Abstract:Theoretical background: Chronic disease has become the major cause of morbidity and mortality in Belgium, similar to other European countries . Diseases of the cardiovascular system 33% and tumors 27 % cause the biggest burden. At the same time multimorbidity offers another challenge : 40 % of the older than 75 have 4 or more chronic diseases.The mean life expectancy in the European Union in 2016 is for men 84.6 and for women 89.1 years. As these figures are getting higher throughout the years, the importance of chronic disease and multimorbidity will raise in the future50 % of the Belgian general practitioners are older than 50 years. The age group between 45 and 65 have 70 % of all patient contacts. Twice as much GP aged 25-30 quit the job compared to collegues in the 50-65 group. The " Third Pedicle " Sign in Oblique Lumbar Puncture

Abstract:Recently we encountered an unusual case in which a left internal carotid artery (ICA), occluded at its origin by atherosclerosis, was reconstituted in its cervical position by small anastomotic branches of the ascending pharyngeal artery. An initial angiogram demonstrated apparent occlusion of the left ICA at its origin. However, on late films, there was opacification with antegrade flow of the ICA, 3 cm distal to its origin. On the basis of the angiogram, we concluded that the left ICA was not completely occluded, but minimally patent, a condition described by Gabrielsen et al. [1] as "pseudo-obstruction. " At surgery, however, the proximal 2 cm of the left ICA proved to be a small fibrotic band without pulsations or blood flow, indicative of a chronic occlusion. Six days later, the patient was again referred for cerebral angiography to evaluate the left external carotid artery for a possible external-ICA bypass. This study demonstrated collateral flow via small branches of the ascending pharyngeal artery to the cervical portion of the ICA 3 cm distal to its occluded origin (fig. 1). The anastomosis probably occurred through hypertrophic branches of the vasa vasorum of the ICA.

Abstract:The concept of continuous and global care is acknowledged today by all as inherent to modern medicine. A working group gathered to propose models for the coordination of supportive care for all severe illnesses in the various private and public health care centres. The supportive care are defined as: "all care and supports necessary for ill people, at the same time as specific treatments, along all severe illnesses". This definition is inspired by that of "supportive care" given in 1990 by the MASCC (Multinational Association for Supportive Care in Cancer): "The total medical, nursing and psychosocial help which the patients need besides the specific treatment". It integrates as much the field of cure with possible after-effects as that of palliative care, the definition of which is clarified (initial and terminal palliative phases). Such a coordination is justified by the pluridisciplinarity and hyperspecialisation of the professionals, by a poor communication between the teams, by the administrative difficulties encountered by the teams participating in the supportive care. The working group insists on the fact that the supportive care is not a new speciality. He proposes the creation of units. departments or pole of responsibility of supportive care with a "basic coordination" involving the activities of chronic pain, palliative care, psycho-oncology, and social care. This coordination can be extended, according to the "history" and missions of health care centres. Service done with the implementation of a "unique counter" for the patients and the teams is an important point. The structure has to comply with the terms and conditions of contract (Consultation, Unit or Centre of chronic pain, structures of palliative care, of psycho-oncology, of nutrition, of social care). A common technical organization is one of the interests. The structure has to set up strong links with the private practitioners, the networks, the home medical care (HAD) and the nurses services at home (SSIAD), when they exist, to guarantee the continuity of the supportive care under all its aspects and in order to take into account the preferences of the patients. According to Hospital 2007 propositions, the extended, flexible and general purpose Group of Sanitary Cooperation (GCS) meets the necessities inherent to the structures of supportive care within the territories of health because it can be established between one or several health care centres and the private health professionals, thus favouring the cooperation between public and private health care centres. Transforming Primary Care: Developing Health Precincts as Models for Sustainable Integrated Community-Based Healthcare

Abstract:Holistic healthcare precincts are an emerging service model to address the growing health service demands of ageing consumers and an increasing prevalence of chronic diseases. In Australia and similar countries with universal publicly funded Medicare systems, the first point of access to healthcare is provided by general medical practitioners. This case report focuses on successful components of a private, integrated, patient-centred primary care model located in a low socioeconomic population in North Brisbane, Queensland. Successful components included a focus on sustainability, general practice as an anchor tenant in the health precinct, the integration of multiple services, team-based care for shared clinical services, flexible expansion options, the use of MedTech, support for small businesses and a cluster structure. The Morayfield Health Precinct (MHP) offers appropriate, safe and individualised healthcare to residents across their life continuum. Its success was built on a foundation of pre-planning, to ensure the design/build, anchor tenant and collaborative ecosystem were sustainable in the long term. MHP planning was based on an adaptation of the WHO-IPCC framework supporting true patient-centred, integrated care. Its shared vision and collaborative care are supported by its internal governance structure, tenant selection, established and emerging referral networks and partnerships. Evidence-based and informed care is further supported by internal and external research and education partnerships.

[From policlinic services to medical service centres--the Swiss perspective]

Abstract:Industrialized countries are confronting the challenge of providing their population with accessible, high-quality, safe and efficient healthcare. They face the growing challenge of caring for an increasing number of patients suffering from chronic and complex diseases. Chronically ill patients see multiple clinicians at different healthcare sites, which increases the risks of errors and poor coordination. These experiences together with rising healthcare costs force the different healthcare systems in the industrialized countries to introduce and test new ways to offer healthcare while taking into account the entire chain of care. Disease management programmes together with institutions and structures like medical homes which are accessible and help to coordinate care make a difference for those patients. The German way to a better coordination of and a better access to healthcare is a step in the right direction, but there is a lack of data demonstrating the increase in effectiveness and efficiency. The opportunity has been missed to conduct pilot studies in order to identify measurable, reliable, and valid outcome indicators and thus create greater transparency. The question is not "whether" but "how" new ways of offering care can be introduced to increase the performance of the whole system, to decrease coordination problems, and to stabilise healthcare spending.
[Evolution and new perspectives of health care financing in developing countries]

Abstract:Over the last twenty five years, the perspective of health care financing has dramatically changed in developing countries. In this context, it is worth reviewing the literature and the experiences in order to understand the major shifts on this topic. During the sixties, health care policies focused on fighting major epidemics. Programs were dedicated to reduce the threat to population health. Financing related to the mobilization of resources for these programs and most of them were not managed within national administrations. The success of these policies was not sustainable. After Alma Ata, primary health care became a priority but it took some years before the management of the health care district was introduced as a major topic. In the eighties, with the district policy and the Bamako Initiative, the economic approach became a major part of all health care policies. At that time, most of health care financing was related to cost recovery strategies. All the attention was then drawn on how it worked: Fee policies, distribution of revenues, efficient use of resources and so on. In the second half of the nineties, cost recovery was relegated to the back scene, health care financing policy then becoming a major front scene matter. Two major reasons may explain this change in perspective: HIV which causes a major burden on the whole health system, and fighting poverty in relation with debts reduction. In most developing countries, with high HIV prevalence, access to care is no longer possible within the framework of the ongoing heath care financing scheme. Health plays a major role in poverty reduction strategies but health care officials must take into account every aspect of public financing. New facts also have to be taken into account: Decentralization/autonomy policies, the growing role of third party payment and the rising number of qualified health care professionals. All these facts, along with a broader emphasis given to the market, introduce a need for a better management of resources through financing mechanisms. Some major reports from WHO and the World Bank are the landmarks of the evolution on how to approach health care financing: The 1993 World Bank report on investing in health, the 2000 WHO report on health in the world and the WHO report on macroeconomics and health. In this early millenium, there is a general agreement on some major aspects of health care financing such as: Lack of resources for financing health care; cost recovery as a part of any sustainable health care system; health as a public good needing some extended subsidies; protecting people from the burden of disease as a part of financing schemes; equity in relation with the public private mix at the center of many debates; financing as a key mechanism for the regulation of the whole health care system and not only as a resource mobilization; HIV in bringing up new problems clearly shows how all these matters are related. Health care financing is at the heart of ongoing questions on health care reforms. Although developing countries have low insurance coverage and weak modern medical care, they share the same questions as developed countries: How to promote technical and allocative efficiency? What place for incentives? What role for the public sector? How can market and contracting bring results? What progress through stewardship and better governance?

Abstract:BackgroundNon-government, not-for-profit community health centres (CHCs) play a crucial role within healthcare systems in fostering equity, acting both as direct providers of services and as sentinels of health and social inequity. In a study of an intervention to promote equity-oriented health care, we enlisted four diverse primary healthcare clinics with mandates to serve highly marginalized populations. All of these CHCs operate as not-for-profit, non-government organizations (NGOs), and have a marginal relationship financially and socially to other parts of the system. The purpose of this paper is to provide an analysis of the factors that shape how CHCs are able to carry out an equity mandate and, from this, to identify what is required at the level of policy to enhance capacity to provide equity-oriented health care.MethodsWe systematically examined the clinics’ policy and funding contexts, and identified influences on the clinics’ capacities to promote equity-oriented health care.ResultsWe identified three key mechanisms of influence, each playing out against the backdrop of a contested and marginal position of CHCs within the health care system: a) accountability and performance frameworks; b) patterns of funding and allocation of resources, and c) pathways for emergent priorities. We examine these mechanisms, considering how each influenced the pursuit of equity, and propose policy directions to optimize the primary health care sectors’ capacity to support equity-oriented health care.ConclusionsAlthough this analysis is based on a study within a high-income country, we argue that because the dynamics between community health centres and broader healthcare systems are similar across national boundaries, the implications have applicability to low and middle-income countries.

Abstract:Objectives: To better understand the experience of patients attending community-based primary healthcare practices (CBPHCPs) aimed at improving equity and access to primary care for underserved patients, which have been implemented locally in several countries, including Canada. There are currently little data on how, or to what extent, they mitigate patients' experience of social inequalities in care and improve their access to health. This study explored the impacts of the sociospatial characteristics of these practices on patients' care experience. Design and methods: Qualitative, multisite, focused ethnographic study based on in-situ observations and interviews, incorporating inductive and deductive analysis, and using the concept of sense of place. Setting: Three CBPHCPs located in deprived urban areas in two provinces of Canada. Participants: 28 structurally marginalised persons (17 women) attending the clinics, ranging in age from 18 to 79 years, and 16 managers, clinicians and practitioners working in these clinics. Results: Data underscored the importance of clinic proximity and accessibility in facilitating patients' navigation of the health system. Patients appreciated the clinics' positive sociospatial characteristics. Non-judgmental environments and informal spaces fostered patients' empowerment and social interaction among themselves and with peer navigators and healthcare professionals. The experience of supportive continuity of care had a positive impact on patients' sense of well-being and, for many, a positive ripple effect and long-term impact on their social integration. Conclusion: These results have important implications for policy given the current context, in which governments are challenged to support primary healthcare that addresses the social determinants of health to achieve greater equity. We conclude that scaling up contextually tailored care and deploying humanistic innovative organisational practices into mainstream care will help narrow the equity gap and reduce current prevalent social inequalities in the health system.

Disturbances of Sperm Maturation and Minipuberty: Is There a Connection?

Abstract:Male reproductive function in the general population raises an increased attention due to reports indicating declining sperm counts, increased occurrence of testicular cancer, cryptorchidism, and hypospadias. It is also hypothesized that prolonged exposure of the developing male, during both fetal and postnatal life, to exogenous estrogens could reduce Sertoli cell number and thus reduce sperm output (and sperm counts) in adult life. Fact is that infertility, which is defined as the inability to conceive after 1 year of unprotected intercourse, has a global prevalence of 9%. A male contributory factor is involved in approximately half of these cases, but most of the causes of reduced semen quality and other disturbances of male reproductive function are unknown. In the most affected men (azoospermic men) 15–20% had a prior history of cryptorchidism. The association between the cryptorchidism and infertility is one of the most studied potential causes of infertility. There are numerous studies that accentuate the importance of minipuberty for future fertility. Is it possible that a normal minipuberty ensures normal fertility despite malpositioned testes? And to move away from cryptorchidism, could impaired minipuberty be responsible for fertility problems in men who were born with both testes in their scrotal sacs?
Historical Roots of Hospital Centrism in China (1835-1949): A Path Dependence Analysis.

Abstract:Stronger primary care has been associated with important contributions to health system performance, yet countries struggle to resource it adequately, given competing demands from hospitals. Although historically China has originated influential models of primary health care, it has an enduring problem with hospital dominance in health service delivery. This paper is a historical analysis of the co-evolution of hospitals and primary care providers in China from 1835 (the year when the first hospital was built in mainland China) to 1949 (the year when the People's Republic of China was founded), which aims to shed light on approaches to primary care strengthening. We develop and use a path dependence analytic framework, specifying the critical juncture, conjuncture and post-juncture development of the institutions shaping the balance between hospitals and primary care providers in China. We find that China had historically formed the hospital-centric model involving four sets of regenerating and mutually reinforcing institutions: 1) financial resources were being disproportionally distributed to hospitals; 2) high-quality medical professionals were largely concentrated in hospitals; 3) large outpatient departments were incorporated in hospitals, which functioned as a first point of care for many patients; 4) hospitals answered primarily to the demand of the more privileged social group. The early institutionalization of a hospital-centric model of Western medicine in China from 1835 became resistant to change, and efforts to strengthen primary care eventually took a divergent low-cost and de-professionalized developmental path towards 1949. As China still has a hospital-centric health system seeded in the nineteenth century, these findings can inform the framing of contemporary options for primary care strengthening. Without addressing these deep regenerating causes using a whole-system approach, China is unlikely to achieve a primary care orientation for health system development.
Sustainability of specialized healthcare in upper-middle-income economies: Innovations despite constraints

Abstract:Countries such as Jamaica have had to be largely self-reliant in funding their health-care systems. The success of an early and sustained focus on primary healthcare, coupled with a change in the disease profile from infectious concerns to chronic non-communicable diseases associated with aging, has resulted in a marked rise in healthcare costs. Rapid technological advances in the diagnosis and treatment of many specialized conditions have further increased the cost of care. Funding these health-care costs sustainably is a challenge for Jamaica and other similarly developed economies due to competing demands for scarce funds. In addition to government health-care allocations, sources of sustainable funding from local public-private partnerships, through mechanisms such as philanthropy, may help address, at least in part, this chronic problem. This review examines strategies to create a framework for such funding, as well as proposes specific practical steps for their achievement. The author’s motivation to participate in the process of change stems from the stubbornly persistent inequalities of healthcare that people and health-care workers in Jamaica – and similar lower- and middle-income countries – routinely cope with. These inequalities must be addressed to truly achieve greater health egalitarianism, social stability, and ultimately, the developed country status they aspire to. The need to think innovatively to achieve this goal is self-evident, given the current inability of governments to achieve this goal.

Abstract: Recent studies on the α globulin fraction from plasma of normal individuals have shown that it possesses immunosuppressive properties and interferes with lymphocyte functions in vitro. Cooperband et al. (1, 2) have shown that human α globulin inhibits lymphocyte transformation in vitro following phytohemagglutinin (PHA) and specific antigenic stimulation. Since α globulin fraction can also suppress antibody production against sheep red blood cells (SRBC) in vivo (3–6), experiments were conducted to assess its biologic activity in an in vitro system by the hemolytic plaque assay and the rosette formation technique. Materials and Methods. Alpha globulin was isolated from normal human plasma by DEAE cellulose chromatography according to the procedure described by Mannick and Schmid (7). Immunoelectrophoresis and acrylamide gel electrophoresis showed this material to be primarily α2 globulin. Male local adult white rats of a strain developed at the Hebrew University were used in the hemolytic plaque formation and the rosette formation experiments.
India’s health and wellness centres: realizing universal health coverage through comprehensive primary health care

Abstract:In common with other countries in the World Health Organization South-East Asia Region, disease patterns in India have rapidly transitioned towards an increased burden of noncommunicable diseases. This epidemiological transition has been a major driver impelling a radical rethink of the structure of health care, especially with respect to the role, quality and capacity of primary health care. In addition to the Pradhan Mantri Jan Arogya Yojana insurance scheme, covering 40% of the poorest and most vulnerable individuals in the country for secondary and tertiary care, Ayushman Bharat is based on an ambitious programme of transforming India's 150 000 public peripheral health centres into health and wellness centres (HWCs) delivering universal, free comprehensive primary health care by the end of 2022. This transformation to facilities delivering high-quality, efficient, equitable and comprehensive care will involve paradigm shifts, not least in human resources to include a new cadre of mid-level health providers. The design of HWCs and the delivery of services build on the experiences and lessons learnt from the National Health Mission, India's flagship programme for strengthening health systems. Expanding the scope of these components to address the expanded service delivery package will require reorganization of work processes, including addressing the continuum of care across facility levels; moving from episodic pregnancy and delivery, newborn and immunization services to chronic care services; instituting screening and early treatment programmes; ensuring high-quality clinical services; and using information and communications technology for better reporting, focusing on health promotion and addressing health literacy in communities. Although there are major challenges ahead to meet these ambitious goals, it is important to capitalize on the current high level of political commitment accorded to comprehensive primary health care.

Abstract:Providing efficient decentralised local public services such as healthcare in hard to reach and remote areas has been the recent concern of public finance experts. Thus public finance theory, the public choice, has been devoted to explain the link between decentralised local public service delivery and efficiency. The theory argues that efficient delivery of decentralised local public service is achieved through the proximity advantage of local management boards, which enables them (that is the local management boards) to provide services tailored to the needs of their respective local communities. Using the public choice's decentralised local service provision, this study adopted a qualitative design, and accidentally selected and interviewed 40 health service users (HUs) and 10 Health professions (HP) in the rural areas of Lesotho. The results show improvement and efficiency in healthcare service accessibility, utilisation and affordability in the study communities. Again, it emerged that decentralised healthcare delivery helped healthcare service providers to gain an in-depth knowledge of the healthcare needs of the local people, thereby enabling them to adjust services to the healthcare needs of the community through innovative healthcare delivery strategies, like mobile healthcare delivery service and the mobilisation of trained assistant nurses deployed to serve in remote and hard-to-reach communities. Results further show that these innovative healthcare strategies meet the health needs and preferences of the local people. The study recommends local and central governments to prioritize and scale up decentralised healthcare delivery across the country, particularly in vulnerable and poor communities, in addition to the provision of state-of-the-art healthcare facilities and intensified human resource training for the healthcare sector.

Abstract:Starting in 2018, the French Hospitals Federation (FHF) has developped a clinical integration model based on "Responsabilité Populationnelle" (accountabilty towards a population). The model rellies on localy developped "action plans" covering targeted populations, from "at risk" people to severly ill patients. The model is developped and tested in five "Pioneer Territories", covering 1.3M habitants, and built around Territorial Hospital Groups. The Five Territories ""share"" two common target populations : people at risk or suffering from Type 2 diabetes and Heart Failure. These populations represent around 60 000 ill persons, and 300 000 ""at risk"" individuals, in the Five regions. They share a common methodology, developped by FHF's teams and leading medical experts in the fields of diabetology, cardiology and public health. The model relies on extensive use of healthcare data, in particular the ability to ""stratify"" target populations into clinical profiles. To each profiles are attached clinical guidelines that must be executed for every patient, according to its profile. It is up to local actors to create their "actions plans", leveraging local assets to create local pathways, and local ressources to adress local needs. Large scale deployment started in January 2022. In September 2022, more than 700 healthcare professionals, as well as 60 ""patient/partners"" were actively involved in the program. More than 450 outreach/prevention actions had been performed, and more than 5 300 at risk individuals had been screend for T2D or HF. More than 1 700 patients were included in ""clinical programs"", according to their clinical profiles. We are already measuring impacts in terms of hospital use for these populations : reduced ER utilization, reduced lenght of stay, increased planned admissions. Key Takeaways: The model is inspired by other Integrated health systems. However, FHF having no regulatory power, the model is primarely built around clinical practices and intrinsic motivations. We rely on the ability of local healthcare professionals and actors to develop actions plans that will improve coordination between them and the timely use of healthcare ressources for the benefit of the populations. So far, the model is producing positive results in five very different regions, providing valuable insight as to the scalabilty of the model in other regions, and/or for other target populations. The Five regions have shown tremedous ability to mobilize their local ecosystems, ranging from community groceries to local authorities, to patient organizations and other community assets. Data analysis and utilization, in particular the stratification methods developped for the program, are based on DRG's, making them highly transferable to other HC organizations that use ICD-10 coding. These tools were well received by local HC professionals and are used on a daily basis in the Five Territories. Among the key challenges at this stage, the question of a unified Information System is the most pressing. At this stage, there is still no single EHR in France, making the connection between hospitals and ambulatory providers cumbersome. Morevoer, if we want to model to endure and to be generalyzed in France, regulatory intervention by National authorities will be necessary.

Abstract:Although France's healthcare system is rich in multi-faceted skills, both in the community and in hospitals, and implemented by a range of medical, paramedical and medico-social professionals, it is no longer able to meet the health needs of all. Today, these social inequalities in health require us to rethink our policies and redesign existing systems, in order to develop new alternatives that will make quality care and health maintenance accessible to all.

Abstract:Context: There is growing international interest in teaching clinical skills in a variety of contexts, one of which is Clinical Skills Centres. The drivers for change making Skills Centres an important adjunct to ward and ambulatory teaching come both from within and outside medical education. Educationally, self-directed learning is becoming the accepted norm, encouraging students to seek and maximize learning opportunities. There are global changes in health care practice, increased consumerism and increasing student numbers. In some countries, professional recommendations influence what is taught. Increasingly, core skills curricula and outcome objectives are being defined. This explicit definition encourages assessment of the core skills. In turn, all students require equal opportunities to learn how to practise the skills safely and competently. The moves towards interprofessional education make joint learning in a"neutral" setting, like a Clinical Skills Centre, appear particularly attractive. Objective: To discuss the potential role of Clinical Skills Centres in skills training in developing countries and to consider alternative options. Discussion: Many developing countries seek to establish Clinical Skills Centres to ensure effective and reliable skills teaching. However, the model may not be appropriate,because fully equipped Clinical Skills Centres are expensive to set up, staff; and run. They are not the only way to achieve high quality clinical teaching. Suggested options are based on the philosophy and teaching methods successfully developed in Clinical Skills Centres that may fulfil the local needs to achieve low cost and high quality clinical teaching which is reflective of the local health needs and cultural expectations.
