Abstract:During tumor progression and especially following cytotoxic therapy, cell death of both tumor and stromal cells is widespread. Despite clinical observations that high levels of apoptotic cells correlate with poorer patient outcomes, the physiological effects of dying cells on tumor progression remain incompletely understood. Here, we report that circulating apoptotic cells robustly enhance tumor cell metastasis to the lungs. Using intravenous metastasis models, we observed that the presence of apoptotic cells, but not cells dying by other mechanisms, supports circulating tumor cell (CTC) survival following arrest in the lung vasculature. Apoptotic cells promote CTC survival by recruiting platelets to the forming metastatic niche. Apoptotic cells externalize the phospholipid phosphatidylserine to the outer leaflet of the plasma membrane, which we found increased the activity of the coagulation initiator Tissue Factor, thereby triggering the formation of platelet clots that protect proximal CTCs. Inhibiting the ability of apoptotic cells to induce coagulation by knocking out Tissue Factor, blocking phosphatidylserine, or administering the anticoagulant heparin abrogated the pro-metastatic effect of apoptotic cells. This work demonstrates a previously unappreciated role for apoptotic cells in facilitating metastasis by establishing CTC-supportive emboli, and suggests points of intervention that may reduce the pro-metastatic effect of apoptotic cells.

What problem does this paper attempt to address?
### Problems the Paper Attempts to Solve The paper attempts to address the following issues: 1. **The impact of apoptotic cells on the survival and metastasis of circulating tumor cells (CTCs)**: - Studies have found that high levels of apoptotic cells are associated with poorer prognosis in patients, but the specific physiological effects of apoptotic cells on tumor progression are not fully understood. The paper explores how apoptotic cells enhance lung metastasis by promoting the survival of circulating tumor cells. 2. **How apoptotic cells promote the survival of circulating tumor cells**: - Using in vivo metastasis models, researchers observed that the presence of apoptotic cells can support the survival of circulating tumor cells in the lung vasculature. Apoptotic cells promote tumor cell survival by recruiting platelets to form a protective microenvironment. 3. **Apoptotic cells promote metastasis through thrombosis mechanisms**: - The paper details how apoptotic cells expose phosphatidylserine (PS), activate tissue factor (TF), and subsequently trigger platelet aggregation, protecting nearby tumor cells from immune attacks and shear forces. 4. **Intervening in the mechanism by which apoptotic cells promote metastasis**: - The paper also explores reducing the pro-metastatic effect of apoptotic cells by inhibiting their ability to induce coagulation (e.g., knocking out tissue factor, blocking phosphatidylserine, or administering the anticoagulant heparin). Overall, the paper reveals that apoptotic cells promote the survival of circulating tumor cells by forming thrombi and proposes several potential interventions to reduce this pro-metastatic effect.
