 1.47). Geolocation was found to be significantly associated with the heat-wave-related stroke mortality risk. Using alternative reference periods generated similar results. A district-level risk assessment revealed similar spatial patterns. The highest stroke mortality risk observed in Luhe district was due to the combination of high heat exposure and high vulnerability. Our findings provide evidence that stroke mortality risk is higher in rural areas during heat waves and that these areas require future interventions to reduce vulnerability.
Effect of ambient temperature on stroke onset: a time-series analysis between 2003 and 2014 in Shenzhen, China

Abstract:Objective Evidence on the relationship between ambient temperature and morbidity of different stroke subtypes in China is limited. This study aimed to assess the influence of ambient temperature on stroke risk in Shenzhen, China. Methods From 1 January 2003 to 31 December 2014, 114 552 stroke cases in Shenzhen were collected. A generalised additive model with quasi-Poisson regression combined with a distributed lag non-linear model was applied to evaluate the temperature effects on stroke subtypes. Furthermore, this study explored the variability of the effects across sex, age and education. Results The immediate heat effects on ischaemic stroke (IS) and the persistent effects of ambient temperature on intracerebral haemorrhage (ICH) were significant. Overall, the cold-related relative risks (RRs) of IS, ICH and subarachnoid haemorrhage (SAH) were 1.02 (0.97–1.07), 1.16 (1.04–1.30) and 1.12 (0.61–2.04), whereas the heat-related RRs were 1.00 (0.97–1.04), 0.80 (0.73–0.88) and 1.05 (0.63–1.78), respectively. For IS, a weakly beneficial cold effect was found among men while a detrimental heat effect among both men and women, the elderly and higher-educated population at lag0. However, regarding ICH, the temperature effects in men, the young and higher-educated population are stronger at lag0–4, lag0–7 as cold reveals threat and heat reveals protection. Conclusion Responses of diverse stroke subtypes to ambient temperature varied. Effective measures should be taken to increase public awareness about the effects of ambient temperature on stroke attack and to educate the public about self-protection.

Abstract:Following a heat wave in January 2014 in Adelaide, state capital of South Australia, we asked the question whether extreme heat was associated with an increase in stroke incidence. We found in the literature that the association between stroke presentation to hospital and meteorological factors has long been a topic of debate and subject to numerous studies. The literature indicated that an association between heat waves and an increase in admissions for stroke was unlikely in Australia and the United States. We suggest that it may be inappropriate to generalize this conclusion to other countries and rural areas. In view of the global climate change debate, we suggest that prospective studies be focused in developing countries and rural areas to assess the real impact of extreme heat on respective populations to better inform stroke physicians and health policy makers.

Abstract:Background: Recent studies have reported the association between heat waves and stroke mortality. However, little evidence exists on the relationship between heat waves and cause-specific stroke mortality, especially in China. Moreover, few studies have reported the effect modification by individual characteristics on heat waves-related stroke mortality. In this study, we aimed to evaluate the effect of heat waves on ischemic, hemorrhagic, and total stroke mortality and its individual modifiers in 12 cities in Jiangsu Province, China during 2009 to 2013. Methods: We first used a distributed lag non-linear model with quasi-Poisson regression to examine the city-specific heat-related total, ischemic, and hemorrhagic stroke mortality risks in each city, while adjusting for long-term trend, season, relative humidity, and day of the week. Then, we used a Bayesian hierarchical model to pool the city-specific risk estimates. Air pollution levels were also controlled in a sensitivity analysis. To explore the potential effect modification by individual characteristics, we conducted separate analyses for total stroke mortality stratified by gender, age, education level, and death location. Results: Overall, the relative mortality risk in 12 Jiangsu cities was 1.54 (95%CI: 1.42 to 1.67) for total stroke, 1.63 (95%CI: 1.44 to 1.85) for ischemic stroke, and 1.36 (95%CI: 1.23 to 1.51) for hemorrhagic stroke, respectively. Estimated total stroke mortality risk was higher for women versus men, older people versus younger people, those with low education levels versus high education levels, and deaths occurred outside of hospital in Jiangsu. Sensitivity analysis showed that air pollutants did not significantly influence the heat waves-related stroke mortality. Conclusions: Heat waves significantly increased both ischemic and hemorrhagic stroke mortality risks in Jiangsu Province, China. These stroke mortality risks were modified by some individual characteristics.

Abstract:BACKGROUND: This study aimed to explore the effects of heatwaves and cold spells on blood pressure, thrombus formation, and systemic inflammation at admission in patients with ischemic stroke.METHODS: Data of patients with ischemic stroke who were admitted to the Second Hospital of Tianjin Medical University between May 2014 and March 2019 were reviewed, along with meteorological data from the same time period. A total of 806 clinically confirmed patients with ischemic stroke (34-97 years old) were included in the final analysis. Heatwaves and cold spells were defined as ≥2 consecutive days with average temperature >95<sup>th</sup> percentile (May-August) and <5<sup>th</sup> percentile (November-March), respectively. Coagulation parameters, inflammation indices, blood pressure, and neurological impairment were evaluated within 24 hours of admission. General linear and logistic regression models were created to investigate the relationships of heatwaves and cold spells with the examination results of patients with ischemic stroke at admission.RESULTS: After adjustment for potential environmental confounders, heatwaves were positively associated with high systolic blood pressure (SBP) (β=8.693, P=0.019), diastolic blood pressure (DBP) (β=3.665, P=0.040), reduced thrombin time (TT) (β=-0.642, P=0.027), and activated partial thromboplastin time (APTT) (β=-1.572, P=0.027) in ischemic stroke patients at admission. Cold spells were positively associated with high SBP (β=5.277, P=0.028), DBP (β=4.672, P=0.012), fibrinogen (β=0.315, P=0.011), globulin (β=1.523, P=0.011), and reduced TT (β=-0.784, P<0.001) and APTT (β=-1.062, P=0.024). Cold spells were also associated with a higher risk of respiratory infection [odds ratio (OR) =2.677, P=0.001].CONCLUSIONS: Exposure to heatwaves or cold spells was associated with blood pressure and coagulation at admission in patients with ischemic stroke. Cold spells also resulted in higher levels of inflammation. These findings suggest that changes in coagulation, blood pressure, and inflammation may be the potential biological mechanisms underlying the cerebrovascular effects of exposure to extreme temperatures.

Abstract:ISEE-0138 Background and Objective: In recent years due to global warming and heatwaves, there is more and more concern by both governments and researchers of most countries, including China, on the potential heath consequences. This study explores the association between the daily maximum temperature and hospital emergency room visits for cardiovascular and cerebrovascular disease (ICD-10: I00∼I99) in different seasons in Beijing, China. Method: The data of the daily hospital emergency room visits for cardiovascular and cerebrovascular disease (ICD-10: I00∼I99) from one of Peking University Hospitals in Haidian District of Beijing, China during 2004-2008, and the corresponding meteorological and air pollution data were collected. The seasonal-stratified case-crossover design, and Logistic multiple regression model was used for the data analysis. Results: After adjusting the influence of relative humidity, wind speed, and atmospheric pressure, it was found that for each 1°C increase in daily maximum temperature, corresponding increases in the hospital emergency room visits for the diseases were 17.3% (OR = 1.173, 95%CI:1.149-1.197) and 4.2% (OR = 1.042, 95% CI: 1.011-1.074) in spring and summer of the years, respectively (P < 0.05); contrarily, there was decrease by 25% in the autumn (OR = 0.75, 95% CI: 0.727-0.773). There was no statistically significant association between both in the winter seasons (P ≤ 0.05). Conclusion: The increase of daily maximum temperature may be a risk factor of daily hospital emergency room visits for cardiovascular and cerebrovascular diseases in spring and summer. However, it maybe a preventive factor in autumn. It suggests that the impacts of daily maximum temperature may be a two-edged sword on human health.

Abstract:Heat stroke is a serious heat-related health outcome that can eventually lead to death. Due to the poor accessibility of heat stroke data, the large-scale relationship between heat stroke and meteorological factors is still unclear. This work aims to clarify the potential relationship between meteorological variables and heat stroke, and quantify the meteorological threshold that affected the severity of heat stroke. We collected daily heat stroke search index (HSSI) and meteorological data for the period 2013-2020 in 333 Chinese cities to analyze the relationship between meteorological variables and HSSI using correlation analysis and Random forest (RF) model. Temperature and relative humidity (RH) accounted for 62% and 9% of the changes of HSSI, respectively. In China, cases of heat stroke may start to occur when temperature exceeds 36°C and RH exceeds 58%. This threshold was 34.5°C and 79% in the north of China, and 36°C and 48% in the south of China. Compared to RH, the threshold of temperature showed a more evident difference affected by altitude and distance from the ocean, which was 35.5°C in inland cities and 36.5°C in coastal cities; 35.5°C in high-altitude cities and 36°C in low-altitude cities. Our findings provide a possible way to analyze the interaction effect of meteorological variables on heat-related illnesses, and emphasizes the effects of geographical environment. The meteorological threshold quantified in this research can also support policymaker to establish a better meteorological warning system for public health.
