Abstract:A well-established interpretation associates the nineteenth-century psychiatrist Pliny Earle's deflation of high cure rates for insanity with the onset of a persistent malaise in patient treatment and public health policy during the Gilded Age. This essay comes not to praise Earle but to correct and clarify interpretations, however well intentioned, that are incomplete and inaccurate. Several points are made: the overwhelming influence of antebellum enthusiasm on astonishing therapeutic claims; the interrogation of high "recovery" rates begun decades before Earle's ultimate provocation; and, however disruptive, the heuristically essential contribution of Earle's challenge to furthering a meaningful model of mental disorder. In spite of the impression created by existing historiography, Earle, a principled Quaker, remained committed to "moral treatment."

The Stethoscope in 19th-Century American Practice: Ideas, Rhetoric, and Eventual Adoption

Abstract:The stethoscope was invented in 1816 by the French physician R.T.H. Laennec, who, after three years of clinical observations, published his treatise Mediate Auscultation in 1819. In his treatise, Laennec included details of his new method of using the stethoscope to provide physiological and pathological evaluation of patients. American physicians attended lectures and clinics at Paris hospitals and carried this information back to their respective medical schools and practices. This was accomplished by a relatively limited number of elite American physicians who were able to take advantage of travel abroad and whose practices were academically affiliated. However, it is a well-substantiated historical claim that the adoption of the stethoscope by most American physicians was slow. There are many reasons for slow adoption of the stethoscope in America, among which are lack of formal education, including bedside training in the stethoscope, complexity of interpretation of auscultatory information, hesitancy of the patient and physician to have an instrument placed between them, and lack of opportunities for continuing education for physicians after leaving medical school. As the nineteenth century progressed, scientific ideas and rhetoric related to auscultation and the stethoscope became more widespread, reflecting gradual acceptance and adoption of the stethoscope by American practitioners. In this article, I examine the ideas and rhetoric in medical journal articles, advertisements, and medical school textbooks to learn what was thought by physicians to be important in their practice. Advertisement of medical school curricula with mention of specific course work or lectures related to auscultation or the stethoscope is noted, reflecting increased interest in the stethoscope as an adjunct to physical examination. This information introduces evidence to test and bolster the existing historical claims of slow adoption of the stethoscope by addressing in more detail when and why adoption by American physicians became widespread.

Abstract:There are currently no biological tests that differentiate patients with bipolar disorder (BPD) from healthy controls. While there is evidence that peripheral gene expression differences between patients and controls can be utilized as biomarkers for psychiatric illness, it is unclear whether current use or residual effects of antipsychotic and mood stabilizer medication drives much of the differential transcription. We therefore tested whether expression changes in first-episode, never-medicated BPD patients, can contribute to a biological classifier that is less influenced by medication and could potentially form a practicable biomarker assay for BPD. We employed microarray technology to measure global leukocyte gene expression in first-episode (n=3) and currently medicated BPD patients (n=26), and matched healthy controls (n=25). Following an initial feature selection of the microarray data, we developed a cross-validated 10-gene model that was able to correctly predict the diagnostic group of the training sample (26 medicated patients and 12 controls), with 89% sensitivity and 75% specificity (p<0.001). The 10-gene predictor was further explored via testing on an independent cohort consisting of three pairs of monozygotic twins discordant for BPD, plus the original enrichment sample cohort (the three never-medicated BPD patients and 13 matched control subjects), and a sample of experimental replicates (n=34). 83% of the independent test sample was correctly predicted, with a sensitivity of 67% and specificity of 100% (although this result did not reach statistical significance). Additionally, 88% of sample diagnostic classes were classified correctly for both the enrichment (p=0.015) and the replicate samples (p<0.001). We have developed a peripheral gene expression biomarker profile, that can classify healthy controls from patients with BPD receiving antipsychotic or mood stabilizing medication, which has both high sensitivity and specificity. Moreover, assay of three first-episode patients who had never received such medications, to first enrich the expression dataset for disease-related genes independent of medication effects, and then to test the 10-gene predictor, validates the peripheral biomarker approach for BPD.

Abstract:Objectives/hypothesis: To document the history of hearing seeing in children and adults. Study design: A literature search in all languages was carried out with the terms of hearing screening from the following sources: Pub Med, Science Direct, World Catalog, Index Medicus, Google scholar, Google Books, National Library of Medicine, Welcome historical library and The Library of Congress. Methods: The primary sources consisting of books, scientific reports, public documents, governmental reports, and other written material were analyzed to document the history of hearing screening. Results: The concept of screening for medical conditions that, when found, could influence some form of the outcome of the malady came about during the end of 19th century. The first applications of screening were to circumscribe populations, schoolchildren, military personnel, and railroad employees. During the first half of the 20th century, screening programs were extended to similar populations and were able to be expanded on the basis of the improved technology of hearing testing. The concept of universal screening was first applied to the inborn errors of metabolism of newborn infants and particularly the assessment of phenylketonuria in 1963 by Guthrie and Susi. A limited use of this technique has been the detection of genes resulting in hearing loss. The use of a form of hearing testing either observational or physiological as a screen for all newborns was first articulated by Larry Fisch in 1957 and by the end of the 20th century newborn infant screening for hearing loss became the standard almost every nation worldwide. Conclusions: Hearing screening for newborn infants is utilized worldwide, schoolchildren less so and for adults many industrial workers and military service undergo hearing screening, but this is not a general practice for screening the elderly. A narrative review of the management of pars flaccida tympanic membrane retractions without cholesteatoma

Abstract:Objective: Review the effectiveness of surgical and non-surgical management strategies for isolated pars flaccida and combined pars tensa and flaccida tympanic membrane retractions in preventing progression or recurrence, improving hearing and preventing development of cholesteatoma. Design: Narrative review. Setting: ENT and otology services worldwide. Participants: Patients with non-cholesteatoma tympanic membrane retractions. Main ouctome measure: Changes in retraction (progression or resolution, or development of a known sequela such as perforation). Results: Eight full text papers are included: three randomised controlled trials and five case series or cohort studies of more than five patients (a total of 238 ears). Data exists for the use of conservative management, ventilation tubes, laser tympanoplasty, cartilage and fascia tympanoplasty, lateral attic reconstruction as well as mastoid procedures. Conclusion: Few high-quality studies on the management of isolated and combined pars flaccida retractions exist. For isolated pars flaccida retractions deemed to require surgical intervention, this review suggests that lateral attic reconstruction and cartilage tympanoplasty carries least risk of recurrence.

Abstract:I would like to thank the authors for the letter to my paper 'Another Evidence For Pressure Transfer Mechanism In Incomplete Partition Two Anomaly Via Enlarged Vestibular Aqueduct'. They indicated that the theory of pressure transfer via enlarged vestibular aqueduct (EVA) was mentioned in the past, and according to the present-day literature, genetics is accepted as the cause of hearing loss, rather than mechanical pressure transfer in EVA and incomplete partition type II (IP-II) cases. They concluded that it is proven that EVA is not a cause for cochlear anomalies.

Abstract:This article aims to analyze the scientific literature on hearing loss (HL) in the field of Otorhinolaryngology published from the past to the present. A comprehensive dataset comprising 8013 articles on HL, spanning from 1980 to 2023, was retrieved from the Web of Science database and analyzed using various statistical and bibliometric methods. Objectives This article aims to analyze the scientific literature on hearing loss (HL) in the field of Otorhinolaryngology published from the past to the present. Methods A comprehensive dataset comprising 8013 articles on HL, spanning from 1980 to 2023, was retrieved from the Web of Science database and analyzed using various statistical and bibliometric methods. Results The leading countries in productivity were the United States, Japan, and Germany. The top productive journals included the International Journal of Pediatric Otorhinolaryngology (n = 652), Otology & Neurotology (579), and Acta Oto‐Laryngologica (512). The journals with the highest h‐index on hearing loss were Hearing Research (h = 65), Ear and Hearing (60), and Laryngoscope (55). Factor analysis revealed that the HL literature consisted of four constructs: main topics, speech/sound, genetics, and pharmaceutical therapies. Sudden sensorineural HL, sensorineural HL, cochlear implant, unilateral, noise‐induced and conductive HL, pediatric and age‐related HL, tinnitus, auditory brainstem response, audiometry, and prognoses for hearing health were the most researched topics from past to present. In recent years, the trending topics are genomic analysis, gene mutations, whole‐exome sequencing, genetic sequencing technique, the use of machine learning in HL, and the effect of COVID‐19 on hearing health. Conclusion The bibliometric analysis results clearly indicate significant progress in the field of hearing loss over the past 40 years. The 8013 articles published between 1980 and 2023 encompass a wide range of research in this area. These analyses can assist in identifying priority areas and focal points for future research. Perioperative and postoperative management of tympanostomy tube insertion: a survey of otorhinolaryngologists in Israel

Abstract:Background: Tympanostomy tube insertion is a standard surgical procedure in children to address middle ear infections and effusion-related hearing and speech development issues. Perioperative treatments like ear drops containing antibiotics, steroids, and tube irrigation with saline aim to prevent complications, yet no universal gold standard treatment exists. Despite guidelines, practice preferences among ENT specialists vary, motivating this study to investigate perioperative management practices in Israel. Method: A survey was distributed among ENT surgeons, collecting data on their main workplace, sub-specialty, preoperative hearing test requirements, tube irrigation practices, tube selection criteria, and timing of tube removal. Distribution and association with main workplaces were examined. Results: The survey achieved a response rate of 27.33%. Most participants routinely required preoperative hearing tests, with a preference for conducting them within three months prior to surgery (62.2%). Tube irrigation during the procedure was less common among surgeons in the public system (p = 0.007). In response to the COVID-19 pandemic, the majority of respondents maintained their established practices (96.3%), while a small proportion (3.7%) adapted by replacing two in-person meetings with one virtual session. Variations in tube removal timing based on the main workplace were noted, with private practitioners opting for earlier removal (p = 0.002) and were less permissive in water deprivation practices (p = 0.053). Conclusion: This study provides insights into the practices and preferences of ENT surgeons in tympanostomy tube insertion procedures in Israel. Adherence to standardized practices was observed, with variations influenced by the primary workplace. Despite the COVID-19 pandemic, minimal changes were made to established practices. Further research and consensus are necessary to optimize patient outcomes and develop tailored guidelines in this field.

Abstract:Objective To summarise the published research evidence on the epidemiology of otitis media, including the risk factors and sequelae associated with this condition. Data sources Medline (PubMed), Embase, and the Cochrane Library covering the period from 2019 to June 1st , 2023. Review methods We conducted a broad search strategy using otitis [Medical Subject Heading] combined with text words to identify relevant articles on the prevalence, incidence, risk factors, complications, and sequelae for acute otitis media, otitis media with effusion, and chronic suppurative otitis media. At least one review author independently screened titles and abstracts of the retrieved records for each condition to determine whether the research study was eligible for inclusion. Any discrepancies were resolved by reviewing the full text followed by discussion with a second review author. Studies with more than 100 participants were prioritised. Results Over 2,000 papers on otitis media (OM) have been published since 2019. Our review has highlighted around 100 of these publications. While the amount of otitis media research on the Medline database published each year has not increased, there has been an increase in epidemiological studies using routinely collected data and systematic review methodology. Most of the large incidence studies have addressed acute otitis media (AOM) in children. Several studies have described a decrease in incidence of AOM after the introduction of conjugate PCV vaccines. Similarly, a decrease was noted when rates of coronavirus disease of 2019 (COVID-19) were high and there were major public health efforts to reduce the spread of infection. There have been new studies on OM in adults and OM prevalence in a broader range of countries and population subgroups. Conclusion Overall, the rates of severe and/or suppurative OM appeared to be decreasing. However, there is substantial heterogeneity between populations. While better use of available data is informative, it can be difficult to predict rates of severe disease without accurate examination findings. Most memorably, the COVID-19 pandemic had an enormous impact on the research and clinical services for otitis media for most of the period under review. Implications for practice The use of routinely collected data for epidemiological studies will lead to greater variability in the definitions and diagnostic criteria used. The impact of new vaccines will continue to be important. Some of the lessons learned during the COVID-19 pandemic concerning behaviours that reduce spread of respiratory viruses can hopefully be used to decrease the burden of otitis media in the future. There are still many countries in the world where the burden of otitis media is not well described. In countries where otitis media has been studied over many years, new potential risk factors continue to be identified. In addition, a better understanding of the disease in specific subgroups has been achieved.

Abstract:Objective We aimed to summarize key articles published between 2011 and 2015 on the treatment of (recurrent) acute otitis media, otitis media with effusion, tympanostomy tube otorrhea, chronic suppurative otitis media and complications of otitis media, and their implications for clinical practice. Data Sources PubMed, Ovid Medline, the Cochrane Library, and Clinical Evidence (BMJ Publishing). Review Methods All types of articles related to otitis media treatment and complications between June 2011 and March 2015 were identified. A total of 1122 potential related articles were reviewed by the panel members; 118 relevant articles were ultimately included in this summary. Conclusions Recent literature and guidelines emphasize accurate diagnosis of acute otitis media and optimal management of ear pain. Watchful waiting is optional in mild to moderate acute otitis media; antibiotics do shorten symptoms and duration of middle ear effusion. The additive benefit of adenoidectomy to tympanostomy tubes in recurrent acute otitis media and otitis media with effusion is controversial and age dependent. Topical antibiotic is the treatment of choice in acute tube otorrhea. Symptomatic hearing loss due to persistent otitis media with effusion is best treated with tympanostomy tubes. Novel molecular and biomaterial treatments as adjuvants to surgical closure of eardrum perforations seem promising. There is insufficient evidence to support the use of complementary and alternative treatments. Implications for Practice Emphasis on accurate diagnosis of otitis media, in its various forms, is important to reduce overdiagnosis, overtreatment, and antibiotic resistance. Children at risk for otitis media and its complications deserve special attention.

J Strength Cond Res 30(11): 3140–3145, 2016—The current study examined the association between individual internal training load (individualized training impulse, iTRIMP) and aerobic fitness variables in male hurling players. Twenty hurling players (age, 25.5 ± 3.2 years; height, 178.9 ± 3.2 cm; body mass, 78.5 ± 4.5 kg) performed treadmill testing for V[Combining Dot Above]O2max, running economy (RE), and the speed at blood lactate concentrations of 2 mmol·L−1 (S2) and 4 mmol·L−1 (S4) on separate occasions before and after an 8-week training period. The Yo-Yo intermittent recovery (Yo-YoIR1, Yo-YoIR2) test performance were also assessed before and after the training period. Individualized training impulse was calculated using the blood lactate and heart rate profile of each individual player and was further assessed for each training session across the intervention period (n = 990). The results showed that iTRIMP had large to very large association with the percentage improvements in V[Combining Dot Above]O2max (r = 0.77; p = 0.002; 95% confidence interval [CI], 0.38–0.93, very large), RE (r = 0.78; p = 0.002; 95% CI, 0.40–0.93; very large), S2 (r = 0.64; p = 0.004; 95% CI, from 0.25 to 0.85; large), S4 (r = 0.78; p = 0.003; 95% CI, 0.45–0.85; very large), Yo-YoIR1 (r = 0.69; p = 0.003; 95% CI, 0.45–0.92; large), and Yo-YoIR2 (r = 0.60; p = 0.005; 95% CI, 0.45–0.92; large) performance. The study shows that iTRIMP is a means of quantifying training load in team sports and can be used to prescribe training for the maintenance or improvement of aerobic fitness during the competitive season with strong relationships seen between weekly iTRIMP measures and improvements in aerobic fitness measures.

Abstract:Surveys of children aged 5--17 in a predominantly rural area showed a high prevalence of inflamed ears and perforated drums in younger subjects and of perforated and scarred drums in older subjects. Abnormal drums were associated wtih lower socioeconomic status, ethnicity and low altitude and low rainfall. Maori children had both higher incidence and lower recovery rates than non-Maori children. The wider use of trained nurses for case detection and follow up is advocated, at an estimated cost of 53 cents per child examination. Further parent education and co-ordination of nurse/audiometrist surveys are necessary.
Tympanoplasty in the Setting of Complex Middle Ear Pathology: A Systematic Review

Abstract:Objective: To assess the prognostic factors for anatomic and hearing success after tympanoplasty in the setting of complex middle ear pathology. Methods: A systematic review was performed in January 2022. English-language articles describing outcome data for tympanoplasty repair variables including underlying pathology, perforation location, smoking status, graft technique, reconstruction material, anatomic success, and hearing success were extracted. Articles were included when tympanosclerosis, retraction pockets, adhesions, cholesteatoma, chronic suppurative otitis media, anterior perforations, and smoking were included. Underlying pathology, perforation location, smoking status, graft technique, reconstruction material, anatomic success, and hearing success were extracted. Any factors analyzed as potential indicators of success were sought out. Results: Data sources included PubMed, OVID, Cochrane, Web of Science, Scopus, and manual search of bibliographies. Ninety-three articles met final criteria, which accounted for 6685 patients. Fifty articles presented data on both anatomic and hearing outcomes, 32 articles presented data on anatomic outcomes only, and 11 articles presented data on hearing outcomes only. This systematic review found that adhesions and tympanosclerosis were prognostic factors for poorer hearing. Additionally, smoking and tympanosclerosis may be predictive of anatomic failure; however, the significance of this finding was mixed in included studies. This analysis is significantly limited by both the heterogeneity within the patients and the lack of controls. Conclusion: Adhesions and tympanosclerosis were prognostic factors for poorer hearing. Clearly documented methods and outcomes for the included pathologies could lead to more definitive conclusions regarding prognostic factors for success. Level of evidence: 3B.

Abstract:The assemblies “PoP” (Package-on-Package) can significantly increase the integration density of microelectronic circuits and systems, by vertically combining discrete semiconductor elements. Related to reliability especially migration phenomena, which are investigated here by simulation and measurements, become more important as the dimension of bumps decreases. To explore the influence of migration phenomena to the lifetime of PoPs a test card for current stress tests was designed. Furthermore, an exploration IMC growth was carried out. For a detailed interpretation of the test results FEM simulations with ANSYS® were performed. Finally, the results were used to predict the lifetime of the daisy chains and to calculate the activation energy for the IMC formation.

Abstract:By using retrieved LAI from remotely-sensed imagery, this paper studied the regional winter wheat yield estimation in Huanghuaihai Plain of North China. In order to improve the quality of remotely sensed data for winter wheat yield estimation, a Savitzky-Golay filter was used to smooth the MODIS-NDVI time series data to reduce the cloud contamination and remove the abnormal data. Then, a Gaussian model was used to simulate the daily crop LAI which was corrected by interpolating the measured LAI to get the average LAI values for each phenological stage. Using these LAI data, the relationships between LAI and crop yield at the main phenological stages of winter wheat was established. After optimizing the yield estimation model, the optimal time period and the best model parameters for winter wheat yield estimation in the study area were selected out. Finally, the established model was applied to estimate winter wheat yield based on the retrieved LAI from MODIS-NDVI, and the model accuracy was tested. Through the comparison of the predicted yield with the measured yield in the field, the mean relative error was 1.21%, and the RMSE was 257.33 kg x hm(-2).
