Abstract:Left anterior fascicular block (LAFB) is a heart disease identifiable from an abnormal electrocardiogram (ECG). It has been reported that LAFB is associated with an increased risk of heart failure. Non-specific intraventricular conduction delay due to the lesions of the conduction bundles and slow cell to cell conduction has also been considered as another cause of heart failure. Since the location and mechanism of conduction delay have notable variability between individual patients, we hypothesized that the impaired conduction in the ventricular myocardium may lead to abnormal ECGs similar to LAFB ECG patterns. To test this hypothesis, based on a computer model with a three dimensional whole-heart anatomical structure, we simulated the cardiac exciting sequence map and 12-lead ECG caused by the block in the left anterior fascicle and by the slowed conduction velocity in the ventricular myocardium. The simulation results showed that the typical LAFB ECG patterns can also be observed from cases with slowed conduction velocity in the ventricular myocardium. The main differences were the duration of QRS and wave amplitude. In conclusion, our simulations provide a promising starting point to further investigate the underlying mechanism of heart failure with LAFB, which would provide a potential reference for LAFB diagnosis. the angulation of impaction having little effect on facture risk.
Analysis of the Influence of the Electrical Asynchrony on Regional Mechanics of the Infarcted Left Ventricle Using Electromechanical Heart Models

Abstract:Asynchronous electrical activation, as induced by myocardial infarction, causes various abnormalities in left ventricle function. The influence of the electrical asynchrony on regional mechanics of the left ventricle is simulated using a mechanical heart model and an electrical heart model. The mechanical model accounts for the ventricular geometry, the fiber nature of the myocardial tissue, and the dependency of the activation sequence of the ventricular wall. The electrical model is based on a heart-torso model with realistic geometry, and different action potential waveforms with variables in duration are used to simulate the abnormal electrical activation after myocardial infarction. Regional deformation, strain and stress are calculated during systole phase. The preliminary results show that asynchronous electrical activation, as an important factor, significantly affects regional mechanical performance of the infarcted left ventricle, it indicates heterogeneous contraction pattern and elevated systolic stresses near the injured region. The simulated results are compared with solutions obtained in the literature. This simulation suggests that such coupled heart models can be used to assess the mechanical function of the left ventricle with diseases such as myocardial infarction, and more realistic models of cardiac function are essential for clinical evaluation of heart disease.
Discussion of Electrocardiographic Diagnostic Criteria for Acute Posteroseptal Myocardial Infarction

Abstract:Objective To approach the infarct sites in 4 patients with inferior wall AMI concomitant ST segment elevation in leads V_ 1-3 and V_ 3R-5R . These patients were diagnosed as inferior, right ventricular and anteroseptal walls AMI at admission. Methods Electrocardiographic data and results of isotope 99m Tc-MIBI myocardial perfusion imaging and coronary angiography were analyzed.Results Electrocardiogram showed ST segment significantly elevated in standard leads Ⅱ,Ⅲ , aVF, and leads V_ 1-3 and V_ 3R-5R , the magnitude of ST segment elevation was maximal in lead V_1. There was isotope 99m TC-MIBI myocardial perfusion defect in inferior and posteroseptal walls in these patients. Coronary angiography showed that right coronary artery was infarct-related artery. Conclusion According to data above, we put forward the diagnostic criteria for the posteroseptal wall AMI as followed: ① ST segment elevation ≥2mm in lead V_1 in the clinical setting of inferior wall AMI. ② The magnitude of ST segment elevation in leads V_ 1-3 and leads V_ 3R-5R are showed as V_1V_2V_3 and V_1V_ 3R V_ 4R .With two conditions above, inferior and posteroseptal walls AMI should be diagnosed.

Abstract:The presence of injured tissues after myocardial infarction (MI) creates substrates responsible for fatal arrhythmia; understanding of its arrhythmogenic mechanism requires investigation of the correlation of local abnormality between phenomenal electrical functions and inherent electrophysiological properties during normal sinus rhythm. This paper presents a physiological-model-constrained framework for imaging post-MI electrophysiological substrates from noninvasive body surface potential measurements. Using a priori knowledge of general cardiac electrical activity as constraints, it simultaneously reconstruct transmembrane potential dynamics and tissue excitability inside the 3D myocardium, with the central goal to localize and investigate the abnormality in these two different electrophysiological quantities. It is applied to four post-MI patients with quantitative validations by gold standards and notable improvements over existent results.
[Acute Insular Infarct Increases Risk of Electrocardiogram Changes and Sudden Cardiac Death].

Abstract:OBJECTIVE:To investigate the influence of location and size of acute insular infarct on stroke-related electrocardiogram (ECG) changes and cardiovascular events.METHODS:Ninety-nine cases admitted to hospital from October 2007 to June 2009, who were diagnosed as acute middle cerebral artery territory infarct within 48 h after onset and without the history of cardiac diseases, were included in the study. The patients were further divided into three groups: major insular infarct, minor insular infarct and control group, according to the infarct size on MRI diffusion-weighted image. The clinical data, ECG changes and cardiovascular events were compared between left and right insular infarct. Logistic regression was applied to determine the independent risk factors of ECG changes and cardiovascular events.RESULT:Large artery atherosclerosis was the main cause of acute insular infarct (71.8 %), which was associated with higher NIHSS score compared to the control group (P < 0.01). Comparing the left and right insular infarct, the frequencies of sinus bradycardia and sudden cardiac death were significantly higher in left insular infarct (P < 0.01 and P < 0.05), while there was a trend that the frequency of atrial fibrillation was higher in right insular infarct (P = 0.079). With the larger size of insular infarct, the frequency of sinus bradycardia, new atrial fibrillation and sudden cardiac death (P<0.01, P<0.05 and P<0.05, respectively) became much higher. Logistic regression analysis showed that major insular infarct was related to the higher frequency of sinus bradycardia (OR = 4.660, 95% CI: 1.646 ~ 13.195; P = 0.004).CONCLUSION:Acute insular infarct is associated with the stroke-related ECG changes and sudden cardiac death. Left insular infarct is related to sinus bradycardia, possibly due to the enhanced parasympathetic tone. It deserves clinical attention that the incidence of cardiac autonomic disturbance becomes higher with the enlarged insular infarct size.

Abstract:Less than half of patients with a chest pain history indicative of acute coronary syndrome have a diagnostic electrocardiogram (ECG) on initial presentation to the emergency department. The physician must dissect the ECG for elusive, but perilous, characteristics that are often missed by machine analysis. ST depression is interpreted and often suggestive of ischemia; however, when exclusive to leads V1-V3 with concomitant tall R waves and upright T waves, a posterior infarction should first and foremost be suspected. Likewise, diffuse ST depression with elevation in aVR should raise concern for left main- or triple-vessel disease and, as with the aforementioned, these ECG findings are grounds for acute reperfusion therapy. Even in isolation, certain electrocardiographic findings can suggest danger. Such is true of the lone T-wave inversion in aVL, known to precede an inferior myocardial infarction. Similarly, something as ordinary as an upright and tall T wave or a biphasic T wave can be the only marker of ischemia. ECG abnormalities, however subtle, should give pause and merit careful inspection since misinterpretation occurs in 20-40% of misdiagnosed myocardial infarctions.

Abstract:Background: Atrial coronary branch occlusion is a hardly recognizable clinical entity that can promote atrial fibrillation. The low diagnostic accuracy of the ECG could deal with the characteristics of the ischemia-induced changes in local atrial electrograms, but these have not been described.Objectives: We analyzed the effects of selective acute atrial branch occlusion on local myocardial structure, atrial electrograms, and surface ECG in an experimental model close to human cardiac anatomy and electrophysiology.Methods: Six anesthetized open-chest anesthetized pigs underwent surgical occlusion of an atrial coronary branch arising from the right coronary artery during 4 h. Atrial electrograms and ECG were simultaneously recorded. One additional pig acted as sham control. In all cases, the hearts were processed for anatomopathological analysis.Results: Atrial branch occlusion induced patchy atrial necrosis with sharp border zone. During the first 30 min of occlusion, atrial electrograms showed progressive R wave enlargement (1.8 ± 0.6 mV vs. 2.5 ± 1.1 mV, p < 0.01), delayed local activation times (28.5 ± 8.9 ms vs. 36.1 ± 16.4 ms, p < 0.01), ST segment elevation (−0.3 ± 0.3 mV vs. 1.0 ± 1.0 mV, p < 0.01), and presence of monophasic potentials. Atrial ST segment elevation decreased after 2 h of occlusion. The electrical border zone was ∼1 mm and expanded over time. After 2 h of occlusion, the ECG showed a decrease in P wave amplitude (from 0.09 ± 0.04 mV to 0.05 ± 0.04 mV after 165 min occlusion, p < 0.05) and duration (64.4 ± 8.0 ms vs. 80.9 ± 12.6 ms, p < 0.01).Conclusion: Selective atrial branch occlusion induces patchy atrial infarction and characteristic changes in atrial activation, R/S wave, and ST segment that are not discernible at the ECG. Only indirect changes in P wave amplitude and duration were appreciated in advanced stages of acute coronary occlusion.

Abstract:The electrocardiogram (ECG) findings in acute coronary syndrome should always be interpreted in the context of the clinical findings and symptoms of the patient, when these data are available. It is important to acknowledge the dynamic nature of ECG changes in acute coronary syndrome. The ECG pattern changes over time and may be different if recorded when the patient is symptomatic or after symptoms have resolved. Temporal changes are most striking in cases of ST-elevation myocardial infarction. With the emerging concept of acute reperfusion therapy, the concept ST-elevation/ non-ST elevation has replaced the traditional division into Q-wave/non-Q wave in the classification of acute coronary syndrome in the acute phase. Keypoints: In acute coronary syndrome, in addition to the traditional electrocardiographic risk markers, such as ST depression, the 12-lead ECG contains additional, important diagnostic and prognostic information. Clinical guidelines need to acknowledge certain high-risk ECG patterns to improve patient care.

Abstract:Objecfive To study the clinical characteristics of acute inferior myocardial infarction.Methods 100 patients with acute inferior infarction were divided into 2 groups according to coronary angiography:group A (n=76) with right coronary artery occlusion;group B(n=24) with left circumflex artery occlusion.Results The frequencies of Electrocardiogram(ECG) ST segment elevation ST Ⅲ>ST Ⅱ and ST segment depression STAVL>STI in group A was significantly higher than that in group B(P<0.05);The frequency of ECG ST segment elevation STⅢO.1 mV in group A was significantly higher than that in group B(P<0.05);The number of ECG with precor-dial lcads V1～6 ST segment depression >0.1 mv with lesion of left anterior descending coronary was more significant than that without precordial Vl-6 leads ST segment depression (P<0.05).Left ventricular ejection fraction (LVEF)(51%±14%) in group A was significant lower than that in group B(57%±10%)(P<0.05);The patients in group A with right ventricular infarction were more significant than those in group B(P<0.05);The total hospital mortality of patients with acute inferior myocardial infarction was 6%,and they were all in group A.But there was no significance between the two groups about cardiogenic shock,heart failure,Ⅱ degree and Ⅲ degree atrial ventricular b1ock,ventrieular tachycardia/ventrieular fibrillation and the hospital mortality.The mortality with cardiogenic shock was 83.3%.Conclusion The changes of ECG ST segment in Ⅲ,Ⅱ,Ⅰ,AVL and V4R leads can predict relatedinfarct coronary artery with acute inferior myocardial infarction.The patients with precordial Vl～6 leads ST segment depression indicates lesion of LAD.The LVEF of patients with RCA occlusion is lower than that with LCX occlusion. The primary cause of death is eardiogenic shock.

Abstract:Objective: To study the clinical characteristics of patients with acute inferior wall myocardial infarction spring from different occulded arteries. Method: patients with acute inferior wall myocardial infarction were divided into 2 groups according to coronary angiograms. Group A included 76 patients with right coronary artery occlusion and 24 patients with left circumflex artery occlusion in group B. Their clinical features were studied. Results: On the eectrocardiogram (ECG) the number of ST segment elevation STIII > STII in group A was significantly higher than that in group B (P < 0.05); The number of ST segment elevation STIII < STII in group A was significantly lower than that in group B (P < 0.001); The number of STV4R elevation > 0.1 mV in A group was significant higher than that in group B (P < 0.05); The number with lesion of left anterior descending coronary in patients with precordial leads ST segment depression > 0.1 mV was significantly higher than that without precordial leads ST segment depression (P < 0.05). Left ventricular ejection fraction (LVEF) of group A was significant less than that of group B [(51 ± 14)% vs (57 ± 10) %, P < 0.05]; The patients in group A with right ventricular infarction, LVEF was significantly greater than that in group B (P < 0.05); There was of no significant difference between the two groups about the incidence of cardiogenic shock, heart failure, II degree and III degree atrial ventricular block, ventricular tachycardia/ventricular fibrillation and the mortality in during hospital stay. Conclusions: The changes of ST segment of ECG in III, II and V4R leads can predict the related coronary artery occluded involving in acute inferior wall myocardial infarction. When patients take precordial leads, ST segment depression indicate lesion of LAD. The LVEF of patients with RCA occlusion is less than that with LCX occlusion. But there was of no significant difference between 2 groups in complications.

Abstract:OBJECTIVE:To explore the infarct sites in patients with inferior wall acute myocardial infarction (AMI) concomitant with ST segment elevation in leads V1-V3 and leads V3R-V5R.METHODS:Five patients diagnosed as inferior, right ventricular, and anteroseptal walls AMI at admission were enrolled. Electrocardiographic data and results of isotope 99mTc-methoxyisobutylisonitrile (MIBI) myocardial perfusion imaging and coronary angiography (CAG) were analyzed.RESULTS:Electrocardiogram showed that ST segment significantly elevated in standard leads II, III, aVF, and leads V1-V3, V3R-V5R in all five patients. The magnitude of ST segment elevation was maximal in lead V1 and decreased gradually from lead V1 to V3 and from lead V1 to V3R-V5R. There was isotope 99mTc-MIBI myocardial perfusion imaging defect in inferior and basal inferior-septal walls. CAG showed that right coronary artery was infarct-related artery.CONCLUSIONS:The diagnostic criteria for basal inferior-septal wall AMI can be formulated as follows: (1) ST segment elevates > or = 2 mm in lead V1 in the clinical setting of inferior wall AMI; (2) the magnitude of ST segment elevation is the tallest in lead V1 and decreases gradually from lead V1 to V3 and from lead V1 to V3R-V5R. With two conditions above, the basal inferior-septal wall AMI should be diagnosed.

Abstract:BACKGROUND Cardiac injury may occur after acute pathology of central nervous system（CNS）without any evidence of primary cardiac diseases.The resulting structural and/or functional changes are called cerebrocardiac syndrome（CCS）.The great majority of studies have been performed in patients with subarachnoid hemorrhage（SAH）, while CCS data after intracerebral hemorrhage（ICH） are rare.It may cause diagnostic and therapeutic pitfalls for the clinician due to a lack of specific clinical manifestations and diagnostic methods.Understanding the underlying pathophysiological and molecular mechanism（s） following cerebrovascular incidents will help to implement prevention and treatment strategies to improve the prognosis.CASE SUMMARY A 37-year-old man with a history of hypertension presented to our department on an emergency basis because of a sudden dizziness and left limb weakness.Cerebral computed tomography（CT） suggested ICH in the occipital and parietal lobes, and the chosen emergency treatment was hematoma evacuation.Left ventricular（LV） dysfunction occurred after the next 48 h and the electrocardiogram（ECG） showed non-ST elevation myocardial infarction.CCS was suspected first in the context of ICH due to the negative result of the coronary CT angiogram.CONCLUSION Misinterpretation of ischemic-like ECGs may lead to unnecessary or hazardous interventions and cause undue delay of rehabilitation after stroke.Our objective is to highlight the clinical implications of CCS and we hope the differential diagnoses will be considered in patients with acute CNS diseases.

Abstract:Background : Immune checkpoint inhibitors (ICIs) are associated with immune-related adverse events including myocarditis, whilst improving cancer-related outcomes. There is thus a clinical need to identify electrocardiographic manifestations of ICI-related myocarditis to guide clinical management. Methods : PubMed was searched for clinical studies and case reports describing electrocardiographic changes in patients with ICI-related myocarditis. Results : A total of six clinical studies and 79 case reports were included. This revealed a range of presentations for patients on ICIs, including supraventricular arrhythmias, ventricular arrhythmias and heart block, and new changes of ST-T segment unrelated to coronary artery disease, ST-segment elevation or depression and T-wave abnormalities. Several patients showed low voltages in multiple leads and new onset Q-wave development. Conclusion : Patients with ICI-related myocarditis may develop new arrhythmia and ST-T changes, and infrequently low voltages in multiple leads.

Abstract:Purpose The role of electrocardiogram (ECG) in the diagnosis of acute myocardial infarction (AMI) is very important. As an emergency technique, electrocardiogram is easy to operate, although, to diagnose acute myocardial infarction (AMI) by electrocardiogram become difficult due to the irregular change of electrocardiogram in the patients with AMI. Diagnostic errors, missed diagnosis even delay therapy appeared. To assess the irregular change of electrocardiogram in the patients with AMI should be necessary in diagnosis and therapy. Methods The retrospective study enrolled 10 subjects with AMI who were hospitalisation in Cangzhou Central Hospital from May 2009 to May 2011. The changes of ECG were irregular in all of them. ECG, enzymes (due to myocardiolysis) and echocardiography were detected, percutaneous coronary intervention (PCI) was given also. Results 10 patients with AMI included four male, six female, aged 32–72 (57±4.16); 1±0.18 h were continuing from chest pain seizures to hospitalisation; No-dynamic change in ECGs in acute phase; Troponin was increased in two cases; serum creatine kinase and troponin I both increased in eight cases; completely morbid change of leftcircumflex in six cases and capital morbid change of leftcircumflex in four cases were detected by coronary angiography. Conclusions The atypical change of electrocardiogram in patients with acute myocardial infarction mostly predicts that infarct related artery is leftcircumflex.

Abstract:Early repolarization(ER) is a common electrocardiographic(ECG) variant,defined as a ≥0.1 mV elevation of the QRS-ST junction manifesting as J-point slurring or notching in at least 2 contiguous leads.It is a long-held belief that ER is a benign ECG variant with favorable prognosis.However,recent studies have highlighted that ER is potentially associated with idiopathic ventricular fibrillation or sudden cardiac death.This article is a review of progress made in defining and reconciling the ECG enigma in the clinic.

Methods Optical mapping was performed in porcine right ventricles with local: 1) repolarization shortening; 2) conduction slowing; or 3) structural heterogeneity induced by locally perfusing: 1) pinacidil (20 μmol/L, n = 13); or 2) flecainide (2 μmol/L, n = 13) via an epicardial catheter; or 3) by local epicardial tissue destruction (9 radiofrequency lesions n = 12). Electrograms were recorded (n = 5 in each group) and spontaneous and induced arrhythmias were quantified and optically mapped. Results Electrograms were normal in (1) but showed local fragmentation in 40% of preparations in (2) with greater effects observed at high pacing frequencies dependent on the wavefront direction. In (3), the structural substrate alone increased the width and number of peaks in the electrograms, and addition of flecainide induced pronounced fragmentation (≥3 peaks and ≥70 ms) in all cases. Occurrence of spontaneous arrhythmias was significantly increased in (1) and (2) ( P < 0.0001 and 0.05, respectively, vs baseline) and were triggered by ectopies. Vulnerability to arrhythmias at high pacing frequencies (≥2 Hz) was the lowest in (1) and greatest in (2). Conclusions Microstructural substrates have the most pronounced impact on electrograms, especially when combined with sodium channel blockers, whereas local action potential duration shortening does not lead to electrogram fragmentation even though it is associated with the highest prevalence of spontaneous arrhythmias.

Alterations of medial prefrontal cortex bioelectrical activity in experimental model of isoprenaline-induced myocardial infarction

Abstract:Clinical and animal studies have found that anxiety and depression are significantly more common after acute myocardial infarction (AMI). The medial prefrontal cortex (PFC) has a dual role: in higher brain functions and in cardiovascular control, making it a logical candidate for explaining the perceived bidirectional heart-brain connection. We used parallel Electrocardiography (ECG) and Electrocorticography (ECoG) registration to investigate AMI-induced changes in medial PFC bioelectrical activity in a rat model of AMI.Adult male Wistar albino rats were used in the study. Gold-plated recording electrodes were implanted over the frontal cortex for ECoG recording. ECG was recorded via two holter electrodes attached on the skin of the back fixed in place by a jacket. Induction of AMI was performed by isoprenaline (150 mg/kg, i.p.). ECoG and ECG signals were registered at baseline, during 3 hours after isoprenaline administration and at 24 hours after isoprenaline administration.Significant increases of theta, alpha, and beta electroencephalographic (EEG) band power were observed in different time intervals after isoprenaline administration. Significant increase of theta band peak frequency was also observed during the first hour after isoprenaline administration. No statistically significant differences in band-power activity were found between the pre-isoprenaline measurements and 24 hours after administration.Our results demonstrate significant increases in EEG band power of alpha beta and theta bands during isoprenaline-induced AMI model. These are the first findings to connect heart damage during isoprenaline- induced AMI to disturbances in the cortical bioelectrical activity.

Abstract:Significance Cardiac arrhythmias often occur in heart failure (HF) patients, but drug therapies using selective ion channel blockers have failed clinical trials and effective drug therapies remain elusive. Here we systematically study the major ionic currents during the cardiac action potential (AP) and arrhythmogenic Ca 2+ release in postinfarction HF. We found that changes in any individual current are relatively small, and alone could mislead as to consequences. However, differential changes in multiple currents integrate to shorten AP in the infarct border zone but prolong AP in the remote zone, increasing AP repolarization inhomogeneity. Our findings help explain why single channel-blocker therapy may fail, and highlight the need to understand the integrated changes of ionic currents in treating arrhythmias in HF.

Abstract:R‐wave amplitude <0.3 mV in lead V1 concomitant with ST‐segment elevation in leads V7–V9 predicts a larger infarct size and longer ventricular activation time in patients with inferior myocardial infarction. Objective To assess electrocardiogram (ECG) for risk stratification in inferior ST‐elevation myocardial infarction (STEMI) patients within 24 h.
