Abstract:A clinico-morphologic study was carried out in 63 patients suffering from leukoplakia of the urinary bladder. Certain differential diagnostic characteristics of clinical course and cystoscopic pattern of the disease as well as morphologic features of squamous cell metaplasia were identified. Malignant leukoplakia was registered in 12%. Papilloma and cancer concomitant to leukoplakia were detected in 22 and 27%, respectively.

Abstract:Malakoplakia is a rare granulomatous disease associated with an infectious etiology, usually involving the urinary tract. Clinical presentation malakoplakia is often non-specific, but is typically characterized by recurrent urinary tract infections and hematuria. The etiology of malakoplakia has not been fully elucidated. Frequent association of bladder malakoplakia with urinary tract infection suggests an infectious etiologic agent Malakoplakia of the genitourinary system is more common in women than in men. It is usually observed between the fifth and seventh decades of life. We report a rare case of isolated bladder malakoplakia in a 25-year-old female patient who presented with lower urinary tract symptoms without any hematuria. Clinically, a 15,0 х 11,0 15,0 mm mass was detected in the bladder, which was confirmed by ultrasound scan and magnetic resonance imaging. Malakoplakia is complicated diagnostic problem, since it occurs very rare and can manifest in a different way. Only histological research allows to determine the correct diagnose. The final diagnosis is confirmed by the presence of Michaelis-Gutmann bodies. The majority of patients after being precisely diagnosed can be successfully cured by the lasting treatment of antibiotics in a smaller dose. Represented clinical observation illustrates highly rare case of malakoplakia of bladder. It demonstrates the possibility of usage low invasive surgery not only for diagnostic, but also for therapy. This case made possible to receive the macro preparate for histological research of excellent quality. Histology is essential, not only to diagnose malakoplakia, but also to exclude other important differential diagnoses, such as malignancy. Histopathological examination of the tumor-like mass revealed malakoplakia. Postoperative course was uneventful. This condition should be included in the differential diagnosis of bladder tumors.
[Malacoplakia involving the abdominal wall and urinary bladder: literature review and case report].

Abstract:The authors describe a 44-year old woman's case where a tumor of her lower abdomen has been clinically found. CT of the abdomen showed involving the abdominal wall and urinary bladder. Histologically has been found the specific von Hansemann cells and Michaelis-Gutmann bodies warranting a diagnosis of malakoplakia Partial resection of urinary bladder and mm recti abdominis has been performed. The patient is clinically healthy until present (36 months) with normal laboratory indicators and CT find. The clinical symptoms of this very rare disease are different and depend upon the organ location. The soft tissue tumor-like lesions can mimic a variety of malignant, inflammatory, and infectious diseases. The accurate histologycal estimate leads to mild surgical intervention and postoperative comfort for the patient.

Abstract:Purpose To study the clinicopathological characteristics, the diagnosis and differential diagnosis of the inverted papilloma of the bladder. Methods The clinical, pathological and following-up data were reviewed in 60 cases of the tumors. HE and immunohistochemical stains were performed in paraffin-embedded tissue sections and observed under a microscope. Results Inverted papilloma of the bladder occurred more commonly in males than in females, with the ratio of male to female of 5~7 ∶ 1. Hematuria was the most common symptom. Histologically, the tumors included 47 cases of trabecular type and 13 cases of glandular type .48 cases were followed up for 0.5 to 8 years , and none recurred. Immunohistochemical staining showed that 5 cases were positive (+) , one strong positive (), and 34 negative for p53; 22 of 40 cases were positive (+), 13 were stronger positive (), and 5 were the strongest positive () for PCNA. Conclusions Inverted papilloma of the bladder is a rare benign tumor, which has a good prognosis and recurrent lesions are very low. It is necessary for the diagnosis to rule out the tumors including the papillary urothelial neoplasm of low malignant potential, urothelial carcinoma and urothelial carcinoma with an inverted growth pattern.

Abstract:Purpose: Leukoplakia is the term given to a white patch or plaque that is found mainly on the oral mucus membrane. It can occasionally be seen on the corneal surface. We report our clinical and histopathological findings in a case of unilateral corneal leukoplakia. Methods: A 26-year-old woman was referred to our hospital because of a white patch on her right cornea that continued to expand. She first noticed the white patch when she was 20 years old, and the white patch had expanded to cover the pupillary area affecting her vision. After plastic surgery on both eyelids for bilateral entropion to alleviate the pain caused by the eyelashes rubbing the cornea, the white corneal patch decreased in size. Because of this reduction, we performed surgery to remove the patch with microforceps under topical anesthesia. The plaque was removed easily and completely, and submitted for histopathological examination. Results: Histopathological examination showed that the specimen had characteristics of epidermis with a basal cell layer, spinous cell layer, granular cell layer, and horny layer with hyperkeratosis. She was diagnosed with leukoplakia of the corneal surface. The basic structure of the squamous cell layer was preserved, and there were no signs of metaplasia. Six months after the removal of the leukoplakia, no recurrence was seen and her corrected decimal visual acuity recovered to 1.0. Conclusion: Our case of unilateral corneal leukoplakia without limbal involvement was most likely caused by chronic irritation of the cornea by the eyelashes. Although it was totally removed with good recovery of vision, we continue to follow the patient because of the potential of malignant transformation.

Abstract:Urothelial carcinoma is the most common histologic subtype of bladder cancer, accounting for approximately 90%. We herein report a case of a 78-year-old man with an unusual association of bladder stones with an aggressive plasmacytoid variant of urothelial cancer. Initially he presented in 2009 with a very large bladder stone and was treated by an open cystolithotomy. Histology from a bladder biopsy at that time was benign. He failed to attend follow-up appointments but subsequently he attended in 2016 with a recurrent urinary tract infection and an acute kidney injury. A CT scan showed multiple bladder stones. The cause of our patients' multiple bladder stones is unclear and unusual. He then underwent a further open cystolithotomy according to our multidisciplinary team recommendation. Post-operatively he unfortunately developed a non-healing vesicocutaneous fistula for which he was performed cystoscopy and biopsy but ended with transurethral resection due to the extent of abnormal/necrotic tissue. Histology confirmed a plasmacytoid variant of urothelial cancer. To our knowledge, this is the first case of an association of bladder stones with a plasmacytoid variant of urothelial cancer.<br/>Curr Urol 2019;13:217-223

Abstract:BACKGROUND Tubular epithelial cells (TECs) are essential in the maintenance of kidney function. Apoptosis of TECs occur during acute and chronic renal allograft rejection as well as other forms of renal injury, including autoimmune nephritis. The regulation of TEC apoptosis by proinflammatory cytokines associated with renal inflammation [e.g., interleukin (IL)-2 and interferon-gamma (IFN-gamma)] has not been extensively investigated. METHODS Apoptosis in murine TECs was determined by FACS with annexin-V or ligation-mediated-polymerase chain reaction (LM-PCR) and mRNA levels by reverse transcription (RT)-PCR or Northern blot. Protein expression was observed using Western blot. RESULTS IL-2R (CD25) was expressed by murine TECs and up-regulated by IL-2. Both IL-2 and IFN-gamma induced TEC apoptosis and activated caspase-8. Apoptosis with IL-2 was concentration-dependent and blocked by z-IETD-fmk, a specific caspase-8 inhibitor. Apoptosis with IFN-gamma was associated with increased surface expression of Fas, while IL-2 had no effect on Fas. IL-2 did not induce apoptosis in Fas-deficient TECs (M3.1-lpr) suggesting IL-2 regulation of caspase-8 activity requires Fas. Consistent with this, IL-2 but not IFN-gamma was found to decrease mRNA and protein expression of c-FLIP, an endogenous caspase-8 inhibitor in murine TECs. Overexpression of c-FLIP in TECs (CS3.7-FLIP) blocked apoptosis and caspase-8 activation with both IFN-gamma and IL-2. c-FLIP expression was found in kidney cortex, primary and cloned TECs, suggesting c-FLIP is likely a key regulator of caspase-8-mediated apoptosis in vivo. CONCLUSION This is the first report of c-FLIP regulation by IL-2 in renal TECs. Augmentation of c-FLIP in TECs may enhance an endogenous mechanism by which TECs normally resist injury to caspase-8-mediated apoptosis and thus may be a useful and novel strategy to prevent tubular injury in transplant rejection and autoimmune nephritis.
Exploring the Histopathological Landscape of Urinary Bladder Diseases: A Tertiary Care Center Study

Abstract:Introduction Urinary bladder lesions encompass a wide spectrum, from benign inflammatory conditions to malignant neoplasms, presenting diagnostic and therapeutic challenges. Urothelial carcinoma predominates among bladder malignancies, exhibiting diverse clinical presentations and prognoses. Objective This study aimed to delineate the histopathological spectrum of urinary bladder lesions and correlate demographic profiles, clinical features, and cystoscopic findings with various bladder lesions. Methods This prospective descriptive observational study spanned 24 months at a tertiary care center, involving 65 cases of urinary bladder biopsies, including transurethral resection of bladder tumors, cystoscopic biopsies, and cystectomy specimens. The histopathological examination followed the WHO 2022 classification of urinary bladder tumors and the American Joint Committee on Cancer eighth edition staging. Clinical data, including age, gender, cystoscopic findings, and presenting symptoms, were correlated with histopathological diagnoses to explore the spectrum of bladder lesions. Results Neoplastic lesions predominated, constituting 92.3% of cases, with urothelial carcinoma comprising 83.33% of these cases. Among neoplastic lesions, invasive high-grade urothelial carcinoma (36.7%) and non-invasive low-grade papillary urothelial neoplasm (20.0%) were the most frequently observed subtypes. Non-neoplastic lesions accounted for 7.7%, including various forms of cystitis. Hematuria was the predominant presenting symptom (81.5%), while cystoscopic examinations revealed that most lesions were situated in the lateral bladder wall. High-grade urothelial carcinomas were mostly associated with muscularis propria invasion. Conclusion This study underscores the critical role of histopathological examination in diagnosing and managing urinary bladder diseases and distinguishing between non-neoplastic and neoplastic lesions. Urothelial carcinoma, prevalent among older age groups, often demonstrated muscle invasion indicative of high-grade tumors. Including the muscle layer in cystoscopic biopsies is crucial for an accurate diagnosis. Conversely, though less common, non-neoplastic conditions encompass various forms of cystitis. These findings highlight the importance of precise diagnostic tools such as cystoscopy and histopathological examination for the early detection and management of bladder neoplasms. Histopathological assessment offers essential prognostic guidance, aids in precise staging and grading, and directs tailored treatment strategies.
Candidiasis Causing Vocal Fold Leukoplakia: Review of Clinical and Pathological Results of 289 Cases With Vocal Fold Leukoplakia

Abstract:Objective: In laryngology practice, vocal fold leukoplakia is frequently evaluated by suspension laryngoscopy and biopsy examination upon the patient's complaints of hoarseness and dysphonia. The purpose of the present study is to investigate and analyze risk factors, diagnosis, treatment, and follow-up results of cases with Candida leukoplakia. Study design: Retrospective case control study. Setting: Tertiary medical center. Subjects and methods: Patients with a diagnosis of vocal fold leukoplakia who underwent direct laryngoscopy and biopsy between 2007 and 2017 and diagnosed as candida or noncandida in their histopathology were assigned into 2 groups. Then they were compared in terms of their demographic characteristics, predisposing factors, diagnosis, treatment, and follow-up results. Results: Of the 289 vocal fold leukoplakia cases, 36 were candida, and 253 were noncandida. The mean age of the patients with Candida leukoplakia was 60.86 years. As for the age groups, the largest group (26.1%) was in the seventh decade (P < .001). The use of inhaled corticosteroids was a significant risk factor (P < .001). For their medical therapy, the patients were administered fluconazole 200 mg per day for 3 weeks, and the treatment yielded successful results in 91.66% of them. In 5 of the patients, candida leukoplakia and superficial epithelial dysplasia were observed, and no malignant transformation was observed during a mean follow-up of 28 ± 13 months. Conclusion: Candidiasis causing vocal fold leukoplakia is rare, and we report the findings of the largest published case series to date. Eliminating predisposing factors and administrating oral fluconazole 200 mg for 3 weeks are sufficient for medical treatment.

Abstract:We report the case of a 79-year-old woman with primary lymphoma of mucosa-associated lymphoid tissue (MALT) in the urinary bladder. The patient, with urinary frequency, urgency and suprapubic pain had several emergency room visits due to recurrent urinary tract infection. Both sonogram and cystoscopy identified bladder tumors near the bladder neck. An abdominal contrast-enhanced computed tomography scan revealed a polypoid lesion on the anterior bladder wall without enlarged lymph nodes. Transurethral resection of the bladder tumor was conducted. The pathology report confirmed extranodal marginal zone MALT lymphoma. The clinical stage was IEA. Follow-up imaging reported residual bladder tumors, prompting adjuvant radiotherapy. The patient was treated successfully and was disease-free at the 9-month follow-up visit. Primary lymphoma is an uncommon pathological subtype. Its clinical and radiological differentiation from urothelial carcinoma (UC) can be challenging, but treatment strategies differ significantly. A definitive diagnosis relies on histopathology and immunohistochemistry. Typically, bladder lymphoma has a favorable prognosis, but further research is required to identify the optimal treatment.

Abstract:Background: Primary bladder lymphoma is generally regarded as having a favorable prognosis due to the predominance of low-grade lymphomas confined to the bladder. However, our investigation reveals that cases with extravesical extension, predominantly involving diffuse large B-cell lymphoma (DLBCL), exhibit a distinct clinical course with varied prognostic outcomes. Methods: In this report, we present and analyzed the clinical features and outcomes of 47 patients with primary bladder lymphoma with extravesical extension, including the case that we experienced. Results: An 77-year-old man who experienced fever, anorexia, and general malaise was referred to our hospital. Initial laboratory tests indicated severe renal failure, pyuria, and Escherichia coli bacteremia, accompanied by diffuse thickening of the bladder walls and increased attenuation in the surrounding adipose tissues. Initially misdiagnosed with a severe urinary tract infection leading to sepsis, the patient was treated with antibiotics and hemodialysis. Upon readmission due to abdominal pressure, imaging identified an intra-abdominal mass connected to the bladder wall. A bladder biopsy was performed, resulting in the diagnosis of primary bladder DLBCL with perivesical extension, classified as germinal center B-cell type. Taking inspiration from this case, the review of 46 patients was implemented. As a result, we resolved that primary bladder lymphoma often includes indolent types like Mucosa-associated lymphoid tissue lymphoma, but cases with extravesical expansion are predominantly DLBCL. Conclusions: This case emphasizes the diagnostic complexities of distinguishing primary bladder lymphoma from urinary tract infections and underscores the prognostic implications of extravesical extension. Our comprehensive review of the literature on primary bladder lymphomas with extravesical involvement highlights the clinical characteristics, therapeutic challenges, and need for heightened diagnostic vigilance and tailored treatment strategies for this subset of patients.

Abstract:In this paper, we propose smart step closed-loop power control (SSPC) algorithm in a direct sequence-code division multiple access (DS-CDMA) receiver in the presence of frequency-selective Rayleigh fading. This receiver consists of three stages. In the first stage, with conjugate gradient (CG) adaptive beamforming algorithm, the desired users' signal in an arbitrary path is passed and the inter-path interference is canceled in other paths in each RAKE finger. Also in this stage, the multiple access interference (MAI) from other users is reduced. Thus, the matched filter (MF) can be used for the MAI reduction in each RAKE finger in the second stage. Also in the third stage, the output signals from the matched filters are combined according to the conventional maximal ratio combining (MRC) principle and then are fed into the decision circuit of the desired user. The simulation results indicate that the convergence speed of the SSPC algorithm is faster than other algorithms. Also, we observe that significant saving in total transmit power (TTP) are possible with our proposed algorithm.

Abstract:In the past decades several definitions of oral leukoplakia have been proposed, the last one, being authorized by the World Health Organization (WHO), dating from 2005. In the present treatise an adjustment of that definition and the 1978 WHO definition is suggested, being : "A predominantly white patch or plaque that cannot be characterized clinically or pathologically as any other disorder; oral leukoplakia carries an increased risk of cancer development either in or close to the area of the leukoplakia or elsewhere in the oral cavity or the head-and-neck region". Furthermore, the use of strict diagnostic criteria is recommended for predominantly white lesions for which a causative factor has been identified, e.g. smokers' lesion, frictional lesion and dental restoration associated lesion. A final diagnosis of such leukoplakic lesions can only be made in retrospect after successful elimination of the causative factor within a somewhat arbitrarily chosen period of 4-8 weeks. It seems questionable to exclude "frictional keratosis" and "alveolar ridge keratosis" from the category of leukoplakia as has been suggested in the literature. Finally, brief attention has been paid to some histopathological issues that may cause confusion in establishing a final diagnosis of leukoplakia.

Abstract:Primary mucinous lesions of the urinary system are extremely rare. We describe two cases of primary mucinous lesions of the urothelial tract. One case is of mucinous metaplasia in the bladder of a 40-year-old man presenting with frequent urination, urgency, and gross hematuria. The other case is of mucinous adenocarcinoma in the pelvis of an otherwise healthy 67-year-old man with left nephrolithiasis. The histological images of the two cases demonstrate a spectrum from benign mucinous metaplasia to malignant mucinous adenocarcinoma, and suggest that mucinous metaplasia in urothelial tract may be the precancerous lesion of mucinous adenocarcinoma.
