Abstract:Objective: The aim of this study was to evaluate the efficacy and safety of the Chinese herbal formula San-Huang Gu-Ben Zhi-Ke (SHGBZK) as a treatment for patients with stable chronic obstructive pulmonary disease (COPD) diagnosed with lung-spleen Qi deficiency. Method: A randomized, double-blind, placebo-controlled trial was designed. 98 adults aged between 40 and 80 years with stable COPD diagnosed with lung-spleen Qi deficiency were included. All participants received basic treatment for COPD. Patients in the experimental group took SHGBZK, while the control group took placebo. The primary outcome was the frequency of acute exacerbation. The secondary outcomes were lung function, symptom score, exercise capacity and quality of life. Results: Of 98 patients who underwent randomization, 50 patients in the SHGBZK group and 48 in the placebo group were included in the full analysis set. After 24-week therapy and 28-week follow-up, patients in treatment group had significant improvements in symptom, exercise capacity and quality of life. After Subgroup analysis, the frequency of acute exacerbation in patients with a COPD Assessment Test (CAT) score of at least 10 or a modified Medical Research Council (mMRC) score of at least 2 was significantly lower in the SHGBZK group than in the placebo group. Lung function in patients with frequent exacerbation was significantly higher in the SHGBZK group than in the placebo group. The incidence of adverse events was generally similar in the two groups. Conclusion: SHGBZK had beneficial effects on symptom, exercise capacity and quality of life in stable COPD patients. SHGBZK also had the potential to reduce the frequency of exacerbation and improve lung function in specific groups of COPD patients. Analytical volume calculation of voxel fragments for iterative, fully 3D PET reconstruction

Abstract:Using iterative algorithms to reconstruct quantitative 3D PET data requires the computation of the system probability matrix. The pure geometrical contribution to the matrix elements can easily be approximated by the length of intersection between detected lines of response (LOR) and individual voxels. However, the natural observables are volume-based, i.e. the coincidence rates which are usually detected in tubes of response (TOR) as well as the activity concentration in each voxel refer to specific spatial fragments. Therefore, we developed a fast method for the analytical calculation of the 3D shape and volume of intersection polyhedrons to build the system matrix with an improved description of the geometrical detection probability. Additionally, we considered effects of detector blurring within the system matrix. Gaussian resolution functions have been assumed in the radial and axial direction describing migrations between adjacent TORs. Finally, exploiting intrinsic symmetry relations and the sparseness of the system matrix allowed to create an efficiently small matrix representation which could be pre-computed and completely stored in memory. Time-consuming voxel addressing calculations due to necessary symmetry operations have been completely avoided by using an octant-wise symmetrically ordered field of voxels. The above algorithm has been applied for an iterative, fully 3D reconstruction (MLEM/OSEM) of pre-corrected 3D sinograms provided by a Siemens/CTI HR+PET scanner. A comparison about the results with other established reconstruction platforms is given where our analytical volume-based approach yields a superior contrast behaviour at a lower level of noise.
Better response to Tanreqing injection in frequent acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients—Real-world evidence from a nationwide registry (ACURE) study

Abstract:Objective: Our aim was to systematically investigate the efficacy of Tanreqing (TRQ) injection on in-hospital outcomes among inpatients with frequent or infrequent AECOPD. Methods: In this ongoing, nationwide multicenter registry designed to investigate clinical characteristics, management, and prognoses of Chinese patients admitted for AECOPD in real-world settings, we collected characteristics, comorbidities, in-hospital prognoses, and information on the COPD assessment test (CAT) questionnaire, PEACE questionnaire, and modified British Medical Research Council (mMRC) questionnaire from each enrolled patient. Frequent AECOPD was determined as being admitted to the hospital ≥1 time or visiting the emergency room (ER) ≥ 2 times due to AECOPD within a year. A propensity match method and univariable and multivariable regression models were performed to analyze the efficacy of TRQ on clinical outcomes for inpatients with frequent AECOPD. Results: A total of 4135 inpatients were involved in the analysis, including 2179 administered with TRQ and 1956 not administered with TRQ. Among those administered with TRQ, 493 had frequent AECOPD and 358 had infrequent AECOPD. After a propensity score match, a significant reduction of CAT score at discharge (TRQ median 12, IQR 8.0–16.0; non-TRQ median 13, IQR 9.0–18.0, p = 0.0297), a lower rate of ICU admission (TRQ 0.8% vs. non-TRQ 2.6%, p = 0.0191), and a shorter length of stay (LOS) (TRQ median 11, IQR 9.0–14.0; non-TRQ median 11, IQR 8.0–14.0, p = 0.004) were observed in the TRQ group, compared with the non-TRQ group among frequent AECOPD patients. In the subgroup analysis, for those with a PEACE score >7 on admission, TRQ contributed to a significantly lower CAT score at discharge ( p = 0.0084) and a numerically lower ICU admission rate with a marginal statistical significance. Among those with phlegm-heat symptom complex on admission ≥2, a lower CAT score at discharge and a lower ICU admission were also observed in the TRQ group. Conclusion: TRQ injection had better efficacy in patients with frequent AECOPD in reducing ICU admission and alleviating respiratory symptoms, especially for those with higher severity on admission or more phlegm-heat symptoms.

[Study of clinical effect on treatment of chronic obstructive pulmonary disease in acute aggravated stage with Tanreqing injection and cell factor level]

Abstract:Objective: To make a study of the clinical effect on the treatment of chronic obstructive pulmonary disease in acute aggravated stage with Tanreqing Injection and the changes of the cell factor level. Method: Eighty-seven cases of COPD patients in acute aggravated stage were chosen and divided into two groups at random. Forty-four cases in treatment group were given intravenous drip of Tanreqing (20 mL) with 5% G/NS (500 mL) once a day. Forty-three cases in control group were given intravenous drip Yanhuning injection (0.4 g) with 5% G/NS (500 mL) once a day. The courses of the treatment were both two weeks and the changes of TL-10, TNF-alpha and MPO were observed separately before and after the treatment. Result: The total effective rate in the treatment group was 94.31% while the control group 83.75%, which had significant difference (P<0.05). IL-10 level in both groups was comparatively lower than the healthy (P<0.01), while TNF-alpha and MPO was higher than the healthy (P<0.01). After the treatment, the TNF-alpha and MPO level in the treatment group decreased more greatly than those in the control groups (P<0.01) but the IL-10 increased a lot more than those in the control group and there existed remarkable difference (P<0.01). Conclusion: The treatment of chronic obstructive pulmonary disease in acute aggravated stage with Tanreqing Injection can have noticeable effect, and can prohibit the release of above cell factors. It is also safe in clinical use without obvious side effects and has played an important role in the treatment of chronic obstructive pulmonary disease in acute aggravated stage.
Effective-Component Compatibility of Bufei Yishen Formula III Combined with Electroacupuncture Suppresses Inflammatory Response in Rats with Chronic Obstructive Pulmonary Disease via Regulating SIRT1/NF- κ B Signaling

Abstract:Objective: To explore more efficient treatments for chronic obstructive pulmonary disease (COPD), effective-component compatibility of Bufei Yishen formula III (ECC-BYF III) and electroacupuncture were tested on rats with COPD, and silent information regulator transcript-1 (SIRT1)/nuclear factor-kappaB (NF-κB) signaling was further investigated to interpret the therapy. Methods: In total, 70 rats were randomly divided into control (Control), model (Model), aminophylline (APL), ECC-BYF III, electroacupuncture (EA), ECC-BYF III+EA, and sham electroacupuncture (SA) groups. Cigarette smoke exposure combined with repeated bacterial infections was used to establish COPD models in 1-12 weeks. From 13 to 20 weeks, the ECC-BYF III and APL groups received corresponding drugs; the EA group received electroacupuncture therapy, wherein Dazhui (GV 14), Feishu (BL 13), and Shenshu (BL 23) points were selected; the ECC-BYF III+EA group received ECC-BYF III intragastrically combined with electroacupuncture; and the SA group received simulated electroacupuncture (nonacupoint). Pulmonary function, pulmonary histopathology, the expressions of SIRT1/NF-κB signaling, and inflammation-related mRNA and protein were detected. Results: Significant deterioration was observed in pulmonary function and pulmonary histopathology in rats with COPD (P < 0.01), and inflammatory state was illustrated by increased levels of interleukin- (IL-) 6 and tumor necrosis factor alpha (TNF-α) and decreased levels of IL-10 (P < 0.01). After the intervention of APL, ECC-BYF III, EA, and ECC-BYF III+EA, both pulmonary function and pulmonary histopathology were improved (P < 0.05 and P < 0.01), whereas the levels of IL-6 and TNF-α were decreased and IL-10 was increased (P < 0.05 and P < 0.01). Additionally, the mRNA expressions of IL-6, TNF-α, NF-κB, and acetylated NF-κBp65 (Ac-NF-κB) were noted to decrease, and SIRT1 and IL-10 were increased (P < 0.05 and P < 0.01); the protein expression of SIRT1 was upregulated, and NF-κBp65 and Ac-NF-κB were downregulated (P < 0.05 and P < 0.01). The effect of ECC-BYF III+EA was better in terms of improving pulmonary function and alleviating inflammation than that of the other treatment groups (P < 0.01 and P < 0.05). Conclusions: ECC-BYF III, electroacupuncture, and their combination can suppress inflammation, among which the combination therapy has been proven to be the most effective treatment, and the mechanism may be involved in activating SIRT1/NF-κB signaling.
Efficacy and safety of Tanreqing injection for cough caused by acute trachea-bronchitis disease: A systematic review and meta-analysis of randomized controlled trials

Abstract:Ethnopharmacological relevance Tanreqing injection (TRQI) is an intravenous herbal preparation derived from 5 types of traditional Chinese medicines including Scutellariae Radix, Lonicerae Japonicae Flos, Forsythiae Fructus, bear bile powder and goral horn, incorporating baicalin, chlorogenic acid, ursodeoxycholic acid, and goose deoxycholic acid and other compounds known for anti-inflammatory properties, is widely used in China to treat cough caused by acute trachea-bronchitis disease (ATB). Aim of the study To investigate the clinical efficacy and safety of Tanreqing injection (TRQI) with and without Western medicine (WM) for cough caused by acute trachea-bronchitis (ATB). Materials and methods We systematically searched eight databases, including CENTRAL, Embase, PubMed, Science Direct, Wiley, China National Knowledge Infrastructure, Chinese Biomedical Literature Database and WanFang, from inception to September 2022 for randomized clinical trials (RCTs) on TRQI for cough caused by ATB. The critical outcomes of interest were time to symptom disappearance, including time for cough symptom to disappear and time to improve cough and sputum production. Important outcomes included symptom disappearance rate, adverse events (AEs) and lung function. We carried out random-effects meta-analysis using Review Manager 5.4 and assessed the certainty of evidence utilizing the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results A total of 2872 citations were identified by our search, of which 26 eligible RCTs enrolled 2731 participants. Low to moderate certainty evidence showed that when compared with WM, TRQI plus WM treatment was associated with a favorable effect on the time for cough symptom to disappear (MD -2.21 d, 95% CI −2.64 to −1.78), time to improve cough and sputum production (MD −0.68 d, 95% CI −0.83 to −0.53), symptom disappearance rate (RR 1.37, 95% CI 1.20 to 1.55), forced vital capacity, and forced expiratory volume in 1 s (MD 0.38 L, 95% CI 0.26 to 0.50; MD 2.92%, 95% CI 1.29 to 4.56, respectively). In terms of AEs, there was no association between TRQI plus WM and WM (RR 0.55, 95% CI 0.14 to 2.21; low-certainty evidence). Very low certainty evidence showed that TRQI alone was associated with reduced time to improve cough and sputum (MD -0.14 d, 95% CI −0.26 to −0.02) and increased symptom disappearance rate (RR 1.89, 95% CI 1.24 to 2.88; low certainty evidence) compared to WM. Conclusions The overall efficacy of TRQI or WM for ATB cough is better than that of WM, and TRQI also effectively improve symptoms in patients with similar adverse events. However, due to the lack of methodological rigor of included studies, the present findings should be interpreted with caution. We advocate better high-quality and convincing clinical studies to be performed to prove the effectiveness and safety of TRQIs.

[Network Meta-analysis of heat-clearing and detoxifying Chinese medicine injections in treatment of acute exacerbation of chronic obstructive pulmonary disease].

Abstract:This study aims to evaluate the efficacy and safety of heat-clearing and detoxifying Chinese medicine injections in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Randomized controlled trial(RCT) on the treatment of AECOPD with heat-clearing and detoxifying Chinese medicine injections were retrieved from 8 databases including CNKI and PubMed(from establishment to July 11, 2021). Related information in eligible articles was extracted, and the quality of the included articles was assessed by Cochrane collaboration's tool for assessing risk of bias. Stata SE 15.1 and ADDIS 1.16.6 were employed for data analysis. A total of 81 RCTs were screened out, involving 7 526 patients(3 782 in the experimental group and 3 744 in the control group). According to the statistical difference and network Meta-analysis, the injections are in the order of(1)Reduning Injection+conventional western medicine>Tanreqing Injection+conventional western medicine in improving the effective rate,(2)Reduning Injection+conventional western medicine>Tanreqing Injection+conventional western medicine in decreasing C-reactive protein(CRP),(3)Reduning Injection+conventional western medicine>Xiyanping Injection+conventional western medicine>Tanreqing Injection+conventional western medicine in reducing white blood cell count(WBC),(4)Yuxingcao Injection+conventional western medicine>Reduning Injection+conventional western medicine>Tanreqing Injection+conventional western medicine in lowering partial pressure of carbon dioxide(PaCO_2),(5)Yuxingcao Injection+conventional western medicine>Reduning Injection+conventional western medicine>Tanreqing Injection+conventional western medicine>Xiyanping Injection+conventional western medicine in improving partial pressure of oxygen(PaO_2), and(6)Qingkailing Injection+conventional western medicine>Tanreqing Injection+conventional western medicine in shortening mean hospital stay. In terms of safety, none of the five injections have serious adverse reactions. The five heat-clearing and detoxifying Chinese medicine injections are effective for AECOPD, but the mechanisms are different. Among them, Reduning Injection+conventional western medicine and Tanreqing Injection+conventional western medicine demonstrate better and more effects. Due to the differences in the quantity and quality of included studies, the conclusion needs to be further verified.

Abstract:Background: Chinese herbal injections (CHIs) are commonly prescribed in China as adjuvant therapy for acute exacerbation of chronic obstructive pulmonary disease (AECOPD). However, evidence supporting the effect of CHIs on inflammatory factors for patients with AECOPD is insufficient, posing a challenge for clinicians to choose the optimal CHIs for AECOPD. This network meta-analysis (NMA) aimed to compare the effectiveness of several CHIs combined with Western Medicine (WM) and WM alone on the inflammatory factors in AECOPD.Methods: Randomized controlled trials (RCTs) on different CHIs for treating AECOPD were thoroughly searched from several electronic databases up to August 2022. The quality assessment of the included RCTs was conducted according to the Cochrane risk of bias tool. Bayesian network meta-analyses were designed to assess the effectiveness of different CHIs. Systematic Review Registration CRD42022323996.Results: A total of 94 eligible RCTs involving 7,948 patients were enrolled in this study. The NMA results showed that using Xuebijing (XBJ), Reduning (RDN), Tanreqing (TRQ), and Xiyanping (XYP) injections combined with WM significantly improved treatment effects compared to using WM alone. XBJ + WM and TRQ + WM significantly changed the level of C-reactive protein (CRP), white blood cells, percentage of neutrophils, interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α). TRQ + WM showed the most significant effect in reducing the level of procalcitonin. XYP + WM and RDN + WM could reduce the level of white blood cells and the percentage of neutrophils. A total of 12 studies reported adverse reactions in detail, and 19 studies demonstrated no significant adverse reactions.Conclusions: This NMA showed that using CHIs combined with WM could significantly reduce the level of inflammatory factors in AECOPD. A combination of TRQ and WM may be a relatively prior adjuvant therapy option for AECOPD treatment considering its effects in reducing the levels of the anti-inflammatory mediators.

Discussion: Strategies for Postoperative Seroma Prevention: A Systematic Review.

Abstract:www.PRSJournal.com 253 I is hard to imagine a busy plastic surgeon who has not struggled with treating seromas. These complications occur more frequently in obese patients and those in whom we perform large dissections, but also occasionally after an operation in which we think everything went perfectly. It is frustrating for both surgeons and patients to have to return for multiple aspirations, drain placements, and occasional operations to resolve seromas. Despite this frustration, there is actually very little that we know about the pathophysiology of seromas and the optimal way to treat them. We were excited to read the article by Janis and colleagues, who conducted a systematic review of postoperative seromas to categorize the available literature. This involved a comprehensive literature review of Level I and II studies published in English and aggregated a group of 7000 patients. The authors reviewed the various strategies for seroma treatment and prevention based on the level of evidence in the literature. They present the data both in a scientific fashion and in such a way that clinicians can easily understand their conclusions. This clearly will be a useful reference for plastic surgeons and residents to refer to when presented with these problems in practice. There are some limitations to this study, however, mostly because of the lack of well-designed studies published in the literature on this matter. Also, because of the small number of good studies, the authors were forced to aggregate and compare seromas from different areas of the body to achieve their conclusions. In our experience, there are differences between seromas developing in a breast implant case, an abdominoplasty case, and a face-lift case, making comparisons difficult. For instance, a typical implant-based breast reconstruction would include a silicone implant/expander and dermal matrix. Together with poorly vascularized mastectomy soft tissues, they form a complex that is difficult to heal, and ongoing fluid production is common and often precedes infections. Control of the fluid output in this setting is challenging and critical.1 The authors cite a study on talc used in the abdomen as being effective, yet our hope is that plastic surgeons would avoid the use of talc in a facial procedure. Also exciting are advances in surgical techniques that seem to have eliminated seroma formation. Typically, surgeons have turned to external drainage devices to prevent seromas, but these are often uncomfortable for our patients and provide a pathway for bacteria to enter the body. Eliminating the use of drains without the risk of seroma formation would be a very nice advance in plastic surgery. Many have already provided innovations in this area, and Level I studies of current breast reduction techniques show no benefit to the use of drains.2 Lipoabdominoplasty, where there is minimal undermining performed, and superficial dermolipectomy also allow surgeons to remove large amounts of skin and subcutaneous tissues with little risk of seroma formation.3 Leaving lymphatic pathways intact is a reasonable hypothesis to explore. Going forward, there are several basic science questions that would be important to answer. What exactly occurs to form a seroma? Why in some cases do they form and others they do not? What are the differences in how they form in different areas of the body, in different patients, and in different disease states? We congratulate the authors on performing a nice literature review of the topic of seromas and challenge our readers to continue to innovate so that we can prevent this annoying problem, possibly without the need for external drainage devices.
Network pharmacology combined with pharmacodynamics revealed the anti-inflammatory mechanism of Tanreqing capsule against acute-exacerbation chronic obstructive pulmonary disease

Abstract:Acute-exacerbation chronic obstructive pulmonary disease (AECOPD) is mainly associated with acute respiratory tract infection. In recent years, a growing number of studies have found that Tanreqing capsule (TRQ) has a favorable anti-inflammatory effect. In this study, we used network pharmacology and pharmacodynamics to explore the molecular mechanism and effects of TRQ in AECOPD treatment. To further understand the molecular mechanism of TRQ in AECOPD treatment, we used the network pharmacology to predict components of TRQ, TRQ-related targets, AECOPD-related targets, and pathways. In addition, we used the cigarette-smoke/lipopolysaccharide -induced AECOPD experimental model in Sprague–Dawley rats (72 rats randomly divided into six groups [ n = 12 each]: control, model, high-TRQ [TRQ-H], medium-TRQ [TRQ-M], low-TRQ, and dexamethasone [Dex]) to evaluate the therapeutic effects of TRQ and to verify the network pharmacology. We found that 59 overlapping targets based on component-and AECOPD-related targets were frequently involved in the advanced glycation end product–receptor for advanced glycation end product signaling pathway in diabetic complications, the phosphatidylinositol-3-kinase–protein kinase B signaling pathway, and the hypoxia-inducible factor 1 signaling pathway, which might play important roles in the anti-inflammatory mechanism of TRQ in AECOPD treatment. Moreover, TRQ groups exerted protective effects against AECOPD by reducing the infiltration of inflammatory cells. Meanwhile, TRQ-M and TRQ-H groups significantly downregulated or upregulated the expression of tumor necrosis factor, interleukin (IL) 6, C-reactive protein, IL10, and serum amyloid A, as key targets in network pharmacology, in the serum and bronchoalveolar lavage fluid to achieve anti-inflammatory efficacy. Our study showed that TRQ had better anti-inflammatory efficacy against AECOPD, and initially elucidated its molecular mechanism. Moreover, our study also provides a new strategy to explore effective mechanism of TRQ against AECOPD; and further studies are needed to validate the biological processes and pathways of TRQ against AECOPD.

Effect of integrative Chinese and western medicine on acute exacerbation of chronic obstructive pulmonary disease combined with respiratory failure

Abstract:Objective To investigate the clinical curative effect of integrative Chinese and western medicine on chronic obstructive pulmonary disease combined with respiratory failure(AECOPD). Methods 90 patients with acute exacerbation of chronic obstructive pulmonary disease with respiratory failure admitted to the People's Hospital of Longhua New District in Shenzhen City from January 2012 to January 2014 were randomly divided into two groups, with 45 cases in each group, the control group was treated with conventional western treatment, the observation group was treated with TCM Qingfei Huatan Decoction orally based on the treatment of control group. The main symptoms of cough, expectoration, gasp and wheeze integral and partial pressure of oxygen(PaO2), carbon dioxide partial pressure(PaCO2), white blood cell count(WBC), serum D- two dimers(D-D) changes before and after treatment in the two groups were compared, and the clinical efficiency of the two groups was compared. Results There were no significant differences in the main symptoms of cough, sputum, wheezing, wheezing integral before treatment(P 0.05), the symptom integrals mentioned above after treatment of the two groups were significantly decreased(P 0.05), the observation group was lower than that of the control group, the difference was statistically significant(P 0.01); there were no significant differences in PaO2, PaCO2, WBC and D-D before treatment(P 0.05), the index mentioned above of two groups were significantly improved after treatment(P 0.05), the observation group was better than the control group, the difference was statistically significant(P 0.01), the total effective rate of the observation group was higher than that of the control group, the difference was statistically significant(P 0.05). Conclusion Integrative Chinese and western medicine on acute exacerbation of chronic obstructive pulmonary disease combined with respiratory failure can effectively alleviate the symptoms, improve the blood gas index analysis, improve the clinical efficacy, it has good safety and clinical application value.

Effects on clinical symptoms, cure rate, and recurrence rate of patients with bacterial pneumonia treated with Tanreqing injection combined with antibiotics

Abstract:Objective: To investigate the effects on the clinical symptoms, cure rate, and recurrence rate of patients with bacterial pneumonia treated with Tanreqing injection combined with antibiotics. Methods: Sixty-eight cases of patients with bacterial pneumonia in our hospital were selected from October 2014 to November 2015. They were randomly divided into observation and control groups. The patients in the control group were treated with antibiotics and the patients in the observation group were given Tanreqing injection. Therapeutic effects, remission time, and relapse in the two groups were determined. Results: The total effective rate of the patients with bacterial pneumonia in the observation group was 100%, which was significantly higher (χ2=3.9813, P=0.0460) than that of the patients in the control group (85%). The remission time of the patients also significantly differed between the two groups. In particular, the remission time of fever, cough, and expectoration in the observation group was significantly less than that of the control group (P<0.05). The recurrence rate of the patients in the observation group was significantly lower than that of the patients in the control group (P<0.05). Conclusion: Tanreqing injection combined with antibiotics elicits good therapeutic effects on patients with bacterial pneumonia. This combination can reduce the recurrence rate and may be recommended for the treatment of this disease.

Efficacy and safety of Qingfei Huatan formula in the treatment of acute exacerbation of chronic obstructive pulmonary disease: A multi-centre, randomised, double-blind, placebo-controlled trial

Abstract:Background: Chronic obstructive pulmonary disease (COPD), a common respiratory disease, can be effectively treated by traditional Chinese medicine (TCM). Qingfei Huatan, a TCM formula, has been reported to effectively alleviate the clinical symptoms of COPD patients. However, there is a lack of multi-centre, randomised, double-blind, controlled clinical trials documenting the clinical efficacy and safety of this formula in the treatment of acute exacerbation of COPD (AECOPD). Objective: This study evaluated the efficacy and safety of Qingfei Huatan formula in the treatment of AECOPD, thereby providing high-quality clinical evidence. Design, setting, participants and interventions: A total of 276 patients with AECOPD were included in this multi-centre, randomised, double-blind, placebo-controlled trial and were randomised into treatment and control groups at a ratio of 1:1. Patients in the treatment and control groups took Qingfei Huatan granules or simulated Qingfei Huatan granules twice a day, for 14 days, in addition to Western medicine treatment. All patients were followed up for 3 months. Main outcome measures: The primary outcome was time taken to symptom stabilisation. The secondary outcomes included duration of antibiotic use, clinical symptom and sign score, TCM syndrome score, dyspnoea score, and quality of life (QOL) score. Meanwhile, the safety of the formula was assessed through routine urine and stool tests, electrocardiograms, liver and kidney function tests, and the observation of adverse events throughout the trial. Results: The time taken for effective stabilisation (P < 0.05) and obvious stabilisation (P < 0.01), and the duration of antibiotic use (P < 0.05) were significantly shorter in the treatment group than in the control group. On days 6, 9, 12 and 14 of treatment, clinical symptom and sign score decreased in both groups, particularly in the treatment group (P < 0.01). On days 9, 12 and 14 of treatment, the TCM syndrome scores of both groups were reduced (P < 0.01), with more significant reductions in the treatment group. At 3 months after the end of treatment, the treatment group continued to have lower clinical symptom and sign score and TCM syndrome score than the control group (P < 0.01). On days 6, 9, 12 and 14 of treatment, dyspnoea and QOL scores were markedly reduced in the two groups (P < 0.05 and P < 0.01, respectively), especially in the treatment group. At 3 months after the end of treatment, dyspnoea and QOL scores were lower in the treatment group than those in the control group (P < 0.01). No serious adverse events were observed in either group. Conclusion: The Qingfei Huatan formula can effectively shorten the duration of AECOPD and antibiotic use, significantly relieve clinical symptoms, and increase QOL for AECOPD patients, with a favourable safety profile. These results suggest that this formula can be used as a complementary treatment for AECOPD patients. Abstract:Objectives: Many studies have confirmed that antibacterial agents can disrupt the human gut microbiota. In China, Tanreqing injection (TRQ) is a drug with antibacterial activity that is widely used in the treatment of respiratory infections. However, its specific influence on gut microbiota remains unclear. This study aimed to investigate the effect of TRQ on the gut microbiota of healthy volunteers. Methods: Twelve healthy adults received 20 ml of TRQ intravenously daily for 7 consecutive days. At six timepoints (Pre, on D1, D3, D5, D7 and follow-up visit) fecal samples were collected and analyzed using 16S rRNA gene sequencing. Results: Eleven people were included in the analysis finally. TRQ did not significantly alter gut microbiota diversity or richness (Shannon and Simpson and Chao1 index) in healthy people during the intervention. Gut microbial structure was stable (weighted and unweighted Unifrac). Using a machine learning method based on PLS-DA analysis, the separation trend on D7 at the genus level was found, returning to baseline two days after discontinuation. The abundance of major genus fluctuated on D7 compared with that prior to treatment, including an increase of unclassified_f_Enterobacteriaceae (13.0611%), a decrease of Bifidobacterium and Escherichia-Shigella (6.887%, 10.487%). Functional prediction analysis did not reveal any significant difference. Conclusions: Our study showed short-term use of TRQ at conventional doses may not cause perturbations to the gut microbiota in healthy adults. This finding provides some useful information for the safe use of TRQ in the treatment of respiratory infections. Abstract:Osteoblast‐like cell cultures have been established from the marrow of adult rat vertebrae. We have simultaneously examined the response to dexamethasone (dex) treatment in cultures of young adult female vertebral and femoral marrow cells. Alkaline phosphatase (AP) activity was analyzed as well as the expression of mRNAs for osteocalcin (OC) and insulin‐like growth factor I (IGF‐I). The vertebral and femoral marrow cells were maintained for 7 days in primary culture with or without 10−8 M dex and then 6 days in secondary culture without dex or with 10−8 M or 10−7 M dex. All cells were examined on day 6 of secondary culture. Vertebral and femoral cultures each expressed the highest AP enzyme levels when grown with dex in primary culture (10−8 M) and secondary culture (10−7 M). Under all experimental conditions, vertebral cultures had lower AP enzyme activity than femoral cultures. When dex was omitted from secondary culture, OC gene expression was not detected in either vertebral or femoral passaged cells even if dex was present in primary culture. For dex conditions where OC was expressed, vertebral cultures had higher OC mRNA steady‐state levels than femoral cultures. IGF‐I gene expression was detected by Northern analysis in both vertebral and femoral secondary cultures. However, vertebral marrow cultures had much higher IGF‐I mRNA levels compared to femoral cultures whether or not dex was present in primary culture. These findings demonstrate that dex supports the differentiation of both vertebral and femoral adult marrow osteogenic cells into osteoblasts. Our results support the hypothesis that osteoblastic marrow cultures differ depending upon which location in the skeleton they are from and that there are skeletal site–dependent differences in the insulin‐like growth factor system components. Abstract:Objective: To compare the clinical effect on chronic obstructive pulmonary disease (COPD) of lung qi deficiency at the stable stage treated with ginger-separated moxibustion and Chinese herbal medicine atomizaiton. Methods: A total of 90 patients with COPD of lung qi deficiency at the stable stage were randomized into an observation group (45 cases, 1 case dropped off) and a control group (45 cases, 2 cases dropped off). In the observation group, the ginger-separated moxibustion was applied to Dazhui (GV 14), Feishu (BL 13) and Gaohuangshu (BL 43), 2 moxa cones at each acupoint in each treatment. Additionally, the atomized inhalation with bufei decoction and erchen decoction was combined for 20 min, once every two days. The treatment for 10 times was taken as one course and 2 courses of treatment were required for 20 min. In the control group, ambroxol hydrochloride oral solution (10 mL) and compound methoxynamine capsules (2 capsules), were taken orally. Additionally, salbutamol aerosol was combined, 1 spray each time, three times a day, consecutively for 40 days. Before treatment and in 1-year followup after treatment, percentage of the forced expiratory volume in the first second predicated value (FEV1%) and its ratio to the forced vital capacity (FVC) (FEV1/FVC%) were determined in patients of the two groups. Before and after treatment, and in 3 months, 6 months and 1 year of followup after treatment, the score of COPD assessment test (CAT) and the score of traditional Chinese medicine (TCM) symptom were observed in the two groups separately. The clinical effect was assessed in 3-month followup after treatment and the frequency of the acute attack of COPD was followed up in 1 year after treatment in patients of the two groups. Results: In 1-year followup, FEV1% and FEV1/FVC% were increased in patients of the two groups (P<0.05). After treatment and in the followup of 3 months, 6 months and 1 year after treatment, CAT scores were reduced as compared with thoes before treatment in the two groups successively (P<0.01, P<0.05). In 1-year followup after treatment, CAT score in the observation group was lower than the control group (P<0.05). After treatment and in the followup of 3 months, 6 months and 1 year after treatment, the TCM symptom scores of cough, asthma, lassitude and spontaneous sweating were all reduced as compared with those before treatment in the observation group (P<0.01), and the scores of cough and asthma were reduced as compared with those before treatment in the control group successively (P<0.01, P<0.05). After treatment and in the followup of 3 months, 6 months and 1 year after treatment, the scores of lassitude and spontaneous sweating in the observation group were lower than the control group successively (P<0.01). In the followup of 6 months and 1 year after treatment, the scores of cough in the observation group were lower than the control group (P<0.05). The total effective rates were 88.6% (39/44) in the observation group and 93.0% (40/43) in the control group, without significance difference (P>0.05). In 1-year followup, the acute recurrence rate of COPD was 20.5% (9/44) in the observation group, lower than 32.6% (14/43) in the control group (P<0.05). Conclusion: As compared with western medication, the combination of ginger-separated moxibustion and Chinese herbal medicine atomizaiton achieves the superior effects of improving the lung functions, reducing the clinical symptoms and decreasing the frequency of acute attack in patients with COPD of lung qi deficiency at the stable stage.

Abstract:Objective To study the effect of Louqin Zhisou decoction ( LQZSD) , a Chinese herbal medi-cine compound formulated by the TCM method of clearing heat and resolving phlegm, on the airway mu-cus hypersecretion of the AECOPD rat model, so as to improve the transportation of antibiotics in lung tis-sue .Methods Altogether 50 Wistar male rats were randomly divided into normal group , model group , ambroxol group , Erchen decoction ( ECD) group, Louqin Zhishou decoction( LQZSD) group ( each n=10) .The rat model of AECOPD with phlegm-heat syndrome was established by passive cigarette smok-ing,intratracheal instillation of lipopolysacchricle ( LPY) and intranose instillation of staphylococcus au-reus combined with wind-heat stimulation.After modeling, ambroxol group was intraperitoneally injected ambroxol at dose of 6 mg/kg per day, ECD group and LQZSD group were intragastrically administered ECD and LQZSD at dose of 3.33 g/kg per day respectively.The treatment duration was seven days.The secretion of acidic mucin in goblet cells was observed by using AB-PAS staining.The goblet cells in each visual field were counted and the proportion of goblet cells to epithelial cells was calculated .The contents of serum neutrophil elastase ( NE) and MUC5AC in bronchoalveolar lavage fluid ( BALF) were detected by using ELISA assay.Results Compared with model group, the secretion of acidic mucin and the number of goblet cells significantly decreased in each treatment group(P<0.05).Furthermore, serum NE and MUC5AC both decreased in LQZSD group significantly(P<0.05), while there were no statisti-cal differences in the other treatment groups.Conclusion The potential mechanism of LQZSD improving the transportation of antibiotics would be related to reduction the secretion of acidic mucin and the number of goblet cells, as well as decrease the contents of serum NE and MUC5AC in BALF;as the result, the reduction of the secretion of mucus in airway led to clearance the airway obstruction and promotion the forward diffusion of antibiotics in rat’ s lung tissue.

Abstract:What is known and objective Tanreqing injection (TRQ) is a traditional Chinese medicine injection. The goal of this study was to assess the clinical efficacy and safety of TRQ injection in combination with azithromycin or ceftriaxone, as well as azithromycin or ceftriaxone alone, in treating Streptococcus pneumoniae pneumonia (SPP). Methods The randomized controlled trial (RCT) of TRQ injection combined with antibiotics versus antibiotics alone in the treatment of SPP was retrieved from Chinese and English databases (the control group was treated with antibiotics alone, while the experimental group received TRQ injection combined with antibiotics). The retrieval period was from the database's inception through February 2022. The data was extracted using the Cochrane Collaboration Network Quality Evaluation Standards, the methodological quality of the included literature was assessed, and the outcome indicators were calculated using RevMan5.4.1 software. Results and discussion A total of 25 RCTs were collected, including 2057 patients. TRQ injection combined with antibiotics significantly improved clinical efficacy and reduced defervescence time, lung rale disappearance time, cough disappearance time, disappearance time of chest pain, and average hospitalization time when compared to control group, according to meta-analysis results (p < 0.05). What is new and conclusion In the treatment of SPP, TRQ injection combination with antibiotics can significantly improve the total effect rate when compared to standard western medicine. Due to the low quality of the randomized controlled trials included in this investigation, more high-quality, multi-center, large-sample, prospective, randomized, double-blind clinical studies are needed to confirm the aforementioned conclusions.
Exploration of biomarkers for efficacy evaluation of traditional Chinese medicine syndromes of acute exacerbation of chronic obstructive pulmonary disease based on metabolomics

Abstract:Background: Traditional Chinese medicine (TCM) is effective for the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD); however, there is no objective index for the evaluation of TCM syndrome efficacy. This study aimed to screen biomarkers related to the efficacy of TCM syndrome using metabolomics. Methods: We recruited AECOPD patients with phlegm-heat congesting lung (PH)/phlegm-damp amassing lung (PD) syndrome and treated them with Chinese herbal medicine (Qingre Huatan or Zaoshi Huatan granules) in addition to conventional medicine for 7 days. Data on clinical symptoms and sign scores, modified British Medical Research Council (mMRC), COPD assessment test (CAT), and inflammation indicators, including white blood cell (WBC) count, percentage of neutrophil count (NEU%), and C-reactive protein (CRP), were collected before and after treatment to evaluate the therapeutic effect. Serum samples were collected before and after treatment for metabolomic analysis to screen differential metabolites. Results: A total of 69 patients with AECOPD were enrolled, including 41 and 28 patients in the PH and PD groups, respectively. The clinical symptoms and sign scores, CAT, mMRC, NEU%, and CRP levels after treatment were lower than those before treatment in both groups ( p < 0.05). Serum metabolomics analysis showed that there were 13 differential metabolites in the PH group and 16 differential metabolites in the PD group before and after treatment ( p < 0.05, variable importance projection (VIP) ≥ 1.00). In the PH group, lysophosphatidylcholine (LPC) (16:0), LPC (17:1), LPC (18:3), LPC (18:2), and LPC (17:0) negatively correlated with clinical symptoms and sign scores ( p < 0.05); LPC (16:0), LPC (17:1), LPC (16:1), and LPC (17:0) negatively correlated with WBC ( p < 0.05) and NEU% ( p < 0.05); and LPC (16:0) negatively correlated with CRP levels. In the PD group, L-phenylalanine positively correlated with CRP levels ( p < 0.05), and 2-methylbutyroylcarnitine positively correlated with clinical symptoms and sign ( p < 0.05) and CAT scores ( p < 0.05). DL-carnitine positively correlated with clinical symptoms and sign scores ( p < 0.05). Conclusion: Serum metabolites may be potential indicators to objectively evaluate the efficacy of TCM syndromes; however, further large controlled trials are required to verify these findings.

Abstract:BACKGROUND:Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is the leading cause of mortality in chronic obstructive pulmonary disease (COPD). Traditional Chinese medicine (TCM) has been widely used in Asia as an adjunct treatment for AECOPD to improve the patients' symptoms. Xuebijing (XBJ) injection is one of the major herbal medicines used in TCM. Previous small-sample clinical trials have proven its efficacy and safety in the treatment of AECOPD; however, the current data on XBJ as an adjunct therapy are insufficient. The present study will be a multi-center randomized clinical trial (RCT) to evaluate the efficacy and safety of XBJ injection in AECOPD and explore its influence on the immune function based on the altered levels of T cells. METHODS:This study will be a prospective, randomized, placebo-controlled, blinded, multi-center trial. A total of 300 eligible patients will be randomly assigned to the treatment or placebo control group in a 1:1 ratio using a central randomization system. The treatment group will receive routine medication plus XBJ injection, and the control group will receive routine medication plus 0.9% NaCl injection. The patients will receive the corresponding treatment for 5 days starting within 24 h of enrollment. The primary outcome, the of rate endotracheal intubation, will be evaluated on day 28 after treatment. The secondary outcomes will include changes in immune and inflammatory indicators, respiratory support, mortality rate after 28 days, blood gas analysis, improvement in Acute physiology and chronic health evaluation (APACHE) II scores and clinical symptoms, and the length and cost of intensive care unit stay and hospitalization. The safety of the interventions will be assessed throughout the trial. DISCUSSION:This is the first and largest randomized, controlled, blinded trial that evaluates the efficacy of XBJ injection as adjuvant therapy for AECOPD. The results of this trial will provide valuable clinical evidence for recommendations on the management of the disease and identify the underlying mechanisms.
