WNT and TGF-Beta Pathway Alterations in Early-Onset Colorectal Cancer Among Hispanic Populations

Abstract:Colorectal cancer (CRC) ranks as the third most prevalent type of cancer in the United States. Hispanics, the largest and one of the fastest-growing minority groups in the U.S., experience disproportionately higher incidence and mortality rates from early-onset CRC (diagnosed before age 50) compared to their non-Hispanic White (NHW) counterparts. While alterations in the WNT and TGF‐Beta signaling pathways are well-established drivers of CRC development in the general population, the specific roles of these pathways in early-onset CRC among Hispanics have yet to be comprehensively explored. In this study, we conducted a bioinformatics analysis using cBioPortal to assess mutations in the WNT and TGF‐Beta pathways in CRC cases, stratified by age (<50 years for early-onset vs. ≥50 years for late-onset) and ethnicity (early-onset Hispanics vs. early onset NHWs). Our analysis revealed no significant differences in WNT and TGF-Beta pathway alterations between early-onset and late-onset CRC cases within the Hispanic cohort, consistent with findings from large cohorts that do not specify ethnicity. However, significant differences were observed in WNT pathway alterations when comparing early-onset Hispanic CRC patients to early-onset NHW patients; WNT pathway showed marginal significance alterations in early-onset colon cancer. In terms of the TGF‐Beta pathway, alterations in the gene BMP7 were significantly more frequent in early-onset Hispanic CRC patients than in early-onset NHW patients. Additionally, the presence of WNT pathway alterations in early-onset CRC among Hispanics was associated with better clinical outcomes, mirroring findings in the early-onset NHW cohort. Conversely, the absence of TGF‐Beta pathway alterations in early-onset Hispanic CRC cases was linked to improved clinical outcomes, a pattern that differed from the NHW early-onset cohort. These findings underscore important ethnic disparities in WNT and TGF‐Beta pathway alterations in early-onset CRC and emphasize the need for further investigation into the molecular characterization of CRC in early-onset Hispanic populations to identify possible targets for new therapies and improve clinical outcomes.
