Abstract:OBJECTIVE To explore the effects of intervertebral disc degeneration on the biomechanical behavior of the lumbar motion segment. METHODS A three-dimensional nonlinear finite element model of L4-L5 segment was established using CAD technique based on CT images. A normal disc model and three degenerative disc models were established by changing the parameters such as disc material properties and disc height. The effects of disc degeneration on the biomechanical properties, including stiffness, nucleus pressure, maximum von Mises stress in the annulus, and force of posterior structure were studied under two moment loads (flexion and extension) and for three different direct forces (compression, and anterior and posterior shear forces), and the stress distribution of vertebral body and end- plate was also analyzed. RESULTS The stiffness of the lightly degenerative disc model was decreased compared with that of the normal disc, while it was increased in the moderately and severely degenerative disc models when compared with the normal disc. The force of posterior structure in the lightly degenerative disc model was increased while the values of force of posterior structure in the moderately and severely degenerative disc models were decreased gradually. The maximum von Mises stress in the annulus increased, and the nucleus pressure decreased as the disc degeneration progressed. Moreover, the stress of intervertebral disc, vertebral body, and end-plate was distributed more peripherally. CONCLUSION Light degeneration of intervertebral disc leads to instability of lumbar spine, while the stability restores with further degeneration of disc. There is a negative correlation between the force of posterior structure and the load on the intervertebral disc. With the disc degeneration progressing, the intervertebral disc load pattern changes, the stress of intervertebral disc, vertebral body, and end-plate is concentrated peripherally.
The Change of Shape and Curvature in Degenerative Lumbar Disc and Its Clinical Significance

Abstract:Objective: To examine the influence of lumber disk degeneration on vertebral disc shape and lumbar lordosis. Methods: 121 cases were adopted and divided into three groups: control group of 33 asymptomatic volunteers, slight degeneration group of 53 cases and severe degeneration group of 35 cases. For all cases, sagittal magnetic resonance imaging of lumbar spine were inputted into personal computer. The anterior, middle and posterior heights of L4/5 intervertebral space, as well as the signal, the angle of the L4/5 disc and lumbar lordosis were measured. Results: The height of intervertebral space and the angle of L4/5 disc and lumbar lordosis gradually decreased among the control group, slight degeneration group and severe degeneration group (P 0.05). The angle of L4/5 disc had positive correlation with lumbar lordosis (P 0.05). Conclusions: For patients with lumbar disc degeneration, changes of the disc shape and lumbar lordosis present. To investigate those changes is very valuable for the therapy of lumbar disc degeneration.
Contribution of Sagittal Orientation of Facet Joints and Disc Degeneration to Degenerative Spondylolisthesis in Lumbar Spine: A Finite Element Research

Abstract:Objective To research the connection between the sagittal orientation of facet joints and disc degeneration in lumbar spine and to investigate the contribution of which to development the degenerative spondylolisthesis. Methods A new effective CAD method was used to accurately establish 9 finite element models(FEM)which were assembled respectively with facet-joint angle 65°, 45°, 25°, with normal disc, light degenerative disc and severe degenerative disc. The biomechanical parameters of 9 finite element models were measured under axial compressive load within physiological range. Results Compared with FEMs with facet-joint angle 45° and 25°, anterior displacement of L4 vertebra in FEMs with facet-joint angle 65° was increased, where the maximum von Mises stresses on facet surface, isthmus and the contact force on facet surface in horizontal orientation were obviously increased. Meanwhile, FEMs with facet-joint angle 65° showed a decrease in end-plate bulge and an increase in stress of annular matrix. The stiffness in light degenerative disc FEMs was reduced and the von Mises stresses on facet surface and isthmus was slightly increased compared with the normal disc FEMs. Among all, the FEM with facet-joint angle 65° combined with light degenerative disc showed the poorest resisting anterior shear force. Conclusion Sagittal orientation of facet joints is not only the primary inducement of the degenerative spondylolisthesis but the secondary pathological change of remodeling of the facet-joints induced by the regional stress change. The inherent instability of lumbar motion segment of sagittal orientation of facet joints is influenced by the lumbar disc degeneration. The lumbar disc degeneration has no manifested contribution to the aggravation of the sagittal orientation of facet joints.
Influence of the Morphology of Lumbar Motion Segment on Isthmic Stress and Facet Contact Force: A Finite Element Analysisangzh

Abstract:Objective To identify the potential relation of geometric characteristics of the lumbar spine to the isthmic spondylolysis and degenerative joints under loading.Methods A series of new effective computer aided design(CAD) methods such as modified "no-seed region segmentation" and non-parallel "best cross-section planes" were used to accurately establish finite element(FE) models of the lumbar L4～L5 segment,representing the variation of disc height,facet-joint angle and lordosis angle of the intervertebral disc.The isthmic stress,facet contact force and load transmission through disc were investigated under axial compression 2700N.Results The isthmic stress and facet contact force of the FE models increased with the increasing of the disc height and facetjoint angle,while decreased with the increase of the lordosis angle of the intervertebral disc.Conclusion The variation of the disc height,facet-joint angle and the lordosis angle of the intervertebral disc had a manifest influence on the isthmic stress and facet contact force.It can be presumed that the occurrence of the lumbar spondylolysis and degenerative joints in athletes was closely correlated to the geometric characteristics of the lumbar spine.

Abstract:Abstract Background To compare potential of ultrashort time-to-echo (UTE) T2* mapping and T2* values from T2*-weighted imaging for assessing lumbar intervertebral disc degeneration (IVDD),with Pfirrmann grading as a reference standard. Methods UTE-T2* and T2* values of 366 lumbar discs (L1/2-L5/S1) in 76 subjects were measured in 3 segmented regions: anterior annulus fibrosus, nucleus pulposus (NP), and posterior annulus fibrosus. Lumbar intervertebral discs were divided into 3 categories based on 5-level Pfirrmann grading: normal (Pfirrmann grade I),early disc degeneration (Pfirrmann grades II-III), and advanced disc degeneration (Pfirrmann grades IV-V). Regional differences between UTE-T2* and T2* relaxometry and correlation with degeneration were statistically analyzed. Results UTE-T2* and T2*value correlated negatively with Pfirrmann grades (P < 0.001). In NP, correlations with Pfirrmann grade were high with UTE-T2* values (r = − 0.733; P < 0.001) and moderate with T2* values (r = -0.654; P < 0.001). Diagnostic accuracy of detecting early IVDD was better with UTE-T2* mapping than T2* mapping (P < 0.05),with receiver operating characteristic analysis area under the curve of 0.715–0.876. Conclusions UTE-T2* relaxometry provides another promising magnetic resonance imaging sequence for quantitatively evaluate lumbar IVDD and was more accurate than T2*mapping in the earlier stage degenerative process.
Noninvasive Assessment of Biochemical and Mechanical Properties of Lumbar Discs Through Quantitative Magnetic Resonance Imaging in Asymptomatic Volunteers

Abstract:Intervertebral disc degeneration is a prevalent phenomenon associated with back pain. It is of critical clinical interest to discriminate disc health and identify early stages of degeneration. Traditional clinical T2-weighted magnetic resonance imaging (MRI), assessed using the Pfirrmann classification system, is subjective and fails to adequately capture initial degenerative changes. Emerging quantitative MRI techniques offer a solution. Specifically, T2* mapping images water mobility in the macromolecular network, and our preliminary ex vivo work shows high predictability of the disc's glycosaminoglycan content (s-GAG) and residual mechanics. The present study expands upon this work to predict the biochemical and biomechanical properties in vivo and assess their relationship with both age and Pfirrmann grade. Eleven asymptomatic subjects (range: 18-62 yrs) were enrolled and imaged using a 3T MRI scanner. T2-weighted images (Pfirrmann grade) and quantitative T2* maps (predict s-GAG and residual stress) were acquired. Surface maps based on the distribution of these properties were generated and integrated to quantify the surface volume. Correlational analyses were conducted to establish the relationship between each metric of disc health derived from the quantitative T2* maps with both age and Pfirrmann grade, where an inverse trend was observed. Furthermore, the nucleus pulposus (NP) signal in conjunction with volumetric surface maps provided the ability to discern differences during initial stages of disc degeneration. This study highlights the ability of T2* mapping to noninvasively assess the s-GAG content, residual stress, and distributions throughout the entire disc, which may provide a powerful diagnostic tool for disc health assessment.
Multimodal Quantitative Magnetic Resonance Imaging for Lumbar Intervertebral Disc Degeneration

Abstract:The present study investigated the application of the T1ρ and T2 relaxation mapping magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) in the evaluation of intervertebral disc degeneration (IDD). A total of 93 asymptomatic subjects were imaged with T1ρ and T2 mapping, as well as DWI. Pfirrmann grading was performed and correlation analysis was conducted for T1ρ, T2 and DWI results with the grading results and age. Pfirrmann grading indicated 69 cases of grade I, 240 cases of grade II, 101 cases of grade III, 43 cases of grade IV and 12 cases of grade V. MRI showed that the T1ρ values of the nucleus pulposi at L4/5 and L5/S1 were significantly reduced (P<0.05) and no significant differences were observed in the T2 values compared with the values of the nucleus pulposus at L1/2, L2/3 and L3/4. The apparent diffusion coefficient (ADC) values of L1/2 were significantly decreased from L2/3 and L3/4 (P<0.05). Correlation analysis revealed that the T1ρ, T, and ADC values were positively correlated with each other. Moreover, the T1ρvalues were significantly decreased with the increase of Pfirrmann grades (P<0.05), with the exception of grades IV and V. However, T2 and ADC values were not significantly different between grades I and II or IV and V. In addition, the T1ρ, T2 and ADC values were significantly decreased with the increase of age in patients with IDD (P<0.05). T1ρ and T2 mapping and DWI are promising techniques for the in vivo diagnosis of IDD, which may be useful in determining the appropriate prevention and treatment options for the disease.
The relationship between degeneration and instability of lumbar spine: study of imaging

Abstract:Objective: To discuss the relationship along disc degeneration, fecet joint osteoarthritis and the instability of the degenerative lumbar spine. Methods: 120 patients suffering from lower back pain were analyzed adopting both MRI and X-rays of flexion and extension radiographs for the lumbar spine. The instability of lumbar spine was classified into abnormal tilting on flexion, rotatory instability in the sagittal plane, and translatory instability. Disc degeneration was classified into five grades according to T2-weighted sagittal images. Facet joint osteoarthritis as seen on axial T1-weighted images was divided into four grades. Results: On three intervertebral level, the grade of disc degeneration increased significantly (P<0.01) with age. Lumbar instability could be identified within 93 segments(26.3%) and correlated with grade 1 and grade 2 degeneration of the intervertebral discs(P<0.01). The kinematics of the lumbar motion segment were correlated with facet joint osteoarthritis(r=0.937,P<0.05). Anterior translatory instability was positively associated with disc degeneration and facet joint osteoarthritis(P<0.01). Conclusions: There is a positive relationship along the type of instability, disc degeneration and facet joint osteoarthritis.
Quantitative T2 Magnetic Resonance Imaging Compared to Morphological Grading of the Early Cervical Intervertebral Disc Degeneration: An Evaluation Approach in Asymptomatic Young Adults

Abstract:OBJECTIVE: The objective of this study was to evaluate the efficacy of quantitative T2 magnetic resonance imaging (MRI) for quantifying early cervical intervertebral disc (IVD) degeneration in asymptomatic young adults by correlating the T2 value with Pfirrmann grade, sex, and anatomic level.METHODS: Seventy asymptomatic young subjects (34 men and 36 women; mean age, 22.80±2.11 yr; range, 18-25 years) underwent 3.0-T MRI to obtain morphological data (one T1-fast spin echo (FSE) and three-plane T2-FSE, used to assign a Pfirrmann grade (I-V)) and for T2 mapping (multi-echo spin echo). T2 values in the nucleus pulposus (NP, n = 350) and anulus fibrosus (AF, n = 700) were obtained. Differences in T2 values between sexes and anatomic level were evaluated, and linear correlation analysis of T2 values versus degenerative grade was conducted.FINDINGS: Cervical IVDs of healthy young adults were commonly determined to be at Pfirrmann grades I and II. T2 values of NPs were significantly higher than those of AF at all anatomic levels (P<0.000). The NP, anterior AF and posterior AF values did not differ significantly between genders at the same anatomic level (P>0.05). T2 values decreased linearly with degenerative grade. Linear correlation analysis revealed a strong negative association between the Pfirrmann grade and the T2 values of the NP (P = 0.000) but not the T2 values of the AF (P = 0.854). However, non-degenerated discs (Pfirrmann grades I and II) showed a wide range of T2 relaxation time. T2 values according to disc degeneration level classification were as follows: grade I (>62.03 ms), grade II (54.60-62.03 ms), grade III (<54.60 ms).CONCLUSIONS: T2 quantitation provides a more sensitive and robust approach for detecting and characterizing the early stage of cervical IVD degeneration and to create a reliable quantitative in healthy young adults.

Diurnal Variation in Hydration of the Cervical Intervertebral Disc Assessed Using T2 Mapping of Magnetic Resonance Imaging

Abstract:OBJECTIVE: The study aimed to investigate the diurnal variation in cervical disc hydration and its relationship with cervical degeneration.MATERIALS AND METHODS: C3-C7 discs of 86 prospectively enrolled participants (37 males, 49 females; mean age ± standard deviation, 23.5 ± 2.5 years) were assessed using T2 mapping in the morning and evening. All discs were stratified by Miyazaki grade or C2-C7 Cobb angle and T2 values (T2). The degree of diurnal T2 variation (T2-DDV), defined as (morning T2 - evening T2)/morning T2 × 100%, was measured for the entire disc, annulus fibrosus (AF), nucleus pulposus (NP), and endplate zones.RESULTS: T2 of the entire disc decreased significantly after the daytime load (p < 0.001), with a T2-DDV of 13.3% for all discs and 16.0%, 12.2%, and 13.0% for healthy (grade I), mild degenerative (grade II), and advanced degenerative (grade III/IV) discs, respectively. T2 of regional NPs and AFs decreased significantly from morning to evening (p ≤ 0.049) except in the healthy anterior inner AF (p = 0.092). Compared with healthy discs, mild degenerative discs displayed lower T2 and T2-DDV in regional NPs (p < 0.001). Advanced degenerative discs showed higher T2-DDV in the anterior inner AF compared with healthy discs (p = 0.050). Significant diurnal T2 changes in the endplate zones were observed only in healthy discs (p = 0.013). Cervical discs in the low Cobb angle group showed higher T2-DDV in the anterior AFs and anterior NP and lower T2-DDV in the posterior AF than those in the high Cobb angle group (p ≤ 0.041).CONCLUSION: This study characterized the diurnal variation in hydration of the cervical discs as assessed using T2 mapping and revealed early chemo-mechanical coupling dysfunction in degenerating discs. Cervical sagittal alignment on MRI can affect the diurnal stress patterns of the cervical discs. T2 mapping is sensitive to disc biomechanical dysfunction and offers translational potential from biomechanical research to clinical application.

The biomechanical influence of facet joint parameters on corresponding segment in the lumbar spine: a new visualization method

Abstract:Background context: Facet joints have been discussed as influential factors in the development of lumbar degeneration, which includes disc herniation and degenerative lumbar spondylolisthesis. Facet orientation (FO) and facet tropism (FT) are two important structural parameters of the lumbar facet joints. Many previous studies have focused on single parameter analysis of the lumbar spine. Owing to the correlation between independent variables, single-factor analysis cannot reflect the interaction between variables; however, there has been no corresponding biomechanical method developed to address this problem. Purpose: To investigate the complex biomechanical influences on the lumbar spine when vertebral FO and FT are varied using finite element analysis (FEA) and contour maps visualization, and analyze the biomechanical role of facet joint structural parameters in the process of lumbar degenerative diseases. Study design: A biomechanical modelling, analysis, and verification study was performed. Methods: A three-dimensional non-linear FEA model of 3 denucleated intervertebral discs (L2-3, L3-4, L4-5) with adjacent vertebral bodies (L2-L5) was created. Previously performed in vitro experiments provided experimental data for the range of motion in each load direction that was used for calibration. For 12 lumbar models, different facet joint angles relative to the sagittal plane at both L3-4 facet joints were simulated for 35°≤FO≤50° and 0°≤FT≤15°. By modifying different values of FO and FT, FEA simulation of different lumbar spine models was performed. Contour maps were used to visualize the FO- and FT-relevant data. Results: Under flexion, extension, and torsion moments, facet joint contact force and intradiscal stress increased with increasing FT. In the condition where FT remained 0° and increasing FO values, facet joint contact force and intradiscal stress remained low with no apparent increasing or decreasing trend when the model was under flexion, extension, and torsion moments. In the condition where FO and the FT values were varied at the same time, the highest force and stress regions in the contour maps were observed when all three types of moments were applied. Stress distributions of the L3-4 disc with different FT and FO values showed disc stress increased significantly with increases of FT and was concentrated on the ipsilateral region of the facet joint with the more sagittal orientation. Conclusions: The combination of FO and FT has an important impact on the corresponding disc and facet joints, but FT played a more significant role. Moreover, disc stress was concentrated on the ipsilateral region of facet joint with greater sagittal orientation when FT existed. FT with high sagittal orientation may increase risk of recurrent LDH due to increase ipsilateral disc pressure. Clinical significance: These biomechanical findings may help clinicians to understand the prognosis of some lumbar degenerative conditions.

Abstract:Abstract Background Degeneration of the intervertebral discs are very common diseases, indicating the specific or malignant changes in intervertebral disc component, structure and function. Imaging examination is currently used to evaluate the severity of lumbar intervertebral disc degeneration. This study was designed to investigate the diagnostic value of 3D LAVA-Flex in lumbar intervertebral disc degeneration. Material and methods Sagittal 3D LAVA-Flex and T2WI scans were performed in 45 patients with lumbar intervertebral disc degeneration. On T2WI, the degenerated intervertebral disc in every patient was evaluated using Pfirrmann grade. Then, the patients were re-evaluated using 3D LAVA-Flex with considerations of the distinction of nucleus pulposus and annulus fibrosus, hypointense signal of intervertebral disc and height of intervertebral disc. The evaluation results were compared between 3D LAVA-Flex and T2WI. Virtual endoscopy was also performed to evaluate the degenerated intervertebral disc. Results The intermediate–intense signal of nucleus pulposus and complete ring-shaped hyperintense signal of annulus fibrosus were found and the distinction of nucleus pulposus and annulus fibrosus was clear in the normal intervertebral disc on 3D LAVA-Flex. The incidence of linear hypointensity of narrowed intervertebral space (65/91) was higher than that of normal intervertebral space (4/134) ( P = 0.000). A good consistency was shown between the LAVA-Flex grade and T2WI-based Pfirrmann grade. Virtual endoscopy based on 3D LAVA-Flex could help clearly show the anatomic relationship between the degenerated disc and intervertebral foramen. Conclusions 3D LAVA-Flex and T2WI show similar efficacy in evaluating lumbar intervertebral disc degeneration. 3D LAVA-Flex-based virtual endoscopy possesses great potential in the study of intervertebral disc abnormalities.

The Effects of Disc Degeneration and Facet Joint Osteoarthritis on the Stability of the Degenerative Lumbar Spine

Abstract:Objective In order to explore the exact relations among disc degeneration,facet joint osteoarthritis,and the stability of the degenerative lumbar spine. Methods Seventy-eight consecutive patients had both magnetic resonance imaging and flexion and extension radiographs of the lumbar spine. The lumbar instability was classified into abnormal tilting on flexion,rotatory instability in the sagittal plane,and translatory instability. Translatory instability was subdivided into anterior, posterior,and anteroposterior translatory instability. Disc degeneration as seen on T2-weighted sagittal images was classified into five grades. Facet joint osteoarthritis as seen on axial T1-weighted images was divided into four grades. The data were performed by using statistical method. Results The results showed that abnormal tilting movement on flexion and anteroposterior translatory instability both had negative associations with facet joint osteoarthritis. However,anterior translatory instability was positively associated with disc degeneration and facet joint osteoarthritis. Rotatory instability in the sagittal plane and posterior translatory instability were not associated with disc degeneration and facet joint osteoarthritis. Conclusion The kinematics of the lumbar motion segment are affected by disc degeneration and facet joint osteoarthritis.
Identifying compositional and structural changes in the nucleus pulposus from patients with lumbar disc herniation using Raman spectroscopy

Abstract:Lower back pain (LBP) is one of the most common musculoskeletal complaints worldwide. Intervertebral disc degeneration (IDD) is considered to be a significant contributor to LBP; however, the mechanisms underlying IDD remain to be fully elucidated. One of the major features of IDD is the decreased content of type II collagen and proteoglycans in the nucleus pulposus (NP). The present study aimed to investigate the biochemical mechanisms of IDD at the microscopic level using Raman spectroscopy. Raman spectroscopy, based on inelastic scattering of light, is an emerging optical technique that may measure the chemical composition of complex biological samples, including biofluids, cells and tissues. In the present study, 30 NP tissue samples from 30 patients who were diagnosed with lumbar disc herniation and received spinal fusion surgery to relieve LBP were obtained and analyzed. Routine pre-operative 3.0T, T2-weighed MRI was used to classify the cases according to Pfirrmann grades and the T2 signal intensity value of the NP was measured. Subsequently, all NP samples were scanned and analyzed using a Laser MicroRaman Spectrometer at room temperature. The Raman spectral results demonstrated that the relative content of proteoglycans, expressed as the relative intensity ratio of two peaks (I1064/I1004), was significantly inversely correlated with the Pfirrmann grade (ρ=-0.6462; P<0.0001), whereas the content of collagen (amide I) was significantly positively correlated with the Pfirrmann grade (ρ=0.5141; P<0.01). In conclusion, the higher relative intensity of the ratio of two peaks (I1670/I1640; Amide I) represented a higher fractional content of disordered collagen, which suggested that the defective collagen structure may lead to NP abnormalities.

Abstract:BackgroundDirect measurement of disc biochemical content is impossible in vivo. Therefore, magnetic resonance imaging (MRI) is used to evaluate disc health. Unfortunately, current clinical imaging techniques do not adequately assess degeneration, especially in the early stage of cartilage endplate, and subchondral bone zone (CEPZ). Therefore, this study aimed to investigate the sensitivity of quantitative MRI methods, namely T2 relaxation time and Magnetic Transfer Ratio (MTR), to identify early disc degeneration, especially for the CEPZ, using an experimental canine model of intervertebral disc injury and to investigate their sensitivity in depicting biochemically and histologically controlled degenerative changes in the disc.MethodsSixteen juvenile dogs underwent iatrogenic annular disruption via stab incisions. The animals underwent repeated 3.0 T MR imaging, and were sacrificed 4, 8, and 12 weeks post-operatively. A continuous rectangle drawing method was used to select regions of interest for the intervertebral disc from the cephalic to caudal CEPZ including the vertebrae, nucleus pulposus (NP) and annulus fibrosus (AF), which resembled pixel measurement for imaging analysis. Presence of degenerative changes was controlled by biochemical and histological analyses. The correlations between histological score, biochemical content, and quantitative MRI signal intensities were also analyzed.ResultsBoth T2 relaxation time and MTR values changed for CEPZ, NP, and AF tissues within 12 weeks. T2 relaxation time values decreased significantly in the NP, AF, and CEPZ separately at pre-operation, 4, 8, and 12 weeks when compared each time (P < 0.05). MTR values showed no significant differences for the CEPZ between 8 and 4 weeks or 12 weeks, or compared to pre-operative values; there were significant differences for the AF. Biochemical and histological analysis showed changes consistent with quantitative MRI signal intensities for early stage degeneration.ConclusionsEarly traumatic or degenerative changes are detectable with both T2 and MTR. T2 changes were more sensitive to the differences in disc status, especially for the CEPZ. Since T2 and MTR reflect different disc properties, performing both imaging under the same conditions would be helpful in the evaluation of disc degeneration. The continuous rectangle drawing can be a sensitive method to detect the changes of CEPZ.

Multi-parameter Evaluation of Lumbar Intervertebral Disc Degeneration Using Quantitative Magnetic Resonance Imaging Techniques.

Abstract:Objective: To quantitatively evaluate lumbar disc degeneration with recently-developed quantitative magnetic resonance imaging (MRI) techniques. A series of MRI parameters, including T2*, T1rho relaxation time, apparent diffusion coefficient and gagCEST, were compared and correlated with the Pfirrmann semi-quantitative classification of lumbar intervertebral disc degeneration; the most accurate and relevant MRI parameters of lumbar disc degeneration were identified. Materials and Methods: Thirty-seven subjects (age range, 18-74 years) with non-specific low back pain (LBP) for more than 6 months were enrolled. The L1/2-L5/S1 discs of each subject were measured and then analyzed. Results: The gagCEST value of the discs showed the best negative correlation with degeneration level (nucleus pulposus: r = -0.951, P < 0.001; annulus fibrosus: r = -0.938, P < 0.001). The discriminant analysis results showed that this parameter also had the highest correct rate using a single index (gagCEST discriminant accuracy = 82%). Conclusion: Early stage lumbar disc degeneration can be quantitatively evaluated with MRI using the chemical exchange saturation transfer technique.
T2 relaxation time for intervertebral disc degeneration in patients with upper back pain: initial results on the clinical use of 3.0 Tesla MRI

Abstract:Background: Magnetic resonance imaging (MRI) is a useful non-invasive tool for evaluating abnormalities of intervertebral discs. However, there are few studies which applied functional MRI techniques to investigate degenerative changes in cervical and cervicothoracic junction (CTJ) spine among adults. The aim of this study was to compare T2 relaxation time measurement evaluation with morphological grading for assessing cervical and CTJ intervertebral discs (IVD) in the patients suffering neck, shoulder, and upper back pain. Methods: Sixty-three patients (378 IVDs) and 60 asymptomatic volunteers (360 IVDs) of the cervical and CTJ discs were assessed using a 3.0 T magnetic resonance imaging (MRI) protocol, including an sagittal T2 relaxation time protocol. The relaxation time values of the nucleus pulposus (NP) were recorded and all discs were visually graded according to Pfirrman's grading system. The correlation between T2 relaxation time values and qualitative clinical grading of degeneration, patient age, sex and anatomic level were analyzed RESULTS: There is a clear trend of decreasing mean T2 values of the NP associate with increasing Pfirrmann grades (C2-T1) for both patients and asymptotic volunteers. Significant T2 differences were seen among grades I-V (P < 0.05). However, grade V was not observed in the CTJ. Linear correlation analysis revealed a strong negative association between T2 values of the NP and Pfirrmann grade (r = -0.588, r = -0.808) of C2-7 and C7T1. Age were also significantly correlated NP T2 values (r = -0.525, r = -0.723) for patients and volunteers. Moreover, the receiver operating characteristic analysis for average measures in a range from 0.70-0.79 (C2-7) to 0.84-0.89 (C7T1) for patients. Conclusions: T2 quantitation provides a more sensitive and robust approach for detecting and characterizing the early stage of IVD degeneration and age-associated disc changes.

Abstract:Objective:To scan and observe the degenerative lumbar facet joints through computed tomography and sum up the pathological changes.Methods:CT images of 88 patients with degenerative facet joints were collected.The facet joint and relative structure were observed.The incidence of every pathological changes were calculated and recorded respectively.Results:The pathological changes were in sequence:joint space narrowing,osteoporosis or sclerosis of the subchondral bone,osteophyte formation,vacuum joint,joint capsule calcification,sagittal articular orientation change,hypertrophy of articular processes,partial dislocation. Conclusion:It's convenient and reliable to observe the structure of the facet joint of lumbar spine through CT scanning,and summarizing pathological changes will be helpful for increasing the knowledge of the degenerative diseases of the lumbar spine.
