Abstract:BACKGROUND: Sellar germ cell tumors (GCTs) occur more frequently in childhood. Some will present as malignancy with infiltration and metastasis. However, the association between the timeliness of diagnosis and outcome has been controversial. We investigated the clinical risk factors associated with a diagnostic delay in patients with sellar GCTs in China. METHODS: The data from 53 patients aged <18 years with histologically confirmed sellar GCTs at Peking Union Medical College Hospital treated from January 2008 to December 2016 were reviewed retrospectively. RESULTS: The median interval between symptom onset and diagnosis was 25 months. Of the 53 patients, 44 (83%) had a delayed diagnosis. Most patients (86%) with a delayed diagnosis presented with polyuria or polydipsia. Of the 53 patients, 48 (91%) exhibited changes in the pituitary stalk. Patients with a germinoma (odds ratio, 4.1; 95% confidence interval, 2.4-6.9) and slow growth (odds ratio, 5.3; 95% confidence interval, 1.2-24.5) were more likely to have a delayed diagnosis. The overwhelming majority of patients with a delayed diagnosis (96%) had been seen by >1 doctor. No statistically significant differences were found in the mean survival time (P = 0.21) or mean progression-free survival time (P = 0.36) between patients with and without delay in diagnosis, respectively. CONCLUSIONS: A significant proportion of patients with sellar GCTs will experience a delay in the time to diagnosis. Although a delay in diagnosis did not reduce the survival time or progression-free survival time for patients with sellar GCTs, it might increase the risk of short stature. Thus, a detailed medical history and an immediate radiological examination are important for the early diagnosis of sellar GCTs in childhood.

Abstract:Purpose: The impact of delayed diagnosis on tumor-related prognosis appears to be minimal in individuals with intracranial germ cell tumors (iGCTs). However, its effect on neuroendocrine functions remains unclear. We aimed to assess the effects of delayed diagnosis on neuroendocrine function in individuals with suprasellar GCTs. Methods: We conducted a retrospective cohort study of 459 individuals with suprasellar GCTs and categorized them into two groups based on disease duration: delayed diagnosis (> 6 months) and non-delayed diagnosis (≤ 6 months). We compared endocrinological symptoms, neuroendocrine dysfunction and its grading (categorized into 0–3 grades based on severity), and recovery from neuroendocrine dysfunction in both groups. Results: Patients with delayed diagnosis exhibited higher incidences of amenorrhea, slow growth, fatigue, and polyuria/polydipsia. Neuroendocrine dysfunction, including central adrenal insufficiency (CAI), central hypothyroidism (CHT), arginine vasopressin deficiency (AVP-D), growth hormone deficiency, hypogonadism, and hyperprolactinemia, was more pronounced in the delayed diagnosis group at diagnosis, the end of treatment, and the last follow-up. Furthermore, individuals with delayed diagnosis showed higher grades of neuroendocrine dysfunction at diagnosis (OR=3.005, 95% CI 1.929–4.845, p<0.001), end of oncologic treatment (OR=4.802, 95% CI 2.878–8.004, p<0.001), and last follow-up(OR=2.335, 95% CI 1.307–4.170, p=0.005) after adjusting for confounders. Finally, less recovery, particularly in CAI, CHT, and AVP-D, was seen among the group with delayed diagnosis after treatment. Conclusion: Among individuals with suprasellar GCTs, delayed diagnosis is associated with increased, more severe, and less recovered neuroendocrine dysfunction, emphasizing the importance of early diagnosis and treatment to reduce neuroendocrine dysfunction.

Abstract:BACKGROUND Non-Germinomatous Germ Cell Tumors (NGGCT) are rare malignancies with potential for treatment delays. This study aims to identify risk factors and systemic challenges contributing to delayed management of NGGCT in China. METHODS We reviewed the clinical features and referral pattern of 56 consecutively diagnosed NGGCT patients at Shanghai Children’s Medical Center (SCMC) from 9/2018-2/2023. Patients were diagnosed by pathology (n=42), or by elevated markers alone (n=14, AFP>10ng/ml, and/or β-HCG>50mIU/ml). Patients were treated per COG ACNS0122. Delay of treatment was defined as chemotherapy initiated more than 6 months after symptom onset. RESULTS Forty-three males and 13 females were included. Median age at diagnosis was 9.17year (4month-16year). Primary tumor locations included the pineal region (n=31), sellar region (n=19), basal ganglia (n=5), and posterior fossa (n=1). Presenting symptoms included features of increased intracranial pressure (ICP, n=29), endocrine symptoms (n=21), blurred vision or squint (n=7), and hemiplegia (n=4). Delayed treatment occurred in 16 cases (29%), and was more likely in patients with non-pineal region primary (Chi-square p=0.003), and absence of symptoms pertaining to increased ICP (Chi-square p<0.001). Reasons for delay included lack of parental recognition on symptoms (n=7), misdiagnosis (n=3), and inappropriate/incomplete workup (n=6). As a national referral center, patients commuted a median of 483 km (range: 12-2943km) to SCMC (median 2 hospitals (1,5) before SCMC), including 10 patients with obvious hydrocephalus travelling before any CSF diversion procedure. Three-year Event-Free Survival (EFS) was 76.9% and Overall Survival was 86.0%. There was a trend to inferior outcome in patients with delay in treatment (3y EFS 69.6%) compared with those without (3year EFS 80.6%, p=0.112). CONCLUSIONS Delay in treatment is common for children with NGGCT managed at a tertiary referral center in China. Improving public awareness, training for primary-care clinicians, and establishing a national referral network are potential strategies to enhancing the outcome.
Results of Biopsy-Proven Sellar Germ Cell Tumors: Nine Years' Experience in a Single Center

Abstract:BACKGROUND: The biopsy is recognized as the most accurate method to determine the histologic characterization of sellar germ cell tumors. It is difficult to evaluate the prognosis before histologic confirmation. OBJECTIVE: This study aimed to evaluate the independent prognostic risk factors of patients with sellar germ cell tumors (GCTs). METHODS: From January 2008 to December 2015, 61 patients who were histologically diagnosed as having sellar GCTs were followed up and were included in this retrospective study. RESULTS: Of 61 patients in this study, 40 (65.6%), 10 (16.4%), and 11 (18.0%) were diagnosed as having pure germinomas, germinomas with syncytiotrophoblastic giant cells and non-germinomatous GCTs (NGGCTs), respectively. The patients with pure germinomas had a significantly better overall survival time than did those with NGGCT (56.47 +/- 3.01 months vs. 43.09 +/- 10.58 months; P = 0.01). Multivariate analysis showed that the independent poor prognostic risk factors of patients with sellar GCTs were diameters >15 mm (odds ratio [013], 7.40; 95% confidence interval [CI], 2.01-27.19), octamer-binding transcription positivity (OR, 5.97; 95% CI, 1.40-25.48), and NGGCT (OR, 11.88; 95% CL 2.37-59.50), whereas the combination of chemotherapy and radiotherapy was associated with a better prognosis (OR, 0.15; 95% CI, 0.04-0.55). CONCLUSIONS: Diameter >15 mm, octamer-binding transcription positivity, or NGGCT was associated with a poorer prognosis for patients with sellar GCTs, whereas the combination of chemotherapy and radiotherapy was associated with a better prognosis.
Early diagnosis and comprehensive treatment for germinoma of sellar region

Abstract:Objective To review the experiences of early diagnosis and comprehensive treatment for germinoma of the sellar region.Methods Clinical data of 23 patients who were suspected as having germinoma in the sellar region were analyzed retrospectively.Nine patients underwent surgery and the postoperative pathological examination confirmed germinoma of the sellar region,then they received chemoradiotherapy.Fourteen patients were diagnosed as having germinoma in the sellar region according to clinical features,serological examination,imaging and diagnostic radiotherapy and received chemoradiotherapy.Results All the patients were followed up for 11 to 57 months with a mean period of 23.8 months.The tumor recurred in one patient whose tumor disappeared after chemotherapy.And 8 patients with high blood level of human chorionic gonadotropin(HCG) recovered after chemoradiotherapy.Headache and visual deterioration of all the patients were significantly relieved,but endocrine abnormality such as polydipsia and polyuria still existed.Conclusions For the patients with germinoma in the sellar region,early diagnosis and comprehensive treatment such as chemoradiotherapy can make good outcome.

Abstract:There were rare clinical reports on clear cell tumor of the lung (CCTL). The clinical characteristics and underlying genetic mutation status of CCTL are poorly understood. From 2012 to 2017, patients pathologically diagnosed with CCTL in our hospital were investigated and analyzed based on clinical manifestations, pathological characteristics, prognosis and full gene mutation status through next generation sequencing (NGS) technology. During a 6-year period, four eligible patients were diagnosed with CCTL through surgical resection and were included in this study. All patients showed solitary nodules or lumps located in the left lung. The average maximum diameter of lesions was 2.5 ± 1.1 cm. Computed tomography (CT) imaging characteristics of these nodules/lumps demonstrated the features of benign tumors. The hematoxylin-eosin (HE) morphology and immunohistochemistry were consistent with the histopathological features of benign CCTL. Subsequent NGS analysis showed frame shift mutations of F2421/E2419, K1466E mutation, and p. 1450_1456 deletion mutation in mTOR gene in two of four patient samples and amplifications of MCL1 were observed in three of four samples. CCTL is a rare type of primary pulmonary mesenchymal tumor with good prognosis. Preliminary diagnosis on CT is usually sclerosing pneumocytoma. It is still unclear whether the occurrence and development of the disease are related to specific gene mutation. In this study, the genomic findings of frame shift mutation of mTOR genes and amplification of MCL1 gene in CCTL suggest that these mutations might play a role in proliferation of CCTL.

Abstract:Objective To explore the MRI features of germ cell tumors in sellar regions for better developing clinical treatment plans.Methods The imaging and pathologic findings of 44 sellar germ cell tumors were retrospectively analyzed.Results There were 1 9 germinoma,most of which had clear boundaries,different extents of cystic necrosis and mild peritumoral edema.Lesions were significantly enhanced in contrast-enhanced MRI (CE-MRI).There were 8 cases with mature teratoma,most showed cystic-solid changes,presenting mixed-intensities on T1-weighted images and T2-weighted images.There were 1 1 malignant teratoma(im-mature teratoma and teratoma malformation ),however,there were more parenchymatous tissue in malignant teratoma than mature teratoma.Two yolk sac tumors showed uniform signals on T1-weighted images and T2-weighted images,significant enhancement on CE-MRI,and visibly higher AFP in serum.Two embryonal carcinomas had clear boundary,inhomogeneous intensities,visibly cyst-ic necrosis,median to servere peritumoral edema,the parenchyma showed significant enhancement in CE-MRI.The signals of two mixed germ cell tumors were not typical,which depended on the components within tumors.Conclusion The MRI features of germ cell tumors in sellar region are characteristic.
RARE-20. ANALYSIS OF CLINICAL FEATURES ON DEATH FROM INTRACRANIAL GERM CELL TUMORS (GCTS): ARETROSPECTIVE COHORT STUDY

Abstract:Abstract OBJECTIVES The present study aimed to investigate the clinical features associating with intracranial germ cell tumors(GCTs) of poor prognosis. METHODS We retrospectively analyzed the death data cohort based on 47 patients (30 males and 17 females) who had been diagnosed with GCTs between 2005 and 2018 in our hospital. The median age at initial diagnosis was 16 years (range, 3¬60 years). Among those, 18 patients were germinoma and 29 patients were non¬seminomatous germ-cell tumors (NGGCTs). RESULTS According to Kaplan-Meier survival analysis, children aged ≤ 10 years were observed with a very short survival compared to patients aged > 10 years, indicating a 5-year survival rate of 0 and 50%, respectively (P < 0.001). To be more specific, disseminated tumors were found to be the most prominent factor for prognosis in children aged ≤ 10 years: the 1-year survival for MR positive versus MR negative of disseminated tumors were 0 and 62.5%, respectively (P < 0.05). On the other hand, the pre¬treatment serum alpha-fetoprotein (AFP) level was found to be an important surrogate for prognosis. In the subgroup of patients > 10 years, the 5-year survival for AFP > 1.5ng/ml was 25%, in comparison to 66% for AFP ≤ 1.5ng/ml (P < 0.05). Further, solid survival advantages were found in patients received stereotactic radiosurgery (SRS) compared to none. The 5¬year survival for SRS versus non-SRS patients were 62% and 29%, respectively (P < 0.05). CONCLUSION Our data highlights a number of critical factors that may facilitate the prognosis stratifications of GCTs. What’s more, neither the locations of the primary tumors nor the numbers of lesions are associated with the prognosis of GCTs. These results warrant further investigation via randomized prospective studies.

Abstract:Objective To summarize the clinical features of primary intracranial germ cell tumors (GCTs).Methods Based on the WHO 2007 classification criteria for CNS tumors,we retrospectively analyzed 162 cases of primary intracranial GCTs admitted into Nanfang Hospital from Jan.2004 to Oct.2013.Results There were 135 cases of single-focus tumors and 27 multi-focus tumors.70.4％ of tumors were originated in pineal region,20.4％ in supra-sellar region,7.4％ in basal ganglion region,and 1.8％ in other regions.The average age of all cases,pineal region cases,supra-sellar region,and basal ganglion region cases,was 14.4 years,14.5 years,14.9 years,and 12.6 years,respectively.The male/female ratio was 7.1∶1 in all cases,and 18∶1 in pineal region cases,1.4∶1 in supra-sellar region cases,and 5∶1 in basal ganglion region cases.The positive rate of tumor markers was 42.5％,and germinomas accounted for 46.9％ of all cases.Conclusions The clinical features of GCTs are special,and detailed analysis of clinical data is helpful for the diagnosis.More than a half of patients will achieve ideal therapeutic results through timely diagnosis and reasonable treatment.
Basal Ganglia Germ Cell Tumors With or Without Sellar Involvement: A Long-Term Follow-Up in a Single Medical Center and a Systematic Literature Review

Abstract:Background Basal ganglia germ cell tumors (BGGCTs) represent an extremely rare subset of tumors about which little is known. Some patients suffer from tumor dissemination, such as sellar involvement. This study aimed to evaluate the independent prognostic risk factors of patients with BGGCTs with or without sellar involvement. Methods Sixteen patients were diagnosed with BGGCTs at Peking Union Medical College Hospital from January 2000 to December 2020. A literature review was performed on the online databases Medline and PubMed, and 76 cases in the 19 retrieved articles were identified at the same time. The data regarding biochemical tests, radiological examinations, and outcomes during follow-up were analyzed. Results Of 92 patients in this study, seven patients were clinically diagnosed as germinomas, with the remaining 85 patients receiving surgery. Fifty-two patients suffered from multifocal lesions or tumor dissemination. The patients with BGGCTs demonstrated a significant male predilection. The patients with delayed diagnosis more likely had cognitive disturbance (p = 0.028), mental disturbance (p = 0.047), and diabetes insipidus (p = 0.02). Multivariate analysis demonstrated that the independent poor prognostic risk factors of patients with BGGCTs were delayed diagnosis [odd ratio (OR) 2.33; 95% CI 1.02–5.31], focal radiotherapy (OR 4.00; 95% CI 1.69–9.49), and non-pure germinoma (OR 4.64; 95% CI 1.76–12.22). Conclusions The delayed diagnosis, focal radiotherapy, and non-pure germinoma were associated with a poorer prognosis for patients with BGGCTs with or without sellar involvement.

Abstract:To the Editor: Testicular tumor represents 1% of male neoplasms and 5% of urogenital tumors. Non-seminomatous germ cell tumor (NSGCT) accounts for nearly 40% of all testicular cancers. The survival of patients with NSGCT has been improved because of advanced multimodal therapies (orchiectomy, and if necessary, subsequent chemotherapy). [1] However, to the best of our knowledge, survival of NSGCT patients has not been studied in a large Chinese cohort yet. Therefore, this study assessed the clinical characteristics, treatments, and outcomes of patients with NSGCT.

Abstract:Background Delay in diagnosis and treatment of cancer may lead to advanced tumor characteristics and poor prognosis. Research and investigation from economically developing countries such as China are warranted to support these conclusions, so we studied the impact on prognosis of delays and factors predicting delay in symptomatic breast cancer patients in China. Methods Medical records and follow-up information were collected. Variables including demographic data, and clinical and tumor characteristics, including patient age, menstrual status, residential status, initial symptom, profession, comorbidities, tumor size, lymph node metastasis, distant metastasis, history of breast disease, and family history of breast cancer, were analyzed, as was survival information. Results A total of 1,431 women diagnosed with breast cancers between 1998 and 2005 in Qilu Hospital were enrolled and studied. Delays in diagnosis and treatment were correlated with larger tumor size, lymph node metastasis, late tumor stage, and worse disease-free survival, as assessed by multivariate logistic regression and Kaplan–Meier regression models. Patient residential status, initial symptom, menopausal status, and history of breast disease were independent predictors of delay. Stratified multivariate analyses confirmed that age was not associated with delay. Conclusions Delay in diagnosis and treatment predicts worse clinical outcomes. Improvement of medical service in rural areas, especially for premenopausal women, can decrease delays and benefit breast cancer patients.

Abstract:Gestational trophoblastic tumors(GTTs) are highly malignant gynecological tumors.However,their curative rates are the highest in clinic among malignant tumors.Early diagnosis and treatment are still very important to improve prognosis of patients with malignant tumors.With development in laboratory technique and improvement of apparatuses,people have more and deep understanding for these diseases,hCG is no longer an exclusive tumor marker in clinical diagnosis,the research on its relevant molecules make its value more exact,advanced imaging equipments can find occult minifocus of these diseases.In short,in recent years,many studies have focused on the morphology and molecular level of these diseases in order to make early and definite diagnosis.In this review,we summarized development in diagnosis of these malignancies.
Clinical characteristics of malignant germ cell tumors in adolescents: A multicenter 10-year retrospective study in Beijing

Abstract:Background: The aim of this study was to review clinical features of adolescent malignant germ cell tumors (MGCTs) in Beijing and analyze the peculiar characteristics of this age group. Methods: Clinical characteristics, pathological presentations, and survival outcomes of 34 patients were analyzed retrospectively. Results: Of 34 patients, 12 girls and 22 boys, 18 (52.9%) had an extra-cranial tumor, including one testicular tumor, five ovarian tumors, one sacrococcygeal tumor, and 11 mediastinal tumors. Histologically, we found immature teratomas (n = 6), yolk sac tumors (n = 5), mixed malignant tumors (n = 5), an embryonic carcinoma (n = 1), and seminoma (n = 1). Three-year event-free survival (EFS) and overall survival (OS) were 48.8% and 62.9%, respectively. Another 16 (47.1%) patients had an intracranial tumor, including nine in the pineal region, five in the suprasellar region, one in basal ganglia, and one in cerebellopontine. All patients had localized disease and an excellent outcome with 3-year EFS and OS of 93.7% and 100%, respectively. Conclusions: Adolescent MGCTs are rare with a strong dependence on gender, and the mediastina and pineal region are the most common tumor locations. The prognosis is promising compared with that of other adolescent tumors and MGCTs in other age groups. MGCTs in mediastina have a tendency to companion with other hematological malignancies, and the prognosis is extremely poor in these patients.

Abstract:Objective To investigate the clinical and pathological characteristics of digestive tract cancer concurrent with gastrointestinal stromal tumors(DTCGIST).Methods Clinical and pathological data of 18 cases with DTCGIST were collected and analyzed.ResultsNo gastrointestinal stromal tumor(GIST)was detected by preoperative CT examination.One case of DTCGIST was found by preoperative endoscopy,and the other 17 digestive tract cancers.The size of GIST in DTCGIST was smaller than the primary GIST cases,and the mitotic score was less than 5/50 HPF.There were no GIST cells in the lymph nodes around the specimen.It was revealed by immunohistochemistry that approximately 88.89% cases(16/18)were positive for CD117,88.89%(16/18)for CD34,33.33%(6/18)for SMA,61.11%(11/18)for Vimentin and 16.67%(3/18)for Desmin.Conclusion Compared with gastric cancer and GIST,DTCGIST exhibits no distinct characteristics.Most GISTs in DTCGIST are in the early period and have low risk of malignance.
Diagnosis and Treatment for Synchronous Occurrence of Gastrointestinal Stromal Tumor and Digestive Tract Cancer: a Report of 6 Cases

Abstract:Objective To explore the clinical characteristics, diagnosis, treatment and prognosis of synchronous occurrence of gastrointestinal stromal tumor( CIST) and digestive tract cancer in 6 cases. Methods The clinical and pathologic data of 6 patients of synchronous occurrence of gastrointestinal stromal tumor and digestive tract cancer from January 2005 to September 2010 in Wuhan Union Hospital were analyzed retrospectively. Results In 4 cases both tumors were located in the same organ, in the other 2 cases tumors were located in the adjacent sites of digestive tract. Only 1 case was found both tumors by preoperative examination , and the others were found digestive tract cancer. Gastrointestinal stromal tumors were all in the very low risk group, the diameter of CISTs was less than 1 cm and the mitotic score was less than 2/50 HPF in all the cases. Digestive tract cancer showed no specical demonstrations both macroscopically and microscopically. Conclusions Clinical features of synchronous occurrence of CIST and digestive tract cancer are atypical. We should pay much attention to this entity in clinical practice. In this series, most CIST are in early stage and of low risk of malignance. The prognosis of patients with synchronous occurrence of GIST and digestive tract cancer mainly depends on digestive tract cancer.
Intracranial germ cell tumors: a view of the endocrinologist

Abstract:Intracranial germ cell tumors (iGCTs) are rare malignant neoplasms that mainly affect children and adolescents. The incidence, clinical presentation, and prognosis of iGCTs exhibit high heterogeneity. Previous studies have primarily focused on eliminating tumors, reducing tumor recurrence, and improving survival rates, while neglecting the impact of the tumors and their treatment on neuroendocrine function. Throughout the entire course of the disease, neuroendocrine dysfunction may occur and is frequently overlooked by oncologists, neurosurgeons, and radiologists. Endocrinologists, however, are more interested in this issue and have varying priorities at different stages of the disease. From onset to the diagnostic phase, most patients with iGCTs may present with symptoms related to impaired neuroendocrine function, or even experience these symptoms as their first indication of the condition. Particularly, a minority of patients with sellar/suprasellar lesions may exhibit typical imaging features and elevated tumor markers long after the onset of initial symptoms. This can further complicate the diagnosis process. During the peritumor treatment phase, the neuroendocrine function shows dynamic changes and needs to be evaluated dynamically. Once diabetes insipidus and dysfunction of the hypothalamic-pituitary-adrenal and hypothalamic-pituitary-thyroid axes occur, hormone replacement therapy should be administered promptly to ensure successful tumor treatment for the patient. Subsequently, during the long-term management phase after the completion of tumor treatment, the evaluation of growth and development as well as corresponding hormone replacement therapy are the most concerning and complex issues. Thus, this paper reviews the interest of endocrinologists in iGCTs at different stages.

Abstract:BACKGROUND: Tenosynovial giant cell tumors (TGCTs), synovial chondromatosis (SC), and synovial sarcoma (SS) exhibit similarities in clinical features and histochemical characteristics, and differential diagnosis remains challenging in clinical practice.METHODS: Data were collected from the pathology database of Shanghai Ninth People's Hospital regarding patients who underwent surgery from 2010 to 2019 with histologically confirmed TGCTs, SC, and SS. Demographic and clinicopathological data of these patients were reviewed. Immunohistochemistry staining of 14 different markers was performed. Correlation analyses of the prognoses were evaluated.RESULTS: A total of 26 patients with TGCTs (8 diffuse TGCTs and 18 localized TGCTs), 16 with SC, and 11 with SS were identified. Pain was the main symptom of patients with both TGCTs and SC, while a palpable mass was the most common symptom for patients with SS. In addition to clinical features, we identified vital risk factors for disease recurrence. The mean follow-up periods were 51, 39, and 14 months for TGCTs, SC, and SS, respectively. Younger patients with diffuse TGCTs or patients with a higher neutrophil/lymphocyte ratio (NLR) displayed a significantly higher frequency of recurrence. We also plotted receiver operating characteristic (ROC) curve analysis for age and NLR. The area under the ROC curve (AUC) was calculated and demonstrated the ability to distinguish recurrent from nonrecurrent cases. In addition, higher CD163 expression was linked to recurrent diffuse TGCT cases.CONCLUSIONS: These data indicated possible characteristics of different aspects of TGCTs, SC, and SS. Further clarification and understanding of these factors will help with differential clinical diagnosis and recurrent risk assessment.

Abstract:Introduction Prepubertal testicular tumors are fundamentally distinct from their postpubertal and adult counterparts. Racial and ethnic differences contribute to the incidence, characteristics, and histological distribution of prepubertal testicular germ cell tumors (PTGCTs). Objective To elucidate the characteristics and treatment out- comes of PTGCTs in a Chinese cohort. Study design Data were retrospectively reviewed from consecutive PTGCT patients aged <12 years who received treatment at the current institution from January 2007 to December 2015. Exclusion criteria were: patients aged >12 years, non-primary testicular germ cell tumors, and para-testicular tumors. Results A total of 167 patients qualified for the study (Table). The median age at diagnosis was 20 months (range 1-142). Pathology revealed 87 (50.9%) teratomas, 50 (29.2%) yolk sac tumors (YSTs), 33 (19.3%) epidermoid cysts, and one (0.6%) mixed germ cell tumor. Of the teratomas, 70 (80.5%) mature teratomas (MTs) and 17 (19.5%) immature teratomas (ITs) were detected. ITs manifested at a younger age and were larger in size compared with MTs (P < 0.001). Testis-sparing surgery (TSS) was performed for all MTs and 10 ITs. Radical orchiectomy (RO) was performed for seven ITs. No metastatic or local recurrence was detected in benign tumors during follow-up. Obviously elevated alpha feto-protein (AFP) levels were detected in all the YSTs. Of these, 47 (94%) were diagnosed as Stage I, one (2%) as Stage II, and two (4%) as Stage IV. After RO, recurrence was detected in one Stage I patient with a median follow-up of 46 months. One Stage II and two Stage IV patients died due to tumor progression. Discussion Racial and ethnic differences were detected in PTGCTs. It is believed that the present study is the largest study detailing the clinicopathologic characteristics and treatment outcomes of PTGCTs in a Chinese cohort. Conclusions Teratoma was the most common subtype of PTGCTs, followed by YST. ITs manifested as benign behavior and were more likely to be present at a younger age and have a larger tumor size compared with MTs. TSS was reliable for benign lesions. For Stage I YST, RO alone was valid. Salvage chemotherapy was effective and preventative retroperitoneal lymph node dissection may not be necessary for YST. The prognosis of PTGCTs was favorable.

Abstract:Objective To appraise the way of improving preoperative diagnosis of gastrointestinal stromal tumors(GIST).Methods Retrospectively analyzed the clinical and diagnostic data of 24 cases with GIST from Jan- uary 2002 to December 2005 in our hospital.Results Among all the patients In this group,gastrointestinal bleed- ing occupied 54%;with stomach as the most common site of bleeding(62%).The positive diagnosis by CT and ul- trasound were 79% and 41% respectively.Preoperative biopsy mostly showed plethoric spindle-shape cells and epi- thelial cells.Tumor less than 2 cm was less invasive.Positive CD117 occurred in 96% and positive CD34 in 66%. Conclusions Heightening our knowledge about GIST,with combined multidetection methods would be able to im- prove the preoperative diagnostic rate,among which,CT scan has its special value as the first choice.Tumor less than 2 cm tends to be benign.(Shanghai Med J,2006,29:656-658)
