Abstract:( )-ketamine (ketamine) has rapid and sustained antidepressant (AD) efficacy at sub-anesthetic doses in depressed patients. A metabolite of ketamine, including ( )-hydroxynorketamine (( )-HNKs) has been reported to exert antidepressant actions in rodent model of anxiety/depression. To further understand the specific role of ketamine’s metabolism in the AD actions of the drug, we evaluated the effects of inhibiting hepatic cytochrome P450 enzymes on AD responses. We assessed whether pre-treatment with fluconazole (10 and 20 mg/kg, i.p.) 1 hour prior to ketamine or HNKs (10 mg/kg, i.p.) administration would alter behavioral and neurochemical actions of the drugs in male BALB/cJ mice with a highly anxious phenotype. Extracellular microdialysate levels of glutamate and GABA (Glu , GABA ) were also measured in the medial prefrontal cortex (mPFC). Pre-treatment with fluconazole altered the pharmacokinetic profile of ketamine, by increasing both plasma and brain levels of ketamine and ( , )-norketamine, while robustly reducing those of ( )-HNKs. At 24 hours post-injection (t24h), fluconazole prevented the sustained AD-like response of ketamine responses in the forced swim test and splash test, as well as the enhanced cortical GABA levels produced by ketamine. A single ( )-HNK administration selectively rescued the antidepressant-like activity of ketamine in mice pretreated with fluconazole within 24 hours of treatment. Overall, these findings are consistent with an essential role of ( )-HNK in mediating the sustained antidepressant-like effects of ketamine and suggest potential interactions between pharmacological CYPIs and ketamine during antidepressant treatment in patients.

What problem does this paper attempt to address?
The problem this paper attempts to address is: investigating the specific role of ketamine metabolites in the antidepressant effects of ketamine by inhibiting hepatic cytochrome P450 (CYP450) enzymes. Specifically, the researchers evaluated the effects of fluconazole pretreatment on the behavioral and neurochemical effects of ketamine and its metabolites, particularly (2R,6R)-hydroxynorketamine ((6)-HNK). ### Main Research Questions: 1. **The role of ketamine metabolites in antidepressant effects**: The researchers aimed to assess whether metabolites such as (6)-HNK play a crucial role in the sustained antidepressant effects of ketamine by inhibiting CYP450 enzymes. 2. **The impact of fluconazole on ketamine metabolism**: The researchers evaluated the effects of fluconazole pretreatment on the pharmacokinetic and pharmacodynamic properties of ketamine and its metabolites. 3. **Changes in behavioral and neurochemical effects**: The researchers assessed the impact of fluconazole pretreatment on ketamine-induced behavioral and neurochemical changes through behavioral tests and microdialysis techniques. ### Research Methods: - **Animal Model**: Male BALB/cJ mice were used for behavioral and microdialysis experiments. - **Drug Treatment**: Fluconazole (10 mg/kg and 20 mg/kg) was administered intraperitoneally 1 hour before ketamine (10 mg/kg) administration. - **Behavioral Tests**: Included the Forced Swim Test (FST), Open Field Test (OF), and Splash Test. - **Microdialysis**: Measured levels of glutamate (Glu) and gamma-aminobutyric acid (GABA) in the medial prefrontal cortex (mPFC). - **Pharmacokinetic Analysis**: Determined the concentrations of ketamine, norketamine, and (6)-HNK in plasma and brain tissue. ### Main Findings: - **Fluconazole pretreatment increased exposure to ketamine and norketamine, reduced exposure to (6)-HNK**: Within the first 4 hours post-administration, fluconazole pretreatment significantly increased the concentrations of ketamine and norketamine in plasma and brain tissue while reducing the concentration of (6)-HNK. - **Fluconazole pretreatment prevented the sustained antidepressant effects of ketamine**: At 24 hours post-administration, mice pretreated with fluconazole showed reduced antidepressant-like behavioral responses in the Forced Swim Test and Splash Test, and decreased GABA levels in the medial prefrontal cortex. - **Selective restorative effect of (6)-HNK**: Administration of (6)-HNK alone partially restored the antidepressant-like behavioral responses in fluconazole-pretreated mice. ### Conclusion: These findings suggest that (6)-HNK plays an important role in mediating the sustained antidepressant effects of ketamine and indicate potential interactions of CYP450 enzymes in the antidepressant treatment with ketamine.
