Abstract:Our choice of partners, so they say, is subconsciously directed by an immune system aiming to making itself more diverse and robust. This strategy seems to work quite well if one looks at the highly complex structures that underlie human immunology, which still surprises with unexpected enigmas even after decades of intense research. Another recent proof of this phenomena is the study by Eller et al. 1 in this issue of JASN that describes a surprisingly aggravated course of glomerulonephritis in mice lacking the chemokine receptor CCR7. The family of chemokines and their receptors are important mediators of directional leukocyte trafficking under inflammatory and homeostatic conditions. Given the complexity of the immune system and the multitude of different leukocytes, guidance by the chemokine family is much needed to facilitate the interaction of the right leukocyte subsets at the right time and in the right location. CCR7 is particularly crucial in the initiation of antigen-specific immune responses.2 Upon encountering a danger signal, antigen-bearing dendritic cells upregulate CCR7 and become responsive to the corresponding ligands CCL19 and CCL21, which are expressed in secondary lymphoid organs. Remote-controlled vascular interventional surgery robot

Abstract:Background: Conventional vascular interventional surgery (VIS) is manually performed under fluoroscopic guidance, requiring lead protection for the surgeons. A remote-control vascular interventional surgery robot (VISR) which can remotely, safely and precisely perform VIS would have clear advantages. Methods: Our robot adopts a master-slave structure. The surgeon sits at the master site, sending controlling instructions to the robot fixed at the slave site, then the robot translates these instructions into catheter motion. The robotic mechanism consists of a supporting manipulator and a catheter navigator; the former adjusts the robot's spatial position, while the latter controls the translation and rotation of the catheter. A 3D vascular model is reconstructed so that the surgeon can perform surgical planning easily. In addition, the tactile force between catheter tip and blood vessel is measured, which prevents the surgeon damaging delicate vessels. In glass model and animal experiments, the surgeon remotely controlled VISR, which inserted a catheter into predefined targets, and the robotic surgery time was measured. Results: The robot was initially tested on a transparent glass vascular model. Under robotic manipulation, the catheter can enter an arbitrary branch of the vascular model and catheter motion can meet the requirements of clinical VIS. Then robotic surgery was performed successfully in an adult dog. Surgery time to access each of the five targets, viz. renal artery, left atrium, right atrium, left ventricle and right ventricle, was measured. Compared with conventional manual surgery, robotic surgery time is a little longer. Conclusions: The experiments show the feasibility and safety of the VISR to facilitate navigation, position precisely and control catheters to specific regions. The VISR system offers surgeon radiation safety and minimizes surgeon-based error.
Application study of vascular interventional robotic mechanism for remote steering

Abstract:Background: Recently, robotic systems have been introduced as a useful method for surgical procedures. But in the field of vascular interventional therapy, the development of robotic system is slower. Objective: The purpose of the study is to verify the reliability and safety of vascular interventional robotic system used in angiography, by the way of in vitro preliminary experiments and animal experiments. Method: The approach is to employ a proprietary vascular interventional robot system to complete glass vessel models and animal angiogram experiments. This robot system consists of a console port (remote steering system), an assistant port (propelled and rotation system) and a hydraulic fixing device, upon which surgeons control remotely to make go forward and rotate in the glass vessel models and animal vessels, on the 3D operation interface. Consequently, the operation time and success rate are counted and evaluated. Result: In the glass vessel model experiments, the Catheter can enter various kinds of vessel models with inside diameter length greater than 3mm and angle less than 90(o). In the animal (adult dogs) experiments, surgeons can accomplish smoothly the angiogram of the renal artery, the vertebral renal and the arteria carotis communis, without any complications of surgery. Conclusion: The angiogram by using vascular interventional robot system is safe and reliable. Surgeons can finish the angiogram in part by remote operation, and the result of angiogram can meet a number of simple expectations. However without wire control and force feedback systems, the applicability of this kind of robot system is not flexible enough and need to be improved in the future.

Abstract:Interventional surgery is widely used in the treatment of cardiovascular and cerebrovascular diseases, and the development of surgical robots can greatly reduce the fatigue and radiation risks brought to surgeons during surgery. In this paper, we present a novel interventional surgical robot which allows surgeons to fully use their operating skills during remote control. Fuzzy control theory is used to guarantee control precision during the master-slave operation. The safety force feedback control is designed based on the catheter and guidewire spring model, and the force-position control is designed to decrease the potential damage due to the control delay. This study first evaluates the force-position control strategy using a vascular model experiment, and then an in vivo experiment is used to evaluate the precision of the surgical robot controlling the catheter and guidewire to the designated position. The in vivo experiment results and surgeon's feedback demonstrate that the proposed surgical robot is able to perform complex remote surgery in clinical application.Surgeons perform remote interventional animal surgery using interventional surgical robots.

Abstract:Remote-controlled vascular interventional robots (RVIRs) are being developed to increase the overall accuracy of surgical operations and reduce the occupational risks of intervening physicians, such as radiation exposure and chronic neck/back pain. Several RVIRs have been used to operate catheters or guidewires accurately. However, a lack of cooperation between the catheters and guidewires results in the surgeon being unable to complete complex surgery by propelling the catheter/guidewire to the target position. Furthermore, it is a significant challenge to operate the catheter/guidewire accurately and detect their proximal force without damaging their surfaces. In this study, we introduce a novel method that allows catheters and guidewires to be operated simultaneously in complex surgery. Our method accurately captures force measurements and enables precisely controlled catheter and guidewire operation. A prototype is validated through various experiments. The results demonstrate the feasibility of the proposed RVIR to operate a catheter and guidewire accurately, detect the resistance forces, and complete complex surgical operations in a cooperative manner.

Feasibility study on robot-assisted retinal vascular bypass surgery in an ex vivo porcine model

Abstract:Purpose: To describe a new robot-assisted surgical system for retinal vascular bypass surgery (RVBS) and to compare the success rate with freehand RVBS. Methods: A robot-assisted system for retinal microsurgery was constructed to include two independent robotic arms. A 23-gauge light probe and an intraocular forceps were affixed to the arm end effectors to perform the intraocular manipulation. Harvested porcine eyes were introduced to be established animal models of closed-sky eyeballs after that pars plana vitrectomy using temporary keratoprosthesis was performed by a skilful surgeon. Retinal vascular bypass surgery (RVBS) was performed by an inexperienced ophthalmologist to test the ease of use. A stainless steel wire (45-μm pipe diameter) was used as an artificial vessel. Before RVBS, the wires were prepositioned at the retinal surface of the eyes. The Control group (n = 20) underwent freehand RVBS, and the Experimental group (n = 20) underwent robot-assisted RVBS. To create the simulated bypass, the distal end of the wire was inserted into the selected vessel and advanced ~4 mm away from the optic disc. If successful, then the proximal wire end was inserted and advanced ~2 mm towards the optic disc. The difference in the success rate for the freehand and robot-assisted procedures was analysed by the chi-square test. Results: The success rate for the freehand RVBS was 5% (1/20 eyes). In contrast, the robot-assisted success rate was 35% (7/20) of eyes (p < 0.05). Conclusion: This study demonstrated the feasibility of robot-assisted RVBS in ex vivo porcine eyes. The robotic system increased the accuracy and stability of manipulation by eliminating freehand tremor, leading to a higher surgical success rate.

Abstract:Remote-controlled vascular interventional robots (RVIRs) are being developed to increase the accuracy of surgical operations and reduce the number of occupational risks sustained by intervening physicians, such as radiation exposure and chronic neck/back pain. However, complex control of the RVIRs improves the doctor’s operation difficulty and reduces the operation efficiency. Furthermore, incomplete sterilization of the RVIRs will increase the risk of infection, or even cause medical accidents. In this study, we introduced a novel method that provides higher operation efficiency than a previous prototype and allows for complete robot sterilization. A prototype was fabricated and validated through laboratory setting experiments and an in-human experiment. The results illustrated that the proposed RVIR has better performance compared with the previous prototype, and preliminarily demonstrated that the proposed RVIR has good safety and reliability and can be used in clinical surgeries.

Abstract:Background At present, neurointerventional surgery requires angiographers to perform operations in the digital subtraction angiography (DSA) room. Ionising radiation and chronic joint damage are still unavoidable for angiographers. Therefore, we researched and developed a neurointerventional robot-assisted system, which is operated by angiographers in an operating room outside the DSA room. We have conducted a prospective, multicentre, randomised controlled trial to evaluate the safety and efficacy of a robot-assisted system in human cerebral angiography. In the future, this research will provide a platform for the research and development of an intelligent surgical system and bring revolutionary progress in neurointerventional surgery. Methods From December 2020 to December 2021, 260 patients were enrolled from three medical centres, who were randomly and equally divided into a robot-assisted system group and a clinical routine cerebral angiography group. The success rate of angiography, the rate of the catheter reaching the target vessel, the operation time, X-ray radiation exposure and the incidence of related adverse events were compared between the two groups. Results A total of 257 patients completed this trial; baseline characteristics of the two groups did not differ significantly. The success rate of angiography in both the control group and the experimental group was 100%. The rate of the catheter reaching the target vessel was 99.23% and 100.00% in the control and experimental groups, respectively. For the control versus experimental groups, the angiographic operation time was 48.59±25.60 min versus 47.94±27.49 min, respectively; the X-ray radiation dose was 735.01±554.77 mGy versus 821.65±705.45 mGy, respectively; and the incidence of adverse events was 23.44% versus 22.48%, respectively. No statistical differences were present between the two groups. Conclusion The robot-assisted surgical system is more convenient for cerebral angiography and is as safe and effective as the traditional cerebral angiography.

Abstract:Vascular interventional surgery (VIS) is an effective treatment method for vascular diseases. However, there are many problems in traditional VIS, such as surgeons are radiated by X-ray, the lack of well skilled surgeons, the security of the surgery will be reduced due to the Surgeons’ fatigue, high risk of the surgery. To solve these problems, a robotic catheter system is needed to protect the surgeons and enhance the safety of the surgery. In this paper, a novel robotic catheter system with master–slave structure for VIS has been developed. This system is designed with the consideration of the operation method in traditional VIS, which allows the surgeon to operate a real catheter on the master side, then the surgeon make full use of the natural catheter manipulation experience and skills obtained in conventional catheter operation. The salve manipulator operates the catheter insert into the blood vessel with following the operation of the surgeon, and the operating force of the salve manipulator is detected. On the master side, a novel damper-based magnetorheological (MR) fluid is designed to realize the force feedback, which is also used to reappear the operation force from the salve manipulator. The damper connected directly with real catheter is a piston structure using the MR fluid to realize the force feedback. It can transmit the feedback force to surgeon’s hand through the operating catheter connected with damper, which seems that the surgeon operates the catheter beside the patient. The operating transparency of the developed system has been enhanced. The mechanism of the developed system has been introduced in detail. Performance evaluation experiments for the developed robotic catheter system have been done. The experimental results indicated that the developed robotic catheter system is fit for VIS.

In-Vivo Validation of a Novel Robotic Platform for Endovascular Intervention

Abstract:Objective: In-vivo validation on animal setting of a pneumatically propelled robot for endovascular intervention, to determine safety and clinical advantage of robotic cannulations compared to manual operation. Methods: Robotic assistance and image-guided intervention are increasingly used for improving endovascular procedures with enhanced navigation dexterity and accuracy. However, most platforms developed in the past decade still present inherent limitations in terms of altered clinical workflow, counterintuitive human-robot interaction, and a lack of versatility. We have created a versatile, highly integrated platform for robot-assisted endovascular intervention aimed at addressing such limitations, and here we demonstrate its clinical usability through in-vivo animal trials. A detailed in-vivo study on four porcine models conducted with our robotic platform is reported, involving cannulation and balloon angioplasty of five target arteries. Results: The trials showed a 100% success rate, and post-mortem histopathological assessment demonstrated a reduction in the incidence and severity of vessel trauma with robotic navigation versus manual manipulation. Conclusion: In-vivo experiments demonstrated that the applicability of our robotic system within the context of this study was well tolerated, with good feasibility, and low risk profile. Comparable results were observed with robotics and manual cannulation, with clinical outcome potentially in favor of robotics. Significance: This study showed that the proposed robotic platform can potentially improve the execution of endovascular procedures, paving the way for clinical translation.
Design and Performance Evaluation of Real-time Endovascular Interventional Surgical Robotic System with High Accuracy

Abstract:Background: Endovascular interventional surgery (EIS) is performed under a high radiation environment at the sacrifice of surgeons' health. This paper introduces a novel endovascular interventional surgical robot that aims to reduce radiation to surgeons and physical stress imposed by lead aprons during fluoroscopic X-ray guided catheter intervention. Methods: The unique mechanical structure allowed the surgeon to manipulate the axial and radial motion of the catheter and guide wire. Four catheter manipulators (to manipulate the catheter and guide wire), and a control console which consists of four joysticks, several buttons and two twist switches (to control the catheter manipulators) were presented. The entire robotic system was established on a master-slave control structure through CAN (Controller Area Network) bus communication, meanwhile, the slave side of this robotic system showed highly accurate control over velocity and displacement with PID controlling method. Results: The robotic system was tested and passed in vitro and animal experiments. Through functionality evaluation, the manipulators were able to complete interventional surgical motion both independently and cooperatively. The robotic surgery was performed successfully in an adult female pig and demonstrated the feasibility of superior mesenteric and common iliac artery stent implantation. The entire robotic system met the clinical requirements of EIS. Conclusion: The results show that the system has the ability to imitate the movements of surgeons and to accomplish the axial and radial motions with consistency and high-accuracy.

Abstract:Purpose: The goal of this preclinical study is to assess the functionality, technical feasibility, and safety of a new vascular robotic LIBERTYR 3 System, in the microcatheterization of vascular targets using a range of guidewires and microcatheters. Material and methods: An anesthetized pig served as an arterial model for the robotic device (LIBERTYR3; Microbot Medical Ltd, Yoqneam, IL). The primary efficacy endpoint was the evaluation of its capability to selectively catheterize predetermined distal arterial branches in the liver, kidneys, and mesenteric arteries (technical success), under fluoroscopy guidance. The primary safety endpoint was the occurrence of angiographic acute catheterization-related complications (dissection, thrombosis, embolism, perforation). The catheterizations were conducted by two interventional radiologists that present different work experience in endovascular procedures (18 and 2 years respectively), using a variety of microcatheters and wires. Various procedural parameters such as functionality, practicality, ease of use, and time required for selective catheterization, were evaluated, and recorded. Results: All pre-determined arteries were successfully selectively catheterized (100% technical success), by both operators. No angiographic acute complications occurred. The microcatheters and wires were manipulated using the remote portable console in an effortless manner that maintained a high level of accuracy. Mean time for selective catheterization was 131 ± 82 s. The robot's conversion function to manual operation was successfully demonstrated. Conclusion: Robotic navigation and catheterization of selected target arteries were accomplished without observable vascular damage, suggesting that the LIBERTYR 3 robotic system is a reliable and safe tool for robotic-assisted endovascular navigation. Further experimental studies are required to evaluate safety and efficacy prior to introduction into clinical practice.

Abstract:Background: Remote-controlled robotic vascular interventional surgery can reduce radiation exposure to interventional physicians and improve safety. However, inconvenient operation and lack of force feedback limit its application. Materials and methods: A new wearable robotic system for vascular interventional surgery is designed, which is more flexible in operation. It ensures the safety of surgery through haptic force feedback. The system was evaluated by human vascular models and animal experiments. Results: The average static error of the system is 0.048 mm when the axial motion is 250 mm and 1.259° when the rotational motion is 400°. The average error of the force feedback is 0.021 N. The results of vascular model experiments and animal experiments demonstrate the feasibility and safety of the system. Conclusions: The proposed robotic system can assist physicians in remotely delivering standard catheters or guidewires. The system is more flexible and uses haptic force feedback to ensure surgical safety.

Abstract:In avian sarcoma and leukemia viruses, the gag protein p19 functions structurally as a matrix protein, connecting internal components with the viral envelope. We have used a combination of in situ cross-linking and peptide mapping to localize within p19 the regions responsible for two major interactions in this complex, p19 with lipid and p19 with p19. Lipid-protein cross-links were localized near the amino terminus within the first 35 amino acids of the polypeptide. Homotypic protein-protein disulfide bridges were found to originate from near the carboxy terminus of p19, from cysteine residues at amino acids 111 and 153. These results suggest that p19 is divided into domains with distinct functions. The peptide maps constructed for p19, and for the related proteins p23 in avian sarcoma and leukemia viruses and p19 beta in recombinant avian sarcoma viruses, should serve as useful tools for other types of studies involving these proteins.

Abstract:Robot-assisted vascular interventional surgery (RVIS) is an emerging technology for the treatment of vascular diseases. It has obvious advantages over traditional manual operation, such as increased accuracy, reduced fatigue, and reduced tremor. However, current research suggests that natural human–robot interaction in RVIS is still a challenge that needs to be addressed. In this article, we developed a novel robotic platform that realized magnetorheological (MR) fluids-based haptic feedback to improve the interventionist’s tactile presence. In addition, we proposed a force sensing method to accurately detect the real-time force of the flexible instrument and a collaborative operation method of the guidewire and the catheter to assist the flexible instruments in selecting the target blood vessel branch and reduce the operation difficulty of the interventionist. To verify the developed robotic platform and the proposed methods, we conducted the performance evaluation experiments in a blood vessel model and an endo vascular evaluator. The results indicated that the developed robotic platform and the proposed methods have great potential to improve the natural human–robot interaction in RVIS and guarantee safety.

Abstract:Background: This study aims to report the treatment process and results of the first officially published robotic peripheral vascular interventional surgery for lower extremity arterial occlusion in China. After a literature search, no domestic or foreign research reports have been found that share the same research content and innovation points. Methods: A 77-year-old female patient with intermittent claudication received drug balloon dilatation using a universal vascular interventional surgery robotic system on June 17, 2023, at Peking Union Medical College Hospital (PUMCH). Results: With the help of the vascular interventional robot, the operation successfully re-opened the patient's nearly occluded blood vessel, and the patient's discomfort symptoms were relieved. The total operation lasted 60 min, which time was similar to surgeries without robots. The postoperative hospital stay was 2 days. Conclusion: This case has preliminarily verified the effectiveness and safety of vascular interventional robotic surgery. The initial study fills in the gaps in related fields and lays a solid foundation for the future development of universal vascular interventional robots. Larger scale trials and more clinical trials are needed to test and support the clinical application of universal vascular interventional robots.

Abstract:Cardiovascular diseases (CVDs) are the deadliest diseases worldwide. Master-slave robotic systems have been widely used in vascular interventional surgery with the benefit of high safety, efficient operation, and procedural facilitation. This paper introduces a remote-controlled vascular interventional robot (RVIR) that aims to enable surgeons to perform complex vascular interventions reliably and accurately under a magnetic resonance imaging (MRI) environment. The slave robot includes a guidewire manipulator (GM) and catheter manipulator (CM) that are mainly composed of a hollow driving mechanism and a linear motion platform. The hollow driving mechanism is based on a traveling wave-type hollow ultrasonic motor (HUM) which has high positional precision, fast response, and magnetic interference resistance and realizes the cooperation of the guidewire and catheter by omitting the redundant transmission mechanism and maintaining good coaxiality. The HUM stator, the core part of the RVIR, is optimized by an adaptive genetic algorithm for better quality and greater amplitude of traveling waves, which are beneficial to the drive efficiency and precision. The robot system features great cooperating performance, small hysteresis, and high kinematic accuracy and has been experimentally verified for its capability to precisely manipulate the guidewire and catheter.
Remote vascular interventional surgery robotics: a literature review.

Abstract:Vascular interventional doctors are exposed to radiation hazards during surgery and endure high work intensity. Remote vascular interventional surgery robotics is a hot research field, in which researchers aim to not only protect the health of interventional doctors, but to also improve surgical accuracy and efficiency. However, the current vascular interventional robots have numerous shortcomings, such as poor haptic feedback, few compatible surgeries and instruments, and cumbersome maintenance and operational procedures. Nevertheless, vascular interventional surgery combined with robotics provides more cutting-edge directions, such as Internet remote surgery combined with 5G network technology and the application of artificial intelligence in surgical procedures. To summarize the developmental status and key technical points of intravascular interventional surgical robotics research, we performed a systematic literature search to retrieve original articles related to remote vascular interventional surgery robotics published up to December 2020. This review, which includes 113 articles published in English, introduces the mechanical and structural characteristics of various aspects of vascular interventional surgical robotics, discusses the current key features of vascular interventional surgical robotics in force sensing, haptic feedback, and control methods, and summarizes current frontiers in autonomous surgery, long-distance robotic telesurgery, and magnetic resonance imaging (MRI)-compatible structures. On the basis of summarizing the current research status of remote vascular interventional surgery robotics, we aim to propose a variety of prospects for future robotic systems.

Cadaveric Feasibility of Robotic-Assisted Radial Artery-Internal Carotid Artery-Middle Cerebral Artery Anastomosis for Neurovascular Surgery

Abstract:Background and objectives: To the best of our knowledge, this is the first reported cadaveric feasibility study of leader-follower type robotic-assisted middle cerebral artery (MCA)-radial artery-internal carotid artery anastomosis in the neurovascular surgery field using the da Vinci Xi system (da Vinci Surgical System; Intuitive Surgical, Inc.). Vascular suturing is a necessary skill in neurosurgery; however, the learning curve for deep and high-flow bypasses is severely low. Thus, robot-assisted surgery has been introduced. Here, we describe the surgical workflow adaptations of vascular anastomosis using the da Vinci system to assess the feasibility of robot-assisted anastomoses of the radial and middle cerebral arteries. Methods: Two fresh cadaver heads were studied using the da Vinci Xi Surgical System with 0° and 30° stereoscopic endoscopes to visualize the neuroanatomy. Results: The da Vinci Xi Surgical System was used throughout the anastomosis of the MCA and intracarotid artery. The optic nerve, optic chiasm, carotid artery, and oculomotor nerve were visualized using standard microdissection techniques. The Sylvian fissure was exposed from the proximal Sylvian membrane to the distal MCA. Using black diamond microforceps and Potts scissors, suturing was achieved on the radial artery-middle cerebral artery using 8-0 Prolene and on the radial artery-internal carotid artery using 7-0 Prolene. Conclusion: A bypass of the MCA-radial artery-internal carotid artery can be achieved using the da Vinci Xi Surgical System in cadaver models. This system provides experts and less experienced neurosurgeons with stable bypass techniques for both superficial and deep-seated arteries. However, further studies are needed to evaluate the safety and benefits of the da Vinci Xi Surgical System for bypass procedures.
