Abstract:BACKGROUND:Precisely localizing the seizure onset zone (SOZ) is critical for focal epilepsy surgery. Existing methods mainly focus on high-frequency activities in stereo-electroencephalography, but often fail when seizures are not driven by high-frequency activities. Recognized as biomarkers of epileptic seizures, ictal spikes in SOZ induce epileptiform discharges in other brain regions. Based on this understanding, we aim to develop a universal algorithm to localize SOZ and investigate how ictal spikes within the SOZ induce seizures. METHODS:We proposed a novel metric called standard deviation of spike amplitude (SDSA) and utilized channel-averaged SDSA to describe seizure processes and detect seizures. By integrating SDSA values in specific intervals, the score for each channel located within SOZ was calculated. Channels with high SOZ scores were clustered as SOZ. The localization accuracy was asserted using area under the receiver operating characteristic (ROC) curve. Further, we analyzed early ictal signals from SOZ channels and investigated factors influencing their duration to reveal the seizure inducing conditions. RESULTS:We analyzed data from 15 patients with focal epilepsy. The channel-averaged SDSA successfully detected all 28 seizures without false alarms. Using SDSA integration, we achieved precise SOZ localization with an average area under ROC curve (AUC) of 0.96, significantly outperforming previous methods based on high-frequency activities. Further, we discovered that energy of ictal spikes in SOZ was concentrated at a specific frequency distributed in [6, 12 Hz]. Additionally, we found that the higher the energy per second in this frequency band, the faster ictal spikes could induce seizures. CONCLUSION:The SDSA metric offered precise SOZ localization with robustness and low computational cost, making it suitable for clinical practice. By studying the propagation patterns of ictal spikes between the SOZ and non-SOZ, we suggest that ictal spikes from SOZ need to accumulate energy at a specific central frequency to induce epileptic spikes in non-SOZ, which may have significant implications for understanding the seizure onset pattern.

Abstract:Intracranial electroencephalography (iEEG) provides the best precision in estimating the location and boundary of an epileptogenic zone. Analysis of iEEG in the routine EEG frequency range (0.5–70 Hz) remains the basis in clinical practice. Low-voltage fast activity is the most commonly reported ictal onset pattern in neocortical epilepsy, and low-frequency high-amplitude repetitive spiking is the most commonly reported ictal onset pattern in mesial temporal lobe epilepsy. Recent studies using wideband EEG recording have demonstrated that examining higher (80–1000 Hz) and lower (0.016–0.5 Hz) EEG frequencies can provide additional diagnostic information and help to improve the surgical outcome. In addition, novel computational techniques of iEEG signal analysis have provided new insights into the epileptic network. Here, we review some of these recent advances. Although these sophisticated and advanced techniques of iEEG analysis show promise in localizing the epileptogenic zone, their utility needs to be further validated in larger studies.
Multimodal and Quantitative Analysis of the Epileptogenic Zone in the Pre-Surgical Evaluation of Drug-Resistant Focal Epilepsy

Abstract:Surgical resection for epilepsy often fails due to incomplete Epileptogenic Zone (EZ) localization from standard electroencephalography (EEG), stereo-EEG (SEEG), and Magnetic Resonance Imaging (MRI). Subjective interpretation based on interictal, or ictal recordings limits conventional EZ localization. This study employs multimodal analysis using high-density-EEG (HDEEG), Magnetoencephalography (MEG), functional-MRI (fMRI), and SEEG to overcome these limitations in a patient with drug-resistant MRI-negative focal epilepsy. A teenage boy with drug-resistant epilepsy underwent evaluation. HDEEG, MEG, fMRI, and SEEG were used, with a novel HDEEG-cap facilitating simultaneous EEG-MEG and EEG-fMRI recordings. Electrical and magnetic source imaging were performed, and fMRI data were analysed for homogenous regions. SEEG analysis involved spike detection, spike timing analysis, ictal fast activity quantification, and Granger-based connectivity analysis. Non-invasive sessions revealed consistent interictal source imaging results identifying the EZ in the right anterior cingulate cortex. EEG-fMRI highlighted broader activation in the right cingulate cortex. SEEG analysis localized spikes and fast activity in the right anterior and posterior cingulate gyri. Multi-modal analysis suggested the EZ in the right frontal lobe, primarily involving the anterior and mid-cingulate cortices. Multi-modal non-invasive analyses can optimise SEEG implantation and surgical decision-making. Invasive analyses corroborated non-invasive findings, emphasising the importance of individual-case quantitative analysis across modalities in complex epilepsy cases.

Integrating Data Across Oscillatory Power Bands Predicts the Epileptogenic Zone: the Frequency Range Explorer Epileptogenic Zone (FREEZ) Identification Algorithm

Abstract:Drug-resistant focal epilepsy affects over 25 million people globally. Identification and removal of the seizure onset site, termed the epileptogenic zone (EZ), is the most successful treatment to stop seizures in these people. Implanting electrodes into the brain to record intracranial electroencephalography (iEEG) is the gold standard test for identifying the EZ. But identification of the EZ with iEEG remains challenging in many cases. We developed a novel methodology using a stacked random forest ensemble machine learning model integrating power across delta, theta, alpha, beta, gamma, and high gamma frequencies over time to identify the EZ. In patients who were seizure-free after surgery, electrodes within the EZ showed significantly higher area under the curve for mean power over time in the first 20 seconds after a seizure compared to electrodes outside the EZ in the alpha (p = 0.0272), beta (p = 0.0263), gamma (p = 0.0013), and high gamma (p = 0.0086) ranges. Additionally, electrodes within the EZ in patients that became seizure-free after surgery had significantly higher AUC compared to electrodes marked within the EZ in patients who did not become seizure-free after surgery in the gamma (p = 0.0145) and high gamma (p = 0.0024) power ranges, but not delta (p=0.8678), theta (p=0.4956), alpha (p=0.1320) or beta (p=0.1624) ranges. A machine learning model was trained to predict electrodes within the EZ. Leave-one-out patient cross validation of the machine learning model yielded a 95.7% positive predictive value and 99.1% specificity for identifying electrodes within the epileptogenic zone, and 95.2% accuracy for predicting seizure outcome based on a planned resection. We implemented this algorithm into the open-source software tool "Reproducible Analysis and Visualization of iEEG" (RAVE) to enable users to reproduce our results and implement this methodology with new datasets, creating a software module titled FREEZ. The software facilitates quantification of the spectral power changes during seizures, including displaying time-frequency spectrograms and projecting results across patient-specific 3D brain maps. Users can also adjust parameters for visualizing multiple frequency ranges from various time regions around seizure onsets in a web-browser-based interface.

Abstract:The identification of the epileptogenic zone (EZ) boundaries is crucial for effective focal epilepsy surgery. We verify the value of a neurophysiological biomarker of focal ictogenesis, characterized by a low‐voltage fast‐activity ictal pattern (chirp) recorded with intracerebral electrodes during invasive presurgical monitoring (stereoelectroencephalography [SEEG]). The frequency content of SEEG signals was retrospectively analyzed with semiautomatic software in 176 consecutive patients with focal epilepsies that either were cryptogenic or presented with discordant anatomoelectroclinical findings. Fast activity seizure patterns with the spectrographic features of chirps were confirmed by computer‐assisted analysis in 95.4% of patients who presented with heterogeneous etiologies and diverse lobar location of the EZ. Statistical analysis demonstrated (1) correlation between seizure outcome and concordance of sublobar regions included in the EZ defined by visual analysis and chirp‐generating regions, (2) high concordance in contact‐by contact analysis of 68 patients with Engel class Ia outcome, and (3) that discordance between chirp location and the visually outlined EZ correlated with worse seizure outcome. Seizure outcome analysis confirms the fast activity chirp pattern is a reproducible biomarker of the EZ in a heterogeneous group of patients undergoing SEEG.

Localization of the epileptogenic zone based on ictal stereo-electroencephalogram: Brain network and single-channel signal feature analysis

Abstract:Accurate localization of the epileptogenic zone (EZ) is crucial for refractory focal epilepsy patients to achieve freedom from seizures following epilepsy surgery. In this study, ictal stereo-electroencephalography data from 35 patients with refractory focal epilepsy were analyzed. Effective networks based on partial directed coherence were analyzed, and a gray level co-occurrence matrix was applied to extract the time-varying features of the in-degree. These features, combined with the single-channel signal time-frequency features, including approximate entropy and line length, were used to localize the EZ based on a cluster algorithm. For all seizure-free patients (n = 23), the proposed method was effective in identifying the clinical-EZ-contacts and clinical-EZ-blocks, with an F1-score of 62.47 % and 72.18 %, respectively. The sensitivity was 96.00 % for the clinical-EZ-block identification, which provided the information for the decision-making of clinicians, prompting clinicians to focus on the identified EZ-blocks and their nearby contacts. The agreement between the EZ identified by the proposed method and the clinical-EZ was worse for non-seizure-free patients (n = 12) than for seizure-free patients. Furthermore, our method provided better results than using only brain network or single-channel signal features. This suggests that combining these complementary features can facilitate more accurate localization of the EZ.

Abstract:Background Epilepsy, characterized as a network disorder, involves widely distributed areas following seizure propagation from a limited onset zone. Accurate delineation of the epileptogenic zone (EZ) is crucial for successful surgery in drug-resistant focal epilepsy. While visual analysis of scalp electroencephalogram (EEG) primarily elucidates seizure spreading patterns, we employed brain connectivity techniques and graph theory principles during the pre-ictal to ictal transition to define the epileptogenic network. Method Cortical sources were reconstructed from 40-channel scalp EEG in five patients during pre-surgical evaluation for focal drug-resistant epilepsy. Temporal Granger connectivity was estimated ten seconds before seizure and at seizure onset. Results have been analyzed using some centrality indices taken from Graph theory (Outdegree, Hubness). A new lateralization index is proposed by taking into account the sum of the most relevant hubness values across left and right regions of interest. Results In three patients with positive surgical outcomes, analysis of the most relevant Hubness regions closely aligned with clinical hypotheses, demonstrating consistency in EZ lateralization and location. In one patient, the method provides unreliable results due to the abundant movement artifacts preceding the seizure. In a fifth patient with poor surgical outcome, the proposed method suggests a wider epileptic network compared with the clinically suspected EZ, providing intriguing new indications beyond those obtained with traditional electro-clinical analysis. Conclusions The proposed method could serve as an additional tool during pre-surgical non-invasive evaluation, complementing data obtained from EEG visual inspection. It represents a first step toward a more sophisticated analysis of seizure onset based on connectivity imbalances, electrical propagation, and graph theory principles.

Methods for Identifying Epilepsy Surgery Targets Using Invasive EEG: A Systematic Review

Abstract:Background: The pre-surgical evaluation for drug-resistant epilepsy achieves seizure freedom in only 50–60% of patients. Efforts to identify quantitative intracranial EEG (qEEG) biomarkers of epileptogenicity are needed. This review summarizes and evaluates the design of qEEG studies, discusses barriers to biomarker adoption, and proposes refinements of qEEG study protocols. Methods: We included exploratory and prediction prognostic studies from MEDLINE and Scopus published between 2017 and 2023 that investigated qEEG markers for identifying the epileptogenic network as the surgical target. Cohort parameters, ground truth references, and analytical approaches were extracted. Results: Out of 1789 search results, 128 studies were included. The study designs were highly heterogeneous. Half of the studies included a non-consecutive cohort, with sample sizes ranging from 2 to 166 patients (median of 16). The most common minimum follow-up was one year, and the seizure onset zone was the most common ground truth. Prediction studies were heterogeneous in their analytical approaches, and only 25 studies validated the marker through post-surgical outcome prediction. Outcome prediction performance decreased in larger cohorts. Conversely, longer follow-up periods correlated with higher prediction accuracy, and connectivity-based approaches yielded better predictions. The data and code were available in only 9% of studies. Conclusions: To enhance the validation qEEG markers, we propose standardizing study designs to resemble clinical trials. This includes using a consecutive cohort with long-term follow-up, validating against surgical resection as ground truth, and evaluating markers through post-surgical outcome prediction. These considerations would improve the reliability and clinical adoption of qEEG markers.

Abstract:Aminoglycoside‐phosphotransferases contain several conserved amino acid sequence motifs. Using hydroxylamine we have obtained five independent missense mutations within the aphA‐2 gene of transposon Tn5. Four of the mutations dramatically reduced antibiotic resistance. Two were identical and included the replacement of His‐188 with Tyr. One other resulted from the replacement of Gly‐189 with Asp. These three mutations map within the first of the conserved motifs. The replacement of Asp‐261 with Asn maps to the third of these structural motifs. A mutation diminishing but not eliminating aminoglycoside resistance resulted from replacement of the conserved Val‐36 with Met. By site‐directed mutagenesis three additional mutants were obtained: His‐188 was replaced with Leu and Ser, and Arg‐211 within the second conserved motif was substituted by Gly. All three showed reduced levels of resistance to kanamycin. Our results show that these conserved motifs are essential for the biological activity of aminoglycoside phosphotransferases.
The "Connectivity Epileptogenicity Index " (cEI), a method for mapping the different seizure onset patterns in StereoElectroEncephalography recorded seizures

Abstract:Objective: Localization of epileptogenic brain regions is a crucial aim of pre-surgical evaluation of patients with drug-resistant epilepsy. Several methods have been proposed to identify the seizure onset zone, particularly based on the detection of fast activity. Most of these methods are inefficient to detect slower patterns of onset that account for 20-30% of commonly observed Stereo-Electro-Encephalography (SEEG) patterns. We seek to evaluate the performance of a new quantified measure called the Connectivity Epileptogenicity Index (cEI) in various types of seizure onset patterns. Methods: We studied SEEG recorded seizures from 51 patients, suffering from focal drug-resistant epilepsy. The cEI combines a directed connectivity measure ("out-degrees") and the original epileptogenicity index (EI). Quantified results (Out-degrees, cEI and EI) were compared to visually defined seizure onset zone (vSOZ). We computed recall (sensitivity) and precision (proportion of correct detections within all detections) with vSOZ as a reference. The quality of the detector was quantified by the area under the precision-recall curve. Results: Best results (in terms of match with vSOZ) were obtained for cEI. For seizures with fast onset patterns, cEI and EI gave comparable results. For seizures with slow onset patterns, cEI gave a better estimation of the vSOZ than EI. Conclusions: We observed that cEI discloses better performance than EI when seizures starts with slower patterns and equal to EI in seizures with fast onset patterns. Significance: The cEI is a promising new tool for epileptologists, that helps characterizing the seizure onset zone in sEEG, in a robust way despite variations in seizure onset patterns.
Quantifying interictal intracranial EEG to predict focal epilepsy

Abstract:Intracranial EEG (IEEG) is used for 2 main purposes, to determine: (1) if epileptic networks are amenable to focal treatment and (2) where to intervene. Currently these questions are answered qualitatively and sometimes differently across centers. There is a need for objective, standardized methods to guide surgical decision making and to enable large scale data analysis across centers and prospective clinical trials. We analyzed interictal data from 101 patients with drug resistant epilepsy who underwent presurgical evaluation with IEEG. We chose interictal data because of its potential to reduce the morbidity and cost associated with ictal recording. 65 patients had unifocal seizure onset on IEEG, and 36 were non-focal or multi-focal. We quantified the spatial dispersion of implanted electrodes and interictal IEEG abnormalities for each patient. We compared these measures against the 5 Sense Score (5SS), a pre-implant estimate of the likelihood of focal seizure onset, and assessed their ability to predict the clinicians choice of therapeutic intervention and the patient outcome. The spatial dispersion of IEEG electrodes predicted network focality with precision similar to the 5SS (AUC = 0.67), indicating that electrode placement accurately reflected pre-implant information. A cross-validated model combining the 5SS and the spatial dispersion of interictal IEEG abnormalities significantly improved this prediction (AUC = 0.79; p<0.05). The combined model predicted ultimate treatment strategy (surgery vs. device) with an AUC of 0.81 and post-surgical outcome at 2 years with an AUC of 0.70. The 5SS, interictal IEEG, and electrode placement were not correlated and provided complementary information. Quantitative, interictal IEEG significantly improved upon pre-implant estimates of network focality and predicted treatment with precision approaching that of clinical experts.

Interictal SEEG Resting‐State Connectivity Localizes the Seizure Onset Zone and Predicts Seizure Outcome

Abstract:This work presents a novel brain network analysis to identify the seizure onset zone and predict seizure outcome with an accuracy of around 90% using short‐time resting‐state stereotactic‐electroencephalography (SEEG) data, suggesting that brief resting‐state SEEG data has tremendous clinical values in diagnosis and aiding surgical planning in drug‐resistant epilepsy without requiring long‐term ictal recordings. Localization of epileptogenic zone currently requires prolonged intracranial recordings to capture seizure, which may take days to weeks. The authors developed a novel method to identify the seizure onset zone (SOZ) and predict seizure outcome using short‐time resting‐state stereotacticelectroencephalography (SEEG) data. In a cohort of 27 drug‐resistant epilepsy patients, the authors estimated the information flow via directional connectivity and inferred the excitation‐inhibition ratio from the 1/f power slope. They hypothesized that the antagonism of information flow at multiple frequencies between SOZ and non‐SOZ underlying the relatively stable epilepsy resting state could be related to the disrupted excitation‐inhibition balance. They found flatter 1/f power slope in non‐SOZ regions compared to the SOZ, with dominant information flow from non‐SOZ to SOZ regions. Greater differences in resting‐state information flow between SOZ and non‐SOZ regions are associated with favorable seizure outcome. By integrating a balanced random forest model with resting‐state connectivity, their method localized the SOZ with an accuracy of 88% and predicted the seizure outcome with an accuracy of 92% using clinically determined SOZ. Overall, this study suggests that brief resting‐state SEEG data can significantly facilitate the identification of SOZ and may eventually predict seizure outcomes without requiring long‐term ictal recordings.

However, SEEG is susceptible to the sampling bias, where no SEEG recording is taken within a circumscribed EZ.METHODS: Nine patients with medically refractory epilepsy underwent SEEG recording, and brain resection got positive outcomes. Ictal neuronal currents were estimated by distributed source modeling using the SEEG data and individual's anatomical magnetic resonance imaging. Using a retrospective leave-one-out data sub-sampling, we evaluated the sensitivity and specificity of the current estimates using MRI after surgical resection or radio-frequency ablation.RESULTS: The sensitivity and specificity in detecting the EZ were indistinguishable from either the data from all electrodes or the sub-sampled data (rank sum test: rank sum = 23719, p = 0.13) when at least one remaining electrode contact was no more than 20 mm away.CONCLUSIONS: The distributed neuronal current estimates of ictal SEEG data can mitigate the challenge of delineating the boundary of the EZ in cases of missing an electrode implanted within the EZ and a required second SEEG exploration.SIGNIFICANCE: Distributed source modeling can be a tool for clinicians to infer the EZ by allowing for more flexible planning of the electrode implantation route and minimizing the number of electrodes.

Abstract:Objective To deconstruct the epileptogenic networks of patients with drug‐resistant epilepsy (DRE) using source functional connectivity (FC) analysis; unveil the FC biomarkers of the epileptogenic zone (EZ); and develop machine learning (ML) models to estimate the EZ using brief interictal electroencephalography (EEG) data. Methods We analyzed scalp EEG from 50 patients with DRE who had surgery. We reconstructed the activity (electrical source imaging [ESI]) of virtual sensors (VSs) across the whole cortex and computed FC separately for epileptiform and non‐epileptiform EEG epochs (with or without spikes). In patients with good outcome (Engel 1a), four cortical regions were defined: EZ (resection) and three non‐epileptogenic zones (NEZs) in the same and opposite hemispheres. Region‐specific FC features in six frequency bands and three spatial ranges (long, short, inner) were compared between regions (Wilcoxon sign‐rank). Abstract:Stereoelectroencephalography (SEEG) is an invasive surgical procedure to record the electrical activities in cortical brain regions, aiming at identifying the Epileptogenic Zone (EZ) in patients with drug-resistant epilepsy. To improve the accuracy of the EZ definition, SEEG analysis can be supported by computational tools, among which the Epileptogenic Index (EI) represents the most common solution. However, the scientific community has still not found an agreement on which quantitative biomarkers can characterize the cortical sites within the EZ. In this work, we design a new algorithm, named Desynchronization Index (DI), to assist neurophysiologists in SEEG interpretation. Our algorithm estimates the effective connectivity between cortical sites and hypothesizes that the EZ is identified by those sites getting abnormally desynchronized from the network during the seizure generation. We test the proposed method over a SEEG dataset of 10 seizures, comparing its accuracy in terms of EZ definition against the EI algorithm and clinical ground truth. Our results indicate that the DI algorithm underscores specific connectivity dynamics that can hardly be identified with a pure visual analysis, increasing sensitivity in detecting epileptogenic cortical sites.

Abstract:The effect of the nucleic acid to hydroxylapatite load ratio (μg/g) has been studied. It was found that as the load ratio was increased from 1 to 100 μg/g there was little change in the elution molarity of the nucleic acids whereas above 1000 μg/g the change could be substantial. Different hydroxylapatite preparations and different nucleic acids gave similar results but the load ratio dependence of the elution molarity was greater at 70 °C than at room temperature.

Abstract:Objective: High frequency oscillations (HFOs) in intracranial electroencephalography (iEEG) recordings are considered as promising clinical biomarkers of epileptogenic regions in the brain. The aim of this study is to improve and automatize the detection of HFOs by exploring the time-frequency content of iEEG and to investigate the seizure onset zone (SOZ) detection accuracy during the sleep, awake and pre-ictal states in patients with epilepsy, for the purpose of assisting the localization of SOZ in clinical practice. Approach: Ten-minute iEEG segments were defined during different states in eight patients with refractory epilepsy. A three-stage algorithm was implemented to detect HFOs in these segments. First, an amplitude based initial detection threshold was used to generate a large pool of HFO candidates. Then distinguishing features were extracted from the time and time-frequency domain of the raw iEEG and used with a Gaussian mixture model clustering to isolate HFO events from other activities. The spatial distribution of HFO clusters was correlated with the seizure onset channels identified by neurologists in seven patient with good surgical outcome. Main results: The overlapping rates of localized channels and seizure onset locations were high in all states. The best result was obtained using the iEEG data during sleep, achieving a sensitivity of 81%, and a specificity of 96%. The channels with maximum number of HFOs identified epileptogenic areas where the seizures occurred more frequently. Significance: The current study was conducted using iEEG data collected in realistic clinical conditions without channel pre-exclusion. HFOs were investigated with novel features extracted from the entire frequency band, and were correlated with SOZ in different states. The results indicate that automatic HFO detection with unsupervised clustering methods exploring the time-frequency content of raw iEEG can be efficiently used to identify the epileptogenic zone with an accurate and efficient manner.

Abstract:Background Stereo-electroencephalography (SEEG) uses a three-dimensional configuration of depth electrodes to localize epileptiform activity, but traditional analysis of SEEG is spatially restricted to the point locations of the electrode contacts. Interpolation of brain activity between contacts might allow for three-dimensional representation of epileptiform activity and avoid pitfalls of SEEG interpretation. Objective The goal of this study was to validate SEEG-based interictal source localization and assess the ability of this technique to monitor far-field activity in non-implanted brain regions. Methods Interictal epileptiform discharges were identified on SEEG in 26 patients who underwent resection, ablation, or disconnection of the suspected epileptogenic zone. Dipoles without (free) and with (scan) gray matter restriction, and current density (sLORETA and SWARM methods), were calculated using a finite element head model. Source localization results were compared to the conventional irritative zone (IZ) and the surgical treatment volumes (TV) of seizure-free vs. non-seizure-free patients. Results The median distance from dipole solutions to the nearest contact in the conventional IZ was 7 mm (interquartile range 4–15 mm for free dipoles and 4–14 mm for scan dipoles). The IZ modeled with SWARM predicted contacts within the conventional IZ with 83% (75–100%) sensitivity and 94% (88–100%) specificity. The proportion of current within the TV was greater in seizure-free patients ( P = 0.04) and predicted surgical outcome with 45% sensitivity and 93% specificity. Dipole solutions and sLORETA results did not correlate with seizure outcome. Addition of scalp EEG led to more superficial modeled sources ( P = 0.03) and negated the ability to predict seizure outcome ( P = 0.23). Removal of near-field data from contacts within the TV resulted in smearing of the current distribution ( P = 0.007) and precluded prediction of seizure freedom ( P = 0.20). Conclusions Source localization accurately represented interictal discharges from SEEG. The proportion of current within the TV distinguished between seizure-free and non-seizure-free patients when near-field recordings were obtained from the surgical target. The high prevalence of deep sources in this cohort likely obscured any benefit of concurrent scalp EEG. SEEG-based interictal source localization is useful in illustrating and corroborating the epileptogenic zone. Additional techniques are needed to localize far-field epileptiform activity from non-implanted brain regions.
