Cyst Distortion: a New Sonographic Sign of Peritoneal Pseudocyst with Regular Morphology

Abstract:Peritoneal pseudocysts (PPC), also known as peritoneal inclusion cysts, have a variety of appearances on ultrasound imaging1-4. Although some characteristic features may help to distinguish these lesions from adnexal cysts, e.g. a ‘bizarre shape’4, ‘ovary entrapped in a spider web’2 and ‘flapping sail sign’4, it is difficult to differentiate PPC from various adnexal cysts when the PPC has a well-defined margin and regular morphology. We report here ‘cyst distortion’ as a new sonographic sign for detection of PPCs with regular morphology. Transvaginal ultrasound scans were performed in 12 patients (mean age 42.7 (range 35–62) years) with a history of gynecological surgery, i.e. ovarian cystectomy, myomectomy, unilateral adnexectomy or hysterectomy. All patients had a pelvic cyst with regular morphology, which was oval in seven cases and round in five cases (Figure 1). The mean cyst diameter was 8.3 (range 6.7–13.8) cm. A total of 10 cysts were confirmed as PPCs by laparotomy, nine of which showed dented walls and appeared distorted when pressed gently through the abdominal wall using one hand and holding the transvaginal transducer by the other hand (Figure 2). The remaining two patients were found to have hydrosalpinx and serous cystadenoma (one case each) on pathological analysis. When pressure was applied as mentioned above in these two cases, the cyst moved in the case with hydrosalpinx, while in the case of serous cystadenoma ultrasound showed the bowel moving against the cyst wall. PPCs are fluid collections which result from inflammatory adhesions and/or abdominal or pelvic surgery. The walls are formed by adhesions between the serosal surfaces of the surrounding organs. Therefore, in contrast to true adnexal cysts, the PPC has no wall and is hypotonic. The cyst thereby becomes distorted and its wall dented when pressure is applied. Of course, our observations were made in relatively large cysts with a mean diameter of 8.3 cm. Whether or not the sign is present when the cysts are small and do not reach the abdominal wall would have to be investigated. In summary, the new sonographic sign of ‘cyst distortion’ may assist in the differentiation of PPCs from adnexal cysts and could help when choosing between conservative treatment or follow-up and surgery.
