Making room for interactivity: using the cloud-based audience response system Nearpod to enhance engagement in lectures

Abstract:Active and collaborative learning provides distinct advantages for students in higher education, yet can often be hampered by the barrier of large class sizes. Solutions that combine a 'bring your own device culture' with cloud-based technologies may facilitate a more interactive learning experience. In this pilot study, we describe the use of one such technology, Nearpod, to enhance interactivity in lectures delivered to pharmacy and bioscience students at Ulster University. Existing material in PowerPoint or Keynote format is uploaded to the instructor area of Nearpod, interactive elements are added, and the lecture is then broadcasted via the internet to student devices. The lecturer may choose to share polling responses or examples of submissions from the drawing tool or open-ended questions, thereby providing instant feedback on learning. Students commented favourably on the interactivity and engagement afforded by Nearpod. Most students were happy to use their own electronic devices (smartphones, tablets and laptops) for such activities with a minority expressing concern over problems with connecting to the institutional Wi-Fi. Nearpod and similar products represent a new class of feature-rich audience response systems that have potential to transform learning even in large classes.
Flipped Classroom With Artificial Intelligence: Educational Effectiveness of Combining Voice-Over Presentations and AI

Abstract:Background: Most theorists and medical educators agree that a curriculum rich in active learning (AL) strategies, such as a flipped classroom, is superior to passive listening to promote better retention and application of new knowledge. Although AL multimodal teaching strategies have been considered the most effective, including online virtual teaching, voice-over pre-recorded lectures, and, more recently, the addition of artificial intelligence (AI), data on the effectiveness of these methods in medical education is scarce. The present educational research study examined the effectiveness of voice-over-style lectures and AI in facilitating learning outcomes as assessed by test scores after participating in basic science lectures in a medical school setting. Methods: Participating students were divided equally into two educational strategy groups: slide decks only traditional way (PPT) or PPT plus AI (PPT+AI) platform (edYOU; Los Angeles, CA, USA). The PPT+AI group comprised the PPT with narration and real-time interaction with an AI being personalized, which leverages natural language processing to tailor customized conversations to each student's current knowledge. Students in the two groups were asked to participate in a formative quiz (not reflective of their academic evaluations) to answer questions relevant to voice-over lectures (PPT and PPT+AI). The statistical strategy for conducting quiz item analysis included item difficulty, item discrimination, and point-biserial correlation R. A student's T-test was conducted to compare the two strategies' effectiveness via test scores. A priori, an alpha level of 0.05 was considered significant. Results: Data are presented as mean ± s.e.m.; Cohen's d. A total of 42 (n=21 in each group) students participated in the study. Students using PPT+AI obtained statistically significant (P <0.043; d = .54) higher quiz scores under challenging questions and less time spent in lectures (54.1 ± 14.3 hrs.) in the PPT+AI group (P <0.001; d = 1.17) compared with the PPT group. Conclusions: The PPT+AI strategy could be the difference between a pass and a fail, as the PPT+AI strategy is particularly efficient in improving difficult question test scores. At the same time, students may learn the material in less time (efficiency). Research on the application of AI as part of educational strategies for improving satirized test scores, including boards, is warranted. The present study is part of the necessary early steps to better understand the impact of AI as an educational strategy for improving educational outcomes.
Development and impact of a simplified approach to didactic powerpoint presentations performed in a drug information course

Abstract:Introduction: To describe multimedia presentation development and assess impact of use on student and educator performance in a drug information course. Methods: Slides were modified from bullet-point format to simplified pictorial format and three-slide handouts became outlined Word documents. Student performance, via exam scores and class average, and educator evaluation scores were compared between 2013 and 2014. Results: Presentation revision resulted in a similar number of slides per presentation and smaller handout packets. Class average and final exam scores decreased (86.65% vs. 85.91%, p = 0.222; 84.88% vs. 84.08%, p = 0.053) while midterm exam scores improved (85.81% vs. 87.80%, p = 0.007). Assessment of teacher effectiveness (scale of 1 = strongly disagree to 5 = strongly agree) rose from a median of 3-4 (p = 0.06). Conclusion: Simplifying presentations provided anecdotal value but did not impact student performance nor student perception of educator effectiveness.

Abstract:For physician associate/assistant (PA) educators, the path of least resistance often is to focus our cognitive energies on topics with which we are most comfortable and to deliver content in the style in which we are most accustomed. The consequence of this approach is that many didactic curricula are delivered in a traditional, lecture-based format, which then aligns with how the faculty were taught. Studies show that lecture-based teaching may not be the most effective and that blending teaching modalities can improve knowledge acquisition. In addition, scheduling several classroom lectures week to week can be challenging, especially in a program that uses numerous guest lecturers to deliver the curriculum. One solution to this problem may be through innovative, engaging resources that enable faculty to deliver content asynchronously. Developing and implementing such tools can be challenging to PA faculty because they are often not formally trained in education nor are they up to date on the most current innovations in educational technology. Furthermore, change can be difficult, and it could seem an insurmountable feat to step outside the comfort zone of a traditional lecture. This work describes a collaboration between PA faculty and a University Instructional Support Specialist where the Analysis, Design, Develop, Implement, Evaluate ADDIE model, a widely used system for developing training tools, was used to create an asynchronous lecture for students in the form of an interactive series of modules to replace a traditional classroom lecture.
"Powerful placebo": A teaching and learning concept addressing placebo and nocebo effects in competency-based communication training

Abstract:Aim: Placebo and nocebo effects are based on expectations that are formed by how doctors communicate and can influence the efficacy of medical treatment. Given the implications for doctor-patient communication and the learning objectives listed in NKLM 2.0, we herein present a novel teaching and learning concept to impart competency-based knowledge about placebo and nocebo effects. Method: The teaching and learning concept was piloted with N=324 third-semester medical students. It combines a self-guided, small-group component to gather communication strategies and apply them in a video-recorded conversation, followed by a classroom-based session to reflect on and discuss the videos and to learn basic scientific and theoretical knowledge. The evaluation involved written feedback from the students and lecturers (structure/process) and an analysis of the videos (students' learning success). To supplement this, the overall course evaluation was included since this new teaching concept was not specifically evaluated by the students. Results: Course structure and process were rated positively. The active involvement of the students in the subject matter and the balance between theoretical, scientific and practical content was emphasized positively. Analysis of the learning success showed that the students were able to effectively transfer the knowledge gained about placebo and nocebo effects to conversational situations. Conclusion: The topic of placebo/nocebo is optimally suited to teach communication skills with its many links to knowledge, translational approaches and added value for medical practice. When doing this, video-recorded conversations appear to be an effective tool to achieve learning objectives. This teaching and learning strategy offers possibilities for expanding communication curricula.
Teaching for understanding in medical classrooms using multimedia design principles

Abstract:OBJECTIVES: In line with a recent report entitled Effective Use of Educational Technology in Medical Education from the Association of American Medical Colleges Institute for Improving Medical Education (AAMC-IME), this study examined whether revising a medical lecture based on evidence-based principles of multimedia design would lead to improved long-term transfer and retention in Year 3 medical students. A previous study yielded positive effects on an immediate retention test, but did not investigate long-term effects.METHODS: In a pre-test/post-test control design, a cohort of 37 Year 3 medical students at a private, midwestern medical school received a bullet point-based PowerPoint™ lecture on shock developed by the instructor as part of their core curriculum (the traditional condition group). Another cohort of 43 similar medical students received a lecture covering identical content using slides redesigned according to Mayer's evidence-based principles of multimedia design (the modified condition group).RESULTS: Findings showed that the modified condition group significantly outscored the traditional condition group on delayed tests of transfer given 1 week (d = 0.83) and 4 weeks (d = 1.17) after instruction, and on delayed tests of retention given 1 week (d = 0.83) and 4 weeks (d = 0.79) after instruction. The modified condition group also significantly outperformed the traditional condition group on immediate tests of retention (d = 1.49) and transfer (d = 0.76).CONCLUSIONS: This study provides the first evidence that applying multimedia design principles to an actual medical lecture has significant effects on measures of learner understanding (i.e. long-term transfer and long-term retention). This work reinforces the need to apply the science of learning and instruction in medical education.

Interactive E-learning module in pharmacology: a pilot project at a rural medical college in India

Abstract:Many medical educators are experimenting with innovative ways of E-learning. E-learning provides opportunities to students for self-directed learning in addition to other advantages. In this study, we designed and evaluated an interactive E-learning module in pharmacology for effectiveness, acceptability and feasibility, with the aim of promoting active learning in this fact-filled subject. A quasi-experimental single-group pre-test/post-test study was conducted with fourth-semester students of the second professionals course (II MBBS), selected using non-probability convenience sampling method. An E-learning module in endocrine pharmacology was designed to comprise three units of interactive PowerPoint presentations. The pre-validated presentations were uploaded on the website according to a predefined schedule and the 42 registered students were encouraged to self-learning using these interactive presentations. Cognitive gain was assessed using an online pre- and post-test for each unit. Students' perceptions were recorded using an online feedback questionnaire on a 5-point Likert scale. Finally, focused group discussion was conducted to further explore students' views on E-learning activity. Significant attrition was observed during the E-learning activity. Of the 42 registered students, only 16 students completed the entire E-learning module. The summed average score of all three units (entire module) was increased significantly from 38.42 % (summed average pre-test score: 11.56/30 ± 2.90) to 66.46 % (summed average post-test score: 19.94/30 ± 6.13). The class-average normalized gain for the entire module was 0.4542 (45.42). The students accepted this E-learning activity well as they perceived it to be innovative, convenient, flexible and useful. The average rating was between 4 (agree) and 5 (strongly agree). The interactive E-learning module in pharmacology was moderately effective and well perceived by the students. The simple, cost-effective and readily available Microsoft PowerPoint tool appealed to medical educators to use this kind of simple E-learning technology blended with traditional teaching to encourage active learning among students especially in a rural setup is attractive.

Abstract:Objective. To evaluate the impact of animated videos of oral solid dosage form manufacturing as visual instructional aids on pharmacy students' perception and learning. Design. Data were obtained using a validated, paper-based survey instrument designed to evaluate the effectiveness, appeal, and efficiency of the animated videos in a pharmaceutics course offered in spring 2014 and 2015. Basic demographic data were also collected and analyzed. Assessment data at the end of pharmaceutics course was collected for 2013 and compared with assessment data from 2014, and 2015. Assessment. Seventy-six percent of the respondents supported the idea of incorporating animated videos as instructional aids for teaching pharmaceutics. Students' performance on the formative assessment in 2014 and 2015 improved significantly compared to the performance of students in 2013 whose lectures did not include animated videos as instructional aids. Conclusions. Implementing animated videos of oral solid dosage form manufacturing as instructional aids resulted in improved student learning and favorable student perceptions about the instructional approach. Therefore, use of animated videos can be incorporated in pharmaceutics teaching to enhance visual learning.

Abstract:Abstract There is a shortage of learners completing nursing training and pursuing roles in geriatrics and dementia care, possibly caused by ageism, misconceptions, and personal experiences such as with family members, along with uncertainty and discomfort and providing care to this population. Using Kern’s six-steps of curriculum design, we set out to design a novel, blended-learning intervention to improve dementia education at a local nursing school. After reviewing the literature, local needs assessments were carried out in the form of stakeholder discussions and semi-structured interviews with a subset of nursing students. Interview results, themes from the literature, and incorporation of the current learning objectives in the existing curriculum, were integrated to create a new lesson plan, including a “flipped classroom” component using 2D vector animation, as well as animation-assisted, interactive, case-based lecture and discussion format. Media was designed through an iterative process including review of content outline, objectives, storyboards, and concept art by stakeholders and content experts throughout the design process. This novel approach to interdisciplinary, blended-learning curriculum design has the potential to improve nursing student attitudes and foundational knowledge about dementia and cognition.

Abstract:1. We report here about the modulation of dorsal spinocerebellar tract (DSCT) activity by limb posture. In principle, DSCT activity could represent limb position in one of several ways. According to a classical notion of DSCT function, DSCT activity might be expected to correlate with changes in individual joint angles. However, given the evidence for extensive polysynaptic convergence onto DSCT units, it is reasonable to propose that DSCT activity represents more global variables such as the orientation of limb segments or the length and orientation of the whole limb. 2. In six anesthetized cats we recorded the activity of 96 antidromically identified DSCT neurons while a robot arm passively positioned the left hindfoot in 20 positions distributed in the sagittal plane, holding each position for 8 s. For each position we measured the joint angles, limb segment angles, and the length and orientation of the limb axis (defined as the line connecting the hip joint to the hindpaw). We used regression statistics to quantify 1) possible relationships among geometric variables of the hindlimb and 2) relationships between DSCT firing rate and limb variables. 3. First, we found a statistically significant relationship among the joint angles that could be described by a covariance plane accounting for approximately 70 percent of the total variance. Thus the 3 degrees of freedom represented by the joint angles in the sagittal plane are effectively reduced to only 2 by the coupling between joints. This finding resembles that described for the behaving cat during stance. However, the correlation between the hip and ankle angles in the passively displaced hindlimb was just the opposite of that observed during active stance. Moreover, we observed that the length and the orientation of the limb axis is determined simply by a linear combination of the three joint angles. 4. Most of the DSCT neurons (82 of 96) were significantly modulated by changes in foot position (1-way analysis of variance, P < 0.001). For those cells, we explored systematically how their activity was related to limb geometric variables. We found mostly linear relationships between individual joint or limb segments angles and DSCT firing rates. However, although these relationships were statistically significant, the random variance was often quite high. Moreover, approximately 70% of the cells were modulated by more than one joint or limb segment angle, suggesting that a model incorporating global geometric variables might explain a larger fraction of the variance in the neural data. 5. Consequently we tested how well DSCT activity was modulated by the length and the orientation of the limb axis with the use of a linear regression model with length and orientation (or the equivalent linear combination of joint angles) as predictors. We found that this model explained a larger fraction of the variability in the firing pattern of nearly every modulated cell than did any of the single joint models tested. 6. We also attempted to account for the effect of the mechanical joint covariance on this result by accounting for correlated independent variables in the analysis. We used a regression model incorporating all three joint or limb segment angles and performed a backward elimination of insignificant or redundant variables. The result was that 67% of the neurons were independently modulated by at least two joint angles, indicating that the modulation did not necessarily reflect the biomechanical constraint of joint angle covariation, but rather a central convergence of sensory information from more than a single joint. 7. From these results we conclude that the firing rates of a majority of DSCT neurons encode the position of the hindfoot relative to the hip joint.(ABSTRACT TRUNCATED)
Patient Narratives as a Teaching Tool: A Pilot Study of First-Year Medical Students and Patient Educators Affected by Intellectual/Developmental Disabilities

Abstract:PROBLEM: People with intellectual and developmental disabilities (IDD) face complex biopsychosocial challenges and are medically underserved. This is in part due to insufficient resources and supports but can also be attributed to a lack of adequate physician training in addressing the unique needs of this population.INTERVENTION: This study aimed to introduce 1st-year medical students to the IDD population using a blended educational experience that included video narratives of and direct interactions with people affected by IDD. The goal of this intervention was to promote person-centered attitudes and communication among early medical trainees.CONTEXT: The study recruited 27 first-year medical students and randomly assigned each to 1 of 2 groups. The control group received an introductory video lecture about IDD healthcare, followed by a quiz. The narrative group received the same lecture, followed by reflective discussion of videos featuring people living with IDD sharing their perspectives and stories. All students then participated in 4 simulated clinical encounters with patient educators (PEs) who have lived experiences of IDD. Focus groups were conducted with students following the simulated encounters to explore their experiences and perceptions of this blended learning activity. Moreover, secondary quantitative data were collected to assess students' performance in the clinical encounters, along with self-reports of comfort, confidence, and competence of interacting with people with IDD (pre- and postparticipation).OUTCOME: All students thought that the blended educational experience was valuable and enjoyable, commenting on the importance of adaptable language and engagement of people with IDD, as well as the merits of reflecting on patient narratives. Students also discussed feelings of discomfort stemming from a lack of knowledge and previous exposure to IDD and how this discomfort might motivate them to learn more and develop their skills further. In addition, descriptive analyses revealed that students in the narrative group showed greater self-rated measures of comfort, confidence, and competence compared to control; they also had higher mean performance scores across all PE interview stations.LESSONS LEARNED: PEs add a powerful real-life dimension to communication skills teaching and have been shown to be a valuable educational modality. Moreover, exposure to and reflection on video-based patient narratives are useful ways of teaching medical students about patients' lived experiences and promoting person-centered communication, both within and beyond IDD.

Abstract:Use of audio visual tools to aid in medical education is ever on a rise. Our study intends to find the efficacy of a video prepared on "fatty liver," a topic that is often a challenge for pre-clinical teachers, in enhancing cognitive processing and ultimately learning. We prepared a video presentation of 11:36 min, incorporating various concepts of the topic, while keeping in view Mayer's and Ellaway guidelines for multimedia presentation. A pre-post test study on subject knowledge was conducted for 100 students with the video shown as intervention. A retrospective pre study was conducted as a survey which inquired about students understanding of the key concepts of the topic and a feedback on our video was taken. Students performed significantly better in the post test (mean score 8.52 vs. 5.45 in pre-test), positively responded in the retrospective pre-test and gave a positive feedback for our video presentation. Well-designed multimedia tools can aid in cognitive processing and enhance working memory capacity as shown in our study. In times when "smart" device penetration is high, information and communication tools in medical education, which can act as essential aid and not as replacement for traditional curriculums, can be beneficial to the students. © 2015 by The International Union of Biochemistry and Molecular Biology, 44:241-245, 2016.

Abstract:This development research aims to analyze learning media to create Islamic education teaching media with interactive media with the help of Articulate Storyline Software. Using research and development methods with qualitative and quantitative approaches, this research was conducted at SMAN 12 Berau as a subject and a test tool for the teaching media. The use of ADDIE as a procedure and guideline for developing teaching media. Data collection techniques by distributing surveys to test subjects and a series of formative assessments. The analysis technique uses qualitative and statistics. The results of the analysis of the development of these teaching materials are in the form of software products or Electronic Student Book (BSE) applications. The teaching material is suitable for use based on test results and analysis, which explains the survey results of design experts are 86%, media experts give a percentage of 87%, and materials experts consider 80%. Due to the development of teaching materials, Articulate Storyline Software can be easily used via a cellphone or laptop with an attractive appearance so that students will efficiently operate and learn by utilizing the application.
Energy Efficient Routing Protocol for Maximizing Lifetime in Wireless Sensor Networks using Fuzzy Logic and Immune System

Abstract:Energy limitations has become fundamental challenge for designing wireless networks systems. One of the important and interested features is network lifetime. Many works have been developed to maximize wireless sensor network lifetime, in which one of the important work is routing. This paper proposes a new adaptive routing technique for prolonging the lifetime in wireless sensor networks by using Fuzzy-Immune System. The artificial immune system is used to solve the packet LOOP problem and to control route direction. While fuzzy logic system is used to determine the optimal path for sending data packets. The proposed routing technique seeks to determine the optimal route path from source to destination so that energy consumption is balanced. The proposed routing technique is compared with classical method. Simulation results demonstrate that the proposed technique shows significant increase in network lifetime of about 0.93. The proposed technique proved that energy consumption is well managed.

Abstract:UNLABELLED Thrombosis of the axillary vein is a rare event. The incidence of 0.5, -1.5% of all venous thromboses makes comparative studies concerning therapy in a single center impossible. 15 own patients and 750 patients with axillary vein thrombosis collected from literature were studied, to compare the different therapeutic modalities. 36% received no therapy (NV), 15% anticoagulation (AG: heparin, warfarin, coumarins), 18% thrombolytic therapy (TL streptokinase, urokinase, locally and systemic) and 31% were treated by surgery (OP). Mean follow up was 32 months in NV, AG: 16, 3, TL: 35, OP: 38 months. RESULTS NV; No complaints: 12%, PTS I: 29%, PTS II: 25%, PTS III: 33%. CONCLUSION Untreated axillary vein thrombosis leads to functional disability of the affected extremity in one third. AG reduces this rate to 25%. After surgery and thrombolytic therapy a severe PTS is found only in 12% and 13.7% respectively. Loss of extremity was found in 5 cases (0.6%). A mechanic factor seems to be important in thrombogenesis. Rapid restitution of venous backflow and promotion of collateralisation is important to avoid disability of the patient. The low difference between AG and OP is pointed out indicating that invasive surgery should be restricted to special cases.
Improving Prelicensure Student Learning and Experience With Interactive Online Learning Modules

Abstract:Background: Fundamentals of Nursing students at a large school of nursing in the Midwest reported difficulty using their course textbooks. There was also an increase in course failures and a decrease in performance on a Fundamentals of Nursing National Council Licensure Examination (NCLEX-RN®) preparatory proctored examination. Method: A pilot project was implemented to evaluate the effectiveness of interactive, online learning modules compared to textbook readings on student performance, access and use of resources, and student experience. Results: Following the pilot there was a significant improvement in course grades among traditional but not accelerated students using the modules compared to those using the textbook. Students reported that the modules were more engaging, cost-effective, and appealing to various learning styles. Weaknesses identified were the module lengths and technological concerns. Conclusion: The online, interactive modules engaged students with course materials, reduced costs, and may have changed student performance. Abstract:The creation of videos in teaching has a high educational potential and is a challenge that can motivate students. There is little evidence on the use of this method when applied to the creation of digital stories. Thus, the aim of this study was to measure student satisfaction with the creation of audiovisual material through digital storytelling, measure its usefulness, and evaluate its impact on their motivation to study the subject. As a secondary objective, we intended to determine the influence of this learning experience on raising awareness of society toward mental illnesses by measuring the impact by the number of views on social networks. A cross-sectional descriptive study design was used. The participants were 90 third-year nursing students enrolled in the subject “Psychiatric Nursing”. The students created eight themed videos (depression, suicide, anxiety, anorexia, mobile phone addiction, obsessive-compulsive disorder, drug addiction, schizophrenia). The students were then asked to complete an ad hoc questionnaire on the matter. A total of 90% of the nursing students thought that creating the videos improved the acquisition of nursing knowledge, 91.2% replied that they would like to use the method in other subjects on the degree in nursing syllabus, and 67.8% thought that their clinical skills improved after using narrated digital stories to create videos. Students acknowledged that this training activity helped them feel better prepared and helped them better understand the subject. They believe that this teaching technique is more stimulating and more enjoyable than the conventional system, giving them more motivation to study the subject. Students acknowledge that the experience gained from this initiative has helped them feel better prepared and helped them better understand the subject, and they think that it will be a useful resource in the future as it has improved the process of the creation of audiovisual material through digital storytelling.
