Abstract:Tolerance to high-intensity constant power exercise can be characterized by the hyperbolic power-duration (or velocity-duration) relationship. The hyperbola is defined by the asymptote (critical power or velocity) and the curvature constant (W' or D'). The effects of thermoregulatory stress on middle-distance running performance are equivocal-possibly due to the complexities of the hyperbolic velocity-duration relationship for these relatively short duration events. We aimed to measure the effects of heat stress on the velocity-duration relationship in amateur runners. Fifteen participants (23 ± 6 years) completed a series of constant-velocity running bouts to intolerance in three heat indices (MILD: 20°C, VERY HOT: 38°C, EXTREME: 55°C). Critical velocity (CV) in MILD (3.52 ± 0.86 m/s) was higher than VERY HOT (3.39 ± 0.82 m/s) and EXTREME (3.29 ± 1.05 m/s; F[2.28] = 3.80, p < 0.035) with no effect of thermal stress on D' (F[2.28] = 2.48, p = 0.11). In amateur competitive/recreational runners, heat stress of ≥38°C heat index negatively affected CV. Thus, even during relatively short events, such as middle-distance running where fluid loss is not a primary concern, heat stress may negatively impact performance.

Abstract:Physical performance in a tropical environment, combining high heat and humidity, is a difficult physiological challenge that requires specific preparation. The elevated humidity of a tropical climate impairs the thermoregulatory mechanisms by limiting the rate of sweat evaporation. Hence, a proper management of whole-body temperature is required to complete an ultra-endurance event in such an environment. In these long-duration events, which can last from 8 to 20 h, held in hot and humid settings, performance is tightly linked to the ability in maintaining an optimal hydration status. Indeed, the rate of withdrawal in these longer races was associated with lower water intake, and the majority of finishers exhibited alterations in electrolyte balance (e.g., sodium). Hence, this work reviews the effects on performance of high heat and humidity in two representative ultra-endurance sports, ultramarathons and long-distance triathlons, and several countermeasures to counteract the impact of these harsh environmental stresses and maintain a high level of performance, such as hydration, cooling strategies and heat acclimation.

Delineating the impacts of air temperature and humidity for endurance exercise

Abstract:New findings: What is the central question of this study? What are the independent effects of air temperature and humidity on performance, physiological and perceptual responses during endurance exercise? What is the main finding and its importance? When examined independently, elevated air temperature increased heat strain and impaired aerobic exercise performance, but to a lesser extent than has been reported previously. These findings highlight the importance of absolute humidity relative to temperature when exercising or working under severe heat stress. Abstract: Many studies have reported that ambient heat stress increases physiological and perceptual strain and impairs endurance exercise, but effects of air temperature per se remain almost unexamined. Most studies have used matched relative humidity, thereby exponentially increasing absolute humidity (water content in air) concurrently with temperature. Absolute (not relative) humidity governs evaporative rate and is more important at higher work rates and air temperatures. Therefore, we examined the independent effects of air temperature and humidity on performance, thermal, cardiovascular and perceptual measures during endurance exercise. Utilizing a crossover design, 14 trained participants (7 females) completed 45 min fixed-intensity cycling (70% V ̇ O 2 peak ${\dot V_{{{\rm{O}}_{\rm{2}}}{\rm{peak}}}}$ ) followed by a 20-km time trial in each of four environments: three air temperatures at matched absolute humidity (Cool, 18°C; Moderate, 27°C; and Hot, 36°C; at 1.96 kPa, air velocity ∼4.5 m/s), and one at elevated humidity (Hot Humid, 36°C at 3.92 kPa). Warmer air caused warmer skin (0.5°C/°C; P < 0.001), higher heart rate (1 bpm/°C; P < 0.001), sweat rate (0.04 l/h/°C; P < 0.001) and thermal perceptions during fixed-intensity exercise, but minimally affected core temperature (<0.01°C/°C; P = 0.053). Time-trial performance was comparable between Cool and Moderate (95% CI: -1.4, 5.9%; P = 0.263), but 3.6-6% slower in Hot (95% CI: ±2.4%; P ≤ 0.006). Elevated humidity increased core temperature (P < 0.001), perceived temperature and discomfort but not skin temperature or heart rate, and reduced mean blood pressure (P = 0.046) during fixed-intensity exercise. Elevated humidity impaired time-trial performance by 3.4% (95% CI: ±2.2%; P = 0.006). In conclusion, these findings quantify the importance of absolute humidity alongside air temperature when exercising under severe heat stress.
Humid Heat Equally Impairs Maximal Exercise Performance in Elite Para-Athletes and Able-Bodied Athletes

Abstract:Purpose: This study aimed to compare the impact of hot-humid environmental conditions on performance outcomes, thermoregulatory responses, and thermal perception during exercise between elite para- and able-bodied (AB) athletes. Methods: Twenty elite para-athletes (para-cycling and wheelchair tennis) and 20 elite AB athletes (road cycling, mountain biking, beach volleyball) performed an incremental exercise test in a temperate environment (mean ± SD, 15.2°C ± 1.2°C; relative humidity, 54% ± 7%) and a hot-humid environment (31.9°C ± 1.6°C, 72% ± 5%). Exercise tests started with a 20-min warm-up at 70% of maximal heart rate, after which power output increased by 5% every 3 min until volitional exhaustion. Results: Time to exhaustion was shorter in hot-humid versus temperate conditions, with equal performance loss for para- and AB athletes (median (interquartile range), 26% (20%-31%) vs 27% (19%-32%); P = 0.80). AB athletes demonstrated larger exercise-induced increases in gastrointestinal temperature (T gi ) in hot-humid versus temperate conditions (2.2 ± 0.7 vs 1.7 ± 0.5, P < 0.001), whereas T gi responses in para-athletes were similar between conditions (1.3 ± 0.6 vs 1.3 ± 0.4, P = 0.74). Para- and AB athletes showed similar elevations in peak skin temperature ( P = 0.94), heart rate ( P = 0.67), and thermal sensation score ( P = 0.64) in hot-humid versus temperate conditions. Conclusions: Elite para-athletes and AB athletes demonstrated similar performance decrements during exercise in hot-humid versus temperate conditions, whereas T gi elevations were markedly lower in para-athletes. We observed large interindividual variation within both groups, suggesting that in both para- and AB athletes, personalized heat mitigation plans should be developed based on individual thermal testing.
Wasted efforts of elite Marathon runners under a warming climate primarily due to atmospheric oxygen reduction

Abstract:Meteorological conditions exert influences on sport performances via complex processes. Previous studies conventionally investigate the effects of weather conditions on marathon performance by following events held at the same places, which brings large uncertainties due to the changed participants. Via following each athlete to eliminate these uncertainties, we show that elite athletes' marathon-running performance tends to monotonically worsen as ambient temperature rises except when it is extremely cold based on the best 16 athletes from each of the sex continents. It worsens by 0.56 (0.39 for men and 0.71 for women) min/°C when it rises beyond 15 °C. Theoretical analysis based on global marathon performance and weather observation datasets shows that more than half of this effect is mediated by reduced oxygen partial density as warmer temperature expands air and increases vapor pressure for both the men and women athletes, which is confirmed by the methods of Baron–Kenny. This atmospheric thermodynamic mechanism has not been emphasized previously. We also show that world-top athletes' marathon performance approximately linearly worsens as ambient pressure decreases and slightly worsens as relative humidity rises if not extremely wet. These results substantially differ from the previous ones following the events instead of each athlete. Multi-variable changes in climate system are projected to slow Olympic marathon athletes by 2.51 and 1.06 min by the end of the 21 st century in relative to 2020 under the high and intermediate emission scenario, respectively. Therefore, future progression of marathon performance is likely to be substantially slowed or even halted by climate changes without efficient greenhouse gas mitigation.

Abstract:ABSTRACT Purpose Muscle mechanics is paramount in our understanding of motor performance. However, little is known regarding the sensitivity of fascicle dynamics and connective tissues stiffness to exercise duration and ambient temperature during running, both increasing muscle temperature. This study aimed to determine gastrocnemius medialis (GM) fascicle dynamics in vivo during running in temperate and hot conditions, as well as muscle-tendon unit responses. Methods Using ultrafast ultrasound, fifteen participants (8 men, 7 women, 26 ± 3 years) were tested before, during (2 and 40 min) and after a running task (40 min at 10 km.h -1 ) in temperate (TEMP; ⁓23 °C) and hot (HOT: ⁓38 °C) conditions. Results While core, skin temperatures and heart rate increased from the beginning to the end of the exercise and in a larger extent in HOT than TEMP ( P < 0.001), the physiological stress elicited did not alter running temporal parameters and GM fascicle operating lengths, with similar behavior of the fascicles on their force-length relationship, over time (2 vs. 40 min) or across condition (TEMP vs. HOT; P ≥ 0.248). Maximal voluntary force production did not reported statistical changes after exercise ( P = 0.060), and the connective tissues stiffness measured ( i.e. , passive muscle and stiffness of the series-elastic elements) did not show neither time ( P ≥ 0.281), condition ( P ≥ 0.256) nor time × condition interaction ( P ≥ 0.465) effect. Conclusions This study revealed that prolonged running exercise does not alter muscle-tendon unit properties and interplay, which are not influenced by ambient temperature. These findings may rule out potential detrimental effects of heat on muscle properties and encourage further investigations on longer and more intense running exercise.

Cycling in the heat: performance perspectives and cerebral challenges

Abstract:Cycling performances require periods with high power output and consequently large endogenous heat production. During cycling in temperate or cold climates, heat is mainly released from the skin to the surroundings via convection, whereas evaporative heat loss becomes the dominant or only mechanism for heat dissipation when the environmental temperature increases. Accordingly, large sweat rates are required, which may challenge the cyclists' electrolyte and water balance. Furthermore, the cooling capacity of the environment may become a limiting factor for the ability to maintain heat balance, for example during cycling in very humid climates or when cycling up-hill as the wind speed decreases and reduces the maximal rate of evaporative heat loss. Hyperthermia may in itself hamper performance, but especially in combination with dehydration it may deteriorate the cyclist's ability to maintain power output. Fatigue mechanisms involve cardiovascular stressing, but it also appears that factors within the central nervous system are of major importance for motor performance during such exercise. However, the influence of the environmental temperature on cycling performance appears to vary markedly depending on the course, the air humidity and the cyclist ability to avoid dehydration. If hyperthermia becomes a major issue, it will deteriorate performance, but as long as temperature and water balance can be established, the high air temperature may actually benefit performance because air density and air resistance will decrease and lower the power output required to maintain a given velocity.
Change in interleukin (IL)-6, 8, and 10 and its association with an increase in core temperature following a 7-mile running race in the warm weather

Abstract:The purposes of this study were 1) to investigate if cytokines were increased following a running road-race, and 2) to examine associations between cytokines and hyperthermia. Seventy-seven recreational runners participated in this study which occurred at the 7-mile race in the heat (ambient temperature, 25.0–26.7°C; %RH, 56.7–79.0 ± 5.0%). Before and following the race, blood draws were performed to measure circulating inflammatory cytokines. Core temperature was measured using an ingestible thermistor throughout the race. Core temperature was significantly higher at post-race (39.5 ± 0.7°C) than pre-race (36.9 ± 0.4°C, p < 0.001). IL-6, IL-8, and IL-10 significantly increased at post-race (IL-6, 48.0 ± 22.3 pg⋅ml −1 ; IL-8, 63.8 ± 23.9 pg⋅ml −1 ; IL-10, 29.2 ± 20.0 pg⋅ml −1 ) compared to pre-race (IL-6, 28.4 ± 13.6 pg⋅ml −1 ; IL-8, 53.2 ± 19.4 pg⋅ml −1 ; IL-10, 18.6 ± 11.9 pg⋅ml −1 , p < 0.001). A greater increase in core temperature pre- to post-race was predicted by the faster finish time, a greater increase in IL-6, and greater body mass loss during the race (r 2 = 0.298, p < 0.001). Small associations were found between IL-8 and core temperature at post-race ( r = 0.255, p = 0.025). In conclusion, cytokines concentrations and core temperature increased following the race. Moreover, post-race hyperthermia is associated with increased IL-6, faster finish times, and higher body mass losses.

Abstract:Thermal safety guidelines with upper thresholds aim to protect athletes' health, yet evidence-based sport-specific thresholds remain unestablished. Experimenting with athletes in severely hot conditions raises ethical concerns, so we used a thermo-physiological model to validate the thresholds of guidelines for outdoor sports. First, the reproducibility of the joint system thermoregulation model (JOS-3) of core temperature has been validated for 18 sports experiments ( n = 213) and 11 general exercise experiments ( n = 121) using the Bland – Altman analysis. Then, core temperatures were predicted using the JOS-3 in conditions corresponding to the upper thresholds, and if the 90 th –99.7 th percentile core temperature value (corresponding to 0.3%–10% of the participants) exceeded 40°C, the thresholds were judged as potentially hazardous. Finally, we proposed revisions for sports with potentially hazardous thresholds. As a result, the JOS-3 could simulate core temperature increases in most experiments (27/29) for six sports and general exercises with an accuracy of 0.5°C. The current upper thresholds for marathons, triathlons, and football are potentially hazardous. Suggested revisions, based on specified percentiles, include: Football: revise from wet bulb globe temperature (WBGT) 32°C to 29–31°C or not revise. Marathon: revise from WBGT 28°C to 24–27°C. Triathlon: revise from WBGT 32.2°C to 23–26°C. If conducting sports events under the revised upper thresholds proves difficult, taking measures for a possible high incidence of heat illness becomes crucial, such as placing additional medical resources, assisting heat acclimatization and cooling strategies for participants, and rule changes such as shorter match times and increased breaks.
Relationship Between Running Biomechanics and Core Temperature Across a Competitive Road Race

Abstract:Background: Outdoor races introduce environmental stressors to runners, and core temperature changes may influence runners' movement patterns. This study assessed changes and determined relationships between sensor-derived running biomechanics and core temperature among runners across an 11.27-km road race. Hypothesis: Core temperatures would increase significantly across the race, related to changes in spatiotemporal biomechanical measures. Study design: Cross-sectional cohort study. Level of evidence: Level 3. Methods: Twenty runners (9 female, 11 male; age, 48 ± 12 years; height, 169.7 ± 9.1 cm; mass, 71.3 ± 13.4 kg) enrolled in the 2022 Falmouth Road Race were recruited. Participants used lightweight technologies (ingestible thermistors and wearable sensors) to monitor core temperature and running biomechanics throughout the race. Timestamps were used to align sensor-derived measures for 7 race segments. Observations were labeled as core temperatures generally within normal limits (<38°C) or at elevated core temperatures (≥38°C). Multivariate repeated measures analyses of variance were used to assess changes in sensor-derived measures across the race, with Bonferroni post hoc comparisons for significant findings. Pearson's r correlations were used to assess the relationship between running biomechanics and core temperature measures. Results: Eighteen participants developed hyperthermic core temperatures (39.0°C ± 0.5°C); core temperatures increased significantly across the race (P < 0.01). Kinetic measures obtained from the accelerometers, including shock, impact, and braking g, all significantly increased across the race (P < 0.01); other sensor-derived biomechanical measures did not change significantly. Core temperatures were weakly associated with biomechanics (|r range|, 0.02-0.16). Conclusion: Core temperatures and kinetics increased significantly across a race, yet these outcomes were not strongly correlated. The observed kinetic changes may have been attributed to fatigue-related influences over the race. Clinical relevance: Clinicians may not expect changes in biomechanical movement patterns to signal thermal responses during outdoor running in a singular event.
Effects of Elevated Body Temperature on Selected Physiological Indices and Thermal Stress in Athletes and Non-Athletes

Abstract:The purpose of this study was to determine the effects of active and passive overheating. A group of athletes (A) and non-athletic men (N) underwent an exercise stress test at elevated ambient temperature and sauna bathing until an increase in rectal temperature of 1.2°C was observed. It was shown that group A performed physical exercise for a longer period of time, which elicited significantly higher amounts of work performed in the stress test. Both forms of overheating caused a significant decrease in BM and a significant change in plasma volume, while greater dehydration was observed after active overheating. Changes observed in group A were higher than in group N. MCV levels were initially higher in group A. The levels in both groups decreased after sauna bathing, although in non-athletes the decrease was greater. Both forms of overheating increased Hb, HCT, and SBP, while only the non-athletic group showed a decrease in DBP after the exercise stress test. However, a decrease in DBP was observed in both groups after sauna bathing. The PSI increased after both tests, yet to a higher extent after the stress test than after sauna bathing. The PSI was negatively correlated with VO 2max in both groups. The increase in cortisol concentration was higher after sauna bathing. There was a correlation between cortisol levels and the work performed during the stress test in group A. Endurance training resulted in more efficient thermoregulatory mechanisms in athletes.

Abstract:Nine male student games players consumed either flavoured water (0.1 g carbohydrate, Na+ 6 mmol x l(-1)), a solution containing 6.5% carbohydrate-electrolytes (6.5 g carbohydrate, Na+ 21 mmol x l(-1)) or a taste placebo (Na+ 2 mmol x l(-1)) during an intermittent shuttle test performed on three separate occasions at an ambient temperature of 30 degrees C (dry bulb). The test involved five 15-min sets of repeated cycles of walking and variable speed running, each separated by a 4-min rest (part A of the test), followed by 60 s run/60 s rest until exhaustion (part B of the test). The participants drank 6.5 ml x kg(-1) of fluid as a bolus just before exercise and thereafter 4.5 ml x kg(-1) during every exercise set and rest period (19 min). There was a trial order effect. The total distance completed by the participants was greater in trial 3 (8441 +/- 873 m) than in trial 1 (6839 +/- 512, P < 0.05). This represented a 19% improvement in exercise capacity. However, the trials were performed in a random counterbalanced order and the participants completed 8634 +/- 653 m, 7786 +/- 741 m and 7099 +/- 647 m in the flavoured water (FW), placebo (P) and carbohydrate-electrolyte (CE) trials, respectively (P = 0.08). Sprint performance was not different between the trials but was impaired over time (FW vs P vs CE: set 1, 2.41 +/- 0.02 vs 2.39 +/- 0.03 vs 2.39 +/- 0.03 s; end set, 2.46 +/- 0.03 vs 2.47 +/- 0.03 vs 2.47 +/- 0.02 s; main effect time, P < 0.01). The rate of rise in rectal temperature was greater in the carbohydrate-electrolyte trial (rise in rectal temperature/duration of trial, degrees C x h(-1); FW vs CE, P < 0.05; P vs CE, N.S.). Blood glucose concentrations were higher in the carbohydrate-electrolyte than in the other two trials (FW vs P vs CE:rest, 4.4 +/- 0.1 vs 4.3 +/- 0.1 vs 4.2 +/- 0.1 mmol x l(-1); end of exercise, 5.4 +/- 0.3 vs 6.4 +/- 0.6 vs 7.2 +/- 0.5 mmol x l(-1); main effect trial, P < 0.05; main effect time, P < 0.01). Plasma free fatty acid concentrations at the end of exercise were lower in the carbohydrate-electrolyte trial than in the other two trials (FW vs P vs CE: 0.57 +/- 0.08 vs 0.53 +/- 0.11 vs 0.29 +/- 0.04 mmol x l(-1); interaction, P < 0.01). The correlation between the rate of rise in rectal temperature (degrees C x h(-1)) and the distance completed was -0.91, -0.92 and -0.96 in the flavoured water, placebo and carbohydrate-electrolyte conditions, respectively (P < 0.01). Heart rate, blood pressure, plasma ammonia, blood lactate, plasma volume and rate of perceived exertion were not different between the three fluid trials. Although drinking the carbohydrate-electrolyte solution induced greater metabolic changes than the flavoured water and placebo solutions, it is unlikely that in these unacclimated males carbohydrate availability was a limiting factor in the performance of intermittent running in hot environmental conditions.

Abstract:Decreasing latitude and increasing frequency of population lactase nonpersistence have been reported to diminish risks for several diseases, but the reason for overlap has not been explained. We evaluate, relationships between calculated national annual ultraviolet light B (UVB) exposure, latitude, and national lactose digestion frequencies. Annual UVB exposure and latitude were based on weighted averages from several cities in different countries. Lactase distribution status was based on published data that have been used previously to derive relations with diseases. We compare univariate regression analyses (r2 adj, slope) of percentage of lactase nonpersistence with UVB or latitude. We determine, differences between European and non-European sources by multiregression analysis of independent variables. Correlation between UVB and latitude is high (r2 = 0.89), and between percentage of lactase nonpersistence and either latitude or UVB the correlation is moderately strong with r2 = 0.51 and 0.46, respectively, with P ≤ 0.01 for both. A more detailed analysis shows that correlations between percentage of lactase nonpersistence and UVB are only significant in Europe, r2 = 0.59, P < 0.001, whereas outside Europe: r2 = 0.06, P = 0.16. These relationships raise hypothetical explanations to account for the observed overlap in similar risk modification by the 2 variables.
Differential patterns of sweat and blood lactate concentration response during incremental exercise in varied ambient temperatures: A pilot study

Abstract:Blood lactate concentration during exercise is a reliable indicator of energy metabolism and endurance performance. Lactate is also present in sweat, and sweating plays an important role in thermoregulation, especially in hot conditions. Recently, wearable sensors have enabled the real-time and noninvasive measurement of sweat lactate concentration, potentially serving as an alternative indicator of blood lactate response. However, the evidence regarding the relationship between sweat and blood lactate responses during incremental exercise in hot conditions is lacking. In a randomized cross-over design, six highly trained male runners completed two incremental treadmill tests under normal (20°C/50%RH) or hot (30°C/50%RH) conditions. The tests include 3-min running stages and 1-min recovery, starting at 12 km/h and increasing by 1 km/h at each stage. Blood and sweat lactate concentrations were measured at each stage to determine blood and sweat lactate thresholds (LT). Blood lactate concentrations were higher under hot conditions (p < 0.01), but there was no difference in the response pattern or velocity at blood LT between conditions. Significant early increase (p < 0.01) in sweat lactate and low velocity at sweat LT (p < 0.05) were observed under hot conditions. A significant correlation between blood and sweat lactate concentrations was found under normal conditions (p < 0.001) but not under hot conditions, and no significant correlations were observed between the velocity at blood and sweat LT. In conclusion, sweat lactate concentration does not consistently reflect blood lactate concentration during incremental exercise.

Abstract:Inducing a heat-acclimated phenotype via repeated heat stress improves exercise capacity and reduces athletes̓ risk of hyperthermia and heat illness. Given the increased number of international sporting events hosted in countries with warmer climates, heat acclimation strategies are increasingly popular among endurance athletes to optimize performance in hot environments. At the tissue level, completing endurance exercise under heat stress may augment endurance training adaptation, including mitochondrial and cardiovascular remodeling due to increased perturbations to cellular homeostasis as a consequence of metabolic and cardiovascular load, and this may improve endurance training adaptation and subsequent performance. This review provides an up-to-date overview of the metabolic impact of heat stress during endurance exercise, including proposed underlying mechanisms of altered substrate utilization. Against this metabolic backdrop, the current literature highlighting the role of heat stress in augmenting training adaptation and subsequent endurance performance will be presented with practical implications and opportunities for future research. Hyperthermia and Exertional Heatstroke During Running, Cycling, Open Water Swimming, and Triathlon Events

Abstract:Few previous epidemiological studies, sports medicine position statements, and expert panel consensus reports have evaluated the similarities and differences of hyperthermia and exertional heatstroke (EHS) during endurance running, cycling, open water swimming, and triathlon competitions. Accordingly, we conducted manual online searches of the PubMed and Google Scholar databases using pre-defined inclusion criteria. The initial manual screenings of 1192 article titles and abstracts, and subsequent reviews of full-length pdf versions identified 80 articles that were acceptable for inclusion. These articles indicated that event medical teams recognized hyperthermia and EHS in the majority of running and triathlon field studies (range, 58.8 to 85.7%), whereas few reports of hyperthermia and EHS appeared in cycling and open water swimming field studies (range, 0 to 20%). Sports medicine position statements and consensus reports also exhibited these event-specific differences. Thus, we proposed mechanisms that involved physiological effector responses (sweating, increased skin blood flow) and biophysical heat transfer to the environment (evaporation, convection, radiation, and conduction). We anticipate that the above information will help race directors to distribute pre-race safety advice to athletes and will assist medical directors to better allocate medical resources (eg, staff number and skill sets, medical equipment) and optimize the management of hyperthermia and EHS.
A unifying model to estimate the effect of heat stress in the human innate immunity during physical activities

Abstract:Public health is threatened by climate change and extreme temperature events worldwide. Differences in health predispositions, access to cooling infrastructure and occupation raises an issue of heat-related health inequality in those vulnerable and disadvantaged demographic groups. To address these issues, a comprehensive understanding of the effect of elevated body temperatures on human biological systems and overall health is urgently needed. In this paper we look at the inner workings of the human innate immunity under exposure to heat stress induced through exposure to environment and physical exertion. We couple two experimentally validated computational models: the innate immune system and thermal regulation of the human body. We first study the dynamics of critical indicators of innate immunity as a function of human core temperature. Next, we identify environmental and physical activity regimes that lead to core temperature levels that can potentially compromise the performance of the human innate immunity. Finally, to take into account the response of innate immunity to various intensities of physical activities, we utilise the dynamic core temperatures generated by a thermal regulation model. We compare the dynamics of all key players of the innate immunity for a variety of stresses like running a marathon, doing construction work, and leisure walking at speed of 4 km/h, all in the setting of a hot and humid tropical climate such as present in Singapore. We find that exposure to moderate heat stress leading to core temperatures within the mild febrile range (37, 38][Formula: see text], nudges the innate immune system into activation and improves the efficiency of its response. Overheating corresponding to core temperatures beyond 38[Formula: see text], however, has detrimental effects on the performance of the innate immune system, as it further induces inflammation, which causes a series of reactions that may lead to the non-resolution of the ongoing inflammation. Among the three physical activities considered in our simulated scenarios (marathon, construction work, and walking), marathon induces the highest level of inflammation that challenges the innate immune response with its resolution. Our study advances the current state of research towards understanding the implications of heat exposure for such an essential physiological system as the innate immunity. Although we find that among considered physical activities, a marathon of 2 h and 46 min induces the highest level of inflammation, it must be noted that construction work done on a daily basis under the hot and humid tropical climate, can produce a continuous level of inflammation triggering moieties stretched at a longer timeline beating the negative effects of running a marathon. Our study demonstrates that the performance of the innate immune system can be severely compromised by the exposure to heat stress and physical exertion. This poses significant risks to health especially to those with limited access to cooling infrastructures. This is due in part to having low income, or having to work on outdoor settings, which is the case for construction workers. These risks to public health should be addressed through individual and population-level measures via behavioural adaptation and provision of the cooling infrastructure in outdoor environments.

Abstract:Objectives Exertional-heat stress generates a thermoregulatory strain that exacerbates splanchnic hypoperfusion and sympathetic drive, but the effects on gastrointestinal function are poorly defined. The study aimed to determine the effects of exertional-heat stress on gastric myoelectrical activity, orocecal transit time (OCTT), and gastrointestinal symptoms (GIS). Design Randomised cross-over study. Methods Endurance runners ( n = 16) completed 2 h of running at 60 % V̇ O 2max in 35 ° C (HOT) and 22 ° C (TEMP) ambient conditions. Surface electrogastrography (cEGG) was recorded pre- and post-exercise to determine gastric myoelectrical activity, a lactulose challenge was used to determine OCTT, and GIS were recorded using a modified visual analogue scale tool. Results Post-exercise T re [HOT:38.8(38.5 to 39.0)°C and TEMP:38.1(37.8 to 38.4)°C] and Δ T re [HOT:2.2(2.0 to 2.4) ° C and TEMP:1.5(1.2 to 1.8) ° C] was higher on HOT compared to TEMP ( p < 0.001). Normal gastric myoelectrical cycle frequency reduced ( p = 0.010) on HOT [−11.7(−20.8 to −2.6)%], but this decrease did not differ ( p = 0.058) from TEMP [−2.7(−8.3 to 3.0)%]. Bradygastria increased post-exercise on both trials (HOT:11.3(2.3 to 20.4)%, p = 0.030; and TEMP:7.4(2.1 to 12.6)%, p = 0.009). OCTT did not differ between trials ( p = 0.864) with transit response classified as very slow on both HOT (99(68 to 131)min) and TEMP (98(74 to 121)min). GIS incidence was higher on HOT (88 %) compared to TEMP (81 %), in accordance with greater total-GIS and upper-GIS severity ( p = 0.005 and p = 0.033, respectively). Conclusions Running for 2 h at 60 % V̇ O 2max in either hot or temperate ambient conditions instigates perturbations in myoelectrical activity and OCTT, with GIS incidence and severity greater in hot conditions.

Abstract:Different strategies for cooling the body prior to or during physical exercise have been shown to improve prolonged performance. Because of ethical and methodological issues, no studies conducted in humans have evaluated the changes in brain temperature promoted by cooling strategies. Therefore, our first aim sought to measure the hypothalamic temperature (Thyp) of rats subjected to treadmill running in a cold environment. Moreover, evidence suggests that Thyp and abdominal temperature (Tabd) are regulated by different physiological mechanisms. Thus, this study also investigated the dynamics of exercise-induced changes in Thyp and Tabd at two ambient temperatures: 25°C (temperate environment) and 12°C (cold). Adult male Wistar rats were used in these experiments. The rats were implanted with a guide cannula in the hypothalamus and a temperature sensor in the abdominal cavity. After recovery from this surgery, the rats were familiarized with running on a treadmill and were then subjected to the two experimental trials: constant-speed running (20 m/min) at 12°C and 25°C. Both Thyp and Tabd increased during exercise at 25°C. In contrast, Thyp and Tabd remained unchanged during fatiguing exercise at 12°C. The temperature differential (i.e., Thyp - Tabd) increased during the initial min of running at 25°C and thereafter decreased toward pre-exercise values. Interestingly, external cooling prevented this early increase in the temperature differential from the 2nd to the 8th min of running. In addition, the time until volitional fatigue was higher during the constant exercise at 12°C compared with 25°C. Together, our results indicate that Thyp and Tabd are regulated by different mechanisms in running rats and that external cooling affected the relationship between both temperature indexes observed during exercise without environmental thermal stress. Our data also suggest that attenuated hypothalamic hyperthermia may contribute to improved performance in cold environments.
