Abstract:Background Since the outbreak of renal diseases among children who had consumed melamine-tainted powdered formula in 2008, urinary system ultrasound examination was carried out for these children at our hospital. This study aimed to investigate the ultrasonographic characteristics of urolithiasis caused by melamine-tainted powdered formula. Methods From September 12 to October 9, 2008, urinary system ultrasound examination for children after consumption of melamine-tainted powdered formula was carried out at the Children’s Hospital of Zhejiang University, China. A total of 15 577 infants and children were subjected to the examination. A questionnaire with items including demographic characteristics, history of exposure and clinical symptoms were completed before examination. Results Among the 15 577 children, 7988 were boys and 7589 girls with age ranging from 1 month to 15 years (mean age, 22 months). Urolithiasis was found in 562 patients with an identification rate of 3.6%. Urolithiasis was identified mainly in the children aged 36 months or younger (88.6%). Unilateral kidney calculi were found in 431 children and bilateral kidney calculi in 131 children. Eleven of these children also had ureteric calculi, 1 had bladder calculi, 2 had urethral calculi, and 2 had gallbladder calculi. Fifteen children had urinary tract obstruction. Ultrasonographic characteristics of these melamine calculi were different from the calcium oxalate calculi, i.e., the lesions were less echogenic, had a more “sandy” appearance and were less dense. The posterior portion of the calculi could be observed and was accompanied with a feeble or absent acoustic shadow. Conclusions Melamine-induced calculi were ultrasonographically less echogenic, sand-like and discrete; the posterior portion of the calculi was accompanied with a feeble or absent acoustic shadow. It might be a new type of urolithiasis in children.
[Acquired pure red cell aplasia associated with relapsed non-Hodgkin's lymphoma: a case report-improvement of PRCA after acute hepatitis].

Abstract:A 47-year-old male patient was admitted because of anemia. He had been diagnosed as non-Hodgkin's lymphoma (Follicular mixed, B cell type, stage ISA) by splenectomy two years before. Bone marrow examination on admission revealed lymphoma cell infiltration and marked decrease in erythroid cells. These findings confirmed relapsed lymphoma with acquired pure red cell aplasia. After several courses of combination chemotherapy, lymphoma cells disappeared from bone marrow, but PRCA was not improved. In this case there were two times remission of PRCA. At first time, acute B type hepatitis occurred during the chemotherapy, anemia improved transiently. At the second time, mild acute hepatitis associated with herpes zoster occurred. Twenty days after hepatic injury, PRCA was improved, and continued in remission state till present day. To disclose the mechanism of PRCA in this case, erythroid colony assay of marrow cells was performed. This showed the presence of inhibitory factor in patient's serum at PRCA state, that was considered to be related to the occurrence of PRCA. These findings suggest that the improvement of PRCA was associated with the changes on immunological condition after acute hepatitis in this case.
Diagnosis, treatment and follow-up of 25 patients with melamine-induced kidney stones complicated by acute obstructive renal failure in Beijing Children’s Hospital

Abstract:A total of 25 Chinese patients aged 6 to 36 months hospitalised at Beijing Children’s Hospital due to melamine-induced kidney stones complicated by acute obstructive renal failure in 2008 were included in a study in order to diagnose and treat these special cases more effectively. Feeding history, clinical presentation, ultrasound findings, treatments and effects were summarised. Twelve to seventeen months follow-up was reported also. Ultrasound examination showed that calculi were located at the kidney and ureters. Stones were composed of both uric acid and melamine in a molar ratio of 1.2:1 to 2.1:1. Treatments providing liquid plus alkalisation of urine proved to be effective in helping the patients pass the stones. Surgical intervention was needed in severe cases. Renal function returned to normal in all 25 patients after various durations of therapy. Sixty-eight percent of the patients expelled all of the calculi within 3 months, 90% in 6 months and 95% in 9 months, without sequelae till now. Melamine-contaminated milk formula can cause kidney stones in infants, which should be diagnosed by feeding history, clinical symptoms and ultrasound examination. Composition of the stones was not only of melamine but also uric acid. Providing liquid orally or intravenously plus alkalisation of urine proved to promote the removal of the stones. Follow-up of 12 to 17 months after discharge showed no sequelae.

Abstract:The paper by Woodbury and Burrow is examined using four criteria: completeness, discrimination, alternative approaches, and combining exploration in different problem domains. Although the paper covers significant aspects of the search space literature it leaves out some relevant aspects of the cognitive approach. It fares much better in terms of discriminating important concepts and alternative approaches to the modeling of the design search space. The structure–function–behavior model is suggested as an analogy for the central parameters of the search space paradigm. The Woodbury and Burrow paper reveals more than what has been accomplished up to now in the design search space area, but its task still remains incomplete.

Abstract:Objective: To investigate the clinical features and therapeutic efficacy in children with urolithiasis caused by ingestion of melamine-contaminated powdered formula. Methods: The clinical presentation, laboratory data, ultrasound findings, and treatment methods used for 619 children with melamine-induced urolithiasis were retrospectively recorded and analyzed. The efficacy of the treatment in these children was summarized and evaluated. Results: A total of 619 children aged 1-88 months were included in the present study (mean age 16.32 months). The male/female ratio was 1.9:1. Of the 619 patients, 577 received conservative treatment and 454 became stone free (78.7%). Of the 151 patients with renal and/or ureteral stones, 1-6 sessions of extracorporeal shock wave lithotripsy were performed in 81 patients (61 successful [35.5%] of 172 sessions) and retrograde ureteral catheterization in 70 patients (56 successfully treated [80.0%]). In centers without expertise in minimally invasive medical techniques, 5 patients with bilateral obstructive renal failure were successfully treated with ureteral lithectomy (n = 4) and percutaneous nephrostomy (n = 1). For the remaining 27 children with refractory renal or ureter stones, 9 were successfully treated with percutaneous nephrolithotomy and 18 children underwent ureteroscopic lithotripsy to become stone free. A total of 22 bladder and/or urethral stones were successfully treated with cystoscopic lithotripsy. Conclusions: Most children with melamine-induced urolithiasis were treated successfully with an infusion of fluids, urine basification, and diuresis. Extracorporeal shock wave lithotripsy is a safe and effective technique for patients in whom conservative treatment fails. However, for patients who have presented with obstructive renal failure, surgical intervention should be considered as early as possible to relieve the obstruction.
Continuous Renal Replacement Therapy for Patients with Acute Kidney Injury Caused by Melamine-Related Urolithiasis

Abstract:Background: In 2008 there was an epidemic of renal disease affecting infants after consumption of melamine-tainted milk products. Most of the infected children were asymptomatic or with mild symptoms, and a few suffered from acute obstructive kidney injury secondary to melamine-contained renal stones (8 of 15 577 children screened at our hospital for urolithiasis). This study was intended to retrospectively review the management of acute kidney injury using continuous renal replacement therapy (CRRT) in the 8 children with acute kidney injury.Methods: The 8 infants with acute kidney injury caused by melamine-related urolithiasis were referred to the pediatric intensive care unit at the hospital in late 2008. CRRT was given to treat their kidney injuries. Medical records of the infants were reviewed for demographic features, diagnosis, CRRT treatment, and outcomes.Results: Before CRRT, hypertension was found in 6 of the 8 children. Varying degrees of oliguria, anuria, elevated levels of blood urea nitrogen (BUN) (13.11-35.6 mmol/L) and creatinine (Cr) (238.8-773.7 mu mol/L) were observed in these patients. After CRRT, the levels of BUN, Cr and electrolytes decreased. Urine output and edema were improved clinically.Conclusion: CRRT can rapidly improve renal function, avoiding such surgical interventions as lithotripsy, percutaneous nephrolithotomy, and ureteroscopy. It is an efficient modality to treat acute kidney injury caused by melamine-related urolithiasis.
Conservative management of pediatric nephrolithiasis caused by melamine-contaminated milk powder

Abstract:Objective: In this article we report our experience with the diagnostic screening and management of children with melamine-induced nephrolithiasis. Methods: A total of 1091 children younger than 4 years who had been exposed to melamine-contaminated formula from September 17 to October 12, 2008, were screened for nephrolithiasis at the department of pediatrics at Shenzhen Nanshan Hospital in China. During the clinical examination, each patient's demographic characteristics were recorded together with the details of his or her milk-consumption profile during the contamination scare and any clinical signs of poisoning. Urinary stones were detected by B-ultrasonography, and renal status was examined by a routine urine test panel and a renal function test. When urinary stones were detected, patients were ordered to cease consumption of the suspected formula, and a conservative treatment course was adopted, including infusion of fluids, urinary alkalinization, increased water consumption, and diuresis. Results: Of the 1091 children screened, 12 (1.1%) were diagnosed with kidney stones. They had been exposed to the contaminated milk from 1 to 24 months. Eleven (91.7%) of these 12 patients had consumed milk with a high level of melamine content (955-2563 ppm); 1 patient (8.3%) had consumed milk with a low-level melamine content (6.2-17.0 ppm). Six patients exhibited dysuria; the remaining 6 patients were asymptomatic. All 12 patients had normal renal function, although 4 had proteinuria, and 1 had hematuria. The kidney stones were resolved within 3 to 5 days of commencing treatment in all 12 cases. Conclusions: Nephrolithiasis was associated with high melamine-exposure levels. A combination of B-ultrasonography and urinalysis is suitable for screening for pediatric nephrolithiasis caused by melamine poisoning. The condition can be resolved with a conservative treatment approach in patients without serious clinical symptoms who have normal kidney function.

Abstract:Objective: To study the impact of melamine-contaminated milk powder on children's growth and development and the elimination of urinary tract stones. Patients and Methods: Seventy-three patients aged years with ultrasonographically diagnosed urolithiasis after intake of contaminated milk powder were compared with 79 healthy controls. Assessment included indicators of growth and development based on reference data from World Health Organisation (WHO) standards, Brightness mode (B-mode) ultrasonography as well as urine and blood lab analysis. Results: According to WHO standards, whether at time of diagnosis or 18 months after follow-up, urolithiasis patients all showed generally lower z-scores, compared with control. At time of diagnosis, the differences were statistically significant for weight-for-age, weight-for-length and head circumference in children age <= 2 years and for weight-for-age, length/height-for-age and head circumference in the age groups 2 to 3 years and 3 to 4 years (all p<0.05). Eighteen months after follow-up, urolithiasis patient, had significant smaller values for length-for-age in children age years and for weight-for-age, length/height-for-age and head circumference in the age groups 2 to 3 years and 3 to 4 years (all p<0.05). While the patient group and the control showed no statistically difference between diagnosis and 18 months follow-up. On follow-up B-mode ultrasonography 18 months after the diagnosis, five (6.85%) children still showed intrarenal calculi, one (1.37%) child suffered from hydronephrosis. Serum levels of alanine aminotransferase, creatinine and uric acid were within normal range. Proteinuria was not observed. Conclusions: Consumption of melamine-contaminated milk power affects growth and development. Melamine-associated urolithiasis can persist for longer periods of time after cessation of melamine intake. A significant long-term effect of contaminated milk powder on liver and kidney function was not observed.
Follow-Up Results of Children with Melamine Induced Urolithiasis: A Prospective Observational Cohort Study

Abstract:Background Melamine-contaminated milk powder was the cause of the 2008 outbreak of urolithiasis in young children and infants in China, but the prognosis of these children remains unknown. We hypothesized that urolithiasis induced by melamine-contaminated milk powder may be associated with secondary renal injury. Methods A total of 8335 children (≤6 years old) with a history of consuming melamine-contaminated milk powder were screened. Urine analysis and urinary system ultrasonography were performed. For children with urolithiasis, the basic information and the results of examination were recorded, and effective therapy was given. They were followed up for 6 months after the original diagnosis, and urinary microprotein profiles were measured. Results Of the 8335 children, 105 (1.26%) were diagnosed with melamine-contaminated milk powder-associated urolithiasis. The size of the stone was correlated with the duration of exposure to melamine. Six months later, 69.8% (67) of the children with urolithiasis passed stones (follow-up rate: 91.4%). Of the 67 children, 28 passed stones within 2 months. The higher possibility of passing a stone was correlated with the smaller diameter of the stone ( P <0.001). The detection rate of abnormal urinary microprotein excretion (microalbumin, immunoglobulin G, and N-acetyl-β-D-glucosidase) was 52.4% in children with persistent stones and 38.2% in those who passed their stones. The detection rate was lower in children who passed stones within 2 months (31.8%) than in those who passed stones in 2 to 6 months (50.0%). The levels of microalbumin/creatinine and immunoglobulin G/creatinine were significantly higher in children with persistent stones than in those who passed their stones. Conclusions Early passage of a stone may reduce the renal injury induced by melamine-contaminated milk powder-associated urolithiasis.
Melamine-contaminated milk products induced urinary tract calculi in children

Abstract:Background Melamine is an industrial chemical used primarily as plastics stabilizer and fire retardant. On September 11, 2008, melamine-contaminated milk products were reported to be responsible for urinary tract calculi in infants and children in China. This study aimed to investigate the prevalence, lesions, risk factors, clinical features, and management of children fed with the melamine-contaminated milk products. Methods A total of 15 577 infants and children fed with the milk products were screened at our hospital. Ultrasonography was performed in all the infants and children. For those found with urinary tract calculion ultrasonography, urnalysis was done. Among them, 846 with detailed data screened from September 17 to 25 were enrolled for further analysis in this study. They were divided into calculus group (326 children) and non-calculus group (520 children) according to the results of ultrasonography. They included 429 boys and 417 girls, aged from 1 month to 5 years (median, 18 months). Their clinical and laboratory data, ultrasonograms, and treatment results were analyzed. Results Of the 15 577 children screened, 562 (3.61%) had urinary tract calculi. The rate was closely related to the melamine concentration in patients fed with formula. In 846 children with detailed data enrolled in this analysis, weight and head circumference Z scores in the calculus group were lower than those in the non-calculus group ( P =0.048, P =0.046). Long duration of formula feeding, high melamine contained formula, and minimal water intake were the risk factors for calculi ( P <0.05, respectively). Of 326 children with calculi, 281 had small calculiless than 0.5 cm in diameter, 227 had multiple calculi, and 34 had urinary tract distention. Moreover, diffuse renal lesions, renal failure and ascites were noted in 4, 3 and 2 patients, respectively. After 1-month treatment with sodium bicarbonate and Chinese traditional medicine, calculi disappeared in 49 of 54 outpatients. In 41 inpatients, 5 had calculi removed operatively and 36 had calculi minimized. Conclusions Melamine-contaminated milk products induced urinary tract calculi, which have a good response to conservative therapy. Long-term follow-up of infants and children fed with melamine-contaminated milk products is required, and food safety should be supervised increasingly for the health of children.
Characterization of melamine-associated urinary stones in children with consumption of melamine-contaminated infant formula

Abstract:Background: Crystalline morphology and compositions of melamine-associated urinary stones caused by consumption of melamine-contaminated infant formula are reported. Methods: Twelve melamine-associated urinary stone samples were obtained from 12 children aged from 3 to 36 months who were treated in Lanzhou University Second Hospital. The crystalline morphology and compositions of these stone samples were analyzed using a combination of infrared spectrum, SEM and XRD. The presence of melamine in 12 stones was determined by HPLC. Results: Diversity of crystalline morphology in 10 stone samples was revealed by SEM observation. Infrared spectral data revealed that 8 of the 10 stone samples predominantly consisted of the crystal forms of urate: 1 of sodium urate monohydrate, 2 of uricite and 5 of uric acid dihydrate and ammonium urate. However, ammonium magnesium phosphate hexahydrate, ammonium urate and carbonate apatite were also identified in 1 stone sample. The content of melamine in all of the 12 stone samples was determined by HPLC and found to range from 200 to 339,000 μg/g. XRD analysis in 6 stone samples showed that amorphous mineral phase was involved in the calculogenesis. Conclusions: Melamine and uric acid are the main possible etiologic factors closely related to melamine-associated urinary stone formation in humans.

Abstract:OBJECTIVE:Our purpose is to describe the characteristics and the outcome of melamine-induced renal and urinary tract injury in young children who consumed melamine-contaminated infant formula.METHODS:This is a descriptive longitudinal study over 2 years in 240 children with melamine-induced urolithiasis screened in our hospital from September 15 to October 31, 2008. Ultrasonography and serum creatinine (SCr), urea, β2-microglobulin (MG), cystatin C (Cys C), urinary Cr (UCr), microalbumin (mALB), α1-MG, β2-MG, n-acetyl-β-d-glucosaminidase (NAG) and retinol-binding protein (RBP) measurements were performed.RESULTS:The children ranged in age from 1 to 82 months, and 145 were males. The largest calculus was 33 mm in diameter. X-ray diffraction pattern of the calculi displayed two diffraction peaks at 10.9° and 27.7° (2θ). Surgical management was performed in 14 patients. In 226 patients without surgical management, the calculi were passed in 59.63% patients within 1 month, in 85.40% within 6 and in 91.15% within 24 months. Increased SCr and urea levels were noted in three and six patients, respectively, at the time of diagnosis. The SCr, serum β2-MG, and Cys C levels at the time of diagnosis were higher than those at 3 and 6 months after diagnosis (P < 0.05, respectively). The levels of mALB/UCr, NAG/UCr, and RBP/UCr at the time of diagnosis were higher than those at 3, 6, 12, and 24 months after diagnosis (P < 0.05, respectively). α1-MG/UCr and β2-MG/UCr levels at the time of diagnosis were similar to those at 3 months after diagnosis, and significantly higher than those in the follow-up period (P < 0.05, respectively).CONCLUSION:Melamine might injure both the renal glomerulus and the tubule, and that the predominant lesion is urolithiasis. The compositions of melamine-induced urolithiasis are melamine and cyanuric acid crystals. The urolithiasis might persist for over 2 years and cause irreversible damage. Therefore, a long-term follow-up for all patients is required.
The clinical profile and prognosis of Chinese children with melamine-induced kidney disease: a systematic review and meta-analysis.

Abstract:Introduction. Studies have reported inconsistent results regarding clinical feature and the prognosis status of the affected children in China melamine-contamination event. We summarized available literatures by performing a review and meta-analysis. Methods. Statistical pooling was performed using random-effects model; the sources of heterogeneity were explored through subgroup analyses. Results. Twenty-six studies involving 2164 patients with kidney abnormalities were identified; 94.4% of the patients had urinary calculi and 95.8% of the calculi were <10mm in diameter. Of 2040 patients with known types of treatment, 5.6% underwent surgical treatment. The pooled recovery rates at 1, 3, 6, and 12 months after diagnosis or treatment initiation were 67.1%, 76.3%, 85.4%, and 92.3%, respectively; these pooled rates did not differ between the study subgroups stratified by mean age at diagnosis, mean duration of melamine exposure, types of patients (inpatient/outpatient), and treatment types (specific/nonspecific), except that the 1-month recovery rate for studies involving a specific treatment (71.9%) was higher than that for studies involving nonspecific treatment (46.2%). Conclusion. The majority of patients had small calculi and could recover without surgical treatment. Kidney abnormalities remained in about 8% of the patients at 12-month followup, indicating a need for longer-term followup.
Large sample follow-up of single centre and assessment of pediatric urolithiasis caused by melamine-contaminated milk powder

Abstract:Objective To assess the prognosis and effect on renal function of pediatric urolithiasis caused by melamine-contaminated milk powder (PUMMP) in a long-term follow-up.Methods One hundred and two of 8335 children (≤ 6-year-old) with history of consuming melamine-contaminated milk powder screened in our hospital were followed up for eighteen months after diagnosis. Urinary system ultrasonography, urinalysis, urinary microprotein profiles [microalbumin (ALBU), immunoglobulin G (IgG), and N-acetyl-β-D-glucosidase (NAG)], urinary melamine and cyanuric acid were examined in the first visit and at the end of follow-up. Results Follow-up was completed in 91 children and the stone was excreted in 82 children (90.1%).Stones less than 5 mm in diameter were most vulnerable to discharge, and stones larger than 10 mm could not be expelled without interventions. At the end of follow-up, no melamine or cyanuric acid was found in the urine samples of 74 patients. Urinalysis showed that incidences of proteinuria, microscopic hematuria and leukocyturia were 0%, 5.1% and 2.0%, which were significant different from those in the first visit (Pproteinutria=0.123, Phemnatuna=0.038 and Pleukocyhuris=0.005).Urinary microprotein profiles revealed that some children whose urinalysis was normal still presented glomerular and renal tubular injury and the abnormal rates were 8.8% and 12.1%respectively. The glomerular injury was mainly related to persistent stone, male and younger.Conclusions 90.1% of children with PUMMP passes urinary stones at the end of follow-up.Stone size is the major risk factor of discharge. No melamine or cyanuric acid is found in the urine of children. After eighteen months, glomerular and renal tubular injury is still found in some patients. Further follow-up is necessary.
Metabonomic evaluation of melamine-induced acute renal toxicity in rats.

Abstract:The recent outbreak of renal failure in infants in China has been determined to be caused by melamine (Mel) and derivatives adulterated in the food. A metabonomic study was performed to evaluate the global biochemical alteration triggered by Mel ingestion in parallel with the acute renal toxicity in rats. Mel at 600, 300, and 100 mg/kg, cyanuric acid (Cya) at 100 mg/kg, and mixture of Mel and Cya (50 mg/kg each) were administered in five groups of Wistar rats, respectively, via oral gavage for 15 days. Urinary metabonomic profiles indicated that Mel perturbed urinary metabolism in a dose-dependent manner, with high-dose group showing the most significant impact. Metabonomic variations also suggest that the toxicity of low-dose (50 mg/kg) Mel was greatly elevated by the presence of Cya (at 50 mg/kg), which was able to induce a significant metabolic alteration to a level equivalent to that of 600 mg/kg Mel. Histological examination and serum biochemical analysis also indicated that the low-dose Mel-Cya mixture and high-dose Mel group resulted in the greatest renal toxicity. The high-dose Mel and low-dose Mel-Cya resulted in disrupted amino acid metabolism including tryptophan, polyamine, and tyrosine metabolism, and altered TCA and gut microflora structure.
The Clinical Profile and Prognosis of Chinese Children with Melamine-Induced Kidney Disease: A Systematic Review and Meta-Analysis

Abstract:Introduction. Studies have reported inconsistent results regarding clinical feature and the prognosis status of the affected children in China melamine-contamination event. We summarized available literatures by performing a review and meta-analysis. Methods. Statistical pooling was performed using random-effects model; the sources of heterogeneity were explored through subgroup analyses. Results. Twenty-six studies involving 2164 patients with kidney abnormalities were identified; 94.4% of the patients had urinary calculi and 95.8% of the calculi were <10 mm in diameter. Of 2040 patients with known types of treatment, 5.6% underwent surgical treatment. The pooled recovery rates at 1, 3, 6, and 12 months after diagnosis or treatment initiation were 67.1%, 76.3%, 85.4%, and 92.3%, respectively; these pooled rates did not differ between the study subgroups stratified by mean age at diagnosis, mean duration of melamine exposure, types of patients (inpatient/outpatient), and treatment types (specific/nonspecific), except that the 1-month recovery rate for studies involving a specific treatment (71.9%) was higher than that for studies involving non-specific treatment (46.2%). Conclusion. The majority of patients had small calculi and could recover without surgical treatment. Kidney abnormalities remained in about 8% of the patients at 12-month followup, indicating a need for longer-term followup.

Time, Age, and the Transition to Retirement: New Evidence on Life-Course Flexibility?

Abstract:The past twenty-five years have seen dramatic changes in the transition to retirement. This article considers an overlooked set of social processes—informal age structuring—within the context of these changes. Data are drawn from a random sample of 319 adults from the Chicago area. For about half of the respondents, age was considered an irrelevant dimension for both men's and women's retirement Those respondents who found age relevant cited deadlines that were clustered not only around the critical points at which researchers have observed regularity in retirement patterns, but they also included the lower junctures that are emerging as part of the shift toward earlier retirement. These deadlines most often marked the place of retirement relative to a larger set of work transitions, or they budgeted enough time to pursue developmental opportunities at the end of life. However, most respondents said there were no serious consequences for retiring late. Important patterns also emerged across the background characteristics of our respondents. These findings feed into several provocative debates that relate time and age to models of life-course flexibility or rigidity.
