Abstract:To understand the mechanism of Wolff's law, a finite element analysis was performed for a human proximal femur, and the principal stress trajectories of the femur were extracted using the principal stress visualization method. The mechanism of Wolff's law was evaluated theoretically based on the distribution of the principal stress trajectories. Due to the dynamics of the loads, there was no one-to-one correspondence between the stress trajectories of the fixed load and the trabeculae in the cancellous architecture of the real bone. The trabeculae in the cancellous bone were influenced by the magnitude of the principal stress trajectory. Equivalent principal stress trajectories suitable for different load changes were proposed through the change in load cycle and compared with the anatomical structure of the femur. In addition, the three-dimensional distribution of the femoral principal stress trajectory was established, and the adaptability potential of each load was discussed. The principal stress visualization method could also be applied to bionic structure design.

Abstract:Wolff's law in biomechanics states that bone microstructure and local stiffness tend to align with the stress principal directions to adapt to their mechanical environments. In this paper, a new method for topology optimization of continuum structure based on Wolff's law is presented. The major idea of the present approach is to consider the structure to be optimized as a piece of bone that obeys Wolff's law and the process of finding the optimum topology of a structure is equivalent to the bone remodeling process. A second rank positive and definite fabric tensor, which is viewed as the design variable in design domain, is introduced to express the porosity and anisotropy properties of material points. The update rule of the design variables is established as: during the iteration process of the optimization of a structure, at any material point, the eigenvectors of the stress tensor in the present step are those of the fabric tensor in the next step based on Wolff's law; the increments of the eigenvalues of the fabric tensor depend on the principal strains and an interval of reference strain, which is corresponding to the dead zone in bone mechanics. The process is called anisotropic growth if the eigenpairs of the fabric tensors need to be updated. Otherwise, the process is called as isotropic growth if all the fabric tensors are proportional to the second rank identity tensor in the simulation. Numerical examples show that the present method is appropriate to solve large-scale problems, such as 3D structures and the geometrical nonlinear structures. As an application in biomechanics, it is extended to predict the mass distribution of the proximal femur and the results are the same as those obtained by using the other models.

Abstract:While physiological loading on lower long bones changes during bone development, the bone cross section either remains circular or slowly changes from nearly circular to other shapes such as oval and roughly triangular. Bone is said to be an optimal structure, where strength is maximized using the optimal distribution of bone mass (also called Wolff’s law). One of the most appropriate mathematical validations of this law would be a structural optimization-based formulation where total strain energy is minimized against a mass and a space constraint. Assuming that the change in cross section during bone development and homeostasis after adulthood is direct result of the change in physiological loading, this work investigates what optimization problem formulation (collectively, design variables, objective function, constraints, loading conditions, etc.) results in mathematically optimal solutions that resemble bones under actual physiological loading. For this purpose, an advanced structural optimization-based computational model for cortical bone development and defect repair is presented. In the optimization problem, overall bone stiffness is maximized first against a mass constraint, and then also against a polar first moment of area constraint that simultaneously constrains both mass and space. The investigation is completed in two stages. The first stage is developmental stage when physiological loading on lower long bones (tibia) is a random combination of axial, bending and torsion. The topology optimization applied to this case with the area moment constraint results into circular and elliptical cross sections similar to that found in growing mouse or human. The second investigation stage is bone homeostasis reached in adulthood when the physiological loading has a fixed pattern. A drill hole defect is applied to the adult mouse bone, which would disrupt the homeostasis. The optimization applied after the defect interestingly brings the damaged section back to the original intact geometry. The results, however, show that cortical bone geometry is optimal for the physiological loading only when there is also a constraint on polar moment of area. Further numerical experiments show that application of torsion along with the gait-analysis-based physiological loading improves the results, which seems to indicate that the cortical bone geometry is optimal for some amount of torsion in addition to the gait-based physiological loading. This work has a potential to be extended to bone growth/development models and fracture healing models, where topology optimization and polar moment of area constraint have not been introduced earlier.

Abstract:The density distribution and internal structure of bone relate to exterior loads on it. To study the remodeling process of trabecular bone, an adaptation algorithm with direction determination and stiffness modification was developed and applied to a 3D femoral finite element model. Material axes of every trabecular element were rotated to three principal stress directions determined from load cases in local coordinate system, then elastic and shear moduli were changed according to mechanical stimulus respectively. The calculation results have shown that the distribution of bone density and anisotropy degree corresponded well to the morphology of real femur, and trabeculae direction aligned with the known trabecular pattern. This orthotropic adaptation algorithm can simulate the remodeling process of femoral trabecular bone accurately.

Abstract:Bone remodeling is a complex dynamic process, which modulates both bone mass and bone microstructure. In addition to bone mass, bone microstructure is an important contributor to bone quality in osteoporosis and fragility fractures. However, the quantitative knowledge of evolution of three-dimensional (3D) trabecular microstructure in adaptation to the external forces is currently limited. In this study, a new 3D simulation method of remodeling of human trabecular bone was developed to quantitatively study the dynamic evolution of bone mass and trabecular microstructure in response to different external loading conditions. The morphological features of trabecular plate and rod, such as thickness and number density in different orientations were monitored during the remodeling process using a novel imaging analysis technique, namely Individual Trabecula Segmentation (ITS). We showed that the volume fraction and microstructures of trabecular bone including, trabecular type and orientation, were determined by the applied mechanical load. Particularly, the morphological parameters of trabecular plates were more sensitive to the applied load, indicating that they played the major role in the mechanical properties of the trabecular bone. Reducing the applied load caused severe microstructural deteriorations of trabecular bone, such as trabecular plate perforation, rod breakage, and a conversion from plates to rods.
An orthotropic continuum model with substructure evolution for describing bone remodeling: an interpretation of the primary mechanism behind Wolff's law

Abstract:We propose a variational approach that employs a generalized principle of virtual work to estimate both the mechanical response and the changes in living bone tissue during the remodeling process. This approach provides an explanation for the adaptive regulation of the bone substructure in the context of orthotropic material symmetry. We specifically focus upon the crucial gradual adjustment of bone tissue as a structural material that adapts its mechanical features, such as materials stiffnesses and microstructure, in response to the evolving loading conditions. We postulate that the evolution process relies on a feedback mechanism involving multiple stimulus signals. The mechanical and remodeling behavior of bone tissue is clearly a complex process that is difficult to describe within the framework of classical continuum theories. For this reason, a generalized continuum elastic theory is employed as a proper mathematical context for an adequate description of the examined phenomenon. To simplify the investigation, we considered a two-dimensional problem. Numerical simulations have been performed to illustrate bone evolution in a few significant cases: the bending of a rectangular cantilever plate and a three-point flexure test. The results are encouraging because they can replicate the optimization process observed in bone remodeling. The proposed model provides a likely distribution of stiffnesses and accurately represents the arrangement of trabeculae macroscopically described by the orthotropic symmetry directions, as supported by experimental evidence from the trajectorial theory.
A simple score normalisation technique for multimodal biometric authentication

Abstract:In multimodal biometric authentication, the unimodal scores are used to produce the final decision, following the score fusion approach. Researchers have often decomposed the score fusion problem into two steps: a 'score normalisation' and a 'fusion' step. This paper shows the fusion studies and experiments carried out by the authors. A special effort has been made to manipulate unimodal scores prior to their fusion. We propose a score normalisation technique consisting of a bilinear transformation applied to the independent scores, so that the unimodal thresholds become uniform and the data closest to these thresholds have greater separation between them. Our results show that this procedure improves the final results when compared with classic normalisation procedures. Several score fusion strategies were used in the experiments. All the experiments were carried out using the same scores, that have been obtained through the processing of three unimodal verification systems on three independent biometrical databases.
Comparison of Methods for Determining Orthotropic Axes in 3D Femur Remodeling

Abstract:The density distribution and internal structure of trabecular bone are modulated by local mechanical environment. In this study, an orthotropic adaptation algorithm was applied to the 3D femur model to simulate the variations of stiffness and orientation of trabecular bone under multi-loading conditions. This algorithm includes the determination of orthotropic axes and the stiffness modification of trabecular bone. Three methods were used to determine the orthotropic axes, i.e. the maximal cycle method, the stress maximum method, and the composite method. We found that the composite method was better than the other two. Firstly, the characteristics of density distribution obtained by using the composite method agreed better with those in real proximal femur. Secondly, the material orientation aligned with the known trabeculaer pattern. Finally, the ratios of the longitudinal modulus to the transverse modulus were also shown to be realistic in three local regions of the proximal femur.

Abstract:Basic multicellular unit (BMU)-based remodeling of lamellar bone causes bone turnover, net gains and losses of bone on some bone surfaces or "envelopes," and a remodeling space comprising bone temporarily absent due to evolving resorption spaces and incomplete refilling of them by new bone. Those features depend a) on how many new BMU arise annually, b) on how much bone each BMU has resorbed and c) formed upon its completion, and d) on how long the typical BMU takes to become completed. Because a, b, and c have limiting or maximal values in life that direct and/or indirect effects of mechanical usage of the skeleton can change, the theory presented here derives mechanical usage functions that express what fractions of those maxima a given mechanical usage history allows to happen. The theory predicts some changes in bone formation, resorption, balance, turnover, and remodeling space that depend on how remodeling responds to the vigor of a subject's mechanical usage. The theory can predict specific effects of specific mechanical challenges that experiments can test, and it fits abundant published evidence. As the kernel of a new approach to the problem it awaits critique and refinement by others. It plus the 3-way rule can redefine Wolff's law conceptually and also in mathematical and quantifiable form.

Abstract:From the nature of a bone's endload and its local surface strains, the theory computes a modeling operator, Gamma (gamma), that predicts whether mechanical factors will cause lamellar bone modeling drifts, and where and of what kind. A given mechanical bone strain history then provides a separate modeling rate function, M, to specify the rate of such modeling drifts as fractions of the largest possible ones. Multiplying the two functions, e.g., gamma.M, then predicts mechanically controlled bone modeling responses for cortical and trabecular bone, both quantitatively and qualitatively. The theory correctly predicts each of the 6 known "principal adaptations" of lamellar bone, which provide a critical test of any such theory for this organ. The theory accounts for biologic, biomechanical, and clinical-pathologic knowledge not available in Wolff's time nor accounted for by most biomechanicians since. Existing proven methods can provide all numerical data needed to satisfy the theory's mathematical equations and already suggest provisional values for most of them. Its originator views the theory as the kernel of more and better theories to come rather than a finished work, a kernel that suggests a new and in some respects novel logical framework for analysing the problems, and a kernel that invites critique, refinement, and/or exploitation by others.
Topology Optimization Driven Bone-Remodeling Simulation for Lumbar Interbody Fusion

Abstract:This study proposes a numerical approach for simulating bone remodeling in lumbar interbody fusion (LIF). It employs a topology optimization method to drive the remodeling process and uses a pixel function to describe the structural topology and bone density distribution. Unlike traditional approaches based on strain energy density or compliance, this study adopts von Mises stress to guide the remodeling of LIF. A novel pixel interpolation scheme associated with stress criteria is applied to the physical properties of the bone, directly addressing the stress shielding effect caused by the implanted cage, which significantly influences the bone remodeling outcome in LIF. Additionally, a boundary inverse approach is utilized to reconstruct a simplified analysis model. To reduce computational cost while maintaining high structural resolution and accuracy, the Scaled Boundary Finite Element Method (SBFEM) is introduced. The proposed numerical approach successfully generates results that closely resemble human lumbar interbody fusion.

Abstract:We propose a three-dimensional macroscopic continuum model designed to predict the remodeling phenomenon of bone tissue. In the proposed model, we focus on the evolution of two crucial stiffness parameters: the bulk and shear moduli. These parameters independently adapt to the mechanical demands to which bone tissue is subjected, mainly to withstand hydrostatic and deviatoric deformations. These mechanical stimulations influence the activity of bone cells, leading to changes in bone structure and strength and, in turn, the above-mentioned moduli. The formulation is simplified, serving as an initial step towards a more comprehensive modeling approach. The evolution of these stiffness parameters is proposed based on an energetic argument to describe the functional adaptation process. Numerical experiments, conducted on a cylindrical specimen resembling a femur, demonstrate the feasibility of modeling the bone remodeling process with distinct evolutions for multiple material parameters, in contrast to the conventional approach that permits only one-parameter evolution.

Computationally efficient dominant load-based local bone microstructure reconstruction method using topology optimization

Abstract:The bone microstructure of the human proximal femur is clinically crucial for diagnosing skeletal pathologies, such as osteoporosis and bone metastases. The topology optimization-based bone microstructure method obtains these bone microstructures by converting low-resolution (LR) images into high-resolution images. However, this method is inherently computationally inefficient as it requires numerous finite elements, iterative analyses, and parallel computations. Therefore, this study proposes a novel topology optimization-based localised bone microstructure reconstruction method using the dominant load, which highly affects the selected region of interest (ROI), for efficient resolution enhancement. The load dependency of selected ROIs is quantified with a load dependency score. Then, the localised finite element model is constructed based on the local load estimation. Finally, the selected dominant load is applied as an input for the topology optimization-based bone microstructure reconstruction method. The reconstructed bone microstructure was similar to that of the conventional method. The localised finite element model applied by the dominant load effectively and accurately reconstructed the bone morphology and exhibited high computational efficiency. In conclusion, the dominant load-based approach can be used to construct a reasonable trabecular bone structure for ROI with high computational efficiency. The predictive performance of the proposed method was validated and showed promise for accurate trabecular bone structure prediction without additional radiation exposure.

Abstract:The ability of Anisotropic Minkowski Functionals (AMFs) to capture local anisotropy while evaluating topological properties of the underlying gray-level structures has been previously demonstrated. We evaluate the ability of this approach to characterize local structure properties of trabecular bone micro-architecture in ex vivo proximal femur specimens, as visualized on multi-detector CT, for purposes of biomechanical bone strength prediction. To this end, volumetric AMFs were computed locally for each voxel of volumes of interest (VOI) extracted from the femoral head of 146 specimens. The local anisotropy captured by such AMFs was quantified using a fractional anisotropy measure; the magnitude and direction of anisotropy at every pixel was stored in histograms that served as a feature vectors that characterized the VOIs. A linear multi-regression analysis algorithm was used to predict the failure load (FL) from the feature sets; the predicted FL was compared to the true FL determined through biomechanical testing. The prediction performance was measured by the root mean square error (RMSE) for each feature set. The best prediction performance was obtained from the fractional anisotropy histogram of AMF Euler Characteristic (RMSE = 1.01 +- 0.13), which was significantly better than MDCT-derived mean BMD (RMSE = 1.12 +- 0.16, p<0.05). We conclude that such anisotropic Minkowski Functionals can capture valuable information regarding regional trabecular bone quality and contribute to improved bone strength prediction, which is important for improving the clinical assessment of osteoporotic fracture risk.

Abstract:Porous metals are found to be suitable as orthopedic scaffolds. However, only limited control over the internal architecture can be achieved using conventional design methods. The architecture with porosity variation strategy mimicking natural bone is critical to gain favorable combination of mechanical and biological properties for orthopedic implants. In this regard, a topology optimization method with customized morphology and mechanical properties derived from the trabecular bone was proposed to design three-dimensional architectures with gradient porosity resembling the porous structure of bone. In particular, the elastic constants for the trabecular bone were better predicted when the bone volume fraction was supplemented with a three-dimensional structural parameter, i.e., degree of anisotropy. These constants were set as the optimization constrains for morphology control. Then the porous titanium structures were manufactured by selective laser melting technology (SLM). The physical characteristics, mechanical properties of the scaffolds were compared systematically. The experimental results revealed that the as-built samples with the proposed method lead to a good match of morphological accuracy and mechanical properties to that of the bone. It demonstrates that the proposed topology optimization method with controlled morphology and mechanical properties provides an efficient manner for the biomimetic design of orthopedic implants.

Abstract:Background and Objective Different bone remodeling algorithms are used to predict bone adaptation and to understand how bones respond to the mechanical stimuli altered by implants. This paper introduces a novel micro-scale bone remodeling algorithm, which deviates from conventional methods by focusing on structure-based bone adaptation instead of density-based approaches. Methods The proposed model simulated cellular activities such as bone resorption, new bone formation, and maturation of newly formed bone. These activities were assumed to be triggered by mechanical stimuli. Model parameters were evaluated for the 3D geometries of trabecular bone from intact femur developed from micro computed tomography (CT) scan data. Two different hip implants, solid and porous were used, and two different bone remodeling methods were performed using the proposed and conventional methods. Results Results showed that micro CT scan-based finite element (FE) models accurately captured the microarchitecture and anisotropy of trabecular bone. The predicted bone resorption rate at the peri-prosthetic regions for the solid and porous implants was in the range of 17 to 27% and 4.5 to 7.3%, respectively, for a simulated period of four years. Conclusions The results obtained from FE analysis strongly align with clinical findings, confirming the effectiveness of the proposed algorithm. By emphasizing the structural aspect of bone adaptation, the proposed algorithm brings a fresh perspective on bone adaptation at the peri-prosthetic bone. This method can help researchers and clinicians to improve implant designs for better clinical outcomes.

Abstract:The aim of this study is the utilization of human medical CT images to quantitatively evaluate two sorts of "error-driven" material algorithms, that is, the isotropic and orthotropic algorithms, for bone remodelling. The bone remodelling simulations were implemented by a combination of the finite element (FE) method and the material algorithms, in which the bone material properties and element axes are determined by both loading amplitudes and daily cycles with different weight factor. The simulation results showed that both algorithms produced realistic distribution in bone amount, when compared with the standard from CT data. Moreover, the simulated L-T ratios (the ratio of longitude modulus to transverse modulus) by the orthotropic algorithm were close to the reported results. This study suggests a role for "error-driven" algorithm in bone material prediction in abnormal mechanical environment and holds promise for optimizing implant design as well as developing countermeasures against bone loss due to weightlessness. Furthermore, the quantified methods used in this study can enhance bone remodelling model by optimizing model parameters to gap the discrepancy between the simulation and real data.

Abstract:Bone adapts in response to its mechanical environment. This evolution of bone density is one of the most important mechanisms for developing fracture resistance. A finite element framework for simulating bone adaptation, commonly called bone remodelling, is presented. This is followed by a novel method to both quantify fracture resistance and to simulate fracture propagation. The authors' previous work on the application of configurational mechanics for modelling fracture is extended to include the influence of heterogeneous bone density distribution. The main advantage of this approach is that configurational forces, and fracture energy release rate, are expressed exclusively in terms of nodal quantities. This approach avoids the need for post-processing and enables a fully implicit formulation for modelling the evolving crack front. In this paper density fields are generated from both (a) bone adaptation analysis and (b) subject-specific geometry and material properties obtained from CT scans. It is shown that, in order to correctly evaluate the configurational forces at the crack front, it is necessary to have a spatially smooth density field with higher regularity than if the field is directly approximated on the finite element mesh. Therefore, discrete density data is approximated as a smooth density field using a Moving Weighted Least Squares method. Performance of the framework is demonstrated using numerical simulations for bone adaptation and subsequent crack propagation, including consideration of an equine 3rd metacarpal bone. The degree of bone adaption is shown to influence both fracture resistance and the resulting crack path.

Biomechanics and Modeling in Mechanobiology

Abstract:Osteocyte apoptosis is known to trigger targeted bone resorption. In the present study, we developed an osteocyte-viability-based trabecular bone remodeling (OVBR) model. This novel remodeling model, combined with recent advanced simulation methods and analysis techniques, such as the element-by-element 3D finite element method and the ITS technique, was used to quantitatively study the dynamic evolution of bone mass and trabecular microstructure in response to various loading and unloading conditions. Different levels of unloading simulated the disuse condition of bed rest or microgravity in space. The amount of bone loss and microstructural deterioration correlated with the magnitude of unloading. The restoration of bone mass upon the reloading condition was achieved by thickening the remaining trabecular architecture, while the lost trabecular plates and rods could not be recovered by reloading. Compared to previous models, the predictions of bone resorption of the OVBR model are more consistent with physiological values reported from previous experiments. Whereas osteocytes suffer a lack of loading during disuse, they may suffer overloading during the reloading phase, which hampers recovery. The OVBR model is promising for quantitative studies of trabecular bone loss and microstructural deterioration of patients or astronauts during long-term bed rest or space flight and thereafter bone recovery.
