Abstract:Lateral lymph node dissection (LLND) for rectal cancer is still not a widely established technique owing to the existing controversy between Eastern and Western countries and the lack of well-designed studies. The risk of complications and the paucity of long-term oncological results are significant drawbacks for further applying this technique. The use of indocyanine green (ICG) near-infrared (NIR) fluorescence for LLND appears as a promising technique for enhancing postoperative and oncological outcomes. This review aims to evaluate the emerging role of ICG during LLND and present the benefits of its application.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is that in rectal cancer surgery, the application of lateral lymph node dissection (LLND) technology is controversial, especially with different views between Eastern and Western countries. Although LLND may be effective in reducing local recurrence, its high complication risk and limited long - term oncological results limit its wide application. The use of indocyanine green (ICG) near - infrared (NIR) fluorescence technology for LLND is considered a promising method to improve postoperative and oncological outcomes. Therefore, this systematic review aims to evaluate the emerging role of ICG in LLND and demonstrate the benefits of its application. Specifically, the main research questions are: 1. **Can the application of ICG in LLND increase the number of resected lateral lymph nodes?** 2. **Can ICG - guided LLND reduce the amount of intraoperative blood loss?** 3. **Can ICG - guided LLND shorten the operation time?** 4. **Can ICG - guided LLND reduce the postoperative hospital stay and the incidence of urinary retention?** By systematically reviewing the literature, researchers hope to answer these questions to evaluate the safety and feasibility of ICG in LLND and provide a basis for further research.
