A Painless Growing Abdominal Mass in a Patient with Systemic Lupus Erythematosus

Abstract:A 59-year-old woman was admitted to Peking University People's Hospital with a 6-month history of a palpable growing abdominal mass. She had no fever, abdominal pain, hematochezia, or vaginal bleeding but had a 13-year history of systemic lupus erythematosus, thrombocytopenia, and hypertension. Physical examination showed a large, painless, firm abdominal mass with limited mobility. Complete blood count showed a hemoglobin concentration of 109 g/L, neutrophilic leukocytosis (9.6 × 109/L), and thrombocytopenia (47 × 109/L). Antinuclear antibodies were present at a titer of 1:640 and antidouble-stranded DNA antibody was positive. Blood tests for carbohydrate antigen (CA) 19-9, carcinoembryonic antigen, CA-125, and alpha-fetoprotein were performed: only CA-125 was elevated. The mass measuring 12 cm by 10 cm by 8 cm had been detected by laparoscopy in another hospital 4 months ago. It was tightly associated with the intestine, with tortuous blood vessels on the surface. However, the surgery had not been done because of her low platelets and extremely high bleeding risk. Contrast-enhanced computed tomography revealed an irregular cystic mass, measuring 12.1 cm by 11.2 cm by 14.8 cm (Figure 1A), with heterogeneous enhancement. The cystic mass contained thickened septa. The patient underwent surgery (the specimen seen in Figure 1B), and severe hemorrhage in the postresection mass was shown. Histopathological evaluation surprisingly revealed hemorrhage, with extensive inflammatory infiltrate comprising mainly degenerative mononuclear cells. Both irregular vascular hyperplasia and focal lymphocytic infiltration at the vessel wall were observed (Figure 1C). All lines of evidence supported that the mass was caused by lupus mesenteric vasculitis (LMV). She was treated with glucocorticoids 30 mg once a day, cyclosporine A 50 mg twice a day, and hydroxychloroquine 200 mg twice a day. At a 3-month follow-up visit, the patient felt well and the disease was controlled (postoperative laboratory data seen in Supporting Information: Table S1). LMV usually presents as acute abdominal pain with sudden onset and diffuse localization.1 This case illustrated that LMV might not present with typical features but showed as chronic continued hemorrhage in the mesentery and organization of local hematoma, which caused the painless growing abdominal mass.
