Abstract:OBJECTIVETo investigate the long-term prognosis of neonates with necrotizing enterocolitis (NEC).METHODSA total of 83 preterm infants with NEC who survived and were discharged between December 2014 and September 2016 were enrolled and divided into surgery group (n=57) and non-surgery group (n=26). There were 0, 33 and 24 cases of stage I, II and III NEC respectively in the surgery group and 7, 19 and 0 cases respectively in the non-surgery group. The physical development and neurodevelopmental outcomes of the infants were followed up after discharge.RESULTSOf the 83 infants, the mean corrected age at the end of follow-up was 21±6 months. Of the 83 infants, 31 (37%) had subnormal body weight, and the surgery group had a higher rate of subnormal body weight than the non-surgery group (P<0.05). Twenty-two infants (27%) had subnormal body length and 14 children (17%) had subnormal head circumference among the 83 infants. Eighteen infants (22%) had motor developmental delay/developmental disorders, and the surgery group had a higher incidence rate of the disorders than the non-surgery group (28% vs 8%; P<0.05). Five infants (6%) were diagnosed with cerebral palsy, among whom 4 were in the surgery group and 1 was in the non-surgery group.CONCLUSIONSLong-term physical development and neurodevelopmental outcomes may be adversely affected in neonates with NEC, in particular in those with severe conditions who need surgical treatment, suggesting that long-term follow-up should be performed for neonates with NEC.

Abstract:Necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) are severe gastrointestinal complications of prematurity. The clinical presentation and treatment of NEC and SIP (peritoneal drain vs laparotomy) can overlap; however, the pathogenesis is distinct. Therefore, a patient initially treated for SIP can subsequently develop NEC. This phenomenon has only been described in case reports, and no risk factor evaluation exists. We evaluate clinical characteristics, risk factors, and outcomes of patients treated for a distinct episode of NEC after SIP.

Comparing healthcare needs in extremely low birth weight infants with NEC and spontaneous intestinal perforation

Abstract:Background Necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) affect 6-8% of extremely low birth weight (ELBW) infants. SIP has lower mortality than NEC, but with similar short-term morbidity in length of stay, growth failure, and supplemental oxygen requirements. Comparative long-term neurodevelopmental outcomes have not been clarified. Methods Data were prospectively collected from 59 North American neonatal units, regarding ELBW infants (401-1000g or 22-27 weeks gestational age) born between 2011-2018 and evaluated again at 16-26 months corrected age. Outcomes were collected from infants with laparotomy-confirmed NEC, laparotomy-confirmed SIP, and those without NEC or SIP. The primary outcome was severe neurodevelopmental disability. Secondary outcomes were weight <10th percentile, medical readmission, post-discharge surgery and medical support at home. Adjusted risk ratios (ARR) were calculated. Results Of 13,673 ELBW infants, 6,391 (47%) were followed including 93 of 232 (40%) with NEC and 100 of 235 (42%) with SIP. There were no statistically significant differences in adjusted risk of any outcomes when directly comparing NEC to SIP (ARR 2.35; 95% CI 0.89, 6.26). However, infants with NEC had greater risk of severe neurodevelopmental disability (ARR 1.43;1.09-1.86), rehospitalization (ARR 1.46;1.17-1.82), and post-discharge surgery (ARR 1.82;1.48-2.23) compared to infants without NEC or SIP. Infants with SIP only had greater risk of post-discharge surgery (ARR 1.64;1.34-2.00) compared to infants without NEC or SIP. Conclusions ELBW infants with NEC had significantly increased risk of severe neurodevelopmental disability and post-discharge healthcare needs, consistent with prior literature. We now know infants with SIP also have increased healthcare needs. Levels of Evidence Level II

Abstract:Objective: To investigate the clinical characteristics of children with early-onset necrotizing enterocolitis (NEC) undergoing enterostomy and analyze the risk factors for postoperative complications. Methods: Retrospective analysis was conducted on the clinical data (perinatal conditions, clinical characteristics, clinical outcomes, etc.) of NEC patients who underwent enterostomy at Beijing Children's Hospital from May 2016 to May 2023. The patients were divided into two groups based on the age of onset: an early-onset enterostomy group (<14 days) and a late-onset enterostomy group (≥14 days). Furthermore, the children with NEC were categorized into complication group and non-complication group based on whether there were complications after enterostomy. The differences in clinical data between these groups were analyzed, and the clinical characteristics of children with early-onset NEC and enterostomy were summarized. Multivariate logistic regression model was employed to analyze the risk factors for postoperative complications in NEC children with enterostomy. Results: A total of 68 cases were enrolled, including 43 cases in the early-onset enterostomy group [26 males and 17 females, aged (6.5±3.0) days] and 25 cases in the late-onset enterostomy group [15 males and 10 females, aged (21.0±3.0) days]. There were 28 cases (17 males and 11 females), age [M (Q1, Q3)] 9 (5, 14) days in the complication group and 33 cases (22 males and 11 females), aged of 14 (6, 21) days in the non-complication group. Compared to the late-onset enterostomy group, the early-onset enterostomy group had significantly higher rates of intraventricular hemorrhage [30.2% (13/43) vs 8.0% (2/25)], hemodynamically significant patent ductus arteriosus [37.2% (16/43) vs 12.0% (3/25)], mechanical ventilation≥72 hours after birth [39.5% (17/43) vs 16.0% (4/25)], stage Ⅲ NEC [(69.8% (30/43) vs 40.0% (10/25)], extensive NEC [27.9% (12/43) vs 8.0% (2/25)], and short-term postoperative complications [56.8% (21/37) vs 29.2% (7/24)] (all P<0.05).Multivariate logistic regression model analysis revealed that residual length of proximal small intestine was a protective factor for postoperative complications after enterostomy in NEC infants (OR=0.764, 95%CI: 0.648-0.901, P=0.001), but stage Ⅲ NEC was a risk factor (OR=1.042, 95%CI: 1.004-5.585, P=0.017). Conclusions: The incidence of postoperative complications is high, and the prognosis is poor in children with early-onset NEC enterostomy. The residual length of proximal enterostomy is a protective factor for postoperative complications of NEC enterostomy, but stage Ⅲ NEC is a risk factor.
Clinical utilization of intestinal pathology in the classification of NEC vs SIP cases and prognostication

Abstract:Despite advances in neonatal care, necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) remain leading reasons for surgical intervention, increased morbidity, mortality, and increased economic burden in preterm neonates [1]. Both NEC and SIP have similar clinical management involving drain placement or laparotomy. A recent meta-analysis found that mortality and neurodevelopment outcomes were disparate among the studies of NEC and SIP [2]. Most studies, however, fail to include the histopathological criteria for SIP diagnosis. With gross examination, the diagnosis of NEC or SIP becomes challenging. The lack of histopathological confirmation not only leads to inaccurate diagnoses within the medical record but also errors in the reporting of co-morbidities, clinical associations, and outcomes. The recent study of 125 infants by Keane et al. [3] investigated the accuracy of preoperative and intraoperative diagnoses compared to the pathologic diagnosis of SIP and NEC. They reported preoperative and intraoperative diagnoses agreement in 90.3%, intraoperative and pathologic agreement in 71.1%, and preoperative and pathologic agreement in 75.2% of patients with a Fleiss' Kappa of 0.55 (95% CI:0.43,0.68), indicating moderate agreement between the diagnoses. Unfortunately, the authors did not evaluate the individual agreements between the SIP and NEC cases. By comparison, in our recent study [4] of 151 cases, of the 19 cases labeled as SIP by the surgery team, only 3 cases (15%) met histological criteria, indicating poor agreement between the intraoperative and postoperative pathological diagnoses. Likewise, of 132 cases labeled as surgical NEC intraoperatively, 34 (25%) met the pathologic criteria for SIP criteria, and 98 (75%) had the pathologic diagnosis for NEC. These cohort studies demonstrate good agreement between pathological and the preoperative/intraoperative diagnosis when the presumed preoperative diagnosis is NEC. The poor agreement between pathological and preoperative/intraoperative diagnoses observed in SIP cases is of clinical relevance as many such infants require later surgical intervention, raising the risk of systemic morbidity and mortality.

Clinical Characteristics and Surgical Treatment of Intestinal Stenosis after Necrotizing Enterocolitis in Neonates

Abstract:Objective To explore the clinical features and surgical treatment of intestinal stenosis after necrotizing enterocolitis (NEC) in neonates.Methods The clinical data were retrospectively analyzed for patients diagnosed as NEC-related intestinal stenosis from 2011 to 2016.Results A total of 107 children were enrolled and there were 68 boys and 39 girls.Sixty-seven cases suffered intestinal stenosis after conservative treatment.The major symptom of intestinal obstruction was feeding intolerance.Radiology indicated flatulence or low ileum obstruction.Colonic stenosis was detected with colonography in some patients.Forty children undergoing ostomy had bowel stenosis during routine exploration of distal bowel and had no symptoms associated with intestinal stenosis.All but 2 cases underwent staged operations with resection of strictures and primary end-to-end anastomosis.Among 31 children with patent ductus arteriosus (PDA),4 cases underwent intraoperative PDA ligation.One PDA case died postoperatively of pulmonary fibrosis without ligation and there were 2 cases of postoperative incision dehiscence.The remainder recovered well.Conclusions Feeding intolerance of patients with NEC after conservative treatment may raise an alert of bowel stenosis.An optimal prognosis may be achieved with primary end-to-end anastomosis.Staged operation is feasible for children with severe illness.During stage Ⅱ surgery for NEC children with stage Ⅰ ostomy,attention should be paid to exploring distal bowel and watching for secondary intestinal stenosis.Concomitant malformations can be synchronized intraoperatively.
Alteration of the gut microbiota after surgery in preterm infants with necrotizing enterocolitis

Abstract:Purpose: To investigate the dynamic changes in the intestinal microbiota in preterm infants with necrotizing enterocolitis (NEC) before and after treatment via a prospective case-control study. Methods: Preterm infants with NEC and preterm infants with similar age and weight (control group) were enrolled in this study. They were divided into NEC_Onset (diagnosis time), NEC_Refeed (refeed time), NEC_FullEn (full enteral nutrition time), Control_Onset, and Control_FullEn groups according to the time of the fecal material collected. Except for basic clinical information, fecal specimens of the infants were obtained as well at indicated times for 16S rRNA gene sequencing. All infants were followed up after discharge from the NICU, and the growth data of the corrected age of 12 months were acquired from the electronic outpatient system and telephonic interviews. Results: A total of 13 infants with NEC and 15 control infants were enrolled. A gut microbiota analysis showed that the Shannon and Simpson indices were lower in the NEC_FullEn group than in the Control_FullEn group ( p < .05). Methylobacterium, Clostridium_butyricum, and Acidobacteria were more abundant in infants with NEC during diagnosis. Methylobacterium and Acidobacteria were remained plentiful in the NEC group until the end of treatment. These bacteria species were significantly positively correlated with CRP and negatively correlated with platelet count. The rate of delayed growth was higher in the NEC group than in the control group (25% vs. 7.1%) at 12 months of corrected age, but there was no significant difference. In addition, the pathways of synthesis and degradation of ketone bodies were more active in the NEC subgroups, including both the NEC_Onset group and the NEC_FullEn group. The pathway of sphingolipid metabolism was more active in the Control_FullEn group. Conclusion: Even after reaching the full enteral nutrition period, alpha diversity in infants with NEC who underwent surgery was lower than that in the control group infants. It may take more time to reestablish the normal gut flora of NEC infants after surgery. The pathways of the synthesis and degradation of ketone bodies and sphingolipid metabolism might be related to the pathogenesis of NEC and physical development after the occurrence of NEC.

Twelve years of experience in the treatment of newborns with intrauterine gastrointestinal perforation

Abstract:Background: Meconium peritonitis occurs when meconium leaks into the peritoneal cavity as a result of intrauterine gastrointestinal perforation. In this study, we aimed to evaluate the results of newborn patients who were followed and treated due to intrauterine gastrointestinal perforation in the pediatric surgery clinic. Methods: All newborn patients who were followed up and treated for intrauterine gastrointestinal perforation in our clinic between December 2009-2021 were analyzed retrospectively. Newborns who had no congenital gastrointestinal perforation were not included in our study. The data were analyzed using NCSS (Number Cruncher Statistical System) 2020 Statistical Software. Results: Within twelve years, intrauterine gastrointestinal perforation was detected in 41 newborns, including 26 (63.4%) males, and 15 (36.6%) patients who were operated on in our pediatric surgery clinic. Surgical findings of 41 patients diagnosed with intrauterine gastrointestinal perforation revealed the presence of volvulus (n=21), meconium pseudocyst (n=18), jejunoileal atresia (n=17), malrotation-malfixation anomaly (n=6), volvulus due to internal hernia (n=6), Meckel`s diverticulum (n=2), gastroschisis (n=2), perforated appendicitis (n=1), anal atresia (n=1), and gastric perforation (n=1). Eleven patients (26.8%) died. Total intubation time was significantly higher in deceased cases. Postoperatively, deceased cases passed their first stool significantly earlier than surviving newborns. Besides, ileal perforation was seen significantly more frequently in deceased cases. However, the frequency of jejunoileal atresia was significantly lower in the deceased patients. Conclusions: Although sepsis has been held primarily responsible for the deaths in these infants from past to present, insufficiency in lung capacity necessitating intubation negatively affects their survival. Early passage of stool is not always an indicator of good prognosis after the operation, and patients may die due to malnutrition and dehydration, even after they are discharged after feeding, defecating and having weight gain.
Necrotizing enterocolitis (NEC)--mortality and long-term results

Abstract:From 1974-88 73 patients were treated for necrotizing enterocolitis (NEC) in the surgical department at the Children's Hospital of Cologne. 70% of these had other complaints and 77% were premature babies, of which 30% were small-for-dates. A greater than expected proportion of deaths occurred in babies not of low birth-weight: Of those above 2500 g 35% died and of those under 1500 g 20%. Initial signs and symptoms became apparent in 80% within the first two weeks. The most frequent signs were distended abdomen (81%), diffuse tenderness (46%), bloody stools (39%) and absent peristalsis (39%). Radiology showed edema of the bowel wall in 49%, free air in the abdomen in 38% and intestinal pneumatosis in 34%. It remains questionable whether peritoneal lavage could lead to earlier diagnosis and referral of the sick children to the hospital. The course of the disease was less severe when babies had received breast milk. Colon and distal ileum were involved most frequently. Laparotomy showed perforations of the bowel in 39 patients (64%), inflammatory alterations in 18 and necroses in 12. The survival rates improved within the period of 1974-88 from 64 to 79%. Thirty-two of the 51 survivors were available for follow-up: 9 patients (28%) had more than 4 stools/day, 8 (25%) were underweight, 8 (25%) suffered from food intolerance, 5 (16%) from diarrhoea, 4 (13%) from limited exercise tolerance. Physical retardation could only be seen in 3 children under the age of 2.
Clinical Predictors of Spontaneous Intestinal Perforation vs Necrotizing Enterocolitis in Extremely and Very Low Birth Weight Neonates

Abstract:Purpose: Spontaneous intestinal perforation (SIP) and necrotizing enterocolitis (NEC) are distinct disease processes associated with significant morbidity and mortality. Initial treatment, laparotomy (LP) versus peritoneal drainage (PD), is disease specific however it can be difficult to distinguish these diagnoses preoperatively. We investigated clinical characteristics associated with each diagnosis and constructed a scoring algorithm for accurate preoperative diagnosis. Methods: A cohort of extreme and very low birth weight (<1500 g) neonates surgically treated for SIP or NEC between 07/2004-09/2022 were reviewed. Clinical characteristics included gestational age (GA), birth weight (BW), feeding history, physical exam, and laboratory/radiological findings. Intraoperative diagnosis was used to determine SIP vs NEC. Pre-drain diagnosis was used for patients treated with PD only. Results: 338 neonates were managed for SIP (n = 269, 79.6%) vs NEC (n = 69, 20.4%). PD was definitive treatment in 146 (43.2%) patients and 75 (22.2%) patients were treated with upfront LP. Characteristics associated with SIP included younger GA, younger age at initial laparotomy or drainage (ALD), and history of trophic or no feeds. Multivariate logistic regression determined pneumatosis, abdominal wall erythema, higher ALD and history of feeds to be highly predictive of NEC. A 0-8-point scale was designed based on these characteristics with the area under the receiver operating characteristic curve of 0.819 (95% CI 0.756-0.882) for the diagnosis of NEC. A threshold score of 1.5 had a 95.2% specificity for NEC. Conclusion: Utilizing clinical characteristics associated with SIP & NEC we developed a scoring system designed to assist surgeons accurately distinguish SIP vs NEC in neonates. Abstract:Necrotizing Enterocolitis (NEC) is among the most devastating complications of preterm birth, with ~25% mortality among very low birthweight (VLBW) infants. 1,2,3 Among survivors, the condition can impart long-term negative health effects, occurring both before and after the index neonatal hospitalization. 1,2,3 Babies born at extremely low birthweight (ELBW) are more vulnerable, and mortality surpasses 40% for infants in need of surgery. 4,5,6 Survivors of NEC require significant resources to meet ongoing healthcare needs, as afflicted infants are at risk for short gut syndrome, liver disease and intestinal failure. 5,6,7,8,9 Beyond the neonatal period, infants with NEC exhibit double the odds of neurodevelopmental impairment, independent of additional risk conferred by intraventricular hemorrhage and periventricular leukomalacia. 10,11 This care is costly, and NEC remains a major source of healthcare utilization. Direct costs of NEC alone are estimated from 600,000 per case of surgical NEC. 12,13,14 Surgical NEC is the costliest diagnosis in children's hospitals, with even higher costs associated with lower gestational age at time of diagnosis. 15 Total annual spending on NEC approaches $1 Billion. 4 Despite its devastating impact, treatment for NEC has not dramatically changed in decades. Bowel rest, antibiotics and supportive care remain mainstays of treatment. Though the mechanism of NEC is not fully known, greater progress has been made in prevention among VLBW infants. Receipt of antenatal steroids, along with postnatal strategies such as early enteral feeding, reductions in early and prolonged antibiotic exposure, use of standardized feeding protocols, promotion of skin-to-skin contact and human milk diets are all associated with reductions in NEC. 16,17,18,19,20

Abstract:Surgically treated necrotising enterocolitis (sNEC) is associated with significantly worse neurodevelopmental outcomes than that seen in premature infants without NEC. We aim to review the association between factors involved in the surgical treatment of NEC and subsequent neurodevelopmental outcomes to identify potential areas for improvement. The PubMed and Embase databases were interrogated for articles reporting neurodevelopmental outcomes in babies treated surgically for NEC using key terms including: "Infant", "Necrotising enterocolitis", "Surgical", "Neurodevelopmental" and "Outcomes". The search strategy yielded 1170 articles and after applying inclusion and exclusion criteria 22 studies remained and formed the review. A diverse range of neurodevelopmental outcomes were reported. Extreme prematurity and lower birth weight were associated with worse neurodevelopmental outcomes. The use of peritoneal drains and enterostomies were associated with worse outcomes. Modifications to surgical strategies in NEC may improve neurodevelopmental outcomes but the effect of confounding factors remains unclear. Further large scale studies are required to define the optimum strategies for treating NEC surgically and to develop a core outcome set for research into NEC.

Abstract:Abstract While neonatal necrotising enterocolitis (NEC) is associated with high mortality rates in newborns, survivors can face long-term sequelae. However, the relationship between NEC and neurodevelopmental impairment (NDI) in preterm infants remains unclear. To explore the relationship between neonatal NEC and neurodevelopmental outcomes in preterm infants, we searched PubMed, EMBASE, and the Cochrane Library from their inception to February 2024 for relevant studies. Studies included were cohort or case–control studies reporting neurodevelopmental outcomes of NEC in preterm infants. Two independent investigators extracted data regarding brain damage and neurodevelopmental outcomes in these infants at a corrected age exceeding 12 months. Odds ratios (ORs) were pooled using a random effects model. We included 15 cohort studies and 18 case–control studies, encompassing 60,346 infants. Meta-analysis of unadjusted and adjusted ORs demonstrated a significant association between NEC and increased odds of NDI (OR 2.15, 95% CI 1.9–2.44; aOR 1.89, 95% CI 1.46–2.46). Regarding brain injury, pooled crude ORs indicated an association of NEC with severe intraventricular haemorrhage (IVH) (OR 1.42, 95% CI 1.06–1.92) and periventricular leucomalacia (PVL) (OR 2.55, 95% CI 1.76–3.69). When compared with conservatively treated NEC, surgical NEC potentially carries a higher risk of NDI (OR 1.78, 95% CI 1.09–2.93) and severe IVH (OR 1.57, 95% CI 1.20–2.06). However, the risk of PVL did not show a significant difference (OR 1.60, 95% CI 0.47–5.40). Conclusions : Our meta-analysis provides evidence suggesting an association between NEC and NDI. Additionally, the severity of intestinal lesions appears to correlate with a higher risk of NDI. Further high-quality studies with comprehensive adjustments for potential confounding factors are required to definitively establish whether the association with NDI is causal. What is Known: • NEC is a serious intestinal disease in the neonatal period with a high mortality rate, and surviving children may have digestive system sequelae. • Compared with non-NEC preterm infants, the reported incidences of brain injury and neurodevelopmental disorders in NEC preterm infants are not the same. What is New: • The risk of neonatal brain injury and neurodevelopmental disorders in preterm infants with NEC is higher than that in non-NEC infants, and the risk of NDI in surgical NEC infants is higher than that in the conservative treatment group. • NEC may increase the risk of motor, cognitive, language development delays, and attention deficits in children.

Abstract:Objective: We aimed to determine the incidence of growth failure in infants with necrotizing enterocolitis (NEC) or spontaneous intestinal perforation (SIP) and whether initial laparotomy versus peritoneal drainage (PD) impacted the likelihood of growth failure. Summary background data: Infants with surgical NEC and SIP have high mortality, and most have neurodevelopmental impairment and poor growth. Existing literature on growth outcomes for these infants is limited. Methods: This is a preplanned secondary study of the Necrotizing Enterocolitis Surgery Trial dataset. The primary outcome was growth failure (Z-score for weight <-2.0) at 18 to 22 months. We used logistic regression, including diagnosis and treatment, as covariates. Secondary outcomes were analyzed using the Fisher exact or Pearson χ2 test for categorical variables and the Wilcoxon rank sum test or one-way ANOVA for continuous variables. Results: Among 217 survivors, 207 infants (95%) had primary outcome data. Growth failure at 18 to 22 months occurred in 24/50 (48%) of NEC infants versus 65/157 (42%) SIP (P=0.4). The mean weight-for-age Z-score at 18 to 22 months in NEC infants was -2.05±0.99 versus -1.84±1.09 SIP (P=0.2), and the predicted mean weight-for-age Z-score SIP (Beta -0.27; 95% CI: -0.53, -0.01; P=0.041). Median declines in weight-for-age Z-score between birth and 18 to 22 months were significant in all infants but most severe (>2) in NEC infants (P=0.2). Conclusions: This first ever prospective study of growth outcomes in infants with surgical NEC or SIP demonstrates that growth failure is very common, especially in infants with NEC, and persists at 18-22 months.

Abstract:Objectives To compare necrotising enterocolitis (NEC) prevention practices and NEC associated factors between units from eight countries of the International Network for Evaluation of Outcomes of Neonates, and to assess their association with surgical NEC rates. Design Prospective unit-level survey combined with retrospective cohort study. Setting Neonatal intensive care units in Australia/New Zealand, Canada, Finland, Israel, Spain, Sweden, Switzerland and Tuscany (Italy). Patients Extremely preterm infants born between 24 0 to 28 6 weeks’ gestation, with birth weights<1500 g, and admitted between 2014–2015. Exposures NEC prevention practices (probiotics, feeding, donor milk) using responses of an on-line pre-piloted questionnaire containing 10 questions and factors associated with NEC in literature (antenatal steroids, c-section, indomethacin treated patent ductus arteriosus and sepsis) using cohort data. Outcome measures Surgical NEC rates and death following NEC using cohort data. Results The survey response rate was 91% (153 units). Both probiotic provision and donor milk availability varied between 0%–100% among networks whereas feeding initiation and advancement rates were similar in most networks. The 9792 infants included in the cohort study to link survey results and cohort outcomes, revealed similar baseline characteristics but considerable differences in factors associated with NEC between networks. 397 (4.1%) neonates underwent NEC surgery, ranging from 2.4%–8.4% between networks. Standardised ratios for surgical NEC were lower for Australia/New Zealand, higher for Spain, and comparable for the remaining six networks. Conclusions The variation in implementation of NEC prevention practices and in factors associated with NEC in literature could not be associated with the variation in surgical NEC incidence. This corroborates the current lack of consensus surrounding the use of preventive strategies for NEC and emphasises the need for research.

Abstract:Objective To explore the clinical characteristics and prognoses of neonatal alimentary tract perforation.Methods Retrospective review was conducted for 65 neonatal cases with alimentary tract perforation confirmed intraoperatively from January 2015 to December 2016.There were 43 boys and 22 girls.They were premature (n =45) and full-term (n =20).The perforations were gastric (n=7,group A),intestinal (n=34,group B),colonic (n=22,group C) and biliary tract (n=1,group D).The major clinical manifestations were abdominal distention and pneumoperitoneum.Results Gastric perforation tended to occur at the age of ＜5 days.Premature alimentary tract perforation was located in small intestine (n =29,64.4％).And the causes were neonatal necrotizing enterocolitis (NEC,n=26,57.8％),H irschsprung's disease (HSCR,n =5,11.1％) and idiopathic intestinal perforation (n =4,8.9 ％).For term infants,alimentary tract perforation was predominantly located in colon (n =11,55.0％).And the causes were Hirschsprung's disease (n =4,20.0％),congenital defects of gastric musculature (n=4,20.0％) and neonatal necrotizing enterocolitis (n =3,15.0％) respectively.No significant difference existed in distribution among groups A,B and C according to birth weight (P＞0.05).Preoperative pH value of group B (7.31 ± 0.10) was significantly lower than that of group C (7.38 ± 0.12) and preoperative hemoglobin of group C (117.6 ± 31.2) g/L was significantly lower than that of group A (161.1 ± 32.9) g/L and B (137.8 ± 34.5) g/L.No significant difference existed in preoperative levels of lactic acid or albumin (P＞0.05) among groups A,B and C.And 4/65(6.2％) died and no significant difference existed in mortality rate with regards to gestational age,birth weight or perforation site.Eight (13.1％) cases had incision infections.Conclusions Neonatal alimentary tract perforation occurs frequently in premature boys.Premature alimentary tract perforation is located predominantly in small intestine and the etiology is NEC.For term infants,alimentary tract perforation is found predominantly in colon and the etiologies include HSCR and congenital defects of gastric musculature.A better prognosis of alimentary tract perforation may be achieved after active,timely and standardized treatments.
OBTURATION INTESTINAL OBSTRUCTION IN THE COURSE OF NECROTIZING ENTEROCOLITIS IN NEWBORN CHILDREN

Abstract:Objective: The aim: To analyze our own results of diagnosis and treatment of newborns with NEC and obturation intestinal obstruction in order to determine theoretically important and practically significant recommendations on this issue, to optimize preventive measures for reducing intestinal obstruction cases in NEC and improving treatment outcome among newborns. Patients and methods: Materials and methods: Our supervision involved 143 newborns with NEC for the period from 2006 to 2020, including 79 boys (55.24%) and 64 girls (44.76%). Gestational age was 26-42 weeks. The majority of children were premature infants and numbered 121 children (84,62%). The assessment of clinical manifestations, progression of the disease, physical examination of newborns were used to help in diagnosing. All children underwent general clinical tests and instrumental methods of diagnosis including ultrasonography (abdominal organs and retroperitoneum, echocardiography, neurosonography) and X-ray examination (two-dimensional plain abdominal radiography, X-ray contrast study of the gastrointestinal tract). According to the survey, all patients were examined by allied health professionals. Results: Results: Obturation intestinal obstruction associated with NEC was diagnosed in 26 children (18.18%). 12 children were in serious condition, representing 46.15%. The condition of other 14 children (53.85%) was assessed as critical one. The first signs of intestinal obstruction were observed in children with NEC at the end of the first week or at the beginning of the second week of life. Intestinal obstruction was acute in 21 children (80.77%); 5 children (19.23%) had relapsing course. Blood test results showed the inflammatory process and were not specific. According to the results of our observation, the dynamics of platelets is indicative as there is a correlation between the deterioration in the child's condition and the onset of symptoms of intestinal obstruction and thrombocytopenia. There was an imbalance in protein and electrolyte metabolism - a decrease in the level of albumin, indicators of K, Na, Ca, and an increase of C-reactive protein by several times. Ultrasonography of the abdominal organs showed irregular intestinal pneumatization, dilated intestinal loops with stagnant intestinal contents, pendulum peristalsis, infiltrative bowel wall thickening, free fluid in the abdominal cavity. Plain radiography revealed typical manifestations of small intestinal obstruction with multiple air-fluid levels. In 15 newborns (57.69%), conservative treatment was effective, in particular intestinal obstruction was relieved; 11 newborns (42.31%) underwent surgery. The mortality was 36.36% (4 children died). Postoperative complication in the form of bowel stenosis in the area of the direct anastomosis was observed in 3 patients approximately 2 weeks after the surgery. Conclusion: Conclusions: 1. NEC can lead to the development of obturation intestinal obstruction in newborns. Obturation intestinal obstruction was a form of NEC course in 18.18% of children under our observation. 2. Obturation intestinal obstruction in NEC in newborns is a reversible condition and requires conservative therapy. If symptoms of obstruction persist within 3 days after the start of conservative therapy, surgical treatment is indicated. 3. In case of obturation intestinal obstruction, it is advisable to perform resection of necrotic bowel, formation of a double enterostomy. 4. Enterostomy closure by placing T-shaped intestinal anastomosis should be performed 4 weeks after the primary surgery. 5. The use of modern techniques for early diagnosis and timely treatment of obturation intestinal obstruction in NEC can reduce mortality and the number of postoperative complications. 6. Newborns with NEC should be under the supervision of surgeons, which will help timely determine the indications for surgical intervention.

Abstract:Background Necrotizing enterocolitis (NEC) is one of the main causes of acute abdomen in neonates. Surgical treatment entails important morbidity and mortality and conservative management, when possible, offers better outcomes. Post-NEC intestinal strictures are one of the main complications. Methods Retrospective analysis from June 2011 to November 2022 of post-NEC strictures (PNS) after conservative management of neonates diagnosed with NEC (modified Bell stage IIA or higher) at a tertiary neonatal surgery center. Results Out of 219 NEC, 126 received initial conservative management (57.5%), 24 (19%) of which eventually underwent surgery for PNS. Average gestational age and weight at birth of our cohort were 31.3 ± 4.9 weeks and 1,694 ± 1,009 g. PNS diagnosis was made 38.4 ± 16.5 days after the NEC episode. 6/24 (25%) were asymptomatic and diagnosed by screening enema, 11 (46%) presented signs of intestinal obstruction before the enema could be performed and 7 (29%) after a normal previous protocol study. Median age at PNS surgery was 56 ± 17.9 days. A total of 2/3 strictures were found in cecum, ascendent, and transverse colon. Primary resection and anastomosis were performed in all cases. Feeds were restarted on postoperative day 4.3 ± 2.9. Two cases presented anastomotic complications (1 dehiscence and 1 stenosis), and no deaths were recorded. Conclusions PNS is a frequent complication after conservative management. Deffered surgical treatment after the acute NEC episode is resolved allows for safer surgeries (since patients have reached hemodynamical stability and overcome septic shock), shorter resections, and favorable postoperative outcomes.
