Abstract:Objectives. This study aimed to assess human papilloma virus (HPV) 16 18/45 typing test results combined with cytology for cervical exfoliated cells from women who screened positive in an HPV E6/E7 mRNA assay (Aptima HPV, AHPV). Methods. In total, 3257 AHPV-positive women aged 25-65 years were underwent AHPV 16 18/45 Genotype assay (AHPV-GT) testing with cytology. Women were referred for colposcopy and further biopsy if indicated. Different triaging strategies were compared. Results. Overall, 624 women (19.2%) tested AHPV-GT positive. When identifying CIN2+, compared with cytology, AHPV-GT achieved a similar AUC (0.72 vs. 0.69, P = 0.158) but a higher specificity (85.1% vs. 79.3%, P < 0.001) and positive predictive value (PPV) (29.6% vs. 23.2%, P < 0.001). When identifying CIN2+, compared with cytology, the cotesting strategy (cytology combined with AHPV-GT) increased the AUC from 0.69 to 0.76 (P< 0.001), with a higher sensitivity (84.6% vs. 59.5%, P< 0.001), higher NPV (97.6% vs. 94.9%, P< 0.001) and similar PPV (21.6% vs. 23.2%, P = 0.054). When identifying CIN2+, the results of combination strategy (AHPV-GT genotyping plus reflex cytology in women positive for the 11 other hrHPV genotypes) were consistent with those of the cotesting strategy. Similar results were achieved when identifying CIN3 +. Conclusions. The AHPV-GT test may be a promising triage approach with high specificity in women receiving AHPV-positive primary screening tests. Although a combination strategy and cotesting strategy detected the same CIN2+ and CIN3+ cases, the former required significantly fewer screening tests. (C) 2018 Published by Elsevier Inc.

Abstract:Objective Our aim was to evaluate the performance of different follow-up strategies after treatment for cervical intraepithelial neoplasia (CIN) 2 or 3, including human papillomavirus (HPV) detection, cytology, or colposcopy, as well as their combinations. Additionally, we compared the influence of the persistence of HPV 16/18 versus that of other high-risk HPV genotypes (HR-HPV) in the recurrence risk. Methods Retrospective register-based study, including women who had an excision of the transformation zone for CIN2 or CIN3 at our institution, between January 2011 and December 2022. The outcome assessed was histopathological recurrence/persistence of CIN2 or worse. Results Of the 721 women included, 6.8% (49/721) had recurrence/persistence. The sensitivity, specificity, and positive and negative predictive values of the HPV test were 97.4%, 80%, 22.3%, and 99.8%, respectively, whereas for cotesting (HR-HPV and cytology), 86.8%, 90.1%, 34.4%, and 99.1%, respectively. The referral rates for colposcopy were 24.3% and 14.2%, respectively. The sensitivity of colposcopy was low (40.0%). Women who were initially positive for non-16/18 genotypes at baseline who became HPV16/18 positive during follow-up, had a statistically significant increased risk of CIN2 or worse, compared with those who tested positive only for other HR-HPV genotypes during both stages (hazard ratio = 4.98; 95% CI = 1.66–14.91). Conclusions Human papillomavirus testing is the best strategy for follow-up after treatment of cervical HSIL. The addition of cytology triage decreases by more than 40% the referrals for colposcopy, without significantly missing cases of recurrence/persistence. Human papillomavirus 16/18 in the follow-up, regardless of being previously positive, is associated with higher risk of recurrence/persistence of HSIL.

Abstract:Objective: To assess the clinical performance of the human papillomavirus (HPV) 16 18/45 genotype assay (AHPV-GT) combined with age stratification in triaging women with histologic low-grade squamous intraepithelial lesion/cervical intraepithelial neoplasia grade 1 (LSIL [CIN1]) preceded by HPV E6/E7 mRNA assay (Aptima HPV, AHPV) positive/normal or mildly abnormal cytology. Design: Longitudinal study. Setting: Gynaecological clinic in Huzhou Maternity & Child Health Care Hospital, China. Population: Women aged ≥21 years with histologic LSIL (CIN1) preceded by AHPV-positive/normal or mildly abnormal cytology. Methods: Women underwent AHPV-GT testing at baseline and were followed at 6-month intervals for up to 2 years. At each follow-up, women with abnormal cytology or AHPV positivity were referred for colposcopy and then biopsy if indicated. Main outcome measures: The 2-year cumulative incidence rates (CIRs) of CIN3+ stratified by baseline AHPV-GT genotyping. Results: Of 349 eligible women, 25 women (7.2%) progressed, 301 (86.2%) regressed, and 23 (6.6%) persisted during the follow-up. The 2-year CIRs of CIN3+ in AHPV-GT-positive women were both significantly higher than those in AHPV-GT-negative women overall (8.6% vs. 1.7%, P = 0.014) and in the ≥25-year-old group (10.9% vs. 1.5%, P = 0.002) but slightly higher in the 21–24-year-old group ( P > 0.05). Conclusions: AHPV-GT testing with age stratification is effective for triaging women with histologic LSIL (CIN1) preceded by AHPV-positive/normal or mildly abnormal cytology. Immediate treatment is a rational recommendation for women ≥ 25 years old with histologic LSIL (CIN1) preceded by AHPV-GT positivity when good surveillance is not assured.

Abstract:Aspects of auditory brain stem responses (ABR) and pure-tone behavioral audiograms were compared in patients with cochlear hearing loss. Click-evoked ABR thresholds appeared to be related most closely to the audiometric thresholds at 2000 and 4000 Hz, with relatively poor agreement at either 1000 or 8000 Hz. These results were related to the amplitude spectrum of the eliciting stimulus. The slope of the wave V latency-intensity function appeared to be related to the configuration of the hearing loss. Patients with high-frequency sensorineural losses had steeper slopes than normal subjects, whereas patients with flat sensonneural losses had shallower slopes. These results were related to the principle that response latency is determined by the cochlear region that predominates the ABR for different stimulus intensities.

Abstract:Children immigrating into Sweden from Finland and Southern Europe (Greece, Yugoslavia and Turkey) were subjected to a longitudinal study on caries activity and gingival condition. For each immigrant child a Swedish "twin" of the same sex and age was used as a control. Out of 124 pairs originally examined, 75 were reexamined after a 2 1/2-year stay in Sweden. The study showed that on arrival in Sweden the Finnish children had more caries and a higher Gingival Index (GI) than the controls and that they acquired more carious lesions during their stay in this country. Their GI, however, did not develop less favorably than that of the Swedish controls. The South-European children did not seem to have more carious lesions than the Swedish controls on arriving, nor did they seem to acquire more carious lesions during the period of observation. Their GI was higher at the first examination but the change in this index ran parallel with that of the controls. It was concluded that the children immigrating from Finland should be considered a risk group with regard to oral health.

Abstract:Introduction: To identify the absolute risk of specific HPV genotype for cervical intraepithelial neoplasia grade 2/3 or worse (CIN2/3+) and form the risk-based management strategy by extended HPV genotyping in HPV positive population.Methods: HPV genotyping was performed based on 3-years cervical cancer screening cohort including 10314 women from 2 clinical trials. The study endpoints were histological CIN2+/3+. The prevalence of specific HPV genotype was calculated by minimum estimate, any type estimate and hierarchical attribution estimate. The absolute CIN2+/3+ risks of specific HPV genotype, cytology and combined test were estimated and risk-based management strategy was established according to ASCCP guideline. The performance of conventional and risk-based management strategies for non-16/18 HPVs to detect CIN2+/3+ were further evaluated.Findings: Eligible data were available for 8370 women with median age of 48 (IQR 42-53). At baseline, HPV positive women were 1062, including 424 multiple infections and 639 single infections. CIN2+/3+ cases were 113/74, 23/8, 20/7 and 52/31 at baseline, the 1st, 2nd and 3rd year of visit. Women with multiple HPV infections at baseline were prone to persistent infection than single infection (p<0.0001). HPV16 and 52 were top two ranking among baseline and 3-years cumulative CIN2+/3+ cases. Based on the absolute risk of specific HPV genotype combined with cytology for CIN2+/3+, all non-16/18 HPVs were divided into 4 risk stratified groups. Compared to conventional strategy, the risk-based strategies had higher specificity (p=0.0000) and PPV (p=0.0322) to detect CIN3+, and needed fewer colposcopies for each CIN3+ case.Interpretation: We proposed a new extended HPV genotyping protocol based on the absolute risk of specific HPV genotype, and corresponding management thresholds were suggested for achieving the precise risk-based management of HPV positive population.Funding Information: Supported by the key research and development program of Zhejiang province, China (Grant No: 2020C03025).Declaration of Interests: All authors have completed the ICMJE uniform disclosure form. Abstract:Objective To investigate the clinical significance of HPV genotyping detection in patients with negative cervi-cal cytology and positive human papilloma virus (HPV). Methods A total of 200 women with negative cervical cytology and positive HPV received the HPV genotyping detection and were referred to colposcopy and histopathological examination. The cor-relation between different HPV subtype infection and cervical intraepithelial neoplasia(CINⅡ) + was analyzed. The women whose pathological diagnosis were CINⅠand cervicitis were followed up for one year by the method of TCT and HPV genotyping detec-tion. Results Of 200 women, 37 patients were CINⅡ+(18. 6%). The risk of patients with HPV16 infection to develop CINⅡ+was 2. 608 times higher than that of patients without HPV16 infection (OR=2. 608, P<0. 05). Of 151 patients with CINⅠand cervicitis who finished the follow-up, the prevalence of cervical intraepithelial neoplasia in patient with homotype HPV infection was higher than that in patients with heterotype HPV infection (P<0. 01). And the risk of higher pathological progression in pa-tients with homotype infection was 14. 89 times higher than that in patients with heterotype HPV infection. Conclusions HPV genotyping detection has important clinical significance for cervical cytology negative and HPV positive women. It is recommend that cervical cytology negative and HPV16 positive women should be referred to colposcopy immediately. HPV genotyping detec-tion can be used for the follow-up of cervical cytology negative and HPV positive women, and patients with positive homotype HPV infection should be referred to colposcopy, especially those with HPV16 infection.
The Risk of CIN II or Greater in a One-Year Follow-Up Period in Patients with ASC-H Interpreted with Cytology

Abstract:Objective: To explore the risk of CIN II or greater in patients with cytologically interpreted atypical squamous cells, cannot exclude high grade squamous intraepithelial lesion (ASC-H) in a one-year follow-up period.Study design: Patients with ASC-H accepted high-risk human papillomavirus (HPV) testing (Hybrid Capture II), colposcopy-directed biopsy, and endocervical curettage at baseline. Patients with CIN 1 or less were re-examined with cytology at 4-6-month intervals for up to 12 months only if the cytologic result was ASC-US or worse. Correlation between HPV infection at baseline and the follow-up results was analyzed using Chi-square statistical method and Fisher's exact test.Results: 34.0% (50/147) patients were diagnosed CIN II or greater at baseline. Four patients were diagnosed CIN II or greater at repeat colposcopy among the 65.0% (63/97) of patients who were diagnosed CIN I or less at baseline and had completed follow-up. The HPV positive rate was 61.9% (91/144) at baseline. The correlation coefficient of HPV negativity with CIN I or less was 100% (32/32).Conclusions: The risk of CIN II or greater among patients with ASC-H was 40% and all patients (100%) with baseline negative HPV showed CIN I or less by cytology during the one-year follow-up period. Value of Liquid-Based Cytology Test Combined with HR-HPV Testing in CIN III after LLETZ During Follow-Up

Abstract:Objective:This study aimed to evaluate the value of liquid-based cytology test(LCT)combined with high-risk human papillomavirus(HR-HPV)testing in cervical intraepithelial neoplasia gradeIII(CINIII)after large-loop excision of the transformation zone(LLETZ)during follow-up. Methods:Clinical analysis was used to investigate 567 patients diagnosed with CINIII and treated with LLETZ from May 2003 to May 2009 at Peking University Shenzhen Hospital. The patients were enrolled in a follow-up program set at 3 months, 6 months, and every 6 to 12 months. LCT and HR-HPV testing were used as monitoring indicators to evaluate HR-HPV testing after LLETZ during follow-up. Results:1)The residual lesion predicting capacity of postoperative LCT obtained the following: sensitivity, 60%: specificity, 88.15%: positive predictive value, 8.30%; and negative predictive value, 99.19%. The residual lesion predicting capacity of HPV testing obtained the following: sensitivity, 100%: specificity, 85.64%: positive predictive value, 11.11%; and negative predictive value, 100%. The recurrent lesion predicting capacity of postoperative LCT obtained the following: sensitivity, 63.64%; specificity, 92.27%; positive predictive value, 14.00%; and negative predictive value, 99.23%. The recurrent lesion predicting capacity of HPV testing obtained the following: sensitivity, 72.73%; specificity 85.97%; positive predictive value 9.30%; and negative predictive value, 99.38%. 2)The recurrent lesion rate was higher in postoperative persistent HPV infection patients than in non-postoperative persistent HPV infection patients , Ther were statistically significant differences between the persistent negative and non-testing preoperative group(P<0.05). The recurrence rate was higher in persistent HPV-positive and HPV-re-infected patients than in the negative conversion group, whereas no recurrent lesion was found in persistent HPV-negative patients. Conclusion:Results showed the importance of predicting residual and recurrent lesions by LCT and HR-HPV testing, which were also confirmed to be effective methods in follow up. Postoperative persistent HPV-positive status is associated with recurrence and requires consistent follow-up.
The residual rate of HPV and the recurrence rate of CIN after LEEP with negative margins: A meta-analysis

Abstract:Background HPV is detected in up to 47% of CIN and up to 70% of cervical cancers. It can cause intraepithelial neoplasia, which can eventually progress to invasive carcinoma. Almost all cervical cancers are caused by HPV. Therefore, it is especially important to treat high-risk HPV. For patients who have undergone LEEP surgery, this procedure can effectively treat CIN. However, it has not been studied in a meta-analysis whether HPV remains after the surgery and whether residual HPV increases the recurrence risk of CIN. To address this gap, our study collected all relevant literature to investigate the residual rate of HPV and its potential influence on the recurrence rate of CIN. We aim to provide valuable recommendations for clinicians and patients. Methods The Cochrane Library, EMBASE, and PubMed databases were searched from the establishment of the database until October 2023. Stata 12.0 software was used for the statistical analysis. Results Twelve studies were included, with a total sample size of 1192 cases. The meta-analysis found that the recurrence rate of CIN was quite low [95% CI = 0.5% (0.001, 0.012); P = 0.006] when the margins were negative after LEEP and there was no residual HPV. When HPV was present, the recurrence rate of CIN was significantly higher [95% CI = 18% (0.089, 0.291), P = 0.000], even if the margins were negative. The recurrence rate of CIN with residual HPV was 3.6 times higher than the recurrence rate of CIN without residual HPV. The residual rate of HPV after LEEP with negative margins was 22.7% [95% CI (0.167, 0.294), P = 0.000], which remained relatively high. Conclusion This meta-analysis found that the recurrence rate of CIN without residual HPV and with negative margins after LEEP was quite low, at 0.5%. However, when HPV was residual, the recurrence rate of CIN significantly increased to 18%, even if the margins were negative. The residual rate of HPV was 22.7%, even when the margins were negative after LEEP.

Abstract:Human papillomavirus (HPV) is the well‐established etiologic factor for cervical neoplasia. Cervical conization constitutes an effective treatment for high‐grade cervical intraepithelial neoplasia (HG‐CIN). We conducted an observational study for long‐term outcomes and HPV genotype changes after conization for HG‐CIN. Between 2008 and 2014, patients with newly diagnosed HG‐CIN before conization (Surveillance New [SN] group) and those who had undergone conization without hysterectomy (Surveillance Previous [SP] group) were enrolled. HPV testing and Pap smear were performed periodically for the SN and SP (collectively S) groups. All other patients receiving conization for HG‐CIN during the study period were identified from our hospital database. Those eligible but not enrolled into this study were assigned to the non‐Surveillance (non‐S) group. For the S group (n = 493), the median follow‐up period was 74.3 months. Eighty‐four cases had recurrent CIN grade 2 or worse (CIN2+) (5‐year cumulative rate: 14.8%), of which 6 had invasive cancer. Among the 84 patients, 65 (77.4%) exhibited type‐specific persistence in the paired HPV results, whereas only 7 (8.3%) harbored new HPV types that belonged to the 9‐valent vaccine types. Among the 7397 non‐S patients, 789 demonstrated recurrent CIN2+, of which 57 had invasive cancer. The stages distribution of those progressed to invasive cancer in the non‐S group were more advanced than the S group (P = 0.033). Active surveillance might reduce the severity of those progressed to cancer. Because a majority of the patients with recurrent CIN2+ had persistent type‐specific HPV infections, effective therapeutic vaccines are an unmet medical need.

Abstract:Objective To investigate the cervical intraepithelial neoplasia grade I ( CINⅠ) points in human papilloma virus ( HPV) infection situation and its outcome in this hospital.Methods A retrospective study was performed in patients with CINⅠform 2010 to 2012 in this hospital,HPV was classified with cape nucleic acids rapid diversion hybridization of HPV gene chip technology.The regular follow-up was once every half a year,in two years.Follow-up projects included TCT,HPV HC-II.Results ①Patients with CINⅠwere mainly with sin-gle high-risk HPV type infections;of 150 patients with CINI,HPV positive was in 134 cases (134/150,89.33%) and negative in 16 cases (16/150,10.67%).There were 86 cases of single high-risk pattern infection,(86/150,57.33%) ,46 cases of multiple infections (46/150, 30.67%),and 2 cases of single low-risk type infection (2/150,1.33%).②CINⅠHPV positive was in 134 cases,in which a single infection was in 88 cases (88/134,65.67%),dual infection in 30 cases(30/134,22.39%),triple infection was in 10 cases (10/134,7.46%),and quadruple infection was in 6 cases (6/134,4.48%).③In turn,HPV52,58,16 infection rate was the highest,which was respectively 27. 61%,17.16%,15.67%of HPV positive patients.④In the process of follow-up,1 year negative conversion rate of 150 patients with CINⅠwas 62.00%;two years conversion rate was 80.00%.Course for two years and pathological upgrade accounted for 20.00%,and the patholog-ical upgrade accounted for 12.67%of all patients.Conclusion A single high-risk HPV infection is the main type,with the highest infection rate of HPV52,58,16 subtypes.But in cases of disease duration and progression,HPV16 and 18 subtypes has highest infection rate.Multiple HPV infections can lead to viral clearance extension of time.Large sample for further investigation is still in need with regard to whether to in-crease the risk of pathological upgrade.
Prognostic Value of Human Papillomavirus 16/18 Genotyping in Low-Grade Cervical Lesions Preceded by Mildly Abnormal Cytology

Abstract:Histological low-grade squamous intraepithelial lesion/cervical intraepithelial neoplasia grade 1 (LSIL/CIN1) preceded by normal or mildly abnormal cytology is recommended for conservative follow-up, with no separated management. In this study, we assessed the triage value of human papillomavirus (HPV) 16/18 genotyping in 273 patients with LSIL/CIN1. HPV16/18 genotyping was performed at baseline and follow-up was at 6-monthly intervals for up to 2 years. At each follow-up, women positive for cytology or high-risk HPV (hrHPV) were referred for colposcopy. Enrollment cytology, HPV16/18 genotyping, and questionnaire-obtained factors were linked to the 2-year cumulative progression rate. Univariate and multivariate analyses were performed taking into account time-to-event with Cox proportional hazard regression. The results showed that 190 cases (69.6%) regressed, 37 (13.6%) persisted, and 46 (16.8%) progressed. HPV16/18 positivity (hazard ratio (HR), 2.708; 95% confidence interval (CI), 1.432-5.121; P=0.002) is significantly associated with higher 2-year cumulative progression rate. Sub-analysis by enrollment cytology and age restricted the positive association among patients preceded by mildly abnormal cytology and aged 30 years or older. Immediate treatment is a rational recommendation for the high-risk subgroup, when good compliance is not assured.
Analysis of One Year Follow-Up of Women with Cervical Cytology Report of Atypical Squamous Cells and the Diagnostic Role of High-Risk Hpv Infection

Abstract:Objective: To investigate the risk of developing cervical intraepithelial neoplasia grade 2 (CIN2) or greater disease in patients with cytology report of atypical squamous cells of undetermined significance (ASC-US) or cannot exclude high-grade atypical squamous cells (ASC-H) in one year follow-up. Study design: Analysis of colposcopy-directed multiple cervical biopsies in all patients. Patients without CIN2 or greater diseases were tested for human papillomavirus (HPV) DNA at the enrollment and at 12th month and followed up by cytology at the 6th and 12th month. Patients with repeated abnormal results were subjected to colposcopy-directed biopsy. Results: A total of 894 ASC-US and 101 ASC-H patients were enrolled. The rate of CIN2 or greater disease was 14.2% in ASC-US group and 46.5% in ASC-H group, at the first test respectively. A total of 65.0% of patients in ASC-US have completed the study and 47.5% repeatedly showed abnormal cytology, while the same rates in ASC-H were 62.7% and 50%. Only four cases were diagnosed with CIN2 in ASC-US group. The rate of HPV DNA becoming negative was 54.9% and 51.5% for ASC-US and ASC-H, respectively. Conclusions: The diagnosis rate of CIN2 or greater lesions in ASC-US and ASC-H patients was about 15% and 46.5%, respectively, within one year.

Abstract:This study aimed to investigate whether persistent human papillomavirus integration at the same loci (PHISL) before and after treatment can predict recurrent/residual disease in women with CIN2-3. A total of 151 CIN2-3 women treated with conization between August 2020 and September 2021 were included. To investigate the precision of HPV integration, we further analyzed HPV integration-positive patients. Abstract:Objective: The aim of this study was to investigate the diagnostic value of HPV (human papillomavirus) E6/E7 mRNA combined with thin-layer liquid-based cytology test (TCT) detection in patients with cervical intraepithelial neoplasia (CIN) in the residual and recurrence of lesions after cervical conization. Methods: A total of 154 patients who had undergone cervical conization and were diagnosed with high-grade cervical intraepithelial neoplasia (CIN 1 and CIN 3) by histopathology and had complete follow-up data were enrolled. TCT, HPV-DNA detection, and HPV E6/E7 mRNA detection combined with TCT detection in 12 months can be used to detect abnormal cytology (>atypical squamous epithelium of undetermined significance). Patients with atypical squamous cell of undetermined significance or high-risk HPV-DNA (+) or HPV E6/E7 mRNA combined with TCT (+) underwent colposcopy and cervical biopsy again for pathological diagnosis. Results: There were 9 cases of residual lesions and 22 cases of recurrence. Thirty of the 57 cases with positive HPV-DNA after surgery were still there or came back. Of the 26 cases with positive HPV E6/E7 mRNA combined with TCT, 24 were still there or came back. The sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy rate of HPV-DNA detection were 96.8%, 78.0%, 52.6%, 99.0%, and 81.8%, respectively; the sensitivity and specificity of HPV E6/E7 mRNA were combined with TCT detection of sex. Conclusion: HPV E6/E7 mRNA combined with TCT detection can be included in follow-up examinations, which can timely and effectively predict the risk of CIN residual or recurrence after cervical conization and reduce excessive examination and treatment.

Abstract:Introduction: Persistent human papillomavirus infection is an important factor in the development of cervical cancer, which is usually a long process evolving from the development of squamous intraepithelial lesions (SIL), also referred to as cervical intraepithelial neoplasia (CIN). Local treatment of advanced squamous intraepithelial lesions, also regarded as High-Grade Squamous Intraepithelial Lesion, may be effective in preventing cancer. Objective: To promptly identify high-risk patients with a tendency to recurrence. Methods: We retrospectively analyzed the clinical data of 300 patients with high-grade squamous intraepithelial lesions of the cervix admitted to the Second Affiliated Hospital of Dalian Medical University from 2019 to 2020 to investigate the relationship between recurrence of cervical lesions and postoperative regression of HPV infection, as well as other related risk factors. Results: We found that the HPV-negative rates were 81.81, 85.71, and 90.91% at 6, 12, and 24 months, respectively, and the average lesion recurrence rate was 8.16%, with a median time to recurrence of 14 months in patients undergoing CKC for HSIL. The risk of cervical squamous intraepithelial lesions was highest in patients with HPV16. Patients over 61 years of age had the lowest postoperative HPV-negative rate. The conversion rate was significantly lower in patients with multiple HPV genotypes than in those with single HPV infection ( p < 0.05). The probability of recurrence was higher in patients with the same HPV infection genotype before and after surgery than in patients with different infection genotypes before and after surgery ( p < 0.05). Conclusion: Combined with the literature review, we believe that patients aged ≥50 years, with ≥3 pregnancies and births, a history of smoking, and consistent genotypes of preoperative and postoperative HPV infection in cervical conization have more HPV re-infection or persistent infection, and that these factors may be high-risk factors for lesion recurrence. For patients with possible potential high-risk factors, we need to carry out individualized follow-up and focused management, take timely and effective management measures, optimize the treatment plan, reduce the recurrence rate, prevent HSIL and cervical cancer, improve the quality of patient's survival, and improve the prognosis.

Abstract:Aim: To evaluate the diagnostic performance of E6/E7 HPV-mRNA overexpression towards HPV-DNA testing and p16/Ki67 immunocytochemistry in a post-op population to verify if this biomarker can be effectively used as indicator of successful cervical intraepithelial neoplasia (CIN) treatment. Methods: Our study retrospectively analyzed 197 patients of our Colposcopy Clinic between January 2013 and September 2020 coming with an abnormal Pap smear suggestive for colposcopy, and after a series of follow-ups including liquid-based cytology (LBC) and punch-biopsy sampling, there were surgically treated. LBC was used for cytology and molecular analysis of the three HPV-related biomarkers. Results: Six months after treatment, 93% of the HPV-mRNA-positive women became negative while this applied to only 80.2% of the HPV-DNA-positive women. HPV persistence was 6.9% at 6-12 months after treatment. The comparison among cytology, colposcopy, HPV-DNA test, and HPV-mRNA test after treatment revealed that the last one is the only with a strong correlation with actual severity (histology during treatment) (ρ = 0.345, p = 0.006) implying that clinical cases with more severe CIN may have higher chances of unsuccessful treatment. HPV-mRNA test had higher sensitivity (100%), specificity (96.88%), and positive predictive value (45.45%) for CIN2+ recurrent lesions when compared with HPV-DNA testing (80%, 82.81%, 10.81% respectively) and p16/Ki67 immunocytochemistry (80%, 95.83%, 33.33% respectively) while their negative predictive values were similar. Conclusions: E6/E7 mRNA detection has higher diagnostic values for the prediction of treatment failure compared with HPV-DNA testing and p16/Ki67 immunocytochemistry, and as an outcome could be used as predictive indicator of CIN-treatment status.

Abstract:Post-treatment follow-up in women with CIN3 is mandatory due to relapse in up to 15% of patients within 2 years. Standard follow-up care based on hrHPV-DNA/cytology co-testing has high sensitivity but limited specificity. The aim of our proof-of-concept case-control study was to evaluate the performance of the methylation test GynTect® for the detection of recurrent CIN2/3 during follow-up. Residual clinical material from a recent, prospective, multicenter, observational study was available for further analysis. We studied a sample of 17 cases with recurrent CIN2/3 diagnosed within 24 months of follow-up and 31 controls without recurrence. DNA from cervical scrapes at baseline (immediately before CIN3 surgery) and up to three follow-up visits were analyzed for hrHPV and GynTect® methylation status. Cytology data were available from the previous study. Overall, 12 cases and 21 controls were GynTect-positive at baseline. In these subgroups, single test sensitivity at first follow-up was 67% (95% CI 39–87%) for GynTect® compared to 83% (95% CI 55–96%) for hrHPV (p = 0.50). Single test specificity was significantly higher for GynTect® (90%, 95% CI 71–98% vs. 62%, 95% CI 40–80%) (p = 0.03). In a co-testing setting, both hrHPV/cytology and GynTect®/cytology detected all recurrences. Specificity for GynTect®/cytology was higher than for hrHPV/cytology, but this difference was not statistically significant. In conclusion, for initially GynTect-positive patients, both hrHPV and GynTect® tests detected recurrent disease with similar sensitivity, but the GynTect® assay has a higher specificity. Incident hrHPV infection and/or persisting multifocal hrHPV infections without clinical disease are most likely responsible for the poorer specificity of the hrHPV test. A future prospective validation study will have to show whether GynTect®/cytology co-testing can outperform hrHPV/cytology co-testing in post-treatment surveillance.
