Abstract:Twenty-six patients with stable angina pectoris, resistant to medical treatment, were randomized to either active or sham acupuncture in a single-blind design. Sham acupuncture was defined as the insertion of needles in a point within the same spinal segments as the active acupuncture, but outside the Chinese meridian system. The effect was evaluated from anginal attack rate, nitroglycerin consumption and exercise tests. Compared to patients receiving sham acupuncture the patients receiving active acupuncture increased cardiac work capacity significantly, expressed as dPRP (difference in pressure-rate-product between rest and maximum exercise) and maximal PRP during exercise (p less than 0.001). None of the other variables showed any significant difference between the two groups. Concerning exercise tolerance the median difference was 138 Wmin (95% confidence limits - 12.5 to 325 Wmin), concerning anginal attack rate the median difference was 29.5% (95% confidence limits 55% to -11%) and with regard to nitroglycerin consumption the median difference was 5% (95% confidence limits +67% to -44%). No significant effect of sham treatment was observed, no adverse effect was observed. We suggest that acupuncture may improve cardiac work capacity in patients with angina pectoris, refractory to medical treatment.

Abstract:In their pioneering paper [1] Stambaugh et al developed the Peng-Hicks concept [2] of a fusion reactor based on a solid copper centre-post Spherical Tokamak (ST). Using the promising results from the START experiment [3], they produced a vision for a path to Fusion Power. This path had two elements: the ability to produce high fusion gain from an ST, and equally importantly, the ability to demonstrate this in a small (and therefore low cost) pilot plant device. In this paper we review various attempts to pursue this vision, and try to elucidate why success has not yet been achieved. However we show that the advent of high temperature superconductors (HTS) may overcome some of the problems, and we suggest a revised version of the small, low entry cost route to Fusion Power.

[Metrological analysis for efficacy of acupuncture on angina pectoris]

Abstract:Acupuncture (Acupoints: Neiguan P6, Shenmen H7, Shaohai H3 or auriculo-points: Heart, Shenmen) was administrated once or 7 times in a week on 40 patients with stable type of angina pectoris. The effect was assessed quantitatively or semi-quantitatively according to the extent, area, frequency, duration of attack, the time of attack during exercise, and the vanishing of suffering after exercise. Just after one performance of acupuncture, 15 patients' angina pectoris were significantly alleviated (P < 0.001) both in degree and area. After 7 times of acupuncture 10 patients' angina pectoris were not only significantly alleviated both in extent and area, but also in frequency and duration of attack. 15 patients were randomized to an acupuncture, non-acupuncture or acupuncture at non-acupoints (ANA) in a single blind design. The time from the beginning of exercise to the anginal attack in active acupuncture group was longer than that in non-acupuncture or ANA group (P < 0.01), but they were similar (P > 0.05) in both non-acupuncture group and ANA group. The time from the end of exercise to the disappearance of angina pectoris in acupuncture group was shorter than that in the other two groups (P < 0.05).

Effects of acupuncture in moderate, stable angina pectoris: a controlled study

Abstract:In order to evaluate the effects of acupuncture in moderate, stable angina pectoris, 49 patients were randomized to either genuine or sham acupuncture. In sham acupuncture needles were inserted into points within the same spinal segment as in genuine acupuncture, but outside the Chinese meridian system. The effect was evaluated from exercise tests, anginal attack rate and nitroglycerin consumption. There were no significant differences between the effects of genuine and sham acupuncture either on exercise test variables or on subjective variables. In patients receiving genuine acupuncture there was a significant increase in exercise tolerance (median 9%) and in delay of onset to pain (median 10%). No significant changes were observed in patients receiving sham acupuncture. Within both groups there was a median reduction of 50% in anginal attack rate and nitroglycerin consumption, and there was no significant difference between the results achieved in the two groups. It is concluded that with the present design it was not possible to demonstrate any significant differences between the effect of genuine and sham acupuncture.

Acupuncture therapy for the treatment of stable angina pectoris: An updated meta-analysis of randomized controlled trials

Abstract:Background and purpose: Stable angina pectoris is a common symptom imperiling patients' life quality. The purpose of this meta-analysis is to assess the effectiveness of acupuncture alone or acupuncture plus medicine for the treatment of stable angina pectoris. Methods: Seven databases were searched ranging from 1959 to February 2018. Quantitative analysis of randomized controlled trials (RCTs) was performed by RevMan 5.3 software and STATA 12.0 program, and Cochrane criteria for risk-of-bias was used to assess the methodological quality of the trials. Results: A total of 12 RCTs involving 974 patients were enrolled in this study. The pooled results showed that both acupuncture group (RR: 0.35, P < 0.00001; RR: 0.49, P < 0.00001) and acupuncture plus medicine group (RR: 0.26, P < 0.00001; RR: 0.52, P = 0.03) were associated with a higher percentage of improved anginal symptoms as well as electrocardiographic (ECG) results compared to medicine group. The acupuncture plus medicine group also had a lower intake rate of nitroglycerin than medicine group (Non-event RR: 0.79, P = 0.03). However, there was no significant difference in the reduction or discontinuation of nitroglycerin intake between acupuncture group and medicine group. No acupuncture-related adverse effects were observed or reported in the included trials. Conclusion: Acupuncture therapy may improve anginal symptoms and ECG results in patients with stable angina pectoris, and can serve as an adjunctive treatment for this condition.

Abstract:AIM Nanotechnology-based fully synthetic carbohydrate vaccines are promising alternatives to classic polysaccharide/protein conjugate vaccines. We have prepared gold glyco-nanoparticles (GNP) bearing two synthetic carbohydrate antigens related to serotypes 19F and 14 of Streptococcus pneumoniae and evaluated their immunogenicity in vivo. RESULTS A tetrasaccharide fragment of serotype 14 (Tetra-14), a trisaccharide fragment of serotype 19F (Tri-19F), a T-helper peptide and d -glucose were loaded onto GNP in different ratios. Mice immunization showed that the concomitant presence of Tri-19F and Tetra-14 on the same nanoparticle critically enhanced the titers of specific IgG antibodies toward type 14 polysaccharide compared with GNP exclusively displaying Tetra-14, while no IgG antibodies against type 19F polysaccharide were elicited. CONCLUSION This work is a step forward toward synthetic nanosystems combining carbohydrate antigens and immunogenic peptides as potential carbohydrate-based vaccines.

Abstract:Abstract Background Stable angina pectoris (SAP) is a common cardiovascular disease, which brings health burden to society. Acupuncture therapy is effective in improving SAP as adjunctive therapy, nevertheless, there were controversies on the effect of acupuncture on disease-affected meridian (DAM), sham acupoints and nonaffected meridian (NAM). This study evaluated the effect of acupuncture on DAM as adjunctive therapy, and the difference in acupuncture on DAM, sham acupoints, and nonaffected meridian NAM. Methods Thirteen online databases were searched from inception to December 2, 2019. Risk of bias and quality of evidence for outcomes were respectively assessed by the Cochrane risk of bias assessment tool and the GRADE approach. RevMan 5.3 was adopted to conduct meta-analysis. Results Thirteen RCTs including 1026 participants were included in this study. Acupuncture therapy could be effective in the improvement of angina severity, ECG results, 6-MWT, SAQ results (physical limitation, angina stability, angina frequency, treatment satisfaction, disease perception), anxiety, HRV (LF, LF/HF), ET, CRP, and reduction of nitroglycerin intake. Acupuncture on DAM could be more beneficial than acupuncture on sham acupoints in aspects of angina severity, 6-MWT, SAQ results (angina stability, angina frequency, treatment satisfaction, disease perception). Compared to acupuncture in NAM, acupuncture on DAM could be more effective in SAQ results (angina stability, angina frequency, treatment satisfaction). Moreover, acupuncture on DAM did not increase the risk of dropout and adverse effect. Conclusions Acupuncture on DAM could effectively improving SAP, and it could be more effective compared with acupuncture on sham acupoints or NAM. Vasoactive intestinal peptide enhances thyroidal iodide uptake during dietary iodine deficiency.

Abstract:The presence of vasoactive intestinal peptide and neuropeptide Y in thyroid nerves and their effects on thyroid blood flow are well known. However, the effects of these two neuropeptides on the various processes involved in thyroid hormone biosynthesis and release have not been fully explored. We have now tested these two peptides for effects on an early step in thyroid hormone biosynthesis, namely iodide uptake, a process which is comprised of trapping and organification. In these experiments, we have used anesthetized adult male rats pretreated with thyroxine or fed a low iodine diet to increase thyroidal sensitivity. Vasoactive intestinal peptide significantly increased iodide uptake in rats fed an iodine deficient diet but not in those fed a normal iodine diet. This effect disappeared if animals were pretreated with propylthiouracil. Neuropeptide Y did not alter iodide uptake in rats on either the low or the high iodine diet, regardless of the presence or absence of propylthiouracil. The effect of vasoactive intestinal peptide on iodide uptake could be due to its influence on the organification of iodine, or on thyroid blood flow, or on both processes.

Abstract:In this paper, we present an on-line learning neuro-fuzzy system which was inspired by parts of the mecha- nisms in immune systems. It illustrates how an on-line learning neuro-fuzzy system can capture the basic elements of the immune system and exhibit some of its appealing properties. During the learning procedure, a neuro-fuzzy system can be incrementally constructed. We illustrate the potential of the on-line learning neuro-fuzzy system on several benchmark classification problems and function approximation problems.
[Impacts on neutrophil to lymphocyte ratio in patients of chronic stable angina pectoris treated with acupuncture at Neiguan (PC 6)]

Abstract:Objective: To observe the clinical efficacy on chronic stable angina pectoris treated with acupuncture at Neiguan (PC 6) and explore the impacts of acupuncture on peripheral blood neutrophil to lymphocyte ratio (NLR) in the patients of stable angina pectoris. Methods: Thirty patients of chronic stable angina pectoris met the inclusive criteria were randomized into an acupuncture group (15 cases) and a medication group (15 cases), and a healthy control group (15 cases of the same ages) was set up separately. In the acupuncture group, at the same time of the basic medication, acupuncture was applied to bilateral Neiguan (PC 6), once every two days, 3 days a week, totally for 4 weeks. In the medication group, the basic medication was applied, without acupuncture intervention. In the healthy control group, no any intervention was applied. The attack frequency of angina pectoris, dose of nitrogly-cerin, the evaluation of visual analogue scale (VAS), Seattle angina questionnaire (SAQ), the six-minute walking test (6MWT), the self-rating anxiety scale (SAS) and the self-rating depression scale (SDS) were observed before and after treatment in the subjects. Additionally, the peripheral blood cells were detected to analyze specifically the changes in NLR before and after treatment and observe the relationship between NLR and clinical efficacy. Results: Compared with the medication group, the attack frequency of angina pectoris was reduced within 30 days (P<0.01); the dose of nitroglycerin was reduced (P<0.01); VAS was reduced (P<0.01) and SAQ was increased (P<0.05) in the acupuncture group. The differences in 6 MWT, SAS and SDS were not significant between the two groups after treatment (all P>0.05). Additionally, compared with the medication group, in 30 days of acupuncture, NLR was reduced apparently in the acupuncture group (P<0.05). Conclusion: Acupuncture relieves the clinical symptoms of chronic stable angina pectoris, but has not apparent effects on motor ability and psychological health. Corresponding to that before treatment, the decreased NRL in the patients of acupuncture group suggests the potential good prognosis on coronary heart disease after acupuncture.

Abstract:Angina pectoris is caused by a relative reduction in oxygen in coronary blood flow and can lead to myocardial ischemia.1 Organic coronary stenosis or functional coronary artery disease (CAD) such as vasospastic angina (VSA) or microvascular angina (MVA) are suggested to be involved in causing angina pectoris.2 Drugs such as coronary vasodilators, β-blockers, renin-angiotensin-aldosterone system blockers, antiplatelet drugs and statins have generally been used as pharmacological treatments for angina pectoris.2,3 Conversely, revascularization therapy, a non-pharmacological treatment for angina pectoris, is an invasive treatment procedure that has been established and adopted for patients with organic coronary stenosis with evidence of myocardial ischemia in the clinical setting.2 Additionally, lifestyle modifications and exercises have been established as important components of the non-pharmacological treatment methods.2 Among such non-pharmacological treatments for angina pectoris, interest in acupuncture seems to be increasing recently, due to its efficacy.4–6 Therefore, we focus on acupuncture as an additional treatment procedure for angina pectoris.

Acupuncture for chronic stable angina pectoris based on the theory of Meridian-Viscera Association: study protocol for a multicenter randomized controlled trial

Abstract:Background: Acupuncture has long been used to treat chronic stable angina pectoris (CSAP), but the acupuncture prescription for CSAP varies. This trial will compare the therapeutic effects between the acupoints on the Heart Meridian and the acupoints on the Lung Meridian for treating CSAP, so as to provide a better prescription for acupuncture treatment of CSAP. Methods: This is a multicenter randomized controlled trial. A total of 148 CSAP patients will be randomly allocated into two groups through central randomization in a 1:1 ratio. This trial will include a 2-week screening period, a 4-week treatment period, and a 4-week follow-up period. The primary outcome is the frequency of angina attacks from baseline to 4 weeks after inclusion. Secondary outcomes include the frequency of angina attacks from baseline to 4 weeks after acupuncture treatment, the pain intensity of angina, total ischemia burden, heart rate variability, QT dispersion, the score of Seattle Angina Questionnaire, and the score of Short-Form of McGill Pain Questionnaire. These outcome measures will be evaluated at baseline, at the end of acupuncture treatment, and at the end of follow-up. Discussion: We hypothesize that the effectiveness of puncturing at acupoints on the Heart Meridian will not be the same as those on the Lung Meridian. The results will provide further evidence of Meridian-Viscera Association theory and references for acupoints selection in the clinical practice. Abstract:Abstract Background Chronic stable angina pectoris (CSAP) is a common cardiovascular condition that endangers a patient’s life quality and longevity. As demonstrated in several clinical trials, acupuncture is attested to be effective for CSAP. Current trials are not adequate enough to provide high-quality evidence for clinical decision making, as a result of inadequate methodology design and small sample size. Notably, stark controversy toward acupoint specificity also exists in the clinical acupuncture trials for CSAP. Therefore, we designed the present study as a randomized controlled trial primarily to investigate the effectiveness of acupuncture in addition to routine care among patients with CSAP. Meanwhile, we examined whether acupoint on the disease-affected meridian (DAM) is superior to either acupoint on the non-affected meridian (NAM) or non-acupoint (NA), to further investigate the meridian-based characteristics of acupoint specificity. Methods/Design This study was a multicenter, assessor and statistician blinded, randomized controlled trial in China. In this study, 404 participants in sum will be randomly assigned to four groups through central randomization in a 1:1:1:1 ratio. The whole study period is 20 weeks including a 4-week baseline period, a 4-week treatment period and a 12-week follow-up. Participants in the DAM group receive acupuncture stimulation at acupoints on the disease-affected meridian, and three different control groups will undergo acupuncture stimulation at the NAM, the non-acupoint and no intervention respectively, in addition to basic treatment. Participants in the acupuncture groups will receive 12 sessions of acupuncture treatment over 4 weeks, while the wait-listed (WL) group would receive free acupuncture treatment after the completion of the study. The outcome measures in this trial include the frequency of angina attack during 4 weeks as the primary outcome and eight other secondary outcomes. Discussion This trial will provide new and relatively high-quality evidence in acupuncture treatment for CSAP. Moreover, this trial may further validate the meridian-based characteristics of acupoint specificity by comparing the strength of acupoints on the disease-affected meridian versus that of the non-affected meridian, to further inspire optimization of acupuncture therapy for CSAP. Abstract:Stable angina pectoris, caused by myocardial ischemia, results in discomfort in the chest and surrounding structures and can be accompanied by cardiac dysfunction. Typically brought about by exertion, most stable angina pectoris events occur behind the sternum, potentially radiating to the anterior and ulnar sides of the left arm, to the lateral side of the right arm, to both arms, or to the neck and mandible. Usually lasting for several minutes. Stable angina pectoris often dissipates rapidly after rest or administration of sublingual nitroglycerin. [ 1–3 ] Angina pectoris is a clinical symptom of myocardial ischemia, which is caused by a temporary imbalance in myocardial oxygen supply and demand. [ 4 , 5 ] Stable angina is the most common type of angina in China, accounting for 49.4% of all types of angina. [ 6 ] This disease is an indication for acupuncture therapy and clinical reports on acupuncture treatment of stable angina pectoris are increasing in number and recognition. Acupuncture is an external therapy of Traditional Chinese Medicine. A large number of studies have shown that acupuncture treatment of stable angina has significant effect. [ 1 , 7 , 8 ] It is necessary to summarize the criteria for acupoint selection and prescription based on the complex compatibility of acupoints to provide theoretical direction for future clinical practice. Feasibility and preliminary efficacy of acupuncture for angina in an underserved diverse population

Abstract:Purpose: Stable angina is ischemic chest pain on exertion or with emotional stress. Despite guideline-directed therapy, up to 30% of patients have suboptimal pain relief. The aims of this study were to: (1) determine the feasibility and acceptability of a randomized controlled trial (RCT) of acupuncture; and (2) evaluate preliminary efficacy of acupuncture with respect to reduction of pain and increased functional status and health-related quality of life (HRQoL). Methods: Participants with stable angina for ⩾1 month received either a standardized acupuncture protocol, twice per week for 5 weeks, or an attention control protocol. Measures included the McGill Pain Questionnaire (average pain intensity (API), pain now) and the Seattle Angina Questionnaire-7 (functional status, symptoms, and HRQoL). Feasibility was defined as ⩾80% recruitment, ⩾75% retention following enrollment, and ⩾80% completion. Descriptive statistics and mixed-effects linear regression were used for analysis. Results: The sample (n = 24) had a mean age of 59 ± 12 years, was predominantly female (63%), and represented minority groups (8% White, 52% Black, 33% Hispanic, and 8% Other). Feasibility was supported by 79% retention and 89% completion rates. The recruitment rate (68%) was slightly lower than expected. Acceptability scores were 87.9% for the acupuncture group and 51.7% for the control group. Outcomes were significantly better for the acupuncture versus control groups (API, b = -2.1 (1.1), p = 0.047; functional status, b = 27.6 (7.2), p < 0.001; and HRQoL, b = 38.8 (11.9), p = 0.001). Conclusions and implications: Acupuncture was feasible and acceptable in our diverse sample. We were slightly under the recruitment target of 80%, but participants who started the study had a high likelihood of completing it. Acupuncture shows promise for stable angina, but its effectiveness needs to be confirmed by a larger, adequately powered RCT. Trial registration number: NCT02914834 (ClinicalTrials.gov).

Abstract:Background and objective: Despite that acupuncture is effective in treating anxiety, depression and chronic stable angina pectoris (CSAP), it remains unclear whether acupuncture can treat CSAP, anxiety and depression simultaneously. This systematic review and meta-analysis aimed to investigate the efficacy of acupuncture on CSAP-associated anxiety and depression. Methods: Eight electronic databases were searched to identify eligible randomized controlled trials (RCTs) or controlled clinical trials (i.e. "acupuncture alone or combined with standard care" versus " sham acupuncture alone, sham acupuncture with standard care, or standard care alone") from their inception to January 2021, which included PubMed, Embase, Web of Science, the Cochrane Library, CBM, CNKI, VIP and Wanfang Database. Data were extracted and meta analyses were performed using the RevMan 5.3. Risk of bias (ROB) 2.0 was used for methodological quality assessment. GRADEprofiler 3.2.2 was used to rate the quality of evidence. Results: Seven trials involving 893 subjects were included. Meta-analysis results showed that acupuncture combined with standard care was more effective in relieving anxiety and depression, reducing angina attack frequency, and angina pain intensity than sham acupuncture with standard care and standard care alone. In addition, the effect remained until 16 weeks after acupuncture. The safety of acupuncture for CSAP-associated anxiety and depression was also high. Nonetheless, the quality of evidence ranged from low to moderate. Conclusion: Acupuncture may be used as an adjunctive therapy to treat CSAP-associated anxiety and depression. However, more high-quality RCTs are required to confirm our findings.
