Abstract:Computational models have become common tools in psychology. They provide quantitative instantiations of theories that seek to explain the functioning of the human mind. In this paper, we focus on identifying deep theoretical similarities between two very different models. Both models are concerned with how fatigue from sleep loss impacts cognitive processing. The first is based on the diffusion model and posits that fatigue decreases the drift rate of the diffusion process. The second is based on the Adaptive Control of Thought – Rational (ACT-R) cognitive architecture and posits that fatigue decreases the utility of candidate actions leading to microlapses in cognitive processing. A biomathematical model of fatigue is used to control drift rate in the first account and utility in the second. We investigated the predicted response time distributions of these two integrated computational cognitive models for performance on a psychomotor vigilance test under conditions of total sleep deprivation, simulated shift work, and sustained sleep restriction. The models generated equivalent predictions of response time distributions with excellent goodness-of-fit to the human data. More importantly, although the accounts involve different modeling approaches and levels of abstraction, they represent the effects of fatigue in a functionally equivalent way: in both, fatigue decreases the signal-to-noise ratio in decision processes and decreases response inhibition. This convergence suggests that sleep loss impairs psychomotor vigilance performance through degradation of the quality of cognitive processing, which provides a foundation for systematic investigation of the effects of sleep loss on other aspects of cognition. Our findings illustrate the value of treating different modeling formalisms as vehicles for discovery.

Optimization of biomathematical model predictions for cognitive performance impairment in individuals: accounting for unknown traits and uncertain states in homeostatic and circadian processes

Abstract:Current biomathematical models of fatigue and performance do not accurately predict cognitive performance for individuals with a priori unknown degrees of trait vulnerability to sleep loss, do not predict performance reliably when initial conditions are uncertain, and do not yield statistically valid estimates of prediction accuracy. These limitations diminish their usefulness for predicting the performance of individuals in operational environments. To overcome these 3 limitations, a novel modeling approach was developed, based on the expansion of a statistical technique called Bayesian forecasting. The expanded Bayesian forecasting procedure was implemented in the two-process model of sleep regulation, which has been used to predict performance on the basis of the combination of a sleep homeostatic process and a circadian process. Employing the two-process model with the Bayesian forecasting procedure to predict performance for individual subjects in the face of unknown traits and uncertain states entailed subject-specific optimization of 3 trait parameters (homeostatic build-up rate, circadian amplitude, and basal performance level) and 2 initial state parameters (initial homeostatic state and circadian phase angle). Prior information about the distribution of the trait parameters in the population at large was extracted from psychomotor vigilance test (PVT) performance measurements in 10 subjects who had participated in a laboratory experiment with 88 h of total sleep deprivation. The PVT performance data of 3 additional subjects in this experiment were set aside beforehand for use in prospective computer simulations. The simulations involved updating the subject-specific model parameters every time the next performance measurement became available, and then predicting performance 24 h ahead. Comparison of the predictions to the subjects' actual data revealed that as more data became available for the individuals at hand, the performance predictions became increasingly more accurate and had progressively smaller 95% confidence intervals, as the model parameters converged efficiently to those that best characterized each individual. Even when more challenging simulations were run (mimicking a change in the initial homeostatic state; simulating the data to be sparse), the predictions were still considerably more accurate than would have been achieved by the two-process model alone. Although the work described here is still limited to periods of consolidated wakefulness with stable circadian rhythms, the results obtained thus far indicate that the Bayesian forecasting procedure can successfully overcome some of the major outstanding challenges for biomathematical prediction of cognitive performance in operational settings.

Abstract:Biomathematical models of fatigue capture the physiology of sleep/wake regulation and circadian rhythmicity to predict changes in neurobehavioral functioning over time. We used a biomathematical model of fatigue linked to the adenosinergic neuromodulator/receptor system in the brain as a framework to predict sleep inertia, that is, the transient neurobehavioral impairment experienced immediately after awakening. Based on evidence of an adenosinergic basis for sleep inertia, we expanded the biomathematical model with novel differential equations to predict the propensity for sleep inertia during sleep and its manifestation after awakening. Using datasets from large laboratory studies of sleep loss and circadian misalignment, we calibrated the model by fitting just two new parameters and then validated the model's predictions against independent data. The expanded model was found to predict the magnitude and time course of sleep inertia with generally high accuracy. Analysis of the model's dynamics revealed a bifurcation in the predicted manifestation of sleep inertia in sustained sleep restriction paradigms, which reflects the observed escalation of the magnitude of sleep inertia in scenarios with sleep restriction to less than ∼ 4 h per day. Another emergent property of the model involves a rapid increase in the predicted propensity for sleep inertia in the early part of sleep followed by a gradual decline in the later part of the sleep period, which matches what would be expected based on the adenosinergic regulation of non-rapid eye movement (NREM) sleep and its known influence on sleep inertia. These dynamic behaviors provide confidence in the validity of our approach and underscore the predictive potential of the model. The expanded model provides a useful tool for predicting sleep inertia and managing impairment in 24/7 settings where people may need to perform critical tasks immediately after awakening, such as on-demand operations in safety and security, emergency response, and health care.

Abstract:Maintaining human alertness and behavioral capability under conditions of sleep loss and circadian misalignment requires fatigue management technologies due to: (1) dynamic nonlinear modulation of performance capability by the interaction of sleep homeostatic drive and circadian regulation; (2) large differences among people in neurobehavioral vulnerability to sleep loss; (3) error in subjective estimates of fatigue on performance; and (4) to inform people of the need for recovery sleep. Two promising areas of technology have emerged for managing fatigue risk in safety-sensitive occupations. The first involves preventing fatigue by optimizing work schedules using biomathematical models of performance changes associated with sleep homeostatic and circadian dynamics. Increasingly these mathematical models account for individual differences to achieve a more accurate estimate of the timing and magnitude of fatigue effects on individuals. The second area involves technologies for detecting transient fatigue from drowsiness. The Psychomotor Vigilance Test (PVT), which has been extensively validated to be sensitive to deficits in attention from sleep loss and circadian misalignment, is an example in this category. Two shorter-duration versions of the PVT recently have been developed for evaluating whether operators have sufficient behavioral alertness prior to or during work. Another example is online tracking the percent of slow eyelid closures (PERCLOS), which has been shown to reflect momentary fluctuations of vigilance. Technologies for predicting and detecting sleepiness/fatigue have the potential to predict and prevent operator errors and accidents in safety-sensitive occupations, as well as physiological and mental diseases due to inadequate sleep and circadian misalignment.

Abstract:Sleep restriction causes impaired cognitive performance that can result in adverse consequences in many occupational settings. Individuals may rely on self-perceived alertness to decide if they are able to adequately perform a task. It is therefore important to determine the relationship between an individual's self-assessed alertness and their objective performance, and how this relationship depends on circadian phase, hours since awakening, and cumulative lost hours of sleep. Healthy young adults (aged 18-34) completed an inpatient schedule that included forced desynchrony of sleep/wake and circadian rhythms with twelve 42.85-hour "days" and either a 1:2 (n = 8) or 1:3.3 (n = 9) ratio of sleep-opportunity:enforced-wakefulness. We investigated whether subjective alertness (visual analog scale), circadian phase (melatonin), hours since awakening, and cumulative sleep loss could predict objective performance on the Psychomotor Vigilance Task (PVT), an Addition/Calculation Test (ADD) and the Digit Symbol Substitution Test (DSST). Mathematical models that allowed nonlinear interactions between explanatory variables were evaluated using the Akaike Information Criterion (AIC). Subjective alertness was the single best predictor of PVT, ADD, and DSST performance. Subjective alertness alone, however, was not an accurate predictor of PVT performance. The best AIC scores for PVT and DSST were achieved when all explanatory variables were included in the model. The best AIC score for ADD was achieved with circadian phase and subjective alertness variables. We conclude that subjective alertness alone is a weak predictor of objective vigilant or cognitive performance. Predictions can, however, be improved by knowing an individual's circadian phase, current wake duration, and cumulative sleep loss.

Abstract:Objective: We developed a computational model of the effects of sleep deprivation on the vigilance decrement by employing the methods of system dynamics modeling. Background: Situations that require sustained attention for a prolonged duration can cause a decline in cognitive performance, the so-called vigilance decrement. One factor that should influence the vigilance decrement is fatigue in the form of sleep deprivation. Method: We employed the methods of system dynamics modeling (numerical-integration techniques for modeling complex feedback systems) to create a computational model of the vigilance decrement. We then simulated the computational effects of sleep deprivation on the behavior of that model, using empirical data obtained from the literature for calibrating such effects. Results: Sleep deprivation of 2 hr over a 14-day period should produce an additional decline of 9% in detection performance over that found with the typical vigilance decrement, whereas 4 hr of sleep deprivation over 14 days should produce an additional decline of 14% in detection performance. Conclusion: With respect to dual-process theory, it is through its deleterious effects on analytical cognition that sleep deprivation should impact the vigilance decrement. Application: Such computational modeling may be advantageous for human-machine teaming by theoretically allowing a future autonomous software agent to anticipate the decline of human performance and compensate accordingly.

Abstract:Objectives: Mathematical models of human neurobehavioral performance that include the effects of acute and chronic sleep restriction can be key tools in assessment and comparison of work schedules, allowing quantitative predictions of performance when empirical assessment is impractical. Methods: Using such a model, we tested the hypothesis that resident physicians working an extended duration work roster, including 24-28 hours of continuous duty and up to 88 hours per week averaged over 4weeks, would have worse predicted performance than resident physicians working a rapidly cycling work roster intervention designed to reduce the duration of extended shifts. The performance metric used was attentional failures (ie, Psychomotor Vigilance Task lapses). Model input was 169 actual work and sleep schedules. Outcomes were predicted hours per week during work hours spent at moderate (equivalent to 16-20 hours of continuous wakefulness) or high (equivalent to ≥20 hours of continuous wakefulness) performance impairment. Results: The model predicted that resident physicians working an extended duration work roster would spend significantly more time at moderate impairment (p = .02, effect size=0.2) than those working a rapidly cycling work roster; this difference was most pronounced during the circadian night (p < .001). On both schedules, performance was predicted to decline from weeks 1 + 2 to weeks 3 + 4 (p < .001), but the rate of decline was significantly greater on extended duration work roster (p < .01). Predicted performance impairment was inversely related to prior sleep duration (p < .001). Conclusions: These findings demonstrate the utility of a mathematical model to evaluate the predicted performance profile of schedules for resident physicians and others who experience chronic sleep restriction and circadian misalignment.

Abstract:BACKGROUND The primary care physicians in the Israeli Defense Forces, as in the Israeli civilian health system, have two major subpopulations. Graduates of Israeli schools of medicine, and graduates of foreign medical schools, most of them in Eastern Europe. OBJECTIVE To evaluate differences in the referral patterns of primary care physicians according to their graduation institution and demographic characteristics. METHODS The study took place in one primary care practice in central Israel. The referrals to consultations and laboratory tests over a period of 1 year were evaluated. Physicians that had less than 37 encounters were excluded from the study. Data were extracted from the central computerized databases of the Medical Corps and Israeli Defense Forces. RESULTS Sixty-eight physicians had a total of 18,402 encounters that resulted in 23,845 outcomes. There were no associations between demographic and training backgrounds of the physicians and their actual referral rates to consultations and laboratory tests. CONCLUSION The background data of the primary care physicians does not predict their referral patterns and their role as "gate keepers".

Abstract:Abstract Introduction Sleep dysfunction increases the risk of mild cognitive impairment (MCI) and Alzheimer’s Disease (AD). Emerging sensor technologies can index real world (RW) sleep and instrumental activities of daily living affected by neurodegeneration, as outlined below. Methods 85 legally licensed active drivers with cognitive decline (76 MCI, 9 mild AD) (mean age = 75.6 years; 37 females) and 54 age and education-matched controls (mean age= 74.7 years; 36 females) participated. Cognitive status was assessed using standardized neuropsychological tests according to NIA-AA guidelines. Sensor systems installed in participants’ own vehicles recorded driving incidence (driving or not each day). Sleep was recorded using wrist-worn actigraphy, and verified with self-reported sleep diaries over three continuous months. A mixed-effect logistic regression analyzed daily driving incidence with predictors including z-scores of total sleep time (TST) and sleep efficiency (SE) in previous night, and cognitive status (MCI or AD v. controls), adjusted for age, gender, employment status, and driving season. Results From 9,214 days of data on nightly sleep and next day driving, increase in TST significantly reduced driving incidence (21% per standard deviation [SD]; Odds Ratio [OR] = 0.79, 95% Confidence Interval [CI] = 0.74 – 0.84, p < 0.001). Increase in SE was associated with increased driving incidence among those with cognitive decline, compared to controls (15% per SD; OR = 1.14, 95% CI = 1.03 – 1.27, p = 0.01). Conclusion Findings underscore the value of RW digital biomarkers to track the effects of neurodegeneration on sleep and instrumental activities of daily living. Deciding to drive increased with greater sleep efficiency, particularly for those with cognitive decline. Quantitative profiles of sleep and driving behavior offer novel opportunities for early detection of the effects of AD. Abstract:Much of the current science on, and mathematical modeling of, dynamic changes in human performance within and between days is dominated by the two-process model of sleep-wake regulation, which posits a neurobiological drive for sleep that varies homeostatically (increasing as a saturating exponential during wakefulness and decreasing in a like manner during sleep), and a circadian process that neurobiologically modulates both the homeostatic drive for sleep and waking alertness and performance. Endogenous circadian rhythms in neurobehavioral functions, including physiological alertness and cognitive performance, have been demonstrated using special laboratory protocols that reveal the interaction of the biological clock with the sleep homeostatic drive. Individual differences in circadian rhythms and genetic and other components underlying such differences also influence waking neurobehavioral functions. Both acute total sleep deprivation and chronic sleep restriction increase homeostatic sleep drive and degrade waking neurobehavioral functions as reflected in sleepiness, attention, cognitive speed, and memory. Recent evidence indicating a high degree of stability in neurobehavioral responses to sleep loss suggests that these trait-like individual differences are phenotypic and likely involve genetic components, including circadian genes. Recent experiments have revealed both sleep homeostatic and circadian effects on brain metabolism and neural activation. Investigation of the neural and genetic mechanisms underlying the dynamically complex interaction between sleep homeostasis and circadian systems is beginning. A key goal of this work is to identify biomarkers that accurately predict human performance in situations in which the circadian and sleep homeostatic systems are perturbed.

Awareness of sleepiness and ability to predict sleep onset: can drivers avoid falling asleep at the wheel?

Abstract:Objectives: Regarding the causes of sleep-related accidents, this study assesses whether individuals can anticipate sleep onset accurately and how individuals acknowledge and use physiological and cognitive cues to make judgments related to sleep onset. Methods: A group of 41 partially sleep-deprived subjects predicted the likelihood of sleep in 30 consecutive two-minute intervals and noted physiological and cognitive signs of sleepiness, including involuntary eye closure, head-nodding, wandering thoughts, yawns, and instances of sleep, collectively referred to as "sleep complaints". Continuous polysomnographic recording compared these predictions to actual instances of sleep. Results: Subjects varied in their ability to predict sleep onset. For all subjects, the mean prediction of the likelihood of sleep prior to sleep was significantly higher than the mean prediction of the likelihood of sleep prior to intervals in which no sleep occurred (78% vs. 42%; p<.05). However, subjects tended to predict much lower likelihoods of sleep onset before their first sleep event (55%) than before later sleep events. On average, the rate at which subjects reported miscellaneous sleep complaints (such as head-nodding, eye closure, and wandering thoughts) was higher prior to sleep than prior to intervals in which sleep did not occur. Conclusion: Subjects who acknowledged a limited number of physiological and cognitive indicators of sleepiness tended to be poor predictors. Subjects whose physiological and cognitive signs of sleepiness failed to provide a strong indication of whether or not sleep onset would occur also tended to be poor predictors. Inability to judge sleep onset and, hence, susceptibility to sleep-related accidents, may be attributable to both a scarcity of meaningful warning signs and a failure to acknowledge the importance of physiological and cognitive indicators.
Machine Learning Models for the Classification of Sleep Deprivation Induced Performance Impairment During a Psychomotor Vigilance Task Using Indices of Eye and Face Tracking

Abstract:High risk professions, such as pilots, police officers, and TSA agents, require sustained vigilance over long periods of time and/or under conditions of little sleep. This can lead to performance impairment in occupational tasks. Predicting impaired states before performance decrement manifests is critical to prevent costly and damaging mistakes. We hypothesize that machine learning models developed to analyze indices of eye and face tracking technologies can accurately predict impaired states. To test this we trained 12 types of machine learning algorithms using five methods of feature selection with indices of eye and face tracking to predict the performance of individual subjects during a psychomotor vigilance task completed at 2-h intervals during a 25-h sleep deprivation protocol. Our results show that (1) indices of eye and face tracking are sensitive to physiological and behavioral changes concomitant with impairment; (2) methods of feature selection heavily influence classification performance of machine learning algorithms; and (3) machine learning models using indices of eye and face tracking can correctly predict whether an individual's performance is "normal" or "impaired" with an accuracy up to 81.6%. These methods can be used to develop machine learning based systems intended to prevent operational mishaps due to sleep deprivation by predicting operator impairment, using indices of eye and face tracking.

Abstract:As a common complaint in contemporary society, mental fatigue is a key element in the deterioration of the daily activities known as time-on-task (TOT) effect, making the prediction of fatigue-related performance decline exceedingly important. However, conventional group-level brain-behavioral correlation analysis has the limitation of generalizability to unseen individuals and fatigue prediction at individual-level is challenging due to the significant differences between individuals both in task performance efficiency and brain activities. Here, we introduced a cross-validated data-driven analysis framework to explore, for the first time, the feasibility of utilizing pre-task idiosyncratic resting-state functional connectivity (FC) on the prediction of fatigue-related task performance degradation at individual level. Specifically, two behavioral metrics, namely ∆RT (between the most vigilant and fatigued states) and TOT<sub>slope</sub> over the course of the 15-min sustained attention task, were estimated among three sessions from 37 healthy subjects to represent fatigue-related individual behavioral impairment. Then, a connectome-based prediction model was employed on pre-task resting-state FC features, identifying the network-related differences that contributed to the prediction of performance deterioration. As expected, prominent populational TOT-related performance declines were revealed across three sessions accompanied with substantial inter-individual differences. More importantly, we achieved significantly high accuracies for individualized prediction of both TOT-related behavioral impairment metrics using pre-task neuroimaging features. Despite the distinct patterns between both behavioral metrics, the identified top FC features contributing to the individualized predictions were mainly resided within/between frontal, temporal and parietal areas. Overall, our results of individualized prediction framework extended conventional correlation/classification analysis and may represent a promising avenue for the development of applicable techniques that allow precaution of the TOT-related performance declines in real-world scenarios.

Abstract:We explored the predictive value of a neurobehavioral performance assessment under rested baseline conditions (evaluated at 8 hours awake following 8 hours of sleep) on neurobehavioral response to moderate sleep loss (evaluated at 20 hours awake two days later) in 151 healthy young participants (18-30 years). We defined each participant's response-to-sleep-loss phenotype based on the number of attentional failures on a 10-min visual psychomotor vigilance task taken at 20 hours awake (resilient: less than 6 attentional failures, n = 26 participants; non-resilient: 6 or more attentional failures, n = 125 participants). We observed that 97% of rested participants with 2 or more attentional failures (n = 73 of 151) and 100% of rested participants with 3 or more attentional failures (n = 57 of 151) were non-resilient after moderate sleep loss. Our approach can accurately identify a significant proportion of individuals who are at high risk for neurobehavioral performance impairment from staying up late with a single neurobehavioral performance assessment conducted during rested conditions. Additional methods are needed to predict the future performance of individuals who are not identified as high risk during baseline.
The effects of different fatigue levels on brain-behavior relationships in driving

Abstract:Background: In the past decade, fatigue has been regarded as one of the main factors impairing task performance and increasing behavioral lapses during driving, even leading to fatal car crashes. Although previous studies have explored the impact of acute fatigue through electroencephalography (EEG) signals, it is still unclear how different fatigue levels affect brain-behavior relationships. Methods: A longitudinal study was performed to investigate the brain dynamics and behavioral changes in individuals under different fatigue levels by a sustained attention task. This study used questionnaires in combination with actigraphy, a noninvasive means of monitoring human physiological activity cycles, to conduct longitudinal assessment and tracking of the objective and subjective fatigue levels of recruited participants. In this study, degrees of effectiveness score (fatigue rating) are divided into three levels (normal, reduced, and high risk) by the SAFTE fatigue model. Results: Results showed that those objective and subjective indicators were negatively correlated to behavioral performance. In addition, increased response times were accompanied by increased alpha and theta power in most brain regions, especially the posterior regions. In particular, the theta and alpha power dramatically increased in the high-fatigue (high-risk) group. Additionally, the alpha power of the occipital regions showed an inverted U-shaped change. Conclusion: Our results help to explain the inconsistent findings among existing studies, which considered the effects of only acute fatigue on driving performance while ignoring different levels of resident fatigue, and potentially lead to practical and precise biomathematical models to better predict the performance of human operators.

Abstract:Fatigue risk in humans has few biomathematical models, and existing biomathematical models have many shortcomings. We developed a biomathematical model of fatigue risk based on the concept of energy to quantify human alertness from the perspective of energy and used alertness to characterise human psychological fatigue risk. The model allows mathematical modelling of activity processes that concern sleep intensity and quality and distinguishes between intrinsic exertion in the waking state and workload exertion in the workload state. This alertness and fatigue risk biomathematical model predicts changes in human alertness and fatigue risk indices at any point in the day. We applied numerical simulation and model analysis to five cases to validate the potential value of the alertness energy fatigue risk biomathematical model. Practitioner summary: To overcome the shortcomings of current biomathematical models that evaluate fatigue risk, this study developed a biomathematical model of fatigue risk based on the concept of energy to quantify human alertness from the perspective of energy and used alertness to characterise human fatigue risk. Abstract:Light and electron microscope studies were conducted on the nasopharynx and the nasopharyngeal tonsil of 15 young horses. The nasopharynx and nasopharyngeal tonsil was lined with pseudostratified columnar ciliated epithelium and goblet cells. The lymphoepithelium of the nasopharyngeal tonsil was folded forming crypts, the mucosa of which was modified into follicle associated epithelium characterized by stratified cuboidal epithelium, loss of cilia, absence of goblet cells and infiltration of lymphocytes. The lamina propria mucosae of the nasopharyngeal tonsil contained well‐developed lymphoid tissue and clusters of seromucus acini. Scanning electron‐microscopy revealed a dense mat of cilia covering the nasopharynx and nasopharyngeal tonsil. The follicle‐associated epithelium consisted of different populations of microvillus cells in addition to M cells with very short microvilli and a few squamous and intermediate cells. Microvillus cells in the deeper part of the FAE had larger microvilli and their cytoplasm contained a dense population of mitochondria, smooth and rough endoplasmic reticulum, Golgi complexes and lysosomes. The flat surfaced M cell had a more electron‐dense cytoplasm and contained small supranuclear vacuoles in addition to the organelles seen in microvillus cells.
