Abstract:Objective To investigate the amplitude of pseudoaccommodation in eyes after multifocal intraocular lens implantation.Methods Thirty-nine cataract patients were divided into two groups.One group received multifocal intraocular lens implants and the other received monofocal intraocular lenses after phacoemulsification.Near add,best corrected near visual acuity,amplitude of pseudoaccommodation and depth of focus were measured at 3 months or later postoperatively.Results The difference in add between the two groups was not statistically significant [(1.99±0.27)DS versus(1.89±0.24)DS,P0.05].The difference in amplitude of pseudoaccommodation between the groups when near corrected was not statistically significant either [(1.20±0.36)DS versus(1.23±0.29)DS,P0.05].Depth of focus was better in the multifocal group than in the monofocal group(+1.5～-3.5 DS versus +1.0～-1.5 DS).Conclusion The difference in amplitude of pseudoaccommodation between the two groups when corrected with near and distance zones was not statistically significant.Compared to the monofocal intraocular lens,the multifocal lens can provide a wider range for depth of focus.
Visual function after aspheric multifocal intraocular lenses implantation

Abstract:Objective To evaluate visual quality and pseudoaccommodation in patients after the implantation of an apodized diffractive aspherical multifocal intraocular lens (MIOL). Methods This was a prospective case series study. Thirty-five patients (50 eyes)who had agreed to undergo phacoemulsification and accept IOL implants were selected for the study. Any other eye disease or systemic disease was criteria for exclusion. The eyes were divided into 2 groups according to the type of IOL implanted. One group was implanted with apodized diffractive aspherical MIOL (AcrySof ReSTOR SN6AD3) and the other group was implanted with apodized diffractive MIOL (AcrySof ReSTOR SN60D3). Twenty-five eyes of 17 patients were implanted with SN6AD3 and 25 eyes of 18 patients were implanted with SN60D3. All patients were assessed 3 months postoperatively for aberrations of the crystalline lens, the modulation transfer function, uncorrected distance visual acuity (UCDVA), uncorrected near visual acuity (UCNVA), best corrected distance visual acuity (BCDVA), distance corrected near visual acuity (DCNVA), pseudoaccommodation,contrast sensitivity and the rate of eyeglass removal for near. Data were compared using an independent samples t test and x2 test. Results UCDVA, BCDVA, UCNVA and DCNVA in the SN6AD3 group were not statistically different from the SN60D3 group (P＞0.05). There were statistical differences between the two groups when spherical, 4th-order higher aberrations and total higher-order aberrations were compared. The SN6AD3 group had lower aberrations than the SN60D3group under 3 and 5 mm pupils. There were statistical differences in the MTF for spatial frequencies of 5, 10, 15, 20, 25 and 30 c/d. For these spatial frequencies, the SN6AD3 group had higher scores than the SN60D3 group. Accommodative range was (0.925±0.549)D in the SN6AD3 group and (0.964 ±0.582)D in the SN60D3 group. There were no statistically significant difference in accommodative range between the SN6AD3 and SN60D3 groups (t=0.217, P＞0.05). There were statistical differences in contrast sensitivity for spatial frequencies of 3, 6, 12 and 18 c/d. At these spatial frequencies, the SN6AD3 group had better contrast sensitivity than the SN60D3 group. The rate of eyeglass removal for near for the two MIOL groups was 80% and 72% (x2=0.439, P=0.508).Conclusion The AcrySof ReSTOR apodized diffractive aspherical MIOL provides excellent distance and near visual acuity, pseudoaccommodation and higher contrast sensitivity.
Comparison of visual performance between monofocal and multifocal intraocular lenses of the same material and basic design

Abstract:To compare the visual performance of a monofocal intraocular lens (IOL) (ZCB00) and a multifocal IOL (ZMB00) of the same material and basic design, we evaluated postoperative parameters at 10 weeks after the last surgery in cataract patients who underwent bilateral ZCB00 or ZMB00 implantation from December 13, 2010, to July 29, 2019, with the right and left lenses implanted within 3 months of each other. The study enrolled 2,230 eyes of 1,115 patients. The monofocal group comprised 904 eyes of 452 patients (72.3 ± 6.8 years; females/males, 268/184), and the multifocal group comprised 1,326 eyes of 663 patients (67.0 ± 7.8 years; females/males, 518/145). Contrast sensitivity (4.0/2.5/1.6/1.0/0.7 degrees), contrast sensitivity with glare (1.6/1.0/0.7 degrees), and the VFQ-25 score for driving at night were significantly better in the monofocal group (p < 0.00068, Wald test). Uncorrected intermediate/near visual acuity and near spectacle independence were significantly better in the multifocal group (p < 0.00068, Wald test). The two IOL groups had different characteristics in terms of contrast sensitivity, night-time driving, uncorrected intermediate/near visual acuity and near spectacle independence.
Comparative analysis of visual quality after implantation of multifocal toric and monofocal toric intraocular lenses

Abstract:Objective To compare visual quality after implantation of either a multifocal toric intraocular lens (IOL) or a monofocal toric IOL.Methods This retrospective case-control study was comprised of 32 eyes of 27 patients with more than 1.0 D of preexisting corneal astigmatism.Ten eyes of 8 patients were implanted with Acrysof IQ Restor toric IOLs (SND1T group) and the other 22 eyes of 19 patients were implanted with Acrysof IQ toric IOLs (SN6AT group).Uncorrected (UDVA)and corrected (CDVA) distance visual acuities,uncorrected (UNVA) and corrected (CNVA) near visual acuities,residual astigmatism,IOL rotation,contrast sensitivity (CS),modulation transfer functions (MTF) and total eye wavefront aberrations were evaluated for 6 months postoperatively.Data were analyzed using an independent-samples t test and repeated-measures ANOVA.Results The UNVA in the SND1T group was better than that in the SN6AT group at the postoperative time points (t=11.683,8.252,7.384,P＜0.01).At 6 months postoperatively,the CS with glare at low spatial frequencies (3 c/d) in the SND1T group was significantly lower than that in the SN6AT group (t=2.086,P＜0.05).MTFs were significantly lower in the SND1T group than that in the SN6AT group with 3-mm pupils at middle spatial frequencies (5,10 c/d) (t=2.050,2.078,P＜0.05).There were no statistically significant differences in UDVA,CDVA and CNVA throughout the postoperative time points between the 2 groups.At 6 months postoperatively,the difference in corneal astigmatism between preoperative and postoperative measurements,the mean IOL axis rotation,CS without glare and with glare at residual spatial frequencies,MTF with 3-mm pupils at medium and high spatial frequencies and with 5-mm pupils at all spatial frequencies,total eye wavefront aberrations,total higher-order aberrations,spherical aberrations and coma under 3-and 5-mm pupils were not statistically significant between the 2 groups.Conclusion Compared with the monofocal toric IOL,the multifocal toric IOL can provide excellent overall visual acuity,effective astigmatic correction and favorable visual quality.

Abstract:Objective To evaluate the visual quality and pseudoaccommodation in patients after phacoemulsification combined with apodized diffractive mutifocal intraocular lens(MIOL) implantation.Methods Seventy-five eyes(54 patients) underwent phacoemulsification combined with IOL implantation were divided into aspherical apodized diffractive multifocal IOL SN6AD3 group(17 cases,25 eyes),apodized diffractive multifocal IOL SN60D3 group(18 cases,25 eyes) and aspherical monofocal IOL SN60WF group(19 cases,25 eyes).The uncorrected distanced visual acuity(UCDVA),uncorrected near visual acuity(UCNVA),best-corrected distance visual acuity(BCDVA),distance corrected near visual acuity(DCNVA),pseudoaccommodation,contrasts sensitivity at postoperative 3 months were examined,and the rate of off-near glasses was compared.Results At postoperative 3 months,there was no statistical difference among three groups in UCDVA and BCDVA(all P>0.05).UCNVA and DCNVA in SN60D3 group(0.205±0.111,0.212±0.114) and SN6AD3 group(0.192±0.120,0.250±0.130) were statistically better than that in SN60WF group(0.661±0.278,0.661±0.278)(all P<0.01),but no difference was found between SN6AD3 group and SN60D3 group(P>0.05).The contrast sensitivity in each spatial frequency with or without glare in SN60WF group and SN6AD3 group was higher than that in SN60D3 group.The accommodative range was(0.925±0.549)D in SN6AD3 group,(0.964±0.582)D in SN60D3 group and(0.390±0.235)D in SN60WF group,there was no statistical difference between SN6AD3 group and SN60D3 group(P=0.993),that in SN6AD3 group and SN60D3 group were higher than that in SN60WF group(P<0.01).The rate of off-near glass in SN6AD3 group(80%) and SN60D3 group(72%) were higher than that in SN60WF group(24%,both P<0.01).Conclusion The aspheric AcrySof ReSTOR SN6AD3 IOL can provide excellent distance and near visual acuity,higher contrast sensitivity than SN60D3,and can effectively improve the visual quality after phacoemulsification.

Abstract:Objective:To compare the one-year postoperative visual quality after trifocal intraocular lens (IOL) implantation and monofocal IOL implantation.Methods:A cohort study was conducted.Forty-one eyes from 41 age-related cataract patients who underwent phacoemulsification extraction combined with IOL implantation in Nanjing Drum Tower Hospital from May 2017 to June 2018 were enrolled.The patients were divided into trifocal IOL group (20 eyes) receiving ZEISS AT LISA tri 839MP trifocal IOL implantation and monofocal IOL group (21 eyes) receiving ZEISS 603P monofocal IOL implantation according to their willingness.One year after surgery, uncorrected distant visual acuity (UCDVA), uncorrected intermediate visual acuity (UCIVA), uncorrected near visual acuity (UCNVA), best corrected distance visual acuity (BCDVA), distance corrected intermediate visual acuity (DCIVA) and distance corrected near visual acuity (DCNVA) were detected in both groups.The patient point spread function (PSF), modulation transfer function (MTF) cutoff frequency, Strehl ratio (SR), OQAS Ⅱ values at 100%, 20%, and 9% contrast (OV 100%, OV 20%, OV 9%) and objective scattering index (OSI) were measured by OQAS Ⅱ.Wavefront aberrations including total aberration (TA), total high order aberrations (tHOAs), spherical aberration, coma, trefoil aberration, total low order aberrations (tLOAs), defocus, and astigmatism were evaluated with the iTrace visual function analyzer.All aberrations were represented by root mean square.The visual acuity of operative eyes was measured with a phoropter, and defocus curves were drawn with visual acuity better than 0.5 LogMAR.The incidence of posterior capsular opacification (PCO) in the IOL region was quantitatively analyzed by Sellman method.Visual function was scored by visual function index (VF-14). This study adhered to the Declaration of Helsinki.The study protocol was approved by an Ethics Committee of Nanjing Drum Tower Hospital, The Affiliated Hospital of Nanjing University Medical School (No.2018-219-01). Written informed consent was obtained from each subject prior to any medical examination.Results:One year after the operation, UCIVA, UCNVA, DCIVA, and DCNVA of trifocal IOL group were significantly better than those of monofocal IOL group, and the differences were statistically significant (all at P<0.001). OQAS Ⅱ visual quality indicators showed that the MTF cutoff frequency, SR, OV 100%, and OSI values of trifocal IOL group were significantly higher than those of monofocal IOL group, showing statistically significant differences (all at P<0.001). No significant difference in wavefront aberrations was found between the two groups (all at P>0.05). Defocus curve showed that the LogMAR visual acuity of patients at -1.0 D, -1.5 D, -2.0 D, -2.5 D, -3.0 D, and -3.5 D (namely, 1 m, 66 cm, 50 cm, 40 cm, 33 cm, and 29 cm) in monofocal IOL group were significantly better than those in trifocal IOL group (all at P<0.05). There was a higher incidence of PCO in trifocal IOL group than monofocal IOL group, with a statistically significant difference ( χ 2=41.0, P<0.001). The VF-14 score of trifocal IOL group was 87.99±1.09, which was significantly higher than 81.49±1.67 of monofocal IOL group ( t=10.301, P<0.001). Conclusions:One year after trifocal IOL implantation, the full range of vision, subjective and objective visual quality of eyes are better than eyes implanted with monofocal IOL.
Visual outcomes and optical quality after implantation of a diffractive multifocal toric intraocular lens

Abstract:Background: This study evaluated the visual function after implantation of a multifocal toric intraocular lenses (IOLs). Materials and methods: This study involved 10 eyes from eight cataract patients with corneal astigmatism of 1.0 diopter (D) or higher who had received phacoemulsification with implantation of an AcrySof IQ ReSTOR Toric IOL. Six-month evaluations included visual acuity, spherical equivalent (SE), defocus curve, residual astigmatism, IOL rotation, contrast sensitivity (CS), wavefront aberrations, modulation transfer function (MTF), and patient satisfaction assessments. Results: At 6 months postoperatively, uncorrected distance visual acuity (logarithm of the minimum angle of resolution) was 0.09 ± 0.04, corrected distance visual acuity was 0.02 ± 0.11, and uncorrected near visual acuity was 0.12 ± 0.07. The mean SE was -0.095 ± 0.394 D (±0.50 D in 90%). Refractive astigmatism at the 6-month follow-up visit was significantly reduced to 0.35 ± 0.32 D from 1.50 ± 0.41 D presurgery (P < 0.05). The mean IOL axis rotation was 3.20 ± 1.55°. Postoperative CS levels were high. Postoperative total order aberrations (TOAs), lower-order aberrations (LOAs), higher-order aberrations (HOAs), and spherical aberrations were decreased compared with preoperative values (P < 0.05). At 3 months postoperatively, TOAs, LOAs, and HOAs with a 3 mm pupil diameter as well as TOAs, LOAs, and astigmatism aberrations with a 5 mm pupil diameter were statistically lower than those at 1-month post surgery, but without subsequent significant changes (P > 0.05). There was an increase in MTF results between preoperative and postoperative evaluations at all spatial frequencies. Conclusions: The diffractive multifocal toric IOL is able to provide a predictable astigmatic correction with apparently outstanding levels of optical quality after implantation.
Visual Performance with Accommodating and Multifocal Intraocular Lenses.

Abstract:AIM:To compare the visual functional outcomes with accommodating and multifocal intraocular lenses (IOLs).METHODS:Our retrospective comparative study included 51 patients (60 eyes) received implantation of an accommodating IOL (Tetraflex; 16 patients, 20 eyes), a refractive multifocal IOL (ReZoom; 18 patients, 20 eyes), or a diffractive multifocal IOL (ZMA00; 17 patients, 20 eyes). Subjective refraction, visual acuity, contrast sensitivity (CS), intraocular aberration, and subjective photic phenomena were detected at 3mo after surgery.RESULTS:The spherical equivalent in the three groups was -0.38±0.54 D, 0.14±0.56 D, and 0.35±0.41 D, respectively. No statistically significant differences were found in uncorrected and corrected distance visual acuity and uncorrected intermediate visual acuity among the groups (P=0.39). The ReZoom group had significantly better distance-corrected intermediate visual acuity than the ZMA00 group (P=0.003). The ZMA00 group had significantly better near visual acuity than the other groups (P<0.05). Better contrast sensitivity values were observed in the Tetraflex group under most of the spatial frequencies conditions (P=0.025). The total aberration was lowest in the ZMA00 group (P=0.000), and the spherical aberration was highest in the Tetraflex group (P=0.000). The three groups had similar frequency of ghosting and glare, and the Tetraflex group had a low rate of halos (P=0.01).CONCLUSION:Both accommodating and multifocal IOLs can successfully restore distance and uncorrected intermediate visual acuities. Tetraflex accommodating IOLs perform better in CS and with less halos of photic phenomena. ReZoom refractive multifocal IOLs have better performance in distance-corrected intermediate visual acuity than ZMA00 diffractive multifocal IOLs, and the latter achieved better near visual acuity and efficiently decreased the optical aberration.
Oxidative stress as a mechanism of chronic cadmium-induced hepatotoxicity and renal toxicity and protection by antioxidants.

Abstract:The role of oxidative stress in chronic cadmium (Cd) toxicity and its prevention by cotreatment with antioxidants was investigated. Adult female Sprague-Dawley rats were injected sc with 5 micromol CdCl2/kg/day, 5 times a week, for up to 22 weeks. Serum alanine amino transferase and lactate dehydrogenase activities were elevated after 9 weeks of Cd administration, indicating hepatic damage. Renal toxicity, indicated by elevation in urinary lactate dehydrogenase activity and protein, was also observed around this time. Chronic Cd administration resulted in a gradual rise in hepatic as well as renal cortex glutathione levels. In spite of this, lipid peroxidation increased in both tissues, particularly during the second half of the Cd exposure period. Depletion of glutathione following buthionine sulfoximine administration at the end of Week 5, or inhibition of catalase by aminotriazole at the end of Week 7, resulted in the development of acute nephrotoxicity within 6 h. Coadministration of antioxidants, N-acetylcysteine (50-100 mg/kg, sc), or vitamin E (100-150 mg/kg, sc) with Cd, starting from the early phases of Cd exposure, controlled Cd-induced lipid peroxidation and protected the animals against hepatic as well as renal toxicity. A Japanese hepatoprotective drug, Stronger Neo-Minophagen C, containing glycyrrhizin, glycine, and cysteine, was also effective in reducing the chronic Cd nephrotoxicity. In conclusion, oxidative stress appears to play a major role in chronic Cd-induced hepatic and renal toxicity since inhibition of components of the antioxidant defense system accelerated and administration of antioxidants protected against Cd toxicity.
[The Study of Long-Term Curative Effect of Array Multifocal Intraocular Lens after Phacoemulsification].

Abstract:PURPOSE To compare the long-term curative effect of multifocal intraocular lens Array (AMO) with conventional monofocal intraocular lens AcrySof (Alcon) on functional visual performance, and to evaluate the safety and the effectiveness of the multifocal intraocular lens. METHODS 30 cases (60 eyes) after the phacoemulsification with IOL implantation were included. Patients presented for cataract surgery were randomized to receive either Array (AMO) intraocular lens or the AcrySof (Alcon). 15 cases (30 eyes) of multifocal intraocular lens implantation were enrolled at investigational sites. 15 cases (30 eyes) of monofocal intraocular lens implantation were enrolled in a control ground. All the patients were followed up for more than 3 months post operatively. Patients were examined uncorrected distant visual acuity (UCDVA), best corrected distant visual acuity (BCDVA), uncorrected near visual acuity (UCNVA), best corrected near visual acuity (BCNVA), contrast sensitivity (CS) in the best corrected. Contrast sensitivity testing served as the principal outcome measure. All of these people with ocular pathology other than other disease known to affect contrast sensitivity were excluded. All of these people were 50-80 years old. The contrast sensitivity function was measured by one person. RESULTS There was no significant difference about BCDVA and BCNVA between two groups (P > 0.05). UCNVA and UCDVA in multifocal intraocular lens group were better than control group (P < 0.05). Postoperative contrast sensitivity testing revealed no statistical significance in 3CPD, 6CPD, 12CPD between two groups (P > 0.05). In 18 CPD, multifocal intraocular lens group were worse than control group (P < 0.05). CONCLUSIONS Long term curative effect shows that multifocal intraocular lens can offer better visual sight.Contrast sensitivity in two groups has no difference except in 18 CPD.
Comparison of Accommodation after Implantation of Intraocular Lenses: Accommodative and Monofocal

Abstract:Objective To compare the accommodation of accommodative and monofocal intraocular lens (IOL),in.order to solve the problem of deficiency after the cataract surgery.Methods A total of 51 patients (69 eyes) who received cataract surgery were recruited.According to the type of IOL,patients were divided into two groups:accommodative IOL group (group A) and monofocal IOL group (group B).The uncorrected and best-corrected distance visual acuities (UCDVA and BCDVA),uncorrected and best-corrected near visual acuities (UCNVA and BCNVA),and amplitude of accommodation were measured at postoperative 3 months.Results Differences in UCDVA,BCDVA and BCNVA among the two groups were not significant.Patients in group A exhibited a better UCNVA and accommodation compared to patients in group B at postoperative 3 months (P ＜0.01).Conclusions All the two types of IOL achieve great distance visual acuity;however,accommodative IOL produce better UCNVA and accommodation.
Preliminary Clinical Study of the Apodized Diffractive Multifocal Intraocular Lens

Abstract:Objective To evaluate the AcrySof ReSTOR mulfifocal intraocular lens (Alcon Laborato-ries ) and compare it with Acrysof Natural (the monofocal IOL, Alcon Laboratories). Methods Thirty-one pa-tients with age-related cataract (38 eyes) were divided into two groups: MIOL, 12 patients (15 eyes) and MoIOL, 19 patients (23 eyes). Distant and near visual acuities, depth of focus, high order wave front aberra-tions and contrast sensitivity were observed at 1 week, 1 month and 3 months postoperatively. Results At 3 months postoperative,in MIOL group,the mean UCDVA,BCDVA,UCNVA,BCNVA and BDNVA were 0.87±0.11,0.91±0.10,0.70±0.14,0.71±0.24,0.72±0.11 respectively. While in MoIOL group,they were 0.85±0.11,0.88±0.12,0.31±0.07,0.63±0.16,0.244±0.08,respoctively. The UCNVA were significantly better than that of MolOL. Conelusion Acrysof ReSTOR IOL can provide excellent outcomes both in distant and near visions and it may reduce the dependence on spectacles in near vision and improve the quality-of-life.

Abstract:Objective To evaluate the visual quality of patients with cataract and myopia after phacoemulsification and multifocal intraocular lens (MIOL) implantation. Methods This study was comprised of 34 eyes with myopia and cataract that had undergone small-incision phacoemulsification. Fourteen of the 34 eyes received MIOLs (Array SA-40N,AMO) and twenty eyes were implanted with monofocal or sing-focal intraocular lens (SIOL,SA60AT,Alcon). Six months after surgery, distance visual acuity, near visual acuity and contrast sensitivity (CS) were measured. High-order aberrations were measured using a Hartmann-Shack aberrometer (WASCA Analyzer,Zeiss). The decentration and tilt of the IOLs were quantified using a Scheimpflug videophotography system (Pentacam,Oculus) and image analysis. Results Six months after surgery, 64.29% of the patients in the MIOL group had uncorrected near visual acuity better than 4.7 and 85.71% of the patients had distance corrected near visual acuity better than 4.7, which were significantly better than those in the SIOL group. CS and glare CS values did not differ significantly between the two groups for the low, intermediate and high spatial frequencies tested (P0.05). For a 5 mm aperture diameter, the spherical aberration was significantly lower in the MIOL group than in the monofocal IOL group (P0.05) and other high-order aberrations showed no statistical differences (P0.05). The extent of IOL decentration and tilt did not show significant differences (P0.05). Conclusion For patients with cataract and myopia, MIOLs can provide a full range of vision with good visual quality, thus greater independence from glasses can be achieved. Eyes receiving MIOLs have relatively normal CS and glare CS values and fewer spherical aberrations compared to those implanted with SIOLs.
Wavefront aberration and accommodation power in eyes with aspheric multifocal intraocular lens implantation

Abstract:Background: The implantation of multifocal intraocular lenses (MIOLs) after cataract extraction can improved the distance and near vision simultaneously. Recently, the research on MIOLs maily focus at the visual quality after implantation of MIOLs. Objective: Present study was to evaluate the visual quality and pseudoaccommodation power in the patients with the implantation of an apodized diffractive mutifocal intraocular lens (MIOL). Methods: Fifty eyes (25 patients age 60-70) received phacoemulsification + intraocular lens (IOL) implantation were included in this study. The patients were randomly divided into apodized diffractive multifocal IOL group and aspherical monofocal IOL group. Acrysoft MIOL and Acrysoft SIOL were used respectively in these two groups. The wavefront aberrations of crystalline, modulation transfer function, uncorrected distanced visual acuity (UCDVA), uncorrected near visual acuity (UCNVA), best-corrected distance visual acuity (BCDVA), best-corrected near visual acuity (BCNVA), distance corrected near visual acuity (DCNVA), pseudoaccommodation and the rate of off-near glasses were studied in 3 months following phacoemulsification + intraocular lens (IOL) implantation. Written informed consent was obtained from each patient prior to any medical procedure. Results: UCDVA and BCDVA in the Acrysoft MIOL group were not statistically significant different from those of Acrysoft SIOL group (P > 0.05), however, the UCNVA and DCNVA were significantly increased in Acrysoft MIOL group compared to Acrysoft SIOL group (P < 0.05). No statistical differences were found in spherical aberration, 4th order high aberration and whole high-order aberrations between Acrysoft MIOL group and Acrysoft SIOL group (P > 0.05). There was no statistically dignificant difference in the MTF in the spacial frequencies of 5, 10, 15, 20, 25 and 30 cycle/degree between two groups (P > 0.05). The accommodative range was 0.925 ± 0.549 D in Acrysoft MIOL group and 0.390 ± 0.235 D in Acrysoft SIOL group, showing a considerably difference between them (t = 4.476, P = 0.000). The rate of off-near glass in Acrysoft MIOL group was higher than in Acrysoft SIOL group (P = 0.000). Conclusion: The Acrysof ReSTOR apodized diffractive MIOL can offer excellent distance and near visual acuity, better accommodation and improved quality of life for the patients after phacoemulsification.

Abstract:Purpose: To compare the visual outcomes and optical quality of 2 presbyopia-correcting intraocular lenses (IOLs) with those of a monofocal IOL. Settings: Eye Center, the Second Affiliated Hospital of Zhejiang University, School of Medicine, Hangzhou, Zhejiang, China. Design: Prospective cohort study. Methods: The study included patients who had cataract surgery and were implanted with a Tecnis Symfony Extended Range of Vision (EROV) IOL (ZXR00), a zonal refractive multifocal IOL (Lentis Comfort LS-313 MF15), or a monofocal IOL (Lentis L-313). Postoperative examinations took place at 1 week, 1 month, and 3 months and included visual acuity at far, intermediate, and near distances, defocus curves, contrast sensitivity, wavefront aberrations, and modulation transfer function (MTF). Patients completed the Visual Function Index questionnaire (VF-14), the Quality of Vision questionnaire (QoV), and a visual quality self-evaluation. Results: One hundred thirteen patients were enrolled. The EROV and multifocal IOLs achieved a significantly better range of intermediate vergences (P < .05), better distance-corrected intermediate visual acuity (P <= .001), higher VF-14 (P < .05) and visual quality self-evaluation scores (P < .05) than the monofocal IOL, but there were no significant differences between the 2 presbyopia-correcting IOLs. The EROV provided lower total wavefront aberrations and better MTF than the multifocal and the monofocal IOLs (P < .05) but demonstrated a worse QoV score (P < .05), especially for severity of halo (P < .01) and starburst (P < .05) symptoms. Conclusions: Both the Tecnis Symfony ZXR00 and the Lentis Comfort LS-313 MF15 offered excellent visual restoration and stable distance and intermediate visual acuity, good subjective visual function, and good contrast sensitivity. Abstract:This study compares the optical quality of zonal refractive multifocal and diffractive bifocal intraocular lenses (IOLs) and visual outcomes after implantation in 93 patients(93 eyes) with cataracts and high myopia. A retrospective analysis was conducted to measure visual acuity at far, intermediate, and near distances, defocus curves, wavefront aberrations, modulation transfer function (MTF), a Quality of Vision questionnaire (QoV) and visual quality self-evaluation were examined at 3 months. The results demonstrated the ZMB group showed better uncorrected near visual acuity ( P < 0.01), and the MF15 group showed better uncorrected intermediate visual acuity ( P < 0.001). The ZMB group provided better MTF than the MF15 group ( P < 0.01) but demonstrated a worse QoV score, especially for the severity of glare, halo, and starburst ( P < 0.05). The satisfaction score and spectacle independence rate in the ZMB group were higher than in the MF15 group ( P < 0.01). LS-313MF15 and TecnisZMB00 offered excellent visual restoration, stable distance visual acuity, and good subjective visual function.

Abstract:Purpose: The aim of this study was to evaluate visual, refractive and patient satisfaction outcomes following implantation of a trifocal diffractive intraocular lens (IOL).Patients and methods: This prospective, consecutive study included patients undergoing lens phacoemulsification of cataract and implantation of a trifocal diffractive IOL (AT LISA tri 839MP). Visual outcomes, including near, intermediate and distance visual acuity (VA), refractive error, contrast sensitivity, defocus curve and patient satisfaction were assessed preoperatively and postoperatively.Results: Thirty IOLs were implanted in 26 patients. Distance VA improved significantly from 0.70 +/- 0.45 to 0.08 +/- 0.11 logMAR (p<0.0001) 1 week postoperatively, and to 0.07 +/- 0.13 logMAR (p<0.0001) at 1 month and 0.05 +/- 0.10 logMAR (p<0.0001) at 3 months. Uncorrected near and intermediate VA, as well as corrected near, intermediate and distance VA, were stable and maintained during the follow-up period. Preoperative anterior chamber depth demonstrated an association with effective adjustment of postoperative spherical equivalent using a regression formula (p=0.007). No significant differences were observed for VA at defocus curves of 0 to -3 D. Contrast sensitivity at each spatial frequency improved significantly at 1 week, 1 month and 3 months under photopic and photopic with glare conditions. Under mesopic and mesopic with glare conditions, significant differences were observed postoperatively at low and medium spatial frequencies. Patients reported a high level of satisfaction and an absence of glare or halo 3 months postoperatively.Conclusion: In this study, the trifocal diffractive IOL provided excellent visual performance at all distances and improved contrast sensitivity under different conditions, resulting in high levels of patient satisfaction and spectacle independence in Chinese patients.

Abstract:PURPOSE:To compare visual function in pseudophakic patients with bilateral implantation of Tecnis multifocal aspheric and conventional monofocal spherical intraocular lenses (IOL).METHODS:A prospective study of 100 consecutive cases (200 eyes) was conducted. All cataract patients underwent phacoemulsification were randomized to receive multifocal aspheric IOL (Tecnis ZM900, AMO, multifocal aspheric group) or conventional spherical IOL (Akreos Adapt, Bausch & Lomb, monofocal spherical group). The following investigations were performed to assess the uncorrected and distance-corrected visual acuity of distance, intermediate and near distances, accommodative amplitude, spherical aberrations of total eye, contrast sensitivity, glare sensitivity and near stereoacuity. Patients were surveyed for visual disturbances and lifestyle visual quality. The independent-samples t test was used to compare the measure data which met normal distribution and the Mann-Whitney U test was used to compare the measure data which didn't meet. The chi-square test was applied to compare categorical variables.RESULTS:The uncorrected and distance-corrected bilateral visual acuity of multifocal aspheric group at 30 cm and 40 cm were 0.24 ± 0.12 and 0.22 ± 0.11 (logMAR), better than monofocal spherical group (Z = -8.261, P = 0.000; Z = -5.508, P = 0.000), but the visual acuity at other distances had no statistical difference between two groups. Patients with multifocal aspheric IOL had significantly higher accommodative amplitude than those with monofocal spherical IOL, improved about 2.3 ∼ 2.8 D (Z = -10.655, P = 0.000; Z = -2.709, P = 0.007). Mean spherical aberration of multifocal aspheric group was (0.027 ± 0.160) µm and (0.006 ± 0.083) µm, significantly lower than that of monofocal spherical group (0.269 ± 0.161) µm, (0.037 ± 0.205) µm at 5 mm and 3 mm pupil diameter (Z = -8.815, P = 0.000; Z = -2.791, P = 0.005). The difference of contrast sensitivity was not significant, but glare sensitivity was higher for monofocal spherical group than for multifocal aspheric group. Multifocal aspheric group showed statistically better uncorrected stereoacuity (72.4 ± 29.9)″ than monofocal spherical group (92.8 ± 35.7)″ (Z = -3.089, P = 0.002).CONCLUSIONS:The present clinical results demonstrated that Tecnis multifocal aspheric group had better near visual acuity, accommodative amplitude and near stereoacuity as compared to conventional monofocal spherical group. The aspheric design reduced spherical aberration of total eye and improved contrast sensitivity in some way.
Monovision versus multifocality for presbyopia during primary phacoemulsification: systematic review and network meta-analysis

Abstract:This systematic review and network meta-analysis (NMA) focuses on comparing monovision and bilateral multifocal lenses (bMFIOL) implantation methods in treating presbyopia, a common age-related condition often seen in post-cataract surgery patients. This review focuses on evaluating the efficacy, user satisfaction and limitations of these two approaches through direct or indirect comparison. A comprehensive search was conducted in Medline, Embase, and Cochrane library, considering studies up to 15 July 2022, with direct or indirect comparisons between any monovision and bMFIOL implantation with bilateral cataract surgery. The study protocol has been published in the International Prospective Register of Systematic Reviews (PROSPERO, CRD42022340257). Thirty-two studies (3082 patients) were included in the NMA for the primary outcome, complete post-operative spectacle independence. NMA showed monovision to be inferior to bMFIOLs, as bMFIOL was more likely to provide complete spectacle independence (RR = 2.06, 95% CI = 1.34 to 3.15, p = 0.002) compared to monovision. Monovision resulted in less glare compared to bMFIOL (RR = 0.343, 95% CI = 0.181 to 0.651, p = 0.001). There was no statistically significant difference between monovision and bMFIOL for binocular unadjusted distance visual acuity (MD = 70.01, 95% CI = −19.88 to 4.60, p = 0.437) and binocular unadjusted near visual acuity (MD = 5.46, 95% CI = −5.24 to 10.94, p = 0.191). When compared to monovision, bMFIOL provided greater complete spectacle independence at the expense of greater glare. This study was limited by data heterogeneity, and further studies with standardized reporting would be useful.
