Abstract:Objective:To investigate the application feasibility of Region 4 Stork (R4S) system, an international collaborative newborn screening data platform, combined with cut-off value analysis in the neonatal screening for very long chain acyl-CoA dehydrogenase deficiency (VLCADD) by tandem mass spectrometry (MS/MS).Methods:The retrospective study was performed in 2, 040 072 neonates screened by MS/MS in Neonatal Screening Center of Zhejiang Province, China from October 2013 to July 2018. Nine hundred and ten cases were determined and identified as suspected positive VLCADD neonates by traditional cut-off method of tandem mass spectrometry. The original data of these 910 screened neonates were further analyzed by R4S system. Based on clinical diagnosis and ACADL gene test results, the screening efficiency between two methods was statistically compared.Results:The data of 910 suspected VLCADD-positive cases interpreted by cut-off method were further analyzed by R4S system, and the positive interpretation was reduced to 238 cases (including 9 confirmed positive cases). A total of 16 different mutations were found in ACADL gene sequencing among the confirmed children. The screening false positive rate (FPR) declined from 0.44‰ (901/2 040 072) to 0.11‰ (229/2 040 072), the rate of positive predictive value (PPV) increased from 0.99% (9/910) to 3.78% (9/238), and the specificity increased from 99.96% (2 039 162/2 040 063) to 99.99% (2 039 834/2 040 063). There was a statistically significant difference between cut-off method alone and cut-off method combined R4S system analysis (χ2=393.5, P<0.05). Conclusions:The R4S system combined with cut-off method applied in VLCADD neonatal screening by MS/MS can effectively improve screening performance, reduce false positive rate, and has certain value in clinical application.
A Multi-Center Study of Biochemical and Hotspot Gene Screening for Neonatal Genetic Metabolic Diseases

Abstract:Objective:To investigate the efficiency of biochemical screening and hotspot gene screening in the detection of neonatal inherited metabolic diseases.Methods:This was a prospective multi-center study.The study was carried out on 21 442 neonatal samples collected from 12 hospitals in 10 provinces from November 2020 to November 2021.The results of biochemical screening and hotspot gene screening were analyzed jointly.Biochemical screening methods included glucose-6-phosphate dehydrogenase deficiency enzyme activity assay and neonatal tandem mass spectrometry.Genetic screening analysis involved 135 genes associated with 75 neonatal diseases.Results:Of all the 21 442 neonates enrolled in the study, 21 205 were subject to biochemical screening.A total of 813 cases were positive in the initial screening, and 0.45% of them (95 cases) were diagnosed after recall.All the 21 442 neonates underwent gene screening.About 168 positive cases were detected in the initial screening, and 0.73% (156 cases) of them were confirmed finally.Biochemical and genetic screening improved the detection sensitivity of such diseases as primary carnitine deficiency, neonatal intrahepatic cholestasis caused by citrin deficiency, and 2-methylbutyrylglycinemia.Moreover, biochemical and genetic screening enabled the detection of more diseases, including the common single-gene genetic diseases such as thalassemia and Wilson disease.Conclusions:In neonatal screening, the combination of biochemical screening and gene screening expands the number of diseases detected and improve screening efficiency.

Abstract:Tandem mass spectrometry is an advanced biochemical analysis method and has been widely used in screening of inherited metabolic disorders (IMDs). Obtained examination results are filtered by cutoff values and then interpreted based on doctor’s knowledge to get diagnoses. However, cutoff-based approaches have difficulties with the correlations of multiple metabolites. Doctor’s experiences affect the diagnostic decision-making as well. The rapidly increasing availability of newborn screening data (1.5M cases in this study) enables the application of machine learning (ML) techniques to provide more accurate diagnoses of IMDs compared to simple cutoff values. We investigated two tasks in this study, i.e. complicated patterns between metabolites and better auxiliary diagnostic means. Experimental results show that novel metabolic patterns found in the study are effective and meaningful. Integrating ML techniques with these patterns improved predictive performance compared to existing diagnostic methods, suggesting ML techniques are becoming valuable as auxiliary diagnostic tools.

Abstract:Newborn screening of inherited metabolic diseases by tandem mass spectrometry is flourishing in our country.With expansion of the coverage and spectrum of diseases, it is important to strengthen the quality management, optimize the performance and reduce the false rate currently, such as quality assurance of the specimens, quality control of the process, quality verification of the test procedure, quality evaluation of the laboratory and quality optimization of the interpretation method. Along with development of the equipment, software and project, promotion of regional collaboration by data and experience sharing will be more critical in the future, and national neonatal screening by tandem mass spectrometry will step into a new stage.

Abstract:Background An elevated level of creatine kinase (CK) is usually the primary screening marker for Duchenne muscular dystrophy (DMD)/Becker muscular dystrophy (BMD). This study investigated the clinical application of next-generation sequencing (NGS) in newborns with a possible diagnosis of DMD/BMD in the neonatal intensive care unit (NICU). Methods NGS data from the NICU between June 1, 2016, and June 30, 2020, were reanalyzed by an in-house pipeline. Other methods confirmed the genetic findings, and clinical follow-up was performed until August 1, 2020. Results Of the 10,481 newborns, 19 (0.18%, 19/10,481) cases with pathogenic variations of the DMD gene were identified, including 13 (68.4%, 13/19) deletions, 4 (21.1%, 4/19) duplications, and 2 (10.5%, 2/19) nonsense mutations. Eight of the cases were diagnosed with DMD. Therapeutic strategies were modified for these patients. Six cases were diagnosed with BMD. Five patients except for 1 deceased patient were further followed-up, and clinical management was adjusted based on the clinical symptoms. The remaining 5 cases were indeterminate for DMD and BMD. Genetic counseling and further follow-up were performed or suggested. Conclusions Our study showed that DMD/BMD could be diagnosed earlier in the neonatal stage before the typical clinical symptoms appear. Early diagnosis may provide an opportunity for guiding the care and treatment of patients. However, ethical issues need to be kept in mind in the process of genetic counseling.
Incorporating second-tier genetic screening for multiple acyl-CoA dehydrogenase deficiency

Abstract:Background: Newborn screening (NBS) for multiple acyl-CoA dehydrogenase deficiency (MADD) has poor sensitivity. This study aimed to evaluate the feasibility of incorporating second-tier genetic screening for MADD. Methods: A total of 453,390 newborns were screened for inherited metabolic disorders using tandem mass spectrometry from January 2017 to May 2022. A matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) assay was developed to identify 23 common ETFDH variants and used for second-tier screening of MADD. Results: Overall, 185 newborns with suspected MADD received second-tier genetic screening. Thirty-three (17.8 %) newborns with positive results, of which 7 were homozygotes, 5 were compound heterozygotes, 21 were heterozygotes. Further genetic analysis revealed that 6 of the 21 newborns had a second ETFDH variant. Therefore, 18 patients were finally diagnosed with MADD, with a positive predictive value of 9.73 %. The detection rate and diagnostic rate of MALDI-TOF MS assay were 83.33 % and 66.67 %, respectively. Thus the incidence of MADD in our population was estimated at 1:25,188. Nine different ETFDH variants were identified in MADD patients. The most common ETFDH variant being c.250G > A with an allelic frequency of 47.22 %, followed by c.524G > A (13.89 %) and c.998A > G (13.89 %). All patients had elevation of multiple acylcarnitines at NBS. However, seven patients had normal acylcarnitine levels and two patients showed mild elevation of only two acylcarnitines during the recall review. Conclusion: We have established a high throughput MALDI-TOF MS assay for MADD screening. Half of the MADD patients would not be detected under conventional screening protocols. Incorporating second-tier genetic screening into the current NBS could improve the performance of MADD NBS.

Abstract:Background: Neonatal screening for inherited metabolic diseases (IMDs) has been revolutionized by tandem mass spectrometry (MS/MS). This study aimed to enhance neonatal screening for IMDs using machine learning (ML) techniques. Methods: The study involved the analysis of a comprehensive dataset comprising 309,102 neonatal screening records collected in the Ningbo region, China. An advanced ML system model, encompassing nine distinct algorithms, was employed for the purpose of predicting the presence of 31 different IMDs. The model was compared with traditional cutoff schemes to assess its diagnostic efficacy. Additionally, 180 suspected positive cases underwent further evaluation. Results: The ML system exhibited a significantly reduced positive rate, from 1.17% to 0.33%, compared to cutoff schemes in the initial screening, minimizing unnecessary recalls and associated stress. In suspected positive cases, the ML system identified 142 true positives with high sensitivity (93.42%) and improved specificity (78.57%) compared to the cutoff scheme. While false negatives emerged, particularly in heterozygous carriers, our study revealed the potential of the ML system to detect asymptomatic cases. Conclusion: This research provides valuable insights into the potential of ML in pediatric medicine for IMD diagnosis through neonatal screening, emphasizing the need for accurate carrier detection and further research in this domain.

Abstract:Background: Long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) and mitochondrial trifunctional protein (MTP) deficiencies are rare autosomal recessive fatty acid β-oxidation disorders. Their clinical presentations are variable, and premature death is common. They are included in newborn blood spot screening programs in many countries around the world. The current process of screening, through the measurement of acylcarnitines (a metabolic by-product) in dried blood spots with tandem mass spectrometry, is subject to uncertainty regarding test accuracy. Methods: We conducted a systematic review of literature published up to 19th June 2018. We included studies that investigated newborn screening for LCHAD or MTP deficiencies by tandem mass spectrometry of acylcarnitines in dried blood spots. The reference standards were urine organic acids, blood acylcarnitine profiles, enzyme analysis in cultured fibroblasts or lymphocytes, mutation analysis, or at least 10-year follow-up. The outcomes of interest were sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). Assessment of titles, abstracts, and full-text papers and quality appraisal were carried out independently by two reviewers. One reviewer extracted study data. This was checked by a second reviewer. Results: Ten studies provided data on test accuracy. LCHAD or MTP deficiencies were identified in 23 babies. No cases of LCHAD/MTP deficiencies were identified in four studies. PPV ranged from 0% (zero true positives and 28 false positives from 276,565 babies screened) to 100% (13 true positives and zero false positives from 2,037,824 babies screened). Sensitivity, specificity, and NPV could not be calculated as there was no systematic follow-up of babies who screened negative. Conclusions: Test accuracy estimates of screening for LCHAD and MTP deficiencies with tandem mass spectrometry measurement of acylcarnitines in dried blood were variable in terms of PPVs. Screening methods (including markers and thresholds) varied between studies, and sensitivity, specificity, and NPVs are unknown.

The Reliable, Automatic Classification of Neonates in First-Tier MALDI-MS Screening for Sickle Cell Disease

Abstract:Previous research has shown that a MALDI-MS technique can be used to screen for sickle cell disease (SCD), and that a system combining automated sample preparation, MALDI-MS analysis and classification software is a relevant approach for first-line, high-throughput SCD screening. In order to achieve a high-throughput "plug and play" approach while detecting "non-standard" profiles that might prompt the misclassification of a sample, we have incorporated various sets of alerts into the decision support software. These included "biological alert" indicators of a newborn's clinical status (e. g., detecting samples with no or low HbA), and "technical alerts" indicators for the most common non-standard profiles, i.e., those which might otherwise lead to sample misclassification. We evaluated these alerts by applying them to two datasets (produced by different laboratories). Despite the random generation of abnormal spectra by one-off technical faults or due to the nature and quality of the samples, the use of alerts fully secured the process of automatic sample classification. Firstly, cases of β-thalassemia were detected. Secondly, after a visual check on the tagged profiles and reanalysis of the corresponding biological samples, all the samples were correctly reclassified without prompting further alerts. All of the samples for which the results were not tagged were well classified (i.e., sensitivity and specificity = 1). The alerts were mainly designed for detecting false-negative classifications; all the FAS samples misclassified by the software as FA (a false negative) were marked with an alert. The implementation of alerts in the NeoScreening® Laboratory Information Management System's decision support software opens up perspectives for the safe, reliable, automated classification of samples, with a visual check solely on abnormal results or samples. It should now be possible to evaluate the combination of the NeoSickle® analytical solution and the NeoScreening® Laboratory Information Management System in a real-life, prospective study of first-line SCD screening.
Medium-chain Acyl-Coa Dehydrogenase Deficiency: Neonatal Screening and Follow-Up

Abstract:OBJECTIVE:To investigate the epidemiological characteristics, phenotype, genotype, and prognosis of medium-chain acyl-CoA dehydrogenase deficiency (MCADD) in the Chinese population.METHODS:A retrospective analysis was performed for the clinical data of the neonates who underwent screening with high-performance liquid chromatography-tandem mass spectrometry from January 2009 to June 2018 and were diagnosed with MCADD by gene detection.RESULTS:A total of 2 674 835 neonates underwent neonatal screening, among whom 12 were diagnosed with MCADD. Gene detection was performed for 10 neonates with MCADD and found 13 mutation types at 16 mutation sites of the ACADM gene, among which there were 7 reported mutations (p.T150Rfs*4, p.M1V, p.R206C, p.R294T, p.G310R, p.M328V, and p.G362E), 5 novel mutations (p.N194D, p.A324P, p.N366S, c.118+3A>G, and c.387+1del G), and 1 exon 11 deletion; p.T150Rfs*4 was the most common mutation (4/16). The detection rate of mutation sites in the ACADM gene was 80%. No phenotype-genotype correlation was observed. Dietary guidance and symptomatic treatment were given after confirmed diagnosis. No acute metabolic imbalance was observed within 4-82 months of follow-up. All neonates had good prognosis except one who had brain dysplasia.CONCLUSIONS:MCADD is relatively rare in southern China, and p.T150Rfs*4 is a common mutation in the Chinese population. Cases with positive screening results should be evaluated by octanoylcarnitine C8 value and gene detection.
Establishment of an auxiliary diagnosis system of newborn screening for inherited metabolic diseases based on artificial intelligence technology and a clinical trial

Abstract: To establish a disease risk prediction model for the newborn screening system of inherited metabolic diseases by artificial intelligence technology. This was a retrospectively study. Newborn screening data (5 907 547) from February 2010 to May 2019 from 31 hospitals in China and verified data (=3 028) from 34 hospitals of the same period were collected to establish the artificial intelligence model for the prediction of inherited metabolic diseases in neonates. The validity of the artificial intelligence disease risk prediction model was verified by 360 814 newborns screening data from January 2018 to September 2018 through a single-blind experiment. The effectiveness of the artificial intelligence disease risk prediction model was verified by comparing the detection rate of clinically confirmed cases, the positive rate of initial screening and the positive predictive value between the clinicians and the artificial intelligence prediction model of inherited metabolic diseases. A total of 3 665 697 newborns screening data were collected including 3 019 cases positive data to establish the 16 artificial intelligence models for 32 inherited metabolic diseases. The single-blind experiment (=360 814) showed that 45 clinically diagnosed infants were detected by both artificial intelligence model and clinicians. A total of 2 684 cases were positive in tandem mass spectrometry screening and 1 694 cases were with high risk in artificial intelligence prediction model of inherited metabolic diseases, with the positive rates of tandem 0.74% (2 684/360 814)and 0.46% (1 694/360 814), respectively. Compared to clinicians, the positive rate of newborns was reduced by 36.89% (990/2 684) after the application of the artificial intelligence model, and the positive predictive values of clinicians and artificial intelligence prediction model of inherited metabolic diseases were 1.68% (45/2 684) and 2.66% (45/1 694) respectively. An accurate, fast, and the lower false positive rate auxiliary diagnosis system for neonatal inherited metabolic diseases by artificial intelligence technology has been established, which may have an important clinical value.

Abstract:Currently, tandem mass spectrometry-based newborn screening (NBS), which examines targeted biomarkers, is the first approach used for the early detection of maple syrup urine disease (MSUD) in newborns, followed by confirmatory genetic mutation tests. However, these diagnostic approaches have limitations, demanding the development of additional tools for the diagnosis/screening of MUSD. Recently, untargeted metabolomics has been used to explore metabolic profiling and discover the potential biomarkers/pathways of inherited metabolic diseases. Thus, we aimed to discover a distinctive metabolic profile and biomarkers/pathways for MSUD newborns using untargeted metabolomics. Herein, untargeted metabolomics was used to analyze dried blood spot (DBS) samples from 22 MSUD and 22 healthy control newborns. Our data identified 210 altered endogenous metabolites in MSUD newborns and new potential MSUD biomarkers, particularly L-alloisoleucine, methionine, and lysoPI. In addition, the most impacted pathways in MSUD newborns were the ascorbate and aldarate pathways and pentose and glucuronate interconversions, suggesting that oxidative and detoxification events may occur in early life. Our approach leads to the identification of new potential biomarkers/pathways that could be used for the early diagnosis/screening of MSUD newborns but require further validation studies. Our untargeted metabolomics findings have undoubtedly added new insights to our understanding of the pathogenicity of MSUD, which helps us select the appropriate early treatments for better health outcomes.

Abstract:The flowchart outlines our research process. A range of evaluations were carried out after the development of DBS reference materials, which are applicable to correct the results of diverse laboratories, kits and platforms. We anticipate our products will ensure dependable harmonization and standardization for multi‐center metabolism analysis in the future. Background The standardization of quantification data is critical for ensuring the reliability and measurement traceability in the screening of neonatal inherited metabolic disorders. However, the availability of national certified reference materials is limited in China. Methods In this study, we developed a series of dried blood spot (DBS) reference materials containing 9 amino acids (AA) and 10 acylcarnitines (AC) for neonatal screening. Four levels of the reference materials were measured with tandem mass spectrometry (MS/MS) by seven laboratories using different commercial In Vitro Diagnostic Device (IVD) kits. Then, 100 clinical samples were measured using both derivatization and non‐derivatization methods by the same laboratory. Results We found high homogeneity and stability at all levels of the reference materials, with the coefficient of variation (CV) of the analytes less than 15%. These reference materials can be used to assess the testing capabilities of different laboratories. Our test also revealed that the correction factors (CF) calculated by the reference materials, along with clinical samples, could increase the consistency for different kits. Conclusion The DBS reference materials proposed in this study provide reliability for the harmonization in multi‐center analysis for the screening of neonatal inherited metabolic disorders. And applying our correction method for the screening could improve the data consistency of the DBS samples prepared by different methods.

Abstract:Early diagnosis and treatment of inherited metabolic diseases (IMDs) is crucial for reducing neonatal mortality rate and improving quality of life in children. The discovery of disease-related biomarkers that can objectively measure the potential pathophysiological changes is vital to improve the prognosis of IMDs. In this study, we collected 90 clinic urine samples of newborns, including two types of IMDs and healthy samples, glutaric aciduria type I (GA I) and propionic acidemia (PA). And 132 metabolites were identified using gas chromatography-mass spectrometry (GC-MS). Then we proposed an integrated chemometrics strategy of assembling discrete particle swarm optimization (DPSO) into stacked autoencoder (SAE) to form a framework called DPSO-SAE for the study of GC-MS metabolomics data. SAE was known for its excellent non-linear feature learning ability. The introduction of DPSO afforded SAE the possibility of biomarker discovery and improving performance on classification via enabling synergetic optimization of variable combinations and the parameter of neuron numbers for SAE modeling. We then invoked DPSO-SAE for the data analysis as compared with random forest (RF), partial least squares discriminant analysis (PLSDA) and conventional SAE. Superior performance was obtained by DPSO-SAE with high accuracy and good generalization ability on classification. We further demonstrated the robustness of DPSO-SAE in variable selection and proofed the statistical significance of identified marker metabolites that account for IMD classification. Six potential biomarkers were proofed, including 3-methylglutaconic, 3-OH-propionic, Methylcitric, Methylmalonic and Uric for PA and Glutaric for GA I. All results indicated that the proposed strategy of DPSO-SAE was feasible for robust classification and biomarker discovery of IMDs. And it may provide a valuable modeling algorithm for metabolomics studies.

Abstract:<p>Screening of stable metabolic biomarkers objectively measuring the underlying pathophysiological changes are vital to boost early diagnosis and treatment prognosis of inherited metabolic diseases (IMDs). In this study, gas chromatography-mass spectrometry (GC-MS) was invoked to perform urinary metabolomics analysis for IMDs and a comprehensive chemometric method was presented to screen stable metabolic biomarkers effectively. For this chemometric method, a novel hybrid perturbation framework based on partial least squares discriminant analysis (PLS-DA) was adopted, in which the data level utilized bootstrap (BS) method and the function level employed the strategy of multiple informative vector fusion (MIVF), forming an algorithm BS-MIVF-PLSDA. Investigated by two common IMDs (methylmalonic acidemia and propionic acidemia), GC-MS allowed the detection of a rich profile of metabolites, characterizing subtle differences between disease and healthy control groups. The proposed chemometric method BS-MIVF-PLSDA exhibited superiorities to simpler techniques in terms of identifying candidate metabolic biomarkers more biologically correlated to the metabolic mechanisms with desirable selection stability. Moreover, the chemometric method enabled good classification performance in discriminating between disease and healthy control groups using the identified top-ranked candidate metabolic biomarkers. All the results showed that GC-MS urinary metabolomics coupled with chemometrics was feasible to offer an efficient path to achieve stable metabolic biomarkers, supporting early diagnosis and guiding treatment in IMDs clinical practice.</p>

Abstract:Inborn errors of metabolism constitute a set of hereditary diseases that impose severe medical and physical challenges in the affected individual, in particular, for the pediatric patient population. Timely diagnosis is crucial for these patients, as any delay could result in irreversible health damage, underscoring the importance of early initiation of personalized treatment. Current routine diagnostic screening for inborn errors of metabolism relies on various targeted analyses of established biomarkers. However, this approach is time-consuming, focuses on a limited number of tests (based on clinical information) with a relatively small number of biomarkers, and does not facilitate the identification of new markers. In contrast, untargeted metabolomics-based screening offers a more efficient diagnostic solution, by assessing thousands of metabolites across multiple metabolic pathways in a single test. This not only saves time but also conserves resources for clinicians, the diagnostic laboratory, and for patients.This chapter describes the computational workflow of our "Next Generation Metabolic Screening" approach, which is a metabolomics-based method that is currently applied at the Translational Metabolic Laboratory of the Radboud University Medical Center (the Netherlands) for the diagnosis of inborn errors of metabolism.

Abstract:<p>Inborn errors of metabolism, also known as inherited metabolic diseases (IMDs), are related to genetic mutations and cause corresponding biochemical metabolic disorder of newborns and even sudden infant death. Timely detection and diagnosis of IMDs are of great significance for improving survival of newborns. Here we propose a strategy for simultaneously detecting six types of IMDs via combining GC-MS technique with the random forest algorithm (RF). Clinical urine samples from IMD and healthy patients are analyzed using GC-MS for acquiring metabolomics data. Then, the RF model is established as a multi-classification tool for the GC-MS data. Compared with the models built by artificial neural network and support vector machine, the results demonstrated the RF model has superior performance of high specificity, sensitivity, precision, accuracy, and matthews correlation coefficients on identifying all six types of IMDs and normal samples. The proposed strategy can afford a useful method for reliable and effective identification of multiple IMDs in clinical diagnosis.</p>

Abstract:Pediatric myelodysplastic syndromes (MDS) are complicated, thus early and accurate diagnosis is essential for treatment planning and patient care. Diagnostic processes often use discrete data domain analysis, which reduces accuracy and delays diagnosis. This work addresses these limitations by introducing an advanced Multi domain Feature Analysis Model (MFAM) enhanced with incremental optimizations to improve pediatric MDS detection. Traditional pediatric MDS diagnosis relies on subjective evaluations and limited data fusion, not modern computational methods. These constraints may reduce diagnosis accuracy and postpone action. The proposed MFAM integrates data from Clinical History, Physical Examination, Blood Cell Counts, Peripheral Blood Smear, Bone Marrow Aspiration and Biopsy, Cytogenetic Analysis, Flow Cytometry, Genetic Testing, Iron Studies, and Bone Marrow Cytology to overcome these challenges. The MFAM increases feature variance by fusing Bidirectional Long Short-Term Memory (BiLSTM) with Bidirectional Gated Recurrent Units (BiGRU). Deep Q Learning with Graph Recurrent Convolutional Neural Networks (DQGRCNN) boosts efficiency. Additionally, the model integrates the Vector Autoregressive Moving Average with Exogenous Inputs (VARMAX) to facilitate early prediction of paediatric MDS. These enhancements have resulted in significant improvements in the precision of paediatric MDS detection by 4.5%, accuracy by 3.5%, recall by 2.3%, Area Under the Curve (AUC) by 1.5%, and specificity by 2.4% while reducing diagnostic delays by 8.5%. Furthermore, the model enhances the precision of predictive analysis by 2.9%, accuracy by 3.5%, recall by 2.5%, AUC by 2.9%, specificity by 5.5%, and reduces delays in predictive analysis by 8.5%. The MFAM presented in this paper revolutionizes the diagnosis and treatment of paediatric MDS by efficiently combining diverse diagnostic data, employing advanced transformation and fusion techniques, and optimizing responses through DQGRCNN. The integration of VARMAX further enables early prediction of the disease. MFAM will enhance diagnostic precision, therapy start, and clinical outcomes for young MDS patients.

Abstract:Efficient subtype classification utilizing multiple biological information sources is clinically practical for precision treatment of inborn errors of metabolism (IEMs). This study utilized urinary GC-MS and dried blood spot MS/MS to enable complementary mass spectrometric characterization of pathological information for methylmalonic aciduria (MMA) subtypes. A novel mid-level data fusion strategy was subsequently described to effectively detect their subtle metabolic perturbations. In the proposed strategy, multivariate advantage was extracted via considering variable relationships in individual data blocks, separately. Further, it was extended to exploiting partial correlation across multiple data blocks, which induced the models between one data block and the first principal component (PC1) of another one for more valuable information recovery. The continuous values of PC1 replaced the traditional binary class labels, enabling suitable representation of disease heterogeneity numerically and subsequent accurate disease detection. Based on the unique role in exploratory data analysis, partial least squares discriminant analysis (PLS-DA) was coupled with bootstrap to form ensemble feature selection framework. It allowed the novel mid-level data fusion strategy to screen stable significant features. Investigated by two common MMA subtypes (mut and cblC) using urinary and blood metabolic profiles, the results showed that the novel mid-level data fusion strategy outperformed the traditional data fusion approaches both in predictive accuracy and biological interpretation. Besides, correlation network of the identified stable informative metabolites was visualized by chord plots, revealing different between-block biological interaction pattern for mut and cblC. Especially, in mut subtype, strong interactions could be occurred between the urinary metabolite of 3OH-propionic-2 and two blood metabolite ratios (C3/C2 and C3/C0), separately. The findings deepened the biological insights into the disease pathology of mut and cblC. The integration of multiple analytical sources combined with the proposed novel mid-level data fusion strategy thus opened the possibilities to achieve desirable subtype classification for MMA. It suitably guided the early and timely medical intervention for IEMs.
