Conceptual clarity is vital to curtail compassion fatigue via preventative and restorative measures at the individual and organizational level.DESIGN: Concept analysis.DATA SOURCES: Databases searched were OVID, CINAHL Complete, Science Direct, Academic One File, Criminal Justice Abstracts, Education Full Text (H.W. Wilson), PsycARTICLES, PsycINFO, Social Work Abstracts, and Teacher Reference Center. Keywords included concept analysis, compassion fatigue, secondary traumatic stress, burnout, vicarious traumatization, compassion satisfaction, nursing, psychology, and social work. The timeline for data collection was from 1990 to 2017.METHODS: Walker and Avant's method of concept analysis.RESULTS: Compassion fatigue occurred across disciplines. Nurses were predisposed to compassion fatigue by repeated exposure of others' suffering, high stress environments, and the continuous giving of self. The consequences of compassion fatigue negatively impacted the nurse, patient, organization, and healthcare system.CONCLUSION: All nurses are at risk for compassion fatigue. Prevention of compassion fatigue is achieved through professional boundaries, self-care measures, self-awareness, and education on the concept at the individual and organizational level.

Abstract:Compassion is a complex abstract concept and is generally perceived as an emotional reaction to another person's vulnerability and suffering that motivates an individual wanting the best for the one who is suffering. It is seen as a virtuous and an inherent quality of nursing care. Nurses are exposed to various work stressors caused by myriads of complex professional and organizational challenges. In response, some nurses have developed coping mechanisms exemplified by detachment and distance in the caring relationship, whereas others have fallen victim to compassion fatigue or burnout, stressors that may increase the risk for suicide. There is a direct cost to health care organizations when the staff begin to show signs of burnout and fatigue. It is incumbent on health care leaders to establish assistance programs to improve the quality of work life of caregivers, prevent and remove work stressors to reduce turnover, and retain talent.

Abstract:AIMS AND OBJECTIVES: To interpret the body of qualitative work focusing on compassion fatigue to distil a common understanding that could then be applied to nursing care.BACKGROUND: Complex demands place extraordinary stress on nurses struggling to work in overburdened healthcare systems. The result can be the inability to care well for others, leading to compassion fatigue, burnout and increased numbers leaving the profession. Metasynthesis offers a means of more fully illuminating compassion fatigue and further understanding of practices which might reduce its negative consequences.DESIGN: Metasynthesis.METHOD: As a method designed to facilitate knowledge development, metasynthesis allowed for integration of qualitative study findings conducted between 1992-2016 using defined search terms. Six databases were searched for articles published in English. Nine papers met the criteria for review and metasynthesis was conducted using the meta-ethnographic approach detailed by Noblit and Hare.RESULTS: Four themes related to compassion fatigue were found by consensus discussion. The themes included: physical ("just plain worn out") and emotional symptoms ("walking on a tightrope"), triggering factors ("an unbearable weight on shoulders" and "alone in a crowded room"), and measures to overcome/prevent ("who has my back?").CONCLUSIONS: Compassion fatigue is a concept of documented relevance to those in nursing and represents a basic inability to nurture others and engenders a temporal component. Synthesis of studies provides evidence of the veracity of the concept for application to clinical practice and research related to nursing care.RELEVANCE TO CLINICAL PRACTICE: Findings provide insight into the clinical milieu needed to prevent compassion fatigue. A theoretical model is presented which can be used to guide future research, as well as the creation of clinical practice policies which might mitigate the development of compassion fatigue and its potential consequences.

Abstract:Community nurses are at particular risk to compassion fatigue because their role attracts people who are compelled to help others, and because the work is emotionally challenging. This article explores the various ways in which compassion fatigue affects work and personal life, highlights three factors that make someone susceptible to it and provides recommendations on how to protect oneself from compassion fatigue. The authors also refer to their own research on super-helper syndrome to elucidate the understanding of what contributes to compassion fatigue.

Abstract:Objectives This study aimed to discuss how compassion fatigue (CF) develops and its repercussions on the personal and professional lives of oncology nurses. Methods A discursive article, with systematic searches were performed in seven databases to find publications on CF in oncology nurses. Results So as to better organize the findings, three categories were developed to present and discuss issues related to CF: (1) Characteristics of CF and its developments: describes the components related to CF and the manifestation of this phenomenon; (2) Repercussions of compassion fatigue: reports on the impact of CF on the personal and professional life of oncology nurses; and (3) Resources for dealing with compassion fatigue: lists interventions, sources of support, professional personal training, qualified nursing care in the face of adversity, and gratitude and recognition. Conclusion the factors that trigger or protect CF are multifactorial, with the need for collective and individual interventions as a way of helping oncology nurses to protect themselves, to avoid or manage this phenomenon. CF has a direct clinical impact on the life of the oncology nurse, causing several changes. It also indirectly impacts the patient's life clinically, as it is a phenomenon that has repercussions on the provision of care. Implications for Nursing Practice CF affects the personal and professional lives of oncology nurses, so nurses need to seek resources to deal with it. Nursing staff employers and managers can use the evidence from this research to help nurses manage and protect themselves from compassion fatigue.

Abstract:Evidence does not suggest but validates the very fact that an unsupportive patient care environment contributes to the development of what is called compassion fatigue in oncology nurses. This retrospective is an attempt to describe through personal story how compassion fatigue developed over time. The purpose of this article is to illustrate through personal narrative the manifestation of compassion fatigue in an individual oncology nurse and to hopefully allow readers to become aware of any presence of compassion fatigue in their personal lives and/or professional nursing practice. The following questions emerged from the writing of this article and may be useful for future research: (1) What is the best definition of compassion fatigue to be used when specifically applied to oncology nurses as guidance for future research? (2) What specific interventions might be explored as a means to prevent compassion fatigue in oncology nurses using prospective trials? (3) What are the best institutional practices used currently in oncology units? Abstract:Compassion fatigue is the cost of caring for others in emotional pain where the helping professional absorbs the trauma of those they help and cannot detach emotionally at the end of the day. Stressful and perceived unsupportive work environments may leave the caregiver at a heightened risk for compassion fatigue. To study the level of compassion fatigue and compassion satisfaction experienced by mental health nurses. A descriptive, cross-sectional, electronic survey design was utilized. The Professional Quality of Life Scale-5 was used. The study was conducted at a large, urban hospital that exclusively treats patients with mental health and addiction issues located in Ontario, Canada. The sample included all nurses who were listed on the hospital's Email list. All nurses who are displayed on this list are mental health nurses. Analyses were conducted to assess for differences between the characteristics and the subscales of professional quality of life. One hundred and forty-eight mental health nurses completed a questionnaire. The response rate was 21%. Seventy-six percent reported moderate levels of burnout, 59% reported moderate levels of secondary traumatic stress, and 21% reported high levels of compassion satisfaction. Predictors were seen for the characteristics of gender, ethnicity, marital status, area, exposure to violence and support felt. Compassion fatigue has clear implications. Organizations willing to invest in reducing it, have the potential to improve patient outcomes and the well-being of nurses. The majority of mental health nurses reported moderate levels for each of the subscales.

Abstract:Psychotherapists who work with the chronic illness tend to disregard their own self-care needs when focusing on the needs of clients. The article discusses the concept of compassion fatigue, a form of caregiver burnout among psychotherapists and contrasts it with simple burnout and countertransference. It includes a multi-factor model of compassion fatigue that emphasizes the costs of caring, empathy, and emotional investment in helping the suffering. The model suggests that psychotherapists that limiting compassion stress, dealing with traumatic memories, and more effectively managing case loads are effective ways of avoiding compassion fatigue. The model also suggests that, to limit compassion stress, psychotherapists with chronic illness need to development methods for both enhancing satisfaction and learning to separate from the work emotionally and physically in order to feel renewed. A case study illustrates how to help someone with compassion fatigue.

Abstract:The perfusion of the sinus node artery in 9 dogs in situ was performed. The selective injection of aminophylline into the sinus node artery induced a positive chrono-tropic response. The positive chronotropic response to aminophylline was suppressed not only by propranolol but also by tetrodotoxin. It is suggested that the positive chronotropic action of aminophylline is partially due to catecholamine release from adrenergic nerve terminals by excitation of local nerve fibers.

Abstract:Introduction Psychiatric nurses are at risk of experiencing compassion fatigue. Despite the economic, emotional and physical effects of compassion fatigue, it has not been sufficiently investigated in the field of psychiatric nursing, and the lived experiences of compassion fatigue, in particular, require further investigation using qualitative methods. Aim The aim of this study was to examine the lived compassion fatigue experiences of nurses working in psychiatric clinics using a phenomenological approach. Method The study was conducted between December 2023 and February 2024 with 22 psychiatric nurses working in acute psychiatric clinics. Data were collected through individual, in‐depth interviews, and analysed using Collaizi's seven‐step descriptive analysis approach. Results Data analysis revealed four main themes and eight subthemes: (1) being a psychiatric nurse (multifaceted role and benefits), (2) tiredness (from traumatic effects to depersonalisation and emotional isolation), (3) causes of compassion fatigue (patient derived, system and working environment derived) and (4) coping methods (individual and expectations). Discussion Due to difficult patient profiles and adverse working conditions, psychiatric nurses experience compassion fatigue and may become desensitised to patient needs over time. Implications for Practice Institutional support in the form of training for nurses on recognising and coping with compassion fatigue, as well as a safe and workload‐balanced working environment, is essential.

Abstract:BACKGROUND: Caring for cancer patients can take a toll on the emotional health of oncology nurses, which may lead to compassion fatigue, resulting in decreased quality of nursing care, absenteeism and decreased retention of staff.AIM: The aim of this study was to describe compassion fatigue from the perspective of oncology nurses. This study is part of a larger mixed-methods action research study to develop an in-facility intervention to manage compassion fatigue in oncology nurses.SETTING: This study was conducted at Durban, KwaZulu-Natal, South Africa.METHODS: The research setting comprised one state hospital (with oncology clinics and wards), a private hospital (with oncology wards) and a hospice in Durban, KwaZulu-Natal, South Africa. Semi-structured individual interviews (guided by Figley's Compassion Fatigue Process, 2005) were conducted with eight participants. Data were analysed using manifest content analysis.RESULTS: Five categories emerged from the data, namely, emotional connection, emotional fatigue, emotional loss, blurring boundaries and acceptance.CONCLUSION: The findings revealed that oncology nurses are affected emotionally in caring for their patients, thus making them prone to compassion fatigue. Oncology nurses need to acknowledge compassion fatigue and be able to self-reflect on how they are managing (both positively and negatively) with the stressors encountered in the oncology wards or units.

Abstract:BackgroundIn the acute medical care hospital setting, nursing the sick and dying is both physically and emotionally demanding, making acute medical care hospital nurses more vulnerable to Compassion Fatigue (CF) or Secondary Traumatic Stress (STS). If not addressed in its earliest stages, CF can adversely change the caregiver’s ability to provide compassionate care. It has been shown that Self-Compassion (SC) can be beneficial for the caregiver, with corresponding benefits for the individual needing care. However, the relationship of this attribute to CF in the acute medical care hospital nursing setting has not been intensively studied. This study explores the environmental and psychosocial factors affecting the prevalence and levels of CF in acute medical care hospital nursing staff and whether SC can be used as a coping strategy that enables nurses to mitigate the prevalence and levels of CF.MethodsUsing a mixed-methods study design, acute medical care hospital nurses were surveyed using a demographic/work-related questionnaire, the Secondary Traumatic Stress Scale (used to measure CF), the Self-Compassion Scale, questions requiring a narrative written response and semi-structured informal interviews. One-way ANOVA was conducted to explore the impact of work-related and demographic characteristics on levels of SC and CF. Pearson correlation co-efficient (r value) was used to explore the relationship between CF and SC and lastly, multiple regression was used to discover whether a predictive relationship existed between SC and CF. The interviews were recorded, transcribed verbatim and subjected to thematic analysis.ResultsThe results of this study have exposed the prevalence of CF in acute medical care nurses and it’s damaging effects. And has also confirmed that SC can have a moderating effect on CF and an ability to be predictive of CF. Factors that influence the distress and vulnerability to CF were also identified, as well as the personal approaches used, whether successful or not, to enhance levels of SC and the ‘barrier’s to achieving it.ConclusionThis unique study could lead to care environments encouraging the self-compassion and well-being of their acute medical care nursing staff and become places where threat cultures, weak leadership, an emphasis on task rather than process and a feeling of being undervalued, do not monopolise.

Effect of compassion fatigue on emotional labor in female nurses: Moderating effect of self-compassion

Abstract:Emotional labor is common in nursing but may be affected by the mental state of nurses. This study explored the effect of compassion fatigue on emotional labor and whether self-compassion moderates this effect of compassion fatigue. Methods: A two-stage survey design with a convenience sample. Participants were female nursing staff recruited from emergency departments, intensive care units, ward nursing units, and outpatient departments of medical centers, regional hospitals, and district hospitals in Taiwan. A total of 300 questionnaire copies in each of the first and second stages were distributed, and 272 pairs of responses were retrieved (valid response rate = 91%). The reliability and validity of the questionnaire were tested, and confirmatory factor analysis was conducted with AMOS 21. The proposed hypotheses were verified using hierarchical regression conducted with SPSS version 25.0. Results: This study revealed that compassion fatigue positively predicted surface acting ( β = 0.12, p < 0.05) and negatively predicted deep acting ( β = −0.18, p < 0.01) and expression of genuine emotions ( β = −0.31, p < 0.01). In addition, self-compassion negatively moderates the relationships between compassion fatigue and surface acting ( β = −0.12, p < 0.05), and positively moderates the relationships between compassion fatigue and expression of genuine emotions ( β = 0.15, p < 0.01). Conclusions: To avoid excessive consumption of emotional resources, nurses with high compassion fatigue may employ surface acting by engaging in emotional labor without making an effort to adjust their feelings. Nurses need also be sympathized with, and such sympathy can come from hospitals, supervisors, colleagues, and, most crucially, the nurses themselves. Hospital executives should propose improvement strategies that can prevent the compassion fatigue on nurses, such as improving nurses’ self-compassion.

Abstract:Abstract Aim To examine the impact of six areas of work‐life, emotional intelligence and psychological capital on compassion fatigue and compassion satisfaction among Canadian Registered Nurses and licensed practical nurses. Design A cross‐sectional survey study. Methods A convenience sample of 296 Registered Nurses and 110 licensed practical nurses answered a self‐administered questionnaire measuring areas of work‐life, psychological capital, emotional intelligence, compassion satisfaction and compassion fatigue in September 2019. The association between variables were analysed with descriptive and correlational analyses, while the hypothesized models were tested using multiple regression analyses. Results This study identified several areas of work‐life and intrapersonal resources that impacted compassion satisfaction and compassion fatigue. Among Registered Nurses, compassion satisfaction was predicted by psychological capital, rewards, values and workload. Compassion fatigue was predicted by psychological capital, workload, control and community. Among licensed practical nurses, compassion satisfaction was predicted by psychological capital and emotional intelligence. Compassion fatigue was predicted by workload and psychological capital. Study results also revealed significant differences in Registered Nurses' and licensed practical nurses' perceptions of workload, rewards and fairness at work, and both compassion satisfaction and compassion fatigue. Registered Nurses perceived their workload to be more manageable and perceived greater rewards and greater fairness at work than licensed practical nurses. Compassion fatigue was higher for Registered Nurses than licensed practical nurses, while compassion satisfaction was higher for licensed practical nurses than Registered Nurses. Future studies should investigate the nature and span of these differences to suggest relevant strategies to mitigate compassion fatigue and promote compassion satisfaction for each of these nursing roles. Conclusion The results of this study underscore the need to create nursing work environments that foster a manageable workload and positive social relationships, where nurses have professional autonomy, decision‐making capacities and access to adequate resources to do their job effectively. The nursing work environment should also provide recognition of nurses' contributions and an alignment between personal and organizational values. Investments in the development and improvement of nurses' psychological capital and emotional intelligence should be prioritized since they are malleable and impactful intrapersonal resources on compassion satisfaction and compassion fatigue. Reporting method This study adhered to the STROBE guidelines. Public contribution A total of 406 nurses were involved in this study by answering a self‐administered study survey.

Abstract:Background: Compassion fatigue is a consequence of chronic work-related stress exposure among healthcare providers. Nursing is a high-risk, stressful profession which increases nurses' vulnerability to compassion fatigue symptoms compared to other healthcare workers. Compassion fatigue has serious consequences for nursing staff, patients and healthcare organizations. Though several studies on the prevalence of compassion fatigue among nurses have been published, the reported data vary considerably across studies; and few meta-analysis have examined the prevalence of compassion satisfaction and compassion fatigue among nurses with large sample sizes. Objectives: To systematically assess the prevalence of compassion satisfaction and compassion fatigue among nurses, and to evaluate the effect of different geographical regions, years and departments on the prevalence of compassion fatigue. Design: Systematic review and meta-analysis DATA SOURCES: The Cochrane Library, PubMed, EMbase, Web of Science, CINAHL, PsyclNFO, China Knowledge Resource Integrated Database (CNKI), Wanfang Database, Weipu Database (VIP), and Chinese Biomedical Database (CBM) were searched in the systematic review. The time frame for the searches included all literature before January 31st, 2020. Review methods: The reviewers independently completed study selection, quality assessments, data extraction and analysis of all included literature. The mean scores and standard deviations of the three subscales of the Professional Quality of Life (ProQOL) scale were pooled using random effects meta-analysis in Stata 16.0 software package. Finally, subgroup analyses were conducted to explore the sources of between-study heterogeneity. Results: A total of 79 studies were included in the systematic review and meta-analysis, consisting of 28,509 nurses worldwide from 11 countries. In our studies, the pooled mean scores of compassion satisfaction, burnout and secondary traumatic stress were 33.12 (95% CI: 32.22-34.03), 26.64 (95% CI: 26.01-27.27) and 25.24 (95% CI: 24.69-25.79), respectively. In addition, the Asian region had the lowest levels of compassion satisfaction but the highest levels of compassion fatigue symptoms, while the Americas and Europe had the lowest levels of compassion fatigue but highest compassion satisfaction. Levels of compassionate fatigue in nurses increased gradually from 2010 to 2019, reaching the highest level in 2019; and nurses from ICU had the highest levels of compassion fatigue symptoms among all nurses. Conclusion: The levels of compassion satisfaction and compassion fatigue among nurses are moderate. Nurses from the Asian region and in ICUs suffer from severe compassion fatigue symptoms, and the prevalence of compassion fatigue has increased over time. These findings may provide hospital administrators with the theoretical basis for the management and treatment of compassion fatigue. Registration number: PROSPERO [CRD42020164327].

Methods: An adapted interaction model of client health behavior guided this cross-sectional survey, which comprised a demographic questionnaire, the Professional Quality-of-Life Scale, the caregiver-provider relationship assessment, the Marlowe-Crowne Social Desirability Scale Short Form (Form C), and the Bakas Caregiving Outcomes Scale. Results: There were 127 family caregivers (FCGs), primarily those who have been a caregiver for longer than 1 year (78.4%), who completed the entire survey. Increased compassion satisfaction and positive caregiving effects were associated with decreased compassion fatigue (p < .001), while increased social desirability was associated with increased compassion fatigue (p < .001). A positive family caregiver-nursing provider relationship was related to decreased compassion fatigue (p < .001). More compassion fatigue was seen with lower spiritual status (p < .001). Implications: The chronic progression of heart failure presents opportunities for nursing providers to assess caregivers and offer frequent interventions. Family caregivers may not ask for help. Nursing providers must foster relationships with family caregivers to prevent negative consequences and mitigate compassion fatigue. Future research on the nursing provider relationship and FCGs is needed.

Abstract:Background This study examines the experiences of geriatric nurses in palliative care. It aims to understand how these experiences influence their well-being and the quality of care. Methods Conducted in the Alahsa region of Saudi Arabia, the study employs qualitative methods, utilizing in-depth interviews with 12 geriatric nurses. The analysis employs a thematic approach, enriched by iterative reflections within a multidisciplinary research team. Results The analysis reveals main themes: 1) the deep emotional connections between nurses and their patients; 2) the challenges faced, including compassion fatigue, high patient mortality, and communication hurdles; 3) the impact of these challenges on the quality of care, highlighting issues like diminished empathy; 4) the coping strategies used by nurses, such as self-care practices and continuous education. Conclusions The study concludes that coping strategies, including self-care and ongoing professional development, are vital for sustaining the nurses' well-being and ensuring the continued provision of high-quality care to Older Adults patients

Abstract:NEW! The Institute's new research initiatives includes a panel study of secondary traumatic stress and related factors and a survey of social workers and other human service professionals. This is an effort to do two things simultaneously: First to provide a resource to professionals and volunteers working with the traumatized people or others who may be in crisis or are experiencing acute stress. Second, the Panel helps generate longitudinal data to be able to understand and help these professionals. For example, we hope to test the utility of a new measure of secondary traumatic stress reactions developed by Brian Bride others. Volunteers seeking information about their own functioning will be treated as Panel Members will take the tests online. Their answers will help compute baseline and psychometric data for particular professionals (e.g., social workers, teachers, nurses) working with particular clients (e.g., acute care patients, rape survivors, inner city children).
