Abstract:Community engagement (CE) and community-engaged research (CEnR) are increasingly viewed as the keystone to translational medicine and improving the health of the nation. In this article, the authors seek to assist academic health centers (AHCs) in learning how to better engage with their communities and build a CEnR agenda by suggesting five steps: defining community and identifying partners, learning the etiquette of CE, building a sustainable network of CEnR researchers, recognizing that CEnR will require the development of new methodologies, and improving translation and dissemination plans. Health disparities that lead to uneven access to and quality of care as well as high costs will persist without a CEnR agenda that finds answers to both medical and public health questions. One of the biggest barriers toward a national CEnR agenda, however, are the historical structures and processes of an AHC-including the complexities of how institutional review boards operate, accounting practices and indirect funding policies, and tenure and promotion paths. Changing institutional culture starts with the leadership and commitment of top decision makers in an institution. By aligning the motivations and goals of their researchers, clinicians, and community members into a vision of a healthier population, AHC leadership will not just improve their own institutions but also improve the health of the nation-starting with improving the health of their local communities, one community at a time.

Abstract:Community-based participatory research (CBPR) has furthered our understanding of the working principles required for academic-community partnerships to address persistent public health problems. However, little is known about how effective these partnerships have been in eliminating or reducing community-based public health issues. To contribute to the literature in this area, the authors conducted a survey of U.S. schools and programs in public health and community groups working with these academic partners to: (1) identify the most common local public health issues addressed; (2) examine the characteristics of the partnership and the actual or perceived benefits and challenges for each partner; (3) assess the perceived effectiveness of the partnership and their evaluation techniques; and (4) analyze the intent to continue or dissolve the partnership and the associated factors that influence this decision. The authors provide recommendations that can improve the development, functioning, and effectiveness of academic-community collaborations aimed at addressing a variety of public health concerns.

Abstract:Standard approaches to teaching the management of psychosocial issues in pediatrics--visits to community-based organizations and stand-alone block rotations in developmental-behavioral pediatrics and community pediatrics--neither expose residents to models of interdisciplinary collaboration between faculty preceptors and community providers nor take advantage of the efficacy of learning in continuity clinics. The authors describe their project, developed from an existing Community Pediatrics Training Initiative with long-standing relationships with a domestic violence shelter, a community center for Latino families, and a special needs resource organization for parents. They lay out in detail the project's innovative use of partners from community-based organizations, colocated within pediatric continuity clinics, who teach both residents and faculty about community resources and linkages with multidisciplinary providers. The authors present lessons learned by faculty preceptors, residents, the community partners, and project staff that can guide future applications of this model in other residency training programs. Faculty and residents indicated an increased awareness of available community resources and how linkages can be incorporated into pediatric outpatient visits. Community partners identified keys to successful co-teaching, including readiness to adopt an assertive communication style and frequent presence in the clinics. Project staff recognized the challenges of staff turnover at community-based organizations and the need to choose community partners with expertise that fits the sociodemographic issues of the clinic's patients.

Promoting minority access to health careers through health profession-public school partnerships: a review of the literature

Abstract:Partnerships between health profession schools and public schools provide a framework for developing comprehensive, creative solutions to the problem of minority underrepresentation in health careers. This review examines the functioning of partner relationships, focusing on elements of the social context that determine success or failure, and stages of partnership development. Influential aspects of the social context include cultural differences between personnel in higher education and K-12 institutions, the resources available to the partnership, and constraints on partnership activity. Stages of the process that partner institutions must negotiate include initiation, ongoing management, and institutionalization. Strategies to improve minority student achievement are reviewed, including specific types of programmatic interventions and best practices. Strategies available to partnerships for improving minority achievement include academic enhancement, science or math instructional enrichment, career awareness and motivation, mentoring, research apprenticeship, reward incentives, and parental involvement. Of these, academic enhancement and instructional enrichment have the greatest potential for improving minority student outcomes. Partnerships need to take a sustained multipronged approach, providing intensive interventions that target students, teachers, and curricula at appropriate educational stages. Documenting program impact is critical for attracting more resources to increase minority access to health careers: sponsoring organizations should dedicate funds for assessment of the partnership's functioning and for rigorous evaluation of interventions.

Abstract:BACKGROUND: Community participation in population health improvement can assist university researchers in targeting intervention resources more effectively and efficiently, leading to more effective implementation of interventions, because of joint ownership of both process and product. Two academic health centers partnered with community based organizations to develop a bidirectional educational seminar series called "Community Grand Rounds" (CGR), which identified health concerns of Chicago's South Side residents and provided information regarding university and community resources that addressed community health concerns.OBJECTIVES: We evaluated the community consultants' perceptions of the quality and effectiveness of the planning and implementation of the seminars that resulted from the partnership.METHODS: We conducted one-on-one interviews and focus groups with community consultants to assess their perceptions of the partnership using a tailored version of a previously developed individual and focus group interview instrument. Analysis of the interview text was conducted using grounded theory where themes were coded as they emerged.CONCLUSIONS: CGR is an effective mechanism for providing needed community health information in an easily accessible format. Additional work is needed to determine whether this format represents a sustainable community-university partnership.

Abstract:Background: Community-academic partnerships are increasingly used in interventions to address health care disparities. Little is known about motivations and perceptions of participating community members. Objectives: To elicit community members' perspectives of involvement in a community-academic partnership to address implicit bias in health care. Methods: With our partnering community organizer, we conducted one-on-one semistructured interviews and a follow-up group interview with participating community members to solicit experiences about involvement in an National Institutes of Health-funded clinician training; responses were organized using content analysis. Results: Community members revealed that their participation was motivated by trust in our community organizer; they derived personal pride from participation in clinician training; the power differential between community members and clinicians in the training environment needed to be levelled. Our community organizer noted that the benefits of community-academic partnerships propagate to the larger community via community members' experiences. Conclusions: Community members note trust, pride, and power as important elements in community-academic partnership.

Abstract:Institutions training future healthcare professionals in healthcare and community engagement play a crucial role beyond traditional classroom settings. Recognizing their potential to support under-represented groups and minorities, institutions increasingly encourage engagement with schools and community organizations. However, work remains to advance meaningful and impactful educational outreach and service-learning programs. This manuscript synthesizes the perspectives of a group of medical school educators to discuss developing sustainable programs to engage youth in Science, Technology, Engineering, Math, and Medicine (STEMM) education with a focus on biomedical science. Through near-peer education and service-learning, healthcare students can impart knowledge, provide mentorship, promote enthusiasm for STEMM fields, and nurture health-related self-efficacy within individuals and communities. Collaborative efforts through student-as-teacher approaches bridge health-related disparities and cultivate healthier, more empowered futures for all. We advocate for community outreach strategies that target future health professionals early in their education and support the scholarship of teaching and learning and program evaluation. Successful long-term programs must ensure that results are systematically assessed, measured, and perpetuated. This perspective aims to highlight the role of service learning and community outreach in increasing individual health literacy and fostering an enduring interest in STEMM careers, thereby empowering the next generation of elementary and secondary school students.
Building a Community-Academic Partnership to Expand Workforce Development in Underserved, High-Need/High-Demand Areas

Abstract:This case study highlights the partnership development between a large, urban, public, community-based behavioral health system and an academic program. Using principles of partnership building and facilitators that enhance partnership building, we describe the process of initiating, building, and sustaining the partnership. The Health Resources and Services Administration (HRSA) workforce development initiative was the primary catalyst for the partnership development. The public, community-based behavioral health system is located in an urban, medically underserved area and health care professional shortage area. The academic partner is a master in social work (MSW) program in Michigan. We assessed partnership development by using process and outcome measures that captured changes in the partnerships and in implementation of the HRSA workforce development grant. The goals of this partnership were to develop the infrastructure to support the training of MSW students, expand workforce skills in integrated behavioral health, and increase the number of MSW graduates who work with medically underserved populations. During 2018-2020, the partnership trained 70 field instructors, engaged 114 MSW students in HRSA field placements, and developed 35 community-based field sites (including 4 federally qualified health centers). The partnership provided training for field supervisors and for HRSA MSW students and developed new courses/trainings focusing on integrated behavioral health assessment/intervention practices, trauma-informed care, cultural awareness, and telebehavioral health practices. Of 57 HRSA MSW graduates who responded to a postgraduation survey, 38 (66.7%) were employed in medically underserved, high-need/high-demand urban areas. Partnership sustainability was helped by formal agreements, regular communication, and a collaborative decision-making approach.

Abstract:Background: The Temple University Fox Chase Cancer Center and Hunter College Cancer Health Disparity Partnership (TUFCCC/HC Cancer Partnership) is a comprehensive collaborative cancer health research infrastructure in Pennsylvania, New Jersey, and New York City (PNN) Region. Its goals are to enhance equity in cancer health through rigorous and sustainable cancer research, train the next generation of researchers, and establish community outreach programs. Establishing and strengthening a bidirectional academic-community partnership is a key component of this partnership. While such collaborative relationships have been strongly endorsed by the National Institutes of Health (NIH), little has been documented demonstrating how such partnership can be created and maintained. Objectives: Building on previous established collaborative relationships with the community-based organizations (CBOs), we created and expanded a bidirectional, academic–community partnership in African American, Asian American, and Latinx communities in the PNN region to (1) assess the level of awareness of cancer prevention knowledge; (2) identify community needs and barriers to cancer prevention healthcare services; (3) jointly develop and implement community-based educational initiatives to promote cancer prevention in medically underserved communities. Methods: In this presentation, we established a comprehensive evaluation metric to assess the bidirectional partnership. The metric, its outputs, and outcomes were adapted from the Evaluation Metrics Manual published by the National Institute of Environmental Health Services of NIH. We also formed a Community Advisory Board (CAB) to guide our bidirectional communication and jointly develop a community-driven cancer education agenda. Results: The CAB played a critical role in the development of the bidirectional partnership. They provided key inputs in identifying the goals and benchmarks of the educational initiative, and in facilitating the recruitment and implementation of the initiative. Our evaluation indicated that a bidirectional partnership is an interactive and non-linear process that requires constant feedback and communication. We identified several successes and challenges during our co-learning and co-implementing process. We also identified areas for improvement, specifically, establishing a more efficient channel for information and resource sharing, and greater efforts on dissemination of scientific findings to the community. Conclusions: Actively engaging CAB and CBOs in the cancer prevention educational initiatives from the onset enhanced the efficacy of the initiatives and strengthened the research training capacity of the academic institutions. We will share best practices in our bidirectional, academic–community partnership building and discuss next steps in adaptation and further development of the partnership. The Community Partnerships Experience: a report of institutional transition at East Tennessee State University

Abstract:The Community Partnerships Program, sponsored by the W. K. Kellogg Foundation, served as a catalyst for significant changes within East Tennessee State University (specifically its schools of medicine, nursing, and public and allied health) and the rural communities involved. The authors describe the development and implementation of the program and its effects on the students, faculty, communities, and the three participating schools over the period 1992-1999. They also review the changes the program fostered in health professions education and the resulting institutional changes at their university. The primary motivation for change at East Tennessee State University was the desire to develop primary care providers who could more effectively function in an interdisciplinary and interprofessional health care system and who would be sensitive to community needs in rural and underserved areas. The planning process, curricular transformation, implementation of inquiry-based learning, community collaboration, and interdisciplinary education involving students from the three health professions schools are described, including challenges and difficulties (e.g., student attrition; retention of volunteer community-based clinical preceptors; initial faculty resistance; a climate of competition rather than cooperation). Outcomes are described, including students' enrollment and attrition in the program over time, performances on the U.S. Medical Licensing Examination, program graduates' career choices, and the types and locations of their practices. The program's students performed as well on professional licensing examinations as did their peers enrolled in traditional programs. Program graduates have been much more likely to select primary care careers and to practice in rural locations than have their non-program peers. The development strategies and experience gained could give useful insights to other universities contemplating a community-based component for health professionals within their existing curricula.
The spectrum of the young star HD 100546 observed with the Infrared Space Observatory

Abstract:It is generally assumed that planets form in the dusty disks that surround young stars (Beckwith & Sargent 1996). The Infrared Space Observatory (Kessler et al. 1996) now enables us to determine the characteristics of these disks with unprecedented spectral resolution and signal-to-noise. We present here ISO spectra of the disk that surrounds the young star HD 100546. A remarkable variety of emission features of carbon- and oxygen-rich dust occurs. Most prominent are a se- ries of emission features that can be attributed to silicates in crystalline form, mostly forsterite. In the interstellar medium and HII regions the silicate dust is mostly amorphous, but crys- talline silicates are found in comets, meteorites and interplan- etary dust particles. The forsterite features of HD 100546 are astonishingly similar to those observed in the ISO spectrum of Comet Hale-Bopp (Crovisier et al. 1997), strengthening the hy- pothesis that the disk around HD 100546 contains a huge swarm of comets (Grady et al. 1997). We argue that the crystallisation process occurs during the early evolution of the circumstellar disks of young stars and speculate about the formation of an Oort cloud around HD 100546.

Abstract:Thirty men and five women aged 36-70 years with angina were questioned about their sexual activity. Twenty-nine (83%) had intercourse more than once a week and six (17%) abstained. In 19 (65%) angina occurred during intercourse, and four also complained of palpitation. Fourteen patients underwent 24-hour ECG (electrocardiogram) tape-recordings before and after beta-blockade, and indicated the time of intercourse in diary cards. Tape analysis before beta-blockade revealed an average maximal 24-hour heart rate of 124.0 +/- 7.2 beats/min, and 122.2 +/- 7.1 beats/min during intercourse. Two patients complaining of palpitation during intercourse developed supraventricular tachycardia (SVT) and two sinus tachycardia. Beta-blockade abolished the SVT and reduced the average maximal 24-hour heart rate to 82.1 +/- 6.0 beats/min (P less than 0.001), and 82.0 +/- 2.8 beats/min during intercourse (P less than 0.001). After 6 months, the 19 patients previously symptomatic were pain-free during intercourse, though six also required prophylactic sublingual nitrates. Four of the six who previously abstained were able to have a normal sex relationship. Twenty-four hour tape ECG recordings have shown that the heart rate during intercourse is similar to that during normal daily life. There is no reason why most patients with angina cannot have a normal sex life, and physicians should ask and advise routinely.
Differential clustering of sperm subpopulations in infertile males with clinical varicocele and carriers of rearranged genomes.

Abstract:Some methods for determining sperm DNA fragmentation, such as the sperm chromatin structure assay (SCSA) and the sperm chromatin dispersion test (SCD), provide additional information about particular subgroups of spermatozoa with specific irregularities. Thus, SCSA recognizes a specific sperm subpopulation, the high-DNA stainability sperm subpopulation (HDS), and SCD recognizes the so-called DNA-degraded sperm (DDS) subpopulation. Although some studies associate the presence of these subpopulations with specific aspects related to infertility, the relationship between both sperm subpopulations and their preponderance in specific clinical groups of infertile males has not been extensively investigated. In this study, HDS and DDS subpopulations were determined in a total of 37 human males: 8 males with proven fertility, 9 infertile males with asthenoteratozoospermia, 10 carriers of chromosomal reorganizations, and 10 infertile males with clinical varicocele. Results showed a significant increase of the DDS subpopulation (P < .001) in both the varicocele patient (16.85 ± 7.24) and carrier of rearranged genome (11.6 ± 5.23) groups, but not in patients with asthenoteratozoospermia (3.88 ± 1.55) or fertile donors (2.62 ± 1.68). No statistical differences were detected for the HDS subpopulation (P = .542), but the highest values were found in the varicocele and rearranged-genome groups. However, no correlation between the HDS and DDS subpopulations were found (r = 0.196; P = .244), suggesting that both represent a different class of sperm subpopulation in the ejaculate. A significant increase in HDS, and especially DDS, can be associated with the presence of varicocele or the rearrangement of chromosomes. Specific diagnostic tests to confirm the diagnosis must be performed in patients with increased DDS and HDS values.
H.O.P.E. grows: An academic-public health partnership to reimagine public health services and increase mental health access among socially vulnerable populations

Abstract:Objective: To illustrate the process of developing and sustaining an academic-public health partnership for behavioral health integration through an expansion of the Aligning Systems for Health (ASfH) framework. Study setting: Practice-informed primary data (2017-2023) from the Holistic Opportunity Program for Everyone (HOPE) Initiative based in Charlotte, NC. Study design: The unit of analysis in this descriptive case study is inter-organizational, specifically focusing on an academic-public health relationship. We illustrate the partnership process across the ASfH four core areas, including key challenges and insights. Data collection: Utilized a Critical Moments Reflection methodology and review of HOPE program data. Principal findings: (1) Formal partnership structures and processes are essential to monitoring the four ASfH core components for on-going system alignment. (2) Aligning systems for health principally involves two ecologies: (i) the health program and (ii) the partnership. The vitality and sustainability of both ecologies require continuous attention and resource investment. (3) Relationships rest at the heart of aligning systems. (4) With comparative advantages in research methods, the academic sector is especially poised to collaborate with healthcare systems and human service organizations to study, develop, implement, and scale evidence-based health interventions. Conclusions: The academic sector shares overlapping purposes with the public health, healthcare, and social services sectors while providing complementary value. It is a critical sectoral partner in advancing population health and health equity.

Abstract:African Americans continue to have worse health outcomes despite attempts to reduce health disparities. This is due, in part, to inadequate access to healthcare, but also to the health care and medical mistrust experienced by communities of color. Churches and worship centers have historically served as cultural centers of trusted resources for educational, financial, and health information within African American communities and a growing number of collaborations have developed between academic institutions and community/faith entities. Herein, we describe the infrastructure of a true and sustainable partnership developed with > 100 prominent faith leaders within the Piedmont Triad region of North Carolina for the purpose of developing or expanding existing health ministries within houses of worship, to improve health literacy and overall health long-term. The Triad Pastors Network is an asset-based partnership between the Maya Angelou Center for Health Equity at Wake Forest University School of Medicine and faith leaders in the Piedmont Triad region of North Carolina that was created under the guiding principles of community engagement to improve health equity and decrease health disparities experienced by African American communities. A partnership in which co-equality and shared governance are the core of the framework provides an effective means of achieving health-related goals in a productive and efficient manner. Faith-based partnerships are reliable approaches for improving the health literacy needed to address health disparities and inequities in communities of color.

Abstract:Two mixed-valent thioarsenates, namely (enH2)(4.5)[In(As(V)S4)3][As2(III)(μ-S2)S3]Cl (1) and (enH2)MnAs(III)As(V)S6 (2) (en = ethylenediamine), have been solvothermally synthesized and characterized. Thermal stability, magnetic and mixed-valent optical properties, as well as theoretical band structures and DOSs, of 1 and 2 have also been studied.

Abstract:Background: The Affordable Care Act mandated triennial community health needs assessments (CHNAs) for greater nonprofit hospital accountability in responding to community health needs. Over 10 years later, hospital spending on community benefits remains largely unchanged. While greater collaboration in CHNA implementation can increase hospital investment in community-based initiatives, nonprofit hospitals in conservative states are subject to policy, political, and economic factors that inhibit public health partnerships and magnify existing disparities in health care access. This participatory action research study explores the decision-making environment of collaborative CHNA implementation within a group of nonprofit hospitals in a north Texas urban county. Methods: In 2017 faculty from an urban anchor institution initiated an academic-community partnership with a coalition of nonprofit hospitals, public health departments, and academic institutions. An interdisciplinary research team engaged in multi-method document review and qualitative data collection to describe historical barriers for local CHNA processes and develop practical strategies for joint CHNA initiatives. Local CHNA documents were first reviewed through team-based content analysis and results applied to develop a qualitative study protocol. Key informants were recruited from county-based nonprofit hospitals, community-based nonprofit organizations, and public health systems. Seventeen senior- and mid-level professionals participated in semi-structured research interviews to describe their perspectives relating to CHNA-related planning and implementation decisions. Through iterative data collection and analysis, the research team explored CHNA-related knowledge, experiences, and processes. A constructivist lens was subsequently applied to examine historical barriers and future opportunities for local collaboration. Results: Findings reveal CHNA implementation is a multi-stage cyclical process in organizational environments with accountability to a wide range of public and private stakeholders. This promotes varied levels of inclusivity and conservatism in data collection and community benefit implementation. Decisions to collaborate are hindered by competing priorities, including compliance with existing guidelines, administrative simplicity, alignment with health care service delivery, and efficient resource use. Efforts to promote greater CHNA collaboration may be facilitated through intentional alignment with organizational priorities and clearly communicated benefits of participation for leaders in both public and private nonprofit health systems. Discussion: We consider implications for policymakers and health systems in restrictive political environments and advance a conceptual framework for greater CHNA collaboration.

Abstract:Community-engaged research (CEnR) is a potent tool for addressing health inequities and fostering equitable relationships among communities, researchers, and institutions. CEnR involves collaboration throughout the research process, demonstrating improvements in study recruitment and retention, intervention efficacy, program sustainability, capacity building among partners, and enhanced cultural relevance. Despite the increasing demand for CEnR, institutional policies, particularly human participation protection training (HPP), lag behind, creating institutional barriers to community partnerships. Here, we highlight challenges encountered in our ongoing study, Fostering Opportunities in Research through Messaging and Education (FOR ME), focused on promoting shared decision-making around clinical trial participation among Black women diagnosed with breast cancer. Grounded in CEnR methods, FOR ME has a partnership with a community-based organization (CBO) that addresses the needs of Black women with breast cancer. Our CBO partner attempted to obtain HPP training, which was administratively burdensome and time-consuming. As CEnR becomes more prevalent, academic and research institutions, along with researchers, are faced with a call to action to become more responsive to community partner needs. Accordingly, we present a guide to HPP training for community partners, addressing institutional barriers to community partner participation in research. This guide outlines multiple HPP training pathways for community partners, aiming to minimize institutional barriers and enhance their engagement in research with academic partners.
