Abstract:Background: The anatomical features of the third occipital nerve (TON) are the least studied among the occipital nerves. This study aimed to analyze the anatomical features and potential compression points of the TON. Methods: The posterior neck and scalp of 39 cadavers were dissected. The TON was carefully followed from the emerging point from C2-C3 vertebrae distally. Its muscular investments were detected. The determined points were marked superficially and measured according to external occipital protuberance with the Image J Software program. Results: The TON revealed 4 different potential entrapment points along its course. The most proximal point was between the C2-C3 vertebrae. The second and third points were at their piercing points of semispinalis capitis and splenius capitis muscles, respectively. The final point was at its penetrating point of the trapezius muscle. Additionally, TON did not pierce the semispinalis capitis muscle on 6 sides and pierced this muscle from 3 different points by splitting into 3 branches on 1 side. Moreover, on 5 sides, the TON split into two branches and perforated the splenius capitis muscle from two different points. Conclusion: There were 4 potential compression points regarding the TON. These entrapment points and variations of TON in this study may play a significant role in understanding the reason for TON-related headaches and the treatment of headaches caused by TON.

Unusual case of resistant actinomycosis following facial trauma.

Abstract:A case of actinomycosis is reported which appeared as a complication of a ski-pole injury of the face. A 16-year-old girl was treated for 10 years for resistant actinomycosis. She took long-term antimicrobial therapies, underwent repeated excisions, hyperbaric oxygen therapy, repeated skin grafting, immunological treatment, and free flap transfer. The conservative antibiotic treatment was complicated by mycosis and pseudomembranous colitis. After the defect healed up, satisfactory contour of the face was restored with additional free flap transfer. The patient has also been satisfied with the final aesthetic result. The authors highlight the importance of early diagnosis and proper antimicrobial treatment in combination with surgical excisions of granulation tissue preserving vital structures.
Single midline incision approach for decompression of greater, lesser and third occipital nerves in migraine surgery

Abstract:Background: The traditional approach for occipital migraine surgery encompasses three separate surgical incisions in the posterior neck to decompress the greater occipital nerves (GON), lesser occipital nerves (LON), and third occipital nerves (TON). Other incisions have been investigated, including singular transverse incisions. We sought to evaluate a single, vertical midline incision approach for decompression of all six occipital nerves. Methods: Using 10 cadaveric hemi-sides (5 fresh cadaver head and necks). Anatomic landmarks and the location of the bilateral GON, LON, and TON were marked according to previous anatomic studies. A single, midline 9-cm incision was made, and lateral skin flaps were raised to decompress or avulse all six nerves. Results: Through the midline incision, the GON and TON were identified at 3.5 and 6.2 cm, respectively, inferior to a line bisecting the external auditory canal (EAC) and 1.5 cm lateral to the midline. The LON was identified as 6-cm inferior and 6.5-cm medial to a line bisecting the EAC in the plane just above the investing layer of the deep cervical fascia until the posterior border of the sternocleidomastoid was encountered. The LON had the greatest amount of variation but was identified lateral to the posterior border of the SCM. Conclusions: A single midline incision approach allows for successful identification and decompression of all six occipital nerves in migraine surgery.

Abstract:Occipital neuralgia (ON) stands out as one of the most distressing forms of headache disorders, distinguished by persistent pain at the base of the skull, recurring occipital headaches, and scalp dysesthesia or allodynia. ON is notorious for its unrelenting agony, severely impacting the lives of those afflicted. The incessant pain, often radiating upward from the base of the skull to the scalp, can be profoundly debilitating. Patients frequently endure excruciating occipital headaches, making even routine daily activities a formidable challenge. The added burden of scalp dysesthesia or allodynia, where seemingly innocuous stimuli elicit severe pain, compounds the suffering. This neuralgia primarily arises from the mechanical compression exerted on the occipital nerves along the nuchal line. In this paper, we present a minimally invasive nerve and muscle-preserving technique aimed at relieving this compression on the occipital nerves. Accurate diagnosis and effective treatment are paramount in providing relief for individuals battling this condition. Nerve blocks with local anesthesia have become a cornerstone of diagnosis, serving as both a confirmation of occipital neuralgia and a potential therapeutic intervention. These procedures offer crucial insights into the source of the pain while offering transient respite. However, the true breakthrough lies in the innovative technique we propose - a procedure that addresses the mechanical compression at the nuchal line, which is a prominent contributing factor to occipital neuralgia. By carefully decompressing the affected occipital nerves while preserving their integrity and surrounding muscle tissue, this minimally invasive approach offers patients a potential path to sustained relief. Remarkably, the procedure can be performed under local anesthesia, reducing the invasiveness of traditional surgeries and minimizing patient downtime.

Abstract:Occipital neuralgia can be due to multiple etiologies. One of these is potential compression of the greater occipital nerve (GON). In this regard, one relationship of the GON, its course through the obliquus inferior capitis muscle (OIC), has yet to be well studied. Therefore, the current anatomical study was performed to elucidate this relationship better. In the prone position, the suboccipital triangle was exposed, and the relationship between the GON and OIC was documented in 72 adult cadavers (144 sides). The GON was found to pierce the OIC on four sides (2.8%), unilaterally in two cadavers and bilaterally in one cadaver. Two cadavers were male, and one was female. Histological samples were taken from GONs with a normal course around the OIC, and nerves were found to pierce the OIC. The GON of all four sides identified histological changes consistent with nerve potential compression (e.g., epineurial and perineurial thickening). This is also the first histological analysis of the trans‐OIC course of the GON, demonstrating signs of chronic nerve potential compression. Although uncommon, entrapment of the GON by the OIC may be an underrecognized etiology of occipital neuralgia.

Iatrogenic Occipital Nerve Injury: Etiologies and Outcomes Following Nerve Decompression Surgery

Abstract:Summary Introduction This study analyses the etiologies and treatment of iatrogenic occipital nerve injuries. Methods Patients with ON who were screened for occipital nerve decompression surgery were prospectively enrolled. Patients with iatrogenic occipital nerve injuries who underwent nerve decompression surgery were identified. Data included surgical history, pain characteristics, and surgical technique. Outcomes included pain frequency (days/month), duration (hours/day), intensity (0-10), migraine headache index (MHI) and patient-reported percent-resolution of pain. Results Among 416 patients with ON who were screened for occipital nerve decompression surgery, 12 (2.9%) cases of iatrogenic occipital nerve injury were identified and underwent surgical treatment. Preoperative headache frequency was 30 (±0.0) days/month, duration 19.4 (±6.9) hours, intensity 9.2 (±0.9). Neuroma excision was performed in five cases followed by targeted muscle reinnervation (TMR) in three, nerve cap in one and muscle burial in one. In patients without neuromas, GON decompression and/or LON neurectomy were performed. At a median follow-up of 12 months (IQR 12-12), mean pain frequency was 4.0 (±6.6) pain days/month (p<0.0001), duration 6.3 (±8.9) hours (p<0.01), and intensity 4.4 (±2.8) (p<0.001). Median patient-reported resolution of pain was 85% (56.3-97.5%) and success rate (≥50% MHI improvement) 91.7%. Conclusion Iatrogenic occipital nerve injuries can be caused by various surgical interventions, including craniotomies, cervical spine interventions and scalp tumor resections. The associated pain can be severe and chronic. Iatrogenic ON should be considered in the differential diagnosis of postoperative headaches and can be treated with nerve decompression surgery or neuroma excision with reconstruction of the free nerve end.

Abstract:The lesser occipital nerve (LON) has one of the most variations among occipital nerves. We aimed to investigate morphological and morphometric features of LON. A total of 24 cadavers, 14 males (58%) and 10 females (42%), were dissected bilaterally. LON was classified into 3 types. The number of branches and the perpendicular distances of the point where LON emerged from the posterior border of sternocleidomastoid muscle to vertical and transverse lines passing through external occipital protuberance were determined. The shortest distance between LON and great auricular nerve (GAN), and linear distance of LON to its branching point were measured. The most common variant was Type 1 (30 sides, 62.5%), followed by Type 2 (12 sides, 25%) and Type 3 (6 sides, 12.5%), respectively. In males, Type 1 (22 sides, 78.6%) was the most common, while Type 1 (8 sides, 40%) and Type 2 (8 sides, 40%) were equally common and the most common in females. On 48 sides, 2–9 branches of LON were observed. The perpendicular distance of said point to vertical and transverse lines was meanly 63.69 ± 11.28 mm and 78.83 ± 17.21 mm, respectively. The shortest distance between LON and GAN was meanly 16.62 ± 10.59 mm. The linear distance of LON to its branching point was meanly 31.24 ± 15.95 mm. The findings reported in this paper may help clinicians in estimating the location of the nerve and/or its branches for block or decompression surgery as well as preservation of LON during related procedures.

Occipital Nerve Electrical Stimulation via the Midline Approach and Subcutaneous Surgical Leads for Treatment of Severe Occipital Neuralgia: A Pilot Study

Abstract:Persistent occipital neuralgia can produce severe headaches that may not be controllable by conservative or surgical approaches. We describe a case series of 6 patients who had chronic headaches over an average of 4.9 yr who underwent occipital nerve electrical stimulation lead implantation using a modified midline approach. The patients had received conservative and surgical therapies in the past including oral antidepressants, membrane stabilizers, opioids, occipital nerve blocks, and radiofrequency ablations. Significant decreases in pain visual analog scale (VAS) scores and drastic improvement in functional capacity were observed during the occipital stimulation trial and during the 3-mo follow-up after implantation. The mean VAS score changed from 8.66 +/- 1.0 to 2.5 +/- 1.3 whereas pain disability index improved from 49.8 +/- 15.9 to 14.0 +/- 7.4. Our midline approach has several advantages compared with the submastoid approach used elsewhere. There is only one small midline incision over the upper neck and the strain on the lead extension occurs only with flexion and is minimal with lateral flexion and rotation, which contributes to overall stability of this system.

11. Cervicogenic headache and occipital neuralgia

Abstract:Introduction: Cervicogenic headache (CEH) and occipital neuralgia (ON) are headaches originating in the occiput and that radiate to the vertex. Because of the intimate relationship between structures based in the occiput and those in the upper cervical region, there is significant overlap between the presentation of CEH and ON. Diagnosis starts with a headache history to assess for diagnostic criteria formulated by the International Headache Society. Physical examination evaluates range of motion of the neck and the presence of tender areas or pressure points. Methods: The literature for the diagnosis and treatment of CEH and ON was searched from 2015 through August 2022, retrieved, and summarized. Results: Conservative treatment includes pain education and self-care, analgesic medication, physical therapy (such as reducing secondary muscle tension and improving posture), the use of TENS (transcutaneous electrical nerve stimulation), or a combination of the aforementioned treatments. Injection at various anatomical locations with local anesthetic with or without corticosteroids can provide pain relief for a short period. Deep cervical plexus block can result in improved pain for less than 6 months. In both CEH and ON, an occipital nerve block can provide important diagnostic information and improve pain in some patients, with PRF providing greater long-term pain control. Radiofrequency ablation of the cervical facet joints can result in improvement for over 1 year. Occipital nerve stimulation (ONS) should be considered for the treatment of refractory ON. Conclusion: The treatment of CEH preferentially consists of radiofrequency treatment of the facet joints, while for ON, pulsed radiofrequency of the occipital nerves is indicated. For refractory cases, ONS may be considered.
Variations in human trigeminal and facial nerve branches and foramina identified by dissection and microcomputed tomography

Abstract:The aim of this study was to identify branches of the trigeminal and facial nerves relevant to surgical incisions and injections and the scalp block techniques in the frontotemporal region, and to determine their relationships with superficial vascular structures and bony landmarks. Half-heads from consenting embalmed donors (6 male, 2 female, mean age at death 78.4 years) were used in this study. Detailed dissection was carried out to identify the position of the auriculotemporal nerve (ATN) relative to the superior temporal artery (STA) and the facial nerve (FN) in six subjects (5 male, 1 female). The results provide a minimum safe distance of 5 mm between the STA and the frontotemporal branches of the FN at the level of the low edge of zygoma and 8mm between the low edge of zygoma and the FN trunk, providing a pre-auricular triangle of safety for incisions and injections. Variability between subjects was up to 60%. Microcomputed tomography (microCT) scans were taken from all eight subjects and the three-dimensional reconstructions were used to identify the supraorbital notch (SON), the zygomaticotemporal foramen (ZTF) and the zygomaticofacial foramen (ZFF). The volume and relative locations of these foramina were calculated for 5-8 subjects. The closest distance between ZTF and the FZS ranged from 9 to 21mm (26% variation); 3 subjects had a single ZTF while 5 subjects had two ZTF. The angle at the centre of the orbit between ZFF and the FZS ranged from 156 to 166 degrees (2.5% variation). These findings demonstrate that both traditional cadaveric dissection methods as well as contemporary microCT methods can be used to investigate the relative locations of nerves or their foramina in the human head. The findings provide anatomical considerations for fronto-temporal incisions and local anaesthesia.

Classification and clinical anatomy of the first spinal nerve: surgical implications

Abstract:Object: Data regarding the first cervical nerve are scanty and conflicting, however this nerve may need to be identified for neurosurgical procedures such as rhizotomy for torticollis and suboccipital pain syndromes. The authors performed the present study to elucidate further the detailed anatomy of the first cervical nerve and review its clinical relevance. Methods: Forty adult cadavers (80 sides) were used in this study. Dissection was performed at the craniocervical junction with special attention to the formation and presence of the components of the C-1 spinal nerve. Additionally, connections between the C-1 nerve and the spinal accessory nerves were recorded. Results: The authors classified the C-1 nerves into Types Ia, Ib, and II, in 34, 9, and 37 sides, respectively. Type Ia was composed of ventral and dorsal roots with a dorsal root ganglion, Type Ib was composed of ventral and dorsal roots and no dorsal root ganglion, and Type II was composed of only ventral roots. All types contained both dorsal and ventral rami. Mackenzie's nerve was identified on 2 left sides (2.5%). On 48 sides (60%), the C-1 nerve received a mean of 2.5 dorsal rootlets. In the remaining specimens, C-1 did not receive any dorsal rootlets. On the sides found to receive C-1 dorsal rootlets, 14 (30%) were found to have a distinct dorsal root ganglion present, and in 21 (44%) the spinal accessory nerve joined with the dorsal rootlets. The first cervical vertebra in these cases did not possess a dorsal root ganglion. A dorsal ramus of the C-1 spinal nerve was identified on all sides. Communication between the dorsal rami of C-1 and C-2 near their posterior elements was found on 12 sides (15%). Conclusions: A detailed knowledge of C-1 nerve anatomy may be of use to the surgeon operating in the vicinity. Specifically, this knowledge may be helpful in procedures involving C-1 rhizotomy, including peripheral denervation procedures for cervical dystonia and occipital neuralgia.

Abstract:Background: Occipital neuralgia is a debilitating condition, and traditional treatments often provide limited or temporary relief. Recently, ultrasound-guided hydrodissection of the greater occipital nerve (GON) has emerged as a promising minimally invasive approach. Objectives: To describe two novel ultrasound-guided hydrodissections with 5% dextrose for GON and discuss their advantages, disadvantages, and considerations. Methods: Two cases are reported. Case 1 describes a lateral decubitus approach for hydrodissecting the GON between the semispinalis capitis (SSC) and obliquus capitis inferior (OCI) muscles. Case 2 details a cranial-to-caudal approach for hydrodissecting the GON within the SSC and upper trapezius (UT) muscles when the GON passes through these two muscles. Results: Both patients experienced significant and sustained pain relief with improvements in function. Conclusions: Ultrasound-guided GON hydrodissection using 5% dextrose is a promising treatment for occipital neuralgia. The lateral decubitus and cranial-caudal approaches provide additional options to address patient-specific anatomical considerations and preferences.

Abstract:Background: Occipital neuralgia is defined as paroxysmal shooting, or stabbing pain in the posterior part of the scalp, in the distribution of the greater and lesser occipital nerves. Occipital neuralgia may present only as an intermittent stabbing pain, but different opinions exist on its cause and diagnostic criteria.Current Concepts: According to the latest version of headache classification, only paroxysmal stabbing pain is included in the diagnostic criteria, and persistent aching pain is excluded. Pain intensity was also limited to severe cases. It has therefore become difficult to classify existing occipital neuralgia, whose main symptom is persistent pain rather than paroxysmal stabbing pain. Occipital neuralgia is classified as either idiopathic or secondary. Secondary occipital neuralgia is caused by structural lesions innervating the trigeminocervical complex (TCC) in the upper spinal cord, the dorsal root of second cervical cord, and the greater occipital nerve (GON).Discussion and Conclusion: Although idiopathic occipital neuralgia has no cause, the entrapment of the GON in the tendinous aponeurotic attachment of the trapezius muscle at the superior nuchal line has recently been proposed as an etiology. Chronic, irritating afferent input of occipital neuralgia caused by entrapment of the GON seems to be associated with sensitization and hypersensitivity of the second-order neurons in the TCC receiving convergent input from trigeminal and occipital structures. TCC sensitization induces referred pain in the facial trigeminal area.

Abstract:The Raman optical radar measurements of the atmosphere presented demonstrate that the technique may be used to obtain quantitative measurements of the spatial distribution of individual atmospheric molecular trace constituents, in particular water vapor, as well as those of the major constituents. In addition, it is shown that monitoring Raman signals from atmospheric nitrogen aids in interpreting elastic scattering measurements by eliminating attenuation effects. In general, the experimental results show good agreement with independent meteorological measurements. Finally, experimental data are utilized to estimate the Raman backscatter cross section for water vapor excited at 3471.5 A as sigmaH(2)O/sigmaN(2) = 3.8 +/- 25%.

Abstract:Objective To investigate the anatomical structures of the neurovascular complex in the cerebellopontine angle region to offer anatomical materials for clinical microvascular decompression.Methods A total of 20 adult cadaveric heads fixed in 10% formalin were used for this study.After cutting cerebrum from the midbrain and the dura covering the cerebellum,the trans-suboccipital retrosigmoid approach to the CPA region was simulated.Dissections were observed and photographed gradually.Results The upper neurovascular complex included trochlear nerve,trigeminal nerve and superior cerebellar artery(SCA),superior petrosal vein,superior petrosal sinus,and anteroinferior cerebellar artery(AICA),et al.The trunk loop and its originating point of the branch of SCA usually compressed the upper trigeminal nerve entering/exiting zone of brainstem.The middle neurovascular complex mainly included abducens,facial,vestibulocochlear nerve,anteroinferior cerebellar artery(AICA),and posteroinferior cerebellar artery(PICA),et al.AICA often folded and entered the internal acoustic meatus with a labyrinthine branch.The lower neurovascular complex included glossopharyngeal,vagus,accessory and hypoglossal nerve,PICA,vertebral artery(VA),et al.Conclusion More than one compressive vessels and compressive points can be identified.A detailed anatomical study could improve the effect of microvascular compression operation.
Accessory Nerves of the Forehead: A Newly Discovered Frontotemporal Neurovascular Bundle and Its Implications in the Treatment of Frontal Headache, Migraine Surgery, and Cosmetic Temple Filler Injection

Abstract:Background: Surgical decompression of peripheral cranial and spinal nerves at several anatomically studied trigger sites has demonstrated significant efficacy in bringing permanent relief to migraine sufferers. In their experience performing frontal nerve decompression on migraine patients, the authors noticed a previously undescribed accessory nerve and vessel in the frontotemporal area, and report its implication in migraine surgery and cosmetic filler injection. Methods: A retrospective review of 113 patients who underwent frontal migraine decompression surgery with the senior author at the University of Texas Southwestern Medical Center from July of 2012 to May of 2016 was performed. For the included 76 patients, measurements of this nerve had been taken intraoperatively using high-definition endoscopic assistance, and topographic measurements were correlated with endoscopic location of the nerve. Results: This frontotemporal nerve (FTN) was present in 55 percent, and the bilateral incidence was 57 percent of those. An accompanying vessel was also present in 81 percent of nerve complexes. Both nerve and vessel varied in size. A large vessel was present in 8 percent of all patients, and a medium vessel was present in 20 percent. Consistently, the nerve exited a foramen in the frontal bone on average 3.4 ± 0.47 cm superior to the lateral canthus. Conclusions: The identification and proper avulsion neurectomy of this newly described sensory FTN may lead to better surgical response rate during migraine surgery. In addition, this nerve should be considered during nerve block and botulinum toxin injections in migraine treatment. The existence of the accompanying vessel could have significant implications in the safety of filler and fat injections to this area.
Evaluation of the Anatomical Reference Point in Posterior Minimally Invasive Atlantoaxial Spine Surgery: A Cadaveric Anatomical Study

Abstract:The intersection of the greater occipital nerve with the inferior border of the inferior cephalic oblique muscle and the muscles was revealed during dissection, including trapezius, splenius capitis, semispinalis capitis, suboccipital triangle. Objective Minimally invasive atlantoaxial surgery offers the benefits of reduced trauma and quicker recovery. Previous studies have focused on feasibility and technical aspects, but the lack of comprehensive safety information has limited its availability and widespread use. This study proposes to define the feasibility and range of surgical safety using the intersection of the greater occipital nerve and the inferior border of the inferior cephalic oblique as a reference point. Methods Dissection was performed on 10 fresh cadavers to define the anatomical reference point as the intersection of the greater occipital nerve and the inferior border of the inferior cephalic oblique muscle. The study aimed to analyze the safety range of minimally invasive atlantoaxial fusion surgery by measuring the distance between the anatomical reference point and the transverse foramen of the axis, the distance between the anatomical reference point and the superior border of the posterior arch of the atlas, and the distance between the anatomical reference point and the spinal canal. Measurements were compared using Student's t test. Results The point where the occipital greater nerve intersects with the inferior border of the inferior cephalic oblique muscle was defined as the anatomical marker for minimally invasive posterior atlantoaxial surgery. The distance between this anatomical marker and the transverse foramen of the axis was measured to be 9.32 ± 2.04 mm. Additionally, the distance to the superior border of the posterior arch of the atlas was found to be 21.29 ± 1.93 mm, and the distance to the spinal canal was measured to be 11.53 ± 2.18 mm. These measurement results can aid surgeons in protecting the vertebral artery and dura mater during minimally invasive posterior atlantoaxial surgery. Conclusions The intersection of the greater occipital nerve with the inferior border of the inferior cephalic oblique muscle is a safe and reliable anatomical landmark in minimally invasive posterior atlantoaxial surgery.

Definition of an Effective Site for Greater and Third Occipital Nerve Block in the Nuchal Region: A Fetal Cadaver Study

Abstract:Aim: To determine the localization of the greater occipital nerve and the third occipital nerve according to palpable bone landmarks and their relationship with muscles in the suboccipital region and to define an effective zone for clinical approaches. Material and methods: This study was conducted on 15 fetal cadavers. Bone landmarks to be used as reference were determined by palpation, and measurements were taken before dissection. The location, relationship and variation of these nerves and muscles (trapezius, semispinalis capitis, obliquus capitis inferior) were noted. Results: It was detected that the nape triangular area formed between the reference points was scalene in males and isosceles in females. It was found that greater occipital nerve pierced aponeurosis of trapezius and passed under obliquus capitis inferior in all fetal cadavers and 96.7% of them had pierced semispinalis capitis. It was determined that the greater and third occipital nerve pierced the trapezius aponeurosis approximately 2 cm below the reference line and 0.5-1 cm lateral to the midline. Conclusion: One of the important factors for the high success rate in suboccipital invasive procedures in the pediatric population is to know the location of the nerves in the region correctly. We believe that the results of this study will contribute to the literature.
