Abstract:Background: Impaction of mandibular third molars (M3) is one of the most common diseases. Extraction of M3 usually exacerbates osseous defects at the distal aspect of the adjacent second molar (M2). BonMaker® ATB has been cited as a novel autogenous bone grafting material. The aim of this pilot study was to introduce a novel method for repairing the distal osseous defects of M2 after the surgical removal of M3 with autogenous tooth graft powder (ATGP). Method: A total of five patients were enrolled in this prospective split-mouth clinical pilot study. Four impacted wisdom teeth were extracted bilaterally from each patient with proximal alveolar bone loss ≥ 5 mm of M3. The ATGP was prepared chairside from two extracted one side third molars and randomly implanted in one of the M3 extraction sockets, and the other side was treated with a blank and considered the control site. Patients were followed up at 6 months. Results: The five patients included three males and two females. Their ages ranged from 25 to 30 years, with a median of 27 years. Primary wound healing without complications was achieved in all the patients. There was a greater tendency for swelling of the cheeks and trismus to occur at the experimental site on the third postoperative day. Compared with the control site, the experimental site exhibited progressive bone filling and ossification in the sixth postoperative month. Moreover, the probing pocket depth of the experimental site was lower than that of the control site. Conclusion: The results of this study demonstrate that ATGP effectively and economically repairs distal osseous defects of M2. Further study is required to validate the effectiveness with a larger study population.
Autogenous Dentin Grafting of Osseous Defects Distal to Mandibular Second Molars After Extraction of Impacted Third Molars

Abstract:Bone loss at the distal aspect of mandibular second molars frequently is reported after extraction of impacted third molars. Typically, osseous grafting of the extraction site is not routinely performed. This study examined osseous healing following guided bone regeneration treatment of osseous defects distal to mandibular second molars after surgical removal of impacted mesioangularly or horizontally inclined third molars using the processed third molar as the graft material. For the study, 13 patients who required impacted third molar extractions were selected based on angulation of impaction. Patients requiring bilateral extractions were designated for a split-mouth study, while others were selected based on impaction angulation as a random study group. After surgical extraction of the third molars, the extracted teeth were stripped of any soft tissue, including the periodontal ligament, then ground and disinfected using a dentin grinding protocol to produce an autogenous dentin graft (ADG). This graft was then placed into the extraction socket and covered with a hemostatic sponge prior to site closure. Patients in the control group underwent the same procedure as those in the study group except that no ADG was placed in the socket and only a hemostatic sponge was placed prior to wound closure. Clinical and radiological examinations were performed, including panoramic radiographs and probing depths at 3 months and 12 months postoperatively. The alveolar bone level distal to the second molar was established by both probing depths and radiographic evaluation, which were compared between the two groups. At 12 months postoperative the study group showed probing distal to the second molar with a mean depth of 1.15 mm, whereas the control group showed probing with a mean depth of 4.45 mm. The authors conclude that autogenous dentin grafting is a viable option for use in the treatment of osseous defects distal to mandibular second molars following extraction of impacted third molars.
Treatment of Osseous Defects after Mandibular Third Molar Removal with a Resorbable Alloplastic Grafting Material: A Case Series with 1- to 2-Year Follow-Up

Abstract:Mandibular third molar (M3) surgical extraction may cause periodontal complications on the distal aspect of the root of the adjacent mandibular second molar (M2). Patients older than 26 years with periodontal pathology on the distal surface of the M2 and a horizontal/mesioangular impacted M3 may benefit from bone regenerative therapy at the time of surgery. In this prospective case series, an alloplastic fully resorbable bone grafting material, consisting of beta-tricalcium phosphate (β-TCP) and calcium sulfate (CS), was used for the treatment of the osseous defects after the removal of horizontal or mesioangular M3s in 4 patients older than 26 years. On presentation, the main radiological finding in all patients, indicating periodontal pathology, was the absence of bone between the crown of the M3 and the distal surface of the root of the M2. To evaluate the treatment outcome, bone gain (BG) was assessed by recording the amount of bone defect (BD) at the time of surgical removal (T0) and at the time of final follow-up (T1) 1 or 2 years post-operatively. The healing in all cases was uneventful, with no complications associated with the use of the alloplastic grafting material. Clinical and radiological examination at T1 revealed that all extraction sites were adequately restored, with significant BG of 6.07 ± 0.28 mm. No residual pathological pockets on the distal surface of the M2 were detected. Pocket depth (PD) at T1 was 2 ± 0.71 mm. Within the limitations of this case series, the results suggest that β-TCP/CS can support new bone formation at M3 post-extraction sites where bone regeneration methods are indicated, thus reducing the risk of having persistent or developing new periodontal problems at the adjacent M2.

Abstract:OBJECTS: Secondary alveolar bone grafting is a method that enables an excellent oral rehabilitation of the patients having alveolar cleft. The aim of this work is to report the closure of the alveolar cleft with the use of harvested autogenous bone graft combined with deproteinized anorganic bovine bone (Bio-Oss) under local anesthesia.SETTINGS AND SAMPLE POPULATION: Nine patients with age range, 8-11 years were consulted for their unilateral alveolar cleft.MATERIALS AND METHODS: A combination of symphyseal bone and deproteinized bovine bone mineral (DBBM) was placed into the alveolar cleft defect. Clinical and radiographical assessments were performed at 1, 3, and 6 months postoperatively.RESULTS: The healing period was uneventful in all cases, and no complications, such as membrane exposure, infection, or harvest site morbidity, were observed. All treated defect sites exhibited excellent bone formation, with an average of 5.45 mm (range, 2-9 mm; standard deviation 1.93 mm) of augmentation achieved overall.CONCLUSION: The treatment of vertically deficient alveolar ridges with guided bone regeneration using a mixture of autogenous bone and DBBM and resorbable collagen membrane can be considered successful, using this technique in an out-patient office setting.

Abstract:A 42-year-old Mediterranean male presented complaining of inability to sustain good oral care at the posterior aspect of the lower right jaw. The main problems were food impaction in the area and the subsequent malodor. The patient reported remarkable medical history. Clinical examination revealed local erytherma with noticeable bone defect distal to the second molar with obvious defect in the mesial wall of the third molar; the penetration depth was found to be up to 6 mm.Radiological evaluation confirmed the defect and it was attributed to the mesioangularly partially impacted lower third molar. It was decided that third molar should be extracted and concentrate of the patient's growth factors (PRGF) to be applied into the bony defect to stimulate bone regeneration and promote healing.The third molar tooth was, then, removed surgically and the PRGF, which was prepared preoperatively, was implanted in the socket. At the first postoperative day, moderate pain was the main complaint and was controlled by NSAIDs. One week postoperatively, the sutures were removed and there was good tissue healing on examination.On the fiftieth postoperative day, radiographic evaluation took place and showed noticeable enhancement of density and radio-opacity in the third molar socket area, in comparison with the baseline image. Further, clinical examination showed significant reduction of periodontal pocketing and evidence of new bone formation.In conclusion, PRGF was very successful in stimulating bone regeneration and promote healing following dental extraction.

Abstract:IntroductionThis paper presents case and technique reports on the use of Bio-Oss® and Bio-Gide® implantable material for the establishment of a mineral and protein scaffold for reconstruction of the distal alveolus of the lower distal second molar (lower dM2).MethodsSuch ‘alveolar bone grafts’ were undertaken immediately following the removal of deep horizontal or mesio-angular impacted wisdom teeth, and which had the following associations. 1) Partial eruption of the third molar, 2) Preoperative radiographic (OPG) evidence of loss of bone height (≥3mm) below the distal Cemento-Enamel Junction (CEJ) of the lower dM2, and 3) Patient aged ≥26 years of age. Following a presentation of radiographs (in three patients) taken preoperatively and postoperatively (at ∼6 months), a brief description of the technique is offered as well as a brief literature review on the subject.ConclusionThe author suggests that for older patients (≥26 years) were there is the presence of partially exposed deep mesio-angular or horizontal impacted wisdom teeth, and which are associated with the radiographic appearance of bone loss to the distal surface of the lower second molar tooth, then alveolar bone grafting utilising Bio-Oss® and Bio-Gide® is an effective and stable treatment option to prevent development of periodontal pocketing in the area. The author advises for more guided and systematised study in this area.

Abstract:OBJECTIVES To evaluate the survival rate, the orobuccal bone resorption and stability of periimplant hard and soft tissues following immediate implant placement of wide diameter implants in molar extraction sites and periimplant defect grafting with autogenous bone (AB) or biphasic bone graft material (BBGM) with 1- to 3-year follow-up examinations. MATERIAL AND METHODS Fifty wide diameter implants were placed in 50 patients immediately into molar extraction sockets by a flapless approach. Periimplant defect augmentation was performed randomized with either AB or BBGM. Primary outcome variable was implant survival. Marginal bone level changes, orobuccal width of the alveolar crest, probing depths and implant success were considered as secondary parameters. RESULTS One implant of the BBGM group was lost, 1 patient withdrew from the study (drop-out). The remaining 48 patients were still in function at a follow-up period up to 31 months after implant insertion. Interproximal marginal bone level regenerated from -7.5 mm to the level of the implant shoulder (AB +0.38 mm, BBGM +0.1 mm) at final follow-up. The width of the alveolar crest changed by -0.08 mm (AB) and +0.72 mm (BBGM) at 1 mm, -0.36 mm (AB) and +0.27 mm (BBGM) at 3 mm, -0.36 mm (AB) and +0.31 mm (BBGM) at 6 mm apical to implant shoulder level. Success rate was 87.5% in the AB and 56.3% in the BBGM group (p=0.058). CONCLUSIONS Medium term results prove a high survival rate, a favourable amount of bone generation in both groups and a low amount of orobuccal resorption in immediate molar implant insertion.

Abstract:Background: Bone defects and deep periodontal pockets often exist distal to the second molar after mandibular third molar extraction, seriously threatening the periodontal health of the second molar. Objective: To evaluate the effect of socket preservation with bone substitute materials on alveolar bone resorption and prevention of the distal periodontal defect of the adjacent tooth after mandibular impacted third molar extraction compared with natural healing. Methods: Ninety-nine patients with mandibular impacted teeth, treated in our hospital from January 2018 to December 2020, were randomly divided into the control and experimental groups. The experimental group underwent minimally invasive tooth extraction and socket preservation using the deproteinised bovine bone mineral, Bio-Oss and the bioabsorbable collagen membrane, Bio-Gide. The control group healed naturally after minimally invasive tooth extraction. The alveolar ridge dimension of the extraction sites, the probing depth, tooth mobility and gingival index on the distal aspect of the mandibular second molars were examined and recorded before and six months after the operations. Results: There was a significant difference between the experimental group and the control group in the alveolar bone width (P< 0.05) and height (P< 0.05) before and after surgery. The probing depth of the extraction sites in both groups was reduced. Conclusion: Using Bio-Oss and Bio-Gide to preserve extraction sites of impacted teeth can promote recovery more effectively than natural healing on the height of the distal alveolar bone and the width of the alveolar crest of the second molar and thus improve the periodontal status of the adjacent second molar.

Abstract:Introduction Oro‐control communication is one of the complications associated with dental extraction and oral surgeries. This case report presents a minimally invasive surgical approach for bone regeneration at the site of oro‐antral communication utilizing a prefabricated computer‐aided design and computer‐aided manufacturing (CAD‐CAM) allogenic bone block. Methods A 20‐year‐old healthy female, nonsmoker, with a badly destructed upper right first molar was referred for dental implant placement after extraction. Cone beam computerized tomography images revealed the presence of a large bone defect associated with oro‐antral communication with the maxillary sinus and insufficient bone for dental implant placement. A prefabricated CAD‐CAM allogenic bone scaffold was fabricated. After surgical exposure, the scaffold was secured in place and covered with a non‐resorbable membrane. A dental implant was placed after 5 months, and a trephining biopsy was processed for histological evaluation. Results Closure of the oro‐antral communication was clinically observed. The average width of the alveolar bone was 12 mm, and the average height was 11 mm. Histological analysis at 5‐month intervals showed thin newly formed bone trabeculae encircling remnants of graft material surrounded by osteoid tissue. The newly formed bone percentages were 32 ± 18% and 28 ± 17% volume remained after the biodegradation of the scaffold. Specific immune‐histochemical staining by anti‐vascular epithelial growth factor expression index value was 32.06%. Conclusions A prefabricated CAD‐CAM scaffold was successfully used to seal a large oro‐antral communication and regenerate sufficient bone to place a dental implant.

Abstract:Objectives: The current study aim was to evaluate the effect of Bioactive Hydrogel versus Advanced-PRF on bone regeneration following impacted mandibular third molar surgery. Material & Methods: sixty patients who had been scheduled for surgical removal of mesio-angular or horizontal impacted mandibular 3rd molar teeth were randomly divided into three groups. Group A involved 20 patients received A-PRF-Xenograft at the surgical site. Group B involves 20 patients received Xenograft-Hydrogel at the surgical site. While group C involved 20 patients left to heal without graft as control. Clinical and radiographic evaluation were performed immediate and at 6 months postoperatively. Data was collected and analysed statistically. Results: At 6-month postoperative evaluation, the current study found that A-PRF-Xenograft group showed significant shorter mean pain duration (5.1±1.4 days) compared to Hydrogel-Xenograft group (5.3±1.5 days) and control group (5.8±1.4 days). A-PRF-Xenograft group showed the lowest mean values of PPD (3.7±0.6) followed by Hydrogel-Xenograft group (5±0.8) and followed by control group (6.6±0.6). A-PRF-Xenograft group showed the highest mean values of bone density (605.3±85.5) followed by Hydrogel-Xenograft group (522.4±83.5) followed by control group (286.4±44.7) Conclusion: According to the results of the present study, there was significant improvement regarding pain duration, mouth opening limitation, periodontal pocket depth and bone density at the surgical site where A-PRF-Xenograft was applied. Hydrogel-Xenograft Group was more superior to the control group regarding pain duration, PPD, and bone density.

Abstract:Surgical extraction of the third molar can often result in the development of a periodontal pocket distal to the second molar that could delay the healing, and the socket could be colonized by bacteria and lead to secondary abscesses, or it may cause mobility or hypersensitivity. The aim of this case report is to assess the efficacy of a dentin autograft in the prevention of periodontal dehiscences after the surgical extraction of the third molar, obtained by the immediate grinding of the extracted tooth. A healthy 18-year-old male patient underwent surgery of both impacted mandibular molars: right postextractive socket was filled with grinded dentin; then, the left one was filled with fibrin sponge. The patient was followed up for six months, and clinical and radiographic assessment were performed: measurements of plaque index (PI), bleeding on probing (BOP), gingival index (GI), clinical attachment level (CAL), and probing pocket depth (PPD) were done before surgery and repeated at 90 and 180 days after the extractions. Measurements made at six months after the surgery revealed that the grafted site was characterized by a minor depth of the pocket if compared with the nongrafted site, with no clinical/radiographic signs of complications.
[Dental implantation and soft tissue augmentation after ridge preservation in a molar site: a case report]

Abstract:For ideal implant rehabilitation, an adequate bone volume, optical implant position, and stable and healthy soft tissue are required. The reduction of alveolar bone and changes in its morphology subsequent to tooth extraction will result in insufficient amount of bone and adversely affect the ability to optimally place dental implants in edentulous sites. Preservation of alveolar bone volume through ridge preservation has been demonstrated to reduce the vertical and horizontal contraction of the alveolar bone crest after tooth extraction and reduce the need for additional bone augmentation procedures during implant placement. In this case, a patient presented with a mandible molar of severe periodontal disease, the tooth was removed as atraumatically as possible and the graft material of Bio-Oss was loosely placed in the alveolar socket without condensation and covered with Bio-Gide to reconstruct the defects of the alveolar ridge. Six months later, there were sufficient height and width of the alveolar ridge for the dental implant, avoiding the need of additional bone augmentation and reducing the complexity and unpredictability of the implant surgery. Soft tissue defects, such as gingival and connective tissue, played crucial roles in long-term implant success. Peri-implant plastic surgery facilitated development of healthy peri-implant structure able to withstand occlusal forces and mucogingival stress. Six months after the implant surgery, the keratinized gingiva was absent in the buccal of the implant and the vestibular groove was a little shallow. The free gingival graft technique was used to solve the vestibulum oris groove supersulcus and the absence of keratinized gingiva around the implant. The deepening of vestibular groove and broadening of keratinized gingiva were conducive to the long-term health and stability of the tissue surrounding the implant. Implant installation and prosthetic restoration showed favorable outcome after six months.
Periodontal regeneration after third molar extraction causing attachment loss in distal and furcation sites of the second molar: a case report with 12 months follow-up

Abstract:Introduction: The periodontal support apparatus is extremely important due to guarantee insertion to the tooth. It helps with the maintenance of the structural resistance and reconstructive property, and its loss may contribute to root exposure and the possibility to obtain an augmentation of the sensitivity. Thereby, this paper aimed to describe a case report, using regenerative techniques in a bone loss involving distal and furcation regions of the mandibular second molar, which was associated with the extraction of the adjacent impacted third molar without bone interposing. Case presentation: The patient was referred to a private dental clinic reporting an unpleasant odor while using dental floss in the distal region of the mandibular left second molar, during the three years. In addition, reported a history of extraction of the wisdom tooth six years ago. Periapical examination and periodontal index were performed and an extensive radiolucent image involving distal and furcation regions, with a 12 mm probing depth involving the distal root at tooth (2nd molar). No other region showed probing depth greater than 3mm. Thus, after rigorous scaling and root planing, periodontal guided tissue regeneration was performed as an attempt to fill the bone defect. Conclusion: The regenerative process is a predictable way to treat vital mandibular second molar after extraction of the adjacent third molar. Clinicians must give special attention to extraction involving the mandibular third molars, mainly when there is no bone interposed between the second and third molars.

Abstract:Adequate bone volume is one of the prerequisites for the successful implantation and osseointegration of an endosseous implant. Where there are localized osseous abnormalities, xture stability and appropriate location for load distribution and aesthetics may be jeopardized. This case report presents a case of a 23 years old male patient who had undergone immediate implant placement following a two-stage protocol in relation to mandibular rst molar on the right side. During second stage implant surgery, it was revealed that there was presence of a buccal bony deformity extending from the mid buccal aspect to the distobuccal aspect of the implant. This bone defect was subsequently managed with placement of alloplastic bone material and a resorbable barrier membrane. The healing was uneventful and at 3 months postoperatively, bone regeneration was achieved and successful osseointegration had been obtained around the implant. Thus, it can be concluded that placement of a bone graft material along with a barrier membrane can provide a predictable outcome for regeneration of bone and achieving osseointegration around an implant.
Design and development of computer controlled active sonar system

Abstract:Nowadays computerization is the most popular, reliable and flexible design for various types of control and communication systems. Sound navigation and ranging (SONAR) is a modern ultrasonic range finding technique used for collecting the information of a distant object without any physical contact. In this paper, a personal computer (PC) based active SONAR system is presented. The developed SONAR system contains both transmitter and receiver parts in the same module. A pair of 40 kHz piezoelectric ultrasonic transducers is used to transmit ultrasound toward the object and receive the reflected from the object respectively. A driver-software named CURF (Computerized Ultrasonic Range Finder) is developed for this system, using Turbo C language aimed at IBM PC and its clone machines. The CURF software provides necessary support for interfacing the developed hardware as well as desired calculations. A number of tests have been done and it is found that the performances of the developed system are good in terms of accuracy, simplicity, cost and power consumption.
Autogenous Dentin Graft in Bone Defects after Lower Third Molar Extraction: A Split-Mouth Clinical Trial

Abstract:Various biomaterials are currently used for bone regeneration, with autogenous bone being considered the gold standard material because of its osteogenic, osteoconductive, and osteoinductive properties. In recent years, the use of autogenous dentin as a graft material has been described. This split-mouth clinical trial assesses the efficacy of autogenous dentin for the regeneration of periodontal defects caused by bone loss associated with impacted lower third molar extraction. Fifteen patients underwent bilateral extraction surgery (30 third molars) using dentin as a graft material on the test side, and leaving the control side to heal spontaneously, comparing the evolution of the defects by evaluating probing depth at three and six months post-operatively. Bone density and alveolar bone crest maintenance were also evaluated six months after surgery, and pain, inflammation, mouth opening capacity on the second and seventh days after surgery. Probing depth, radiographic bone density, and alveolar bone crest maintenance showed significant differences between the test and control sides. Autogenous dentin was found to be an effective biomaterial for bone regeneration after impacted lower third molar extraction.

Periodontal Regenerative Therapy for Preventing Bone Defects Distal to Mandibular Second Molars After Surgical Removal of Impacted Third Molars: A Systematic Review and Meta-Analysis of Randomized Clinical Trials

Abstract:Purpose: The purpose of this study was to assess which regenerative techniques are most effective for preventing periodontal defects after extraction of the third molars, as well as to compare these procedures with spontaneous healing of the socket. Materials and methods: Five electronic databases were searched to identify randomized clinical trials that fulfilled the eligibility criteria. Two independent reviewers conducted literature screening, article selection, and data extraction. The outcome measures were mean clinical attachment level (CAL) gain, mean probing depth (PD) reduction, mean alveolar bone level (ABL) gain, and adverse events. The influence of several variables of interest on the outcomes of periodontal regenerative therapy was explored via subgroup analyses. Results: Among 1,205 potentially eligible articles, 21 randomized clinical trials were included. Eighteen trials assessed periodontal regenerative therapy as an alternative to extraction alone. Statistically significant differences were found in CAL gain (1.98 mm; 95% confidence interval [CI], 1.44 to 2.52 mm; P < .001), PD reduction (1.76 mm; 95% CI, 1.20 to 2.31 mm; P < .001), and ABL gain (1.21 mm; 95% CI, 0.21 to 2.21 mm; P = .018). The risk of complications developing at treated sites did not increase with the regenerative procedures (odds ratio, 1.49; 95% CI, 0.71 to 3.14; P = .290). There was no evidence of any regenerative procedure being better than any other. However, osseous grafting techniques were associated with a significantly higher adverse event rate. Conclusions: Regenerative periodontal therapy, in comparison with spontaneous healing of the wound, is more effective regarding initial improvement in CAL gain, PD reduction, and ABL gain, without increasing the risk of postoperative complications.

Abstract:The treatment of furcation defects is a complex and difficult task that may compromise the success of periodontal therapy. Here we report a new clinical treatment of a Class II furcation defect using an autogenous bone graft associated with a buccal fat pad (BFP) used as a membrane. The surgical treatment was performed following initial periodontal therapy. Post-operative follow-up appointments were performed at 3, 7, and 12 months. Clinically, after 3 and 7 months, a reduction in probing depth without bleeding on probing and an increase in vertical and horizontal clinical attachment level were observed. After 7 post-operative months, an increase in keratinized gingiva was observed. Radiographically, a significant improvement was noted, with the furcation defect almost completely closed. These results could also be observed after 12 postoperative months. It can be concluded that the combined use of autogenous bone graft and a BFP yielded clinically favorable outcome in the treatment of a mandibular Class II furcation defect.
