Abstract:Hypertrophic cardiomyopathy (HCM) is a genetically heterogeneous disorder with several established genotype-phenotype relationships. While biallelic truncating variants in the ALPK3 gene cause severe congenital HCM, recent studies have associated heterozygous truncating variants (ALPK3tv) with milder adult-onset HCM. Here we describe a multicenter cohort of 21 individuals with heterozygous ALPK3tv from 10 institutions in the United States, highlighting distinctive clinical characteristics compared to a control group of 132 patients with HCM caused by deleterious variants in sarcomeric genes. As compared to other HCM genotypes, ALPK3tv patients present at an older age (mean 57.25 years) with significantly lower left ventricular wall thickness (14.09 vs 19.78 mm with echocardiogram and 16.13 vs 21.13 mm with cardiac MRI), a lower prevalence of obstructive HCM (15% of ALPK3tv vs 45% of controls), and a strikingly higher incidence of apical aneurysm (22.22% vs. 2.40% in the control group). These results suggest a milder degree of hypertrophy in heterozygous ALPK3-related HCM as compared to other Mendelian causes of HCM, although the increased occurrence of apical aneurysms could have implications for ventricular arrhythmia risk. Our study underscores the importance of recognizing heterozygous ALPK3tv as a cause of adult-onset HCM and provides a comprehensive characterization of its clinical phenotype.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to explore the relationship between heterozygous ALPK3 truncating variants (ALPK3tv) and late - onset hypertrophic cardiomyopathy (HCM), especially its clinical features and pathological mechanisms. Specifically, the main research questions include: 1. **Association between ALPK3tv and HCM**: Confirm whether ALPK3tv can cause adult - onset HCM and describe the clinical manifestations of patients in this case. 2. **Comparison with traditional HCM**: Compare HCM patients related to ALPK3tv with HCM patients caused by other pathogenic genes (such as sarcomere genes) to determine the specific clinical features of ALPK3tv - related HCM. 3. **Incidence of apical aneurysm**: Explore the high incidence of apical aneurysm in ALPK3tv - related HCM and its impact on the prognosis of patients. ### Research background Hypertrophic cardiomyopathy (HCM) is a genetically heterogeneous disease, usually manifested as thickening of the left ventricular wall. Although it is known that biallelic ALPK3 truncating variants can cause severe congenital or childhood HCM, recent studies have shown that monoallelic ALPK3 truncating variants may also cause adult - onset HCM. However, the specific role of ALPK3tv in adult HCM and its clinical features are still controversial. ### Main findings Through a multicenter cohort study, the authors found: - **Late onset age**: The average onset age of HCM patients related to ALPK3tv was 57.25 years, which was significantly later than that of other types of HCM patients. - **Lower ventricular wall thickness**: The maximum left ventricular wall thickness (MWT) of ALPK3tv patients was significantly lower than that of other HCM patients. - **High incidence of apical aneurysm**: The incidence of apical aneurysm in ALPK3tv patients was 22.22%, much higher than 2.40% in the control group. - **Less obstructive HCM**: The incidence of obstructive HCM in ALPK3tv patients was only 15%, while that in the control group was 45%. - **Less arrhythmia**: The incidence of arrhythmia in ALPK3tv patients was relatively low, especially premature ventricular contractions (PVCs). ### Clinical significance These findings are helpful to better understand the role of ALPK3tv in HCM and provide more accurate diagnosis and treatment suggestions for clinicians. Especially for patients with apical aneurysms, closer monitoring and preventive measures may be required, such as anticoagulant therapy to reduce the risk of thromboembolism. ### Summary This study emphasizes the importance of identifying ALPK3tv as the cause of adult - onset HCM and provides a comprehensive description of its clinical phenotype, providing an important basis for future research and clinical management.
